MR-Neurography of the facial nerve in parotid tumors: intra-parotid nerve visualization and surgical correlation.

One of the most severe complications in surgery of parotid tumors is facial palsy. Imaging of the intra-parotid facial nerve is challenging due to small dimensions. Our aim was to assess, in patients with parotid tumors, the ability of high-resolution 3D double-echo steady-state sequence with water excitation (DE3D-WE) (1) to visualize the extracranial facial nerve and its tracts, (2) to evaluate their relationship to the parotid lesion and (3) to compare MRI and surgical findings. A retrospective study was conducted including all patients with parotid tumors, who underwent MRI from April 2022 to December 2023.Two radiologists independently reviewed DE3D-WE images, assessing quality of visualization of the facial nerve bilaterally and localizing the nerve's divisions in relation to the tumor. MRI data were compared with surgical findings. Forty consecutive patients were included (M:F = 22:18; mean age 56.3 ± 17.4years). DE3D-WE could excellently visualize the nerve main trunk and the temporofacial division in all cases. The cervicofacial branch was visible in 99% of cases and visibility was good. Distal divisions were displayed in 34% of cases with a higher visibility on the tumor side (p < 0.05). Interrater agreement was high (weighted kappa 0.94 ± 0.01 [95% CI 0.92-0.97]). Compared to surgery accuracy of MRI in localizing the nerve was 100% for the main trunk, 96% for the temporofacial and 89% for the cervicofacial branches. Facial nerve MR-neurography represents a reliable tool. DE3D-WE can play an important role in surgical planning of patients with parotid tumors, reducing the risk of nerve injury.

Estimation of effective b-value for a diffusion-weighted double-echo steady-state sequence with bipolar gradients

Diffusion-weighted double-echo steady-state (dwDESS) MRI with bipolar diffusion gradients is a promising candidate to obtain diffusion weighted images (DWI) free of geometric distortions and with low motion sensitivity. However, a wider clinical application of dwDESS is currently hindered as no method is reported to explicitly calculate the effective b-value of the obtained DWI from the diffusion-gradients applied in the sequence. To this end, a previously described signal model was adapted for dwDESS with bipolar diffusion gradients, which allows to estimate an effective b-value, dubbed b’. Evaluation in phantom examinations was performed on a clinical 1.5 T MR system. Experimental results were compared with theoretical predictions, including the apparent diffusion coefficient (ADC) based on b-values from a standard EPI-DWI sequence and ADC’ based on the effective b’ from the dwDESS sequence. The adapted signal model was able to describe the experimental results, and the obtained values of ADC’ were in line with conventional ADC measurements.

Rapid whole brain 3D T2 mapping respiratory-resolved Double-Echo Steady State (DESS) sequence with improved repeatability.

To propose a quantitative 3D double-echo steady-state (DESS) sequence that offers rapid and repeatable T2 mapping of the human brain using different encoding schemes that account for respiratory B0 variation. A retrospective self-gating module was firstly implemented into the standard DESS sequence in order to suppress the respiratory artifact via data binning. A compressed-sensing trajectory (CS-DESS) was then optimized to accelerate the acquisition. Finally, a spiral Cartesian encoding (SPICCS-DESS) was incorporated to further disrupt the coherent respiratory artifact. These different versions were compared to a standard DESS sequence (fully DESS) by assessing the T2 distribution and repeatability in different brain regions of eight volunteers at 3 T. The respiratory artifact correction was determined to be optimal when the data was binned into seven respiratory phases. Compared to the fully DESS, T2 distribution was improved for the CS-DESS and SPICCS-DESS with interquartile ranges reduced significantly by a factor ranging from 2 to 12 in the caudate, putamen, and thalamus regions. In the gray and white matter areas, average absolute test-retest T2 differences across all volunteers were respectively 3.5 ± 2% and 3.1 ± 2.1% for the SPICCS-DESS, 4.6 ± 4.6% and 4.9 ± 5.1% for the CS-DESS, and 15% ± 13% and 7.3 ± 5.6% for the fully DESS. The SPICCS-DESS sequence's acquisition time could be reduced by half (<4 min) while maintaining its efficient T2 mapping. The respiratory-resolved SPICCS-DESS sequence offers rapid, robust, and repeatable 3D T2 mapping of the human brain, which can be especially effective for longitudinal monitoring of cerebral pathologies.

Diagnostic performance of a 3D double-echo steady-state sequence at 3 T using radial reformats for detecting and grading rotator cuff tears: a pilot diagnostic accuracy study with magnetic resonance imaging and arthroscopic correlation.

