The effect of mulligan mobilization technique application in addition to conventional physiotherapy on pain and joint range of motion in people with neck pain

BackgroundTo evaluate the effect of the Mulligan mobilization technique on pain intensity and range of motion in individuals with neck pain. MethodsForty individuals with mechanical neck pain were enrolled in the study. The patients were randomly divided into 2 groups and a total of 10 sessions of treatment were administered to all 2 groups for 2 weeks, 5 days a week. Mulligan mobilization technique, electrophysical agents, active range of motion, and stretching exercises were carried out in the Mulligan group. In contrast, only electrophysical agents and exercises were applied to the conventional physiotherapy group. Range of motion (ROM) of the neck, Visual Analog Scale (VAS), Neck Pain and Disability Scale (NPDS), and Short-Form 36 Health Survey (SF-36) were used for evaluation. ResultsStatistical analyses were done to compare the amounts at the baseline and immediately after treatment. Statistically significant improvements were found in the post-treatment ROM, VAS, NPDS values in both groups (p < 0.05). When the differences were compared, the results of the Mulligan group were significantly better than the conventional physiotherapy group (p < 0.05). There was no significant difference between the groups in terms of SF-36 parameters (p > 0.05). ConclusionsThis study showed that the Mulligan mobilization technique plus conventional physiotherapy is more effective than conventional physiotherapy in increasing joint range of motion, reducing pain, and reducing neck disability. The effect of conventional physiotherapy and median nerve radiofrequency therapy on kinesiophobia in lumbar facet syndrome

Aim: This study was carried out to investigate the effect of conventional physiotherapy (CPT) and median nerve radiofrequency therapy (MNRFT) on kinesiophobia in patients with lumbar facet syndrome (LFS).&#x0D; Methods: The sample of this study consisted of 60 patients over 18 with facet joint pain persisting for at least 3 months and unresponsive to medical treatment were included in the study. The patients were divided into CPT and MNRFT groups of 30 patients each according to the treatment they received. The patients were evaluated for pain severity, kinesiophobia, sleep quality, functional status, and depression both before the treatment and one month after the treatment using the Visual Analogue Scale (VAS), Tampa Scale for Kinesiophobia (TKS), Pittsburgh Sleep Quality Index (PSQI), Oswestry Disability Index (ODI) and Beck’s Depression Inventory (BDI), respectively.&#x0D; Results: There was a significant difference between the pre- and post-treatment VAS, PSQI, ODI, and BDE scores in both the CPT and MNRFT groups. Additionally, there was a significant difference between the pre- and post-treatment TKS scores in the RFT group, but not in the CPT group. There was no significant difference between the groups in any of the pre-treatment scores. There was also no significant difference between the groups in the post-treatment scores, except for the TKS scores.&#x0D; Conclusion: The study findings indicated that MNRFT and median nerve blockade reduced kinesiophobia more than CPT.

Effect of post-extubation inspiratory muscle training on diaphragmatic function in mechanically ventilated patients: A randomized controlled trial.

Diaphragmatic dysfunction is a common problem in patients who have been mechanically ventilated. The study aimed to evaluate the effectiveness of inspiratory muscle training (IMT) on diaphragm muscle thickness and function in mechanically ventilated patients. A single-blind trial was conducted. Twenty patients were randomly assigned to either the conventional physiotherapy (CP) group or to the IMT group for 5 days following extubation. The CP group received only CP, while the IMT group received CP in addition to IMT. Ten healthy controls (HCs) underwent IMT. Maximum inspiratory pressure (MIP) and physical function were recorded. Diaphragm excursion (DE), diaphragm thickness at the end of inspiration (Tdi), diaphragm thickness at the end of expiration (Tde), peak contraction velocity (PCV), and peak relaxation velocity (PRV) were evaluated with ultrasonography before and after the intervention. The IMT group and HCs showed significant improvements in DE (p = 0.005; p = 0.005, respectively), PCV (p = 0.028; p = 0.015, respectively) and PRV (p = 0.029; p = 0.020, respectively) after 5 days of IMT. A significant increase in MIP was recorded in all groups after the intervention (CP: p = 0.044; IMT: p = 0.005; HC: p < 0.001). There was a significant improvement in the Medical Research Council (MRC) and the Physical Function in Intensive Care Test (PFIT) scores in both the CP and IMT groups (p < 0.001 and p < 0.001, respectively). Inspiratory muscle training improves diaphragmatic functions, including MIP, diaphragm excursion, PCV, and PRV. We think that IMT applied after extubation may serve as a tool to prevent and facilitate the recovery of diaphragmatic function.

