Countering Calcium Blooming With Personalized Contrast Media Injection Protocols: The 1-2-3 Rule for Photon-Counting Detector CCTA.

Photon-counting detector computed tomography (PCD-CT) enables spectral data acquisition of CT angiographies allowing for reconstruction of virtual monoenergetic images (VMIs) in routine practice. Specifically, it has potential to reduce the blooming artifacts associated with densely calcified plaques. However, calcium blooming and iodine attenuation are inversely affected by energy level (keV) of the VMIs, creating a challenge for contrast media (CM) injection protocol optimization. A pragmatic and simple rule for calcium-dependent CM injection protocols is investigated and proposed for VMI-based coronary CT angiography with PCD-CT. A physiological circulation phantom with coronary vessels including calcified lesions (maximum CT value >700 HU) with a 50% diameter stenosis was injected into at iodine delivery rates (IDRs) of 0.3, 0.5, 0.7, 1.0, 1.5, 2.0, 2.5, and 3.0 g I/s. Images were acquired using a first-generation dual-source PCD-CT and reconstructed at various VMI levels (between 45 and 190 keV). Iodine attenuation in the coronaries was measured at each IDR for each keV, and blooming artifacts from the calcified lesions were assessed including stenosis grading error (as % overestimation vs true lumen). The IDR to achieve 300 HU at each VMI level was then calculated and compared with stenosis grading accuracy to establish a general rule for CM injection protocols. Plaque blooming artifacts and intraluminal iodine attenuation decreased with increasing keV. Fixed windowing (representing absolute worst case) resulted in stenosis overestimation from 77% ± 4% at 45 keV to 5% ± 2% at 190 keV, whereas optimized windowing resulted in overestimation from 29% ± 3% at 45 keV to 4% ± 1% at 190 keV. The required IDR to achieve 300 HU showed a strong linear correlation to VMI energy (R2 = 0.98). Comparison of this linear plot versus stenosis grading error and blooming artifact demonstrated that multipliers of 1, 2, and 3 times the reference IDR for theoretical clinical regimes of no, moderate, and severe calcification density, respectively, can be proposed as a general rule. This study provides a proof-of-concept in an anthropomorphic phantom for a simple pragmatic adaptation of CM injection protocols in coronary CT angiography with PCD-CT. The 1-2-3 rule demonstrates the potential for reducing the effects of calcium blooming artifacts on overall image quality.

Evaluation of a Tube Voltage-Based Contrast Media Adaptation in Coronary Computed Tomography Angiography Using Personalized Triphasic Injection Protocols: A Matched Case-Control Study

Rationale and ObjectivesCoronary CT angiography (CCTA) has recently been established as a first line test in patients with suspected coronary artery disease (CAD). Due to the increased use of CCTA, strategies to reduce radiation and contrast medium (CM) exposure is of high importance. The aim of this study was to evaluate the performance of automated tube voltage selection (ATVS)-adapted CM injection protocol for CCTA compared to a clinically established triphasic injection protocol in terms of image quality, radiation exposure, and CM administration Material and MethodsPatients undergoing clinically indicated CCTA were prospectively enrolled from July 2021 to July 2023. Patients underwent CCTA using a modified triphasic CM injection protocol tailored to the tube voltage by the ATVS algorithm, in a range of 70 to 130kV with a 10kV interval. The injection protocol consisted of two phases of mixed CM and saline boluses with different proportions to assure a voltage-specific iodine delivery rate, followed by a third phase of saline flush. This cohort was compared to a control group identified retrospectively and scanned on the same CT system but with a standard triphasic CM protocol. Radiation and contrast dose, subjective and objective image quality (contrast-to-noise-ratio [CNR] and signal-to-noise-ratio [SNR]) were compared between the two groups. ResultsThe final population consisted of 120 prospective patients matched with 120 retrospective controls, with 20 patients in each kV group. The 120kV group was excluded from the statistical analysis due to insufficient sample size. A significant CM reduction was achieved in the prospective group overall (46.0 [IQR 37.0-52.0] vs. 51.3 [IQR 40.1-73.0] ml, p<0.001) and at all kV levels too (all pairwise p<0.001). There were no significant differences in radiation dose (6.13 ± 4.88 vs. 5.97 ± 5.51 mSv, p=0.81), subjective image quality (median score of 4 [3–5] vs 4 [3–5], p=0.40), CNR, and SNR in the aorta and the left anterior descending coronary artery (all p>0.05). ConclusionATVS-adapted CM injection protocol allows for diagnostic quality CCTA with reduced CM volume while maintaining similar radiation exposure, subjective and objective image quality.

Using Patient-Specific Contrast Enhancement Optimizer Simulation Software During the Transcatheter Aortic Valve Implantation-Computed Tomography Angiography in Patients With Aortic Stenosis.