In diagnosing rotator cuff tears (RCTs), magnetic resonance imaging (MRI) is the imaging modality of choice, and its accuracy is improving constantly. To evaluate the diagnostic performance of a high-resolution 3D double-echo steady-state (DESS) sequence with radial and paracoronal 3-T MRI regarding the grading of RCTs in correlation with conventional 2D MRI and arthroscopic findings. We retrospectively compared arthroscopic findings of RCTs with preoperative MRI, including conventional 2D sequences and radial and paracoronal DESS images in 20 patients. Two observers evaluated supraspinatus (SSP), infraspinatus (ISP), and subscapularis (SSC) tendon tears using a grading system. For statistical analysis, arthroscopy was used as the reference standard. Inter-observer agreement for detecting and grading SSP, ISP, and SSC tendon tears on radial and paracoronal sliced 3D DESS MRI was excellent (intraclass-correlation [ICC] = 0.92-0.98; all P < 0.001). Regarding the detection of SSP lesions, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were 93.8%, 50%, 88.2%, and 66.7% for both radial and paracoronal DESS imaging. A sensitivity of 100%, specificity of 61.1%, PPV of 22.2%, and NPV of 100% were noted for detecting ISP tendon tears using radially reformatted DESS images. Regarding detecting SSC tendon tears using radially reformatted DESS images, sensitivity, specificity, PPV, and NPV were 100%, 81.3%, 50%, and 100%, respectively. The results with standard 2D MRI were similar. The DESS technique with radially reformatted images provided excellent sensitivity and high inter-observer agreement in detecting RCTs. It showed a moderate to high correlation between MRI and arthroscopy for diagnosing SSP and SSC tendon tears.

3D characterization of the triple-bundle Achilles tendon from in vivo high-field MRI.

The Achilles tendon consists of three subtendons that transmit force from the triceps surae muscles to the calcaneus. Individual differences have been identified in Achilles subtendon morphology and twist in cadavers, which may have implications for triceps surae mechanics and function. High-field magnetic resonance imaging (MRI) can be used to identify boundaries within multi-bundle tissues, which could then enable studies of subtendon structure-function relationships in humans. The objective of this study was to use high-field MRI (7T) to image and reconstruct Achilles subtendons arising from the triceps surae muscles. We imaged the dominant lower leg of a cohort of healthy human subjects (n = 10) using a tuned musculoskeletal sequence (double echo steady state sequence, 0.4 mm isotropic voxels). We then characterized the cross-sectional area and orientation of each subtendon between the MTJ and calcaneal insertion. Image collection and segmentation was repeated to assess repeatability. Subtendon morphometry varied across subjects, with average subtendon areas of 23.5 ± 8.9 mm2 for the medial gastrocnemius, 25.4 ± 8.9 mm2 for the lateral gastrocnemius, and 13.7 ± 5.9 mm2 for the soleus subtendons. Repeatable subject-specific variations in size and position of each subtendon were identified over two visits, expanding on prior knowledge that high variability exists in Achilles subtendon morphology across subjects.

Acetabular Labral Repair and Selective Labral Debridement Show No Significant Difference in Clinical Outcomes at a Minimum 2-Year Follow-Up

To compare the outcomes of arthroscopic labral repair using looped type suture with a matched-pair selective labral debridement with a minimum 2-years follow-up. We identified 378 patients undergoing primary arthroscopic labral repair using loop-suture and selective labral debridement from January 2, 2018, to December 28, 2020. The labral repair group was matched 1:1 to a selective labral debridement control group by age, sex, body mass index, follow-up period, lateral center-edge angle, Tönnis grade, and preoperative joint space. Before surgery, 3-Tesla radial magnetic resonance imaging with a 3-dimensional double-echo steady-state sequence was obtained following failed nonoperative treatment lasting more than 3 months. Follow-up imaging was conducted at a minimum of 2 years. In both groups, the ratio of positive slices in which a disrupted chondrolabral junction was observed between the 2 o'clock and 11 o'clock positions was measured. Patient-reported outcome scores included the Harris Hip Score, visual analog score, Hip Outcome Score Activities of Daily Living Subscale, and Hip Outcome Score of Sport-Specific Subscale. In total, 76 patients of the repair group were matched to 76 controls with a minimum 2-years follow-up (repair vs control: 2.6 ± 0.4 vs 2.6 ± 0.4 years, P= .775). The repair group experienced a 2-fold improvement (0.6 ± 0.1 to 0.3 ± 0.1, P＜.001). Although the selective debridement group experienced a 3-fold improvement (0.3 ± 0.1 to 0.1 ± 0.1, P＜.001). Significant improvement of the patient-reported outcome was shown in both groups at final follow-up without significant difference between the 2 groups. The mid-term clinical outcomes are comparable between the labral repair using looped type suture and selective labral debridement group. Although a gap between the labrum and articular cartilage may appear in 3-dimensional double-echo steady-state magnetic resonance imaging results after labral repair, it does not correspond with clinical outcomes. Imaging of Facial Nerve With 3D-DESS-WE-MRI Before Parotidectomy: Impact on Surgical Outcomes.

