ULTRASOUND DIAGNOSIS OF THE RENAL ARTERY STENOSIS IN A TRANSPLANTED KIDNEY IN THE EARLY POSTOPERATIVE PERIOD

In this clinical case, stenosis of a renal artery of transplant was detected in time by angiography using dynamic Doppler ultrasound control. The chosen treatment method allowed the restoration of arterial patency and thus renal graft function in the shortest possible time and with minimal trauma. However, due to the high risk of restenosis in the plastic area, continuous dynamic ultrasound monitoring and monitoring of the patient's creatinine level are required. The purpose of this presentation is to present a case of early detection of a postoperative complication by Doppler ultrasound, which made it possible to prevent vascular dysfunction, often leading to loss of the renal graft. Materials and Methods. In this article, we report a case of timely detection of renal artery stenosis by ultrasound in a patient, followed by confirmation by angiography and restoration of renal artery and graft function by endovascular balloon angioplasty. All major hemodynamic changes in Doppler ultrasound parameters characteristic of renal artery stenosis are described in this article. Results. Dynamic Doppler ultrasound monitoring and angiography allowed early detection of the described complication of renal transplantation. The optimal choice of treatment tactics allowed the restoration of arterial patency and renal graft function in the shortest possible time, thus preventing graft rejection. Conclusion. Our clinical case demonstrates the importance of the monitor in the assessment of renal perfusion. The outcome of kidney transplantation largely depends on the successful resolution of these issues, which determines the relevance.

Non-contrast MR angiography versus contrast enhanced MR angiography for detection of renal artery stenosis: a comparative analysis in 400 renal arteries.

In this study, we compared non-contrast MR angiography (NC-MRA) with conventional 3D contrast-enhanced MRA (CE-MRA) in patients suspected to have renal artery stenosis (RAS). From March 2014 to March 2020, patients who were evaluated for RAS and had a glomerular filtration rate > 30ml/min/1.73 m2 underwent MR imaging on a 3T MR Scanner (Signa Hdxt General Electrics, Milwaukee, USA) using a Torso PA coil. The NC-MRA sequence was performed using a 3D fat-suppressed inflow inversion recovery balanced steady state free precession (SSFP) sequence (Inhance 3D Inflow IR, GE Medical) whereas the CE-MRA sequence was a 3D fast spoiled gradient echo (FSPGR). Overall quality of images was rated 1 to 4. Stenosis was reported as grade 1 (Normal), 2 (< 50% narrowing), 3 (> 50% narrowing) and 4 (Total occlusion). Grade 3 and 4 were considered haemodynamically significant. During the study period, 201 patients were enrolled (400 renal arteries). For hemodynamically significant (grade 3/4) stenosis, NC-MRA correctly diagnosed 72 patients (95 arteries) while in 2 patients (2 arteries), NC-MRA underdiagnosed the stenosis as grade 2 (these were found to have grade 3 stenosis on CE-MRA). The kappa value of agreement between NC-MRA and CE-MRA for detection of RAS showing excellent agreement (p < 0.001). In one of the largest series of patients so far, we found that NC-MRA is a viable alternative to CE-MRA for detection of RAS, highly correlating with CE-MRA for grade of stenosis and with additional advantage of lack of gadolinium based contrast agents toxicity.

Renal Artery Stenosis As Etiology of Recurrent Flash Pulmonary Edema and Role of Imaging in Timely Diagnosis and Management.

Renal hypoperfusion from renal artery stenosis (RAS) activates the renin-angiotensin system, which in turn causes volume overload and hypertension. Atherosclerosis and fibromuscular dysplasia are the most common causes of renal artery stenosis. Recurrent flash pulmonary edema, also known as Pickering syndrome, is commonly associated with bilateral renal artery stenosis. There should be a high index of clinical suspicion for renal artery stenosis in the setting of recurrent flash pulmonary edema and severe hypertension in patients with atherosclerotic disease. Duplex ultrasonography is commonly recommended as the best initial test for the detection of renal artery stenosis. Computed tomography (CT) angiography (CTA) or magnetic resonance (MR) angiography (MRA) are useful diagnostic imaging studies for the detection of renal artery stenosis in patients where duplex ultrasonography is difficult. If duplex ultrasound, CTA, and MRA are indeterminate or pose a risk of significant renal impairment, renal angiography is useful for a definitive diagnosis of RAS. The focus of medical management for RAS relies on controlling renovascular hypertension and aggressive lifestyle modification with control of atherosclerotic disease risk factors. The restoration of renal artery patency by revascularization in the setting of RAS due to atherosclerosis may help in the management of hypertension and minimize renal dysfunction.

