Usefulness of “AcT ratio” in diagnosis of internal carotid artery stenosis: a multicenter, retrospective, observational study

PurposeThe ratio of the internal carotid artery (ICA) to the common carotid artery (CCA), especially the “AcT ratio,” which is a modified measurement method of acceleration time, is useful for diagnosing ICA-origin stenosis. However, previous studies were single-center studies. Therefore, this multicenter, retrospective, cross-sectional study aimed to determine whether a method using the AcT ratio is useful for estimating stenosis rates.MethodsThis study included 461 vessels subjected to carotid artery ultrasonography and evaluation for ICA-origin stenosis via NASCET at four hospitals. The duration from the steep rise point to the inflection point or the first peak was defined as AcT on pulsed wave Doppler. The AcT ratio was calculated as AcT of ICA/AcT of ipsilateral CCA. The AcT ratio and rate of ICA-origin stenosis were analyzed using Pearson's correlation coefficient, simple regression analysis, and ROC curve.ResultsA significant positive correlation was observed between the AcT ratio and NASCET stenosis. NASCET stenosis of ≥ 50% had a sensitivity, specificity, and negative predictive value (NPV) of 70.2%, 71.6%, and 91.5%, respectively, when the cut-off value of the AcT ratio was 1.17. NASCET stenosis of ≥ 70% had a sensitivity, specificity, and NPV of 70.5%, 72.1%, and 95.9%, respectively, when the cut-off value of the AcT ratio was 1.22.ConclusionsThe findings of this multicenter, retrospective, cross-sectional study suggest that the AcT ratio is useful for diagnosing ICA-origin stenosis, especially for diagnosis by exclusion. NASCET stenosis of ≥ 50% was considered unlikely if the Act ratio was ≤ 1.17, whereas NASCET stenosis of ≥ 70% was considered unlikely if it was ≤ 1.22.

The case of treatment for carotid web - Double-layer micromesh stent implantation in our hospital.

A carotid web is a shelf-like structure on the posterior wall of the origin of the internal carotid artery, and it is believed to cause cerebral infarction due to thrombus formed by turbulent flow with stagnation of blood flow. Recently, it has been suggested that recurrent cerebral infarction cannot be prevented in patients with a symptomatic carotid web by conventional medical management alone. However, there is still no consensus on the treatment of carotid webs. Carotid artery stenting (CAS) with the CASPER stent (Microvention, Terumo, Tustin, CA, USA) was performed in six consecutive patients with symptomatic carotid webs, and the results are reported along with a review of the literature. Six consecutive patients with a diagnosis of internal carotid artery stenosis due to a carotid web on magnetic resonance imaging and digital subtraction angiography (DSA) were included in this study. All patients underwent dual antiplatelet therapy approximately 10 days before surgery and after 6 months, and then, a CASPER stent was implanted under general anesthesia. All patients were evaluated postoperatively by DSA 6 months after treatment. In all patients, no in-stent stenosis was seen 6 months after the operation, and no symptomatic cerebral infarction occurred within 1 year after the procedure. CASPER stent implantation may be effective for treating carotid webs.