This study assessed whether patient-specific contrast enhancement optimizer simulation software (p-COP) can reduce the contrast material (CM) dose compared with the conventional body weight (BW)-tailored scan protocol during transcatheter aortic valve implantation-computed tomography angiography (TAVI-CTA) in patients with aortic stenosis. We used the CM injection protocol selected by the p-COP in group A (n = 30). p-COP uses an algorithm that concerns data on an individual patient's cardiac output. Group B (n = 30) was assigned to the conventional BW-tailored CM injection protocol group. We compared the CM dose, CM amount, injection rate, and computed tomography (CT) values in the abdominal aorta between the 2 groups and classified them as acceptable (>280 Hounsfield units (HU)) or unacceptable (<279 HU) based on the optimal CT value and visualization scores for TAVI-CTA. We used the Mann-Whitney U test to compare patient characteristics and assess the interpatient variability of subjects in both groups. Group A received 56.2 mL CM and 2.6 mL/s of injection, whereas group B received 76.9 mL CM and 3.4 mL/s of injection (P < 0.01). The CT value for the abdominal aorta at the celiac level was 287.0 HU in group A and 301.7HU in group B (P = 0.46). The acceptable (>280 HU) and unacceptable (<280 HU) CT value rates were 22 and 8 patients in group A and 24 and 6 patients in group B, respectively (P = 0.76). We observed no significant differences in the visualization scores between groups A and B (visualization score = 3, P = 0.71). The utilization of p-COP may decrease the CM dosage and injection rate by approximately 30% in individuals with aortic stenosis compared with the body-weight-tailored scan protocol during TAVI-CTA.

Optimal Abdominal CT Image Quality in Non-Lean Patients: Customization of CM Injection Protocols and Low-Energy Acquisitions.

We compared the image quality of abdominopelvic single-energy CT with 100 kVp (SECT-100 kVp) and dual-energy CT with 65 keV (DECT-65 keV) obtained with customized injection protocols to standard abdominopelvic CT scans (SECT-120 kVp) with fixed volumes of contrast media (CM). We retrospectively included 91 patients (mean age, 60.7 ± 15.8 years) with SECT-100 kVp and 83 (mean age, 60.3 ± 11.7 years) patients with DECT-65 keV in portovenous phase. Total body weight-based customized injection protocols were generated by a software using the following formula: patient weight (kg) × 0.40/contrast concentration (mgI/mL) × 1000. Patients had a prior abdominopelvic SECT-120 kVp with fixed injection. Iopamidol-370 was administered for all examinations. Quantitative and qualitative image quality comparisons were made between customized and fixed injection protocols. Compared to SECT-120 kVp, customized injection yielded a significant reduction in CM volume (mean difference = 9-12 mL; p ≤ 0.001) and injection rate (mean differences = 0.2-0.4 mL/s; p ≤ 0.001) in all weight categories. Improvements in attenuation, noise, signal-to-noise and contrast-to-noise ratios were observed for both SECT-100 kVp and DECT-65 keV compared to SECT-120 kVp in all weight categories (e.g., pancreas DECT-65 keV, 1.2-attenuation-fold increase vs. SECT-120 kVp; p < 0.001). Qualitative scores were ≥4 in 172 cases (98.8.4%) with customized injections and in all cases with fixed injections (100%). These findings suggest that customized CM injection protocols may substantially reduce iodine dose while yielding higher image quality in SECT-100 kVp and DECT-65 keV abdominopelvic scans compared to SECT-120 kVp using fixed CM volumes.

"ASSESSING THE EFFECTIVENESS OF OPTIMIZED CONTRAST AGENT USAGE IN CT SCANNING"

Background: To assess individually optimized protocol of contrast medium injection in enhanced CT scan for liver imaging.Materials &amp; methods: 50 patients who underwent liver CT scan were included in the present study.Random division of all the patients was done into two study groups as follows:Group A and group B (25 patients in each group).All the patients were scanned using CT machine.All patients underwent both unenhanced and enhanced CT scans during hepatic arterial phase (HAP) and portal venous phase (PVP).All the enhanced CT scans during HAP and PVP started at thirty-five seconds and sixty-five seconds respectively, after the contrast injection, from the level of diaphragm to inferior hepatic edge. Both groups received the same contrast medium. Group A adopted an individually optimized protocol of the platform, which automatically calculated the contrast medium dose. Patients of group B received a standard contrast medium injection protocol with a contrast medium to weight dose of 1.5 ml/kg. CT values of unenhanced liver parenchyma, CT values of liver parenchyma during HAP and PVP, and CT values of the portal vein during PVP were measured.CT examinations were performed in both group A and group B patients.Analysis of results was done using SPSS software.Results: Mean CT value of liver parenchyma during HAP (HU) among patients of group A and group B was 78.3 and 77.1 respectively. Mean CT value of liver parenchyma during PVP (HU) among patients of group A and group B was 103.1 and 108.3 respectively.Mean CT value of portal vein during PVP (HU) among patients of group A and group B was 145.3 and 158.7 respectively.Mean Contrast medium dose (ml) among patients of group A and group B was 79.2 and 95.3 respectively.Significant results were obtained while comparing the outcome variables among the two study groups. Conclusion: From the above results, the authors concluded that in enhanced CT scan for liver imaging,individualizing the contrast dose on the basis of body weight through contrast medium injection protocol could efficaciously decrease contrast medium dose

Effect of contrast material injection protocol on first-pass myocardial perfusion assessed by dual-energy dual-layer computed tomography.

