The influence of training with free-fight means on motor skills, sensory functioning and quality of life of a child with autism (unique case)

The motor sphere of children with autism is characterized by the presence of stereotyped movements, difficulties in the formation of objective actions and household skills, and violations of fine and gross motor skills.
 The aim of the work was to establish the influence of adaptive training with elements of free-fight on the development of motor skills, sensory profile and quality of life of a child with autism.
 A 12-year-old boy A. with a diagnosis of childhood autism, social maladaptation (disability subgroup A) was chosen for the experiment. The child does not speak, cannot read or write, has impaired coordination of movements (walking on tiptoes, motor stereotypies, motor disinhibition), insufficient understanding of language. Somatically, the boy is healthy, loves motor activity.
 An assessment of the child's sensory functioning was carried out according to the Short Sensory Profile method, developed and recognized by The Psychological Corporation (USA), a survey of parents about the child's quality of life before the start of training and after a forced break in training, in addition, an assessment of the quality of direct blows with the hand and foot was carried out punching bags at each session.
 During the research period, the boy A. performed, under the guidance of a coach, exercises for coordination of movements ("obstacle course"), interaction with the coach (gymnastic exercises, various available games with a ball), exercises for developing the strength of various muscle groups (work with "Bulgarian bag", dumbbells) and special free-fight exercises (hits with hands and feet on punching bags and a dummy).
 A significant improvement of the child's sensory functioning was established, namely all types of sensitivity, except auditory filtering and sensitivity to taste and smell. The child's parents noted a significant increase in the quality of life of both the boy himself and the entire family. Significant progress was observed in learning the technique and the success of the boy's performance of special free-fight exercises. Adaptive training with elements of free fight served as a significant sensory stimulus for a child with autism, contributed to the learning of motor skills, and improved the quality of life of the family.

“I received a leaflet and that is all”: Father experiences of a diagnosis of autism

Accessible summary Little is known about what fathers think and feel about a diagnosis of autism. Fathers experienced strong emotion around diagnosis and were given little support. Services should support mother and fathers in ways which meet their individual needs. More positive messages should be given to families at diagnosis. AbstractBackground: A diagnosis of childhood autism can be an extremely stressful experience for parents, but little is known concerning paternal perceptions of the process.Methods: This study investigated father perspectives on a diagnosis of autism, through an online survey.Results: An analysis of 184 replies to an open‐ended question identified the following themes: strong initial emotional response and a range of immediate anxieties about the future, struggle to gain a diagnosis; anger in response to insensitive delivery of diagnosis together with insufficient information at the time and lack of support afterwards.Conclusion: Fathers experienced a range of significant challenges during a diagnostic process in which most felt unsupported. Service provision should be more gender‐differentiated, taking into account of the particular needs and perspectives of both mothers and fathers at this critical juncture in family life.

ICD-10 and alternative diagnostic criteria for childhood autism among children with intellectual disability.

The diagnosis of Childhood Autism (CA) among children with Intellectual Disability (ID) based on clinical criteria validated for populations with average intelligence compromises it's diagnostic accuracy in this special population. This study documents the diagnostic accuracy of ICD-10 and alternative criteria for CA in ID population. Consensus clinical diagnosis of autism made by a multi-disciplinary team as the reference data were extracted from the case notes and psychological tests details by two trainee psychologists, and ICD-10 based clinical diagnosis, demographics, ID, CA related data documented independently of the psychiatrist. The appropriate statistical analyses were completed. Repetitive behaviors formed the most common symptom cluster. The high internal consistency (κ = 0.75) among the three groups of ICD-10 symptoms indicate their usefulness in the diagnosis of CA among children with ID, but significantly more children with ID failed to meet the ICD-10 criteria for CA. The fourth alternative criteria had the highest concordance with the reference standard (100%) and the first alternative criteria had the highest agreement with the reference standard (κ = 0.88) in identifying CA among ID. The diagnostic accuracy of all the alternative criteria was significantly better than that of ICD-10 with the first alternative criteria having the best diagnostic accuracy (Sn = 98%; Sp = 100%; PPV = 1; NPV =0.83) closest to the reference standard. The symptom clusters in ICD-10 for CA should be retained, however reorganising the diagnostic criteria is required for diagnosing CA accurately among children with ID. The first alternative criteria can significantly improve the case identifying ability and diagnostic accuracy altering there by the epidemiological data on Childhood Autism among children with Intellectual Disability.

