Unexplained infant death in Houston, TX: A 17-year retrospective

SIDS/SUID (henceforth referred to as SIDS) is a leading cause of neonate death, taking the lives of ~10 infants every day in the US alone. SIDS is defined as the death of an infant for which no cause of death can be determined after a complete autopsy, death scene investigation, and medical history review, all of which makes SIDS a very diffcult disease to study. Death scene investigations have identified several contributing environmental factors. However, etiological investigation has been complicated by inconsistencies in classification of death by medical examiners or coroners and unreliable resources for law enforcement to complete death scene investigations, which leads to epidemiological studies often lacking rigor or suffcient data. Using data from Child Fatality Review Team (CFRT) of Harris County, the Medical Examiner’s Offce, and the Texas Department of State Health Services, we performed a retrospective analysis of 17-years of SIDS cases in Harris County, Texas. We applied a novel birth rate correction for SIDS rates over the first year of life. Associations were tested using a Tukey HSD and linear regression models with co-linear factors considered. To focus on biologically unexplained SIDS cases, we subsetted our dataset to only include SIDS and Undetermined causes of death. Our preliminary data includes 1,094 cases comprised of ~43% black, ~32% Hispanic, ~20% white, ~3% Asian, and ~0.4% unknown and 57% male infants. Distribution of death across the first 12 months of life shows a peak of death around 2 months of age. We do not see an association with month or season in the raw SIDS data nor when using our novel correction for birth rates. When we apply the standard per 1,000 live births normalization, we see a significant increase in Spring and Winter. We also report race-dependent significant differences in yearly and monthly case numbers, male-to-female ratios, and age at death and additional significant race-dependent associations between SIDS cases and adults diagnosed with asthma, income level, and overcrowding based on Zip Code. This dataset represents one of the most racially and socio-economically diverse in the United States. Our data suggests that standard trends may be driven by majority white datasets and that response to socioeconomic stress varies by race/ ethnicity, which leads to differences in risk between classically described minority groups. Survival of neonates, infants, and children with birth defects: a population-based study in Texas, 1999–2018

Birth defects are a leading cause of neonatal, infant, and childhood mortality, but recent population-based survival estimates for a spectrum in the U.S. are lacking. Using the statewide Texas Birth Defects Registry (1999-2017 births) and vital records linkage to ascertain deaths, we conducted Kaplan-Meier analyses to estimate survival probabilities at 1, 7, and 28 days, and 1, 5, and 10 years. We evaluated survival in the full cohort of infants with any major defect and for 30 specific conditions. One-year survival analyses were stratified by gestational age, birth year, and case classification. Among 246,394 live-born infants with any major defect, the estimated survival probabilities were 98.9% at 1 day, 95.0% at 1 year, and 93.9% at 10 years. Ten-year survival varied by condition, ranging from 36.9% for holoprosencephaly to 99.3% for pyloric stenosis. One-year survival was associated with increasing gestational age (e.g., increasing from 46.9% at <28 weeks to 95.8% at ≥37 weeks for spina bifida). One-year survival increased in more recent birth years for several defect categories (e.g., increasing from 86.0% among 1999-2004 births to 93.1% among 2014-2017 births for unilateral renal agenesis/dysgenesis) and was higher among infants with an isolated defect versus those with multiple defects. This study describes short- and long-term survival outcomes from one of the largest population-based birth defect registries in the world and highlights improved survival over time for several conditions. Our results may lend insight into future healthcare initiatives aimed at reducing mortality in this population. FRI520 Disparities In Available Surgeons And Specialty Physicians Providing Endocrine-related Health Care Coverage To Populations In South Texas And The Rio Grande Valley