Dual-energy dual-layer computed tomography (CT) scanners can provide useful tools, such as iodine maps and virtual monochromatic images (VMI), for the evaluation of myocardial perfusion defects. Data about the influence of acquisition protocols and normal values are still lacking. Clinically indicated coronary CT-angiographies performed between January-October 2018 in a single university hospital with dual-energy dual-layer CT (DE-DLCT) and different injection protocols were retrospectively evaluated. The two protocols were: 35 mL in patients <80 kg and 0.5 mL/kg in patients >80 kg at 2.5 mL/s (group A) or double contrast dose at 5 mL/s (group B). Patients with coronary stenosis >50% were excluded. Regions of interest were manually drawn on 16 myocardial segments and iodine concentration was measured in mg/mL. Signal-to-noise, contrast-to-noise ratios (CNR) and image noise were measured on conventional images and VMI. A total of 30 patients were included for each protocol. With iodine concentrations of 1.38±0.41 mg/mL for protocol A and 2.07±0.73 mg/mL for protocol B, the two groups were significantly different (P<0.001). No significant iodine concentration differences were found between the 16 segments (P=0.47 and P=0.09 for group A and B respectively), between basal, mid and apical segments for group A and B (P=0.28 and P=0.12 for group A and B respectively) and between wall regions for group A (P=0.06 on normalised data). In group B, iodine concentration was significantly different between three wall regions [highest values for the lateral wall, median =2.03 (1.06) mg/mL]. Post-hoc analysis showed highest contrast-to-noise and signal-to-noise in VMI at 40 eV (P<0.05). Iodine concentration in left ventricular myocardium of patients without significant coronary artery stenosis varied depending on the injection protocol and appeared more heterogeneous in different wall regions at faster injection rate and greater iodine load. Signal-to-noise and contrast-to-noise gradually improved when decreasing VMI energy, although at the expenses of higher noise, demonstrating the potential of DE-DLCT to enhance objective image quality.

Individualized Scan Protocols in Abdominal Computed Tomography: Radiation Versus Contrast Media Dose Optimization.

In contrast-enhanced abdominal computed tomography (CT), radiation and contrast media (CM) injection protocols are closely linked to each other, and therefore a combination is the basis for achieving optimal image quality. However, most studies focus on optimizing one or the other parameter separately. Reducing radiation dose may be most important for a young patient or a population in need of repetitive scanning, whereas CM reduction might be key in a population with insufficient renal function. The recently introduced technical solution, in the form of an automated tube voltage selection (ATVS) slider, might be helpful in this respect. The aim of the current study was to systematically evaluate feasibility of optimizing either radiation or CM dose in abdominal imaging compared with a combined approach. Six Göttingen minipigs (mean weight, 38.9 ± 4.8 kg) were scanned on a third-generation dual-source CT. Automated tube voltage selection and automated tube current modulation techniques were used, with quality reference values of 120 kVref and 210 mAsref. Automated tube voltage selection was set at 90 kV semimode. Three different abdominal scan and CM protocols were compared intraindividually: (1) the standard "combined" protocol, with the ATVS slider position set at 7 and a body weight-adapted CM injection protocol of 350 mg I/kg body weight, iodine delivery rate (IDR) of 1.1 g I/s; (2) the CM dose-saving protocol, with the ATVS slider set at 3 and CM dose lowered to 294 mg I/kg, resulting in a lower IDR of 0.9 g I/s; (3) the radiation dose-saving protocol, with the ATVS slider position set at 11 and a CM dose of 441 mg I/kg and an IDR 1.3 g I/s, respectively. Scans were performed with each protocol in arterial, portal venous, and delayed phase. Objective image quality was evaluated by measuring the attenuation in Hounsfield units, signal-to-noise ratio, and contrast-to-noise ratio of the liver parenchyma. The overall image quality, contrast quality, noise, and lesion detection capability were rated on a 5-point Likert scale (1 = excellent, 5 = very poor). Protocols were compared for objective image quality parameters using 1-way analysis of variance and for subjective image quality parameters using Friedman test. The mean radiation doses were 5.2 ± 1.7 mGy for the standard protocol, 7.1 ± 2.0 mGy for the CM dose-saving protocol, and 3.8 ± 0.4 mGy for the radiation dose-saving protocol. The mean total iodine load in these groups was 13.7 ± 1.7, 11.4 ± 1.4, and 17.2 ± 2.1 g, respectively. No significant differences in subjective overall image or contrast quality were found. Signal-to-noise ratio and contrast-to-noise ratio were not significantly different between protocols in any scan phase. Significantly more noise was seen when using the radiation dose-saving protocol (P < 0.01). In portal venous and delayed phases, the mean attenuation of the liver parenchyma significantly differed between protocols (P < 0.001). Lesion detection was significantly better in portal venous phase using the CM dose-saving protocol compared with the radiation dose-saving protocol (P = 0.037). In this experimental setup, optimizing either radiation (-26%) or CM dose (-16%) is feasible in abdominal CT imaging. Individualizing either radiation or CM dose leads to comparable objective and subjective image quality. Personalized abdominal CT examination protocols can thus be tailored to individual risk assessment and might offer additional degrees of freedom.

