The rich tradition of radiology at the University of Michigan.

The evolution of radiology at the University of Michigan after Roentgen's discovery in 1895 has had a profound influence on radiology. The school has had notable firsts in medicine: it was among the first of the state-supported universities to have a medical school (1850), had the first teaching hospital owned by a university (1869), and had the first university department of roentgenology (1917). Given these circumstances, teaching and research were early high priorities. The original small hospital, a remodeled private residence in Ann Arbor (1869), proved inadequate and was replaced in 1891 by the Catherine Street Hospital, viewed then as the largest teaching hospital in the nation. This served the medical school well for many years but was succeeded by the magnificent new University Hospital in 1925. Built for $4.4 million with a bed capacity of 700 in a double-Y architecture, it was an entirely new concept in hospital design and was the pride of the state [1].

Aortic calcification in Israel. An epidemiological study.

Lateral roentgenograms of the lumbar spine from 1,463 ambulatory outpatients of the Department of Roentgenology of the Hadassah-Hebrew Hospital in Jerusalem were graded with respect to the degree of osteoporosis and aortic calcification. Aortic calcification increased in both sexes with an increase in age and to approximately the same degree, although men over the age of 60 showed somewhat more calcification than women of the same age. Many of the subjects were immigrants to Israel, but little effect of the area of origin upon the degree of aortic calcification could be proven. The association between the degree of osteoporosis and aortic calcification in both men and women over age 60 was highly significant, but this association is not explained by the fact that both disorders are age- dependent.

Dr. Bo Jacobsson has drawn together much of his recent work, previously published in eight separate articles in four different journals, to form this very readable short monograph. The individual papers deal with many aspects of thromboembolic complications of vascular catherisation and include clinical studies and experimental work both in vivo and in vitro.

RADIOGRAPHS FROM CINEFLUOROGRAPHS: A SIMPLE TECHNIC.

One of the frustrations of cineradiography is, to use a Latin pun, that cineradiography is radiography sine radiographs. This article describes a simple technic for obtaining radiographs from cinefluorographic films. The method can be employed while viewing cineradiography in any department of roentgenology. The cine projector is used as a photoenlarger, and no accessory photographic equipment is necessary. By this process, enlargements can be made of selected frames from cineradiographic studies which may be kept in the patient's radiographic file to be studied, demonstrated, and reviewed without continued reference to the original motion-picture film. The author first considered this process in November 1963, when he was asked to moderate a panel of radiologists on newer developments in the roentgenology of the heart and lung for the American Thoracic Society. He wanted to demonstrate some congenital cardiac lesions, but no facilities were available for cine projection. The thought occurred that the cine projector could be used as a photoenlarger, and an image projected upon an x-ray film should give an accurate reproduction. The first attempt was successful, and during the subsequent weeks the process was developed. A review of the literature and personal inquiry among many radiologists with extensive experience in the field of cineradiography has revealed no record of previous use of this technic. An excellent method utilizing a fixed projection system and a camera has recently been described by Blackstone (1). Technic Our cineradiographs are made on Shell-burst film with a camera attached to an 8-in. Picker amplifier. They are viewed on a 14 × 17-in. screen supported by a chest rack in a fluoroscopic radiographic room which is connected by a pass box to the darkroom. We use the Weinberg-Watson modification of the Kodak 16-mm. cine projector. When a frame is selected for enlargement the camera is stopped. The camera is focused on a blank screen interposed at the proper distance for the size of film desired. The projector light is extinguished and under red light visualization a cassette containing an unexposed film is brought from the pass box and placed in position. Exposure is made by turning on the projector light for the proper interval. The cassette containing the newly exposed film is returned to the pass box, and the film is processed in the X-omat. One may now resume cine projection, and the process may be repeated when another frame is selected for enlargement. To contain the film, we use cassettes in which the Bakelite front has been replaced by window glass. These may be supported on the chest rack, but a portable film-holder of the type employed in surgery has been found convenient as a cassette-holder which may be interposed at the proper distance. Several types of film may be used for the enlargements.

Barium Sulfate and Iodine Determination-Reply

<h3>To the Editor:—</h3> I have not conducted any studies on the actual change in protein bound iodine, nor on the effect of the administration of barium sulfate prior to I¹³¹-uptake studies; however, the following references are pertinent: In view of this information, it has been the policy of the Department of Roentgenology at Northwestern University Medical School and of our Department of Internal Medicine to insist that iodine studies be conducted prior to gastrointestinal x-ray examinations to avoid possible aberrations in results.