Introduction: The Rio Grande Valley and South Texas is home to some of the most medically underserved populations in the nation, with gaps in health care ranging from low rates of adequately insured individuals, to the amount of primary and specialty healthcare services available [1]. Populations living in these areas near the Texas-Mexico border have some of the highest incidence of metabolic and endocrine-related disease in the U.S., while paradoxically having a large deficit in endocrine-specialists available to meet the medical and surgical needs of the community [1,2,3]. Methodology: We analyzed data from the National Center for Health Workforce Analysis (NCHWA), as well as data from the Texas department of state health services (TDSHS) to determine current and projected endocrine-related healthcare coverage needs in South Texas and the Rio Grande Valley (RGV) between 2013 to 2025 and 2018 to 2032 respectively [2,3]. Results: Analysis of the data provided by the NCHWA revealed that by 2025 the southern region of the U.S. is projected to have the largest shortage of full-time equivalent (FTE) surgical specialty physicians in the entire country with a deficit of 10,210 [3]. ​Additionally, data displaying critical-shortages provided by the TDSHS stated that in 2018 the percent demand met (PDM) for FTE endocrinologists in South Texas was only 76.6% with a projected decline to 65.7% by 2032 [2]. Noteworthy data from the TDSHS also stated that in 2018 the RGV specifically had a significant deficit in endocrinologists with a PDM of only 53.6%, while simultaneously having a projected increase to 66.4% by 2032 [2]. The TDSHS classified specialties in South Texas to be critically short when they are projected to have a deficit of ≥100 FTEs and &amp;lt;80% of the physician demand being met [2]. Conclusion: The collection of data analyzed led us to effectively identify significant disparities in endocrine-related healthcare coverage in the South Texas region, specifically the RGV. Additionally, new multidisciplinary healthcare institutions built in the RGV are striving to improve patient access to primary and subspecialty care by recruiting and training physicians in the area, but further action is still needed to counteract the described critical-shortages in physicians responsible for the long-term management of endocrine-related diseases. In-Hospital Mortality Risk and Discharge Disposition Following Hip Fractures: An Analysis of the Texas Trauma Registry.

In-hospital mortality and discharge disposition following traumatic hip fractures previously reported in the literature, has mainly focused on a nationwide scale, which may not be reflective of unique populations. Our aim was to characterize demographics, hospital disposition, and associated outcomes for patients with the most common hip fractures. A retrospective study utilizing the Trauma Registry from the Texas Department of State Health Services. Patient demographics, injury characteristics, and outcomes, such as in-hospital mortality, and discharge dispositions, were collected. The data were analyzed via univariate analysis and multivariate regressions. There were 17,104 included patients, composed of 45% femoral neck fractures (FN) and 55% intertrochanteric fractures (IT). There were no differences in injury severity score (ISS) (9 ± 1.8) or age (77.4 ± 8years old) between fracture types. In-hospital mortality risk was low but different among fracture types (intertrochanteric, 1.9% vs femoral neck, 1.3%, P = .004). However, when controlling for age, and ISS, intertrochanteric fractures and Hispanic patients were associated with higher mortality (P < .001, OR 1.5, 95% CI 1.1-2.0). Uninsured, and Black/African American (P = .05, OR 1.2, 95% CI 1.1-1.3) and Hispanic (P < .001, OR 1.2, 95% CI 1.1-1.3) patients were more likely to be discharged home after adjusting for age, ISS, and payment method. Regardless of age, severity of the injury or admission hemodynamics, intertrochanteric fractures and Hispanic/Latino patients had an increased risk of in-hospital mortality. Patients who were uninsured, Hispanic, or Black were discharged home rather than to rehabilitation, regardless of age, ISS, or payment method.

Use of intraoperative platelet-rich plasma during rotator cuff repair is correlated with increased patient-level charges across multiple categories.