Effect of different contrast medium injection protocols for analysis of hepatic computed tomography perfusion in healthy dogs.

To evaluate the hepatic CT perfusion (CTP) for determining the appropriate protocol for the dual-input maximum-slope model in dogs. 5 healthy dogs. Each dog underwent CTP with different contrast medium administration protocols. Combinations of three different injected doses of iohexol (450, 600, and 750 mg/kg) and injection durations (5, 10, and 15 seconds) were used. The CT values at the aorta, portal vein, and hepatic parenchyma were measured to create a time-density curve, and CTP parameters were measured simultaneously on each hepatic lobe using a 320-row multidetector CT scanner. The maximum peak enhancement at the aorta, portal vein, and hepatic parenchyma was greater with the 750-mg/kg dose than with the 450-mg/kg dose. With an injection duration of 15 seconds, the aortic enhancement peak was less, and the arrival time at the aortic enhancement peak was longer compared to that with a 5-second injection duration. The CTP parameters in the caudate process of the caudate lobe and left lateral lobe differed with different injection durations. The CTP parameters in the caudate process of the caudate lobe, left lateral lobe, and right lateral lobe differed with varying injected doses. Our study demonstrated that rapid administration of the contrast medium was required for quantitative analysis of hepatic CTP in healthy dogs. The CTP parameters differed with respect to the contrast medium administration protocol, and it was necessary to administer the contrast medium within a fixed duration and at a fixed dose to evaluate CTP correctly.

A Heart Rate-Dependent Protocol of "One-Stop" Computed Tomography Angiography of Coronary Combined with Pulmonary Arteries Reduces the Dosages of Contrast Agent.

Background:The personalized, heart rate-dependent computed tomography angiography (CTA) protocol can reduce the use of contrast medium (CM) and the radiation dose. This is especially beneficial for patients with CTA of coronary combined with pulmonary arteries.Purpose:To evaluate the feasibility of low CM in one-stop coronary and pulmonary arterial CTA tailored by patients’ heart rate.Material and Methods:94 patients set to undergo CTA of coronary combined with pulmonary arteries with one-stop scans. Patients were prospectively randomized into two groups: For group A (n = 47), the timing of the scans was determined according to the patient’s HR using 30 mL CM; For group B (n = 47), in which the routine bolus tracking was applied by setting the ascending aortic threshold of 80 HU with 70 mL CM, scans were performed simultaneously.Results:Compared with group B, group A had slightly higher computed tomography (CT) value and image quality of pulmonary artery (CT value: group A 484.7HU; group B 457.9HU; t = 2.446, P = .016; image quality: χ2 = 8.292, P = .016), but in coronary artery wasn’t statistically different between two groups(image quality: χ2 = 2.516, P = .642).Conclusion:The heart rate-dependent CM injection protocol can greatly reduce the use of CM, simplify the work-flow, and may obtain comparable or even better image quality compared with the routine bolus tracking.

Clinical effectiveness of contrast medium injection protocols for 80-kV coronary and craniocervical CT angiography-a prospective multicenter observational study.

Decreasing X-ray tube voltage is an effective way to reduce radiation and contrast dose, especially in non-obese patients. The current study focuses on CTA in non-obese patients to evaluate image quality and feasibility of 80-kV acquisition protocols with varying iodine delivery rates (IDR) and contrast concentrations in routine clinical practice. A prospective observational study in patients ≥ 18years and ≤ 90kg referred for coronary or craniocervical CTA at 10 centers in China (ClinicalTrials.gov: NCT02840903). Patients were divided into four groups: a standard 100-kV protocol (370 mgI/ml, IDR 1.48 gI/s), and three 80-kV protocols (370 mgI/ml, IDR 1.2 gI/s; 300 mgI/ml, IDR 1.2 gI/s; 300 mgI/ml, IDR 0.96gI/s). The primary outcome was contrast opacification of target vascular segments. Secondary outcomes were image quality (contrast-to-noise ratio, signal-to-noise ratio, visual image quality, and diagnostic confidence assessment), radiation, and iodine dose. From July 2016 to July 2017, 1213 patients were enrolled: 614 coronary and 599 craniocervical CTA. The mean contrast opacification was ≥ 300 HU for 80-kV 1.2 gI/s IDR scanned segments; IDR 0.96 gI/s led to lower opacification. Image quality and diagnostic confidence were fair to excellent (≥ 98% of images), despite lower contrast-to-noise ratios and signal-to-noise ratios in 80-kV images. Compared to the standard protocol, 80-kV protocols led to 44-52% radiation dose reductions (p < 0.001) and 19% iodine dose reductions (p < 0.001). Eighty-kilovolt 1.2 gI/s IDR protocols can be recommended for coronary and craniocervical CTA in non-obese patients, reducing radiation and iodine dose without compromising image quality. • Using low-voltage scanning CTA protocols, in which tube voltage and iodine delivery rate are reduced proportionally (voltage: 80kV, IDR: 1.2 gI/s), reduces radiation and contrast dose without compromising image quality in routine clinical practice. • Reducing iodine delivery rate beyond direct proportionality to tube voltage is not beneficial.

