Outcomes of Total Knee Arthroplasty in Patients Who Have Ehlers-Danlos Syndrome: A Matched Cohort Study

BackgroundTotal knee arthroplasty (TKA) is an excellent surgical option for patients who have end-stage knee osteoarthritis. While rates of major postoperative complications have steadily decreased with modern implants and operative techniques, contemporary outcome data for patients who have Ehlers-Danlos syndrome (EDS) are scarce. The goal of this study was to compare complication rates after primary TKA in patients who have EDS versus matched controls. MethodsA large administrative database was used to identify patients who underwent primary TKA from 2009 to 2020. Patients who had a diagnosis of EDS were identified by International Classification of Diseases Coding. Propensity scores were utilized to match these patients with controls at a 1:4 ratio based on age, sex, and various comorbidities. Multivariable logistic regression analysis was used to compare the rates of medical and surgical complications at 90 days and 2 years. A total of 188 patients who had EDS and 752 controls were included in this study. ResultsAfter univariate analysis, Ehlers-Danlos patients exhibited significantly higher rates of wound complications (4.8 versus 0.9%, P = .001) at 90 days. When adjusted for comorbidities, Ehlers-Danlos patients still exhibited significantly increased odds of developing wound complications (odds ratio: 7.06; P < .001). ConclusionsPatients who have EDS undergoing TKA exhibited significantly higher rates of wound complications within 90 days postoperatively compared to matched controls. Rates of instability, manipulation under anesthesia, periprosthetic joint infection, aseptic loosening, and aseptic revision arthroplasty did not significantly differ between the cohorts. This study found generally favorable short-term outcomes of TKA in this population; however, the inability to control for implant type and other confounding variables may have influenced the lack of difference in complication rates at 2 years. Surgeons should monitor for the potentially increased risk of wound complications and consider the possible need for increased constraint in this population during preoperative planning.

Scalp incision technique for decompressive hemicraniectomy: comparative systematic review and meta-analysis of the reverse question mark versus alternative retroauricular and Kempe incision techniques

Decompressive hemicraniectomy (DHC) is a critical procedure used to alleviate elevated intracranial pressure (ICP) in emergent situations. It is typically performed to create space for the swelling brain and to prevent dangerous and potentially fatal increases in ICP. DHC is indicated for pathologies ranging from MCA stroke to traumatic subarachnoid hemorrhage—essentially any cause of refractory brain swelling and elevated ICPs. Scalp incisions for opening and closing the soft tissues during DHC are crucial to achieve optimal outcomes by promoting proper wound healing and minimizing surgical site infections (SSIs). Though the reverse question mark (RQM) scalp incision has gained significant traction within neurosurgical practice, alternatives—including the retroauricular (RA) and Kempe incisions—have been proposed. As choice of technique can impact postoperative outcomes and complications, we sought to compare outcomes associated with different scalp incision techniques used during DHC. We queried three databases according to PRISMA guidelines in order to identify studies comparing outcomes between the RQM versus “alternative” scalp incision techniques for DHC. Our primary outcome of interest in the present study was postoperative wound infection rates according to scalp incision type. Secondary outcomes included estimated blood loss (EBL) and operative duration. We identified seven studies eligible for inclusion in the formal meta-analysis. The traditional RQM technique shortened operative times by 36.56 min, on average. Additionally, mean EBL was significantly lower when the RQM scalp incision was used. Postoperatively, there was no significant association between DHC incision type and mean intensive care unit (ICU) length of stay (LOS), nor was there a significant difference in predisposition to developing wound complications or infections between the RQM and retroauricular/Kempe incision cohorts. Superficial temporal artery (STA) preservation and reoperation rates were collected but could not be analyzed due to insufficient number of studies reporting these outcomes. Our meta-analysis suggests that there is no significant difference between scalp incision techniques as they relate to surgical site infection and wound complications. At present, it appears that outcomes following DHC can be improved by ensuring that the bone flap is large enough to enable sufficient cerebral expansion and decompression of the temporal lobe, the latter of which is of particular importance. Although previous studies have suggested that there are several advantages to performing alternative scalp incision techniques during DHC, the present study (which is to our knowledge the first to meta-analyze the literature on outcomes in DHC by scalp incision type) does not support these findings. As such, further investigations in the form of prospective trials with high statistical power are merited.