Effect of Closed Kinetic Chain Exercise to Reduce Pain and Increase Functional Ability in Patient with Osteoarthritis Genu

Background: Osteoarthritis genu is a degenerative disease with symptoms of chronic inflammation of the joint cartilage which causes pain, limited movement and function. Closed kinetic chain exercise is an active movement that involves many joints and muscle groups simultaneously. This study aims to analyze the effect of closed kinetic chain exercise on reducing pain and increasing functional ability in osteoarthritis genu.&#x0D; Subjects and Method: Quasi experimental research with a research approach in the form of pretest and posttest with control group design. This study was conducted at RST dr. Soedjono Magelang in May-June 2023. A total of 20 osteoarthritis genu patients were divided into two groups: (1) The intervention group was given closed kinetic chain exercise and (2) The control group was given conventional physiotherapy. Pain level was measured using the Visual Analog Scale (VAS). Functional ability was measured by the Western Ontario and McMaster Osteoarthritis Index (WOMAC). Mean differences between the closed kinetic chain exercise group and the conventional physiotherapy group were analyzed using the independent t-test.&#x0D; Results: Providing closed kinetic chain exercise is effective in reducing the level of silent pain (Effect Size = 1.20; p= 0.014), movement pain (Effect Size= 0.99; p= 0.004), movement pain (Effect Size= 1.37; p= 0.023), and increasing functional ability in osteoarthritis genu patients (Effect Size = 0.10; p= 0.023).&#x0D; Conclusion: Providing closed kinetic chain exercise can reduce pain and increase functional ability in osteoarthritis genu.&#x0D; Keywords: Closed Kinetic Chain Exercise; Painful; Functional Capabilities; Osteoarthritis Genu&#x0D; Correspondence: Siti Fadhilah, Bachelor of Physiotherapy, Universitas Muhammadiyah Surakarta, Indonesia. Efficacy test for intermittent theta burst stimulation during motor rehabilitation in patients post‐stroke in a tertiary centre in Hong Kong

AbstractAimStroke represents a significant cause of long‐term disability in adults. Repetitive transcranial magnetic stimulation (rTMS) is a non‐invasive brain stimulation method known for its potential to enhance motor recovery. Intermittent theta burst stimulation (iTBS), a standardised rTMS protocol initially utilised in patients with depression, has garnered attention in the realm of stroke rehabilitation. This study seeks to establish the efficacy profile of iTBS in motor rehabilitation for patients with subacute or chronic stroke.Patients and methodsThis study is a prospective cohort trial in which eligible patients with stroke were recruited into the iTBS group. Ten sessions of iTBS were conducted, followed by physiotherapy. An equal number of matched controls were recruited into the conventional physiotherapy group. The differences in various scoring systems related to motor performance at 6 months were analysed using independent two‐sample t test.ResultsForty patients were enrolled in the study. At 6 months, iTBS demonstrated a statistically significant improvement compared with conventional physiotherapy across various scoring systems, including motor power grading (47.4% vs 0%, P &lt; .0001), Upper Extremity Fugl–Meyer Assessment (19.8% vs 0.7%, P &lt; .0001), Box and Block Test (25.6% vs 1.7%, P = .007) and Barthel Index (17.8% vs 0.1%, P = .0002). No major adverse events were reported in the iTBS group at 6 months.ConclusioniTBS resulted in significant improvement in motor performance across various domains at 6 months compared with conventional physiotherapy alone. This method proves to be a safe approach for motor rehabilitation.

