Trajectory of cognitive performance among virologically suppressed aging people living with HIV

AbstractBackgroundDespite the success of combination antiretroviral therapy (cART), cognitive impairment remains a problem for people living with HIV (PLWH), including those who are virologically suppressed. The presentation and course of cognitive impairment have changed significantly since the introduction of cART. We aimed to determine the trajectory of cognitive performance in a cohort of virologically suppressed aging PLWH.MethodThai PLWH aged ≥50 years followed in the HIV‐NAT 006 cohort were enrolled during 2015‐2017 to assess cognitive performance using the Thai‐validated Montreal Cognitive Assessment (MoCA), a cutoff of ≥25 as normal. The follow‐up MoCA was consecutively performed since 2021. Participants who had plasma HIV RNA of ≤50 copies/mL and completed the follow‐up MoCA were included in the analysis. Changes in the MoCA scores were calculated between the two visits.ResultAmong 181 eligible participants (69 [38.1%] female; mean [SD] age, 57.0 [5.7] years; 46 [25.4%] had education ≤6 years; mean duration of HIV infection, 18.0 [4.2] years), 104 (57.5%) had cognitive impairment at enrollment. There were statistically significantly higher proportion of female sex (45.2% vs 28.6%, p = 0.023) and participants who had education >6 years (85.6% vs 59.4%, p<0.001) among cognitively impaired (CI) than cognitively unimpaired (CU) participants. After mean 5.6 (0.4) years of follow‐up period, the mean difference of the MoCA score was ‐0.72 (95%CI ‐1.29 to ‐0.15, p = 0.01, d = 0.19) and there were statistically significant differences in the MoCA score changes between groups categorized by baseline cognitive performance (mean difference 3.25, 95%CI 2.09 to 4.41, p<0.001, after adjusting for sex assigned at birth, age, education levels, smoking status, depression, hypertension, and diabetes mellitus).The CU group showed a statistically significant decrease in the MoCA score (mean difference ‐2.58, 95%CI ‐3.26 to ‐1.90, p<0.001, d = 0.86), and 43/77 (55.8%) of CU participants developed cognitive impairment at follow‐up, while the CI group had comparable MoCA scores between the two visits (mean difference 0.65, p = 0.09) (figure).ConclusionCU virologically suppressed PLWH demonstrated a statistically significant reduction in cognitive performance compared to their CI counterparts over time. Further studies on the underlying pathological mechanisms and implementation of cognitive care services among virologically suppressed PLWH are warranted.

Cognitive functioning in schizophrenia: a lifespan perspective

Cognitive disorders are now well known in schizophrenia, but their evolution over time and particularly during aging remains poorly described. Current evidence indicates that cognitive deficits including attention, working memory, verbal learning and memory, and executive functions are present in 80% of individuals with schizophrenia, which, on average, are around two standard deviations below that in healthy controls. Cognitive impairments are important predictors of functioning in daily life, including quality of life, inability to live independently and unemployment. From a lifespan perspective, they are evident in infancy (18months) and continually increase between infancy and adulthood (20 years) in full-scale IQ and most of cognitive domains, accelerating during the prodromal phase. Cognitive deficits are significant predictor of the psychotic transition, and they stabilize after the first psychotic episode until the age of fifty suggesting that cognitive deficits are mainly established before the prodromal phases of psychosis. Time course of cognitive impairments in elderly patients with schizophrenia appears to be more heterogeneous. While the data suggest that they are stable with advancing age, a subgroup of institutionalized older patients stands out for a significant cognitive decline, without it being possible to determine the causal direction of this association. The twofold increased risk of developing neurodegenerative dementia in this population, combined with institutionalization, could partly explain this cognitive decline in some patients. Long-term longitudinal studies in older adults with schizophrenia are clearly needed, particularly in France. Protective factors such as recovery-focused and psychosocial approaches need to be studied in this population, which has a 15-year shorter life expectancy than the general population.