A NSQIP study comparing surgical outcomes between primary and non-primary TEPs after total laryngectomy

ObjectiveTracheoesophageal puncture with voice prosthesis (TEP) is considered the gold standard for voice rehabilitation after total laryngectomy; however, there is debate as to whether it should be inserted concurrently with removal of the larynx (primary TEP), or as a separate, additional procedure at a later date (secondary TEP). We utilized the National Surgical Quality Improvement Program Database (NSQIP) to compare postoperative complications, readmission rates, and reoperation rates among individuals who underwent total laryngectomy with or without concurrent TEP placement. MethodsWe conducted a retrospective study using the American College of Surgeons National Surgical Quality Improvement Program database (ACS-NSQIP) from 2012 to 2019. Patients were categorized into primary and non-primary TEP groups using a variation of CPT codes for total laryngectomy, tracheoesophageal prosthesis, and type of reconstruction. Univariate analyses were performed and significance was determined at p < 0.05. ResultsA total of 1974 patients who underwent total laryngectomy were identified from the database: 1505 (77.3 %) in the non-primary TEP group and 442 (22.7 %) in the primary TEP group. Patients in the non-primary TEP group were more likely to have an ASA class greater than or equal to three (91.2 % primary vs. 84.6 % non-primary, p < 0.001). Patients in the non-primary TEP group were also more likely to require intraoperative or postoperative blood transfusions within the first 72 h of surgery (20.5 % non-primary vs. 15.3 % primary, p = 0.016). Both groups had similar rates of wound breakdown and dehiscence. There remained no significant difference based on type of reconstruction. ConclusionsThis study suggests that patients receiving primary TEPs are not at a greater risk of developing wound complications such as pharyngocutaneous fistulas in the 30-day postoperative period. This remained true when patients were stratified by type of flap reconstruction. Patients in the non-primary TEP group were more likely to have an ASA category of 3 or greater, which may explain why they experienced higher rates of complications such as blood transfusions intra-operatively or post-operatively.

Wound Complication and Surgical Site Infection in Sarcoma Patients Undergoing Internal Hemipelvectomy

Abstract Background Hemipelvectomy is associated with a significant risk of wound complications, including infections, bleeding, and injuries to nearby neurovascular structures as well as the gastrointestinal and genitourinary tract. This study aimed to determine the patient characteristics and approach to treatment that could affect the occurrence of surgical site infection or wound complications in sarcoma patients who have undergone hemipelvectomy. Methods We conducted a retrospective analysis of 33 adult patients who underwent hemipelvectomy at a large tertiary academic cancer center from 2008 to 2016. We used χ 2 (exact Fisher) test to investigate the association between wound complication and categorical variables. We used a t test to evaluate the difference in numerical variables for outcomes. Results Among 33 patients, 12 patients (36.4%) experienced wound complications after hemipelvectomy. The average age of patients with wound complications was 63.3 years (57.1 ± 15.4 years), significantly higher than that of patients without wound complications (P = 0.004). Without adjustment, the use of computer navigation had a lower wound complication rate (P = 0.027). Patients with wound complications had longer hospital length of stay (14.8 vs 7.0 days, P = 0.016). Patient and surgical characteristics were not associated with surgical site infection. Five patients (15%) developed surgical site infection, and they had longer hospital stay (19.4 vs 8.1 days, P = 0.001). Conclusion Older patients undergoing hemipelvectomy are at an increased risk of developing wound complications with a prolonged hospital stay. Initial antimicrobial therapy for suspected surgical site infection should include broad-spectrum coverage to include skin and gastrointestinal flora.

Relationship Between Wound Complication Rates after Breast Reconstruction and Fluorescence Ratio of SPY Indocyanine Green Immunofluorescence: A Quantitative Study

INTRODUCTION: Wound complications are dreaded outcomes of breast reconstruction and can be reduced by adequate perfusion of the mastectomy flap. The use of SPY (Novadaq) immunofluorescence technology gives a quantitative estimate of tissue perfusion. Our study aims to determine what percentage perfusion scores obtained from SPY imaging correlate with development of wound complications after breast reconstruction. METHODS: All female adult patients undergoing immediate breast reconstruction in which SPY was used intraoperatively from January 2018 through January 2021 were reviewed. There were 60 patients with 72 individual breast images. Demographics, number of reoperations, and postoperative wound complications were studied. Major wound complications were determined as return to the operating room for debridement or wound revision. Percent perfusion values were collected at various places on the flap: the incision margin (perfusion at margin, PAM), the lowest percent perfusion (LPP), the highest percent perfusion (HPP), and the average percent perfusion (APP) in the image. RESULTS: Wound complication rates were significantly higher when perfusion scores were lower for all 4 perfusion scores. Using ROC analysis, a cutoff LPP score of 22.5% and above showed a 91.4% negative predictive value (NPV; p = 0.01) and PAM of 33.5% and above showed a 83% NPV (p = 0.01) for having a major wound complication. The LPP, APP and PAM were negatively correlated with the number of re-operations. CONCLUSION: Our study provides a quantitative cutoff value to predict major wound complications in patients undergoing breast reconstruction.

