Comparative Study on Efficacy of Topical Application of Triamcinolone Acetonide and Curcumin in Management of Oral Lichen Planus

Introduction: Oral lichen planus (OLP) is a chronic inflammatory and immune mediated disease that affects the mucous membranes. OLP is cell-mediated, immune damage to basal keratinocytes that are recognized as being antigenically foreign or altered. Many treatment modalities have been tried since the time immemorial, steroids being the mainstay of the treatment, with more benefits than adversities, but now-a-days alternative/ herbal medicines are used keeping in view the chronicity of disease and long term treatment required to cure it. Curcuminoids, the major constituent of turmeric, has anti-inflammatory, antioxidant and anticancer properties. Natural / herbal product has minimal adversities so this can be more helpful for patients in the upcoming time. Aim: The study was conducted to compare the efficacy of conventional topical corticosteroid, triamcinolone acetonide 0.1% with 1% curcumin gel in OLP patient and to assess reduction in burning sensation, regression of size and ulcero-erosive appearance of lesion. Materials and Methods: An intervention study was conducted on 40 patients diagnosed with OLP which were divided into two groups. Group A was provided with Ointment 0.1% triamcinolone acetonide while Group B was provided with 1% curcuma longa extracts gel for local application on the affected regions The results were analysed using Students t-test,discrete (categorical) data were summarised in number (n) and percentage (%) and compared by chi-square (χ2) test. Groups were also compared by two factor (groups and periods) repeated measure (RM) analysis of variance (ANOVA). Results: Intragroup comparison showed statistically significant reduction in burning sensation, size and extension of lesion (p<0.001) as well as erythema and ulceration (p<0.001) in both the groups. In inter-group comparison, Group A showed reduction in burning sensation (97.3%), size and extension of lesion (100%), reduction in erosive pattern (94.7%) and reduction in ulcerative .......

A disorder in which endometrial tissues develop outer side the uterus or uterine cavity is called as endometriosis. It affects mainly females in their reproductive years, and is an oestrogen-dependent condition. Endometriosis a gynaecological disease or disorders, occurs in about 10% of female in reproductive age and in upto 50% of females with in-fertility. It is serious type of Hormonal disease or disorders in female which is caused due to hormonal imbalance these diseases totally change the quality of womens life. Decreased quality of life is the most significant predictor of direct health care and total costs so avoid this next step greater way to take care “Prevention is always better than cure”. Curcumin is a natural polyphenolic compound extracted from the spice, turmeric and has been reported to evidence anti-inflammatory (agent which reduce inflammation), antioxidant (the agent which reduce oxidation), and anti-proliferative properties via the modulation of multiple cellular. However, it remains to be determined whether effect of curcumin affects in the management of process of endometriosis. Turmeric contain especially its most active compound Curcumin have many scientifically proven health benefits, it’s a potent Anti-inflammatory and antioxidant and may also help improve symptoms of endometrosis.

A Systematic Review and Meta-Analysis on the Efficacy of Curcumin/Turmeric for the Prevention and Amelioration of Radiotherapy/Radiochemotherapy Induced Oral Mucositis in Head and Neck Cancer Patients.

