A Case of Chondromyxoid Fibroma Observed in the Nasal Cavity

Chondromyxoid fibroma (CMF) is an extremely rare, benign cartilaginous tumor found in the craniofacial region. It is often misdiagnosed due to its rare occurrence in the nasal and paranasal sinus; moreover, it has similarities with other cartilaginous tumors, showing the same radiologic and histopathologic features. A 62-year-old female consulted our clinic with the chief complaint of a left nasolabial mass of one-month history. The mass was located in the nasal cavity: from there it extended anteriorly to the anterior nasal spine, posteriorly to the end of the hard palate, and laterally to the maxillary sinus. The nasal septum was deviated to the right side due to the mass effect. Complete removal of the mass was conducted via the modified endoscopic Denker’s approach, and the diagnosis of CMF was established. During the 36-month follow-up, no recurrence was observed.

Imaging features of chondromyxoid fibroma: report of 15 cases and literature review.

Chondromyxoid fibroma (CMF) is a rare benign bony tumour. Our objectives are three-fold: first, comparing MRI, conventional radiography (CR) and CT characteristics of CMF; second, providing a literature review; and third, summarizing the role of imaging landmarks in the differential diagnosis with other bony lesions. 15 patients with histopathologically proven CMF were retrospectively included. MR images were reviewed for typical findings and compared with imaging features on CR and CT. All lesions were isointense on T1 weighted images with a low intensity rim in nine cases corresponding to the perilesional sclerosis on CR and CT. Internal trabeculations were more prominent on CR than on CT due to corrugation and scalloping at the tumour's edge. CT was superior to CR in analysing the expansion of the lesion, cortical breakthrough and internal mineralization. T2 short tau inversion recovery or T2 weighted fat saturation images showed an intermediate to hyperintense signal in all lesions, mostly heterogeneous. Some lesions contained small internal cysts, and one lesion demonstrated a low signal intensity centrally, corresponding to internal mineralization on CT. Intense contrast enhancement was present in all lesions. Minimal bone and soft tissue oedema were seen in, respectively, six and three patients. The diagnosis of CMF is difficult because of overlap of characteristics with other bony lesions. Our comparative study puts forward advantages and limitations of different imaging modalities in the diagnosis of CMF. For the first time, imaging features of CMF are analysed and compared on CR, CT and MRI.

Chondromyxoid Fibroma of the Calcaneus: Two Case Reports and Literature Review

Chondromyxoid fibroma occurs primarily in the long tubular bones of the lower extremity, with the foot representing the second most frequent location after the knee. This benign cartilaginous tumor of bone is currently the rarest reported neoplasm of cartilaginous origin. This mass can mimic other benign and malignant bone tumors owing to its variable histologic features. We report 2 cases of chondromyxoid fibroma of the calcaneus with varying presentations. Initially, advanced imaging studies pointed to a diagnosis of a unicameral bone cyst. Pathologic examination is difficult but can be used to differentiate this lesion from more serious conditions. A quick and accurate diagnosis of chondromyxoid fibroma can prevent unnecessary treatment that could be harmful to the patient.

Chondromyxoid Fibroma of the Orbit

A 51-year-old woman with a history of migraine headaches was found to have an incidental right orbital mass on MRI during neurologic evaluation for headaches. The orbital mass was a well-defined, lobulated, intraosseous soft tissue lesion with circumscribed margins. Clinically, there was noted proptosis, tenderness to palpation, and slight limitation to right abduction. An orbitotomy with incisional biopsy revealed a lesion arising within the lateral orbital rim extending to the subperiosteal space. Intraoperative frozen sections indicated a low grade sarcoma, possibly metastatic. The extraosseous component was excised, and the bone was curetted until all visible tumor was removed. A diagnosis of chondromyxoid fibroma was made. The patient did well until 5 months postoperatively, when right-sided proptosis returned due to recurrent tumor. Repeat surgical resection with removal of the lateral orbital rim was performed. Histopathology was consistent with recurrent chondromyxoid fibroma.

Chondromyxoid fibroma of C1: first case report

BACKGROUND: Chondromyxoid fibroma (CMF) is a rare, benign primary bone tumor. The cervical spine is an uncommon site for this tumor, with only 10 reported cases to date and none involving the first cervical vertebra (C1). CASE REPORT: Female patient, 25-year-old monozygotic female twin, presented with cervical pain. Radiographic imaging demonstrated a contrast-enhanced, right-sided lytic lesion of the insufflated type in C1, with a punched-out appearance and extending to the anterior arch. A postero-lateral and a posterior approach were performed in two steps to resect the tumor followed by occipitocervical fixation. Pathology confirmed the diagnosis of CMF. At one year, the patient remains disease free with excellent spinal stability. CONCLUSION: Spinal surgeons may need to treat rare spinal tumors. Despite the proximity to neural and vascular structures, the goal of surgery is always a radical resection due to high recurrence rates.