Effect of Drain Output on the Timing of Closed Suction Drain (CSD) Removal After Head and Neck Surgery.

Background: A closed suction drain (CSD) is often utilized in head and neck surgical procedures to obliterate dead space. CSDs reduce seroma and hematoma formation, thereby improving skin apposition and wound healing. The use of drains for prolonged periods of time, however, may increase the risk of wound infection. Interestingly, the evidence regarding the need for, and management of, post-operative CSDs after head and neck surgery is scarce. The current criterion of drain removal when output is less than 30 cubic centimeters (cm3) within a 24-hour period and/or on the third post-operative day (POD) is widely utilized. The aforementioned criterion is based on anecdotal evidence from small studies with specific surgical procedures. In this study, we aim to evaluate the criteria for drain removal and to lay the groundwork for an updated paradigm for drain management in head and neck oncologic surgery.Study Design: Retrospective cohort studySetting: Academic tertiary care hospitalMethods: A retrospective study was performed. Patients were included if they underwent head and neck surgery at the University of Miami Hospital between January 1, 2019 and July 1, 2020 and had at least one CSD. Volume of drain output on each POD was recorded until the day of drain removal. The development of post-operative wound complications (i.e., seroma, hematoma, infection/abscess, and dehiscence) was also recorded.Results: From our initial cohort of 302 patients, 145 patients met inclusion criteria. A total of 10 patients developed a post-operative wound complication. Patients had a mean age of 58.3 ± 15.0 years. The median inter-quartile range (IQR) drain output (cm3) on the day of CSD removal from patients who developed a wound complication was similar (15; IQR, 5-37.5) when compared to those who did not develop a wound complication (25; IQR, 10-30). This difference was not statistically significant (p = 0.60). Additionally, the cohort who developed a post-operative wound complication had their drain removed on an earlier POD (1; IQR, 1-1 (Mean 1.2)) when compared to the cohort who did not develop any complications (1; IQR, 1-1 (Mean 1.5)). This difference was also not statistically significant (p = 0.48) .Conclusion: There is no association between drain output (cm3) or day of CSD removal with the development of wound complications. These results warrant further studies to prospectively evaluate earlier CSD removal in head and neck surgery.

Preoperative Radiation Performed at a Nonsarcoma Center May Lead to Increased Wound Complications Following Resection in Patients With Soft Tissue Sarcomas.

Preoperative radiation therapy (RT) followed by wide-local excision with or without chemotherapy is widely accepted as management for soft tissue sarcomas (STS). Although studies have demonstrated excellent local control with this technique, there can be significant morbidity with the development of wound complications. It has been shown that sarcoma resections performed at a high-volume center lead to improved survival and functional outcomes. It is unclear, however, if radiation performed in a high-volume center leads to improved outcomes especially related to morbidity. The goal of this study was to determine whether preoperative RT performed at an academic cancer center have lower rates of wound complication compared with RT performed in community cancer centers. A total of 204 patients with STS were treated with preoperative RT±chemotherapy followed by limb-sparing resection. Of these, 150 patients had preoperative RT performed at an academic sarcoma center. wound complication were defined as those requiring secondary operations or prolonged wound care for 4 months following surgery. Predictors for wound complication were evaluated using a Fisher exact test for univariate analysis and logistic regression for multivariate analysis. The overall incidence of wound complication was 28.3%. Significant predictors for wound complication include tumor location and radiation delivered at a community hospital. The postoperative incidence of wound complication was 21% when the preoperative RT was performed at an academic cancer center versus 39% when performed at a community cancer center (P=0.009). On multivariate analysis, both tumor location (P=0.0012, 95% confidence interval: 0.03-0.45, odds ratio: 0.13) and RT performed at a community cancer center (P=0.02, 95% confidence interval: 1.13-4.48, odds ratio: 2.25) remained significant in correlation with postoperative wound complication. Preoperative RT performed at an academic cancer center led to lower rates of postoperative wound complication. This may support the recommendation that preoperative RT and resection of STS be performed at an experienced sarcoma center.