Effects of structured training on spinal posture and selective motor control in children with unilateral spastic cerebral palsy

BackgroundChildren with Unilateral Spastic Cerebral Palsy (USCP) have an asymmetrical postural pattern. Although functional limitations are less, deteriorations in spinal posture are observed. Research questionWhat is the effect of structured training on spinal posture and selective motor control of upper extremity? MethodsForty five children with USCP were included in the study. Participants were children ages 3–18 with GMFCS levels 1 and 2. Spinal posture and mobility was assessed by Spinal Mouse (SM) and the Spinal Alignment and Range of Motion Measure (SAROMM), and selective motor control of upper extremity was evaluated by the Selective Control of the Upper Extremity Scale (SCUES). Children were divided into two groups: structured training group (STG) (n = 22) and conventional physiotherapy group (CPG) (n = 23). Groups received treatment sessions lasting 45 min, 2 days a week for 8 weeks. Evaluations were made baseline and after treatment. ResultsIn sagittal plane, there was a significant decrease in the degree of thoracic kyphosis after treatment in the STG (p = 0.004). A significant difference was found in total spine angulation (p = 0.015) and mobility from flexion to extension in the STG group. There was a difference in total spine angulation (p = 0.014) in the CPG group, but no difference in spinal mobility. Post-training differences were found in thoracic angulation (p = 0.006) and lateral flexion mobility to the affected side in the STG in thoracic (p = 0.020), lumbar (p = 0.035) and total spine (p = 0.008) in the frontal plane. When SCUES-shoulder, elbow, wrist, total scores changes was significant in CPG (p < 0.001), SCUES-forearm (p = 0.002) and fingers (p = 0.007) changes was significant in STG. SignificanceThis study showed that although children with USCP are more mildly affected, there are adverse effects on their selective motor control and spinal posture. This study reveals the contribution of structured training in terms of selective movement, spinal smoothness and mobility in children.

Investigating the Effect of Addition of Cervical Proprioceptive Training to Conventional Physiotherapy on Visual, Vestibular and Proprioceptive Dependency of Postural Control in Patients with Chronic Non-Specific Neck Pain: A Randomized Controlled Clinical Trial.

While cervical proprioception deficit has been suggested as a contributing factor to clinical consequences of chronic non-specific neck pain (CNSNP), the effect of addressing such impairments on postural control strategies has remained unexplored. The aim of this study was to compare the response of the postural control system to alteration of sensory afferents in CNSNP with asymptomatic individuals. Furthermore, we examined whether proprioceptive training would yield superior outcomes to routine physiotherapy for improvement of postural control, pain and disability. Center of pressure (CoP) variables of sixty CNSNP patients equally distributed in any of the proprioception-specific or conventional physiotherapy groups and 30 asymptomatic participants were evaluated under four standing conditions:1) normal, 2) foam, 3) cervical extension/eyes open and 4) cervical extension/eyes closed standing. CoP anteroposterior range and anteroposterior and mediolateral velocity in patients were significantly higher than the control group under condition 2 (P<0.05). Patients also demonstrated lower anteroposterior lyapunov exponent under conditions 2 and 4 (P<0.05). Both interventions significantly decreased anteroposterior range and anteroposterior velocity(P<0.05). Anteroposterior lyapunov exponent also increased under condition 2 (P<0.05).. After the interventions, CoP anteroposterior range and anteroposterior velocity were significantly lower in the proprioceptive exercise group than the conventional physiotherapy group (P<0.05). Anteroposterior lyapunov exponent was also significantly higher in the proprioceptive exercise group (P<0.05).This while there was no significant difference between these patients and control group participants in any of the CoP variables after intervention. Our results rejected the hypothesis that impaired neck proprioception in the presence of CNSNP is compensated by overweighting other sources of sensory afferent information. The findings also revealed that while proprioceptive exercises successfully returned postural strategies of CNSNP patients to those in asymptomatic participants, they do not add to clinical recovery of these patients.