As the efficacy of platelet-rich plasma (PRP) as an adjunct in rotator cuff repair (RCR) is debated, the decision to use PRP may be more heavily affected by its cost. We sought to quantify whether augmenting arthroscopic RCR with PRP injections at the time of surgery is correlated with increased patient-level charges. All outpatient records reported in Texas from 2010 to 2018 were obtained from the publicly available Texas Healthcare Information Collection database through the Texas Department of State Health Services. All records including a Current Procedural Terminology code for arthroscopic RCR were included. Of the 139,587 records identified within this group, 1662 also contained a Current Procedural Terminology code for intraoperative PRP injection. Patient-level charge data were compared between those who received and those who did not receive concomitant PRP injection during the same outpatient surgical encounter. Subgroup analyses were performed across surgical facilities and insurance types. Mann-Whitney U tests were used to compare charges between PRP and non-PRP cases. Linear regression was used to predict the change in billed charges according to standard charge categories. P values less than .05 were considered statistically significant. The total charges for arthroscopic RCR over the 8-year period were $4.66 billion, coming to $33,371±$22,118 per case. Cases that included PRP injection were found to have significantly greater overall charges than cases that did not ($54,452±$33,637 vs. $33,117±$21,818; P<.001). Linear regression indicated that concomitant PRP injections predicted an increase in combined total charges by $22,027 (95% confidence interval, $20,425-$23,628; P<.001). PRP utilization at the time of rotator cuff surgery is correlated with increased patient-level charges overall, which occur across all charge subcategories and persist across surgical facility, surgeon volume, and insurance type. Detailed cost analysis is recommended to explore this charge correlation, and future cost-benefit analyses of PRP use in RCR should explore costs beyond that solely associated with PRP preparation, as these may have previously been overlooked.

Association Between Publicly Funded Contraceptive Services and the Abortion Rate in Texas, 2010-2015.

To evaluate how the availability of contraceptive services was associated with a change in the abortion rate before and after Texas' legislative changes to the family planning budget in 2011 and abortion access in 2013. In this cross-sectional study, we obtained 2010 and 2015 data on contraceptive provision (number of publicly funded clinics and number of contraceptive clients served per 1,000 reproductive-aged women) from the Guttmacher Institute and county-level abortion data from the Texas Department of State Health Services. We categorized counties as having an abortion rate that increased or declined less than the national rate between 2010 and 2015 ( low-decline counties ) compared with those having an abortion rate that declined equal to or greater than the national rate between 2010 and 2015 ( high-decline counties ). We evaluated differences in contraceptive provision between high-decline and low-decline counties and evaluated county characteristics (racial and ethnic composition, unemployment, poverty, uninsured, education, distance to an abortion clinic, deliveries covered by Medicaid, and Catholic hospital marketplace dominance) as potential confounders. Of 157 counties that had at least one contraceptive clinic in either 2010 or 2015, 49 were low-decline counties and 108 were high-decline counties. Although the total number of publicly funded family planning clinics increased by 10.8%, there was a 4.7% decrease in the total number of contraceptive clients served statewide. Compared with low-decline counties, high-decline counties had a higher median number of contraceptive clients served per 1,000 women aged 18-44 years (31.9 vs 60.7, P <.05) in 2015. Between 2010 and 2015, the abortion rate decreased 19.7% for each 1.0% increase in contraceptive clients served. Texas counties with higher abortion-rate declines had more publicly funded contraceptive clinics and served more contraceptive clients than counties with lower declines, which may indicate the importance of greater access to publicly funded contraceptive services.

Longstanding Rural-Urban Disparities Exacerbated by COVID-19: The Experiences of Older Residents in West Texas: Lessons Learned

Purpose: The primary objective of this study was to examine differences in COVID-19 testing, diagnosis, symptoms, preventive measures, and healthcare resources and services between urban- and rural-dwelling older adults of the West Texas region. Methods: Data from surveys of 303 adult urban and rural residents (&gt;40 years old) between June 2020 and January 2021 were conducted. County-level data from the Texas Department of State Health Services (TDSHS) were analyzed for the period between March 2020 and February 2021. Findings: County-level data from TDSHS revealed greater access to COVID-19 testing and higher case numbers yet lower COVID-19 fatalities among urban residents. On the contrary, rural counties had fewer overall COVID-19 cases, less access to COVID-19 testing, and higher COVID-19 fatalities. Conclusion: The COVID-19 pandemic exasperated the long-standing healthcare disparities documented between rural and urban neighborhoods, resulting in devastating consequences in rural areas suffering from poor healthcare infrastructure. Findings revealed disparities in COVID-19 experiences among urban and rural communities, which highlight the need for strategies aimed at improving healthcare access in rural areas through telemedicine and other innovative approaches.