A ten-year longitudinal repeated assessment study of cognitive improvement in patients with first-episode schizophrenia and healthy controls: The Oslo Schizophrenia Recovery (OSR) study.

The mapping of cognitive trajectories after a first episode of schizophrenia has been the aim in several studies, but the longitudinal course of cognitive impairments remains an important question. Due to methodological limitations, it has been challenging to pinpoint specific periods of improvement or stability in cognitive functioning over time. The objective of this study is to further clarify the longitudinal course of cognitive change after a first episode of schizophrenia through frequent repeated measurement. A total of 56 persons participated in the study (28 first episode patients and 28 healthy pairwise matched controls) with 79% of patients retained at the 10-year follow-up. The Oslo Schizophrenia Recovery study has a repeated measurement design and includes data from nine cognitive assessments over 10years. Cognition was assessed with the MATRICS Consensus Cognitive Battery, which is well suited for repeated measurements. Data were analyzed with linear multilevel models. The results challenge some of the views about the course of cognitive impairment in first-episode schizophrenia patients. Using quadratic time effects in our analyses and balancing the patient group with regards to the most relevant confounding demographic variables such as age, gender, and education, we showed that cognitive deficits change and improve more than in healthy individuals until year 6, when both groups stabilize. The patient group improved on some of the most important cognitive domains associated with functional outcome with 63.5% full recovery at 10-year follow-up.

Mesenchymal Stem Cell-Derived Exosomes: A Novel Approach to Diabetes-Associated Cognitive Impairment.

The progression of diabetes frequently results in a myriad of neurological disorders, including ischemic stroke, depression, blood-brain barrier impairment, and cognitive dysfunction. Notably, diabetes-associated cognitive impairment, a prevalent comorbidity during the course of diabetes, progressively affects patients' cognitive abilities and may reciprocally influence diabetes management, thereby severely impacting patients' quality of life. Extracellular vesicles, particularly nanoscale exosomes, have garnered considerable attention in recent years. These exosomes carry and transfer various functional molecules, such as proteins, lipids, and diverse non-coding RNAs, serving as novel regulators and communicators in intercellular interactions. Of particular interest, mesenchymal stem cell-derived exosomes (MSC-Exos) have been reported to traverse the blood-brain barrier and ameliorate intracerebral pathologies. This review elucidates the role of MSC-Exos in diabetes-related cognitive impairment, with a focus on their applications as biomarkers, modulation of neuronal regeneration and synaptic plasticity, anti-inflammatory properties, antioxidative effects, and their involvement in regulating the functionality of β-amyloid proteins during the course of cognitive impairment. The immense therapeutic potential of MSC-Exos in the treatment of diabetes-induced cognitive dysfunction is emphasized.

Evolution of Cognitive Impairments in Treatment-Resistant Depression: Results from the Longitudinal French Centers of Expertise for Treatment-Resistant Depression (FACE-DR) Cohort.

Previous studies set out profound cognitive impairments in subjects with treatment-resistant depression (TRD). However, little is known about the course of such alterations depending on levels of improvement in those patients followed longitudinally. The main objective of this study was to describe the course of cognitive impairments in responder versus non-responder TRD patients at one-year follow-up. The second aim was to evaluate the predictive aspect of cognitive impairments to treatment resistance in patients suffering from TRD. We included 131 patients from a longitudinal cohort (FACE-DR) of the French Network of Expert TRD Centers. They undertook comprehensive sociodemographic, clinical, global functioning, and neuropsychological testing (TMT, Baddeley task, verbal fluencies, WAIS-4 subtests, D2 and RLRI-16) at baseline (V0) and one-year follow-up (V1). Most patients (n = 83; 63.36%) did not respond (47 women, 49.47 ± 12.64 years old), while one-third of patients responded (n = 48, 30 women, 54.06 ± 12.03 years old). We compared the cognitive performances of participants to average theoretical performances in the general population. In addition, we compared the cognitive performances of patients between V1 and V0 and responder versus non-responder patients at V1. We observed cognitive impairments during the episode and after a therapeutic response. Overall, each of them tended to show an increase in their cognitive scores. Improvement was more prominent in responders at V1 compared to their non-responder counterparts. They experienced a more marked improvement in code, digit span, arithmetic, similarities, and D2 tasks. Patients suffering from TRD have significant cognitive impairments that persist but alleviate after therapeutic response. Cognitive remediation should be proposed after therapeutic response to improve efficiency and increase the daily functioning.