Comparative Analysis of the Impact of Virtual Reality Training and Conventional Physiotherapy on Balance and Gait in Older Adults with Parkinson's Disease

Background: Parkinson's disease (PD) is a neurodegenerative condition predominantly manifesting in motor impairments, notably in balance and gait. Recent advancements have highlighted Virtual Reality (VR) training as a potent alternative or adjunct to conventional physiotherapy (CPT) for the rehabilitation of these motor disturbances in PD patients. Objective: This study aimed to evaluate and compare the efficacy of VR training and CPT in ameliorating balance and gait issues in older adults diagnosed with PD. Methods: In this randomized controlled trial, 56 patients with PD were assigned either to VR training or to CPT. The VR training entailed immersive and interactive exercises specifically designed for balance and gait enhancement using VR technology. Conversely, CPT consisted of traditional exercises targeting similar motor skills. Standardized assessments for balance and gait were administered both prior to and following the intervention to all participants. Results: The intervention demonstrated that VR training led to more pronounced improvements in balance and gait parameters as compared to CPT. Notably, the Timed Up and Go test (TUG) exhibited an improvement of 4.2 seconds in the VR group versus 3.6 seconds in the CPT group, a difference statistically significant (p&lt;0.05). Similarly, the Berg Balance Scale (BBS) scores increased by 12.4 points in the VR group and 10.8 points in the CPT group, again showing a significant advantage for VR (p&lt;0.05). The Mini-BESTest scores improved by 2.6 points in the VR group compared to 2.1 points in the CPT group, reinforcing the superiority of VR (p&lt;0.05). Conclusion: The study conclusively finds that VR training surpasses CPT in enhancing balance and gait in older adults with PD. Offering an immersive, engaging, and tailored therapeutic environment, VR training emerges as a potent modality for augmenting rehabilitation outcomes in this demographic.

Effects of Vojta Therapy on the Motor Function of Children with Neuromotor Disorders: Study Protocol for a Randomized Controlled Trial

Background: Infantile cerebral palsy is a neurological pathology that causes great morbidity, mortality, and disability in people who suffer from it, mainly affecting motor development. There are a multitude of non-pharmacological methods or therapies for its treatment. One of the main methods is Vojta therapy. This methodology acts on ontogenetic postural function and automatic postural control. Objective: This study aims to demonstrate that there are changes in the motor development of children with cerebral palsy with the application of Vojta therapy. Methods and analysis: This is a randomized controlled trial on the effectiveness of two neurorehabilitation techniques in patients with cerebral palsy conducted at the Physical Medicine and Rehabilitation Service of the Teresa Herrera Maternal and Child Hospital of the A Coruña and Cee Health Area. The study will be conducted from January 2023 to December 2024. There will be two groups: the Vojta therapy group (n = 30) and the conventional physiotherapy group (n = 30). The measurement variables will be gross motor function as measured by the Gross Motor Function Measure (GMFM) and Infant Motor Profile (IMP) scales. Ethics and dissemination: The study was approved by the Research Ethics Committee of the University of Murcia (1823/2018) and Comité de Ética de la Investigación de A Coruña-Ferrol (2022/099). EFFECTIVENESS OF AXIAL RELEASE AND SUSTAINED MOBILIZATION ON SHOULDER ELEVATION IN INDIVIDUALS WITH ADHESIVE CAPSULITIS – A PROSPECTIVE AND COMPARATIVE STUDY

Objective: The aim of the study was to investigate the comparative effectiveness of axial release and sustained mobilization on shoulder elevation in individuals with adhesive capsulitis. Methods: The present study recruited total 30 participants diagnosed as adhesive capsulitis as per the inclusion and exclusion criteria. They were allocated into Group A and Group B according to convenient sampling. Participants in Group A were treated with conventional physiotherapy, axial release, and sustained mobilization technique and Group B received Maitland mobilization with conventional physiotherapy. The interventions were given for 4 weeks, 3 sessions/week for 30 min. The outcome measures used in the study were numerical pain rating scale, shoulder pain and disability index, and universal goniometer which was assessed before the intervention immediately after the first session and at 2nd week and on completion of 4th week. Results: When the data statically analyzed intragroup group comparisons found to be significantly reduced in terms of pain and shoulder pain and disability index and improvement in Shoulder range of motion (p&lt;0.05). Intergroup comparisons also showed significant difference in both the groups, but the results were more significant in Group A (p&lt;0.05). Conclusion: Both interventions in the form of articular and soft-tissue mobilization should be incorporated in the management of adhesive capsulitis.

