Research Trend Analysis of Community-based Occupational Therapy in Korea through Key Word Network Analysis

Objectives: For this study, we analyzed research trends of community-based occupational therapy in Korea using key word network analysis (KNA). Methods: Our sample was 155 studies in the Journal of the Korean Society of Community-Based Occupational Therapy in Korea. We used key word network analysis (KNA), a method used to examine the flow and trends of research by using key words that represent the core topics of the literature, allowing for the identification of relationships, influence, and importance of key topics in a specific field. The analysis was divided into 2 parts – network distribution and centrality. Indicators for network distribution include average degree, density, mean distance, inclusiveness, and the number of isolated nodes. Network centrality is assessed through degree centrality, betweenness centrality, and eigenvector centrality. Results: The analysis of network distribution indicated that the average degree was 7.064, density was .024 (2.4%), mean distance was 3.195, and there were no isolated nodes. Key words with the highest degree centrality values were occupational therapy, stroke, elderly, quality of life, activity of daily living, and occupational therapist. Key words with the highest betweenness centrality values were occupational therapy, stroke, quality of life, elderly, occupational therapist, and depression. Key words with the highest eigenvector centrality values were curriculum, service provider qualification, sensory integration therapy, sensory developmental rehabilitation, developmental rehabilitation service, and program development. Conclusions: These results have important academic implications regarding the research trend of community-based occupational therapy in Korea. These findings can enhance understanding of community-based occupational therapy and provide guidance for future studies. Particularly, the analysis results of degree centrality and cohesion structure offer insights into various aspects of occupational therapy, facilitating the development of more effective treatment approaches. It is anticipated that these results will contribute to practical value in clinical settings.

Job Conditions and Practice of Private Office-Based Speech-Language Pathologists in Korea: A Cross-Sectional Survey

Purpose: Private speech therapy centers remain an ongoing concern, especially as the primary working environment for speech-language pathologists (SLPs) in Korea. The purpose of this study is to investigate the current working conditions and contents of the speech therapy services of private practice-based SLPs from the perspectives of employed SLPs to find factors that can positively affect future job retention, recruitment, and satisfaction.Methods: This study conducted a web-based survey across Korea targeting private practicebased SLPs. A total of 265 SLPs were invited to participate, and 219 completed the response. The study was divided into three parts, covering demographic information, work type, environment information, service contents, and client-related information, to ensure a comprehensive understanding of the current landscape.Results: Regular full-time jobs with social insurance were the most employed types of Korean SLPs in private practice. Fisher’s exact tests showed no significant effects on the type of employment and salary according to the kind of certificate. The location of the private speech therapy center affected the unit price per session. In addition, there was no significant difference in salary level depending on the employment conditions and certificate type.Conclusions: The findings of this study are significant for the field of speech therapy. Despite being the main service of private speech therapy centers, the costs related to evaluation and treatment were found to have lower unit fees than other psychological and developmental rehabilitation service areas. This disparity could potentially impact the recruitment and job retention of private practice-based SLPs. Therefore, it is crucial to consider supporting incentives based on the type of certificate and career, along with fee adjustments suitable for the current reality, to address these issues.

A Study on the Perception of Parent Counseling Status and Language Rehabilitation Effectiveness according to Language Therapy Experience of Developmental Rehabilitation Services