Creating Novel Surveillance Data Products for Briefing Health Department Leadership and Elected Officials During the COVID-19 Pandemic in Texas.

Beginning in March 2020, the Texas Department of State Health Services (DSHS) developed several internal surveillance tools for briefing state health department leadership and elected officials on the status of the COVID-19 pandemic in Texas. This case study describes the initial conceptualization and daily production of 3 of these internal surveillance tools: (1) a COVID-19 data book displaying daily case, fatality, hospitalization, and testing data by county; (2) graphs and data files displaying new daily COVID-19 fatalities among residents of long-term care facilities in Texas; and (3) graphs and data files comparing COVID-19 cases and hospitalizations between the 4 COVID-19 waves in Texas. In addition, this case study uses qualitative interview data to describe how DSHS leadership used these surveillance products during the pandemic. Finally, details on challenges and lessons learned around creating and maintaining these tools are provided. These surveillance products are easily replicable, and our methods and lessons learned may be helpful for researchers or health department officials working on COVID-19 or other disease surveillance teams.

23 Identifying HPV vaccination initiation hot and cold spots and associated disparities in Texas

Over 90% of HPV-associated cancers are preventable through HPV vaccination. Only 43.5% of Texas adolescents, however, have completed the vaccine series, and the state ranks 47th in vaccination rates across the country. The goal of this study was to examine the geospatial uptake of the HPV vaccine in Texas and the associated disparities. HPV vaccine data from 2017–2021 was obtained from the Texas Department of State Health Services. Zip code-level estimates of individuals 9 years and older who received at least one HPV vaccine since 2017 in the Texas Immunization Information System (ImmTrac2) database were used to create a choropleth map. Through the Environmental Science Research Institute's (ESRI) ArcGIS Pro Version 2.2.0 software, the Cluster and Outlier Analysis Tool was used to determine a “neighborhood” around each zip code. Zip codes were subsequently demarcated as hot or cold spots (or neither) based off Local Moran's I score calculations, meaning they had a statistically significant greater or lesser concentration of vaccinations than would be expected from a random distribution. After identifying hot spots and cold spots of vaccine initiation, we explored the demographic neighborhood characteristics. 346 statistically significant hot and 374 cold spot zip codes of HPV vaccine initiation were identified (Fig. 1). The remainder of the zip codes (1115) were outliers or not part of statistically significant clusters. Hot spot zip codes, or areas of high vaccine initiation, were found along the Gulf coast, in large cities, and in less developed areas near the Texas panhandle. Cold spot zip codes were in the northern Texas panhandle, along the Texas–Louisiana border, and on the periphery of major cities. Compared to cold spot zips, hot spots had significantly higher percentages of children on public insurance, had higher poverty rates, and had higher percentages of African American and Hispanic residents.Download : Download high-res image (217KB)Download : Download full-size image In Texas, areas with higher poverty rates, increased use of public insurance, and higher percentages of minority populations had significantly higher rates of HPV vaccine initiation. This may be due to differences in patient interest in the vaccine, to vaccine access through government insurance, or to outreach efforts that have promoted the vaccine in especially vulnerable populations.

Journal Club: Remote infection control assessments in long-term care facilities during COVID-19 pandemic in Texas, 2020

This article, Remote Infection Control Assessments in Long-term Care Facilities (LTCF) during COVID-19 Pandemic in Texas, 2020,1 reported the results of remote infection control assessments (tele-ICARs) performed predominately in nursing homes and/or skilled nursing facilities (NH/SNF) and assisted living facilities (ALF) in both a proactive and responsive fashion by the Texas Department of State Health Services (DSHS) during the SARS-CoV-2 pandemic. The results from 438 LTCFs tele-ICARs conducted using a standardized assessment tool during the first 8 months of the COVID-19 pandemic (March – October 2020) were analyzed.

Connecting children with birth defects to health and social service programs: A collaboration between the Texas Birth Defects Registry and agency social workers.