Bolstering Cognitive and Locomotor Function in Post-Stroke Dementia Using Human-Robotic Interactive Gait Training.

Studies have reported inconclusive results regarding the effectiveness and clinical indications of the exclusive use of human-robotic interactive gait training (HIT) in patients with post-stroke dementia (PSD). This study aimed to compare the effects of human-robotic interactive gait training (HIT) and conventional physiotherapy (CPT) on cognitive and sensorimotor functions, trunk balance and coordination, dynamic and static balance, and activities related to daily living performance in patients with PSD. Forty-eight patients with PSD who received 60-minute therapy sessions three times per week for 6 weeks were assigned to either the CPT (n = 25) or HIT (n = 23) group. The clinical outcomes included the scores of the mini-mental state examination (MMSE), Fugl-Meyer assessment (FMA), trunk impairment scale (TIS), Berg balance scale (BBS), and modified Barthel index (MBI). Friedman tests were conducted at p < 0.05. The Friedman tests showed that HIT had superior effects to CPT in relation to MMSE, FMA, and TIS (p < 0.05), but not in relation to BBS and MBI (p > 0.05). Our results provide promising clinical evidence that HIT significantly improves cognitive and sensorimotor recovery functions, as well as trunk balance and coordination, in patients with PSD who cannot concurrently perform dual cognitive-locomotor tasks.

A comparison between hypertonic dextrose prolotherapy and conventional physiotherapy in patients with knee osteoarthritis.

The aim of the present study was to compare the efficacy of hypertonic dextrose prolotherapy (HDP) with conventional physiotherapy (CPT) in improving symptoms in females with knee osteoarthritis (OA). The present study included 60 patients with a diagnosis of knee OA. The patients were randomly assigned to the HDP (n=30) and CPT (n=30) groups. The patients in the HDP group were treated with a dextrose injection into the knee joint (25% dextrose) and around the knee (15% dextrose) in two sessions for 1 month, while those in the CPT group received a hot pack, transcutaneous electrical nerve stimulation and therapeutic ultrasound in five sessions a week for 4 weeks. Prior to commencing the treatment, and at 1 and 3 months post-treatment, all the patients were evaluated using the visual analog scale (VAS), Western Ontario and McMaster Osteoarthritis Index (WOMAC), the goniometric measurement of active knee range of motion (ROM), a 50-m walking test and isokinetic knee muscle strength measurements. There were no statistically significant differences between the two groups as regards the demographic characteristics at pre-treatment (P>0.05). However, at 1 and 3 months post-treatment, the scores of all the outcome parameters were significantly improved in the HDP group compared with the CPT group (P<0.05 for all). In both groups, a significant improvement was observed in the VAS scores, WOMAC total values and ROM following the treatments, with the greatest improvement observed in the HDP group (P<0.001). The isokinetic quadriceps peak torque measurements were increased in both groups following treatment. All the scores exhibited a statistically significant improvement in the HDP group at both 1 and 3 months post-treatment. On the whole, the results of the present study demonstrate that both HDP and CPT are effective treatment modalities to relieve pain, and increase functionality and strength in patients with knee OA. However, greater improvements in pain and functionality can be achieved with prolotherapy.

Randomized controlled trial to compare conventional physiotherapy with task-oriented exercises after total hip replacement.