Diagnostic criteria to classify severity of internal carotid artery (ICA) stenosis vary across vascular laboratories. Consensus-based criteria, proposed by the Society of Radiologists in Ultrasound in 2003 (SRUCC), have been broadly implemented but have not been adequately validated. We conducted a multicentered, retrospective correlative imaging study of duplex ultrasound versus catheter angiography for evaluation of severity of ICA stenosis. Velocity data were abstracted from bilateral duplex studies performed between 1/1/2009 and 12/31/2015 and studies were interpreted using SRUCC. Percentage ICA stenosis was determined using North American Symptomatic Carotid Endarterectomy Trial (NASCET) methodology. Receiver operating characteristic analysis evaluated the performance of SRUCC parameters compared with angiography. Of 448 ICA sides (from 224 patients), 299 ICA sides (from 167 patients) were included. Agreement between duplex ultrasound and angiography was moderate (κ = 0.42), with overestimation of degree of stenosis for both moderate (50–69%) and severe (⩾ 70%) ICA lesions. The primary SRUCC parameter for ⩾ 50% ICA stenosis of peak-systolic velocity (PSV) of ⩾ 125 cm/sec did not meet prespecified thresholds for adequate sensitivity, specificity, and accuracy (sensitivity 97.8%, specificity 64.2%, accuracy 74.5%). Test performance was improved by raising the PSV threshold to ⩾ 180 cm/sec (sensitivity 93.3%, specificity 81.6%, accuracy 85.2%) or by adding the additional parameter of ICA/common carotid artery (CCA) PSV ratio ⩾ 2.0 (sensitivity 94.3%, specificity 84.3%, accuracy 87.4%). For ⩾ 70% ICA stenosis, analysis was limited by a low number of cases with angiographically severe disease. Interpretation of carotid duplex examinations using SRUCC resulted in significant overestimation of severity of ICA stenosis when compared with angiography; raising the PSV threshold for ⩾ 50% ICA stenosis to ⩾ 180 cm/sec as a single parameter or requiring the ICA/CCA PSV ratio ⩾ 2.0 in addition to PSV of ⩾ 125 cm/sec for laboratories using the SRUCC is recommended to improve the accuracy of carotid duplex examinations.

Ultrasonography for the diagnosis of extra-cranial carotid occlusion - diagnostic test accuracy meta-analysis.

Background: The correct diagnosis of internal carotid artery (ICA) occlusion is crucial as it limits unnecessary intervention, whereas correct identification of patients with severe ICA stenosis is paramount in decision making and selecting patients who would benefit from intervention. We aimed to evaluate the accuracy of ultrasonography (US) in the diagnosis of ICA occlusion. Methods: We conducted a systematic review in compliance with the Preferred Reporting Items for a Systematic Review and Meta-analysis (PRISMA) of diagnostic test accuracy studies. We interrogated electronic bibliographic sources using a combination of free text and thesaurus terms to identify studies assessing the diagnostic accuracy of US in ICA occlusion. We used a mixed-effects logistic regression bivariate model to estimate summary sensitivity and specificity. We developed hierarchical summary receiver operating characteristic (HSROC) curves. Results: We identified 23 studies reporting a total of 5,675 arteries of which 722 were proven to be occluded by the reference standard. The reference standard was digital subtraction or cerebral angiography in all but two studies, which used surgery to ascertain a carotid occlusion. The pooled estimates for sensitivity and specificity were 0.97 (95% confidence interval (CI) 0.94 to 0.99) and 0.99 (95% CI 0.98 to 1.00), respectively. The diagnostic odds ratio was 3,846.15 (95% CI 1,375.74 to 10,752.65). The positive and negative likelihood ratio were 114.71 (95% CI 58.84 to 223.63) and 0.03 (95% CI 0.01 to 0.06), respectively. Conclusions: US is a reliable and accurate method in diagnosing ICA occlusion. US can be used as a screening tool with cross-sectional imaging being reserved for ambiguous cases.

Suitable methods of measuring acceleration time in the diagnosis of internal carotid artery stenosis

PurposeTo enhance the utility of acceleration time (AcT) in the diagnosis of internal carotid artery (ICA) stenosis, we assessed the value of AcT measurements with different waveform patterns.MethodsNinety-three patients with acute atherothrombotic cerebral infarction were enrolled, and they underwent both carotid ultrasonography and digital subtraction angiography (DSA). AcT was determined by a conventional procedure (using the first peak point or the bending point) and the peak systolic velocity (PSV) procedure. The AcT ratio was calculated as (AcT of ICA)/(AcT of the ipsilateral common carotid artery). We evaluated the correlation of stenosis rate as assessed by the North American Symptomatic Carotid Endarterectomy Trial method using DSA (DSA-NASCET) with the AcT of ICA (ICA-AcT), the AcT ratio measured by the conventional procedure (conventional AcT ratio), and the AcT ratio measured by the PSV procedure (PSV AcT ratio). The area under receiver operating characteristic curves (AUC) for DSA-NASCET was calculated based on the ICA-AcT and AcT ratio.ResultsForty-five vessels had 50% or greater ICA stenosis. DSA-NASCET was positively correlated with the conventional AcT ratio (r = 0.723), conventional ICA-AcT (r = 0.638), and PSV AcT ratio (r = 0.245). The corresponding AUCs for ICA stenosis ≥ 50% were 0.971, 0.886, and 0.572, respectively.ConclusionWe demonstrated the usefulness of the conventional procedure for diagnosing stenosis of ICA origin using AcT and showed that the AcT ratio was a more beneficial parameter than AcT.

