Abstract PO2-07-08: The efficacy of an MRI/US fusion technique for MRI-detected lesions previously undetected by conventional B-mode second-look US

Abstract Introduction Breast magnetic resonance imaging (MRI) is performed for preoperative screening of breast cancer and surveillance of BRCA gene carriers, and some lesions, called MRI-detected lesions, can only be detected by MRI. When MRI-detected lesions are found, second-look ultrasound (US) is recommended as the first choice for confirmation. However, the detection of MRI-detected lesions by conventional B-mode (cB-mode) second-look US varies from institution to institution, and it is often difficult to ensure objectivity and reproducibility. We use an MRI/US fusion technique called real-time virtual sonography (RVS) for the detection of MRI-detected lesions. RVS uses a magnetic position tracking system to display real-time US images on the same monitor as MRI images. Few reports on second-look US using RVS have examined its detection rate of MRI-detected lesions and the histopathological results of these lesions. In this study, we retrospectively reviewed MRI-detected lesions identified by second-look US using RVS but not by cB-mode second-look US from January 2018 to December 2022, with the goal of evaluating these lesions’ pathological characteristics. Materials and methods Consecutive patients who had one or more MRI-detected lesions not detected by cB-mode second-look US were enrolled in this study between January 2018 and December 2021. Second-look US using RVS was conducted after an additional supine MRI with a body surface coil was performed. Results A total of 36 patients with 38 lesions were included in this study. The mean age was 54 years old (range: 31–76). We were able to detect 31 (82%) of the 39 MRI-detected lesions by second-look US using RVS. These lesions were characterized as follows: mass, 14; non-mass-enhancement (NME), 13; and focal, 4. US-guided biopsy or excisional biopsy was performed on all lesions, and 11 (35%) of 31 lesions were malignant. We were able to detect 14 (74%) of the 19 mass lesions, and 3 were malignant (invasive ductal carcinoma (IDC) [luminal A type] in all 3 cases). We detected 13 (93%) of the 14 NME lesions, and 7 were malignant: 6 were ductal carcinoma in situ (DCIS) (high grade, 3; intermediate grade, 2; and low grade, 1), and 1 was IDC (luminal A type). Finally, we were able to detect 4 (80%) of the 5 focal lesions, 1 of which was malignant (DCIS [low grade]). Of 22 patients aged 55 years or younger, 3 had malignant lesions (IDC [luminal A type] in 2, DCIS [high grade] in 1). Of 15 patients aged 56 years or older, 8 had malignant lesions (DCIS in 7 [high grade, 2; intermediate grade, 2; low grade, 1] and IDC [luminal A type] in 1). Conclusions In this study, second-look US using RVS identified 82% of MRI-detected lesions that were not identified with cB-mode second-look US. Of these MRI-detected lesions, 35% were malignant. These results suggest that second-look US using RVS is useful for identifying MRI-detected lesions that are not detected by cB-mode second-look US. Hybrid spherical array for combined volumetric optoacoustic and B-mode ultrasound imaging.

Optoacoustic (OA) imaging has achieved tremendous progress with state-of-the-art systems providing excellent functional and molecular contrast, centimeter scale penetration into living tissues, and ultrafast imaging performance, making it highly suitable for handheld imaging in the clinics. OA can greatly benefit from efficient integration with ultrasound (US) imaging, which remains the routine method in bedside clinical diagnostics. However, such integration has not been straightforward since the two modalities typically involve different image acquisition strategies. Here, we present a new, to our knowledge, hybrid optoacoustic ultrasound (OPUS) imaging approach employing a spherical array with dedicated segments for each modality to enable volumetric OA imaging merged with conventional B-mode US. The system performance is subsequently showcased in healthy human subjects. The new OPUS approach hence represents an important step toward establishing OA in point-of-care diagnostic settings.

Improved cancer risk stratification of isoechoic thyroid nodules to reduce unnecessary biopsies using quantitative ultrasound.