Does the measurement of the difference of resistive indexes in spleen and kidney might be used for characterization of intrarenal tardus parvus phenomenon in chronic kidney disease?

Doppler sonography is used as a routine test for detection of renal artery stenosis (RAS). Although increased peak systolic velocity at the site of the narrowing is a widely accepted method for assessing main renal artery stenosis, tardus parvus phenomenon detected on the downstream blood flow has been used as an alternative to direct insonation of the main artery. However, the uncertainty about the best Doppler parameter to be used for characterization of tardus parvus has yielded a variety of studies. Recently, the difference between the resistive index of the spleen and kidney (RISK) has been proposed as a potential marker of kidney damage and Doppler criterion for RAS in hypertensive patients without chronic kidney disease (CKD). The hypothesis of this study was to speculate further and propose the RISK as valuable new Doppler diagnostic criteria for the tardus parvus in CKD too. Data from 183 control patients and 135 chronic kidney disease patients (CKD) were included in the study to test the feasibility of the method and plausibility of the hypothesis. Criteria inclusion was a color Doppler ultrasound measurement of the renal (RRI) and spleen resistive index (SRI). Serum creatinine (0.83 ± 0.20 vs 3.27 ± 1.74), RRI (0.60 ± 0.06 vs 0.69 ± 0.09), SRI (0.55 ± 0.06 vs 0.58 ± 0.09), RISK (0.06 ± 0.04 vs 0.11 ± 0.08), and spleen size (94.1 ± 13.2 vs 100.8 ± 16.5) were higher in CKD patients (p < 0.05). RRI correlated with SRI in both groups; however, the coefficient of determination was different (Z = 2.29, p = 0.022). In the CKD group, RISK correlated inversely with age (r = −0.276; p = 0.001) and positively with creatinine (r = 0.509; p < 0.001). In the multivariate analysis, creatinine remained an independent predictor of the high RISK (Wald = 13.07; p < 0.001; odds ratio = 1.87; 95% confidence interval 1.33–2.62). Since extraneous factors similarly influence RRI and SRI, the SRI can be considered the correction factor that can estimate the cardiovascular burden on the RRI. Consequently, according to the suggested hypothesis, RISK helps to distinguish the tardus parvus related to RAS from tardus parvus related to systemic influences on Doppler morphology. The identification of the underlying mechanisms of tardus parvus, would help a more accurate characterization of the phenomenon and improve screening and diagnostic test for RAS in CKD patients.

Difference between renal and splenic resistive index as a novel criterion in Doppler evaluation of renal artery stenosis

Detection of renal artery stenosis (RAS) using Doppler is difficult to evaluate, particularly under conditions such as bilateral RAS or difficultly accessible renal arteries (RA). The objective of the present study was to assess the utility of splenic arterial compared to renal flow as an additional parameter in the Doppler evaluation of RAS. The difference between the resistive indices (RI) determined in renal and splenic parenchymal arteries (ΔRIK-S ) was evaluated in 181 hypertensive subjects without any evidence of RAS. Subsequently 47 RA in 24 patients with suspected RAS were angiographically assessed. A ΔRIK-S of 0.055 (median) was determined in the population without any evidence of RAS similar to RA with angiographically excluded stenosis (ΔRIK-S 0.068). In contrast, in angiographic proven RAS, ΔRIK-S was significantly lower (-0.050; P<.005). The assessment of the ΔRIK-S , proved to be an easily feasible parameter, which improves the diagnostic accuracy in the detection of RAS.