211. Patient, surgery, and hospital-related risk factors for hospital acquired conditions following common spinal procedures during inpatient stays

<h3>BACKGROUND CONTEXT</h3> A total of 48,771 hospital-acquired conditions (HACs) have been reported in United States hospitals. These incidents resulted in excess costs of more than $2 billion, which translated to roughly $40,000 per patient per HAC. Along with this direct fiscal burden, enduring an HAC added on average 8.17 days to each patient's hospital stay, which can lead to indirect hidden costs. <h3>PURPOSE</h3> To critically analyze the classical and novel risk factors of HACs following the most prevalent inpatient spine procedures; spinal fusion (463,200 cases/year) and laminectomy (438,200 cases/year). <h3>STUDY DESIGN/SETTING</h3> Systematic review of observational cohort studies. <h3>PATIENT SAMPLE</h3> This study included 6,382 patients who underwent spine fusion or laminectomy. <h3>OUTCOME MEASURES</h3> Patient-, surgical-, hospital-, related risk factors and genetic associations. <h3>METHODS</h3> Several online databases were searched. Using illustrated figures and diagrams, we aim to provide spine surgeons with the knowledge and evidence about the economic impact, patient-, surgery-, and hospital-related risk factors of HACs. Additionally, we present the novel risk factors such as genetic biomarkers. <h3>RESULTS</h3> Patients undergoing fusion and laminectomy experience HACs at rates of 1.4% and 6.5%, respectively. The incidence of HACs most commonly comprises falls and trauma (87%) and SSIs (11%) for thoracolumbar fusion patients, while laminectomy patients experience venous thromboembolism (VTE) (1.4%) and surgical site infection (SSI) (0.2%) most frequently. Increased age is correlated with higher incidence of HACs. Complication rates in those over 65 are as high as 13.46% with wound complications (1.2%-1.69% of cases) and deep vein thrombosis (DVT) (2.1-2.2%) occurring at rates greater than that of those under 65. BMI less than 40 is associated with increased risk of VTE events and SSI, whereas those over 40 have higher risk of urinary tract infection (UTI). Patients who have had previous spinal surgery have a strong risk of developing wound complications (4.6% vs 1.0% among those without prior surgery). Insulin-dependent diabetes (OR=1.72) may increase the rate of complications and deep SSI following thoracolumbar spinal surgery (p=0.050). Steroid use (OR=1.55) may increase a patient's likelihood of complication. Patients with a preoperative psychiatric disorder are at greater risk of a surgical complication. Regarding hospital-related factors, increased operative time is associated with greater risk of HACs. In patients over the age of 75, the length of operation time (p=0.003) is a contributing factor to wound complications. Moreover, teaching hospitals and those with larger bed sizes may be associated with increased risk for HACs. Lastly, weekend admissions are associated with a 45.3% HAC incidence rate compared to a 4.59% rate in weekday admissions. However, higher surgeon volume is associated with lower complications. The pre-admission testing for novel genetic biomarkers of HACs could stratify the risk of HACs for patients. Patients with higher concentrations of miR-582, miR-195, miR- 532, nitric oxide synthase 3 polymorphism are at higher risk to develop VTE. <h3>CONCLUSIONS</h3> Identifying patient-, surgery-, and hospital- related risk factors in the setting of fusion and laminectomy is crucial to providing patients with optimal care and avoiding the penalties of HACs. <h3>FDA DEVICE/DRUG STATUS</h3> This abstract does not discuss or include any applicable devices or drugs.

The benefit of negative pressure dressings in vascular surgery patients with infrainguinal incisions

ObjectiveWithin the vascular patient population, there is an increased risk of developing wound complications especially in infrainguinal incisions. There has been increasing interest in using closed incisional negative pressure dressings to decrease the risk of wound complications. To assess the efficacy of these incisional wound dressings, we studied surgical site infections (SSI) and seroma rates of infrainguinal incisions in our vascular patient population. MethodsThis was a multi-institutional, retrospective study from July 2015 to June 2019. In 2017, our institution began using the Prevena incisional wound system. Wound complication rates were compared with the non-Prevena group before 2017. There were a total of 100 infrainguinal incisions (left and right combined) that received the Prevena wound system and 138 infrainguinal incisions that had not. The primary end point was to assess for wound complication rates, including SSIs and seroma formation. SSIs were graded based on the American College of Surgeons National Surgical Quality Improvement Plan SSI criteria. Seroma formation was diagnosed based on clinical diagnosis, imaging studies (ultrasound examination, computed tomography scan) or needle aspiration of fluid collection. ResultsThis analysis showed a statistically significant decrease in the rate of SSIs in the Prevena group when compared with the non-Prevena group (P = .012). There was no statistical difference between the two groups in the rate of seroma formation (P = .155). Of the 100 incisions that received the Prevena wound system, 1.2% (1/82) had a femoral SSI and 22.0% (4/18) had a popliteal SSI. For seroma formation, 24.4% (20/82) had a femoral seroma and 11.1% (2/18) had a popliteal seroma. Of the 138 incisions that did not have the Prevena wound system, 9.6% (10/104) had a femoral SSI and 8.8% (3/34) had a popliteal SSI. For seroma formation, 24.0% (25/104) had a femoral seroma and 8.8% (3/34) had a popliteal seroma. Comorbid conditions were assessed in the two study groups and there was no statistical significance regarding rates of SSIs between the groups. ConclusionsThe use of an incisional negative pressure dressing decreases the rate of SSIs in infrainguinal incisions. Regarding the use of these wound systems for seromas, our study did not show a statistically significant decrease in seroma rates.