The intra-parotid facial nerve (FN) can be visualized using three-dimensional double-echo steady-state water-excitation sequence magnetic resonance imaging (3D-DESS-WE-MRI). However, the clinical impact of FN imaging using 3D-DESS-WE-MRI before parotidectomy has not yet been explored. We compared the clinical outcomes of parotidectomy in patients with and without preoperative 3D-DESS-WE-MRI. This prospective, non-randomized, single-institution study included 296 adult patients who underwent parotidectomy for parotid tumors, excluding superficial and mobile tumors. Preoperative evaluation with 3D-DESS-WE-MRI was performed in 122 patients, and not performed in 174 patients. FN visibility and tumor location relative to FN on 3D-DESS-WE-MRI were evaluated in 120 patients. Rates of FN palsy (FNP) and operation times were compared between patients with and without 3D-DESS-WE-MRI; propensity score matching (PSM) and inverse probability of treatment weighting (IPTW) were used to adjust for surgical and tumor factors. The main trunk, temporofacial branch, and cervicofacial branch of the intra-parotid FN were identified using 3D-DESS-WE-MRI in approximately 97.5% (117/120), 44.2% (53/120), and 25.0% (30/120) of cases, respectively. The tumor location relative to FN, as assessed on magnetic resonance imaging, concurred with surgical findings in 90.8% (109/120) of cases. Rates of temporary and permanent FNP did not vary between patients with and without 3D-DESS-WE-MRI according to PSM (odds ratio, 2.29 [95% confidence interval {CI} 0.64-8.25] and 2.02 [95% CI: 0.32-12.90], respectively) and IPTW (odds ratio, 1.76 [95% CI: 0.19-16.75] and 1.94 [95% CI: 0.20-18.49], respectively). Conversely, operation time for surgical identification of FN was significantly shorter with 3D-DESS-WE-MRI (median, 25 vs. 35 min for PSM and 25 vs. 30 min for IPTW, P < 0.001). Preoperative FN imaging with 3D-DESS-WE-MRI facilitated anatomical identification of FN and its relationship to the tumor during parotidectomy. This modality reduced operation time for FN identification, but did not significantly affect postoperative FNP rates.

Diagnostic values of 2 different techniques for controversial lumbar disc herniation by conventional imaging examination: 3D-DESS vs. CT plain scan.

Background: The aim of this study was to explore the significance of three-dimensional double-echo steady-state (3D-DESS) sequence and multidetector computed tomography (CT) plain scan in the diagnosis of lumbar disc herniation (LDH) remaining controversial in conventional magnetic resonance imaging (MRI), and to compare the efficiency between 3D-DESS and CT in diagnosing controversial patients by conventional MRI. Methods: A total of 61 patients with controversial LDH diagnosed by conventional MRI were collected. Before operation, the disease of these patients was further confirmed by 3D-DESS sequences and continuous CT plain scan from L3 to S1. Finally, for patients whose postoperative curative effect was marked and symptoms were obviously alleviated, the sensitivity, specificity and accuracy. Results: Among, 59 patients with remarkably relieved symptoms after operation were included, and 2 patients with varying degrees of non-remission of pain and partial dysfunction after operation were excluded. The sensitivity, specificity and accuracy of 3D-DESS were 94.6, 100 and 94.9%, respectively, and those of CT were 75.0, 33.3 and 72.9%, respectively. Conclusion: 3D-DESS is a very useful diagnostic method for patients with some special types of LDH that remain controversial in conventional imaging diagnostic methods. Through 3D-DESS, the morphology of lumbosacral nerve roots can be directly observed, which is conducive to the improvement of the sensitivity, specificity and accuracy, thus further reducing the misdiagnosis rate. Moreover, 3D-DESS plays a guiding role in the formulation of operative methods.