본 연구는 언어재활사와 발달재활서비스의 언어치료를 받고 있는 대상자 부모 상담 언어발달 치료의 효과성 인식에 대하여 조사하고 더욱 효과적인 언어재활 제공의 기초 자료를 제공하고자 하였다. 본 연구는 대상자들의 특성과 상담 실태를 파악하기 위하여 빈도분석을 실시하였다. 치료기간에 따른 효과성 인식에 대한 차이를 알아보기 위하여 일원배치분산 분석을 실시하였다. 사후분석은 Bonferroni 사후 검정을 실시하였다. 셋째, 효과성 인식 항목들 간의 상관관계를 알아보기 위하여 Spearman 상관분석을 실시하였다. 본 연구의 결과는 다음과 같았다. 부모 상담 현황에 따르면, 상담의 시기는 주로 치료 후에, 상담 빈도는 매회기 마다, 주 상담 내용은 치료 회기 동안의 치료 내용과 결과에 대하여, 주 상담 방법은 대면상담인 것으로 조사되었다. 언어치료 효과성 인식 조사에서, 보호자들은 언어치료가 의사소통능력, 사회성, 자존감, 자기 이해력, 수업 적응력, 학습 능력의 향상과 생활 스트레스 감소에 효과가 있는 것으로 인식하는 정도가 높은 것으로 조사되었다. 언어치료 기간에 따른 언어치료 효과 인식 간 차이도 살펴보았으며 언어치료의 기간이 길어짐에 따라 효과성 인식은 높아지는 것으로 조사되었다. ‘언어치료에 대해 얼마나 알고 있는가?’ 항목은 다른 효과성 항목들과 상관관계를 나타내었다. 위 결론을 살펴본 결과, 언어재활사와 부모 간 다양한 상담방법을 통하여 부모와 언어재활사의 신뢰적인 관계를 형성하고 언어치료의 효과를 부모에게 인식시킬 수 있으며 전반적인 언어장애와 치료의 인식을 높이기 위한 정보가 상담 시 제공되어야 한다.

발달재활서비스 이용자 부모의 선택과 참여행동이 서비스 질과 만족도에 미치는 영향에 대한 다층 분석

본 연구는 세종시 발달재활서비스 바우처 시장에서 이용자 부모의 선택과 참여행동이 서비스 질과 만족도에 미치는 영향에 대해 실증 분석하는 것을 목적으로 한다. 분석 자료로는 세종시 사회서비스원에서 2020년 조사한 발달재활서비스 제공 기관 전수 자료와 기관의 이용자 자료를 활용하였다. 분석방법으로는 이용자 차원의 요인과 기관 차원의 요인을 모두 고려하기 위해 다층모형(multilevel model)을 수행하였다. 분석 결과 발달재활서비스 이용자 부모의 서비스 기관 선택과 인력 선택이 서비스 질과 만족도에 유의미한 영향을 미치지 않았다. 대신 서비스 이용자 부모가 기관의 서비스 제공 과정에 직접 참여하는 행동 즉 이용계획 공동수립과 공식 창구를 통한 불만접수 행동이 서비스 질과 만족도에 유의미한 영향을 미치고 있었다. 기관 차원의 요인으로는 영리기관 여부, 지원금 비율이 서비스 질과 만족도에 유의미한 영향을 갖고 있었다. 이러한 결과를 근거로 하여 결론에서는 발달재활서비스 바우처 시장에서 서비스의 질과 만족도는 시장원리를 배경으로 하는 이용자 선택보다 참여행동을 통해 높아질 수 있다고 보았다. 그리고 서비스 이용계획의 공동수립, 불만 접수를 위한 창구의 설치, 이용자 참여 지원 정책의 필요성, 영리기관 지원의 필요성 등을 논의하였다.

발달재활서비스 품질요인 규명에 관한 질적 연구

본 연구는 발달재활서비스 제공현장에서 활동하고 있는 9명의 연구참여자(운영자 2명, 제공인력 7명)의 경험과 체험을 현상학적 연구를 통해 발달재활서비스의 품질요인을 규명하고자 하였다. 품질을 결정짓는 요소들이 무엇인지에 대해 제공기관에게 요구되는 품질요소와 제공인력에게 필요한 품질요소 그리고 서비스의 목적을 달성할 수 있는 품질요소 등의 세 가지 연구문제에 대해 반구조화된 인터뷰를 활용하여 자료를 수집하고, 반복적 비교분석법을 통해 3개의 대주제와 13개의 하위주제를 도출하였다. 그 결과는 첫째로, 서비스의 신뢰성, 협력관계, 문제대응능력, 관리체계, 근무여건을 포함하는 서비스 구조화 강화 노력과, 둘째로, 서비스의 책무성과 준비된 서비스 그리고 서비스의 전문성을 대표하는 서비스 적합성 제공 노력, 셋째로, 성과관리체계, 전문가의 수퍼비전, 가족의 협조, 정보의 공유, 통합적 계획 수립 등의 서비스 효과성 확보 노력 등이었다. 이상과 같이, 본 연구는 재활과 치료가 중심이 되는 발달재활서비스 과정을 통해 장애아동의 삶의 수준을 높일 수 있는 제공기관과 제공인력의 노력이 무엇인지 살펴보고, 그 과정에서 서비스의 질적 수준, 즉, 품질에 영향을 미치는 요인들을 질적 연구의 방법을 통해 규명해봄으로써 발달재활서비스의 품질특성을 체계화하는 토대를 마련하였다는데 큰 의미가 있으며, 발달재활서비스의 품질관리를 위해 품질지표의 지향점을 확인할 수 있는 유의미한 결과를 도출하였다는 점도 중요한 성과라 하겠다.