Duplex evaluation of distal renal artery to predict the presence of proximal significative acceleration: An experience of 2,200 explorations

Purpose: Detection of renal artery stenosis (RAS) (diameter reduction ≥60%) by means of duplex Doppler ultrasound with direct scanning of the main renal arteries is subject to numerous limitations. The aim of this study was to determine the accuracy of indirect hemadynamic parameters obtained by evaluation of the segmental or interlobal arteries within the kidney as predictors of the presence of hemodynamically significative proximal renal artery stenosis.

Non-enhanced MR angiography of renal arteries: comparison with contrast-enhanced MR angiography

The main causes of renal artery stenosis (RAS) are atherosclerosis and fibromuscular dysplasia. Despite contrast-enhanced magnetic resonance angiography (CE-MRA) being a safe and reliable method for diagnosis of RAS especially in young individuals, recently it has been possible to adopt innovative technologies that do not require paramagnetic contrast agents. To assess the accuracy of steady-state free-precession (SSFP) non-contrast-enhanced magnetic resonance angiography (NC-MRA) by using a 1.5 T MR scanner for the detection of renal artery stenosis, in comparison with breath-hold CE-MRA as the reference standard. Sixty-three patients (33 men, 30 women) with suspected renovascular hypertension (RVHT) were examined by a 1.5T MR scanner; NC-MRA with an electrocardiography (ECG)-gated SSFP sequence was performed in 58.7% (37/63) of patients; in 41.3% (26/63) of patients a respiratory trigger was used in addition to cardiac gating. CE-MRA, with a three-dimensional gradient echo (3D-GRE) T1-weighted sequence, was performed in all patients within the same session. Maximum intensity projection (MIP) image quality, number of renal arteries, and the presence of stenosis were assessed by two observers (independently for NC-MRA and together for CE-MRA). The agreement between NC-MRA and CE-MRA as well as the inter-observer reproducibility were calculated with Bland-Altman plots. MIP image quality was considered better for NC-MRA. NC-MRA identified 143 of 144 (99.3%) arteries detected by CE-MRA (an accessory artery was not identified). Fourteen stenoses were detected by CE-MRA (11 atherosclerotic, 3 dysplastic) with four of 14 (28.5%) significant stenosis. Bland-Altman plot demonstrated an excellent concordance between NC-MRA and CE-MRA; particularly, the reader A evaluated correctly all investigated arteries, while over-estimation of two stenoses occurred for reader B. Regarding NC-MRA, inter-observer agreement was excellent. NC-MRA is a valid alternative to CE-MRA for the assessment of renal arteries.

Time-resolved MR angiography of renal artery stenosis in a swine model at 3 Tesla using gadobutrol with digital subtraction angiography correlation

To establish the minimum dose required for detection of renal artery stenosis using high temporal resolution, contrast enhanced MR angiography (MRA) in a porcine model. Surgically created renal artery stenoses were imaged with 3 Tesla MR and digital subtraction angiography (DSA) in 12 swine in this IACUC approved protocol. Gadobutrol was injected intravenously at doses of 0.5, 1, 2, and 4 mL for time-resolved MRA (1.5 × 1.5 mm(2) spatial resolution). Region of interest analysis was performed together with stenosis assessment and qualitative evaluation by two blinded readers. Mean signal to noise ratio (SNR) and contrast to noise ratio (CNR) values were statistically significantly less with the 0.5-mL protocol (P < 0.001). There were no statistically significant differences among the other evaluated doses. Both readers found 10/12 cases with the 0.5-mL protocol to be of inadequate diagnostic quality (κ = 1.0). All other scans were found to be adequate for diagnosis. Accuracies in distinguishing between mild/insignificant (<50%) and higher grade stenoses (>50%) were comparable among the higher-dose protocols (sensitivities 73-93%, specificities 62-100%). Renal artery stenosis can be assessed with very low doses (~0.025 mmol/kg bodyweight) of a high concentration, high relaxivity gadolinium chelate formulation in a swine model, results which are promising with respect to limiting exposure to gadolinium based contrast agents.