Usefulness of the patient-specific contrast enhancement optimizer simulation software during the whole-body computed tomography angiography.

To evaluate whether the patient-specific contrast enhancement optimizer simulation software (p-COP) is useful for predicting contrast enhancement during whole-body computed tomography angiography (WBCTA). We randomly divided the patients into two groups using a random number table. We used the contrast material (CM) injection protocol selected by p-COP in group A (n = 52). The p-COP used an algorithm including data on the individual patient's cardiac output. Group B (n = 50) was assigned to the conventional CM injection protocol based on body weight. We compared the CT number in the abdominal aorta at the celiac artery level between the two groups and classified them as acceptable (> 280 HU) and unacceptable (< 279 HU) based on the optimal CT number for the WBCTA scans. To evaluate the difference in both injection protocols, we compared the visual inspection of the images of the artery of Adamkiewicz in both protocols. The CM dosage and injection rate in group A were significantly lower than those in group B (480.8 vs. 501.1mgI/kg and 3.1 vs. 3.3ml/s, p < 0.05). The CT number of the abdominal aorta at the celiac level was 382.4 ± 62.3 HU in group A and 363.8 ± 71.3 HU in group B (p = 0.23). CM dosage and injection rate were positively correlated to cardiac output for group A (r = 0.80, p < 0.05) and group B (r = 0.16, p < 0.05). The number of patients with an acceptable CT number was higher in group A [46/6 (86.7%)] than in group B [43/7 (71.4%)], but not significant (p = 0.71). The visualization rate for the Adamkiewicz artery was not significantly different between groups A and B (p = 0.89). The p-COP was useful for predicting contrast enhancement during WBCTA with a lower CM dosage and a lower contrast injection rate than that based on the body weight protocol. In patients with lower cardiac output a reduction in contrast injection rate and CM dosage did not lead to a reduced imaging quality, thus particularly in this group CM dosage can be reduced by p-COP.

Low kV Computed Tomography of Parenchymal Abdominal Organs-A Systematic Animal Study of Different Contrast Media Injection Protocols.

Objectives: To evaluate multiphase low kV computed tomography (CT) imaging of the abdomen with reduced contrast media (CM) dose using different injection protocols. Methods: Two injection protocols were evaluated for use with low kV (80 kV) multiphase abdominal imaging in comparison to the standard procedure acquired at 120 kV (500 mgI/kg; 5 mL/s). This evaluation was conducted in a highly standardized animal study (5 Goettingen minipigs). The low kV protocols consisted of (a) a single-flow (SF) injection with 40% reduced CM dose and injection rate (300 mgI/kg; 3 mL/s) and (b) a DualFlow (DF) injection protocol consisting of 60%/40% contrast to saline ratio administered at 5 mL/s. Dynamic CT was first performed within representative liver regions to determine optimal contrast phases, followed by evaluation of the three protocols in multiphase abdominal CT imaging. The evaluation criteria included contrast enhancement (CE) of abdominal organs and vasculature. Results: The 80 kV DF injection protocol showed similar CE of the abdominal parenchymatous organs and vessels to the 120 kV reference and the 80 kV SF protocol. Hepatic parenchyma showed comparable CT values for all contrast phases. In particular, in the portal venous parenchymal phase, the 80 kV DF protocol demonstrated higher hepatic parenchymal enhancement; however, results were statistically non-significant. Similarly, CE of the kidney, pancreas, and abdominal arterial/venous vessels showed no significant differences between injection protocols. Conclusions: Adapted SF and DF injection protocols with reduced IDR/iodine load offer the potential to calibrate optimal CM doses to the tube voltage in abdominal multiphase low kV CT imaging. The data suggest that the DF approach allows the use of predefined injection protocols and adaption of the contrast to saline ratio to an individualized kV setting and yields the potential for patient-individualized CM adaption.