A grounded theory of parents’ attendance, participation and engagement in children’s developmental rehabilitation services: Part 2. The journey to child health and happiness

Purpose: Parents’ attendance, participation and engagement are thought to be critical components of children’s rehabilitation services; however, these elements of therapy are typically under-investigated. The purpose of this study was to develop a substantive theory of parents’ attendance, participation and engagement in children’s rehabilitation services.Methods: A constructivist grounded theory study was conducted. Data collection included interviews with parents (n = 20) and clinicians (n = 4), policies regarding discharge, and child-health records. Data was analyzed using constant comparison, coding and memoing. To promote credibility, authors engaged in reflexivity, peer debriefing, member checking, triangulation and recorded an audit trail.Results and conclusions: The Phoenix Theory of Attendance, Participation and Engagement was developed. This theory is described metaphorically as a journey to child health and happiness that has six components including: parent’s feelings, skills, knowledge, logistics, values and beliefs and parent’s relationship with the professional. The child, parent, service provider, and organizational factors that impact engagement are described. Service providers, policy makers, organizational leaders and researchers can use this information to promote engagement in children’s developmental rehabilitation services.

Parents' attendance, participation and engagement in children’s developmental rehabilitation services: Part 1. Contextualizing the journey to child health and happiness

Purpose: A family-centered approach to developmental rehabilitation services emphasizes that children grow and develop in their family environment, and that parents are an integral part of therapy. The purpose of this study was to develop a theory of how parents attend, participate and engage in their child’s developmental rehabilitation services.Methods: A constructivist grounded theory study was conducted. Recruitment was done at a single children’s treatment center in Ontario, Canada and data collection included parent and clinician interviews, policy collection and child-health record review. Grounded theory methods of constant comparison and theoretical sampling were applied. Credibility was enhanced through reflexivity, triangulation, and peer debriefing.Results and conclusions: The results show that in order to understand how parents engage in therapy, we must understand the conditions related to family health and service complexity that affect parent engagement. This contextual understanding may increase service providers’ sensitivity to the conditions that affect engagement as they join families on their journey towards child health and happiness.Implications for rehabilitationService providers can understand the ease or difficulty that parents may experience in attending, participating and engaging in their child’s therapy if they ask about (i) who is a part of the family, (ii) the health of all family members, and (iii) the number of services and professionals that are involved with the familyAsking parents about their own mental health and ensuring they are connected with appropriate resources may help them to attend, participate and engage in their child’s rehabilitation service.When families are involved with multiple services and professionals, they may benefit from support with service navigation and a collaborative interprofessional approach to care.