External auditory canal haemorrhage as the first sign of internal carotid artery pseudoaneurysm: a rare case: a case report

Assessing the cause, severity of bleeding and strategies to control bleeding is crucial. We describe a rare case of a patient who was presented with epistaxis and left ear haemorrhage, as a probable complication of a ruptured internal carotid artery pseudoaneurysm. The massive haemorrhage compelled blood transfusion and clinical intervention. The diagnosis of internal carotid artery (ICA) pseudoaneurysm measuring 2.9 cm x 3.7 cm was concluded by computed tomography. Several coils were used to embolize the internal carotid artery pseudoaneurysm and arrest the bleeding with the guidance of an angiography. Coiling the pseudoaneurysm is highly recommended. Yet, the best methods to completely treat aneurysm are still in question. After the clinical intervention, the patient remained symptom-free and no episodes of bleeding were noted.

Background: Carotid duplex ultrasound (DUS) is widely used for diagnosis of internal carotid artery (ICA) stenosis and therapeutic decisions are often on the basis of DUS findings. There are no universal standard DUS criteria to grade the severity of ICA stenosis. A 2003 publication from the Society for Radiologists in Ultrasound (SRU) provided consensus criteria for grading ICA stenosis, but their integration into clinical practice is unknown. Methods: Random sample of 10% of vascular laboratories accredited in extracranial carotid testing. Data were abstracted from the ICAVL database, including most recent accreditation application. A single reviewer (HLG) compared laboratory diagnostic criteria to published references. Results: 152 vascular laboratories were sampled, 57.2% of which were outpatient based. Mean annual carotid volume 1,330 + 124.1 studies (range 100-9,858). There were 17 diagnostic criteria in use among 117 labs with the remaining facilities utilizing locally developed (6 labs) or unreferenced or hybrid criteria (29 labs). Mean number of categories of ICA stenosis was 5.80 + .084 (range 3-12). Cut-points for the mildest disease stratum ranged from 0 to 0-60% with normal or 0% most commonly reported (67.1% of labs). The cut-point for most severe (non-occlusive) stenosis ranged from 70 to 91% with 80% most commonly reported (64.5% of labs). SRU consensus-based criteria were used in only 27.0% of laboratories, although there was a statistically significant increase in SRU utilization among labs accredited after versus before 2005 (50.0% vs. 18.8%, P <0.001). Despite divergent diagnostic criteria and severity cut-points, self-reported accuracy of DUS for diagnosis of ICA stenosis from correlation studies was high (mean 85.8 + 0.65%). Conclusion: Diagnostic criteria and cut-points for grading severity of ICA stenosis using DUS vary widely in accredited vascular laboratories. Despite this, diagnostic accuracy is acceptable. Uptake of SRU consensus criteria has been limited, but is more common among recently accredited facilities. Health care providers must be aware of the lab-to-lab variation in reporting of ICA stenosis while further efforts to achieve consensus reporting are considered by professional societies.

MR angiography in internal carotid artery dissection: improvement of diagnosis by selective demonstration of the intramural haematoma.