Evidence to support a drain-free strategy in kidney transplantation using a retrospective comparison of 500 consecutively transplanted cases at a single center

IntroductionRoutine placement of surgical drains at the time of kidney transplant has been debated in terms of its prognostic value.ObjectivesTo determine whether the placement of a surgical drain affects the incidence rate of developing wound complications and other clinical outcomes, particularly after controlling for other prognostic factors.MethodsRetrospective analysis of 500 consecutive renal transplant cases who did not (Drain-free, DF) vs. did (Drain, D) receive a drain at the time of transplant was performed. The primary outcome was the development of any wound complication (superficial or deep) during the first 12 months post-transplant. Secondary outcomes included the development of superficial wound complications, deep wound complications, DGF, and graft loss during the first 12 months post-transplant.Results388 and 112 recipients had DF/D, respectively. DF-recipients were significantly more likely to be younger, not have pre-transplant diabetes, receive a living donor kidney, receive a kidney-alone transplant, have a shorter duration of dialysis, shorter mean cold-ischemia-time, and greater pre-transplant use of anticoagulants/antiplatelets. Wound complications were 4.6% (18/388) vs. 5.4% (6/112) in DF vs. D groups, respectively (P = 0.75). Superficial wound complications were observed in 0.8% (3/388) vs. 0.0% (0/112) in DF vs. D groups, respectively (P = 0.35). Deep wound complications were observed in 4.1% (16/388) vs. 5.4% ((6/112) in DF vs. D groups, respectively (P = 0.57). Higher recipient body mass index and ≥ 1 year of pre-transplant dialysis were associated in multivariable analysis with an increased incidence of wound complications. Once the prognostic influence of these 2 factors were controlled, there was still no notable effect of drain use (yes/no). The lack of prognostic effect of drain use was similarly observed for the other clinical outcomes.ConclusionsIn a relatively large cohort of renal transplant recipients, routine surgical drain use appears to offer no distinct prognostic advantage.

The average age of patients with wound complications was 63.3 3 (57.1±15.4) years old, significantly higher than that of patients without wound complications (p=0.004). Among patients with surgical site infection (Table 2), there were no patients’ characteristics or surgical characteristics associated with this outcome. Five (15%) patients developed surgical site infection and they had longer hospital stay (19.4 vs. 8.1 days, p=0.001). The organisms identified from wound cultures include methicillin-resistant Staphylococcus aureus, viridans Streptococcus, Peptostreptococcus asaccharolyticus, Enterobacter cloacae, Pseudomonas aeruginosa, Candida albicans. The organisms in late infections (more than 6 months since surgery), included above organisms plus Stenotrophomonas maltophilia and Achromobacter xylosidans. ConclusionOlder patients undergoing hemipelvectomy are at an increased risk of developing wound complications with a prolonged hospital stay. Metabolic Syndrome Is Associated With Higher Risk of Wound Complications After Total Hip Arthroplasty.

