A systematic review of the efficacy and safety of topical cyclosporine in dermatology.

Cyclosporine-A (Cyc-A) was initially prescribed as systemic therapy for patients receiving solid organ transplants or in patients with graft versus host disease (GVHD). Topical Cyc-A is an ideal form of cyclosporine in the treatment of mucocutaneous disorders as it causes fewer systemic side effects and has more stable results than steroids; however, poor absorption through the skin makes the development of new formulations necessary to improve skin permeability. The aim of this study was to evaluate the efficacy and safety of topical Cyc-A in different dermatological conditions. A thorough systematic review was performed on PubMed/Medline, Embase, Scopus, and Web of Science databases as well as Google Scholar, and relevant studies from 2000 until January 3, 2022, were selected. The study was conducted according to the Preferred Reporting Items for Systematic reviews and Meta-Analysis (PRISMA). Topical Cyc-A was observed to be an effective medication in the treatment of oral lichen planus, psoriasis, burning mouth syndrome, Pyoderma Gangrenosum, and Zoon's balanitis. Adverse side effects such as dysphagia, burning sensation, lips swelling, and gastrointestinal upset were reported following Cyc-A mouthwash use, whereas mild erythema, dryness, and fissuring of the skin were observed following the Cyc-A lipogel application. Topical Cyc-A was found to be a good alternative to traditional treatment regimens for immune-mediated mucocutaneous conditions. Cyc-A can be considered as a safe and efficient option in cases of long-term treatment as it does not have the same adverse effects of long-term steroids.

AbstractCyclosporine is one of the most effective agents available for the treatment of psoriasis. It is also widely used in atopic dermatitis and other dermatological disorders. Recent consensus guidelines for psoriasis recommend an initial dosage of 2.5–5 mg/kg/day (in one or two daily doses), with adjustments of 0.5–1 mg/kg/day every 1–2 weeks; the maximum daily dosage is 6 mg/kg/day. The most widely used cyclosporine schedule is intermittent short‐term therapy for 12–16 weeks; subsequently, when relapse occurs, treatment is restarted at the previously established, effective dosage. Short‐term cyclosporine therapy is also particularly useful as a “rescue” treatment for severe disease flares, and as a “bridging” treatment to other maintenance schedules. Hypertension and renal dysfunction are the major tolerability issues, but these are related to dose and treatment duration, and can be markedly reduced if appropriate dosage and monitoring guidelines are followed. Generally, if cyclosporine dosage is maintained at ≤5 mg/kg/day, and treatment duration at ≤2 years, these events are reversible after treatment is stopped. As definitive clinical experience continues to accrue for several cyclosporine monotherapy and combination regimens in various dermatological disorders, cyclosporine is likely to markedly extend its already integral therapeutic role in dermatology. THE LONG-TERM safety of cyclosporine in the treatment of psoriasis is discussed by Grossman et al1in this issue of theArchives. However, today, cyclosporine is used by dermatologists not only in the treatment of psoriasis but also increasingly in the treatment of atopic dermatitis2as well as of a number of other conditions such as bullous disorders, pyoderma gangrenosum, lichen planus, dermatomyositis, systemic lupus erythematosus, and various forms of vasculitis. It has also received recognition as an effective diseasemoderating agent in psoriatic arthritis. The time for evaluation of long-term safety seems appropriate, especially in the setting of psoriasis, since the drug has now been used for this condition for more than 10 years. The findings could also, to a certain extent, provide a basis for deciding whether cyclosporine should be used in the treatment of other chronic dermatologic diseases. A Review and Update of the Clinical Uses of Cyclosporine in Dermatology

Cyclosporine is a hitherto unique immunosuppressive agent that has provided a new approach to therapy in autoimmune diseases and has been increasingly used as a molecular tool for the investigation of immune regulation. It has produced excellent therapeutic responses in the treatment of psoriasis, acrodermatitis continua of Hallopeau, pyoderma gangrenosum, lichen planus, pityriasis lichenoides chronica, Behçets disease, and alopecia areata. Cyclosporine's mechanism of action, side effects, pharmacokinetics, and guidelines for patient monitoring, are discussed in this article.

Cyclosporine is a potent immunosuppressive agent with no appreciable effect on the bone marrow and a selective inhibitory effect on helper T cells. Oral cyclosporine was first used to prevent organ rejection but also has been reported to be effective in other disorders. In cutaneous diseases that respond to oral cyclosporine helper T cells appear to be involved in their pathogenesis. This article reviews the cutaneous diseases that have been treated with cyclosporine and its pharmacology and side effects. Two significant adverse side effects are renal dysfunction and hypertension, both of which are reversible when short-term low-dose (less than 5 mg/kg per day) oral cyclosporine is discontinued. Lymphoma is unlikely in an otherwise healthy patient who has received low-dose oral cyclosporine for limited periods. The use of oral cyclosporine in any patient should be carefully considered in terms of the risk/benefit ratio and needs to be carried out under close medical supervision. In view of the limited experience with cyclosporine in dermatology, whenever possible its use should be confined to formal clinical studies with established protocols and guidelines. Further controlled studies need to be performed to evaluate the efficacy of low-dose cyclosporine in many dermatoses and its side-effect profile, particularly over the long term.