Different interventions are used during rehabilitation after hip replacement surgery, but it is unclear if task-oriented exercise is more effective than conventional physiotherapy. This study compared the effectiveness of conventional physiotherapy (CPT) programme with task-oriented exercise (TOE) on hip pain, function, static and dynamic balance during the early rehabilitation stage after total hip replacement. This randomized controlled parallel-group trial was executed in an inpatient physical medicine and rehabilitation centre. The study included 40 patients who were randomly classified into CPT and TOE groups (n= 20 in each). Hip pain (assessed by visual analogue scale), function in the operated leg (assessed by modified Harris Hip Score, mHHS), static balance (assessed by Abili Balance analyser system) and dynamic balance (assessed by Berg Balance Scale) were evaluated at baseline and after 18 days of rehabilitation. Cohen's effect size (d) was calculated. Greater pain reduction (p< 0.05; d= 0.08) was observed after TOE (1.6 ± 0.68 scores) compared with the CPT programme (2.2 ± 0.83 scores). Hip function assessed by the modified mHHS improved more (p< 0.05; d= 0.30) in the TOE group (73.45 ± 6.23 scores) than in the CPT group (54.90 ± 6.28 scores). Static balance improved significantly in both groups after the interventions but did not differ significantly between the groups. The improvement in dynamic balance was significantly greater (p< 0.05; d= 0.45) after TOE (50.55 ± 1.57 scores) than after CPT (38.55 ± 3.43 scores). Both interventions reduced pain and improved function of the operated leg and static and dynamic balance. The effect on hip function was superior for TOE compared with CPT.

Effects of a Novel Proprioceptive Rehabilitation Device on Shoulder Joint Position Sense, Pain and Function.

Background and Objectives: Shoulder disorders are associated with pain, restricted range of motion and muscular strength, moderate disability and diminished proprioception. This study aimed to compare the effectiveness of an innovative technology-supported and a classical therapist-based proprioceptive training program in addition to conventional physiotherapy, on joint position sense (JPS), pain and function, in individuals with different musculoskeletal shoulder disorders, such as rotator cuff tear, subacromial impingement syndrome and superior labrum anterior and posterior tear. The innovative element of the proprioceptive training programme consists of the use of the Kinesimeter, a device created for both training and assessing shoulder JPS. Materials and Methods: The shoulder JPS test and the DASH outcome questionnaire were applied to fifty-five individuals (28 females, 27 males, mean age 56.31 ± 6.75), divided into three groups: 17 in the conventional physiotherapy group (control group); 19 in the conventional physiotherapy + classical proprioceptive training program group (CPT group); and 19 in the conventional physiotherapy + innovative proprioceptive training program group (KPT group). Assessments were performed before and after a four-week rehabilitation program, with five physiotherapy sessions per week. Results: When baseline and post-intervention results were compared, the value of the shoulder JPS and DASH outcome questionnaire improved significantly for the KPT and CPT groups (all p < 0.001). Both KPT and CPT groups showed statistically significant improvements in JPS, pain and function, compared to the control group which received no proprioceptive training (all p < 0.05). However, the KPT group showed no significant benefits compared to the CPT group. Conclusions: Our findings indicate that using the Kinesimeter device as a novel, innovative proprioceptive training tool has similar effects as the classical proprioceptive training programs among individuals with different non-operated musculoskeletal shoulder disorders such as: rotator cuff tear, subacromial impingement syndrome, and superior labrum anterior and posterior tear.

Effect of virtual reality and conventional physiotherapy on rehabilitation of distal radius fracture

Rehabilitation sometimes may be an uncomfortable and tiring method that incorporates multiple activities and sets of exercises which patients require to perform to achieve good prognosis, for a significant period of time. Unfortunately, most of times the patients frequently become not willing, aggravated, and need inspiration and feel exhausted which makes recovery from the condition will less proficient. Use of the Virtual Reality have been broadly executing into rehabilitation program intends to make the process of recovery more energizing, invigorating and engaging for patients. Within this protocol, we propose to utilization of head wearing gadget name Oculus Quest for patient's wrist recovery after Colle’s fracture. After receiving Institutional Ethical committee approval, we will include 40 Colle’s fracture patients, and randomly divide them in two groups. Further group A will receive conventional physiotherapy treatment and group B will receive treatment from Virtual reality as well as conventional treatment for 4 weeks. Pre and post treatment assessment for Pain, Range of motion and functional parameters will be evaluated. The primary aim study is to check the impact of virtual reality and conventional physiotherapy after Colle’s Fracture. This exploration will help in distinguishing quick and longterm impacts of Virtual reality in DRF patients. The investigation results would assist in forming the newly planned strategies with better prognosis in rehabilitation of patients with Colle’s fracture.

