3D-printed titanium porous prosthesis combined with the Masquelet technique for the management of large femoral bone defect caused by osteomyelitis

BackgroundThe treatment of infected bone defects remains a clinical challenge. With the development of three-dimensional printing technology, three-dimensional printed implants have been used for defect reconstruction. The aim of this study was to investigate the clinical outcomes of three-dimensional printed porous prosthesis in the treatment of femoral defects caused by osteomyelitis.MethodsEleven patients with femoral bone defects following osteomyelitis who were treated with 3D-printed porous prosthesis at our institution between May 2017 and July 2021, were included. Eight patients were diagnosed with critical-sized defects, and the other three patients were diagnosed with shape-structural defects. A two-stage procedure was performed for all patients, and the infection was eradicated and bone defects were occupied by polymethylmethacrylate spacer during the first stage. The 3D-printed prosthesis was designed and used for the reconstruction of femoral defects in the second stage. Position of the reconstructed prostheses and bone growth were measured using radiography. The union rate, complications, and functional outcomes at the final follow-up were assessed.ResultsThe mean length of the bone defect was 14.0 cm, union was achieved in 10 (91%) patients. All patients showed good functional performance at the most recent follow-up. In the critical-sized defect group, one patient developed a deep infection that required additional procedures. Two patients had prosthetic dislocations. Radiography demonstrated good osseous integration of the implant–bone interface in 10 patients.ConclusionThe 3D printed prostheses enable rapid anatomical and mechanically stable reconstruction of extreme femur bone defects, effectively shortens treatment time, and achieves satisfactory clinical outcomes.

The 3D printed models technology for the management of intracranial aneurysms

Management of intracranial aneurysms is still a therapeutic challenge, especially in cases of complex lesions. Thus, the improvement of the study and intervention planning possibilities correlated with the access to continuous professional training based on simulation and clinical diversity represent optimal conditions for the efficient solution of this pathology. The development of three-dimensional printing technology offers a new opportunity in the modern treatment of intracranial aneurysms. The aim of this study is to present some aspects related to the materials and methods of manufacturing simulation models of individual 3D printed aneurysms and their influence in the optimal management of these lesions.

Efficacy of three-dimensionally printed polycaprolactone/beta tricalcium phosphate scaffold on mandibular reconstruction

It has been demonstrated that development of three-dimensional printing technology has supported the researchers and surgeons to apply the bone tissue engineering to the oromandibular reconstruction. In this study, poly caprolactone/beta tricalcium phosphate (PCL/β-TCP) scaffolds were fabricated by multi-head deposition system. The feasibility of the three-dimensionally (3D) -printed PCL/β-TCP scaffolds for mandibular reconstruction was examined on critical-sized defect of canine mandible. The scaffold contained the heterogeneous pore sizes for more effective bone ingrowth and additional wing structures for more stable fixation. They were implanted into the mandibular critical-sized defect of which periosteum was bicortically resected. With eight 1-year-old male beagle dogs, experimental groups were divided into 4 groups (n = 4 defects per group, respectively). (a) no further treatment (control), (b) PCL/β-TCP scaffold alone (PCL/TCP), (c) PCL/β-TCP scaffold with recombinant human bone morphogenetic protein-2 (rhBMP-2) (PCL/TCP/BMP2) and (d) PCL/β-TCP scaffold with autogenous bone particles (PCL/TCP/ABP). In micro-computed tomography, PCL/TCP/BMP2 and PCL/TCP/ ABP groups showed significant higher bone volume in comparison to Control and PCL/TCP groups (P < 0.05). In histomorphometric analysis, a trend towards more bone formation was observed in PCL/TCP/BMP2 and PCL/TCP/ABP groups, but the results lacked statistical significance (P = 0.052). Within the limitations of the present study, 3D-printed PCL/β-TCP scaffolds showed acceptable potential for oromandibular reconstruction.

Development of Three-Dimensional Printing Technology Based on Deterministic Electrochemical Deposition Method

Three-dimensional printing technology based on deterministic electrochemical deposition method has been developed, which is generated from the electroplating. The traditional electroplating system has been improved to ensure the printing process controllable. Through accurate control of the deposition region, deposition rate, and the dwelling time in the printing process, a three-dimensional object of almost any shape can be obtained by deterministic manufacturing process. Through addition of alcohol to the electrolyte, Marangoni effect is generated, which will improve the property of the deposited layer. The deterministic electrochemical deposition not only can decrease the cost of the printing process, but also will promote the application of the 3D printing technique in many fields.

The future of three-dimensional printing: Intellectual property or intellectual confinement?

For the first 25 years, three-dimensional printing was the domain of industrial manufacturing and engineering. Through the open-source movement and peer production, this disruptive technology has become more widely available. This article considers how the future development and use of three-dimensional printing may be facilitated or inhibited through intellectual property law and practice. Using a combination of technical, legal, and policy analysis, we conclude that the potential for patent infringement is a primary issue in the continued development of three-dimensional printing technology. When it comes to the printed objects themselves, copyright issues will be the main focus of this debate. As these issues become more pervasive, some copyright holders will find technology-based solutions to enforce copyright laws and restrict printing activities. A common recourse is digital rights management as a means for protecting intellectual property, but its use has the potential for imposing unnecessarily stringent limitations on the use of three-dimensional printing.