PSYCHOGENIC FACTORS AS A CAUSE OF DEPRESSIVE DISORDERS IN THE ELDERLY

Individuals with particular qualities or characteristics are predisposed to develop cognitive impairment non dementia. The mainaim to study socio-demographic factors and personality traits, that contribute tothe development of depressive disorders in elderly patients with cognitive deficits. The purpose of the article was to show that it is important to investigate the socio-psychological factors that can worsen the course of cognitive impairment, which already comes with age.During the research an observational study involving 111 patients from Kazakhstan with cognitive deficits was conducted: 59 patients, whose depressive disorders had been identified by clinical and psychopathological research using the Hamilton Depression Rating Scale (HRSD) and 52 patients for whom psychogenic depressive disorders had not been defined. The recruitment of the study participants was carried out in the state nursing home for the elderly and disabled. The study included male and female individuals aged 60-74 years with different national and ethnic backgrounds, with mild to moderate cognitive impairment, who wished to participate in the study and signed an informed consent to participate in the study.This study uncovered the primary factors that increase the chances of individuals with cognitive impairments developing a depressive disorder. These factors include personal attributes of the patients, such as their sex, whether they are married or not, and the quality of their relationship with their family members. In addition, personality traits from before their cognitive decline, such as a tendency for isolation, insecurity, and passivity, were also significant.This study could have an exceptional value for medicine and psychological workers on the whole and in Republic of Kazakhstan in particular to prevent depressive diseases were mentioned.

Assessing frailty at the centers for dementia and cognitive decline in Italy: potential implications for improving care of older people living with dementia

IntroductionFrailty is strongly associated with the clinical course of cognitive impairment and dementia, thus arguing for the need of its assessment in individuals affected by cognitive deficits. This study aimed to retrospectively evaluate frailty in patients aged 65 years and older referred to two Centers for Cognitive Decline and Dementia (CCDDs).MethodsA total of 1256 patients consecutively referred for a first visit to two CCDDs in Lombardy (Italy) between January 2021 to July 2022 were included. All patients were evaluated by an expert physician in diagnosis and care of dementia according to a standardized clinical protocol. Frailty was assessed using a 24-items Frailty Index (FI) based on routinely collected health records, excluding cognitive decline or dementia, and categorized as mild, moderate, and severe.ResultsOverall, 40% of patients were affected by mild frailty and 25% of the sample has moderate to severe frailty. The prevalence and severity of frailty increased with decreasing Mini Mental State Examination (MMSE) score and advancing age. Frailty was also detected in 60% of patients with mild cognitive impairment.ConclusionFrailty is common in patients referring to CCDDs for cognitive deficits. Its systematic assessment using a FI generated with readily available medical information could help develop appropriate models of assistance and guide personalization of care.