Muscle activity with 0.5 T upright MRI-DESS to measure T2 in biceps and triceps.

The purpose of this study was to determine if muscle activity of the biceps followed by isometric flexion changes T2 measured in the biceps. It is hypothesized that an increase in T2 will be observed in the biceps but not in the triceps after flexion exercise. Ten healthy volunteers were imaged with a one-channel neck coil while seated in a 0.5 T upright open magnetic resonance imaging (MRI) scanner using a three-dimensional double echo steady-state (DESS) sequence. Volunteers were imaged while relaxing their arm for 10, 20, and 30 minduring an isometric biceps flexion immediately following performance of biceps curls to exhaustion, and again after relaxing for 10 and 20 min. Voxel-wise T2 was calculated by fitting to a DESS signal equation in regions segmented at muscle centers to determine mean T2 . During isometric biceps flexion immediately following biceps curls, mean T2 increased (average 33%, p < 0.05) in the biceps but not in the triceps. By 20 min after curls, mean T2 decreased (p < 0.05), and was near preactivity values. In contrast, there was no change in triceps T2 across any activity or postactivity time points. Intra-rater repeatability was excellent (ICC: 0.90-0.97). This study demonstrated that measuring T2 in an active muscle is feasible using a DESS sequence in an upright open MRI scanner. This could enable the study of muscle function while the muscle is working and weight-bearing, rather than of the "fatigue" of the muscles after activity. In comparison to electromyography, MRI also enables the study of deep muscles and allows simultaneous assessment of activity and function.

Preoperative visualization of the lingual nerve by 3D double-echo steady-state MRI in surgical third molar extraction treatment

ObjectivesTo assess the lingual nerve (LN) visualization using a 3D double-echo steady-state MRI sequence (3D-DESS).Materials and methodsThree readers prospectively evaluated the LN for its continuous visibility in 3D-DESS MRI in 19 patients with an indication for removal of mandibular impacted third molars, using a 5-point scale (4 = excellent to 0 = none). Six LN anatomical intermediate points (IP) were selected and checked for their detectability by a 4-point scale (4 = yes to1 = no). Inter- and intra-rater agreement was evaluated using intraclass correlation coefficient and percentage of agreement.ResultsThe average nerve continuity score was 3.3 ± 0.46. In 35% of the cases, the entire course was continuously visible. In 10%, the proximal and 60%, the distal part of the nerve was not continuously visible. Inter- and intra-reader agreement was good (ICC = 0.76, ICC = 0.75). The average detectability score of all IP was 3.7 ± 0.41. From IP1 to IP5, the detectability was excellent; meanwhile, IP6 had lower visibility. The inter- and intra-reader percentage of agreement was 77% and 87%.ConclusionsThe 3D-DESS sequence allowed accurate and continuous visualization of the LN with high reproducibility in more than one-third of the patients. This could improve the preoperative clarification of the LN position and thereby reduce complications during dentoalveolar surgical interventions.Clinical relevance3D-DESS MRI might be beneficial in clinical scenarios where the second molar is elongated or presents a difficult rotational position while simultaneously having a close positional relationship to the third molar. Thereby, osteotomy performed more lingually, indicating extended lingual flap detachment may increase the risk of LN damage.

Microfracture of Acetabular Rim After Segmental Labral Resection to Restore the Morphology and Function of Labrum: A Retrospective Study of More than 2 Years Follow-up.

