A cough that lasts for more than 8 weeks in adults (those older than 15 years) and more than 4 weeks in children is considered chronic and poses a significant health problem, as it can lead to feelings of fatigue, sleep disturbances, hoarseness, disruption of daily activities, vomiting, and often in women, even incontinence. The differential diagnosis of chronic cough in adults and children has its specificities. In certain cases, the etiological factor remains unknown when referring to unexplained (idiopathic) chronic cough. This paper presents the results of a systematic review of publications in medical databases (PubMed) and guidelines from the European Respiratory Society (ERS) and the American College of Chest Physicians (CHEST), related to the diagnosis and treatment of chronic cough. Diagnostic investigations should be focused on the most common causes of chronic cough. In adults, these include asthma, chronic obstructive pulmonary disease, bronchiectasis, interstitial lung disease, lung malignancy, non-asthmatic eosinophilic bronchitis, gastroesophageal reflux disease, and upper airway cough syndrome. In children, the most common causes of chronic cough are asthma, protracted bacterial bronchitis, cystic fibrosis, primary ciliary dyskinesia, bronchiectasis, tracheobronchomalacia, pertussis, habitual (psychogenic) cough, and foreign body aspiration. The treatment primarily involves excluding medications that might have chronic cough as an adverse effect. Therapy is directed toward treating the underlying cause of chronic cough. If the cough persists despite treatment (refractory cough) or if it's idiopathic, neuromodulatory medications are employed (opioids, gamma-aminobutyric acid analogs, tricyclic antidepressants). Therapeutic options also include breathing and vocal techniques, superior laryngeal nerve blocks, and vocal cord medialization injections. Drugs directly inhibiting sensory receptors (P2X3, TRPV-1, and NK-1 inhibitors) are currently under clinical trial investigation. The use of validated tests is crucial for assessing therapeutic effectiveness. The etiological factors of chronic cough can have both infectious and non-infectious origins and are not always related to lung diseases. Considering that chronic cough is regarded as a condition of hypersensitivity, when causal treatment does not yield satisfactory results or when dealing with idiopathic chronic cough, therapy is directed towards central or peripheral neuromodulation, as well as techniques involving breathing and vocal production.

A Case Report on Cyanotic Spell: Suspected Foreign Body

Tracheobronchial Foreign Body Aspiration (FBA) remains a significant cause of morbidity and mortality in children less than three years of age. High index of suspicion leads to early diagnosis and treatment thus preventing further deleterious complications. This case report is about a 15-month-old female child who presented with intermittent low-grade fever, cough, cold for 20 days and rapid breathing since one day. On examination, she had pallor, grade 3 protein energy malnutrition according to Indian Academy of Paediatrics classification, heart rate was 128/min, respiratory rate was 44/min, oxygen saturation was 88% on room air with nasal flaring, intercostal and subcostal retractions. Bilateral rhonchi with reduced air entry was present in all areas of the lung. Bilateral bronchopneumonia was seen on chest X-ray and tuberculosis work up was negative. Child was started on oxygen, antibiotics, nebulisations and intravenous fluids. On day two of admission child developed rigorous bouts of cough associated with hypoxia, bradycardia, cyanosis and listlessness which improved on bag and mask ventilation. Following a strong suspicion of obstruction in the tracheobronchial tree, a Computed Tomography (CT) neck and chest was done which was suggestive of either an intratracheal or intrabronchial foreign body. Child was urgently taken up for rigid bronchoscopy and a ground nut was found in the right main bronchus. The FBA as a differential diagnosis of chronic cough in children should always be ruled out.