Assessing cognitive changes in multiple sclerosis: criteria for a reliable decision

ABSTRACT Introduction Quantifying a significant cognitive change on a neuropsychological battery is essential to assess patients’ decline or recovery and offer appropriate care. The reliability of change indices is particularly important in multiple sclerosis (MS), as the course of cognitive impairment is quite unpredictable, due at least in part to substantial interindividual variability. The main objective of this study was to compare six different methods for assessing cognitive change in an MS sample: the SD method, two reliable change indices, two standardized regression-based methods (SRB), and the generalized regression-based method (GSRB). Method One hundred and twenty-three patients with clinically definite MS and 89 healthy controls underwent a battery of standardized neuropsychological tests assessing cognitive functions that are frequently affected in this disease (i.e., verbal episodic memory, working memory, processing speed and verbal fluency). Results We observed fairly similar proportions of improvement, decline or stability in the control group whatever the method. By contrast, in the MS sample, regression-based methods with one predictor (i.e., score at T1) and four predictors (i.e., score at T1 and demographic factors: age, sex, education level) detected a significant worsening more often than the reliable change indices while the GSRB method was more consistent with the RCI methods in tasks associated with ceiling effects. Conclusions The interpretation of a patient’s cognitive changes depends on which method is used. The (G)SRB methods appear to be relevant indicators for assessing cognitive change in MS. The addition of demographic factors does not seem to play an important role in the prediction of significant worsening in the MS sample, regardless of cognitive domain. For clinicians, an easy-to-use free shiny app is provided.

Examining the association between exposome score for schizophrenia and cognition in schizophrenia, siblings, and healthy controls: Results from the EUGEI study

BackgroundPeople with schizophrenia spectrum disorders (SSD) frequently present cognitive impairments. Here, we investigated whether the exposome score for schizophrenia (ES-SCZ) - a cumulative environmental exposure score - was associated with impairments of neurocognition, social cognition, and perception in patients with SSD, their unaffected siblings, and healthy controls. MethodsThis cross-sectional sample consisted of 1200 patients, 1371 siblings, and 1564 healthy controls. Neurocognition, social cognition, and perception were assesed using a short version of the Wechsler Adult Intelligence Scale–Third Edition (WAIS-III), the Degraded Facial Affect Recognition Task (DFAR), and the Benton Facial Recognition Test (BFR), respectively. Regression models were used to analyze the association between ES-SCZ and cognitive domains in each group. ResultsThere were no statistically significant associations between ES-SCZ and cognitive domains in SSD. ES-SCZ was negatively associated with T-score of cognition in siblings (B=-0.40, 95% CI -0.76 to -0.03) and healthy controls (B=-0.63, 95% CI -1.06 to -0.21). Additionally, ES-SCZ was positively associated with DFAR-total in siblings (B=0.83, 95% CI 0.26 to 1.40). Sensitivity analyses excluding cannabis use history from ES-SCZ largely confirmed the main findings. ConclusionsLongitudinal cohorts may elucidate how environmental exposures influence the onset and course of cognitive impairments in trans-syndromic psychosis spectrum.

Longitudinal course of cognitive impairment in patients with atopic dermatitis.

Atopic dermatitis (AD) is a common inflammatory skin disease. Cognitive dysfunction was recently demonstrated to be increased in adults and children with AD. However, little is known about the longitudinal course of cognitive impairment in AD and its relationship with pruritus. To investigate the longitudinal course and predictors of cognitive impairment in AD a prospective dermatology practice-based study was performed using questionnaires and evaluation by a dermatologist (n = 210). Patients with ≥ 2 visits were included (mean follow-up time: 318days). Cognitive function was assessed using the Patient-Reported Outcomes Measurement Information System (PROMIS) Cognitive Function 8-item Short-Form. At baseline, 20.85% of patients had PROMIS T scores ≤ 45, indicating cognitive impairment (CI). Among patients with CI at baseline, 34.09% had persistent CI, 47.72% had a fluctuating course, and 18.18% had sustained improvement of cognitive function. In repeated-measures regression models, cognitive function scores declined overtime in patients with worse AD severity [SCORing Atopic Dermatitis (SCORAD): p = 0.01, Atopic Dermatitis Severity Index: p = 0.001], increased itch (p = 0.01), skin pain (p = 0.001), and sleep disturbance (p = 0.001). In multivariable logistic regression models, persistent CI was associated withfemale gender and depressive symptoms [moderate-to-severe Patient Health Questionnaire-9 score (PHQ9)]. Latent class analysis identified two classes of cognitive dysfunction: normal cognition (77.23%), moderate dysfunction (16.21%) and severe impairment (6.55%). Black/African-American race (p = 0.02), moderate-to-severe SCORAD (p = 0.03), dermatology life quality index (p < 0.0001), PHQ9 (p < 0.0001), itch (p = 0.02) and skinpain (p < 0.0001) were more likely to experience moderate dysfunction or severe cognitive impairment. AD is associated with a heterogeneous longitudinal course of cognitive function in adults, with some patients experiencing persistent CI over time.

