To PE or Not to PE: Perfusion SPECT/CT Avoids Overdiagnosis.

A 41-year-old woman with metastatic breast cancer presented with dyspnea, hypoxia, and elevated d -dimer. Perfusion planar imaging followed by SPECT/CT of the chest was performed due to the patient's iodinated contrast allergy. Planar images showed multiple pleural-based wedge-shaped defects concerning for bilateral pulmonary embolism (PE). Perfusion SPECT/CT of the chest confirmed multiple areas of perfusion defects but was considered negative for PE and attributed the perfusion defects to the compressing of pulmonary vasculature from metastatic lymph nodes and pulmonary masses. Given the high pretest probability of PE, a CT pulmonary angiogram was performed after premedication for contrast allergy confirming absence of PE.

Establishing Local Diagnostic Reference Levels for CT Angiography Examinations

Objective: Computed tomography angiography (CTA) examinations are performed to diagnose blood vessel anomalies. However, this examination results in a significant radiation dose being received by the patient. Local diagnostic reference levels (DRLs) are necessary for determining the extent of radiation dose during examinations. Therefore, the main objective of this study is to establish local DRL values for CTA examinations.Material and Methods: In this study, a total of 213 participants underwent CTA examination; wherein: 67 underwent computed tomography (CT) cerebral angiography (CTCA), 80 underwent CT pulmonary angiography (CTPA), 40 underwent CT lower limb angiography with aortogram (CTLLA), and 26 underwent CT upper limb angiography (CTULA). Body mass index along with the circumference of the body were calculated. A number of dose descriptors; such as computed tomography dose index volume (CTDIvol) and dose length product (DLP), were collected, and DLP was used to calculate the effective dose, using the conversion factor.Results: For the angiography phase, CTCA, CTPA, CTLLA, and CTULA had median CTDIvol values of: 31.62, 6.38, 12.16, and 10.12 mGy, respectively. Median DLP and effective dose for CTCA were: 1388.64 mGy*cm & 4.30 mSv, CTPA were 243 mGy*cm & 3.41 mSv, CTLLA were 1855.86 mGy*cm & 12.06 mSv, and CTULA were 945.98 mGy*cm & 2.93 mSv, respectively. Conclusion: The estimated local DRLs were lower than the international standard for the angiographic phase; however, the dose exceeded the international standard when the entire examination was considered. This study revealed the need for dose optimization in CTA examinations.

Role of Virtual Monoenergetic Images in the Assessment of Vessel Enhancement in Segmental Level in Third-Generation Dual-Source Dual-Energy CT Pulmonary Angiography—A Prospective Study

Abstract Introduction Pulmonary embolism is the third most common cause of cardiovascular death worldwide and imaging plays a pivotal role in establishing the diagnosis. Computed tomography pulmonary angiography (CTPA) scores over other modalities and is the current diagnostic investigation of choice. In this study, we assessed the main pulmonary artery and its corresponding segmental artery attenuation in reconstructed virtual monoenergetic (mono plus) images (VMI-MP) and linear blended images (spectral post processing, SPP) obtained from dual-energy CTPA. The values were compared using contrast-to-noise ratio (CNR) and signal-to-noise ratio (SNR). Materials and Methods Forty patients (mean age group, 53.6 years; 26 females and 14 males) with segmental pulmonary thromboembolism were included in this prospective study. The patients underwent CTPA study using bolus tracking in the dual-source CT-SOMATOM Force, Siemens. Postcontrast datasets (90 kV, 150 kV, and SPP) were used to reconstruct the monoenergetic images using syngo.via software virtually. Comparison was done between bivariate samples using the paired sample t-test. Results The mean Hounsfield unit (HU) artery in the left lung for VMI-MP and SPP images were 886.9 ± 242 and 356.8 ± 121.3 HU, respectively. Similarly, for the right lung, it was 868.3 ± 243.5 and 336.1 ± 105.5 HU, respectively. The mean attenuation of the arteries in MP images was higher and statistically significant (p-value <0.005). Likewise, the CNR) and SNR were found to have a statistically significant p-value (<0.005). An acceptable increase in image noise was seen in VMI as compared with SPP images. Conclusion Low-keV VMIs perform more effectively than the conventional polyenergetic spectrum to evaluate vessel attenuation, which in turn increases thrombus detectability. The increased CNR in VMI enables improved lesion conspicuity.