The Role of Bronchoscopy in Diagnosis of Chronic Cough in Adults: A Retrospective Single-Center Study

Cough is one of the most frequent symptoms reported to pulmonologists. The role of bronchoscopy in the diagnostic work-up of chronic cough is not clearly defined. The aim of this study was to evaluate the utility of fiberoptic bronchoscopy (FOB) and additional testing of samples collected during FOB in the differential diagnosis of chronic cough in adults. This was a single-center retrospective study. Out of 7115 conventional white light FOB examinations, we finally selected 198 with cough as the only indication. In 40.9% of bronchoscopic examinations, no visible cause of cough was found. Visual signs of chronic bronchitis (CB) were detected in 57.6% of reports. Only in 3 cases (1.5%) bronchoscopy revealed a potential cause of chronic cough other than CB. Mycobacterium tuberculosis or other mycobacteria were spotted in none of the samples. In 91.1% of bronchoalveolar lavage (BAL) cytologic examinations, at least one cell count abnormality was detected, but only in case of increased percentage of eosinophils, it might be considered clinically relevant. In 53% of bacteriological culture results, at least one potentially pathogenic bacterium was isolated. The present study results strengthen the evidence that FOB combined with additional testing of airway specimens obtained during FOB is not a powerful tool in the differential diagnosis of chronic cough, and FOB as a diagnostic tool may be overused. The appropriate timing and decision regarding referral for FOB and additional testing of achieved material requires careful clinical consideration.

Classification of asthma.

Asthma is a chronic inflammatory disorder of the airways that results, physiologically, in hyperreactivity and, clinically, in recurrent episodes of wheezing, chest tightness, or coughing. Airway inflammation, smooth-muscle contraction, epithelial sloughing, mucous hypersecretion, bronchial hyperresponsiveness, and mucosal edema contribute to the underlying pathophysiology of asthma. Diagnostic tests such as methacholine or mannitol challenges or spirometry (pre- and postbronchodilator responses) help to identify such underlying pathophysiology via assessments of bronchial hyperreactivity and lung mechanics but are imperfect and, ultimately, must be viewed in the context of a patient's clinical presentation, including response to pharmacotherapy. Asthma can be classified into either intermittent or persistent, and the latter is either mild, moderate, or severe. Some patients change, in either direction, from intermittent to persistent asthma. In addition, patients with asthma may be classified as allergic (immunoglobulin E mediated), nonallergic (often triggered by viral upper respiratory tract infections or no apparent cause), occupational, aspirin-exacerbated respiratory disease, potentially fatal, exercise-induced, and cough variant asthma. In the latter, the patients have a nonproductive cough that responds to treatment for asthma but not with antibiotics, expectorants, mucolytics, antitussives, or beta₂-adrenergic agonists, and to treatment for acid reflux and rhinosinusitis. Thus, cough variant asthma is in the differential diagnosis of chronic cough.

Is protracted bacterial bronchitis a new nosological group or an old problem of differential diagnosis of chronic cough in children?

The purpose is to study the clinical-anamnestic and microbiological features of the protracted bacterial bronchitis (PBB), recurrent bronchitis (RB), chronic bronchitis (CB) in children and to determine the risk factors for the development of chronic bronchitis.Materials and methods. A total of 89 children were examined, among them 21 children with PBB, 27 children with RB and 41 children with CB. The study included the collection of anamnesis, objective examination, chest X-ray, bronchoscopy and microbiological examination of the bronchial mucosa and upper respiratory tract composition.Results. An analysis of РВВ clinical features showed that their total duration was 4.1 ± 0.3 weeks. In children with RB, the duration of exacerbations did not differ significantly and was 3.8 ± 0.4 weeks. In patients with CB, the annual number of bronchitis and the total duration of cough were two times higher than in patients with RB. Bacteriological analysis showed that Streptococcus pneumonia was the most frequent causative agent of the disease in children with PBB and RB. In children with exacerbation of CB, Haemophilus influenzae was more commonly identified. At the same time, such a representative of the normal bronchial microbiome as Aerococcus viridans, was identified in 81.0 % of PBB patients, in 33.3 % of RB patients, and in 8.7 % of CB patients.Conclusions. The study showed that risk factors for СВ development in children with PBВ and RB are the lower respiratory tract H. influenzae colonization and decreased activity or absence of the normal airway microbiota (Aerococcus viridans).

Non-astmatic Eosinophilic Bronchitis.