BackgroundObesity is prevalent among patients undergoing total hip arthroplasty and has been associated with the risk of wound complications, particularly when an anterior approach is used. However, most studies have focused on obesity defined by the body mass index (BMI), without considering the metabolic effects of adiposity. Thus, in this study, we investigated the independent effects of the BMI and metabolic syndrome on wound complications after total hip arthroplasty.MethodsAmong 804 consecutive patients undergoing total hip arthroplasty between October 2013 and July 2016, we evaluated the associations between obesity (BMI ≥30 mg/kg2), metabolic syndrome (defined by the National Cholesterol Education Program Adult Treatment Panel III guidelines), and wound complication (defined as documented wound dehiscence, drainage, erythema, hematoma, infection, or seroma) over a 1-year follow-up period. We used Cox proportional hazards models adjusting for demographics, smoking status, and hospital length of stay.ResultsPatients’ mean age at time of surgery was 62.0 ± 11.9 years. Forty-seven percent were male, 27.9% were obese, and 11.6% met the definition for metabolic syndrome. Metabolic syndrome was associated with a 4-fold higher risk of wound complication (95% confidence interval: 1.4-11.1) after adjusting for all covariates including the BMI. In unadjusted analysis, obesity was associated with a higher risk of wound complication (hazard ratio: 2.8, 95% confidence interval: 1.3-6.2). However, obesity was not associated with the risk of wound complication after adjusting for the metabolic syndrome (P = .16).ConclusionsMetabolic syndrome, but not obesity, defined by a BMI ≥30, was associated with wound complications, suggesting that metabolic effects of adiposity may represent a distinct risk factor in the development of wound complications from a higher BMI alone.

Letter: The Use of Absorbable Sutures in Neurosurgical Procedures in the Time of COVID-19.

To the Editor: Nonabsorbable suture is considered the standard of care for the closure of surgical wounds following cranial procedures. However, the removal of sutures requires an additional medical appointment for patients after discharge. This appointment is associated with the exposure of patients to situations where they are at risk of being infected with COVID-19 or spreading it to other patients. NEUROSURGERY STUDIES We performed a literature search for studies that included neurosurgical patients. Prospective randomized clinical trials, prospective study of cases (with and without controls), and retrospective reviews of cases were included. Additionally, studies were ranked according to the number of patients studied. We identified 6 articles addressing this issue. INTERPRETATION OF THE RESULTS Data from general surgery suggest that the use of absorbable suture material for the closure of surgical wounds is noninferior to nonabsorbable suture material with regard to the incidence of wound healing disturbances. Much less data is available for neurosurgical patients, but the available evidence seems to support findings from general surgery. Importantly, no study has yet demonstrated disadvantages in the use of absorbable material in neurosurgical patients. Given the present situation with an urgent need of reducing person-to-person contact as much as possible, the use of absorbable material in (neuro)surgical patients could have considerable advantages. This approach reduces the number of contacts between patients and physicians after discharge. Patients who are at high risk of developing wound complications should be exempted from this approach, eg diabetic or immune-compromised patients. Specific neurosurgical aspects such as possible cerebrospinal fluid leakage also have to be considered when deciding over wound closure techniques. A risk-benefit assessment has to be carried out in every patient in order to avoid controversial effects if a wound complication occurs necessitating operative management. Predictors of Postoperative Complications in Patients Undergoing Below Knee Amputation

To identify risk factors for development of wound complications in patients undergoing a below knee amputation (BKA). Retrospective chart review was performed of patients who underwent BKA between January 1, 2014, and December 31, 2016. Data on patient demographics, preoperative comorbidities, limb salvage attempts (LSA) before amputation, and outcomes after BKA were collected for each limb analyzed. LSAs were defined as endovascular revascularization, open surgical revascularization, wound debridement requiring intravenous sedation or general anesthesia, and amputation at or below the level of transmetatarsal amputation. Postoperative wound complications were defined as residual limb wound infection or nonhealing wound. Primary outcome was the need for procedural reintervention on the ipsilateral limb within 12 months of the initial amputation (washout, debridement, or revision to higher level). There were 155 BKAs identified in 129 patients. Mean age at amputation was 63.7 ± 11.7 years. Of BKAs, 72.3% were performed on male patients. Mean follow-up was 836 days after index amputation. Patients with a history of deep venous thrombosis (DVT) in either lower extremity before amputation had a higher risk of developing post-BKA wound complications (26.7% vs 12.7%; P = .034). Of patients who did not have a preoperative popliteal pulse, 60.0% developed wound complications within 12 months, compared to 24.2% of those with a popliteal pulse (P = .004). Additionally, of patients without a popliteal pulse, 31.3% eventually needed revision to above the knee amputation, compared to 9.7% of those with a popliteal pulse (P = .013). If a patient experienced a complication after any LSA (postoperative DVT, Clostridium difficile, community-acquired infection, need for postoperative transfusion, acute cardiopulmonary event, or need for unplanned surgical reintervention within 24 hours), their risk for developing wound complications after BKA more than doubled (45.6% vs 22.0%; P = .003). Patients admitted more than 17 cumulative days after LSAs were more likely to experience a BKA complication (38.1% vs 22.0%; P = .030). Patients who underwent two or more prior revascularizations experienced a higher rate of BKA complications (36.7% vs 21.0%; P = .032). Risk factors for wound complications after BKA include prior DVT, absence of a popliteal pulse, complications with prior LSA, longer total admissions for LSA, and multiple revascularizations before amputation. Absence of a popliteal pulse increases the risk of eventual need for above the knee amputation by over three-fold. This research suggests that a complete pulse examination and greater scrutiny regarding the usefulness of multiple LSAs may decrease rates of wound complications after BKA.