ObjectiveTo report on the clinical outcome of patients undergoing combined arthroscopic treatment of labral resection and microfracture at the rim of acetabulum at a minimum 2‐year follow‐up.MethodsThe retrospective study included 38 patients undergoing hip arthroscopy for irreparable labral injury from 24 February 2014 to 26 February 2018. Thirteen patients were excluded owing to patient refusal of participation and concomitant diseases like synovial chondromatosis and dysplasia hip. The study group consisted of patients undergoing combined arthroscopic labral resection and microfracture at the rim of acetabulum (MICRO Group: 20 patients), arthroscopic labral resection alone (RESEC Group: five patients). Postoperative three‐dimensional (3D) double‐echo steady‐state (DESS) sequence with radial imaging at 3 Tesla were obtained and fluoroscopic image of the involved hip under distraction were used to observe the restoration of vacuum effect. Patient‐reported outcome scores (PROs) including the Harris Hip Score (HHS), Visual Analogue Score (VAS), Hip Outcome Score Activities of Daily Living Subscale (HOS‐ADL), Hip Outcome Score Sport‐Specific Subscale (HOS‐SSS) were collected and compared between two groups.ResultsAll patients were followed up for at least 6 months. The follow‐up time of RESEC group is longer than MICRO group (46.6 months vs 23.9 months, P < 0.05). The 3D DESS imaging demonstrated intermediate signal intensity at the relative area where the labrum resected followed by microfracture at the acetabular rim in MICRO group. Meanwhile, regrowth of labrum‐like tissue was not observed in MRI imaging of the RESEC group. Furthermore, vacuum effect was more apparent in MICRO group compared with RESEC group. All PROs in both groups showed a statistically significant improvement at follow‐up compared with preoperative levels. RESEC group: HHS (73.0 vs 93.8, P < 0.05); HOS‐ADL (51.5 vs 89.1, P < 0.05); HOS‐SSS (47.8 vs 88.3, P < 0.05); VAS (6.4 vs 2.0, P < 0.05). MICRO group: HHS (70.5 vs 91.5, P < 0.05); HOS‐ADL (52.4 vs 87.0, P < 0.05); HOS‐SSS (48.1 vs 86.5, P < 0.05); VAS (6.3 vs 1.6, P < 0.05). One patient of MICRO group had transient neurapraxias of the pudendal nerve that resolved completely by 3 months. There showed no statistically significant difference between groups regarding the preoperative and postoperative PROs.ConclusionCompared to labral resection, combined arthroscopic labral resection and microfracture at the rim of acetabulum is able to fulfill the labral defect area with the potential to restore the seal effect of labrum as an effective and safe option for irreparable segmental labral tears.

Multiple-echo steady-state (MESS): Extending DESS for joint T2 mapping and chemical-shift corrected water-fat separation.

PurposeTo extend the double echo steady‐state (DESS) sequence to enable chemical‐shift corrected water‐fat separation.MethodsThis study proposes multiple‐echo steady‐state (MESS), a sequence that modifies the readouts of the DESS sequence to acquire two echoes each with bipolar readout gradients with higher readout bandwidth. This enables water‐fat separation and eliminates the need for water‐selective excitation that is often used in combination with DESS, without increasing scan time. An iterative fitting approach was used to perform joint chemical‐shift corrected water‐fat separation and T2 estimation on all four MESS echoes simultaneously. MESS and water‐selective DESS images were acquired for five volunteers, and were compared qualitatively as well as quantitatively on cartilage T2 and thickness measurements. Signal‐to‐noise ratio (SNR) and T2 quantification were evaluated numerically using pseudo‐replications of the acquisition.ResultsThe water‐fat separation provided by MESS was robust and with quality comparable to water‐selective DESS. MESS T2 estimation was similar to DESS, albeit with slightly higher variability. Noise analysis showed that SNR in MESS was comparable to DESS on average, but did exhibit local variations caused by uncertainty in the water‐fat separation.ConclusionIn the same acquisition time as DESS, MESS provides water‐fat separation with comparable SNR in the reconstructed water and fat images. By providing additional image contrasts in addition to the water‐selective DESS images, MESS provides a promising alternative to DESS.

Mandibular Third Molar Surgery: Intraosseous Localization of the Inferior Alveolar Nerve Using 3D Double-Echo Steady-State MRI (3D-DESS).

The aim of this study was to evaluate the inferior alveolar nerve’s (IAN) intraosseous position within the inferior alveolar canal (IAC) using a 3D double-echo steady-state MRI sequence (3D-DESS). The IAN position was prospectively evaluated in 19 patients undergoing mandibular third molar (MTM) surgery. In the coronal reference layer, the IAC was divided into six segments. These segments were checked for the presence of hyperintense tubular MRI signals representing the IAN’s nervous tissue and assessed as visible/non-visible. Furthermore, the IAN in MRI and the IAC in MRI and CBCT were segmented at the third and second molar, determining the maximum diameter in all planes and a conversion factor between the imaging modalities. Regardless of the positional relationship at the third and second molar, the IAN showed the highest localization probability in the central segments (segment 2: 97.4% vs. 94.4%, segment 5: 100% vs. 91.6%). The conversion factors from IAC in CBCT and MRI to IAN in MRI, respectively, were the following: axial (2.04 ± 1.95, 2.37 ± 2.41), sagittal (1.86 ± 0.96, 1.76 ± 0.74), and coronal (1.26 ± 0.39, 1.37 ± 0.25). This radiation-free imaging modality, demonstrating good feasibility of accurate visualization of nervous tissue within the nerve canal’s osseous boundaries, may benefit preoperative assessment before complex surgical procedures are performed near the IAC.