Non-asthmatic eosinophilic bronchitis (NAEB) is eosinophilic inflammation of the respiratory tract, without any bronchospasm. In this article, we want to draw attention to the NAEB. It should also be considered in differential diagnosis of chronic cough. Eosinophilia is present in all induced or spontaneous sputum samples of NAEB patients. NAEB patients and asthmatic patients have similar airway inflammation. Remarkably, NAEB mainly occurs in the lower airways. Unlike asthma, mast cells in NAEB are active in the bronchial epithelium. Diagnosis is based on the clinical, radiological, and spirometric measurements of other causes of chronic cough (Post-nasal discharge syndrome, asthma, gastroesophageal reflux etc.) and the assessment of inflammation in the lower respiratory tract. Airway inflammation can be assessed by sputum induction. The main treatment is anti-inflammatory therapy with inhaled corticosteroids and taking protective measures if inflammation is due to occupational exposure or allergen inhalation. If NAEB is untreated, it may be transient, episodic, or persistent; rarely, long-term oral steroid treatment may be required in patients. There is a requirement for studies that investigate the role of non-invasive markers of chronic inflammation associated with NAEB and the effectiveness of other treatments.

Chronic Non-Productive Cough in Patients With Primary Sjögren's Syndrome.

This study aims to evaluate the frequency of chronic cough due to dryness in the trachea and the relationship between depression and chronic cough in patients with primary Sjögren's syndrome (pSS). Eighty non-smoking patients (7 males, 73 females; mean age 47.6±9.2 years; range 18 to 70 years) with newly diagnosed pSS were included. All patients were evaluated clinically, radiologically, and physiologically. Patients with cough were assessed using the Leicester Cough Questionnaire and visual analog scale. Beck Depression Inventory was used to determine the risk of depression in patients with cough. Patients with asthma, rhinosinusitis, gastrooesophageal reflux, and drug use which cause cough and pulmonary involvement in pSS were excluded. Non-productive cough was detected in 30 patients (37.5%). Fourteen patients (17.5%) were admitted to the hospital with cough at least once before the diagnosis of pSS. The median time between onset of symptoms and firm diagnosis was 24 months (25-75 interquartile ranges: 3-60 months). A significant difference was observed in the Beck Depression Inventory scores between patients with and without cough. There was a significant negative relationship between Leicester Cough Questionnaire and Beck Depression Inventory scores. Patients with pSS often have symptoms for a long duration that mimic those of more commonly encountered non-specific pulmonary conditions. Xerotrachea should be considered in the differential diagnosis of chronic cough, especially in patients with normal pulmonary function tests and pulmonary imaging.

Differential diagnosis of chronic cough in children

A cough is considered chronic when it lasts >4 weeks. Chronic cough can be from a variety of causes. This article provides a structured approach to evaluating the child with chronic cough. Beginning with the disturbing cough that is absent once asleep, consistent with the habit cough syndrome, the diagnostic criteria for 10 causes of chronic cough are discussed. Using a structured approach to the differential diagnosis, common and uncommon causes can be identified. Well-established causes of chronic cough, such as asthma, are likely to be well known to the reader, whereas more recently identified etiologies, such as protracted bacterial bronchitis, are presented in more detail. The differential value of flexible and rigid bronchoscopy and bronchoalveolar lavage for aiding in the differential diagnosis is included for those entities where their use is essential.

Kommerell’s Diverticulum: An Unusual Cause of Chronic Cough

A 62-year-old male presented to the outpatient department of chest with history of dry cough since two months and swelling on the anterior aspect of neck of 30-year duration. Physical examination revealed a goitre. However, further imaging studies revealed presence of another associated pathology, a Kommerell's diverticulum in association with a right aortic arch with aberrant left subclavian artery. The enlarged thyroid was not compressing the trachea, and its occurrence in this case could be incidental. The diverticulum was considered as the cause of chronic cough in our case as it was causing tracheal compression, and also there were no other obvious causes which could explain the symptom. Vascular anomalies such as Kommerell's diverticulum, though uncommon, should be considered in the differential diagnosis of chronic cough particularly when other common causes have been ruled out.