Does essential tremor increase risk of cognitive impairment and dementia? Yes.

Essential Tremor (ET), by definition, is a disorder of movement. Yet over the years, epidemiologic, clinical, pathologic, and neuroimaging studies have converged to reveal a cognitive side of ET. The cognitive symptoms in ET are heterogeneous and are likely to reflect heterogeneous underlying mechanisms. In this chapter, we review and synthesize a diverse set of studies from both population-based settings to cohorts with more detailed investigations into cognition to consider the various mechanisms by which cognitive symptoms may emerge in a subset of individuals with ET. As part of our analysis, we consider questions surrounding ET diagnosis and the possibility of comorbid disease as potential factors that, upon closer examination, appear to strengthen the argument in favor of ET as a risk factor for dementia. Importantly, we also consider the clinical relevance of cognitive impairment in ET. While ET is not universally characterized by significant cognitive deficits, the data from epidemiological, cognitive, neuroimaging, and postmortem neuropathologic studies converge to reveal an increased risk for cognitive impairment and dementia among individuals with ET. We conclude by offering directions for future research, and a neurocognitive framework with which to consider existing findings and to use in the design of novel studies dedicated to clarifying the basis, nature, and course of cognitive impairments in ET.

Clinical and Dynamic Features and Prognostic Value of Non-Cognitive Psychopathological Symptoms in Mild Cognitive Impairment (MCI)

Background: high prevalence of pre-dementia cognitive disorders (in particular mild cognitive impairment, MCI) in older people, unfavorable predictive value of MCI with a high risk of conversion to dementia, lack of a unified view of facultative, noncognitive psychopathological symptoms (NPS) in its syndromic structure, their clinical heterogeneity, close relationships with mild cognitive impairment syndrome is actualized by the analysis of the mutual influence of the components of MCI syndromes.The aim: to investigate clinical and dynamic features and determine the prognostic value of non-cognitive psychopathological symptoms in MCI, including various psychopathological symptoms (affective, psychotic, behavioral disorders).Patients and methods: a comparative dynamic prospective selective observational study of 264 older patients with MCI was carried out. The main study group included 189 patients with NPS; compared group made up 75 cases with MCI represented only by cognitive impairments. Repeated clinical, psychopathological and psychometric assessments (MMSE, MoCa, NPI) were performed at 12 and 24 months. Results: differences were revealed in the structure and dynamics of cognitive indicators in patients with dominance of affective, psychotic or behavioral symptoms. Patients with affective symptoms showed low scores on subtests of line drawing, phonemic fluency, and ability to verbal categorization (p &lt; 0.05). In these individuals, with repeated measurements, the indices of delayed reproduction, retention, abstract thinking were significantly reduced in comparison with the results of the rest of the study participants (p &lt; 0.05). Patients with psychotic symptoms showed the lowest indices of delayed reproduction, spatial-visual, speech tests, false reproductions both at the initial assessment and in the dynamics of observation (p &lt; 0.05). But their parameters of counting, attention, working memory, “recognition” are higher in them than in representatives of other psychopathological subgroups (p &lt; 0.05). It was found that mild cognitive impairment without psychopathological inclusions has a lower tendency to transform into dementia over a two-year follow-up period than MCI, in the structure of which non-cognitive psychopathological symptoms are present.Conclusions: non-cognitive psychopathological symptoms of MCI are heterogeneous in terms of clinical and dynamic characteristics. The structure and dynamics of cognitive indicators is different in the affective, psychotic, behavioral subgroups of patients with MCI. Individuals with affective pathology had the highest rate of MCI conversion to dementia. In patients with MCI without neuropsychiatric symptoms, a favorable course of cognitive impairment was more common than in patients with MCI with non-cognitive psychopathological symptoms.