Timely referral to services for children born with birth defects can improve health outcomes. Birth defects surveillance registries may be a valuable data source for connecting children to health and social service programs. Population-based, state-wide data from the Texas Birth Defects Registry (TBDR) at the Texas Department of State Health Services (DSHS) were used to connect children 9-18 months old, born with select birth defects with DSHS social workers. The social workers reviewed developmental milestones and referred children and their families to various health and social service programs. We tabulated the proportions of children meeting milestones and referral characteristics by referral program type and type of birth defect. Social workers reached 67% (909/1,362) of identified families. Over half of children (54%, 488/909) were not meeting the developmental milestones for their age. Social workers provided over 3,000 program referrals, including referring 21% (194/909) of children to Early Childhood Intervention (ECI) and 28% (257/909) to case management. Our results illustrate a method of leveraging a birth defects surveillance system for referral services. Given the large number of referrals made, our findings suggest that birth defects registries can be a valuable source of data for referring children to programs.

Measuring clinical lead testing coverage to support a population health initiative

Abstract Despite national efforts to reduce lead exposure, some children are still exposed to lead, leading to damage to the nervous system, delayed development, and learning and behavioral problems. CDC has recommended targeting high-risk groups for screening. In addition, Texas Department of State Health Services reported a concerning 17% lead testing coverage of 0-to-5-year-olds in in Harris County (where Houston is located). With the eventual goal of improving lead-related clinical care in a large pediatric hospital network, we asked the following questions: 1) What are the high-risk groups specific to our patient population? 2) For patients who are screened positive, are we adequately confirming the results? 3) For those with confirmed lead exposure, are we providing adequate follow-up care? To answer these questions, we analyzed lead testing and hemoglobin testing data on all children less than 6 years of age from the Texas Children’s Hospital’s electronic health record between 2017 and 2021. Lead screening could be performed on capillary or venous blood, but for elevations on capillary blood we analyzed if the results were confirmed with a venous blood test, as recommended by CDC. We used hemoglobin testing as an indicator of follow-up care of lead-exposed children, because an anemia work-up is recommended for such children. We repeated analysis using two different lead level thresholds, because of recent change to the CDC blood lead reference value from 5 mcg/dL to 3.5 mcg/dL in May 2021. We found 273 children (0.33% of children with capillary blood test) to have cBLL = 3.5mcg/dL. There was a significant racial disparity in the rate of lead screening. For example, the screening rate of Black children was 14%, whereas that of White children were 23%. There was a geographic discrepancy between the areas with most elevated cBLL and testing volume. For example, the area where cBLL was most frequently elevated (3.7%) received less than a fifth of tests compared to the most well-tested area. Only 75% of children with a cBLL = 5mcg/dL received a timely venous blood lead test for confirmation, and this dropped to 43% for children with a cBLL = 3.5mcg/dL. Similarly, only 75% of children with a confirmed venous lead level = 5 mcg/dL received a follow-up repeat vBLL test, and 55% received a follow-up hemoglobin measurement. There is room for improvement in population lead health, in terms of both equity and resource distribution. Improvement of lead-related clinical practice could benefit hundreds of children in the Houston metropolitan area.

Remote infection control assessments in long-term care facilities during COVID-19 pandemic in Texas, 2020

