Evaluation of the cross-sectional area of small pulmonary vessels in the diagnosis of chronic obstructive pulmonary disease by quantitative computed tomography: A case-control study.

Patients with chronic obstructive pulmonary disease (COPD) have a reduced cross-sectional area (CSA) of small pulmonary vessels and decreased pulmonary function test (PFT) indexes. This study investigated the value of small pulmonary vessel CSA in diagnosing and evaluating the severity of COPD and its correlation with PFT.This retrospective case-control study included patients with COPD who underwent multi-slice spiral computed tomography (CT) between March 2015 and December 2018. COPD severity was graded. Patients with normal CT results were included as controls. The CSA of small pulmonary vessels at the sub-segmental (5–10 mm2) and sub-sub-segmental (<5 mm2) levels was measured. Receiver operating characteristic (ROC) curves were used to evaluate the effect of CSA for COPD risk prediction. The correlation between CSA% and PFT indexes was evaluated.There were 124 and 106 patients in the COPD and control groups, respectively. The %CSA <5 and %CSA5–10 were smaller in the COPD group than in controls (P < .05). The %CSA <5 in each subgroup stratified by COPD severity was smaller than in controls (P < .05). The % CSA5–10 was significantly smaller in the moderate and severe groups than in controls (P < .05). At 0.655%CSA <5 cut-off, the ROC area under the curve (AUC) was 0.765. For %CSA5–10, a 0.565 cut-off led to an AUC of 0.752. Both %CSA <5 and %CSA5–10 were positively correlated with all PFT indexes (r = 0.180–0.462, all P < .05).CSA was positively correlated with PFT. Analysis of small pulmonary vessel CSA based on CT images contributes to diagnosing and assessing the severity of COPD.

Adverse effect of smoking on cross-sectional area of small pulmonary vessel and arterial stiffness in healthy smokers without COPD.

Because it induces systemic inflammation, smoking is a risk factor of atherosclerosis and pulmonary hypertension. The brachial-ankle pulse wave velocity (baPWV) and cross-sectional area (CSA) of small pulmonary vessels can be useful markers to assess early changes of arterial stiffness and pulmonary vascular alteration in smokers. This study aimed to explore association between the CSA of small pulmonary vessel and arterial stiffness in healthy male smokers. We enrolled 90 male non-smokers and 90 male smokers (age: 51.5±9.7 years and 52.1±7.9 years, respectively). All subjects underwent chest computed tomography (CT), pulmonary function test and baPWV measurement. We evaluated the total CSAs less than 5 mm2 using ImageJ software and divided by the total lung area (%CSA<5). We compared the association between baPWV and %CSA<5 in two groups as well as correlations among the amount of smoking, baPWV and %CSA<5. Multiple linear regression analysis using %CSA<5 as the dependent variable was also performed. The mean baPWV and mean %CSA<5 were significantly different between the smokers and non-smokers. The pack-years was significantly correlated with %CSA<5 (r=-0.631, P<0.001) and baPWV (r=0.534, P<0.001) in smokers. In multiple linear regression analysis, age, pack-years, FEV1 /FVC and baPWV were associated with %CSA<5, regardless of body mass index, blood pressure and heart rate. There is a dose-response relationship between cigarette smoking and the CSA of small pulmonary vessels and arterial stiffness, respectively. Arterial stiffness, age, pack-years and mild airflow impairment are independent predictors of small pulmonary vascular destruction in smokers.

Decrease in Small Pulmonary Vessels on Chest Computed Tomography in Light Smokers Without COPD: An Early Change, but Correlated with Smoking Index.

The aim of this study was to evaluate the relationship between the amount of smoking and the cross-sectional area (CSA) of small pulmonary vessels in light smokers without a diagnosis of chronic obstructive pulmonary disease (COPD). This retrospective study was approved by our institutional review board, which waived the need for informed consent from patients. The study included 34 current smokers without COPD, who were defined as light smokers based on their smoking history (≤25 pack years). The CSA of small pulmonary vessels (<5 mm2 [CSA<5]) was measured on computed tomography (CT) scans, and the percentage of total CSA of the lung (%CSA<5) was calculated. The extent of emphysema was also assessed as the percentage of low attenuation area (%LAA, <-950 Hounsfield units). The correlations of %CSA<5 and %LAA with pack years were determined using the Spearman rank correlation. There was a significant negative correlation between %CSA<5 and pack years, whereas no significant correlation was found between %LAA and pack years. The correlations between pack years and percent predicted forced expiratory volume in 1s (FEV1) and FEV1/forced vital capacity were not significant. The percentage of total CSA of the lung made up of small pulmonary vessels in light smokers without COPD significantly decreases with increasing amount of smoking, in contrast to emphysema measurements. This suggests that small pulmonary vessels might have been injured or might have degenerated because of smoking, and might represent an initial stage in the development of COPD.