Predictive value of serum neurofilament light chain for persistent cognitive deficits in elderly depressive patients

• Late-life depression is very often associated with cognitive deficits. • Serum NfL is a promising biomarker for the prediction of the course of cognitive impairment in elderly depressive patients. • It facilitates an earlier diagnosis and more targeted therapy for patients with affective and cognitive deficits. Elderly patients with depression often exhibit severe cognitive impairment. Neuropsychological tests and psychiatric exploration may not differentiate between reversible cognitive deficits due to depression and persistent cognitive impairment due to dementia. Neurofilament light chain (NfL) level in the interstitial fluid following axonal damage is known to be reflected in the easily accessible serum and predestinates this parameter for clinical investigations in search for a biomarker of cognitive deficits. Sixteen depressive patients aged ≥ 60 years were recruited. For comparison, nine cognitively healthy individuals and eight patients with dementia were included in the study. At baseline, venous blood draws were performed in all participants and serum NfL measurements were carried-out. NfL was able to predict a consecutive dementia diagnosis after remission of depressive symptoms. Despite the relatively small number of patients included in this study, we found statistically significant changes in serum NfL levels between the groups that speak in favor of this - admittedly unspecific - biomarker. The fact that serum NfL changes could predict a consecutive dementia diagnosis after remission of depressive symptoms is of clinical importance: It facilitates an earlier diagnosis and more targeted therapy for the patients with affective and cognitive deficits.

Cognitive profiles in adult-onset neuronal intranuclear inclusion disease: a case series from the memory clinic.

This study aimed to analyze the characteristics of cognitive impairment in adult-onset neuronal intranuclear inclusion disease (NIID). Seven patients with adult-onset NIID were collected consecutively from the memory clinic of Xuanwu hospital from February to December 2019. These cases were diagnosed with skin biopsy triggered by DWI high-intensity signals in corticomedullary junction on brain MRI. We used a battery of neuropsychological scales to detect the patient's performance in each cognitive domain, and made a detailed analysis on the characteristics of cognitive impairment. All seven cases had cognitive impairment, and four of them had met the criteria for dementia. The scores of Montreal Cognitive Assessment and Frontal Assessment Battery were abnormal in all patients. The executive dysfunction was confirmed by the abnormal scores of Trail Making Test (5/7, 71%) and Clock Drawing Test (4/7, 57%). Bad performance in Auditory Verbal Learning Test (6/7, 86%) demonstrated that the memory was also a very commonly impaired cognitive domain. The low score on the animal fluency (4/7, 57%), Boston Naming Test (3/7, 43%), and Pentagon and Cube Copying Test (4/7, 57%) indicated that the language and visuospatial skills were also impaired. Fazekas scores were significantly correlated to the global cognition, executive and language functions (r = 0.788-0.906, P < 0.05). There is obvious impairment in multiple cognitive domains in adult-onset NIID, and both the executive dysfunction and memory deficit are very common. Leukoencephalopathy may be the main course of cognitive impairment in adult-onset NIID.

Prevalence and Progression of Cognitive Impairment in Atrial Fibrillation Patients after Treatment with Catheter Ablation or Drug Therapy.