Serum Immunoglobulin G Analysis to Establish a Delayed Diagnosis of Chronic Cough due to Bordetella pertussis

Incidence of Bordetella pertussis infection among adults has risen significantly throughout the United States, but pertussis is not often considered in the differential diagnosis of chronic cough in adults. The authors hypothesized that serum IgG testing can establish a diagnosis of pertussis infection late in disease presentation when cultures and polymerase chain reaction (PCR) testing are not reliable. Case series with chart review. Tertiary care hospital. Institutional B pertussis serum IgG and PCR tests were reviewed since 2007. Clinical factors assessed included vaccination history, duration and severity of cough, and general medical history. Forty-eight patients had B pertussis fimbrial agglutinogen IgG levels tested since 2007, with a significant increase in positive IgG tests (>27 IU/mL, 3 times the upper limit of normal) since fall 2009. Nineteen patients (39.5%) met IgG criteria for likely recent pertussis infection. Six IgG-positive patients also had PCR swab testing performed, with 50% positive for B pertussis. IgG values were similar for patients with positive or negative B pertussis PCR testing with positive IgG titers. IgG-positive patients were much more likely to have posttussive syncope. Recent vaccination for pertussis within the 3 years prior to IgG testing did not significantly increase IgG levels. One-time B pertussis serum IgG testing and patient history can establish a likely diagnosis of recent pertussis infection in the adult patient with chronic cough late in disease presentation when PCR testing is often negative. Pertussis should be considered in the differential diagnosis of all patients with chronic cough.

Values of fractional exhaled nitric oxide in the diagnosis of chronic cough

To evaluate the diagnostic value of fractional exhaled nitric oxide (FeNO) in the diagnosis of chronic cough. A total of 106 subjects with chronic cough and normal chest radiographs were recruited from October 2009 to September 2010. Based on the management guidelines of the Chinese Respiratory Society for cough, the golden standard methods were used to make the definite diagnosis of chronic cough, including sputum cell counts, pulmonary function tests, bronchial hyperresponsiveness, 24-h esophageal pH monitoring, skin pricking test and serum immunoglobulin E. All subjects received a FeNO test by a NIOXMINO analyzer. The values of FeNO to diagnose cough variant asthma (CVA) from chronic cough and EB from non-asthma cough were respectively assessed by the receiver operating characteristic (ROC) curves. Among them, the definite diagnoses were cough variant asthma (CVA, n = 39), eosinophilic bronchitis (EB, n = 30) and other causes (n = 37). The FeNO levels in CVA [(54 ± 21) ppb)] (1 ppb = 1 × 10(9) mol/L) were significantly higher than those in EB [(34 ± 17) ppb, P < 0.01] and other causes [(21 ± 10) ppb, P < 0.01]. And the FeNO levels in EB were higher than those in other causes (P < 0.01). To diagnose CVA from chronic cough, the optimal FeNO cutoff value was 40 ppb with a sensitivity of 75%, a specificity of 86%, a positive predictive value of 77%, a negative predictive value of 86% and an accuracy of 81%. To diagnose EB from non-asthma chronic cough, the optimal FeNO cutoff value was 31 ppb with a sensitivity of 63%, a specificity of 92%, a positive predictive value of 88%, a negative predictive value of 92% and an accuracy of 72% respectively. There are significant differences between the FeNO levels of different causes of chronic cough. A marked elevation of FeNO level helps to make a final diagnosis of CVA or EB. FeNO test is useful for making the diagnosis and differential diagnosis of chronic cough in clinic practices.