Diagnostics and treatment of pulmonary artery embolisms

In recent years the diagnostics of pulmonary artery embolisms (PE) has gained significance, with confirmation occurring in only about 15-25 % of suspected cases. Despite technological advances, radiological methods remain problematic due to radiation and contrast medium exposure. Clinical scores play acrucial role in the risk assessment of PE. High-risk situations call for specific measures, while negative D‑dimers can help avoid overdiagnosis. Computed tomographic pulmonary angiography (CTPA) remains the gold standard with high sensitivity and specificity. Treatment requires an interdisciplinary team (pulmonary embolism response team, PERT). Anticoagulation is an option for stable patients, while in unstable or unsuccessful courses, thrombolysis or interventional procedures can be considered. Side effects, especially the risk of bleeding, need to be considered for both forms of treatment.

Epidemiology of pulmonary embolism diagnosis and management among United States emergency departments over an eight-year period

IntroductionPulmonary embolism (PE) is a major cause of morbidity and mortality in the United States. However, as diagnostic imaging, risk stratification tools, and treatment have evolved over time, there is a critical need for current data on the incidence, testing, admission rates, and medical management of PE in the ED setting. MethodsThis was a cross-sectional study of ED patients with PE diagnoses from 1/1/2016 to 12/31/2023 using the Epic Cosmos national database. ED visits were identified using ICD-10 codes corresponding to acute PE. Chronic PEs were excluded. Outcomes included total ED visits, admission rates, anticoagulant treatment prescribed from the ED, and thrombolytic therapy. Anticoagulant prescriptions included warfarin, enoxaparin, dabigatran, apixaban, rivaroxaban, edoxaban, and betrixaban. Thrombolytic agents included alteplase, tenecteplase, and urokinase. We also assessed changes in the overall rate of CT pulmonary angiograms (CTPA) performed. ResultsOut of 186,138,130 total ED encounters, PE represented 531,968 (0.29 %). The overall rate of PE diagnosis rose slowly from 0.20 % in 2016 to a peak of 0.35 % in 2021. Among those with PE, 363,584 (68.3 %) were admitted. The rate of admission declined over time from 75.6 % to 66.1 %. Among those prescribed anticoagulation, the most common medication was apixaban (40.0 %), followed by rivaroxaban (17.3 %), enoxaparin (6.1 %), warfarin (2.6 %), and dabigatran (0.4 %). Thrombolytics were administered in 4.5 % of cases, with the rate of thrombolytics peaking at 5.3 % in 2018 before lowering to 3.5 % in 2023. The overall rate of CTPA increased from 2.4 % to 5.0 %, while the rate of proportion of PEs diagnosed declined from 8.7 % to 6.4 %. ConclusionThis study highlights significant shifts in the epidemiology and management of PE within the ED setting. Overall rates of PE rose, while a larger proportion were discharged. Direct oral anticoagulants have become the predominant therapy with the majority of patients receiving apixaban. Thrombolytic use occurs in a small subset and has been declining over time. CTPA rates have risen, while the overall diagnostic yield has declined.

Pulmonary Embolism Risk Assessment in Acute Isolated Distal Deep Venous Thrombosis.

The clinical risk factors and risk of developing pulmonary embolism (PE) in patients with acute isolated distal deep venous thrombosis (IDDVT) were assessed using single complete-duplex ultrasound (CDUS) imaging to reduce over-testing and over-treatment. This observational study was conducted from January 2020 to December 2022. Retrospective analysis was performed on CT pulmonary angiography (CTPA), blood coagulation markers, myocardial injury markers, blood gas analysis, and CDUS imaging of the lower extremity blood vessels of 146 patients with newly diagnosed IDDVT. Binary logistic regression was used to evaluate the relationship between these indicators and PE. After stepwise regression analysis, the predictors included in the regression model were D-dimer (DD), the sum of the thrombus length, and the maximum value of the thrombus width, with odds ratios (ORs) of 1.307 (p <0.001), 1.018 (p = 0.005), and 1.613 (p = 0.018), respectively. The combined prediction model achieved an area under the receiver operating characteristic curve (AUC) of 0.832 [95% confidence interval (CI): 0.761, 0.902]. By balancing the sensitivity and specificity of DD, combined single CDUS improves the predictive value for PE in patients with IDDVT. Simultaneous high-pitch multi-energy CT pulmonary angiography using a dual-source photon-counting-detector CT: A phantom experiment.