Purpose In atrial fibrillation (AF) patients, the effect of catheter ablation or drug therapy on cognition is currently not well investigated. Therefore, we prospectively evaluated AF patients who were either treated 'with drug therapy or underwent catheter ablation for the prevalence and progression of cognitive impairment (CI). Methods Randomized participants of the CABANA trial (catheter ablation versus antiarrhythmic drug therapy for atrial fibrillation) and the CASTLE-AF (catheter ablation versus standard conventional treatment in patients with left ventricular dysfunction and atrial fibrillation) study were assessed twice within 6 months by Montreal Cognitive Assessment (MoCA) and Mini-Mental State Examination (MMSE) in our institution. Results Forty-five patients from both trials were investigated, and twenty-eight patients received catheter ablation, whereas seventeen patients received drug therapy for rhythm or rate control. The mean age of the twenty-one CABANA trial patients (AF group) was 68.8 ± 7.0 years and of the twenty-four CASTLE-AF study patients (AF/HF group) was 66.8 ± 8.1 years, respectively. Mean time from ablation/randomization to the first interview was 16.8 ± 11 months in the AF group and 28.3 ± 18.4 months in the AF/HF group, respectively. All patients investigated were classified as cognitively impaired with mean cutoff scores <24 by MoCA. Overall, we could not detect significant differences in medically treated versus catheter ablation patients within both groups in mean MMSE or MoCA scores between the first and the second interview (p > 0.09). Moreover, patients who received catheter ablation did not show statistically significant differences in the prevalence or progression of cognitive impairment compared to patients who were treated medically, neither within the two groups nor between AF and AF/HF patients (p > 0.05). Conclusions Prevalence of cognitive impairment in AF patients with comorbidities is substantial. However, in this preliminary prospective study, no apparent impact of AF pretreatment on the prevalence and course of cognitive impairment could be observed.

Features of the structure of epileptic syndrome and cognitive disorders in patients with cerebral arteriovenous malformation

To assess the frequency, structure of epileptic syndrome and cognitive impairment in patients with cerebral AVM (PAVM). The main group consisted of 78 PAVM: average age of 36.2. The results of the study:1 subgroup- a right-hemisphere localization - 41(53%); 2 subgroup - a left-hemisphere localization – 37(47%). Of these, 33%frontal lobe; 12% the temporal lobe;17% area central convolutions; 28% the parietal and the occipital lobe;10% - subcortical structures. In all cases evaluated, the frequency of epileptic seizures: frequent 11%; moderate 50%; rare 28%; seizures were not observed in 11% cases. The structure of epileptic syndrome was presented in the following seizures: partial sensorimotor in 29%; vegetative-visceral - 40%; mixed: complex partial; secondary generalized - 31%. Changes in EEG in 14% cases, changes were not observed; moderate diffuse changes at the level of diencephalic structures –23%; paroxysmal activity of malformations with an interest in diencephalic structures of the brain-41%;the formation of a mirror focus –10%, moderate irritative changes of EEG –7%; the presence of the pattern of epileptiform activity-5%. Cognitive impairment of varying severity degree were identified in PAVM: mild cognitive disorders – 21%; mild cognitive impairment – 64%; dementia from mild to severe, severity – 13%. The duration and frequency of the epileptic syndrome of the complex structure aggravate the course of cognitive impairment, thereby reducing quality of life of PAVM. A comprehensive assessment of cognitive disorders, structure and frequency of epileptic syndrome can be a diagnostic criterion of the results of the immediate outcomes of endovascular treatment of AVM.

Cognitive function during exacerbations of chronic obstructive pulmonary disease.