To the Editors: Chronic cough is one of the most common symptoms of medical importance with 12% of the population reporting significant symptoms on a daily or weekly basis. The differential diagnosis of chronic cough is extensive. Uncommon causes of chronic cough may be missed unless an adequate history is obtained, a detailed examination performed and appropriate investigations arranged. Herein, we present a case of chronic cough, the aetiology of which was previously unreported. A 57-yr-old female presented with a history of chronic cough for 12 months. The cough had features of reflux 1 as it was precipitated by food and phonation, and associated with frequent clearing of the throat. Moreover, the cough was worse when she was lying flat and started upon rising in the morning. In addition, she described her cough to be precipitated by upward movement of her neck, for example looking at the top shelf in a supermarket. There was no history of decreased appetite or weight loss. She was known to have a hiatus hernia. Her past medical history was otherwise unremarkable. She was a life long nonsmoker and was not on any regular medications. Nontuberculous mycobacterial infection in the differential diagnosis of chronic cough

Patients presenting with chronic coughs are seen frequently by allergists/immunologists. When the usual diagnostic and therapeutic maneuvers do not control symptoms, it is worthwhile to consider whether a non-tuberculous mycobacterial (NTM) infection might be playing a role in the pathogenesis of the coughing. Sputum culture should be considered along with a pulmonary computerized axial tomography scan. NTM infection should be added to the differential diagnosis list for patients with chronic coughs unresponsive to conventional therapy.

An annoying persistent cough

Chronic cough is a stressful condition and can lead to extensive investigations. We report a case of a 48-year-old woman who had suffered from persistent chronic cough for more than 3 months. She had been treated with cough suppressant. However, her cough was not alleviated by these treatments, and the patient was referred to our hospital. She did not exhibit typical gastroesophageal reflux disease (GERD) symptoms heartburn and regurgitation. Esophagoscopy did not disclose reflux esophagitis. The patient was treated with a proton-pump inhibitor, which markedly alleviated her cough. Chronic cough due to GERD was diagnosed. Although the diagnosis of chronic cough due to GERD is not easy when traditionally symptoms are not present, our case report underscores the importance of this association to the differential diagnosis of chronic cough. In these cases a relatively simple test as high dose proton pump-inhibitors trial may be useful to confirm GERD related cough.

Giant Cell Arteritis Manifesting as Chronic Cough and Fever of Unknown Origin

A 57-year-old white man sought medical attention because of chronic cough and fever of unknown origin. An extensive work-up over 4 weeks, including repeated blood cultures, chest roentgenograms, a gallium scan, and computed tomographic scans of the sinuses, chest, and abdomen, was nondiagnostic. The patient was referred to our institution for bronchoscopy. Further analysis of his history revealed that he had a headache in conjunction with the cough and an episode of a flashing color design in his left eye 1 week before assessment. The erythrocyte sedimentation rate was 115 mm in 1 hour. A biopsy of the temporal artery showed granulomatous inflammation of the vessel wall with multinucleated giant cells, histiocytes, lymphocytes, plasma cells, and few eosinophils. The multinucleated giant cells were closely related to the fragmented elastic lamina. Corticosteroid therapy resulted in prompt resolution of the chronic cough and fever. Giant cell arteritis should be considered in the differential diagnosis of chronic cough.

ADVERSE PULMONARY EFFECTS OF INVOLUNTARY SMOKING ON ADOLESCENT ATHLETES

In a previous study we showed a relationship between involuntary smoking and increased frequency of cough and decreased pulmonary function in teenage athletes. In order to further examine this relationship we performed a second study using a larger group (314 12-17 year old high school athletes) and an interviewing instrument developed by the American Thoracic Society (ATS-DLD). A structured interview assessed pulmonary symptoms, personal smoking habits and passive cigarette smoke exposure. All subjects performed forced expiratory maneuvers on a computer assisted portable spirometer. We measured forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and maximal mid-expiratory flow rate [FEF(25-75)]. The best of three acceptable measures was utilized. Of the 314 subjects, 3.2% were active smokers and were excluded. Of the remaining 304 athletes, 65% were currently exposed to involuntary smoking. We found, in multiple regression analyses, that exposure was significantly associated with lower FEV1 and lower FVC (p 0.05), independent of cough. The prevalence of cough was found to be five times as frequent in the exposed subjects. The two studies, taken together, establish the association of involuntary smoking with compromised airways function and cough in adolescent athletes. Pediatricians need to consider involuntary smoking in the differential diagnosis of chronic cough and decreased pulmonary function.