New technic for accurately projecting the dorsum sellae into the foramen magnum.

For many years the routine anteroposterior oblique Chamberlain-Towne projection of the skull has been used as originally recommended by Towne for studies of the petrous pyramid, especially for evidence of erosion by acoustic nerve tumors (1). In his paper published in December 1926, Towne, who was a neurologist, said: “Dr. W. E. Chamberlain of the Department of Roentgenology of Stanford University Hospital has recently succeeded in showing the destruction of the petrous bone in a verified case of acoustic nerve tumor. Instead of making lateral projections of the skull, in which the internal and external auditory meati of the two sides are identified with difficulty, he used a posterior projection in which the petrous bones are shown in profile. The two bones can thus be compared on one set of stereoscopic films. With the patient on his back, the head is flexed at the top of the neck, so that the chin is drawn backward. Radiographic examination of surgically removed specimens with a radiologic-pathologic correlation.

Roentgen study of gross surgical specimens has been found to offer certain advantages under a variety of circumstances. Some of the information that can be obtained in this way will be considered here. After the superfluous blood and fluid are drained off, the specimen may be placed upon absorbent paper and carried to the department of roentgenology for immediate radiography. The hemostats may be left in place, especially for studies of the intestinal tract. A lead arrow will serve readily to indicate either the oral or caudal end of the specimen. Suitable identification similar to that used in routine roentgen examinations should be incorporated in the radiograph—the date, case number, and indication as to the side, left or right. The specimen, with clean absorbent paper, is placed upon a cassette of suitable size and the exposure is made. A brief résumé of the technical factors is given in Table I. All examinations should be made on the table top of the radiographic unit, with par-speed intensifying screens in the cassette. The cassette size depends on the size of the specimen; the usual dimensions are 6 1/2 × 8 1/2, 8 × 10, or 10 × 12 inches. All exposures are made at 30 inches, with a cone to cover the cassette. The films are developed like any other film, usually for three and a half to four minutes. The selection of kilovoltage is based upon the usual thickness of the part, on the assumption that moderate collapse and flattening of the specimen will occur. For every additional 1.0 cm. thickness of part 2.0 kv may be added. The only exception to this is Category 4, where an occlusal film may be used. A small calculus or chicken bone does not measure 1 cm. in thickness, but with the reduced milliampere-second ratio, increased kilovoltage is needed to obtain a satisfactory image. Since screens are not used in the small film holders employed for this type of exposure, a longer developing time may be needed. With practice, a skilled technician will readily become adept in this procedure. Cardiac and Vascular Lesions Frequently on acute fluoroscopic scrutiny a calcified plaque may be identified in the pericardium, a vascular orifice, or in one of the cardiac valvular cusps. The aortic and mitral valves are more commonly the site of such deposits, but accurate localization by radiographic or fluoroscopic study is difficult. A black and white photograph will not show to advantage the location of these plaques, but on a radiograph of the specimen they are readily localized as to chamber or vascular stem. Similarly, the identity of a cardiac or aortic aneurysm with or without calcification can definitely be determined. Coronary artery sclerosis is also easily demonstrated. Pulmonary Lesions Verification of partial pulmonary sequestration incident to a broncholith is readily accomplished by a radiograph of a segment of resected lung. The same is true for an hemangiomatous vascular anomaly.