Background:Oral Mucositis(OM) is an acute debilitating dose limiting toxicity of Radiotherapy/Radiochemotherapy(RT/RCT) in management of Head and Neck Cancer (HNC). Curcumin/Turmeric may reduce OM in patients. Aim: Efficacy of Curcumin/Turmeric for preventing and ameliorating the onset and severity of RT/RCT induced OM was analysed in this review. Methods:A systematic literature search with meta-analysis were performed using Mesh terms in PubMed, Google scholar, Science Direct, Cochrane library and manual searching, articles published from 2010 to April 2021 were included. Clinical trials that studied the efficacy/effects of turmeric / curcumin in management of RT/RCT induced OM in HNC patients were included. Statistical Analysis were done to calculate the pooled Risk ratio at 95%confidence interval with significance at p <0.05. Results:Nine studies with overall 582 patients with HNC undergoing RT/RCT were included qualitatively. From evidence, orally 1,500-2,000 mg/day of Curcumin/Turmeric, (80mg/day/0.1%mouthwash) of nanocurcumin, topically gel/mouthwash used with increase in frequency prior and the entire course of RT/RCT with long follow ups are beneficial with no serious adverse effects. Meta-analysis of 5 prophylactic trials favoured curcumin/turmeric in reducing the severity of OM (RR 0.48 at 95% CI=0.23,0.99,P=0.05), did not prevent the overall incidence of OM (RR 0.99 at 95%CI=0.95,1.03,P=0.67) but delayed the onset of OM (RR 0.38 at 95% CI=0.18,0.80, P=0.01) during RT/RCT compared to control. Mean mucositis grade (Grade 3) was reduced in curcumin/turmeric with a mean difference of (-0.85 at 95%CI=-1.02,0.67, P<0.00001) over control. Pooling of 2 therapeutic trials favoured Curcumin/Turmeric with significant reduction of pain score at a mean difference of -2.17 at 95%CI =-2.77, -1.58,P<0.00001 over chlorhexidine. Conclusion:Curcumin/Turmeric are safe, efficacious, well tolerated in preventing the delay in onset and severity of OM, therapeutically ameliorated pain in patients with cancer therapy induced OM. We recommend novel curcumin formulations, high quality randomised controlled trials to further improve its therapeutic effects.

POTENTIAL ROLE OF CURCUMIN AS A TREATMENT OPTION FOR COVID-19: A REVIEW

Severe Acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causes CoV disease 2019 (COVID-19) which is presently a pandemic declared by world health organization. Treatment modalities for this disease are being explored. Curcumin the phytoconstituent of Curcuma longa, can be a potential treatment for COVID-19 because various findings suggest its therapeutic properties like antioxidant, antiemetic, antifatigue effects which can potentially manage symptoms and signs of COVID-19, also it has antiviral properties such as impeding the viral binding to host cell as well as replication. It has restrictive actions on viral protease as well and having inhibitory actions on pro-inflammatory cytokine molecules.Thus, it alleviates symptoms due to inflammation such as fibrosis and edema in pulmonary region. Broncho dilatory effects of curcumin as well as suppression of cough via acting on bradykinin thus curcumin can alleviate symptoms such as cough in COVID-19 sufferers. Curcumin also has preventive actionson Cardiovascular and renal damage due to COVID-19. All these potential activities suggest the possible use of curcumin in management of COVID-19. Clinical studies should be considered to explore the same. In this review we highlight the potential therapeutic effects of curcumin against COVID-19.

Potential Therapeutic Targets of Curcumin, Most Abundant Active Compound of Turmeric Spice: Role in the Management of Various Types of Cancer.

Curcumin, an active compound of turmeric spice, is one of the most-studied natural compounds and has been widely recognized as a chemopreventive agent. Several molecular mechanisms have proven that curcumin and its analogs play a role in cancer prevention through modulating various cell signaling pathways as well as in the inhibition of the carcinogenesis process. To study the potential role of curcumin in the management of various types of cancer through modulating cell signalling molecules based on available literature and recent patents. A wide-ranging literature survey was performed based on Scopus, PubMed, PubMed Central, and Google scholar for the implication of curcumin in cancer management, along with a special emphasis on human clinical trials. Moreover, patents were searched through www.google.com/patents, www.freepatentsonline.com, and www.freshpatents.com. Recent studies based on cancer cells have proven that curcumin has potential effects against cancer cells as it prevents the growth of cancer and acts as a cancer therapeutic agent. Besides, curcumin exerted anti-cancer effects by inducing apoptosis, activating tumor suppressor genes, cell cycle arrest, inhibiting tumor angiogenesis, initiation, promotion, and progression stages of tumor. It was established that co-treatment of curcumin and anti-cancer drugs could induce apoptosis and also play a significant role in the suppression of the invasion and metastasis of cancer cells. Accumulating evidences suggest that curcumin has the potential to inhibit cancer growth, induce apoptosis, and modulate various cell signaling pathway molecules. Well-designed clinical trials of curcumin based on human subjects are still needed to establish the bioavailability, mechanism of action, efficacy, and safe dose in the management of various cancers.