Early Plastic Surgery Involvement Improves Outcomes in Total Joint Arthroplasty Patients With Soft Tissue Complications

The authors reviewed the outcomes of patients who have undergone total hip or knee arthroplasty and concurrent soft tissue reconstruction at our institution and describe our combined orthoplastic approach to such patients. This was a retrospective series of 8 patients who underwent the soft tissue reconstruction procedure before or after joint replacement by a single orthopedic surgeon in collaboration with a single plastic surgeon. Five men and 3 women met inclusion criteria. Flap reconstruction was performed in 7 of 8 cases, skin grafting in 3 of 8 cases, and complex wound closure in 1 of 8 cases. The majority of patients had 1 or 2 risk factors for developing wound complications. Five had a good outcome at the final follow-up and 3 had poor outcomes. Of 5 patients who had good outcomes, 4 underwent plastic surgery evaluation for prophylactic soft tissue reconstruction because of prior failed arthroplasty procedure, soft tissue scarring, atrophy, or previous infection. Of 3 with poor outcomes, 2 underwent plastic surgery evaluation for a salvage procedure after they developed soft tissue complications. Joint arthroplasty patients undergoing early plastic surgery evaluation for prophylactic soft tissue reconstruction had better outcomes compared with patients undergoing evaluation for a salvage procedure after soft tissue complications have developed.

Relation Between Wound Complication and Lymphocele After Kidney Transplantation: A Monocentric Study

IntroductionWound complication frequently arises after kidney transplantation and its risk factors are well known. In a previous paper we analyzed these factors, and in this new retrospective study we evaluate the influence of lymphocele in the development of wound complications. Patients and methodsFrom January 2000 to December 2018, 731 consecutive kidney transplants have been performed in our center. We have analyzed the incidence of wound complication and lymphocele and their risk factors. ResultsOut of 731 kidney transplants, we have observed wound complications in 115 patients (15.7%) and lymphocele in 158 patients (21.7%). Of these, 70 patients developed both complications (9.5%), but 6 patients have been excluded because they were in therapy with mammalian target of rapamycin inhibitors. Twenty-nine patients (45.3%) presented a first level and 35 patients (54.7%) showed second level wound complications. Lymphocele was the only present factor in just 3 cases (4.6%). The other patients showed diabetes in 28 cases (43.7%), overweight/obesity in 38 (59.3%), delayed graft function in 17 (26.5%), and 60 years or more in 38 (57.8%). The association has been found in 30 out 64 patients treated with tacrolimus (46.8%) and in 34 with cyclosporine (53.1%); 40 patients did not receive muscular layer’s reconstruction (62.5%). ConclusionOur experience shows that lymphocele alone is not a predisposing factor for wound dehiscence after kidney transplantation, and they often coexist because they share the same risk factors, the most important being obesity, diabetes and delayed graft function, older age, and surgical techniques. No relation has been observed with calcineurin inhibitor therapy.

Epicardial Pacemaker in Neonates and Infants: Is There a Relationship Between Patient Size, Device Size, and Wound Complicatıon?

We aimed to investigate the complications after epicardial pacemaker (PM) implantation in neonates and infants and their relationship with factors such as device size and patient size. Between May 2010 and July 2018, 55 patients under 1year of age who underwent epicardial PM placement were retrospectively evaluated. PM-related complications requiring rehospitalization were determined as wound site problems requiring surgical intervention, battery pocket infection, battery pocket dehiscence without infection, PM removal, relocation of the PM system, and replacement of the PM system with another system. The patients were divided into three groups: < 3kg, 3-5kg and > 5kg. Fifty-five patients underwent PM implantation, 43 (78.2%) because of postoperative atrioventricular block (AVB), 10 (18.2%) because of congenital AVB, and two (3.6%) with diagnoses of c-TGA and AVB. Five (9%) patients incurred 18 complications. No statistically significant difference was observed in complication development between the groups (p > 0.05). Single- or dual-chamber device implantation did not affect complication development (p > 0.05). Despite the role of factors such as low weight, low age, and device volume in the development of wound complications, the relationship between these factors and complications is not statistically significant. Therefore, our results are encouraging in terms of the use of dual-chamber PMs instead of single-chamber ones in heart diseases in which AV synchronization is important.