BackgroundThe Texas Department of State Health Services (DSHS) Healthcare Safety Unit (HCSU) conducts remote infection control assessments (tele-ICARs) in long-term care facilities (LTCFs) to evaluate COVID-19 infection prevention and control (IPC) knowledge and practices using a standardized assessment tool. Tele-ICARs are used to gauge different IPC measures specific to SARS-CoV-2 and are either proactive––conducted prior to identified cases––or responsive to an outbreak, which is defined as a new SARS-CoV-2 infection in any staff or any facility-onset infection in a resident. State and local partners use findings from the assessments to aid LTCFs by providing targeted and timely resources and support to mitigate identified gaps.MethodsData from tele-ICARs conducted between March 1 and October 30, 2020 were analyzed to assess major gaps across LTCF types. A major gap was defined as 10% or more of facilities not satisfying a specific IPC measure, excluding missing data. Gaps were also assessed by tele-ICAR type: proactive or responsive. Fisher's exact tests and univariate logistic regression were used to characterize significant associations between major IPC gaps and LTCF or tele-ICAR type.ResultsDSHS conducted tele-ICARs in 438 LTCFs in Texas during 8 months; 191 were nursing homes/skilled nursing facilities (NH/SNFs), 206 were assisted living facilities (ALFs) and 41 were other settings. Of the assessments, 264 were proactive and 174 responsive. Major gaps identified were: (1) 22% did not have a preference for alcohol-based hand sanitizer (ABHS) over soap and water; (2) 18.1% were not aware of the contact time for disinfectants in use; (3) 17.9% had not stopped resident communal dining; (4) 16.8% did not audit hand hygiene and PPE compliance; and (5) 11.8% had not stopped inter-facility group activities and extra-facility field trips. When restricting analyses to proactive tele-ICARs, one additional gap was identified: 11.1% of facilities lacked a dedicated space to care for or cohort residents with confirmed SARS-CoV-2 infection. Significantly more ALFs than NH/SNFs had not suspended resident communal dining (P < .001) nor identified a dedicated space to cohort residents with confirmed SARS-CoV-2 infection (P < .001). Significantly more LTCFs that received a responsive ICAR compared to a proactive ICAR reported a preference for ABHS over soap and water (P = .008) and reported suspending communal dining (P < .001) and group activities (P < .001). Also, significantly more LTCFs that received a responsive ICAR compared to a proactive ICAR had identified a dedicated space to cohort residents with confirmed SARS-CoV-2 (P = .009).ConclusionsIncreased facility education and awareness of federal and state guidelines for group activities and communal dining is warranted in Texas, emphasizing the importance of social distancing for preventing the transmission of SARS-CoV-2 in LTCFs, particularly ALFs. CDC recommendations for ABHS versus hand washing should be emphasized, as well as the importance of monitoring and auditing HCP hand hygiene and PPE compliance. Facilities may benefit from additional education and resources about disinfection, to ensure proper selection of disinfectants and understanding of the contact time required for efficacy. Analysis by tele-ICAR type suggests facilities may benefit from identifying space for dedicated COVID-19 units in advance of an outbreak in their facility. Conducting tele-ICARs in LTCFs enables public health agencies to provide direct and individualized feedback to facilities and identify state-wide opportunities for effective interventions in response to SARS-CoV-2.

Language Proficiency and Academic Success of Bilingual Hispanic Nursing Students along the US-Mexico Border

Background: The Texas Department of State Health Services reported an enrollment of 29.7% Hispanic heritage students in nursing schools in 2020. Hispanics comprise of 18.5% of the total US population. Hispanic communities are less likely to speak proficient English than non-Hispanic Whites due to lack of language concordance. A diverse nursing workforce is necessary to accommodate the changing community. English as an additional language is frequently identified as a barrier to a nursing degree. This study examined how English proficiency affects nursing academic success of non-BSN and BSN nursing students. Methodology: Non-experimental descriptive quantitative research with two open-ended questions were employed. Students from 27 non-BSN and BSN programs along the U.S.-Mexico border were surveyed. Participants (N = 158) self-reported Grade Point Average (GPAs) on a 4.0 scale and completed two language proficiency instruments, the Language Experience and Proficiency Questionnaire (LEAP-Q) and the English Language Acculturation Scale (ELAS). Participants responded to questions related to strategies that were either beneficial or barriers to their learning and success. Results: Regression of GPA on predictors of English proficiency yielded non-significant models. Convergent validity between the two language proficiency instruments was supported, and reliability was similarly high. MANOVA analysis revealed students on a BSN versus non-BSN program hold higher GPAs. Non-BSN students showed higher Spanish language proficiency than those on a BSN program. Seven themes emerged from the question data, three themes showed that English as an Additional Language (EAL) benefits students (support, communication, and academic resources) and four were barriers to learning (finances, time management, language, and self-perception). Conclusion: A diverse bilingual nursing workforce is essential to meet the future needs of the US. Even though language proficiency did not predict academic success, students enrolled in non-BSN and BSN students who utilized both languages were academically successful.