Gray-scale ultrasound (US) is the standard-of-care for evaluating thyroid nodules (TNs). However, the performance is better for the identification of hypoechoic malignant TNs (such as classic papillary thyroid cancer) than isoechoic malignant TNs. Quantitative ultrasound (QUS) utilizes information from raw ultrasonic radiofrequency (RF) echo signal to assess properties of tissue microarchitecture. The purpose of this study is to determine if QUS can improve the cancer risk stratification of isoechoic TNs. Patients scheduled for TN fine needle biopsy (FNB) were recruited from the Thyroid Health Clinic at Boston Medical Center. B-mode US and RF data (to generate QUS parameters) were collected in 274 TNs (163 isoechoic, 111 hypoechoic). A linear combination of QUS parameters (CQP) was trained and tested for isoechoic [CQP(i)] and hypoechoic [CQP(h)] TNs separately and compared with the performance of conventional B-mode US risk stratification systems. CQP(i) produced an ROC AUC value of 0.937+/- 0.043 compared to a value of 0.717 +/- 0.145 (p >0.05) for the American College of Radiology Thyroid Imaging, Reporting and Data System (ACR TI-RADS) and 0.589 +/- 0.173 (p >0.05) for the American Thyroid Association (ATA) risk stratification system. In this study, CQP(i) avoids unnecessary FNBs in 73% of TNs compared to 55.8% and 11.8% when using ACR TI-RADS and ATA classification system. This data supports that a unique QUS-based classifier may be superior to conventional US stratification systems to evaluate isoechoic TNs for cancer and should be explored further in larger studies.

Analysis of Using Multi-Angle Conventional Ultrasound Scanning for Efficient 3-D Object Imaging

The purpose of this work is to examine the possibility of using multi-angle conventional ultrasound B-mode scanning in efficient 3-D imaging. In the paper, the volume of an object is reconstructed from vertical projections registered at fixed angular positions of the multi-element linear ultrasonic probe rotated in relation to the object submerged in water. The possible configurations are: vertical lateral, vertical top or vertical bottom. In the vertical lateral configuration, the ultrasonic probe acquires 2-D images of object’s vertical cross-sections, turning around its lateral surface. In the vertical top or bottom configuration, the ultrasonic probe acquires 2-D images of the object’s vertical cross-sections, turning on the horizontal plane over the top or under the bottom surface of the object. The method of recording 3-D volume of an object’s structure and reconstruction algorithm have been designed. Studies show the method in the vertical top or bottom configuration could be successfully applied to the effective 3-D visualisation of the structure of the female breast in vivo as the new complement ultrasonic imaging modality in the prototype of the developed ultrasound tomography scanner.

Diagnostic value of strain elastography and shear wave elastography in differentiating benign and malignant breast lesions.

Conventional B-mode breast ultrasonography, though the primary modality to determine benign or malignant nature of a solid breast lesion, sometimes encounters overlapping sonographic morphological features in a single lesion. Elastography leads to improvement by evaluating the structural aspects and characterization of the lesion as benign or malignant on the basis of multi-parametric assessment. Determine the role of strain elastography (SE) and shear wave elastography (SWE) in differentiating benign and malignant breast lesions. Cross sectional SETTING: Radiology department of hospital PATIENTS AND METHODS: Patients meeting inclusion criteria referred to our hospital for ultrasonography followed by biopsy or surgical excisions were examined with B-mode ultrasonography and by both strain and shear wave elastography. Mean values of SE and SWE in benign and malignant breast lesions, determination of cutoff using AUC curves and sensitivity and specificity of both techniques. One hundred breast lesions from 95 consecutive patients. The mean (SD) strain elastography ratio in the overall patient population was 4.1 (2.0). Cutoff for benign vs. malignant lesions was 2.86 on the ROC curve. The AUC was 0.911 (95%CI; 0.835-0.988: SE, 0.039) with a sensitivity of 95.8% and a specificity of 89.3%. For the SWE kPa values, the ROC curve showed the AUC was 0.929 (95% CI, 0.870-0.988; SE: 0.030, P<.001). Assigning 45.3 as a cut off value provided a sensitivity of 95.8% with a specificity of 85.7%; the positive predictive value was 94.5% and the negative predictive value was 89.6%. The Breast Imaging Reporting and Data System (BI-RADS) category alone was able to differentiate between benign and malignant lesions with a sensitivity of 91.7% and a specificity 100% keeping the cut off value between 4a and 4b. The area under the ROC curve was 0.979. Combining the three (BI-RADS + SE + SWE) distinguished benign vs. malignant lesions with a sensitivity up to 100% and specificity up to 96.3%. Combining SE and SWE as a complementary tool with conventional B-mode ultrasonography has a significant potential for better characterization of solid breast lesions and decreasing unnecessary biopsies of BI-RADS IVa lesions. Role of shear wave elastography in assessment of chronic allograft nephropathy