Walter Herman Ude, M.D. 1896-1956

The profession of medicine, and particularly radiology, suffered a severe loss with the sudden passing on Feb. 9, 1956, of Dr. Walter Herman Ude, who succumbed to coronary thrombosis while vacationing in Honolulu with his wife, who survives him. Dr. Ude was born March 19, 1896, in Minnesota, and except for a period prior to his entrance into medical school when he served with the Armed Forces in France in World War I, spent his entire life in that state. Following his graduation from the University of Minnesota, where he was elected to Alpha Omega Alpha, Dr. Ude practiced medicine in Minneapolis for some thirty years. He entered the specialty of radiology as a student in 1926 and soon became an accomplished radiologist, completing his career as the senior member of the firm of Ude, Borman, Ahern, and Stone. In addition to his private practice, he acted at various times as roentgenologist to St. Andrews Hospital, the Eitel Hospital, and St. Mary's Hospital in Minneapolis. For many years he was chief, and in his last years consultant of the Department of Roentgenology of the Minneapolis General Hospital. He was a devoted teacher and had attained the rank of Clinical Professor of Radiology at the University of Minnesota. It is notable that his last act before leaving for Honolulu was to lecture to the senior class at the University on obstetrical and gynecological radiology, a field to which he had contributed so much, A member of many medical associations, including local and national radiological societies, he took an active part in organized medicine. For many years he was a Councilor, and during the past year a Vice-President, of the Radiological Society of North America. His activities in the Minnesota campaign to stamp out tuberculosis were of great importance and he is cited by Dr. Jay Myers, in a book on this subject as having made many personal sacrifices and rendered valuable service working with physicians to demonstrate how to produce satisfactory films of the chest. As president of the Minnesota Radiological Society in 1937 and 1938, and as a member of the Committee on Tuberculosis of the State Medical Association, Dr. Ude cooperated fully with the Minnesota Public Health Association in regard to problems in the diagnosis of tuberculosis. Dr. Ude's major scientific contribution was his original description, in 1934, of a method for the roentgenologic diagnosis of placenta previa, an important development in obstetrical management. He also was the author of papers on such subjects as diaphragmatic hernia, osteitis condensans ilii, early lobar pneumonia, and others. Walter Ude was a fine example of the physician who is a good citizen, taking an active part in the affairs of his community, while at the same time making an outstanding contribution to his own specialty. Above all, he was a man who loved and helped his family and friends to the full extent of his ability.

Introduction of the Carman Lecturer

Members of the Radiological Society of North America and Guests : We are assembled here tonight for the Carman Leeture, established in memory of Dr. Russell D. Carman, who died at the peak of his career while serving as Chief of the Department of Roentgenology at the Mayo Clinic. The first Carman Lecture was delivered by Dr. B. R. Kirklin in 1939 and it has been an annual event since. Tonight's lecture will be presented by Dr. Axel Norman Arneson. Dr. Arneson was born in Fort Worth, Texas. He attended the public schools of that city and received his B.S. degree from Texas Christian University (which also has a football team). He received his M.D. from Washington University in St. Louis and was a Fellow at Memorial Hospital in New York for the next three years. He has the distinction of being a Diplomate of both the Board of Radiology and the Board of Obstetrics and Gynecology. We have first claim on him, however, because he became a Diplomate of the Board of Radiology in 1936 and did not become a Diplomat...

Early roentgen diagnosis in massive bleeding from the upper gastrointestinal tract: I. Clinical evaluation of safety and reliability of the method in 123 patients

D ESPITE the value of roentgenology in the diagnosis of gastrointestinal tract lesions it has not been widely used in the past for the emergency diagnosis of the cause of massive upper gastrointestinal hemorrhage, largely because of the fear of precipitating renewed bleeding. Hampton, Schatzki and others,l-s however, permitted carefully restricted examination of selected patients and contradicted the prevalent conservatism. At the same time they emphasized that emergency roentgen diagnosis cannot be used indiscriminately. There is need for further clarification of its indications, contraindications and complications, as well as a critical evaluation of its diagnostic reliability. These are the objects of the present study. Emergency roentgen examination of the gastrointestinal tract of 123 severely bleeding patients was performed. The examination was earlier and more complete in almost all cases than that recommended by other workers. The patients were divided into two groups: Group A consisted of the first fifty-tw-o patients examined. These were subjected to special attention and precautions not regularly available to ward patients. They were attended by a large group of roentgenologists, surgeons, ward physicians and others. Decisions regarding management were made in consultation by the several specialists. Certain patients were carefully exeluded from examination. Patients were regularly accompanied to the x-ray examining room by a member of the clinical group. One resident roentgenologist (T. P. C.) performed almost all of the examinations. After return to the ward the patient was again followed closely by various members of the team. Group B consisted of the last seventy-one patients examined. These patients were studied with the object of determining the accuracy and safety of the emergency roentgen method when applied without the special supervision afforded patients in Group A. They were managed almost entirely by the resident staff. Selection of patients was made by the ward physician. They were not regularly accompanied to the x-ray department by a clinician. Eight resident roentgenologists participated in the emergency roentgen examinations; one roentgenologist (S. E. H.) performed forty of the examinations. Age and Sex. The ages of the 123 patients ranged from thirteen to eighty-eight, averaging fifty-four years. One hundred one of the patients were male, twenty-two female. Severity of Bleeding. All patients manifested gross hematemesis or tarry stools or both, and were considered medical emergencies at the time of admission. Many were admitted in shock. Patients with occult bleeding were not included in the study.