Clinical usefulness of thallium-201 scintigraphy and magnetic resonance imaging in the diagnosis of chondromyxoid fibroma

Chondromyxoid fibroma (CMF) is a benign bone tumor. However, it is sometimes difficult to distinguish this tumor from chondrosarcoma. We report a rare case arising from the proximal fibula, presenting multimodality imaging features. An 18-year-old man presented with a 2-year history of gradually increasing pain and swelling in his left knee. Radiograph showed an area of osteolysis with lobulation. Magnetic resonance (MR) imaging demonstrated that almost the whole lesion was enhanced with Gd-DTPA. Scintigraphy examination with (99m)Tc-biphosphonate showed strong accumulation in the periphery. On Ga-67-citrate scintigraphy, there was a little uptake. Thallium-201 scintigraphy showed strong accumulation of the whole lesion in early and late scans. The tumor was diagnosed as CMF by open biopsy. It is important that CMF is correctly distinguished from other tumors because this may be histologically overdiagnosed as chondrosarcoma. Contrast-enhanced MR imaging and thallium-201 scintigraphy may be useful to distinguish CMF from benign bone tumors or chondrosarcoma.

Chondromyxoid fibroma of the clavicle in a child

We present a 6-year-old child with a chondromyxoid fibroma (CMF) of the clavicle. In view of the atypical site, clinical features and imaging was not useful to pin point a diagnosis. The diagnosis was confirmed on histopathology. CMF is a rare benign bone tumor and only three cases of CMF clavicle are reported in world literature. Our patient is the first child reported with CMF of the clavicle. A very good result was obtained following an en-block resection of the clavicle. The report highlights the importance of keeping a diagnosis of CMF even at atypical sites like clavicle.

Chondromyxoid fibroma: radiographic appearance in 38 cases and in a review of the literature.

Thirty-eight cases of histologically confirmed chondromyxoid fibroma were reviewed and their radiographic features recorded. These findings, coupled with a review of the English-language medical literature, suggest that this rare, benign bone tumor has a characteristic but not specific radiographic appearance and may often mimic more common tumors. Chondromyxoid fibroma may occur anywhere in the skeleton, but almost half of the cases occur around the knee. The possibility of chondromyxoid fibroma should always be considered when a focal bone lesion is evaluated that has geographic bone destruction, a sclerotic rim, lobulated margins, and septation. The diagnosis of chondromyxoid fibroma is most likely when the patient is in the 2nd decade of life.

Chondromyxoid Fibroma in a Carpal Bone

An unusual lesion developed in the trapezoid of a 25-year-old man. The tumor had histologic features most consistent with a diagnosis of chondromyxoid fibroma and had not recurred four years after curettage and bone grafting.

We reviewed the records of all patients with the diagnosis of chondromyxoid fibroma at the Bone Tumor Center of the Istituto Ortopedico Rizzoli. The records of twenty-seven patients were available for the study of clinical, radiographic, and pathological findings Surgical treatment was evaluated in twenty two patients with a follow-up of longer than two years; their average follow-up was 117 months (range, twenty-four to 380 months) Seventeen patients had been followed for at least five years. A questionnaire including a request for radiographs and questions pertaining directly to functional status was sent to the patients or their physicians Seventeen (77 per cent) of the patients responded, and two other patients were examined by us; the remaining three patients had died due to unrelated causes, with no evidence of tumor at the time of death. In our experience, the lesion was most commonly seen in the tibial metaphysis of adolescent boys. Symptoms of pain and swelling may precede detection of the lesion by years. Our results are similar to those reported by other authors. There was an over-all rate of recurrence of 27 per cent. With curettage alone there was an 80 per cent recurrence rate, but when curettage was combined with corticocancellous bone-grafting the recurrence rate decreased to 7 per cent. Two of the recurrences had been treated by amputation in the mistaken belief that the lesion was malignant The remaining recurrences were successfully treated by wide excision. The recurrence rate was not found to be dependent on the age of the patient or on an atypical histological appearance. On the basis of this experience, we believe that thorough curettage combined with corticocancellous bone-grafting remains the treatment of choice for patients with chondromyxoid fibroma.