BackgroundThe principal cause of renal graft loss after the first year is chronic allograft nephropathy which is represented histologically by tubulo-interstitial fibrosis. Its early diagnosis and treatment are crucial to prevent late graft failure. Ultrasound is unequivocally the first-line imaging modality for the evaluation of renal transplants in the immediate postoperative period and for long-term follow-up. Ultrasound shear wave elastography is an imaging technique based on estimation of the elastic properties of tissues.Elastography is performed in the same clinical setting with conventional B-mode ultrasonography. Tissue elasticity is displayed as an absolute number and color-coded real-time estimation. So, it can be used in screening and diagnosing chronic allograft nephropathy. However, the accurate diagnosis and prognosis of renal parenchymal complications still relies on tissue biopsy. Many studies have proved the high specificity of ultrasound elastography in decreasing the number of unnecessary biopsies.ResultsIn our study, we included 36 patients with biopsy-proven chronic allograft nephropathy. All patients had a B-mode ultrasound examination and followed by ultrasound shear wave elastography in the same session. The results were compared to the histopathological results.Time since transplantation was directly correlated with mean renal stiffness, revealing that with longer time of transplantation renal stiffness and interstitial fibrosis and tubular atrophy (IF/TA) percentage increased with r = 0.72, 0.90 and p value < 0.001.Antero-posterior (AP) diameter of the renal graft was significantly correlated with mean renal stiffness as the larger the AP diameter, the higher the mean kidney stiffness with r = 0.47, 0.73 and p value 0.001.Sensitivity analysis showed that US shear wave elastography through mean kidney stiffness can significantly predict moderate Banff score of renal fibrosis using cutoff value 28.67 kPa with sensitivity 87.5%, specificity 90%, AUC 0.91 and p value < 0.001.ConclusionShear wave elastography (SWE) may be useful for the prediction of fibrosis in renal transplant patients, especially in the case of moderate Banff score, where the accuracy reached 87.5% using a cutoff value 28.67 kPa.We conclude that US SWE can be of great help during the regular follow-up of renal transplant patients. It can act as a screening tool to identify patients with stiffness values that suggest moderate tubulo-interstitial fibrosis, so eventually helping in the early diagnosis, management and help in selecting patients who are candidate for biopsy and in avoiding the repeated unnecessary biopsies for others.

Testicular rupture in a young patient: diagnostic value of contrast-enhanced ultrasonography

Testicular rupture after a blunt scrotal trauma is characterized by tearing of the tunica albuginea that result in the extrusion of the seminiferous tubules.&#x0D; Imaging, particularly ultrasonography, plays a crucial role in the assessment of scrotal trauma and directs patient management toward conservative or surgical treatment. Conventional B-mode and color Doppler ultrasonography are the main imaging techniques in the evaluation of the testicle in trauma but may underestimate the extent of injury. The most important information for the surgeon is the integrity or interruption of the tunica albuginea and the extent of vital testicular tissue. The latter is often difficult to assess with conventional ultrasonography because the injured testicle is often hypovascular even in vital regions due to testicular edema that compromises vascular flow. The selective use of advanced techniques such as contrast-enhanced ultrasonography is important in identifying testicular viability when color Doppler ultrasonography is equivocal.&#x0D; This case report describes the evaluation and management of a blunt testicular trauma in a 15-year-old football player.

The Role of Ultrasonography and Elastography in Differentiating Benign From Malignant Breast Masses With Pathologic Correlation