A Comparison of Autism Prevalence Trends in Denmark and Western Australia

Prevalence statistics for autism spectrum disorders (ASD) vary widely across geographical boundaries. Some variation can be explained by diagnostic methods, case ascertainment and age at diagnosis. This study compared prevalence statistics for two distinct geographical regions, Denmark and Western Australia, both of which have had population-based registers and consistent classification systems operating over the past decade. Overall ASD prevalence rates were higher in Denmark (68.5 per 10,000 children) compared with Western Australia (51.0 per 10,000 children), while the diagnosis of childhood autism was more prevalent in Western Australia (39.3 per 10,000 children) compared with Denmark (21.8 per 10,000 children). These differences are probably caused by local phenomena affecting case ascertainment but influence from biological or geographical factors may exist.

Assessment of knowledge about childhood autism among paediatric and psychiatric nurses in Ebonyi state, Nigeria

BackgroundThere is increasing public and professional awareness of autism spectrum disorders with early recognition, diagnosis and interventions that are known to improve prognosis. Poor knowledge about childhood autism among paediatric and psychiatric nurses who are members of multidisciplinary teams that care for such children may be a major barrier to early interventions that could improve quality of life and prognosis in childhood autism. Factors that influence knowledge about childhood autism among these nurses are not known. This study assessed knowledge about childhood autism among paediatric and psychiatric nurses in Ebonyi state, Nigeria and determined the factors that could be influencing such knowledge.MethodsForty specialist paediatric and forty psychiatric nurses, making a total sample of eighty, were randomly selected from all the health care facilities in Ebonyi state, Nigeria. A socio-demographic questionnaire and knowledge about childhood autism among health workers (KCAHW) questionnaire were administered to them and the study was a point survey.ResultsThe total mean score on the KCAHW questionnaire among the nurses that participated in the study was 12.56 ± 3.23 out of a total of 19 possible. The mean score for the paediatric nurses was 11.78 ± 3.64 while psychiatric nurses had mean score of 13.35 ± 2.58. The mean scores in Domain 1 were 6.17 ± 1.75 for the paediatric nurses and 6.52 ± 1.43 for the psychiatric nurses. The mean scores in Domain 2 were 0.65 ± 0.48 for the paediatric nurses and 0.80 ± 0.41 for the psychiatric nurses. Domain 3 showed mean scores of 1.97 ± 1.25 for the paediatric nurses while psychiatric nurses scored 2.62 ± 1.23. Domain 4 yielded the mean scores of 2.97 ± 1.54 and 3.42 ± 0.98 for the paediatric and psychiatric nurses respectively.There was significant relationship between the total mean score on the KCAHW questionnaire for the two groups and the area of specialisation of the nurses (t = -2.23, df = 78, p = 0.03) and there was also significant relationship between previous involvement in managing children with childhood autism as a specialist paediatric or psychiatric nurse and the total mean score on the KCAHW questionnaire (t = 6.90, df = 78, p = 0.00).ConclusionThe scores reflect deficits in knowledge about childhood autism among the study cohorts. Paediatric and psychiatric nurses as members of multidisciplinary teams that care for children with childhood autism are expected to provide holistic care and adequate counselling to the families of these children. Unfortunately in this environment, they are not fully equipped with enough knowledge about childhood autism. Education on childhood autism is therefore needed and can be provided through continuing medical education and emphasizing childhood autism in their training curriculum. This will enhance early identification and diagnosis of childhood autism with early interventions that are known to improve prognosis.

The Effects of Fluorescent and Incandescent Lighting on the Repetitive Behaviours of Autistic and Intellectually Handicapped Children

Repetitive behaviours of five autistic and five intellectually handicapped children were observed under both fluorescent and incandescent lighting conditions. The findings of this study support the hypothesis that autistic children engage in a significantly greater frequency of stereotypies under fluorescent as compared to incandescent lighting, and that there is no significant difference in the frequency of stereotypic behaviours among intellectually handicapped children when exposed to the different lighting conditions. Implications of the findings are discussed in terms of the lighting requirements of therapeutic settings and the differential diagnosis of childhood autism.