516-P: A One-Year Curriculum for Diabetes Self-Management Education to Promote Lasting Lifestyle Changes

The use of telehealth for Diabetes Self-Management Education and Support (DSMES) is expanding. There is a need among people with diabetes (PWD) to manage their diabetes, as they represent 20% of ICU admissions, 30% of hospitalized cases and have an increased risk of COVID-related mortality. While people at risk for diabetes or prediabetes may join a 1-year CDC accredited diabetes prevention program to make lifestyle changes to reduce their diabetes risk, PWD are often offered short-term DSMES programs that do not necessarily incorporate long-term goals. We developed a year-long intensive Diabetes Self-Management Education and Support (iDSMES) program combining Virtual Diabetes Conversation Map® and selected modified modules from the PreventT2® curriculum to suit PWD. The curriculum included 18 maps/modules. Our team of certified lifestyle coaches/diabetes educators/Conversation Map® Trainers used shared decision-making and problem-solving approaches to improve glycemic control and diabetes self-management. We hypothesized that iDSMES would improve diabetes outcomes in PWD. Fourteen PWD (12F/2M) with a BMI of 25 or above underwent the iDSMES in a group with their lifestyle coaches using the Zoom® platform. We collected their fasting blood glucose, weight, blood pressure, and physical activity minutes at each session (every three weeks) and assessed their Hemoglobin A1c quarterly. Nine PWD completed the iDSMES and lost an average of 8.8% of their initial body weight (a total of 203lbs) . We observed a 1% decrease in their A1c, 10mg/dL decrease in fasting blood glucose, and mm Hg decrease in systolic and diastolic blood pressure, respectively. There was also a 110-minute increase in their physical activity. A year-long iDSMES distance learning program effectively enhances lasting lifestyle changes to improve diabetes self-management in PWD. COVID-19 data driven planning: The SouthEast Texas approach.

This coautoethnographic case study used the Open-Source Public Health Intelligence process to explore and share the South East Texas Regional Advisory Councils' (SETRAC) experience in collecting, processing, disseminating/visualizing, and analyzing COVID-19 data during the pandemic in the largest national medical setting in the United States. Specifically, it details the production of Business Intelligence reports powered by PowerBI both with general publics and with Regional Healthcare Preparedness Program (HPP) Coalition Coordinators, County Judges and City Mayors, Texas Department of State Health Services (DSHS) executive leadership, the Offices of the Texas Governor, and the Federal Pandemic Task Force led by the US Vice President, in order to provide a foundation for situational awareness, inter-regional collaboration, allocation of scare resources, and local, regional, and state policy decisions. It highlights best practices in risk and crisis communications during the COVID-19 response, underscores cross-sector collaboration and standardization of data collection for effective planning and response, discusses pervasive data revealed during the analysis, and evaluates collaborative and feedback processes that have implications for the Health Care System and Homeland Security Enterprise information sharing.

Factors that Increase Risk of an HIV Diagnosis Following a Diagnosis of Syphilis: A Population-Based Analysis of Texas Men.

Studies have consistently shown that diagnosis of a syphilis infection increases the risk of human immunodeficiency virus (HIV) infection. This study examines patterns in HIV incidence among men following a Primary and Secondary (P&S) syphilis diagnosis. P&S syphilis cases among men reported to the Texas Department of State Health Services between January 2010 and June 2018 were linked to the Enhanced HIV/AIDS Reporting System. Risk factors for HIV diagnosis were identified using univariate and multivariate extended Cox proportional hazards models. The 9113 men with syphilis without a concurrent or prior HIV diagnosis contributed a cumulative 35,674 person-years with a mean follow-up time of 3.9years. The multivariate model showed that among men with a P&S syphilis diagnosis, age, race/ethnicity, transmission risk, comorbid STDs, subsequent STDs, and syphilis diagnosing facility were independently associated with a new HIV diagnosis. The results highlight disparities in HIV diagnoses, despite current prevention recommendations. Reducing these disparities will require multi-level, comprehensive interventions that are appropriate for the diverse populations around the state.