Objective: Elastography has the potential in differentiating benign from malignant masses. The objectives of the study were to evaluate morphology of the breast masses with routine ultrasonography and elastography, to assess the role of elastography and conventional B-mode ultrasonography in differentiating benign from malignant breast masses and to correlate elastography and B-mode ultrasonography results with pathologic findings. Materials and Methods: This prospective observational study was conducted over a period of 18 months from January 2018 to June 2019 on 86 patients with 101 clinically palpable breast lumps who underwent B-mode ultrasonography and elastography of the breast. Baseline data, sonographic features, a modified color score, and mean strain ratio were recorded and compared with final diagnosis. Results: Sonography showed a sensitivity of 89.8%; specificity of 96.15%; positive predictive value (PPV) and negative predictive value (NPV) of 95.65% and 90.91%, respectively; and overall diagnostic accuracy of 93.07%. New modified dual color score showed sensitivity of 97.8%, specificity of 87.0%, PPV of 86.79%, and NPV of 87.08% with a diagnostic accuracy of 92.08%. The risk of missing a malignant case with the new modified dual color score was 2.1%. Mean strain ratio showed sensitivity of 100%; specificity of 98.11%; PPV and NPV of 97.96% and 100%, respectively; and diagnostic accuracy of 99.01%. Conclusion: This study demonstrates the promise of elastography in identifying possible breast malignancies, thus preventing unnecessary invasive procedures.

The Characteristics of Real-time Transvaginal Sono-elastography in Endometrial Cancer.

Background:Endometrial cancer is the most common gynecological cancer among women in developed countries. Sono-elastography is an extended ultrasonographic technique that has been shown to be useful in a wide range of conditions ranging from breast, prostate, and thyroid nodules to chronic liver disease and musculoskeletal conditions. The aim of this study is to compare the sonoelastographic features of endometrial malignancy and normal endometrium.Methods:This case–control observational study was conducted at a single institution. Participants with histologically proven endometrial cancer according to the results from microcurettage or hysteroscopic biopsy and scheduled for total hysterectomy were included as cases, while asymptomatic women scheduled for routine screening ultrasound examination were recruited as controls. Both cases and controls underwent conventional B-mode transvaginal ultrasonography and strain elastography. Demographic, ultrasonographic, and histopathologic findings were analyzed.Results:A total of 29 endometrial cancer patients (cases) and 28 normal females (controls) were included in the analysis. There was no significant difference in the mean age between the two groups, but the mean body weight was significantly higher in the case group (P < 0.001). The strain ratio and elastographic thickness ratio of the endometrium were statistically significantly different between the case and the control group (P ≤ 0.05) due to increased endometrial stiffness in cancer patients as compared to the normal group.Conclusion:Our results suggest that endometrial cancer can result in increased stiffness that is detectable by transvaginal sonoelastography. Sonoelastography may serve as an adjunct to conventional ultrasound in evaluating the endometrium of women with abnormal uterine bleeding.

Detecting pulmonary nodules by using ultrasound multiple scattering.

Although X-Ray Computed Tomography (CT) is widely used for detecting pulmonary nodules inside the parenchyma, it cannot be used during video-assisted surgical procedures. Real-time, non-ionizing, ultrasound-based techniques are an attractive alternative for nodule localization to ensure safe resection margins during surgery. Conventional ultrasound B-mode imaging of the lung is challenging due to multiple scattering. However, the multiple scattering contribution can be exploited to detect regions inside the lung containing no scatterers. Pulmonary nodules are homogeneous regions in contrast to the highly scattering parenchyma containing millions of air-filled alveoli. We developed a method relying on mapping the multiple scattering contribution inside the highly scattering lung to detect and localize pulmonary nodules. Impulse response matrices were acquired in ex-vivo pig and dog lungs using a linear array transducer to semi-locally investigate the backscattered field. Extracting the multiple-scattering contribution using singular-value decomposition and combining it with a depression detection algorithm allowed us to detect and localize regions with less multiple scattering, associated with the nodules. The feasibility of this method was demonstrated in five ex-vivo lungs containing a total of 20 artificial nodules. Ninety-five percent of the nodules were detected. Nodule depth and diameter significantly correlated with their ex-vivo CT-estimated counterparts (R = 0.960, 0.563, respectively).

Diagnostic role of contrast-enhanced ultrasonography versus conventional B-mode ultrasonography in cirrhotic patients with early hepatocellular carcinoma: a retrospective study.