Renal artery stenosis: comparative assessment by unenhanced renal artery mra versus contrast-enhanced MRA

To evaluate steady-state free precession (SSFP) non-contrast-enhanced MR angiography (Unenhanced-MRA) versus conventional contrast-enhanced MR angiography (CE-MRA) in the detection of renal artery stenosis (RAS). Retrospective analysis of 70 consecutive patients referred for suspected RAS, examined by SSFP Unenhanced-MRA and CE-MRA. Image quality, quality of visible renal arterial segments, presence and grade of RAS were evaluated. The Unenhanced-MRA were compared against reference standard CE-MRA results. 149 renal arteries were assessed with 21 haemodynamically significant stenoses (≥ 50% stenosis) demonstrated by CE-MRA. Combined sensitivity and specificity for RAS detection by Unenhanced-MRA was 72.8% and 97.8% respectively. There is substantial correlation for RAS detection between Unenhanced-MRA and CE-MRA with kappa values of between 0.64 and 0.74. There was excellent inter-observer correlation for RAS on Unenhanced-MRA (kappa values 0.82-1.0). Our study has shown Unenhanced-MRA to be a viable alternative to CE-MRA, yielding images equal in quality without the requirement for gadolinium contrast agents. The sensitivity and specificity for the detection of haemodynamically significant stenoses are comparable to CE-MRA. Potentially, Unenhanced-MRA could be used as an initial investigation to avoid performing CE-MRA in patients with normal renal arteries, however we suggest that its real value will lie in being complementary to CE-MRA.

Non-enhanced MR angiography of renal artery using inflow-sensitive inversion recovery pulse sequence: A prospective comparison with enhanced CT angiography

To prospectively evaluate the diagnostic value of non-enhanced inflow-sensitive inversion recovery (IFIR) MR angiography for the detection of renal artery stenosis (RAS), with enhanced CT angiography performed as the reference standard. Sixty consecutive patients suspected of RAS underwent both of IFIR MR and enhanced CT angiography. Subjective image quality, renal artery depiction and renal artery grading were all evaluated on artery-by-artery basis. Spearman rank correlation analysis was used to assess agreement between the two techniques. The diagnostic sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) for RAS detection at IFIR MR angiography were calculated. One hundred twenty-six main renal arteries were visualized on enhanced CT and non-enhanced MR angiographic images, respectively. The Spearman rank correlation was 0.773 (P<.001) for renal artery depiction, 0.998 (P<.001) for renal arteries grading and 0.833 (P<.001) for RAS detection between the two modalities. The sensitivity, specificity, PPV and NPV of IFIR MR angiography for RAS detection demonstrated 100%, 99.0%, 92.0% and 100%, respectively. Non-enhanced IFIR MR angiography had high sensitivity, specificity, PPV and NPV for RAS detection. It could be the first choice of renal artery imaging methods to avoid ionizing irradiation and renal toxicity from contrast media.

Non‐contrast renal artery MRA using an inflow inversion recovery steady state free precession technique (Inhance): Comparison with 3D contrast‐enhanced MRA

To assess the performance of a three-dimensional (3D) non-contrast respiratory-triggered steady state free precession (SSFP) pulse sequence for detection of renal artery stenosis. A total of 64 patients who had non-contrast MR angiography (NC MRA) and 3D contrast-enhanced MRA (CE MRA) performed during the same exam and three patients who had NC MRA followed by conventional catheter angiography within one month of the MRI exam were included in this retrospective study. Two blinded readers evaluated NC MRA images for the presence of significant renal artery stenosis and also rated their diagnostic confidence and evaluated the images for artifact. A similar analysis was performed for CE MRA images by two additional blinded readers, and discrepancies were resolved by consensus reading. The 67 patients had 168 main and accessory renal arteries, with significant (>50%) stenosis in 34 arteries on CE MRA or conventional angiography. The two NC MRA readers had sensitivity and specificity for detection of significant stenosis of 94%/82% and 82%/87% respectively on a per renal artery basis. There was good agreement between CE MRA and NC MRA for detection of significant renal artery stenosis. This technique should prove useful in evaluating patients with suspected renovascular hypertension who are unable to undergo CE MRA.