Challenging the Surgical Axiom: Albumin Level Does Not Reliably Predict Development of Wound Complications in Patients Undergoing Body Contouring.

Hypoalbuminemia has traditionally been associated with a poor nutritional status and subsequent high incidence of postoperative wound complications in surgical patients. Recent evidence, however, suggests that traditional nutritional markers are inadequate in predicting postoperative morbidity. To test the hypothesis that preoperative albumin levels are not associated with adverse outcomes in patients undergoing body contouring. All patients undergoing body contouring from 2015 to 2017 were identified using the American College of Surgeons National Surgical Quality Improvement Program database. Demographics, comorbidities, and wound classification were extracted from the database. The independent predictors of developing wound complications were identified. Logistic regressions were used to identify the impact of albumin on outcomes. During the study period, 4496 patients were identified. Wound complications developed in 202 patients (4.5%). Increasing body mass index, history of diabetes mellitus, American Society of Anesthesiologists classification, history of prior open wound, and tobacco use were independently associated with the development of postoperative complications. Albumin levels were not associated with the development of wound complications. Similarly, albumin levels were not associated with the need for a repeated operation, with readmission, or with the total hospital length of stay. Albumin values were not associated with wound complications or need for reoperation in patients undergoing body contouring. Further research is warranted.

Minimally Invasive Repair of Acute Achilles Tendon Rupture Using Gift Box Technique.

This study aimed to introduce a modified technique for minimally invasive Achilles tendon (AT) rupture repair using gift box sutures. The preliminary clinical and functional outcomes were investigated among a number of patients. In a consecutive case series study, 24 patients with acute AT rupture underwent modified minimally invasive AT repair using two mini-incisions and gift box sutures. The patients were followed up for 24 months. The AT rupture score (ATRS) and the American Orthopedic Foot and Ankle Society (AOFAS) measure score were obtained from all patients. The other measured variables included pain intensity and satisfaction using a visual analog scale (VAS), calf atrophy, the range of sagittal ankle motion, development of wound complications, sural nerve injury, and re-rupture. After two years, the mean scores of AOFAS and ATRS were obtained at 83±4 and 81.9±6.3, respectively. Approximately 87.5 % of the patients regained their previous level of activity. The mean VAS score was 7.7±0.9 regarding the satisfaction with the outcomes. Moreover, isokinetic testing of plantar flexion and dorsiflexion strength were 82.7±5.8 and 87.7±4.1%, respectively, compared to those of the normal side. The calf atrophy was not statistically significant. In total, five patients reported pain during their activities. The range of operated ankle motion decreased significantly, compared to that of the other side; however, the differences were not significant functionally. There was no patient with wound complications, nerve injury, or complaint about problem with footwear. Minimally invasive repair of acute AT rupture using two mini-incisions and gift box sutures offers good functional and clinical outcomes without wound complications which can be usually observed following open repair of AT ruptures.

Surgical wound care: A survey of parental knowledge and expectations

BackgroundParents play an integral role in postoperative wound care. Existing parental knowledge is potentially variable. Our aim was to evaluate for any gaps or misconceptions in the parental knowledge base to determine the need for targeted educational programs. MethodsParents of children presenting to a tertiary pediatric center were surveyed in the preoperative stage. This consisted of 26 questions relating to wound healing, complications, and postoperative care. Participants were asked to complete the questionnaire for a same-day, clean abdominal procedure with a 2 cm incision to standardize responses. Expert responses from pediatric surgeons were utilized as a benchmark for nominating concordant answers. Results200 parents were surveyed. 129 parents (64.5%) had previous experience with surgical wound care. Only 64.5% of parents recognized the most concerning features of wound infection, with 65% of parents correctly identifying when infection is most likely to occur. Parents misjudged the time required before return to school (73.5%) and physical activity (51%). More than half of parents (51.5%) expected postoperative antibiotics to be given. Contrary to our routine surgical practice, 80% and 46% of parents respectively felt that regular dressing changes and application of antiseptics to the wound were necessary postoperatively. ConclusionsThere is a deficit of parental knowledge concerning wound healing in children. Therefore, there is a requirement for standardization of advice provided to parents with regards to wound and dressing care, development of wound complications, antibiotic and antiseptic use, and the timing of return to regular activities.