MRI study of changes in knee bone marrow edema-like signal in asymptomatic amateur marathon runners before and after half-marathon running

ObjectiveTo evaluate the incidence of knee bone marrow edema-like signal and its changes before and after running a half marathon running in asymptomatic amateur marathon runners to explore the impact of the half marathon on knee bone marrow edema-like signal. Methods50 asymptomatic amateur marathon runners (30 males, 20 females) were recruited. T1-weighted imaging (T1WI), fat-suppressed protein density weighted imaging (fs-PDWI) and three-dimensional double-echo steady-state (3D-DESS) sequence on the right knee were performed before and within 3 h after a half-marathon running. 20 healthy volunteers were recruited as control. According to the whole-organ magnetic resonance imaging score (WORMS) system, the involvement of bone marrow edema-like signal in 15 regions of knee was graded from 0 to 3. The results were classified and Mann Whitney U test was used for comparison between groups. ResultsThe total incidence of bone marrow edema-like signal in amateur marathon group was 62%. Among them, the incidence of grade 1–3 was 48% (24/50), 12% (6/50), 2% (1/50), respectively, which was statistically significant compared with the controls (P = 0.007). There was no significant difference between gender before running (P = 0.172) and after running (P = 0.162). There was no significant difference before and after running (P > 0.05). However, 3 subjects showed new lesions, 8 subjects showed progression and 4 subjects showed decreased signal. ConclusionThe occurrence of knee bone marrow edema-like signal in amateur marathon runners is more common. The lesions of bone marrow edema-like signal will show aggravation or improvement in a certain extent after the half marathon.

Reproducibility of an Automated Quantitative MRI Assessment of Low-Grade Knee Articular Cartilage Lesions.

ObjectiveThe goal of this study was to assess the reproducibility of an automated knee cartilage segmentation of 21 cartilage regions with a model-based algorithm and to compare the results with manual segmentation.DesignThirteen patients with low-grade femoral cartilage defects were included in the study and were scanned twice on a 7-T magnetic resonance imaging (MRI) scanner 8 days apart. A 3-dimensional double-echo steady-state (3D-DESS) sequence was used to acquire MR images for automated cartilage segmentation, and T2-mapping was performed using a 3D triple-echo steady-state (3D-TESS) sequence. Cartilage volume, thickness, and T2 and texture features were automatically extracted from each knee for each of the 21 subregions. DESS was used for manual cartilage segmentation and compared with automated segmentation using the Dice coefficient. The reproducibility of each variable was expressed using standard error of measurement (SEM) and smallest detectable change (SDC).ResultsThe Dice coefficient for the similarity between manual and automated segmentation ranged from 0.83 to 0.88 in different cartilage regions. Test-retest analysis of automated cartilage segmentation and automated quantitative parameter extraction revealed excellent reproducibility for volume measurement (mean SDC for all subregions of 85.6 mm3), for thickness detection (SDC = 0.16 mm) and also for T2 values (SDC = 2.38 ms) and most gray-level co-occurrence matrix features (SDC = 0.1 a.u.).ConclusionsThe proposed technique of automated knee cartilage evaluation based on the segmentation of 3D MR images and correlation with T2 mapping provides highly reproducible results and significantly reduces the segmentation effort required for the analysis of knee articular cartilage in longitudinal studies.

Refixation of osteochondral fragments with resorbable polylactid implants : Long-term clincal and MRI results