Ultrasound is usually the first imaging technique used in clinical practice for the visualization of renal blood vessels, especially when there is suspicion of renal artery stenosis. Apart from B-mode ultrasound, colour Doppler ultrasound, pulsed-wave Doppler are used as well as contrast-enhanced ultrasound, which is being used more and more as a routine technique. Ultrasound is important in nephrology for the detection of renal artery stenosis and the resistance indices from pulse-wave Doppler are also helpful in acute renal failure. Nevertheless resistance indices should be used carefully and always by being aware of the pitfalls of this parameter.Another field of application for ultrasound is imaging of renal blood vessel damage as a result of trauma or iatrogenic damage caused during a biopsy. In this setting contrast-enhanced ultrasound appears to be a suitable and highly sensitive tool for the detection of active bleeding and for visualizing the extent and dynamics of expansion of a hematoma.Ultrasonography is frequently used in the field of kidney transplants for the diagnostics of drainage obstructions, perfusion disturbances, transplant arterial stenoses and lymphoceles. Furthermore, ultrasound is exceptionally well suited for follow-up observation after diagnostic or therapeutic measures (e.g. biopsy and placement or removal of DJ catheters).

Contrast-enhanced MR angiography utilizing parallel acquisition techniques in renal artery stenosis detection

Significant renal artery stenosis (RAS) is a potentially curable cause of renovascular hypertension and/or renal impairment. It is caused by either atherosclerosis or fibromuscular dysplasia. Correct and timely diagnosis remains a diagnostic challenge. MR angiography (MRA) as a minimally invasive method seems to be suitable for RAS detection, however, its diagnostic value widely differs in the literature (sensitivity 62–100% and specificity 75–100%). The aim of our prospective study was to compare the diagnostic value of contrast-enhanced MRA utilizing parallel acquisition techniques in the detection of significant RAS with digital subtraction angiography (DSA). A total of 78 hypertensive subjects with suspected renal artery stenosis were examined on a 1.5 Tesla MR system using a body array coil. Bolus tracking was used to monitor the arrival of contrast agent to the abdominal aorta. The MRA sequence parameters were as follows: TR 3.7 ms; TE 1.2 ms; flip angle 25°; acquisition time 18 s; voxel size 1.1 mm × 1.0 mm × 1.1 mm; centric k-space sampling; parallel acquisition technique with acceleration factor of 2 (GRAPPA). Renal artery stenosis of 60% and more was considered hemodynamically significant. The results of MRA were compared to digital subtraction angiography serving as a standard of reference. Sensitivity and specificity of MRA in the detection of hemodynamically significant renal artery stenosis were 90% and 96%, respectively. Prevalence of RAS was 39% in our study population. Contrast-enhanced MRA with high spatial resolution offers sufficient sensitivity and specificity for screening of RAS.

Atherosclerotic Peripheral Vascular Disease Symposium II

The Atherosclerotic Peripheral Vascular Disease Interdisciplinary Working Group was commissioned by the American Heart Association (AHA) to provide a forum to address important and emerging issues in this multidisciplinary area of clinical science. The working group was a primary outgrowth of the AHA Atherosclerotic Vascular Disease Conference held in Boston, Mass, in July 2002. It was created in recognition of the fact that atherosclerosis is a systemic disease with important sequelae in many regional circulations in addition to the heart, including the brain, kidneys, mesentery, and limbs. Its mission is to provide a forum for the multiple disciplines engaged in research, evaluation, and management of patients with noncoronary atherosclerosis. The goals of the Atherosclerotic Peripheral Vascular Disease Interdisciplinary Working Group are to develop a strategy to increase awareness of atherosclerotic vascular disease, identify important gap areas in knowledge that require further clinical investigation, and develop programs that will facilitate prevention and treatment of peripheral atherosclerotic diseases. Developments in research and technology that are relevant to atherosclerotic vascular disease are emerging rapidly. As a result, greater opportunities to translate science to clinical practice are available. The American College of Cardiology/AHA practice guidelines for the management of patients with peripheral arterial disease (lower extremity, renal, mesenteric, and abdominal aortic) provide many evidenced-based recommendations for diagnosing and treating patients with atherosclerotic vascular diseases.1 Nevertheless, in some areas, the evidence has not matured sufficiently for definitive guidelines. Some of these areas have engendered considerable controversy among practitioners. Among these are the efficacy and outcome of screening programs for vascular disease and the appropriate and timely use of endovascular interventions. Accordingly, the Atherosclerotic Peripheral Vascular Disease Interdisciplinary Working Group convened the second AHA conference on Atherosclerotic Vascular Disease, which took place in Boston, Mass, in July 2006. Renal Artery Assessment with Nonenhanced Steady-State Free Precession versus Contrast-enhanced MR Angiography

