NFT-based online prescription for data management in healthcare services

ABSTRACT The development of electronic health (e-health) systems in the last decades led to the emergence of effective infrastructures for managing health records. Due to the vitality and massive amount of healthcare records, blockchain is considered one of the most important infrastructures to store and exchange health records. Access to patient’s medical histories and health record management are essential to healthcare systems. Patients’ records must be exchanged among various parts of the healthcare system and users should have access to manage their profiles and use online facilities in a secure manner. In this paper, for the first time, the non-fungible token (NFT), which is a nascent technology, is used to certify the information immutability of healthcare records in the blockchain infrastructure. The ownership of property and prevention of information disclosure is guaranteed using the encryption strategy is used in the proposed NFT technology. Furthermore, the proposed model satisfies all security criteria such as data integrity, non-repudiation, anonymity, confidentially and privacy. As a result, the proposed model outperforms the related research in the healthcare domain. The proposed model brings many facilities for both patients and physicians such as secure accessibility, data quality, patient consent, efficient record management and better availability and reliability.

Acceptance and Expectations of Medical Experts, Students, and Patients Toward Electronic Mental Health Apps: Cross-Sectional Quantitative and Qualitative Survey Study.

BackgroundThe acceptability of electronic mental (e-mental) health apps has already been studied. However, the attitudes of medical experts, students, and patients taking into account their knowledge of and previous experiences with e-mental health apps have not been investigated.ObjectiveThe aim of this study was to explore the attitudes, expectations, and concerns of medical experts, including physicians, psychotherapists and nursing staff, students of medicine or psychology, and patients toward e-mental health apps when considering their knowledge of and former experiences with e-mental health apps.MethodsThis cross-sectional quantitative and qualitative survey was based on a self-developed questionnaire. A total of 269 participants were included (104 experts, 80 students, and 85 patients), and 124 eligible participants answered a paper version and 145 answered an identical online version of the questionnaire. The measures focused on existing knowledge of and experiences with e-mental health apps, followed by a question on whether electronic health development was generally accepted or disliked. Further, we asked about the expectations for an ideal e-mental health app and possible concerns felt by the participants. All items were either presented on a 5-point Likert scale or as multiple-choice questions. Additionally, 4 items were presented as open text fields.ResultsAlthough 33.7% (35/104) of the experts, 15.0% (12/80) of the students, and 41.2% (35/85) of the patients knew at least one e-mental health app, few had already tried one (9/104 experts [8.7%], 1/80 students [1.3%], 22/85 patients [25.9%]). There were more advocates than skeptics in each group (advocates: 71/104 experts [68.3%], 50/80 students [62.5%], 46/85 patients [54.1%]; skeptics: 31/104 experts [29.8%], 20/80 students [25.0%], 26/85 patients [30.6%]). The experts, in particular, believed, that e-mental health apps will gain importance in the future (mean 1.08, SD 0.68; 95% CI 0.94-1.21). When asked about potential risks, all groups reported slight concerns regarding data security (mean 0.85, SD 1.09; 95% CI 0.72-0.98). Patient age was associated with several attitudes toward e-mental health apps (future expectations: r=–0.31, P=.005; total risk score: r=0.22, P=.05). Attitudes toward e-mental health apps correlated negatively with the professional experience of the experts (rs(94)=–0.23, P=.03).ConclusionsAs opposed to patients, medical experts and students lack knowledge of and experience with e-mental health apps. If present, the experiences were assessed positively. However, experts show a more open-minded attitude with less fear of risks. Although some risks were perceived regarding data security, the attitudes and expectations of all groups were rather positive. Digital Technology and Mobile Health in Behavioral Migraine Therapy: a Narrative Review.