Early diagnosis of hepatocellular carcinoma (HCC) is very important for the prognosis of patients. However, there are very few studies that compared the diagnostic accuracy of contrast-enhanced ultrasonography (CEUS) and B-mode ultrasonography for early HCC in cirrhotic patients. This retrospective study included cirrhotic patients, who were suspected of early HCC between January 2020 and June 2021. The diagnosis of patients was based on the pathology results of surgery or biopsy. Demographic and clinical characteristics of included patients were recorded. The diagnoses of suspected lesions using both types of ultrasonography were recorded, and the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and diagnostic accuracy of early HCC in cirrhotic patients were calculated. Eventually, 137 patients with solitary lesions in the liver were included in this study, including 89 patients diagnosed with HCC and 48 patients diagnosed with non-HCC. The median diameter of suspected lesions was 26 mm, and the median level of alpha fetoprotein (AFP) was 37.2 ng/mL. When comparing the demographic and clinical characteristics of cirrhotic patients with HCC and non-HCC, it was found that patients with HCC had significantly higher levels of AFP than those with non-HCC (P=0.03). The sensitivity, specificity, PPV, NPV, and accuracy of CEUS in early HCC were 73%, 93.8%, 95.6%, 65.2% and 80.3%, respectively. In CEUS, all of these parameters were much higher than those in B-mode ultrasonography, i.e., 64%, 75%, 82.6%, 52.9%, and 67.9%. It was also found that the diagnostic accuracy of CEUS was much higher than that of B-mode ultrasonography especially regarding lesions <20 mm. To further improve the sensitivity of CEUS in early HCC, AFP was combined with CEUS for the diagnosis of early HCC. As a result, the sensitivity, specificity, PPV, NPV, and accuracy of CEUS combined with AFP level were 83.1%, 87.5%, 92.5%, 73.7%, and 84.7%, respectively. Our study confirmed that CEUS' diagnostic accuracy for early HCC in cirrhotic patients was significantly higher than that of B-mode ultrasonography. However, the sensitivity of CEUS needs to be improved further, and the combination of CEUS and AFP level may be a potential solution.

Quantitative assessment of fatty liver using ultrasound attenuation imaging.

Non-alcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease, with an incidence of 20-40% worldwide, making it a major healthcare problem. Because NAFLD can progress to liver fibrosis and cirrhosis through non-alcoholic steatohepatitis, early detection and monitoring of hepatic steatosis are essential for management of NAFLD patients. Even though conventional B-mode ultrasound (US) has been widely used for the evaluation of NAFLD owing to its safety and easy accessibility, its subjective nature and limited accuracy in detecting mild steatosis are major limitations. To overcome the current limitations of conventional B-mode US, attenuation imaging (ATI) based on two-dimensional B-mode US has been developed. ATI can quantitatively assess the degree of hepatic steatosis by calculating the attenuation coefficient, which reflects the degree of US beam attenuation transmitted into the liver tissue. After the first study was published in 2019, several clinical studies using ATI to evaluate hepatic steatosis have reported promising results. In this review, the basic physics, measurement protocol, and reported diagnostic accuracy of ATI in evaluating hepatic steatosis will be discussed.

Effect of Quantitative and Semi-quantitative Elastography Methods for the Management of Borderline Lesions on Ultrasonography.

Elastography (strain or shear-wave) is a method that estimates tissue stiffness. The aim of this study is to evaluate the quantitative and semi-quantitative ultrasound elastography methods for the diagnosis of BI-RADS 4a and BI-RADS 3 lesions, which are borderline for biopsy and follow-up. 175 consecutive women with 193 ultrasound-visible breast lesions were classified on Conventional B-mode Ultrasonography (CUS) according to the BI-RADS scoring system. Quantitative and semiquantitative values from ultrasound elastography in the form of strain Elastography Ratio (SER), shear Wave Elastography (SWE) and Shear Wave Elastography Ratio (SWER) were obtained. The lesions categorized as BI-RADS 4a and BI-RADS 3 on ultrasound were subsequently re-categorized according to the elastography values. Except for the 13 BI-RADS 2 lesions, the remaining 180 lesions were biopsied. Pathology showed 83 lesions to be benign and 97 to be malignant. The sensitivity and specificity of the CUS were 96.9% and 75.0%, respectively with an accuracy of 86.0%. Cut-off points calculated based on ROC curves were 56.8 kPa for SWE, 3.53 for SWER and 3.81 for SER. When we downgraded BIRADS 4a lesions based on elastography results, the specificity (CUS+SER 96.9%, CUS+SWE 91.7%, and CUS+SWER 90.6%) and the accuracy (CUS+SER 95.3%, CUS+SWE 92.7%, and CUS+SWER 92.2%) were shown to be better than CUS. When we upgraded BI-RADS 3 lesions based on elastography results, the sensitivity of combined sets of SWE (99,0%) and SWER (100,0%) was better than CUS. The rate of false-negative biopsies can be decreased with the combined use of elastography and ultrasonography.

