Cow’s Milk Anemia In Childhood Manifesting As Severe Cerebral Venous Sinus Thrombosis (P14-9.004)

<h3>Objective:</h3> To describe a series of toddler aged children with iron deficiency anemia, cerebral venous sinus thrombosis (CVST), and no other significant risk factors for thrombosis. <h3>Background:</h3> Up to 5% of toddlers (age 1–3) in the United States have iron deficiency anemia, a known risk factor for thrombosis. Childhood CVST has been reported in the setting of iron deficiency anemia due to excessive cow’s milk intake. <h3>Design/Methods:</h3> We performed a retrospective study of all children seen at Seattle Children’s Hospital between 2009 and 2021 with CVST. Charts and imaging reports of patients identified using diagnostic codes for CVST were reviewed to confirm diagnosis. Chart review was used to extract data about clinical presentation, neuroimaging and outcome. <h3>Results:</h3> Six children (15, 33 and 42 month-old boys, and 18, 24 and 38 month-old girls) had CVST, iron deficiency anemia, and apart from some degree of dehydration, no other significant CVST risk factors. They had a negative thrombophilia workup and no documented infection or cerebrovascular abnormalities. Milk intake ranged from 30 to 80 oz/day. Hemoglobin ranged from 2.4–7.5 (median 5.4) g/dL and ferritin from 3.0–4.0 (median 3.2) ng/mL. The deep venous system was involved in all patients, including straight sinus in all 6, and bilateral internal cerebral vein involvement in 5. Five patients suffered venous infarcts, including thalamic infarcts and deep white matter infarcts. All 5 of these children required rehabilitation services and have neurocognitive sequelae. <h3>Conclusions:</h3> In our series, CVST in the setting of cow’s milk anemia was associated with deep venous system involvement and deep venous infarcts. Neurocognitive sequelae were present in most patients. Cow’s milk anemia may result in a more severe form of CVST due to deep venous system involvement and associated anemic hypoxia. Spectrum of neurological disorders during pregnancy and postpartum period from a tertiary hospital

Background: Pregnancy and puerperium are states of hypercoagulability. This predisposes women in these phases to increased risk of cerebrovascular diseases. These neurological complications are important causes of maternal and foetal morbidity and mortality. These must be recognized and managed to decrease their burden on maternal and child health.Methods: The aim of this study was to consider the occurrence of neurological complications in pregnancy and puerperium and to analyse the clinical and radiological spectrum of them. This was a prospective study carried out with 1200 patients from January 2014 to August 2015.Results: Of the 1200 women, 87(7.25%) were diagnosed to have neurological complications. Overall mortality was 11.4%. Eclampsia (63.2%), Cerebral Venous sinus Thrombosis (CVT) (18.3%) and Posterior Reversible Encephalopathy Syndrome (PRES) in (8%) accounted for the majority of cases (Table 1). Eclampsia carried a significant mortality rate of 12.7% whereas CVT and PRES had favourable outcomes. Imaging of CVT revealed involvement of deep venous system in 12% and haemorrhagic infarcts in 69%.Conclusions: The commonest neurological complications seen in this cohort were eclampsia, cerebral venous thrombosis and posterior reversible encephalopathy syndrome. Hence a physician needs to bear in mind the common neurological complications that can occur during pregnancy and puerperium to avert poor pregnancy outcomes for both mother and child.

Clinical and radiological features of cerebral venous thrombosis in a children cohort

Cerebral venous thrombosis (CVT) is an uncommon and poorly studied condition in the pediatric population. To describe and compare the clinical and radiological features of non-neonatal children with CVT according to age and to analyze their association with functional impairment or mortality at hospital discharge. An observational cohort study of chil dren older than 30 days with a first CVT diagnosed with imaging/venography by magnetic resonance (IMR/VMR). We measure functionality with the modified Rankin scale defining marked impairment with 3 to 5 points. We used U-Mann-Whitney test to compare ages averages between groups with and without the different studied variables (significance < 0.05). We used logistic regression analyses to estimate the risk of adverse outcome for each variable expressed in Odds Ratios (ORs) and 95% confidence intervals (CI). Among 21 patients recruited, 42.8% were girls, median age 6.27 years (Interquartile range: 0.74-10). The average age was lower in children with diagnostic delay > 48 hours (p = 0.041), score < 12 in the Glasgow coma scale (p = 0.013), seizures (p = 0.041), sinus rectus thrombosis (p = 0.011), and intracranial hemorrhage (p = 0.049); while it was significantly higher in children with intracranial hypertension syndrome (p = 0.008). The presence of some chro nic systemic condition (OR = 11.2; CI = 1.04-120.4), deep CVT (OR = 14; CI = 1.3-150.8), and brain ischemia (OR = 15.8; CI = 1.4-174.2) was associated with marked functional impairment or mor tality at discharge. Clinical and radiological features of CVT are age-related. Chronic illnesses, deep venous system involvement, and brain ischemia predict adverse short-term outcomes.

Role of penumbra mechanical thrombectomy device in acute dural sinus thrombosis.