The reported prevalence of cognitive impairment in patients with stable chronic obstructive pulmonary disease (COPD) ranges 36-77%. Few studies report the prevalence of cognitive impairment in acutely unwell COPD patients. To determine the prevalence and time course of cognitive impairment in patients with COPD during and after an admission to hospital with an exacerbation of the disease. Patients admitted to hospital with an exacerbation of COPD between October 2013 and November 2014 were administered the Montreal Cognitive Assessment tool, COPD assessment test and modified Borg dyspnoea scale at three points in time: within 24 h of admission, between 48 and 72 h after admission and 6 weeks post discharge. Twenty-five patients agreed to participate. Four withdrew from the study after the initial evaluation. The mean (range) COPD assessment test score 24 h after admission was 26 (18-37). Cognitive impairment was found in 19/25 (76%) patients at the initial evaluation, 16/21 (76%) patients at the second evaluation. Overall, 22/25 (88%) showed cognitive impairment within 72 h of an exacerbation of COPD. Fourteen out of 21 (66%) patients showed cognitive impairment at the final evaluation. The mean Montreal Cognitive Assessment scores improved from admission (22.6) to the second evaluation (23.3) to the final evaluation 3 (24.4), but this change was not statistically significant. Cognitive impairment is highly prevalent during hospital admissions with an exacerbation of COPD. This impairment does improve with time, but only a minority recover within a normal range. This will affect patients' abilities to understand and remember information given to them in hospital and adhere to medication regimens.

Cognitive impairment in psychotic illness: prevalence, profile of impairment, developmental course, and treatment considerations .

Despite effective pharmacological treatments for psychotic symptoms (eg, hallucinations, delusions), functional outcomes for people with psychotic disorders are often disappointing. Although it is not included in the diagnostic criteria for psychotic disorders, cognitive impairment is one of the strongest determinants of community functioning in this clinical population, and thus it is an important target for intervention. In this review, we discuss the major areas of research regarding impaired cognition in psychotic illness. The specific topics covered include: (i) the prevalence of cognitive impairment in psychotic disorders; (ii) the profile and magnitude of cognitive impairment in psychotic disorders; (iii) the developmental course of cognitive impairment; (iv) the longitudinal stability of cognitive impairment; and (v) treatment approaches to improve cognitive performance in people with psychotic disorders.

Interaction Between Physical Activity and Genes Related to Neurotrophin Signaling in Late-Life Cognitive Performance: The Cache County Study.

Research indicates that lifestyle and genetic factors influence the course of cognitive impairment in aging, but their interactions have not been well-examined. This study examined the relationship between physical activity and genotypes related to brain-derived neurotrophic factor (BDNF) in predicting cognitive performance in a sample of older adults with up to 12 years of follow-up. Physical activity levels (sedentary, light, and moderate/vigorous) were determined for the sample of 3,591 participants (57% female) without dementia. The genotypes examined included BDNF gene single nucleotide polymorphisms (SNPs) (rs6265 and rs56164415) and receptor gene SNPs (NTRK2 rs2289656 and NGFR rs2072446). Cognition was assessed triennially using the Modified Mini-Mental State Exam. Unadjusted linear mixed models indicated that sedentary (β = -5.05) and light (β = -2.41) groups performed worse than moderate-vigorous (p < .001). Addition of interaction effects showed significant differences in rate of decline between activity levels, particularly among males (p = .006). A three-way interaction with sex, NGFR SNP rs2072446, and physical activity suggested that the C/C allele was associated with better cognitive performance among males engaging in light activity only (p = .004). Physical activity and sex, but not BDNF-related SNPs, predicted rate of cognitive decline in older adults, while NGFR rs2072446 may modify main effects.

Aggressive course of cognitive impairment in multiple sclerosis: A new term for the management of patients

Multiple sclerosis (MS) can create different kinds of symptoms by involving different fields of the central nervous system. Cognitive impairment (CI) is one of the symptoms that can be caused by the onset of the disease and provide a change in the patient’s quality of life. Considering the course of the disease, CI usually worsens increasingly, and there is usually a severe CI in patients with end stage of MS. However, progressing CI can be so fast and severe in a few patients that it can disrupt the daily affairs of the individual. This paper plans to define the aggressive course of CIs in MS patients according to the reported cases. This definition explains that, the aggressive course of CI in MS patients happens in less than 5 years from the onset of the disease, and during this time, it causes dementia in the patient.