A dual-source CT system can be operated in a high-pitch helical mode to provide a temporal resolution of 66ms, which reduces motion artifacts in CT pulmonary angiography (CTPA). It can also be operated in a multi-energy (ME) mode to provide iodine maps, beneficial in the evaluation of pulmonary embolism (PE). No energy-integrating detector (EID) CT can perform simultaneous ME and high-pitch acquisition. This phantom study aimed to evaluate the ability of a photon-counting-detector (PCD) CT to perform simultaneous high-pitch and ME imaging for CTPA. A motion phantom was used to mimic the respiratory motion. Two tubes filled with iodine with intravascular thrombus mimicked by injecting glue within the tubes were placed along with 5, 10, and 15mg/mL iodine samples, on a motion phantom at 20 and 30 revolutions per minute. Separate high-pitch and ME EID-CT scans and a single high-pitch ME PCD scan were acquired and virtual monoenergetic images and iodine maps reconstructed. Percent thrombus occlusion was measured and compared between static and moving images. When there was motion, EID-CT ME suffered from significant motion artifacts. The measured iodine concentrations with PCD-CT in high-pitch ME were more stable when there was a motion, with a lower standard deviation than EID-CT in ME mode. The estimated percent thrombus occlusion dropped significantly with applied motion on EID-CT, while PCD-CT high-pitch ME mode showed good agreement between measurements on static or moving images. PCD-CT with combined ME and high-pitch mode facilitates simultaneous accurate iodine quantification and assessment of intravascular occlusion.

Diagnostic management of acute pulmonary embolism

Straightforward, accurate diagnostic management in patients presenting with clinically suspected pulmonary embolism (PE) is essential, since starting anticoagulant treatment may give important adverse effects of bleeding, while false exclusion of the disease may lead to recurrent VTE, with associated morbidity and mortality. In the past three decades, considerable improvement in the diagnostic management of PE has been made. Computed tomography pulmonary angiography (CTPA) has largely replaced conventional pulmonary angiography and ventilation-perfusion lung scanning as the imaging methods of choice. Several diagnostic algorithms, all able to minimize the need for radiological imaging have been developed and validated. Lastly, within the diagnostic algorithms, varying D-dimer cut-off levels have successfully been introduced to further downsize the need for radiological imaging.

Cardiogenic shock secondary to immune checkpoint inhibitor associated myocarditis.

Immune checkpoint inhibitors have transformed the treatment for multiple cancers and are increasingly used in recent years, but they can cause potentially life-threatening cardiac toxicity. We report a case of a 64-year-old gentleman who presented to the Emergency Department with symptoms of fatigue and breathlessness whilst receiving treatment with an immune checkpoint inhibitor, pembrolizumab, for cholangiocarcinoma. He was found to be in cardiogenic shock with an abnormal electrocardiogram and elevated cardiac troponin at presentation. Echocardiogram demonstrated severely impaired right and left ventricular function. Computed tomography pulmonary angiography and invasive coronary angiography excluded pulmonary embolism and acute myocardial infarction, respectively, and he was diagnosed with immune checkpoint inhibitor associated myocarditis. He was treated with high-dose methylprednisolone and a dobutamine infusion. Within days, his troponin and C-reactive protein levels decreased, and his left ventricular function improved. He was established on heart failure therapies and discharged from hospital 12 days later.

CT Imaging Findings of Pulmonary Artery Stenosis: A Pictorial Review.

Pulmonary artery stenosis represents a group of disorders involving main, branch or peripheral pulmonary arteries with pain, dyspnea, hemoptysis or even no symptoms. Early diagnosis and timely intervention are crucial for reducing mortality, but timely diagnosis is challenging due to the non-specific symptoms. Computed tomography pulmonary angiography (CTPA) is useful in the diagnosis because it can provide more details about abnormal changes in the lumen, vessel wall and adjacent mediastinal structures. Congenital and acquired pulmonary artery anomalies have some characteristics on CTPA, which can be useful for differential diagnosis. Awareness of these conditions is important for radiologists. This pictorial review provides an overview of CTPA imaging features of pulmonary artery stenosis.