Background:In dural venous sinus thrombosis (DVST), the mortality ranges 5–30%. Deep venous system involvement and septic dural sinus thrombosis have a higher mortality rate. In acute occlusion, collateral flow may not be established, which may result in significant edema and mass effect. Endovascular interventions may be considered as a treatment option in appropriate high-risk patients with DVST.Materials and Methods:Eight patients with magnetic resonance imaging (MRI)-confirmed dural sinus thrombosis, who did not respond to the conventional standard medical treatment, were subsequently treated with mechanical thrombectomy using the Penumbra System®. In all cases, medical treatment including anticoagulants were continued following the procedure for a minimum period of 1 year.Results:Recanalization of the dural sinus thrombosis was achieved in all 8 cases. There were no immediate or late endovascular-related complications. One death occurred due to an unrelated medical event. At 6 months, there was notable improvement in the modified Rankin Score (mRS), with 5/8 (62%) patients achieving mRS of 2 or less. The follow-up ranged between 3 months and 26 months (mean: 14.5 months), and there were no new neurological events during the follow-up period.Conclusion:Cerebral venous sinus thrombosis is a rare but life-threatening condition that demands timely diagnosis and therapy. In cases of rapidly declining neurological status despite standard therapy with systemic anticoagulation and anti-edema measures, mechanical thrombectomy could be a lifesaving and effective option. In this study, good outcomes were observed in the majority of patients at long-term follow up.

Underlying prothrombotic states in pregnancy associated cerebral venous thrombosis

The exact pathogenesis of pregnancy associated cerebral venous thrombois is still unsettled. Aims : To identify possible inherited and acquired prothrombotic risk factors and also identify the factors associated with mortality in pregnancy associated CVT. Prospective cohort study to identify prothrombotic risk factors and case control study of influence of local traditional practice of puerperal water restriction on postpartum CVT. Consecutive patients with pregnancy associated CVT seen over a period of three years. Thrombotic workup included genetic markers, protein assays, and other factors. Univariate and chi-square analysis. Of the 41 patients studied during the study period, 71% of patient had a single and 34% had multiple prothrombotic risk factors. Methylene tetrahydro-folate reductase (MTHFR) heterozygosity (19.5%) and factor V Leiden heterozygous (7.3%) were the commonest genetic markers. Hyperhomocysteinemia (34%) and elevated factor VIII levels (14.6%) were the other important risk factors. In this cohort the mortality was 17%. Mortality increased by odds of 1.3 for every additional prothrombotic marker. The factors associated with increased mortality included: status epileptics (P = 0.05, OR 13.2, 95% CI 1.002 - 173), deep venous system involvement (P = 0.016, OR 9.64, 95% CI 1.53 - 60.6), presence of midline shift (P = 0.012, OR 24.7, 95% CI 2.05 - 29.8) and diffuse cerebral edema (P = 0.006, OR 14.5, 95% CI 2.18- 96.4). The traditional practice of decrease intake of water during puerperium was significant in woman with pregnancy associated CVT when compared to control subjects (P < 0.02). In patients with pregnancy associated CVT, prothrombotic markers can be multiple and are associated with increased odds of mortality. Deep venous system involvement, presence of midline shift and diffuse cerebral edema increased mortality. Peuperial water restriction may be a modifiable risk factor.

Intrasinus thrombolysis in cerebral venous sinus thrombosis: Single-center experience in 19 patients

Evidence from small case series suggests that the intrasinus thrombolysis (IST) is relatively safe and effective in rapid recanalization of thrombosed sinuses and reversal of neurological deficits in patients with cerebral venous and sinus thrombosis (CVST) However, in the absence of randomized controlled trials, the exact role of IST in the management of CVST is unclear, To study the safety and efficacy of IST in patients with CVST. Adult patients with CVST who received IST during a two-year period (January 2003-December 2004) were included. Data regarding demographic, clinical and radiological features were collected. Follow-up data were obtained at 3-6 months. Magnetic resonance venography (MRV) was repeated to assess the recanalisation of venous sinuses. Nineteen patients (11 women) with a mean age of 32 years (range 17-46 years) received IST during the study period. Common clinical features at presentation included headache, altered consciousness and seizures. Indications for thrombolysis included clinical deterioration despite adequate anticoagulation and rapid worsening of consciousness or neurological deficits. Thirteen patients (68%) had dural sinus thrombosis alone and six others had coexisting deep venous system involvement. Venous infarcts were present in 13 patients. At discharge, 15 patients (79%) had good outcome and were either asymptomatic or had only mild deficits and were independent for activities of daily living. Three patients died and one survived with severe neurological deficits. Angiographic improvement (as per digital subtraction angiography) was noted in 12 patients (complete in five and partial in seven) and seven patients had poor or no recanalization of the involved venous sinuses. At a median follow-up of 6.3 months, 14 (74%) patients had no or mild neurological deficits. IST is safe and effective in patients with CVST who fail to respond to conventional medical treatment. However, the subgroup of patients who are likely to benefit the most from this procedure is not clear from our data. Large randomized controlled trials are required to further clarify this issue.

Venous malformations: MR imaging features that predict skin burns after percutaneous alcohol embolization procedures

To examine the value of magnetic resonance (MR) imaging for predicting the occurrence of skin burns in patients with venous malformations who undergo percutaneous alcohol embolization was the objective of the study. Pre-procedural MR imaging at 1.5 T from 40 patients with venous malformations who had undergone percutaneous alcohol embolization was retrospectively reviewed by two observers for these features: anatomic location, definition (well-defined or ill-defined), and the presence of skin, subcutaneous tissue, muscle, tendon, bone, joint, and deep venous system involvement. One observer recorded the length of skin involvement and volume of the malformation. Univariate and multivariate analysis tests were used to determine whether an association between the occurrence of skin burns and MR imaging features existed. The anatomic locations of the venous malformations were the lower extremity (20 out of 40), upper extremity (11 out of 40), trunk (four out of 40), head/neck (three out of 40) and pelvis (two out of 40). Of the 40 subjects, 15% (six out of 40) experienced skin burns. There was a significant association between the absence of muscle involvement (p = 0.0198) as well as the length of skin involvement (p = 0.027), with the occurrence of skin burns. Malformation size and all other features were not significantly associated with skin burns. Skin burns in patients with venous malformations treated with alcohol embolization are associated with the length of skin involvement and with the absence of deeper tissue involvement, as depicted on MR imaging.
