Adenoid Cystic Carcinoma of Maxillary Sinus

ABSTRACT Adenoid cystic carcinoma is a rare, slow-growing tumor that develops from the mucous glands’ epithelium. Most commonly, it develops in salivary glands and is rare in the nose and paranasal sinuses. It is common in the fourth to sixth decade of life, with a female preponderance. We present a case of a 40-year-old male who came with complaints of right-sided nasal obstruction, which was clinically and radiologically diagnosed as a mucocele. He underwent endoscopic excision of the mass. Histopathology was suggestive of adenoid cystic carcinoma. Later, the patient underwent right orbital floor-sparing total maxillectomy, followed by adjuvant radiotherapy.

The incidence of maxillary sinus carcinoma in nasal mass

The external nose is pyramidal in shape with its root above and base directed downwards. The framework is composed of bones and cartilages. The bones are the two nasal bones articulating medially. Laterally they articulate with the frontal process of maxilla, superiorly to the medial part of nasal notch of frontal bone and inferiorly to the lateral cartilage of nose and posteriorly with the perpendicular plate of ethmoid bone. Considering the malignant conditions of nose & PNS, the most common issq. cell carcinoma of maxillary sinus (37.S°lc>), followed by S. q cell carcinoma of nose (25%), and then Adenocarcinoma of maxillary sinus (1 2.5%), Sq. cell carcinoma of Ethmoid sinus (12.5%) & adenoid cystic carcinoma of maxillary sinus (12.5°/o) These findings are comparable with reports of Cheesman (1957) , who had noted that Sq. cell carcinoma comprised of 80% of cases & Bahadur et al (1984), who reported that most common was Sq. cell carcinoma (65%).Followed by adenocysticcarcinoma (12%), Adinocarcinoma (12%) Meliginate lanoma (2%) olfactory neuroblastoma(2%).

FNAC diagnosis of adenoid cystic carcinoma of the maxillary sinus: A case report with emphasis on cytological differential diagnosis

Fine needle aspiration cytology (FNAC) has been employed with increasing frequency as a diagnostic tool for lesions of the major and minor salivary glands. The reported sensitivity and specificity of FNAC of salivary gland lesion ranges approximately between 60 and 100% and 90 and 100%, respectively. The major reason for varied statistics in diagnosis is due to its diverse morphological patterns and overlapping features that makes it a challenging job for pathologists to give a precise diagnosis. If established diagnostic criteria are present and strictly followed, a great majority of common variations of non-neoplastic lesions, benign and malignant salivary gland tumors can be diagnosed with high level of accuracy with FNAC. A patient with high operation risk may benefit from the information obtained by FNAC. We report a case of adenoid cystic carcinoma of maxillary sinus in a 46-year-old male patient diagnosed mainly through FNAC and also discuss the possible differential diagnosis and pitfalls of FNAC in diagnosing adenoid cystic carcinoma.

Adenoid cystic carcinoma of maxillary sinus: diagnosis, treatment and prognostic factors

To study the clinical characters, the outcomes of treatments and the factors affecting long-term treatment results of adenoid cystic carcinoma (ACC) of the maxillary sinus. The clinical data of 80 patients with ACC of the maxillary sinus treated initially were analyzed retrospectively. Survival rate, local recurrence and distant metastasis were analyzed using Kaplan-Meier method. Prognosis factors were analyzed by Log-rank test and Cox regression. The 5-, 10- and 15-year cumulative overall survival rates were 65.2%, 37.1%, 26.3% respectively and 5-, 10-, and 15-year disease-free survival rates 50.7%, 30.7% and 24.5% respectively. The 5-, 10- and 15-year cumulative local control rates were 68.5%, 47.3% and 47.3% respectively and the cumulative distant metastasis rate were 32.8%, 48.8% and 48.8% respectively. Prognostic factors affecting survival included T stage, pathologic grade and the modes of treatment (P < 0.05). Patients with combined therapy composed of surgery and radiation had a better local control, compared with surgery or radiation alone (χ(2) = 18.33, P < 0.01), and surgery combined with postoperative radiation was prior to preoperative radiation combined with surgery (χ(2) = 6.64, P < 0.05). Patients treated with surgery combined with preoperative radiation, either with doses of ≥ 60 Gy or with negative margins, had a better local control, compared with doses < 60 Gy and with positive margins (χ(2) = 5.06, P < 0.05). The most of patients (62.8%) died of local recurrence. The most of failure was due to recurrence. Combined therapy composed of surgery and radiation improves the local control and survival in patients with ACC of the maxillary sinus, compared with surgery or radiation alone. Surgery combined with postoperative radiation provides the best overall survival and local control and should be the first choice of treatments.

CT and MRI features of perineural tumor spreading along the trigeminal nerve in malignant head and neck tumors

目的 探讨头颈部恶性肿瘤沿三叉神经转移的CT和MRI征象,提高头颈部恶性肿瘤诊断的准确性,帮助临床提高肿瘤的治疗效果. 方法回顾分析9例经临床及影像学诊断、有沿三叉神经分支扩散现象的头颈部恶性肿瘤的CT和MRI资料. 结果 9例中,硬腭腺样囊性癌3例,鼻咽癌2例,上颌窦腺样囊性癌、上颌窦鳞状上皮细胞癌、颊部腺样囊性癌及腮腺腺样囊性癌各1例.9例中, 腺样囊性癌共有6例,占67%.在3例硬腭腺样囊性癌和1例上颌窦鳞状上皮细胞癌中可见远离原发灶的腭大孔和翼腭窝扩大,其内密度增高,其中1例还可见同侧卵圆孔扩大,三叉神经节有强化的软组织影.1例上颌窦腺样囊性癌中可见眶下孔破坏,同侧翼腭窝扩大.1例颊部腺样囊性癌可见同侧翼腭窝扩大,圆孔扩大,其内可见强化软组织侵入三叉神经节.1例腮腺腺样囊性癌可见耳颞神经处有软组织影,并破坏同侧卵圆孔侵入颅内.2例鼻咽癌中可见肿瘤侵入咀嚼肌间隙并侵犯同侧三叉神经下颌支. 结论头颈部恶性肿瘤可以沿三叉神经扩散,CT和MRI可以对其准确判断,熟悉三叉神经及其周围的头颈部解剖结构对诊断极为重要。

Adenoid cystic carcinoma of maxillary sinus with metastatic hepatocellular carcinoma

A case of collision tumor in the left maxillary sinus composed of adenoid cystic carcinoma (ACC) and metastatic hepatocellular carcinoma (HCC) is reported. Radiographic examination revealed masses in the liver and bilateral lung metastases. Histologically, proliferation of tumor cells with resemblance to HCC was observed, in addition to the ACC. For this reason, differential diagnosis between a second primary tumor and metastasis was made. The metastatic lesion immunohistochemically showed positivity for hepatocyte antigen (OCH1E5) and protein induced by vitamin K absence or antagonist II (PIVKA-II), sustaining the HCC diagnosis. Primary ACC and metastatic HCC in the maxillary sinus are rare, and this may therefore be the first case of maxillary sinus tumor with both these elements.