ë°°ê²½ 및 ëª©ì  발달재활서비스 지원금은 물가나 ìµœì €ìž„ê¸ˆ 등의 상승을 ê³ ë ¤í•˜ì§€ 않은 채 10여 ë „ì´ 지난 현재까지 동일하게 ìœ ì§€ë˜ê³ ìžˆì–´ 질 높은 재활치료서비스를 ì œê³µí•˜ì§€ ëª»í•˜ê³ ìžˆë‹¤. 이에 본 연구는 발달재활서비스를 이용하는 아동의 부모를 대상으로 국내 발달재활서비스 지원금에 대한 인식 및 발달재활서비스 이용 현황, 특히 언어치료서비스의 이용 현황에 대한 인식을 ì¡°ì‚¬í•˜ê³ ìž 하였다. 방법 발달재활서비스를 이용하는 아동의 부모를 대상으로 발달재활서비스 지원금 인식, 발달재활서비스 이용 현황과 언어치료 발달재활서비스 이용 현황에 관한 24문항으로 구성된 설문지를 ë°°ë¶€í•˜ê³ ì„±ì‹¤í•˜ê²Œ 응답된 276부의 자료를 분석하였다. ê²°ê³¼ 발달재활서비스 현황에서 현행 발달재활서비스 지원금이 ì ì •í•˜ì§€ ì•Šë‹¤ê³ ì‘ë‹µí•œ 비율이 85%ì˜€ê³ , 31-40만 원이 가장 ì ì •í•œ ì§€ì›ê¸ˆì´ë¼ê³ ì‘ë‹µí•œ 비율이 가장 높았으며, 발달재활서비스 이용기간이 길수록 ì ì •í•˜ë‹¤ê³ ì¸ì‹í•˜ëŠ” 발달재활서비스 지원금의 액수가 높아졌다. 아동이 현재 발달재활서비스로 ì œê³µë°›ëŠ” 치료의 개수는 2개가 가장 많았으며, 가장 í•„ìš”í•˜ê³ ê°€ìž¥ 많이 이용하는 치료 ìœ í˜•ì€ 언어치료였다. 발달재활서비스를 이용한 ì›”í‰ê· ì–¸ì–´ì¹˜ë£Œ 횟수는 4회가 가장 많았으나 ì ì •í•˜ë‹¤ê³ ì¸ì‹í•˜ëŠ” 언어치료 횟수는 8회가 가장 많았다. 발달재활서비스 지원금 외 추가로 부담하는 자부담금은 응답자의 약 74%가 80,000원 이상을 ì§€ë¶ˆí•œë‹¤ê³ í•˜ì˜€ë‹¤. 또한 응답자의 약 94%가 아동이 성인이 된 후에도 ì •ë¶€ë¡œë¶€í„°ì˜ 치료지원금이 í•„ìš”í•˜ë‹¤ê³ í•˜ì˜€ë‹¤. ë ¼ì˜ 및 ê²°ë¡ ìž¥ì• ì•„ë™ì˜ 치료지원을 위해 ë§ˆë ¨ëœ 발달재활서비스는 현행 ì œê³µë˜ëŠ” 지원금이 이용 아동의 부모들이 인식하기에 충분하지 않으며 지원금의 인상이 반드시 í•„ìš”í•˜ë‹¤ê³ ë³´ì—¬ì§„ë‹¤. A whole new world: a qualitative investigation of parents’ experiences in transitioning their preterm child with cerebral palsy to developmental/rehabilitation services

ABSTRACTBackground: Parents’ experiences transitioning their children from neonatal to developmental/rehabilitation services (DRS) are unknown.Methods: A qualitative descriptive approach was used, including interviews with 18 parents (13 mothers and 5 fathers) of children born preterm and diagnosed with cerebral palsy (CP), located in a large urban center in Canada. Interview data underwent thematic analysis.Results: Parents’ experiences with transition to DRS were a whole new world with three key themes: Wanting to know what to expect, feeling supported in their transition, and getting there emotionally and physically. Transition broke an emotional bond with neonatal services while parents were simultaneously entering DRS, experiencing their child’s CP diagnosis, and reliving prior emotional trauma.Conclusions: The findings reveal a cumulative emotional burden for parents in the first 3 years of life; a known critical period for parenting and early childhood development. Early transition interventions should consider including enhanced supports and services for parents.

Improving Transition Practices: Understanding Family Experiences and Needs in Transitioning Their Child to Developmental/Rehabilitation Services