To prospectively assess the diagnostic accuracy of nonenhanced three-dimensional (3D) steady-state free precession (SSFP) magnetic resonance (MR) angiography for detection of renal artery stenosis (RAS), with breath-hold contrast material-enhanced MR angiography performed as the reference standard. The study was local ethics committee approved; all patients gave written informed consent. Fifty-three patients (30 male, 23 female; mean age, 58 years) with arterial hypertension and suspected of having RAS were examined with 1.5-T 3D SSFP renal MR angiography. Stenosis grade, maximal visible vessel length, and subjective image quality were compared. Sensitivity, specificity, accuracy, and negative predictive value (NPV) were calculated on artery-by-artery and patient-by-patient bases. The significance of the results was assessed with the paired two-sided t test for continuous variables and with the marginal homogeneity test for categorical variables. Cohen kappa statistics were used to estimate interobserver agreement. One hundred eight renal arteries with 20 significant (>or=50%) stenoses were detected with contrast-enhanced MR angiography. At artery-by-artery analysis, sensitivity, specificity, accuracy, and NPV of nonenhanced SSFP MR angiography for RAS detection were 100%, 93%, 94%, and 100%, respectively, for observer 1 and 95%, 95%, 95%, and 99%, respectively, for observer 2. Corresponding patient-by-patient values were 100%, 92%, 94%, and 100%, respectively, for observer 1 and 100%, 95%, 96%, and 100%, respectively, for observer 2. Overestimation of stenosis grade with SSFP MR angiography resulted in six and four false-positive findings for readers 1 and 2, respectively. Mean maximal visible lengths of the renal arteries were 69.9 mm at contrast-enhanced MR angiography and 61.1 mm at SSFP MR angiography (P<.001). Both techniques yielded good to excellent image quality. Slab-selective inversion-prepared 3D SSFP MR angiography had high sensitivity, specificity, accuracy, and NPV for RAS detection, without the need for contrast material. However, RAS severity was overestimated in some patients.

Navigator-Gated MR Angiography of the Renal Arteries: A Potential Screening Tool for Renal Artery Stenosis

The purpose of our study was to determine how well unenhanced navigator-gated steady-state free precession (Nav SSFP) MR angiography (MRA) performs as a screening test for the detection of renal artery stenosis. Forty patients referred to rule out renal artery stenosis were imaged using an optimized Nav SSFP MRA sequence before conventional contrast-enhanced MRA (CE-MRA). Two radiologists evaluated Nav SSFP for maximum stenosis measurement, and comparison was made with CE-MRA results. Fifteen of the 40 patients had greater than 50% renal artery stenosis as determined on CE-MRA. Sensitivity for detecting renal artery stenosis with Nav SSFP was 100%; specificity, 84%; negative predictive value, 100%; and positive predictive value, 79%. The average mean stenosis difference between Nav SSFP and CE-MRA was 10% +/- 9%. Sensitivity and negative predictive value for the detection of renal artery stenosis using Nav SSFP were perfect, with an acceptable specificity of 84%. This suggests Nav SSFP is a promising technique for simple unenhanced screening for the detection of renal artery stenosis.

Imaging Modalities for Renal Artery Stenosis in Suspected Renovascular Hypertension: Prospective Intraindividual Comparison of Color Doppler US, CT Angiography, GD-Enhanced MR Angiography, and Digital Substraction Angiography