Point shear wave elastography of the liver in healthy adult cats.

To evaluate stiffness of the liver parenchyma in healthy adult cats by means of point shear wave elastography (PSWE). 18 client-owned adult (1- to 6-year-old) healthy cats. Echogenicity and echotexture of the liver parenchyma were assessed by means of conventional B-mode ultrasonography. The shear wave velocity (Vs) of the right and left portions of the liver were measured by means of PSWE. B-mode ultrasonography revealed no abnormalities in echotexture or echogenicity of the liver parenchyma in any cat. Mean (95% CI) Vs in the liver parenchyma was 1.46 m/s (1.36 to 1.55 m/s) for the right portion, 1.36 m/s (1.26 to 1.47 m/s) for the left portion, and 1.43 m/s (1.35 to 1.51 m/s) overall. The difference in mean Vs between the 2 portions of the liver was significant. No significant correlation was found between Vs and body weight or between Vs and the depth at which this variable was measured. Quantitative PSWE of the liver was feasible in healthy adult cats. The obtained values for Vs may be useful for interpretation of and comparison with values measured in cats with liver disease. Additional research is needed to explore the potential usefulness of PSWE for diagnostic purposes.

ROLE OF ULTRASONOGRAPHIC ELASTOGRAPHY IN THE DIFFERENTIAL DIAGNOSIS OF BREAST MASSES AND ITS CONTRIBUTION TO CLASSICAL ULTRASONOGRAPHIC EVALUATION

Introduction: India is experiencing an unprecedented rise in the number of breast cancer cases across all sections of society. Breast cancer is now the most common malignancy in women and the second leading cause of cancer- related mortality. Breast cancer is quite easily and effectively treated, provided it is detected in it's early stages. There is a drastic drop in the survival rates when women present with advanced stage of breast cancer, regardless of the setting. Unfortunately, women in resource-poor and developing countries, like India, generally present at a later stage of disease than women elsewhere, partly due to the absence of effective awareness programs and partly due to the lack of proper mass screening programs Aims And Objectives: The diagnostic performance of elastography in differentiating benign from malignant breast lesions. To assess whether elastography has the potential to reduce the need for breast biopsy /FNAC. Cut off value of Strain Ratio for benign versus malignant breast lesions. Further characterize BI-RADS 3 lesions using elastography Materials And Methods: The study was approved by the GSVM MEDICAL COLLEGE AND LLR HOSPITAL Ethics Committee. All patients that presented to the Radiology and Imaging Department of LLR HOSPITAL for diagnostic work up for breast pathology were included in the study. After obtaining a written and signed informed consent from all patients, they were subjected to conventional B-Mode ultrasonography followed by elastography. All diagnostic breast imaging was done with Samsung RS80A ultrasound machine using linear array transducer of frequency 5-12MHz.Observations &amp; Results: The elastography patterns for each lesion were assessed and documented in color scale. Color images were constructed automatically and displayed as a color-overlay on the B-mode image. The color pattern of each lesion was then evaluated on a scale of 1-5 according to the Tsukuba elasticity scoring system. Conclusion: Strain Ratio cutoff of 3.3 is a sensitive parameter to differentiate benign and malignant breast lesions. Elastography is a specic test for differentiating benign and malignant breast lesions. The combined use of elasticity score, strain ratio and B- Mode sonographyincreases the diagnostic performance in distinguishing benign from malignant breast masses.

Elastography and Doppler May Bring a New Perspective to TIRADS, Altering Conventional Ultrasonography Dominance