This article reviews the recent research and development of electronic health (eHealth) and, in particular, mobile health (mHealth) strategies to deliver behavioral treatment for migraine. Prospects for future development and research of mobile health in migraine are suggested. Advances in digital technology and mobile technology have led to an era where electronic and mobile approaches are applied to several aspects of healthcare. Electronic behavioral interventions for migraine seem to be acceptable and feasible, but efficacy measures are uncertain. Clinical trials on mHealth-based classical behavioral therapies, such as relaxation, biofeedback, and cognitive behavioral therapy are missing in the literature. Within mHealth, headache diaries are the most researched and scientifically developed. Still, there is a gap between commercially available apps and scientifically validated and developed apps. Digital technology and mobile health has not yet lived out its potential in behavioral migraine therapy. Application of proper usability and functionality designs towards the right market, together with appraisal of medical and technological recommendations, may facilitate rapid development of eHealth and mHealth, while also establishing scientific evidence.

AS THE DEVELOPMENT of electronic health records progresses, the amount of patient information entered manually into hospital clinical systems is increasing rapidly. But this rise in the use of computer keyboards, with their bumps and holes in which debris can accumulate, also increases the risk of cross-infection in hospital environments.

Improving the use, analysis and integration of patient health data

Health Information Technologies (HIT) are being deployed world- wide to improve access to individual patient information. Primarily this is through the development of electronic health records (EHR) and electronic medical records (EMR). While the proper collection of this data has reached a high level of maturity, the use and analysis of it is only in its infancy. This data contains information which can potentially improve treatment for the individual patient and for the cohort of patients suffering a similar disease. The data can also provide valuable information for broader research purposes. In this paper we discuss the research contributions we are making in improving the use and analysis of patient data. Our projects include the analysis of physiological data, the extraction of information from multi-modal data types, the linking of data stored in heterogeneous data sources and the semantic integration of data. Through these projects we are providing new ways of using health data to improve health care delivery and provide support for medical research.

Developing the electronic health record: what about patient safety?

This paper examines the development of electronic health records within the National Health Service (NHS) by an analysis of a series of pilot projects funded by the Electronic Record Development and Implementation Project (ERDIP), one aspect of the work of the NHS Information Authority (NHSIA) (As of 1 April 2005, the NHSIA ceased to operate. Much of its work is continued by Connecting for Health and the Health and Social Care Information Centre.) The focus of the analysis is on the extent to which identifying and correcting error within health records was explored through these projects. The inherent potential for error and resultant impact on patient safety is highlighted, by considering the context of the record, the content of the record and the process of change from paper-based or piecemeal electronic health records to integrated electronic health records. While the process of change highlights issues of data security and access, it is the variability in starting points for different organizations that possibly poses most risk to patient safety. Issues relating to the content of the record can to some extent be minimized by the effective use of technology, but the tension between coding and qualitative data requires further consideration in terms of its impact on patient safety. This paper concludes that the development of electronic health records has to be viewed within the context of governance and patient safety, and the implications articulated.

An online survey of nurses’ perceptions, knowledge and expectations of the National Health Service modernization programme

We conducted an online survey to investigate nurses' perceptions, knowledge and expectations of the National Health Service (NHS) modernization programme in the UK. The questionnaire was available for 28 days via the Website of the Royal College of Nursing. The questionnaire was completed by 2020 nurses, midwives and health visitors working in all sectors of the health service in a wide range of specialties and environments of care. Less than one-quarter of respondents felt that they had adequate information about NHS information technology (IT) developments. In all, 528 (26%) said this was the first they had heard of the initiatives. Only 383 respondents (19%) felt adequately informed about the development of electronic health records; 470 (23%) felt inadequately informed and 456 (23%) had only heard something about it. The findings of this survey suggest that nursing staff are not widely aware of current IT plans and programmes in the NHS. They suggest that nurses also lack confidence in using advanced IT, which is compounded by lack of training.

Predicting the Influence of the Electronic Health Record on Clinical Coding Practice in Hospitals

The key drivers of change to clinical coding practice are identified and examined, and a major shift is predicted. The traditional purposes of the coding function have been the provision of data for research and epidemiology, in morbidity data reporting and, latterly, for casemix-based funding. It is contended that, as the development of electronic health records progresses, the need for an embedded nomenclature will force major change in clinical coding practice. Clinical coders must become expert in information technology and analysis, change their work practices, and become an integral part of the clinical team.