Unsupervised physics-informed deep learning for assessing pulmonary artery hemodynamics

Deep learning advancements have significantly benefited medical applications. One such helpful application is noninvasive fractional flow reserve (FFR) evaluation along the pulmonary artery tree, which aids in planning optimal balloon pulmonary angioplasty. This study proposes a surrogate model that employs an unsupervised physics-informed neural network (UPNN) to predict FFR based on pulmonary CT angiography. To ensure the UPNN strictly follows the unique solution of the governing equations, we implemented a hard boundary conditions enforcement approach. Subsequently, a finite difference convolutional filter was developed to enhance connectivity among neighboring points. This allows the neural networks to propagate boundary conditions into the unseen vessel interior and perceive the geometric structure of the computational domain. We also introduced regularization constraints on the three components contributing to pressure drop within the artery tree. A total of 4500 synthetic pulmonary artery trees were used to train the UPNN with impedance outlet boundary conditions. We found that the limits of agreement of FFR trained by UPNN ranged from −0.05 to 0.04 with computational fluid dynamics (CFD) simulation results. The testing results showed that the correlation coefficient between FFR predicted by UPNN and CFD were 96.1% and 94.7% for synthetic and patient-specific data, respectively. Using invasive FFR as reference, the accuracy of FFR predicted by UPNN and CFD was 81.4% and 84.8%, respectively. These results demonstrate that our UPNN-based surrogate model’s performance in evaluating FFR aligns closely with CFD simulations.

The prevention, diagnosis and management of pregnancy-associated pulmonary embolism

Pregnancy-associated pulmonary embolism (PE) is a type of venous thromboembolism (VTE) that seriously threatens the lives of pregnant women and has gained more attention in clinical practice. Due to physiological and anatomical characteristics, the incidence of VTE during pregnancy and postpartum is significantly higher than that during non-pregnancy periods. Currently, guidelines widely acknowledge the exploration of appropriate risk assessment models to evaluate the risk of pregnancy-associated VTE and to take corresponding preventive measures from mechanical to medication methods according to different risk levels. For patients suspected of PE, initial assessments can be made based on whether they exhibit clinical manifestations of deep vein thrombosis, with options including vascular compression ultrasonography or chest X-ray examination. Confirmation relies on CT pulmonary angiography or V/Q imaging. For patients with shock, echocardiography can be quickly used for the diagnosis and differential diagnosis of PE. The management of pregnancy-associated pulmonary embolism PE depends on the patient's hemodynamic status, with options including reperfusion therapy or anticoagulation therapy. Extracorporeal membrane oxygenation may also be beneficial in high risk patients. The overall evidence level for the prevention and management of pregnancy-associated PE is low, and further exploration in clinical practice is still needed to promote the safety of pregnant women.

Incidence of fatal and non-fatal pulmonary thromboembolism after removal of tunnelled central venous haemodialysis catheters without ultrasound scan and anticoagulation.

Catheter related thrombosis is a common complication of tunnelled central venous catheter (TCVC) usage. There are concerns that TCVC removal could dislodge a thrombus to cause pulmonary thromboembolism (PE). The incidence of PE following TCVC removal is unclear and so the aim of this study was to investigate the incidence of PE and whether it is high enough to warrant screening with ultrasound with a view to systemic anticoagulation prior to TCVC removal. 1102 consecutive TCVC removals without ultrasound and systemic anticoagulation were included in this retrospective study. Data were extracted from electronic health records. Measures to identify PE events included: deaths, computed tomography pulmonary angiogram (CT-PA), isotope lung perfusion scans and D-dimers blood tests within 7 days of removal. Of the 1102 TCVC removals, the mean age of patients was 56.9 years and 57.3% were male. The primary renal diagnosis for 24.5% of patients was diabetic nephropathy. There were seven deaths following removal, none of which had PE as a contributing cause on review of their clinical history and death certificates. Five CT-PAs and one isotope lung perfusion scan were carried out in the 7 days after TCVC removal and none had a positive finding of PE. Three patient had D-dimers measured in blood within 7 days and none of these patients were subsequently diagnosed with PE. There was no evidence of fatal or non-fatal PE's occurring in the 7 days following TCVC removal. This would support the practice of removing TCVCs without the need for ultrasound screening and without a period of systemic anticoagulation.

An abysmal conundrum associated with a misplaced dialysis catheter.

A 25-year-old female patient with chronic glomerulonephritis, initiated on haemodialysis presented with high-grade fever, a dysfunctional catheter, low oxygen saturation and unstable blood pressure. Upon evaluation, the patient was febrile with elevated white blood cell counts. She was intubated, started on inotropes and transferred to the intensive care unit. Chest radiography showed that the tunnelled internal jugular dialysis catheter had migrated to the inferior vena cava. Echocardiography and transoesophageal echocardiography showed severe heart dysfunction with a large thrombus attached to the catheter, indicating catheter-related atrial thrombus (CRAT). CT pulmonary angiography revealed blockage of the segmental pulmonary arteries, suggesting pulmonary embolism. Blood and tracheal cultures revealed Methicillin-resistant Staphylococcus aureus (MRSA). The patient was administered appropriate antibiotics and anticoagulants and underwent surgical removal of the thrombus and the catheter. This case demonstrates the risks associated with improper placement of dialysis catheters and highlights the brief management of CRAT.