A practical biphasic contrast media injection protocol strongly enhances the aorta and pulmonary artery simultaneously using a single CT angiography scan

BackgroundEnhancement profiles of the pulmonary artery (PA) and aorta differ when using computed tomography (CT) angiography. Our aim was to determine the optimal CT protocol for a one-time CT scan that assesses both blood vessels.MethodsWe prospectively enrolled 101 cases of CT angiography in patients with suspected pulmonary embolism or aortic dissection from our center between 2018 and 2020. We also retrospectively collected the data of 40 patients who underwent traditional two-time CT scans between 2015 and 2018. Patients were divided into four groups: test bolus (TB) I, TB II, bolus-tracking (BT) I, and BT II. The enhancement of the PA and aorta, and the radiation doses used in the four groups were collected. Those who underwent two-time scans were classified into the traditional PA or aorta scan groups. Data were compared between the BT and traditional groups.ResultsThe aortic enhancement was highest in BT II (294.78 ± 64.48 HU) followed BT I (285.18 ± 64.99 HU), TB II (186.58 ± 57.53 HU), and TB I (173.62 ± 69.70 HU). The radiation dose used was lowest in BT I (11.85 ± 5.55 mSv) and BT II (9.07 ± 3.44 mSv) compared with that used in the traditional groups (20.07 ± 7.78 mSv) and accounted for half of the traditional group (45.17–59.02%). The aortic enhancement was also highest in BT II (294.78 ± 64.48 HU) followed by BT I (285.18 ± 64.99 HU) when compared with that in the traditional aorta scan group (234.95 ± 94.18 HU).ConclusionOur CT protocol with a BT technique allows for a lower radiation dose and better image quality of the PA and aorta than those obtained using traditional CT scans.Trial registration: NCT04832633, retrospectively registered in April 2021 to the clinical trial registry.

Performance evaluation of using shorter contrast injection and 70 kVp with deep learning image reconstruction for reduced contrast medium dose and radiation dose in coronary CT angiography for children: a pilot study.

Iterative reconstruction algorithms are often used to reduce image noise in low-dose coronary computed tomography angiography (CCTA) but encounter limitations. The newly introduced deep learning image reconstruction (DLIR) algorithm may provide new opportunities. We assessed the image quality and diagnostic performance of DLIR in low radiation dose and contrast medium dose CCTA of pediatric patients with 70 kVp and a shortened injection protocol. This was a prospective study. A total of 27 consecutive arrhythmic pediatric patients were enrolled in the study group and underwent CCTA using a prospective ECG-triggered single-beat protocol: tube voltage 70 kVp, automatic tube current modulation for a noise index (NI) of 22, and contrast dose of 0.4-0.6 mL/kg. Images were reconstructed with DLIR. They were compared with 27 matched patients in the control group scanned with 80 kVp, a lower NI setting (NI =19), and a higher contrast dose (0.8-1.2 mL/kg). The images in the control group were reconstructed using the adaptive statistical iterative reconstruction (ASIR-V) algorithm. The image contrast, image quality, and diagnostic confidence were assessed by 2 experienced radiologists using a 5-point scale (1: nondiagnostic and 5: excellent). The CT value and standard deviation of the aorta and perivascular tissue were measured, and the contrast-to-noise ratio (CNR) for the aorta was calculated. The contrast medium and radiation doses were compared. The study and control groups had similar image contrast scores (4.75±0.57 vs. 4.78±0.42), image quality scores (3.67±0.47 vs. 3.44±0.51), and diagnostic confidence (4.74±0.44 vs. 4.74±0.45) (all P>0.05). There was an adequate enhancement in the aorta (614.74±127.73 vs. 705.89±111.20 HU) and similar CNR (20.34±4.64 vs. 20.99±4.14) in both groups. The image noise of the study group was lower in the aorta (30.61±3.88 vs. 34.77±3.49) and similar in perivascular tissue (27.66±6.24 vs. 27.55±3.33) compared with the control group. The study group reduced the total contrast medium dose by 53% to 15.07±3.68 mL and radiation dose by 36% to 0.57±0.31 mSv. The DLIR algorithm in CCTA for children using 70 kVp tube voltage with a shortened contrast medium injection protocol maintains image quality and diagnostic confidence while significantly reducing contrast medium dose and radiation dose compared with the use of the conventional CCTA protocol.

Influence of Contrast Material Temperature on Patient Comfort and Image Quality in Computed Tomography of the Abdomen: A Randomized Controlled Trial.