Infant Mortality and Maternal Risk Factors in Texas: Highlighting Zip Code Variations in 2 At-Risk Counties, 2011-2015.

IntroductionStark differences in the infant mortality rate (IMR) exist by geography in Texas. The Healthy Families initiative sought to understand how evidence-informed practices implemented in the community can improve pregnancy-related outcomes in 2 counties in Texas with a high prevalence of maternal chronic conditions. The objective of this study was to examine associations between maternal risk factors and infant deaths to inform strategies to improve outcomes.MethodsTwo counties with high prevalence of maternal chronic conditions were selected as Healthy Families sites: one with lower prenatal care usage than other counties in the state but an IMR lower than Texas, and the other with a higher IMR among minority racial and ethnic groups compared with other women in the county and Texas overall. Cohort-linked birth and infant death records from 2011 through 2015 provided by the Texas Department of State Health Services were analyzed by using logistic regression to examine associations of maternal sociodemographic and pregnancy risk factors with infant death. The data were mapped at the zip code level. Analyses were limited to births to women aged 15 to 49 years who resided in Texas from 2011 through 2015 (n = 1,942,899 births).ResultsThe Texas IMR was 5.4 per 1,000 live births, compared with 4.6 and 7.5 per 1,000 live births for Hidalgo and Smith counties, respectively. Congenital malformations were the leading cause of infant death in both counties for infants born in 2015, which was similar to Texas overall. In both counties, maternal marital status, education, multiple gestation, and cesarean delivery were significantly associated with infant mortality. Wide zip code–level variations in IMR and maternal risk factors were observed in both counties.ConclusionVariations in IMR and key maternal risk factors observed at the zip code level helped drive local strategies to maximize outreach of services to disproportionately affected communities.

Level II trauma centers have highest charges for hip fractures

Objectives: To evaluate the charges for pertrochanteric hip fracture care in Texas. Design: Database review. Setting: Texas Hospital Inpatient Public Use Data File (PUDF) Intervention: Charges reported by hospitals to the Texas Department of State Health Services for pertrochanteric hip fractures. Main outcome measurement: Charges associated with pertrochanteric hip fracture stratified by implant type and according to facility trauma level designation, urban versus rural, teaching versus non-teaching, and border versus inland status. Results and conclusions: There were a total of 44,853 pertrochanteric hip fracture surgeries performed over the three-year period in the state of Texas. The vast majority were treated at urban (93.4%), inland (non-border) (92.3%), non-teaching (74.2%) facilities with intramedullary fixation (56.9%). A significant increase in charges was associated with treatment at an urban ($32,412), border ($44,919), or teaching ($10,501) facility. Mean inpatient charges at Level II facilities was $113,700. Further study is warranted to determine what drives the differences in charges associated with hip fracture treatment. How Would Medicaid Expansion Affect Texas Hospitals? Evidence From a Retrospective Quasi-Experimental Study.

This study aims to estimate the impact of a potential Medicaid expansion on Texas hospitals. The Affordable Care Act (ACA) Medicaid expansion increased access to health care and improved health outcomes. Still, several states, including Texas, have not adopted the expansion. This is a retrospective quasi-experimental study. We obtained inpatient data containing discharges from Texas hospitals between 2010 and 2017 from the Texas Department of State Health Services. Texas hospitals receive a significant number of patients from the adjacent states. We use a difference-in-differences methodology, where the patients from the neighboring states that expanded Medicaid in 2014 are the treatment group, and those that reside in Texas are the control group. The outcome variables are the payer mix and the cost of treatment, proxied by Diagnoses Related Group (DRG) weights assigned by the Centers for Medicare and Medicaid Services (CMS). The Medicaid expansion is associated with 4.15% lower costs of treatment among the patients from the expansion states (P < .01). Also, the uninsured rate decreased by 4.7 percentage points (from 11.3%, P < .01), while the share of Medicaid patients increased by 10.9 percentage points (from 30.7%, P < .01). There are no significant changes in the share of privately insured or Medicare patients. Texas hospitals can benefit significantly from Medicaid expansion due to reductions in average treatment costs and the share of the uninsured.