Refixation with resorbable implants is acommon surgical treatment in patients who suffer an injury with shearing of an osteochondral flake due to trauma of the knee or the upper ankle joint. To date there are no studies which outline long-term outcomes for this procedure. The aim of this study was to evaluate long-term clinical and magnetic resonance imaging (MRI) results after refixation with resorbable polylactide (PLLA) implants. In this retrospective study 12patients with 13injuries were examined 13.9 years (±1.2 years) after refixation of an osteochondral fragment of the knee (10patients) and the upper ankle joint (2patients) with amean size of 3.33 cm2 (±2.33) by resorbable polylactide (PLLA) implants (nails, pins, screws, Bionx, Tampere, Finland). To objectify the clinical results eight established clinical scores (VASS, Tegner, Lysholm, McDermott, KSS, WOMAC, AOFAS, FADI+Sports) were used. Furthermore, the morphological integration of bone and cartilage was assessed by MRI (3 T) using proton-weighted and cartilage-sensitive 3D double-echo steady-state (DESS) sequences. The morphological results were objectified with amodified MRI score according to Henderson etal. After 13.9 years (±1.2) the patients with an injury of the knee as well as of the upper ankle joint showed good to excellent results (knee: VASS 1.2 (±1.7), Tegner 4.4 (±1.3), Lysholm 85.7 (±12.2), McDermott 90.7 (±8.6), KSS 189 (±14.2), WOMAC (6.16% (±8.45)) (upper ankle joint: VASS 2.5 (±2.5), Tegner 5.5 (±1.5), Lysholm 87 (±13), McDermott 88 (±12); WOMAC (8.54% (±8.54), AOFAS 75.5 (±24.5), FADI+Sports 118 (±18)). In all cases there was evidence of good integration of the osteochondral fragment in MRI. In five patients there was moderate subchondral cyst formation (∅ ≤1 mm); however, mild changes of the cartilage contour were found in all patients. The mean modified Henderson score achieved was 14.4 (±2.0, best 8, worst 32), which corresponds to a good morphological result. Because of good clinical and morphological results shown by MRI, refixation through resorbable implants (PLLA) can be recommended for treatment of traumatic osteochondral flakes.

Comparing Relative Contrast in Three-dimensional Double-echo Steady State With Routine Sequences for a Better Diagnosis of Knee Cartilage Anomalies

Aims This study compared relative contrast values in three-dimensional Double Echo Steady State (DESS) sequences with two flip angles of 40 and 90 degrees and other routine sequences. The obtained data could help to prove the effects of this sequence, compared to the other routine sequences; accordingly, it could be applied for better detection of lesions resulting from cartilage abnormalities. Methods &amp; Materials Total, 15 patients, including 9 men (14-56 y) and 6 women (36-65 y) with knee pain symptoms were referred to us by an orthopedic physician. All of the investigated patients underwent Magnetic Resonance Imaging (MRI) using DESS sequences with two flip angles of 40 and 90 degrees, as well as STIR , PD, T2, and 3D FLASH. We analyzed the acquired images using the Radiant DICOM Viewer4.6.9 software. We calculated relative contrast values for the selected SAGITAL images per sequence for each patient. The obtained data were analyzed using SPSS. Findings Friedman and Wilcoxon statistical tests data revealed significant P values (&lt;0.05) for all the sequences examined in this study. The DESS sequence with a flip angle of 90° presented the most relative contrast value with the average score of 77.2667. The FLASH sequence provided the worst result of 3.733 for the relative contrast value. Conclusion The DESS sequence with 90° flip angle indicated the best relative contrast values and provided the best possible signal from the synovial fluid and the cartilage; therefore, it leads to the better examination of articular cartilage in terms of lesion. Accordingly, the DESS sequence with a flip angle of 90° was the optimal sequence to evaluate cartilage abnormalities among the examined sequences in this study.

Comparison of Three-Dimensional Double-Echo Steady-State Sequence with Routine Two-Dimensional Sequence in the Depiction of Knee Cartilage

Introduction: There are some routine two-dimensional sequences, including short tau inversion recovery (STIR), T2-weighted fast-spin echo (T2W-FSE), and proton-density fast spin-echo for diagnosing osteoarthritis and lesions of the knee cartilage. However, these sequences have some disadvantages, such as long scan time, inadequate spatial resolution, and suboptimal tissue contrast which results in loss of image details, as well as missing the visualization of knee cartilage lesions. Three-dimensional (3D) sequences, such as the double-echo steady-state (DESS) sequence can decrease and overcome these problems to the smallest possible amount. Material and Methods: A total of 15 volunteers with knee pain were examined by a 1.5 Tesla magnetic resonance imaging.The contrast-to-noise ratio (CNR) and thickness values of the knee articular cartilage were measured. The CNR and thickness values were comparedby the Friedman test and the Wilcoxon signed-rank test. Results: The obtained results showed significant differences between sequences in CNR and thickness values. The DESS sequence with a flip angle of 40°showed the best CNR values and 3D fast low-angle shot (FLASH) showed the worst results. In addition, the results showed no significant differences between FLASH, 3D DESS 40° and 90° in terms of cartilage thickness. However, thickness values of these sequences were much higher than that of the PD, T2, and STIR sequences. Conclusion: The 3D DESS sequence with two flip angles of 40°and 90° are the best sequences for visualizing the cartilage and the synovial fluid. Because they provide the best contrast between the cartilage and the synovial fluid, it is recommended to use DESS sequences in the evaluation of cartilage defections.