The aim of the study was to evaluate the diagnostic accuracy of Color Doppler US, CT Angiography (CTA), and GD-enhanced MR Angiography (MRA) compared with digital subtraction angiography (DSA) for the detection of renal artery stenosis in patients with clinically suspected renovascular hypertension. Fifty-eight patients with suspected renovascular hypertension were enrolled in the study. All patients underwent Color Doppler US, CTA and GD-enhanced MRA. DSA was the gold standard method for the number of renal arteries, existence and degree of stenosis, or evidence of fibromuscular dysplasia. DSA depicted 132 renal arteries, 16 stenoses, and 4 arteries with fibromuscular dysplasia. Color Doppler US failed to detect 1 main and 14 polar arteries. CTA depicted all main renal arteries and 7/16 polar arteries, but failed to detect stenosis in two accessory vessels. Likewise, MRA did not detect stenotic accessory renal arteries, depicted 9/16 polar renal arteries, but missed two main renal arteries. All methods depicted the four main renal arteries with fibromuscular dysplasia. The overall sensitivity, specificity, and positive and negative predictive accuracy were 75%, 89.6%, 60% and 94.6%, respectively, for color Doppler US; 94%, 93%, 71%, and 99%, respectively, for CTA; and 90%, 94.1%, 75%, and 98%, respectively, for GD-enhanced MRA. CTA and GD-enhanced MRA have comparable and satisfactory results with respect to the negative predictive accuracy of the suspected renal artery stenosis. The concept of an imaging algorithm including US as screening test when appropriate and CTA or MRA as the second step-procedure is suggested. Therefore, DSA may be reserved for cases with major discrepancies or therapeutic interventions.

Comparison of Intraarterial MR Angiography at 3.0 T with X-ray Digital Subtraction Angiography for Detection of Renal Artery Stenosis in Swine

To compare the accuracy of catheter-directed intraarterial (IA) magnetic resonance (MR) angiography at 3.0 T with that of x-ray digital subtraction angiography (DSA) for the measurement of renal artery stenosis (RAS) in swine. Unilateral hemodynamically significant RAS (>50%) was induced surgically in six pigs with use of reverse cable ties. One to two weeks after surgery, each pig underwent x-ray DSA and MR angiography before and after percutaneous transluminal balloon angioplasty (PTA). X-ray DSA was performed before and after PTA of RAS by injection of iodinated contrast agent through a 5-F multiple-side hole angiographic catheter placed in the abdominal aorta under fluoroscopic guidance. MR angiography of RAS was performed before and after PTA of RAS on a 3.0-T clinical MR imager with use of gadolinium-based contrast agent. MR angiography and DSA images were analyzed with the full width at half maximum method. Percent stenosis measurements between x-ray DSA and MR angiography were compared with a paired t test and were correlated with linear regression and Bland Altman analysis (alpha = 0.05). Six cases of RAS were induced and imaged successfully with DSA and MR angiography techniques before and after PTA. On x-ray DSA, median stenoses was 64% (95% CI 57%-80%) before PTA and 20% (95% CI 5%-32%) after PTA. Corresponding MR angiography median stenosis measurement was 69% (95% CI 58%-80%) before PTA and 26% (95% CI 16%-36%) after PTA. A paired t test comparison did not show a difference between DSA and MR angiography (P = .16). RAS measurements on MR angiography correlated closely (P < .01) with DSA measurements (r(2) = 0.92). In swine, the accuracy of catheter-directed IA MR angiography with use of a clinical 3.0-T MR imaging unit for the measurement of RAS was similar to that of conventional x-ray DSA.

Technology Insight: clinical role of magnetic resonance angiography in the diagnosis and management of renal artery stenosis

Noninvasive modalities, including duplex ultrasonography, renal scintigraphy, CT angiography and magnetic resonance angiography (MRA), may usefully contribute to diagnosis and treatment planning in patients with suspected renal artery stenosis. Important technical developments have increased the accuracy and feasibility of MRA for the detection of renal artery stenosis. A number of different MRA techniques can be applied to the study of renal arteries, but contrast-enhanced MRA represents the most valuable approach; several studies corroborate the high diagnostic accuracy of this technique, especially for the detection of atherosclerotic renal artery stenosis. A combined MRA protocol, which might include angiographic information provided by contrast-enhanced technique in addition to renal flow information derived from phase-contrast imaging, could help in classifying patients appropriately. Limitations of renal MRA include low accuracy in the evaluation of renal fibromuscular dysplasia and in the assessment of patients who undergo stenting of the renal arteries. This review describes the MRA techniques applied to the study of renal artery stenosis, including the technical features of current approaches and forthcoming developments. An overview of the clinical role of MRA, in conjunction with the other diagnostic modalities, in the identification and management of patients with renal artery stenosis, is also presented.