A Comparative Study to Check the Effectiveness of Task Oriented Activity on Walking Speed in Sub-Acute Stroke Patients

Stroke which is also known as cerebrovascular accident (CVA) is a leading cause of impairment and disability in the world. There are many factors that affect the functional independence in post stroke people in which gait impairment plays a significant role. The purpose of stroke rehabilitation is to help patients to achieve as high level of functional independence as possible within their particular impairments. It is found that Task oriented activity has been shown to be an effective intervention to improve mobility. However, little is known about the impact of this intervention on the walking speed. The objective of study is to determine the combined effects of task oriented activity with conventional physiotherapy over conventional physiotherapy alone on walking speed in subjects with sub-acute stroke. This was a comparative study where pre and post design were used. Thirty (30) subjects with sub-acute stroke were taken considering the selection criteria; where Experimental group (Group-A) received task oriented activities and conventional physiotherapy and Control group (Group-B) received Conventional physiotherapy with 15 subjects in each group. Six minutes walk test (MWT) was used to assess the walking endurance and Functional ambulation categories (FAC) was used to evaluate the level of dependency of subjects in performing functional activities. Statistical analysis was done by using paired ‘t’ tests which showed significant improvement as the mean value of Group A (experimental group) mean 6MWT increase from -0.517 to -17.582, mean FAC increase from -1.288 to -3.055 but when intergroup analysis was done, its result was significant only in 6MWT were P= 0.046. task oriented activity after stroke could significantly improve walking endurance but not much significant on the level of dependency of the subject in performing functional activities.

Effectiveness of graded motor imagery in subjects with frozen shoulder: a pilot randomized controlled trial.

BackgroundSubjects with frozen shoulder (FS) might not be comfortable with vigorous physical therapy. Clinical trials assessing the effect of graded motor imagery (GMI) in FS are lacking. The aim of this study was to determine the effect of GMI as an adjunct to conventional physiotherapy in individuals with painful FS.MethodsTwenty subjects aged 40–65 years having stage I and II of FS were randomly divided into two study groups. The conventional physiotherapy group (n = 10) received electrotherapy and exercises while the GMI group (n = 10) received GMI along with the conventional physiotherapy thrice a week for 3 weeks. Pre- (Session 1) and post- (Session 9) intervention analysis for flexion, abduction, and external rotation range of motion (ROM) using a universal goniometer, fear of movement using the fear avoidance belief questionnaire (FABQ), pain with the visual analogue scale, and functional disability using the shoulder pain and disability index (SPADI) was done by a blinded assessor.ResultsStatistically significant difference was seen within both the groups for all the outcomes. In terms of increasing abduction ROM as well as reducing fear of movement, pain, and functional disability, the GMI group was significantly better than control group. However, both groups were equally effective for improving flexion and external rotation ROM.ConclusionsAddition of GMI to the conventional physiotherapy proved to be superior to conventional physiotherapy alone in terms of reducing pain, kinesiophobia, and improving shoulder function for stage I and II of FS.