International guideline recommendations on safe use of contrast media (CM) are conflicting regarding the necessity to prewarm iodinated CM. Aim of the study was to evaluate the effects of room temperature CM compared with prewarmed CM on image quality, safety, and patient comfort in abdominal computed tomography (CT). CATCHY (Contrast Media Temperature and Patient Comfort in Computed Tomography of the Abdomen) is a double-blinded, randomized noninferiority trial. Between February and August 2020, 218 participants referred for portal venous abdominal CT were prospectively and randomly assigned to 1 of 2 groups. All patients received iopromide at 300 mg I/mL: group 1 at room temperature (~23°C [~73°F]) and group 2 prewarmed to body temperature (37°C [99°F]). A state-of-the-art individualized CM injection protocol was used, based on body weight and adapted to tube voltage. Primary outcome was absolute difference in mean liver attenuation between groups, calculated with a 2-sided 95% confidence interval. The noninferiority margin was set at -10 HU. Secondary outcomes were objective (signal-to-noise ratio and contrast-to-noise ratio) and subjective image quality; CM extravasations and other adverse events; and participant comfort (5-point scale questionnaire) and pain (numeric rating scale). This trial is registered with ClinicalTrials.gov (NCT04249479). The absolute difference in mean attenuation between groups was + 4.23 HU (95% confidence interval, +0.35 to +8.11; mean attenuation, 122.2 ± 13.1 HU in group 1, 118.0 ± 15.9 HU in group 2; P = 0.03). Signal-to-noise ratio, contrast-to-noise ratio, and subjective image quality were not significantly different between groups (P = 0.53, 0.23, and 0.99 respectively). Contrast extravasation occurred in 1 patient (group 2), and no other adverse events occurred. Comfort scores were significantly higher in group 1 than in group 2 (P = 0.03); pain did not significantly differ (perceived P > 0.99; intensity P = 0.20). Not prewarming iodinated CM was found noninferior in abdominal CT imaging. Prewarming conferred no beneficial effect on image quality, safety, and comfort, and might therefore no longer be considered a prerequisite in state-of-the art injection protocols for parenchymal imaging.

Variation in arterial input function in a large multicenter computed tomography perfusion study

ObjectivesTo report the variation in computed tomography perfusion (CTP) arterial input function (AIF) in a multicenter stroke study and to assess the impact this has on CTP results.MethodsCTP datasets from 14 different centers were included from the DUtch acute STroke (DUST) study. The AIF was taken as a direct measure to characterize contrast bolus injection. Statistical analysis was applied to evaluate differences in amplitude, area under the curve (AUC), bolus arrival time (BAT), and time to peak (TTP). To assess the clinical relevance of differences in AIF, CTP acquisitions were simulated with a realistic anthropomorphic digital phantom. Perfusion parameters were extracted by CTP analysis using commercial software (IntelliSpace Portal (ISP), version 10.1) as well as an in-house method based on block-circulant singular value decomposition (bSVD).ResultsA total of 1422 CTP datasets were included, ranging from 6 to 322 included patients per center. The measured values of the parameters used to characterize the AIF differed significantly with approximate interquartile ranges of 200–750 HU for the amplitude, 2500–10,000 HU·s for the AUC, 0–17 s for the BAT, and 10–26 s for the TTP. Mean infarct volumes of the phantom were significantly different between centers for both methods of perfusion analysis.ConclusionsAlthough guidelines for the acquisition protocol are often provided for centers participating in a multicenter study, contrast medium injection protocols still vary. Automated tube voltage selection and adapted contrast media injection protocols in CT angiography of the thoracoabdominal aorta

PurposeThe aim was to assess the image quality (IQ) in computed tomography angiography (CTA) of the thoracoabdominal aorta utilizing automated tube voltage selection (ATVS) with a subsequently adapted contrast media (CM) injection protocol.Materials and methodsA total of 104 consecutive patients referred for CTA of the thoracoabdominal aorta were included. Scans were acquired on a 3rd-generation DSCT using ATVS with a quality reference tube voltage and current of 100 kV and 150 mAs. CM protocols were adapted to kV settings by modifying iodine delivery rate (IDR) whilst maintaining an identical injection time (13.3 s): 0.9 gI/s (70 kV), 1.0 gI/s (80 kV), 1.1 gI/s (90 kV) 1.2 gI/s (100 kV). Both objective (attenuation, contrast-to-noise and signal-to-noise) and subjective (4-point Likert scale: 1 = poor/2 = sufficient/3 = good/4 = excellent) IQ were assessed.ResultsATVS assigned a 70 kV (n = 88) and 90 kV (n = 16) protocol in most patients. Fewer patients were assigned to an 80 kV (n = 4) and 100 kV (n = 1) protocol, these protocols were, therefore, excluded from further analysis. Attenuation on all designated levels of the thoracoabdominal aorta was diagnostic in 99.0% of the scans; 87/88 scans (98.9%) in the 70 kV group and in 16/16 (100%) in the 90 kV group. Overall mean attenuation was 349 ± 72HU for 70 kV and 310 ± 43HU for 90 kV. Subjective IQ was diagnostic in all scans. Overall effective radiation dose for 70 kV and 90 kV was 1.8 ± 0.2 mSv and 3.4 ± 0.7 mSv, respectively.ConclusionsAdaptation of CM injection protocols (IDR) to ATVS in CTA of the aorta—scanned with 70 kV and 90 kV—is feasible and results in diagnostic image quality.

Contrast media injection protocol for portovenous phase abdominal CT: does a fixed injection duration improve hepatic enhancement over a fixed injection rate?