CT Pulmonary Angiogram Clinical Pretest Probability Tool: Impact on Emergency Department Utilization

ObjectiveCurrently, computed tomographic pulmonary angiogram (CTPA) for evaluating acute pulmonary embolism (PE) in Emergency Departments (EDs) is overused and with low yields. The goal of this study is to assess the impact of an evidence-based clinical decision support (CDS) tool, aimed at optimizing appropriate use of CTPA for evaluating PE. MethodsThe study was performed at EDs in a large healthcare system and included 9 academic and community hospitals. The primary outcome was the percent difference in utilization (number of CTPA performed/number of ED visits) and secondary outcome was yield (percentage of CTPA positive for acute PE), comparing 12 months before (6/1/2021-5/31/2022) vs. 12 months after (6/1/2022-5/31/2023) a system-wide implementation of the CDS. Univariate and multivariable analyses using logistic regression were performed to assess factors associated with diagnosis of acute PE. Statistical process control (SPC) charts were used to assess monthly trends in utilization and yield. ResultsAmong 931,677 visits to Emergency Departments, 28,101 CTPAs were performed on 24,675 patients. 14,825 CTPAs were performed among 455,038 visits (3.26%) pre-intervention; 13,276 among 476,639 visits (2.79%) post-intervention, a 14.51% relative decrease in CTPA utilization (chi-square, p<0.001). CTPA yield remained unchanged (1371/14825=9.25% pre- vs. 1184/13276=8.92% post-intervention; chi-square, p=0.34). Patients with COVID diagnosis prior to CTPA had higher probability of acute PE. SPC charts demonstrated seasonal variation in utilization (Friedman test, p=0.047). DiscussionImplementing a CDS based on validated decision rules was associated with a significant reduction in CTPA utilization. The change was immediate and sustained for 12 months post-intervention.

Utility of D-dimer in predicting pulmonary embolism in patients with COVID-19 presenting to the emergency department.

While our understanding of coronavirus disease 2019 (COVID-19) has evolved, uncertainty remains regarding utility of previously established pulmonary embolism (PE) screening guidelines in patients with COVID-19. Many studies have investigated the efficacy of D-dimer (DD) screenings for patients with COVID-19 admitted to inpatient services, but few have evaluated patients in the emergency department (ED). The purpose of this study was to investigate utility of DD threshold for PE screening in patients with COVID-19 presenting to the ED. This was a retrospective, multicenter cohort including patients presenting to three EDs between March 1, 2020 and February 1, 2021 who tested positive for COVID-19 during ED visit or in 60 days prior to presentation and had DD ordered in ED. Patients were grouped by those who underwent computed tomography pulmonary angiogram (CTPA) to evaluate for PE and those who did not, and descriptive statistics were performed. Those who underwent CTPA were further divided into PE-positive and PE-negative groups. The discriminative ability of DD in predicting PE in patients with COVID-19 was analyzed using the receiver operating characteristic (ROC) curve. A total of 570 patients with COVID-19 were included in the study, of which 107 underwent CTPA to evaluate for PE. History of diabetes, elevated glucose, elevated lactate dehydrogenase, elevated white blood cell count, elevated platelets, elevated respiratory rate, and lower temperature were associated with increased risk for PE. Compared to those without PE, patients with PE were significantly more likely to be hospitalized (100%vs. 82%, p=0.020) and admitted to the ICU (64%vs. 24%, p=0.002). Those with PE had a significantly higher median DD value (21,177ng/mL) compared to PE-negative group (952ng/mL, p<0.001). The ROC curve for DD in predicting PE had an area under the curve of 0.91 (95% confidence interval [0.84, 0.98]). In our study population, the optimal DD threshold for predicting PE was 1815ng/mL (sensitivity 93% and specificity 80%). A conservative threshold of 1089ng/mL could be used with sensitivity 100% and specificity 58%. DD is often elevated in patients with COVID-19, regardless of PE. While the classically used DD cutoff is 500ng/mL, our study demonstrated a threshold of 1089ng/mL safely predicted PE in patients withCOVID-19 .

Single-center outcomes of artificial intelligence in management of pulmonary embolism and pulmonary embolism response team activation.