The investigation of acute effects of fascial release technique in patients with arthroscopic rotator cuff repair: A randomized controlled trial

BackgroundFascial release technique is used for releasing fascial tissue to decrease pain, to increase range of motion, and to improve functional level. ObjectiveTo investigate the acute effects of fascial release technique on pain, range of motion, upper extremity functional level, and fear of movement in individuals with arthroscopic rotator cuff repair. MethodsThirty volunteers with an arthroscopic rotator cuff repair were included in the study. The individuals were randomly divided into two groups. One group received only conventional physiotherapy including hot pack, interferential current, and exercise program, while the fascial release technique was applied to the other group in addition to the conventional physiotherapy program for 2 weeks with 2 sessions per week. Individuals were assessed for pain intensity, range of motion, functional level, fear of movement before and after treatment, and satisfaction level after treatment. ResultsDemographic and clinical characteristics of the groups were similar (p > 0.05). Both groups showed improvement in pain, range of motion, and upper extremity functions (p < 0.05). Improvements in the treatment group were better than the conventional physiotherapy group (p < 0.05). Fear of movement was significantly decreased in the treatment group (p < 0.05). Both groups were similar in terms of satisfaction level (p > 0.05). ConclusionFascial release technique is acutely effective on the pain, joint range of motion and upper extremity functions in the treatment of individuals with arthroscopic rotator cuff repair and more successful results can be obtained by including it in the early rehabilitation program after arthroscopic rotator cuff surgery.

Comparison of the effects of conventional physiotherapy and proprioception exercises on pain and ankle proprioception in patients with lumbar radiculopathy.

Lumbar radiculopathy is characterized by a significant amount of backache causing loss of workforce and is a significant health problem frequently seen in the general population. The purpose of this study was to compare the effects of conventional physiotherapy (CT) and proprioception exercises (PE) on ankle proprioception and lumbar pain between patients with lumbar radiculopathy and a healthy control group. In this randomized clinical trial, 89 patients referred to the Physical Medicine and Rehabilitation outpatient clinic were selected through convenience sampling. They were randomly assigned to three groups: CT (n= 27), PE (n= 31), CT&PE (n= 31). Thirty healthy volunteers were included in the study as the control group. Proprioception measurements were made with an isokinetic dynamometer at 10∘ dorsiflexion (DF), 11∘, and 25∘ plantarflexion (PF) angles. Lumbar pain was assessed by using the Numerical Pain Rating Scale (NPRS). The data were analyzed by IBM SPSS Statistics version 22.0 via the Kruskal-Wallis and Mann-Whitney U tests. There was a statistically significant difference between the groups in terms of ankle proprioception and NPRS measurements in post-treatment evaluations (p< 0.05). Statistically significant differences were found between CT and PE groups and CT&PE and control groups. There was no statistically significant difference in comparing CT and PE groups and CT&PE and control groups within themselves (p> 0.05). The combined use of CT and PE is an effective method that can be used in the clinic to reduce angular differences in ankle proprioception which is one of the primary factors of balance and coordination and lumbar pain.

Effects of Paraplegia Fitness Integrated Training on Physical Function and Exercise Self-Efficacy and Adherence Among Individuals With Spinal Cord Injury.

ObjectiveTo determine the effects of the Paraplegia Fitness Integrated Training (PARAFiT) program, which is an integrated graded physical exercise and health education program for individuals with spinal cord injury (SCI).MethodsThis nonrandomized single-blind study included 44 participants, who were assigned to either an intervention (PARAFiT) group or an active control (conventional physiotherapy) group. The intervention group underwent the PARAFiT program (8 weeks), which consisted of circuit-based interval training, progressive upper limb resistance training, and health education sessions. During the unsupervised period, the intervention group continuously underwent health education program once a month for 2 months (8 weeks). Repeated-measures analysis of variance was used for the analysis.ResultsThe intervention group presented with a higher level of physical activity than did the control group; however, the difference was not significant (p=0.36). Additionally, the intervention group presented with better exercise self-efficacy and cardiorespiratory fitness and stronger bilateral shoulder muscle and handgrip than did the control group (all p<0.05). Exercise adherence was higher in the intervention group than in the control group during both the supervised (80% vs. 75%) and unsupervised (40% vs. 20%) periods.ConclusionThe PARAFiT program enhanced the level of physical activity, exercise self-efficacy, physical fitness, and exercise adherence among the patients with SCI. Future studies should incorporate guidelines for home-based exercises and regular monitoring to promote long-term adherence to exercise and physical activity among individuals with SCI.