Pulmonary Vessel Cross-sectional Area before and after Liver Transplantation: Quantification with Computed Tomography

Pulmonary vascular complications of liver disease have a substantial impact on morbidity and mortality in patients who undergo liver transplant. The effect of liver transplantation on the pulmonary vasculature in patients without pulmonary vascular disease, however, has not been described. This study was undertaken to characterize the regional effect of liver transplant on the cross-sectional area (CSA) of pulmonary vessels. We performed a single-center, retrospective, cohort study of patients who had a liver transplant between 2002 and 2012 and who had chest computed tomography scans within 1 year before and after transplant. Using ImageJ software, we measured the CSA of small pulmonary vessels (0-5 mm(2)) and the total lung CSA to calculate the percent CSA of pulmonary vessels <5 mm (%CSA<5) at the level of the aortic arch, carina, and right inferior pulmonary vein (RIPV). Pretransplant and posttransplant, %CSA<5 were compared, and associations of pretransplant %CSA<5 with clinical parameters were measured. There was a significant decrease in %CSA<5 at the level of the RIPV (0.19% [interquartile range {IQR}, 0.15-0.26] before vs. 0.15% [IQR, 0.12-0.21] after; P = .0003), with a median change of -16.2% (IQR, -39.3 to 3.9) posttransplant. Changes at the level of the aortic arch and carina were not significant. Pretransplant RIPV %CSA<5 was not significantly correlated with severity of liver disease or oxygenation but was inversely correlated with percent change in %CSA<5 (r = -0.39; P = .0039). This is the first study to describe a significant regional change in the pulmonary vessels of patients without known pulmonary vascular disease who undergo liver transplant.

Quantitative CT measurements of small pulmonary vessels in chronic obstructive pulmonary disease: do they change on follow-up scans?

The aims of this study were to perform a longitudinal evaluation of the cross-sectional area (CSA) of small pulmonary vessels and the extent of emphysema measured on computed tomography (CT) scans of patients with chronic obstructive pulmonary disease (COPD), and to correlate the pulmonary vascular measurements with extent of emphysema. The institutional review board approved this retrospective study and waived the need for patients' informed consent. Seventy-four patients with COPD who underwent both initial and follow-up CT scans at an interval of ≥12 months were analysed. The CSA of small pulmonary vessels <5 mm(2) was measured, and the percentage of total CSA of the area of the lung (%CSA<5 ) was calculated. The extent of emphysema was assessed as the percentage of low attenuation area (%LAA, <-950 Hounsfield units). Comparisons between initial and follow-up measurements were performed using the Wilcoxon signed-rank test. The relationship between longitudinal changes in %CSA<5 and %LAA during the follow-up period was assessed using the Spearman rank correlation. The %LAA increased significantly on follow-up CT scans (P<0·0001). The %CSA<5 was slightly decreased on follow-up scans, but the difference was not significant. Although longitudinal change in %LAA was positively correlated with duration of follow-up period (ρ = 0·505, P<0·0001), longitudinal change in %CSA<5 was not. In conclusion, there was a progressive increase in the extent of emphysema over time, but no significant decrease in the CSA of small pulmonary vessels over the same time period.

Quantitative CT Measurement of Cross-sectional Area of Small Pulmonary Vessel in COPD: Correlations with Emphysema and Airflow Limitation

Pulmonary vascular alteration is one of the characteristic features of chronic obstructive pulmonary disease (COPD). Recent studies suggest that vascular alteration is closely related to endothelial dysfunction and may be further influenced by emphysema. However, the relationship between morphological alteration of small pulmonary vessels and the extent of emphysema has not been assessed in vivo. The objectives of this study are: to evaluate the correlation of total cross-sectional area (CSA) of small pulmonary vessels with the extent of emphysema and airflow obstruction using CT scans and to assess the difference of total CSA between COPD phenotypes. We measured CSA less than 5 mm(2) and 5-10 mm(2), and calculated the percentage of the total CSA for the lung area (%CSA < 5, and %CSA5-10, respectively) using CT scans in 191 subjects. The extent of emphysema (%LAA-950) was calculated, and the correlations of %CSA < 5 and %CSA5-10 with %LAA-950 and results of pulmonary function tests (PFTs) were evaluated. The differences in %CSA between COPD phenotypes were also assessed. The %CSA < 5 had significant negative correlations with %LAA-950 (r = -0.83, P < .0001). There was a weak but statistically significant correlation of %CSA < 5 with forced expiratory volume in 1 second (FEV1)% predicted (r = 0.29, P < .0001) and FEV1/forced vital capacity (r = 0.45, P < .0001). A %CSA 5-10 had weak correlations with %LAA-950 and results of PFTs. %CSA < 5 was significantly higher in bronchitis phenotype than in the emphysema phenotype (P < .0001). Total CSA of small pulmonary vessels at sub-subsegmental levels strongly correlates with the extent of emphysema (%LAA-950) and reflects differences between COPD phenotypes.