Imaging Biomarkers of the Physis: Cartilage Volume on MRI vs. Tract Volume and Length on Diffusion Tensor Imaging.

Current methods to predict height and growth failure are imprecise. MRI measures of physeal cartilage are promising biomarkers for growth. In the physis, to assess how 3D MRI volume measurements, and diffusion tensor imaging (DTI) measurements (tract volume and length) correlate with growth parameters and detect differences in growth. We compared patients exposed to cis-retinoic acid, which causes physeal damage and growth failure, with normal subjects. Case-control. Twenty pediatric neuroblastoma survivors treated with cis-retinoic acid and 20 age- and sex-matched controls. 3T; DTI and 3D double-echo steady-state (DESS) sequences. On distal femoral MR studies, physeal 3D volume and DTI tract measurements were calculated and compared to height. We used partial Spearman correlation, analysis of covariance, logistic regression, Wald test, and the intraclass correlation coefficient (ICC). The height percentile correlated most strongly with DTI tract volumes (r = 0.74), followed by mean tract length (r = 0.53) and 3D volume (r = 0.40) (all P < 0.02). Only tract volumes and lengths correlated with annualized growth velocity. Relative to controls, patients showed smaller tract volumes (8.00 cc vs. 13.71 cc, P < 0.01), shorter tract lengths (5.92 mm vs 6.99 mm, P = 0.03), and smaller ratios of 3D cartilage volume to tract length; but no difference (4.51 cc vs 4.85 cc) in 3D MRI volumes. The 10 patients with the lowest height percentiles had smaller tract volumes (5.07 cc vs. 10.93 cc, P < 0.01), but not significantly different 3D MRI volumes. Tract volume is associated with abnormal growth, with an accuracy of 75%. DTI tract volume of the physis/metaphysis predicts abnormal growth better than physeal cartilage volumetric measurement and correlates best with height percentile and growth velocity. 2 TECHNICAL EFFICACY: Stage 2 J. Time-saving opportunities in knee osteoarthritis: T2 mapping and structural imaging of the knee using a single 5-min MRI scan

ObjectivesTo assess the discriminative power of a 5-min quantitative double-echo steady-state (qDESS) sequence for simultaneous T2 measurements of cartilage and meniscus, and structural knee osteoarthritis (OA) assessment, in a clinical OA population, using radiographic knee OA as reference standard.MethodsFifty-three subjects were included and divided over three groups based on radiographic and clinical knee OA: 20 subjects with no OA (Kellgren-Lawrence grade (KLG) 0), 18 with mild OA (KLG2), and 15 with moderate OA (KLG3). All patients underwent a 5-min qDESS scan. We measured T2 relaxation times in four cartilage and four meniscus regions of interest (ROIs) and performed structural OA evaluation with the MRI Osteoarthritis Knee Score (MOAKS) using qDESS with multiplanar reformatting. Between-group differences in T2 values and MOAKS were calculated using ANOVA. Correlations of the reference standard (i.e., radiographic knee OA) with T2 and MOAKS were assessed with correlation analyses for ordinal variables.ResultsIn cartilage, mean T2 values were 36.1 ± SD 4.3, 40.6 ± 5.9, and 47.1 ± 4.3 ms for no, mild, and moderate OA, respectively (p < 0.001). In menisci, mean T2 values were 15 ± 3.6, 17.5 ± 3.8, and 20.6 ± 4.7 ms for no, mild, and moderate OA, respectively (p < 0.001). Statistically significant correlations were found between radiographic OA and T2 and between radiographic OA and MOAKS in all ROIs (p < 0.05).ConclusionQuantitative T2 and structural assessment of cartilage and meniscus, using a single 5-min qDESS scan, can distinguish between different grades of radiographic OA, demonstrating the potential of qDESS as an efficient tool for OA imaging.Key Points• Quantitative T2values of cartilage and meniscus as well as structural assessment of the knee with a single 5-min quantitative double-echo steady-state (qDESS) scan can distinguish between different grades of knee osteoarthritis (OA).• Quantitative and structural qDESS-based measurements correlate significantly with the reference standard, radiographic degree of OA, for all cartilage and meniscus regions.• By providing quantitative measurements and diagnostic image quality in one rapid MRI scan, qDESS has great potential for application in large-scale clinical trials in knee OA.