We studied eight consecutive patients with clinical and intra-arterial angiographic (DSA) diagnosis of internal carotid artery (ICA) dissection to analyse the accuracy of MRA in dissections, using a new technique with presaturation pulses. Spin-echo images of the head were followed by three-dimensional time-of-flight (TOF) MRA at the site of the dissection, with and without a special caudal saturation pulse in addition to the cranial one. The accuracy of MRA was assessed in 64 segments of 16 ICA, all examined with DSA and MRA. High-signal intramural haematoma in the ICA at the level of the dissection was observed in all patients either on the maximal-intensity projection (MIP) reconstructions or on the partitions with this presaturation pulse technique. MRA had a sensitivity to detect dissected vessels of 100%. Specificity for vessels correctly identified as not having a lesion was also 100%. There was good correlation between DSA and MRA in demonstrating the site of the dissected ICA segment and the degree of stenosis. In only two cases was there overestimation of the degree of stenosis on the MIP reconstructions of the 3-D MRA.

Magnetic resonance angiography or IADSA for diagnosis of carotid pseudo occlusion?

Accurate diagnosis of internal carotid artery (ICA) occlusion is essential in the investigation of carotid disease yet may be difficult using Duplex. Traditionally contrast arteriography has been used to confirm the diagnosis despite its cost and potential dangers. Twenty-one patients with 23 ICA occlusions were evaluated by a 3-dimensional time of flight magnetic resonance angiography (MRA) technique. The cervical carotids and circle of Willis were imaged during the MRA examination which lasted 30 minutes. Confirmatory conventional angiography was performed in all patients. Using angiography as the gold standard, all occlusions were correctly diagnosed by MRA and 22 of 23 occlusions correctly diagnosed by Duplex. There was good agreement between MRA and angiography for all 42 ICAs imaged (Kappa statistic 0.83). Diagnosis of internal carotid artery occlusion is critical as it determines the need for operation. In this situation MRA provides a useful non-invasive complement to Duplex. A combination of non-invasive studies may enable arteriography to be rejected with greater confidence in this high risk group.

Non-invasive diagnosis of internal carotid artery dissections.

Arteriography is thought to be mandatory for the diagnosis of internal carotid artery (ICA) dissection. With the introduction of transcranial Doppler sonography (TCD) and magnetic resonance imaging (MRI), however, this is no longer the case. In 13 consecutive patients with ICA dissections the diagnosis was made by means of non-invasive tests including extracranial and transcranial Doppler sonography, contrast enhanced computed tomography (ceCT), and, in five patients, MRI. Intra-arterial digital subtraction angiography used as the gold standard in all cases was confirmative. Extracranial and transcranial ultrasound findings indicative of the diagnosis could be identified. MRI directly demonstrated the intramural haematoma and the false lumen of the dissected artery. These non-invasive techniques also allowed for repetitive follow up examinations. They were, however, unable to demonstrate false aneurysms in the chronic state. Results show that the diagnosis of carotid dissection can be made by means of cerebrovascular ultrasound and MRI.

Internal Carotid Artery Occlusion: Diagnostic Inaccuracy of the Ocular Pneumoplethysmography

The diagnosis of internal carotid artery (ICA) occlusion has serious management and prognostic consequences. The accuracy of noninvasive techniques in this setting remains questionable, with the result that contrast arteriography remains the diagnostic method of choice in patients in whom ICA occlusion is suspected. Ocular pneumoplethysmography-Gee (OPG-Gee) is one of the noninvasive methods that is still utilized extensively in the evaluation of carotid artery disease. In order to determine the real utility of OPG-Gee, the authors conducted a retrospective comparison of the results of OPG-Gee and arteriography in 26 patients (52 internal carotid arteries) who had undergone both examinations. [table: see text] With the angiography results as the standard of comparison, 4 incorrect diagnoses were obtained by OPG-Gee, thus yielding an overall accuracy of 92 for this method. However, duplex ultrasonography, also performed in the same 26 patients, correctly identified the 2 ICA occlusions defined by angiography and further demonstrated patency in the 48 arteries so shown by angiography. The authors conclude from this experience that OPG-Gee is less reliable than duplex ultrasonography in the diagnosis of ICA occlusion. Furthermore, despite the 92% overall accuracy of their results with OPG-Gee, the occurrence of both false-positive and false-negative results renders this examination modality suspect in the diagnosis of ICA occlusion.