Multidisciplinary pulmonary embolism response teams (PERTs) have shown that timely triage expedites treatment. The use of artificial intelligence (AI) may help improve pulmonary embolism (PE) management with early CT pulmonary angiogram (CTPA) screening and accelerate PERT coordination. This study aimed to test the clinical validity of an FDA-approved PE AI algorithm. CTPA scan data of 200 patients referred due to automated AI detection of suspected PE were retrospectively reviewed. In our institution, all patients suspected of PE received a CTPA. The AI app was then used to analyze CTPA for the presence of PE and calculate the right-ventricle/left-ventricle (RV/LV) ratio. We compared the AI's output with the radiologists' report. Inclusion criteria included segmental PE with and without RV dysfunction and high-risk PE. The primary endpoint was false positive rate. Secondary end points included clinical outcomes according to the therapy selected, including catheter-directed interventions, systemic thrombolytics, and anticoagulation. Fifty-seven of 200 exams (28.5%) were correctly identified as positive for PE by the algorithm. A total of 143 exams (71.5%) were incorrectly reported as positive. In 8% of cases, PERT was consulted. Four patients (7%) received systemic thrombolytics without any complications. There were six patients (10.5%) who developed high-risk PE and underwent thrombectomy, one of whom died. Among 46 patients with acute PE without right heart strain, 44 (95%) survived. The false positive rate of our AI algorithm was 71.5%, higher than what was reported in the AI's prior clinical validity study (91% sensitivity, 100% specificity). A high rate of discordant AI auto-detection of suspected PE raises concerns about its diagnostic accuracy. This can lead to increased workloads for PERT consultants, alarm/notification fatigue, and automation bias. The AI direct notification process to the PERT team did not improve PERT triage efficacy.

The reversed halo sign with a reticulonodular pattern as unusual findings of pulmonary infarct in acute pulmonary embolism: case report and literature review

BackgroundThe reversed halo, also known as the atoll sign, is a distinct sign with ring-shaped consolidation and central lucency. The reversed halo sign, initially emerging in cryptogenic organizing pneumonia, has been observed in various pulmonary diseases, including pulmonary embolism. The presence of this sign in the subpleural, posterior basal parts of the lower lobes, predominantly when solitary, should raise the possibility of other causes, such as pulmonary infarction.Case presentationWe present a case of acute pulmonary embolism with pulmonary infarction discovered on a CT pulmonary angiogram in our emergency department, where the pulmonary infarction appears as a reverse halo sign with a reticulonodular pattern. The reticular nodular pattern in the halo sign is not typical of a pulmonary infarct presenting with a reverse halo sign.ConclusionsTo our knowledge, this is the first case of pulmonary infarction in acute pulmonary embolism with the appearance of a reverse halo sign with a reticulonodular pattern.

A retrospective analysis of transudative pleural effusion due to fibrosing mediastinitis

BackgroundPleural effusion caused by fibrosing mediastinitis is rarely reported. This study aimed to summarize the clinical manifestations, diagnosis and treatment of transudative pleural effusion due to fibrosing mediastinitis.MethodsMedical records and follow-up data of 7 patients with transudative pleural effusion due to fibrosing mediastinitis in Beijing Chaoyang Hospital between May 2014 and Feb 2018 were retrospectively analyzed.ResultsThese patients included 4 males and 3 females, with an average age of (64 ± 9) years. There were 3 left-sided effusions, 2 right-sided effusions and 2 bilateral effusions. Previous or latent tuberculosis was found in 6 patients. Pulmonary hypertension was indicated by echocardiography in all the 7 patients. Computed tomography pulmonary angiography (CTPA) of all the 7 cases showed increased soft tissue images visible in the mediastinum and bilateral hilus, different degrees of stenosis or occlusion in the pulmonary artery and pulmonary vein. In addition, 4 cases were found of right middle lobe atelectasis with a mediastinal window setting. There was interstitial pulmonary edema on the side of pleural effusion with a lung window setting. All the 7 patients were treated with intermittent drainage of pleural effusion combined with diuretic therapy. Five patients were treated with antituberculosis therapy. Up to now, two patients died of right heart failure and respiratory failure after 2 and 16 months respectively; The remaining 5 patients were still in follow up.ConclusionFibrosing mediastinitis can lead to pulmonary vein stenosis or occlusion, and thus cause transudative pleural effusion, which can be detected by CTPA. Pulmonary hypertension, long time of cough, and a history of tuberculosis are common in these patients. The common therapy is intermittent drainage of pleural effusion combined with diuretic therapy.