To assess whether a fixed contrast media (CM) injection duration improves the magnitude and inter-patient variability in hepatic enhancement over a fixed injection rate. Outpatients who underwent portovenous phase abdominal CT (fixed duration, February-November 2018; fixed rate, January-July 2020) with 1.22mL/kg iohexol 350 were included. Subjects with liver, kidney or heart disease were excluded. The number of subjects and injection protocols were as follows: fixed duration arm, 56 women, 60 men, 35s injection duration; fixed rate arm, 66 women, 62 men, 3mL/s injection rate. Liver attenuation measurements were obtained from regions of interest on pre- and post-contrast images. Mean hepatic enhancement (MHE) and MHE normalized to iodine dose (MHE/I) were compared (unpaired t-tests and F-tests). There was no statistically significant difference in age, weight, body mass index or CM dosing (p > 0.05). Enhancement indices were significantly lower in the fixed rate group as compared to the fixed duration group, as follows: MHE, 50.0 ± 12 vs. 54.8 ± 11 HU (p = 0.001); and MHE/I, 1.53 ± 0.43 vs. 1.66 ± 0.51 HU/g, (p = 0.04). However, there was no significant difference in the variances of MHE (p = 0.51) and MHE/I (p = 0.08). A fixed CM injection duration yields a greater magnitude in hepatic enhancement indices than a fixed injection rate. Inter-patient variability in hepatic enhancement indices do not significantly differ between the two injection protocols.

Evaluation of a Tube Voltage-Tailored Contrast Medium Injection Protocol for Coronary CT Angiography: Results From the Prospective VOLCANIC Study.

OBJECTIVE. The purpose of this study was to prospectively evaluate, using software support, the feasibility and the quantitative and qualitative image quality parameters of a tube voltage-tailored contrast medium (CM) application protocol for patient-specific injection during coronary CT angiography (CCTA). SUBJECTS AND METHODS. In the Voltage-Based Contrast Media Adaptation in Coronary Computed Tomography Angiography (VOLCANIC-CTA) study, a single-center trial, 120 patients referred for CCTA were prospectively assigned to a tube voltage-tailored CM injection protocol. Automated tube voltage levels were selected in 10-kV intervals and ranged from 70 to 130 kV, and the iodine delivery rate (IDR) was adapted to the tube voltage level using dedicated software. The administered CM volume (370 mg I/mL) ranged from 33 mL at 70 kV (IDR, 0.7 g I/s) to 65 mL at 130 kV (IDR, 1.7 g I/s). Attenuation was measured in the aorta and coronary arteries to calculate quantitative signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR), and 5-point scales were used to evaluate overall image quality. Radiation metrics were also assessed and compared among the protocols. RESULTS. The mean age of the study patients was 62.5 ± 11.9 (SD) years. Image quality was rated as diagnostic in all patients. Contrast attenuation peaked at 70 kV (p < 0.001), whereas SNR and CNR parameters showed no significant differences between tube voltage levels (p ≥ 0.085). Additionally, no significant differences in subjective image quality parameters were found among the different protocols (p ≥ 0.139). The lowest radiation dose values were observed in the group assigned to the 70-kV protocol, which had a median radiation effective dose of 2.0 mSv (p < 0.001). CONCLUSION. The proposed tube voltage-tailored injection protocol allows individualized scanning of patients undergoing CCTA and significantly reduces CM and radiation dose while maintaining a high diagnostic image quality.

Dual-energy CT angiography in suspected pulmonary embolism: influence of injection protocols on image quality and perfused blood volume

To compare intravenous contrast material (CM) injection protocols for dual-energy CT pulmonary angiography (CTPA) in patients with suspected acute pulmonary embolism with regard to image quality and pulmonary perfused blood volume (PBV) values. A total of 198 studies performed with four CM injection protocols varying in CM volume and iodine delivery rates (IDR) were retrospectively included: (A) 60 ml at 5 ml/s (IDR = 1.75gI/s), (B) 50 ml at 5 ml/s (IDR = 1.75gI/s), (C) 50 ml at 4 ml/s (IDR = 1.40gI/s), (D) 40 ml at 3 ml/s (IDR = 1.05gI/s). Image quality and PBV values at different resolution settings were compared. Pulmonary arterial tract attenuation was highest for protocol A (397 ± 110 HU; p vs. B = 0.13; vs. C = 0.02; vs. D < 0.001). CTPA image quality of protocol A was rated superior compared to protocols B and D by reader 1 (p = 0.01; < 0.001), and superior to protocols B, C and D by reader 2 (p < 0.001; 0.02; < 0.001). Otherwise, there were no significant differences in CTPA quality ratings. Subjective iodine map ratings did not vary significantly between protocols A, B, and C. Both readers rated protocol D inferior to all other protocols (p < 0.05). PBV values did not vary significantly between protocols A and B at resolution settings of 1, 4 and 10 (p = 0.10; 0.10; 0.09), while otherwise PBV values displayed a decreasing trend from protocol A to D (p < 0.05). Higher CM volume and IDR are associated with superior CTPA and iodine map quality and higher absolute PBV values.