Abstract BACKGROUND: Children born preterm (<37 weeks) with cerebral palsy (CP) have long-term complex medical needs that require transitions between multiple healthcare services, necessitating parental support. The experiences of parents undergoing early transitions from the neonatal intensive care unit (NICU) to neonatal follow-up clinics (NFUC) to home are well-documented. Our prior research conducted found parents described positive transitions as: access to information, feeling involved in care and witnessing improvement in their child’s health outcomes post-transition. Negative experiences entail: unanticipated differences between healthcare services, feeling separated from their child and having inadequate information. Unfortunately, the next transition from children with CP born preterm (<37 weeks) to developmental/rehabilitation services (DRS) is grossly under-studied and knowledge from well-studied healthcare transitions has yet to be applied to this population. OBJECTIVES: To identify and describe the experience and needs of parents of children with CP born preterm, as they transition from NFUC to DRS. Additionally, to make recommendations building upon characteristics of supportive/positive transition experiences to meet the emotional, informational, and instrumental needs of parents during transition to DRS. DESIGN/METHODS: A descriptive qualitative study was conducted recruiting English-speaking parents of children born preterm with CP that required NICU hospitalization, and DRS post CP diagnosis. Purposeful and maximum variation strategies were used to recruit a sample of 15 parents. To ensure rigor, sampling continued until data saturation was reached. Interviews were audio-recorded, transcribed verbatim, coded using NVivo 10 software and underwent thematic analysis. Data collection and analyses were performed concurrently, with themes from early interviews incorporated, collapsed and used to inform later interviews. RESULTS: A majority of participants were mothers, whose child had received DRS for the past one to 12 years. Preliminary themes iterated by parents’ transition experiences included: Poor communication of diagnosis caused anxiety surrounding transition of care; a lack of timely access to appropriate care and resources negatively affected parents’ experiences; continuity and consistency of healthcare providers improved families’ perception of accessibility to care; and social support was desired throughout the transition period. CONCLUSION: The transition between NFUC and DRS for families with CP is significant, but lacks best practice research to advise care. Preliminary recommendations derived from data reveal the need to better support families earlier on in the transition process, beginning at the NFUC. Further research is required to identify best practices to strengthen communication, family involvement, continuity of care, and timely access to informational supports for parents.

시계열 모형을 활용한 사회서비스 수요·공급모형 구축 : 발달재활서비스를 중심으로

The primary goal of the study is to examine the possibility of applying the time series model to forecasting demand and supply of social services. In the study, we used survey data based on a nationally represented sample which is secondary processed data. We selected developmental rehabilitation service. The analysis, we made models of a demand and a supply using time series analysis. Utilizing the estimates, we identified each model`s pattern. This study provides an empirical evidence to suggest benefits of using the time series model for forecasting the demand and the supply pattern of newly introduced social services. We also provide discussions on policy implications of utilizing demand and supply time series models in the process of developing new social services.

Proposal on the Use of Assessment Tools for Selection of Developmental Rehabilitation Service Participants

이 연구는 발달재활서비스 대상자 선정을 위한 진단ㆍ평가 과정에서 활용될 수 있는 진단ㆍ평가 검사도구들의 정보를 제공하여 전문가들이 검사도구 선택의 혼선을 줄이고 객관적으로 장애아동이 필요로 하는 서비스를 평가할 수 있도록 도와주고자 검사도구의 일반적인 정보제공과 동시에 활용방안에서 논하고자 하였다. 이 연구에서 조사한 검사도구는 총 218개로 인지, 신체발달, 감각ㆍ운동, 적응ㆍ행동, 정서ㆍ심리, 학습, 의사소통, 장애특성의 8개 영역으로 분류하여 검사도구를 정리하였으며 구체적으로 분석하였다. 그 결과, 국외 편을 그대로 번안하여 사용하는 검사도구는 105개로 국내 실정에 맞추어 개발되거나 표준화하지 않은 검사도구가 많았으며, 영역별 하위영역을 평가하거나 측정하는 검사도구의 수가 부족한 경우도 있었다. 또한 국외에서 개발된 검사도구를 국내에서 표준화하여 개발한 년도가 많게는 30년 이상 차이가 나고 있었고 최신 버전의 검사도구가 부재한 경우가 많았다. 그리고 국내에서 표준화를 실시하였다고 하더라고 어떤 검사도구의 표준화 지역이 1곳 이였던지 표준화 인원수가 500명 미만을 대상으로 표준화를 했다던지, 각기 다른 기준으로 두고 실시를 하고 있었다. 이러한 분석 결과를 토대로 검사도구의 활용 방안을 제시해 보았다.