The main aim of ultrasonography (US) examining thyroid nodules is to differentiate malignant nodules from benign nodules. Several professional societies and groups of investigators have defined guidelines such as Thyroid Imaging Reporting and Data System (TIRADS) to provide the standardized language and approach to thyroid nodules. This study is aimed to investigate the compatibility of such classification systems with the pathological diagnosis of nodules and evaluate the contribution of the Shear-wave elastography (SWE) and Doppler ultrasonography (DUS) findings. This is a prospective study. Patients with thyroid US exams between December 2017 and April 2019 were included. In the study, eligible 210 nodules from 210 patients were enrolled. For stratification, the conventional B-mode US, SWE and DUS were performed. According to Kwak, American College of Radiology (ACR), and European (EU)-TIRADS, Nodules were classified separately, and a new scoring system whose the criteria was put defined in the study has developed. For SWE; Emean cut-off value was 33 kPa with a sensitivity and specificity of 95,6% (95% CI: 0,85-0,98) and 95% (95% CI:0,90-0,97) respectively (p <0.001). For spectral DUS; resistivity index (RI) cut-off value was 0.64 with a sensitivity and specificity of 73,3% (95% CI:0,59-0,83) and 80% (95% CI:0,73-0,85) respectively (p <0.001). Kwak TIRADS, American College of Radiology TIRADS, EU-TIRADS, and new system were compared by ROC curve analysis. The new system has the highest sensitivity, specificity, PPV, NPV, accuracy, and AUC compared to others. The new scoring system has shown that SWE and DUS findings may alter the categorization in TIRADS and increase sensitivity and specificity.

Performance of Ultrasound Techniques and the Potential of Artificial Intelligence in the Evaluation of Hepatocellular Carcinoma and Non-Alcoholic Fatty Liver Disease.

Simple SummaryThe increasing prevalence of non-alcoholic fatty liver disease (NAFLD) represents a challenge for the current medical systems. If NAFLD is left undetected and untreated, it can progress towards fibrosis, cirrhosis, and hepatocellular carcinoma (HCC). To date, ultrasonography (US) is the first-line examination indicated to NAFLD patients that also screens for other focal liver lesions (FLLs). The downside of conventional B-mode US is that it cannot accurately quantify steatosis and fibrosis and cannot further characterize FLL certainly—is it cancer or is it not? Ultrasound contrast agents (UCAs) allowed physicians to further evaluate the FLL for the diagnosis of HCC. This review discusses the performance of US techniques in NAFLD and NAFLD-related HCC diagnosis, as well as of artificial intelligence (AI)-based methods, specifically the usefulness and assistance of deep learning algorithms for improving liver US image processing.Global statistics show an increasing percentage of patients that develop non-alcoholic fatty liver disease (NAFLD) and NAFLD-related hepatocellular carcinoma (HCC), even in the absence of cirrhosis. In the present review, we analyzed the diagnostic performance of ultrasonography (US) in the non-invasive evaluation of NAFLD and NAFLD-related HCC, as well as possibilities of optimizing US diagnosis with the help of artificial intelligence (AI) assistance. To date, US is the first-line examination recommended in the screening of patients with clinical suspicion of NAFLD, as it is readily available and leads to a better disease-specific surveillance. However, the conventional US presents limitations that significantly hamper its applicability in quantifying NAFLD and accurately characterizing a given focal liver lesion (FLL). Ultrasound contrast agents (UCAs) are an essential add-on to the conventional B-mode US and to the Doppler US that further empower this method, allowing the evaluation of the enhancement properties and the vascular architecture of FLLs, in comparison to the background parenchyma. The current paper also explores the new universe of AI and the various implications of deep learning algorithms in the evaluation of NAFLD and NAFLD-related HCC through US methods, concluding that it could potentially be a game changer for patient care.

Feasibility and Reliability of Two-Dimensional Shear-Wave Elastography of the Liver of Clinically Healthy Cats

Given the broad overlap of normal and abnormal liver tissue in the subjective evaluation of the liver in conventional B-mode ultrasonography, there is a need for a non-invasive and quantitative method for the diagnosis of liver disease. Novel two-dimensional shear-wave elastography (2-D SWE) can measure tissue stiffness by propagation of the shear wave induced using acoustic radiation force impulse in real time. To the best of our knowledge, two-dimensional shear-wave measurement of the liver in cats has not been reported to date. This study assessed the feasibility, reliability, normal values, and related influencing factors of 2-D SWE for assessment of the feline liver without anesthesia and breath-holding. Two-dimensional shear-wave ultrasonography was performed by two evaluators at the right and left sides of the liver. Twenty-nine client-owned clinically healthy adult cats were included. The means and standard deviations for the shear-wave speed and stiffness in the right liver were 1.52 ± 0.13 m/s and 6.94 ± 1.26 kPa, respectively, and those for the left liver were 1.61 ± 0.15 m/s and 7.90 ± 1.47 kPa, respectively. Shear-wave speed (P = 0.005) and stiffness (P = 0.002) were significantly lower in the right liver when compared to the left. The intraclass correlation value for liver stiffness was 0.835 and 0.901 for the right and left liver, respectively, indicating high interobserver agreement. Age, weight, body condition score (BCS), gabapentin administration, and measurement depths were not significantly correlated with liver stiffness or elastography measurements (P > 0.05). Our findings suggest that 2-D SWE measurements of the liver are not influenced significantly by age, weight, or BCS and can be reliably performed without anesthesia and breath-holding in cats. The values determined here can help form the basis for reference elastography values for evaluation of the feline liver.

Comparison between B-mode ultrasonography and contrast-enhanced ultrasonography for the surveillance of early stage pancreatic cancer: a retrospective study.

Contrast-enhanced ultrasonography (CE-US) brings a higher signal-to-noise ratio and a higher sensitivity for slow flow than traditional B-mode ultrasonography (US). However, it remains unclear whether CE-US is also superior to B-mode US in detecting early-stage pancreatic cancer (PC). This was a retrospective study enrolling patients suspected of pancreatic insufficiency between June 2015 and December 2019. Enrolled patients successively received B-mode US and CE-US examinations, and some their demographic and clinical data were collected. The diagnostic capacity of the two examinations was calculated and receiver operating characteristic (ROC) curves was used to compare the area under the curve (AUC). A subgroup analysis was performed to explore the effects of tumor size on the diagnostic accuracy of B-mode US and CE-US. There were 128 patients enrolled in this study; 74 patients were diagnosed as early-stage PC patients and the remaining 54 were diagnosed with benign pancreatic lesions. The mean size of the PC was 17.8±4.9 mm. The results revealed that 68 of the 74 PC patients were correctly diagnosed by CE-US, and all 54 patients with benign pancreatic lesions were also correctly diagnosed. Meanwhile, only 55 of the 74 PC patients and 50 of the 54 patients with benign pancreatic lesions were diagnosed correctly using B-mode US. The ROC curve showed that the AUCs of CE-US and B-mode US were 0.959 and 0.835, respectively. According to the subgroup analysis, CE-US exhibited better accuracy than B-mode US for smaller tumors (size <20 mm, P=0.002; size <10 mm, P=0.043; size <5 mm, P=0.025). CE-US was clearly superior to the conventional B-mode US in detecting early-stage PC, especially smaller sized PC.

Reducing Unnecessary Biopsy of Breast Lesions: Preliminary Results with Combination of Strain and Shear-Wave Elastography

The aim of our study was to compare strain elastography (SE), acoustic radiation force impulse-inducing Virtual Touch Imaging ([VTI] Siemens Medical Solutions, Mountain View, CA, USA), Virtual Touch Imaging Quantification ([VTIQ] Siemens Medical Solutions) and combined methods in the evaluation of ultrasound (US) Breast Imaging-Reporting and Data System (BI-RADS) category 4 lesions to explore an applicable way to reduce unnecessary biopsy by reducing false positives of conventional US without yielding false-negative cases. A total of 267 patients with 278 BI-RADS category 4 lesions (151 benign and 127 malignant) were evaluated with conventional B-mode US, SE, VTI and VTIQ implemented on a Siemens Acuson S2000 US system. Diagnostic performance, including area under the receiver operating characteristic curve, sensitivity, specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) were evaluated. Overall, VTI alone exhibited the highest NPV (91.74%), although combined elastic methods exhibited higher NPV than single methods, with the highest NPV at 100% when the VTI, SE and VTIQ methods were combined. Compared with conventional US, PPV increased from 45.7% (127 of 278) to 63.18% (127 of 201) when adding combined elastography (VTI + SE +VTIQ). In addition, 52.5% (63/120) and 50.8% (61/120) of BI-RADS 4 A lesions were downgraded when using combined methods (VTI + SE and VTI + SE + VTIQ, respectively) without missing any cancer. However, 2 intraductal papillomas and 1 phyllodes tumor were not identified. In conclusion, the combination of different elastic methods have the potential to downgrade BI-RADS 4A lesions to reduce false-positive biopsies without increasing the risk of missing cancers.
