Evaluation of the Auditory System in Autistic Children Using Evoked Otoacoustic Emissions and a Contralateral Suppression Test.

In this study, we evaluated the cochlea, medial olivocochlear system, and brainstem function in autistic children using evoked otoacoustic emissions (OAEs) and a noninvasive contralateral suppression (CLS) test. In total, we included 21 autistic children with normal hearing (study group) and 11 healthy children (control group). Transient-evoked OAEs (TEOAEs) and CLS of TEOAE were evaluated in the left and right ears of all patients. In a silent room, spontaneous, transient, and dP ILO292 were evaluated. The mean age of the study and control group was 9.1 years (range: 6-13 and 6-12 years, respectively). For the study group, there was no statistically significant difference between the OAE and CLS values of the right ear (P > .05). However, for the left ears, OAE values were statistically significantly higher than the CLS values (P < .05). In the control group, the OAE values of both ears were statistically significantly higher than the CLS values (P < .05). In autistic children with normal hearing, the medial olivocochlear system functions more effectively in the right ear than the left ear. Asymmetry between the ears islikely responsible for the peripheral auditory lateralization and independence in auditory function between the left and rightears.

Controlled (re)evaluation of the relationship between speech perception in noise and contralateral suppression of otoacoustic emissions

In people with normal hearing (NH), speech perception in noise (SPIN) improves when the speech signal is presented not gated with noise but after a delay. The medial olivocochlear reflex (MOCR) was thought to be involved in the neural dynamic range adaptation (NDRA) responsible for this adaptive SPIN; however, some of the recent studies do not support this hypothesis and suggest that adaptive SPIN involves the NDRA to noise-level statistics, irrespective of MOCR activation. A plausible reason for this discrepancy could be the variations and limitations of the experimental designs used in different studies. Using a relatively controlled and comprehensive study design, this study attempts to verify whether a delay between the delivery of speech and the noise improves the SPIN and whether MOCR mediates such effects. The SPIN was estimated by measuring speech reception thresholds (SRT) in noise under simultaneous-onset and delayed-onset (noise precedes speech onset by 300 ms) conditions. The SPIN in both ears was independently examined for ipsilateral, contralateral, and bilateral noise in women with normal hearing (N = 18; age range, 18–25 years). Contralateral suppression of transient-evoked otoacoustic emissions (CSOAEs) was used to estimate the MOCR based cochlear gain reduction. Under all test conditions, SPIN was improved in delayed-onset than in simultaneous-onset conditions, and the mean improvement in the SRT ranged from 0.7±1.7 to 1.8±1.8 dB. No significant correlation was obtained between CSOAEs and the mean temporal improvement in SRT, suggesting that MOCR may not be a predominant mechanism for the temporal improvement in SPIN.

The Influence of Efferent Inhibition on Speech Perception in Noise: A Revisit Through Its Level-Dependent Function.

Purpose The study aimed to assess the relationship between the level-dependent function of efferent inhibition and speech perception in noise across different intensities of suppressor and across different signal-to-noise ratios (SNRs) of speech. Method Twenty-six young normal-hearing adults participated in the study. Contralateral suppression of transient evoked otoacoustic emissions (TEOAEs) was measured for 3 levels of suppressor (40, 50, and 60 dB SPL). Speech identification score (SIS) was measured at 5 ipsilateral SNR conditions (quiet, 0, -5, -10, and -15 dB), with and without contralateral broadband noise at 3 levels (40, 50, and 60 dB SPL). Furthermore, SNR-50 was measured with and without the same 3 levels of contralateral broadband noise. Results The results showed that the suppression magnitude of TEOAE increased with an increase in suppressor level. However, neither SIS nor SNR-50 was influenced by the contralateral noise. In addition, SIS and SNR-50 did not show significant correlation with contralateral suppression of TEOAEs. This was true at all the SNRs and contralateral noise levels used in the study. Conclusions The findings suggest that the intensity of noise directly influences medial olivocochlear bundle-mediated efferent inhibition. Adversarial relationship between combined medial olivocochlear (MOC) and middle-ear-muscle (MEM) reflexes and alarm-in-noise detection thresholds under negative signal-to-noise ratios (SNRs)

The role of auditory efferent feedback from the medial olivocochlear system (MOCS) and the middle-ear-muscle (MEM) reflex in tonal detection tasks for humans in the presence of noise is not clearly understood. Past studies have yielded inconsistent results on the relationship between efferent feedback and tonal detection thresholds. This study attempts to address this inconsistency. Fifteen human subjects with normal hearing participated in an experiment where they were asked to identify an alarm signal in the presence of 80 dBA background (pink) noise. Masked detection thresholds were estimated using the method of two-interval forced choice (2IFC). Contralateral suppression of transient-evoked otoacoustic emissions (TEOAEs) was measured to estimate the strength of auditory efferent feedback. Subsequent correlation analysis revealed that the contralateral suppression of TEOAEs was significantly negatively correlated (r = −0.526, n = 15, p = 0.0438) with alarm-in-noise (AIN) detection thresholds under negative signal-to-noise conditions. The result implies that the stronger the auditory efferent feedback, the worse the detection thresholds and thus the poorer the tonal detection performance in the presence of loud noise.

Olivocochlear Efferent Activity Is Associated With the Slope of the Psychometric Function of Speech Recognition in Noise.

The medial olivocochlear (MOC) efferent system can modify cochlear function to improve sound detection in noise, but its role in speech perception in noise is unclear. The purpose of this study was to determine the association between MOC efferent activity and performance on two speech-in-noise tasks at two signal-to-noise ratios (SNRs). It was hypothesized that efferent activity would be more strongly correlated with performance at the more challenging SNR, relative to performance at the less challenging SNR. Sixteen adults aged 35 to 73 years participated. Subjects had pure-tone averages ≤25 dB HL and normal middle ear function. High-frequency pure-tone averages were computed across 3000 to 8000 Hz and ranged from 6.3 to 48.8 dB HL. Efferent activity was assessed using contralateral suppression of transient-evoked otoacoustic emissions (TEOAEs) measured in right ears, and MOC activation was achieved by presenting broadband noise to left ears. Contralateral suppression was expressed as the decibel change in TEOAE magnitude obtained with versus without the presence of the broadband noise. TEOAE responses were also examined for middle ear muscle reflex activation and synchronous spontaneous otoacoustic emissions (SSOAEs). Speech-in-noise perception was assessed using the closed-set coordinate response measure word recognition task and the open-set Institute of Electrical and Electronics Engineers sentence task. Speech and noise were presented to right ears at two SNRs. Performance on each task was scored as percent correct. Associations between contralateral suppression and speech-in-noise performance were quantified using partial rank correlational analyses, controlling for the variables age and high-frequency pure-tone average. One subject was excluded due to probable middle ear muscle reflex activation. Subjects showed a wide range of contralateral suppression values, consistent with previous reports. Three subjects with SSOAEs had similar contralateral suppression results as subjects without SSOAEs. The magnitude of contralateral suppression was not significantly correlated with speech-in-noise performance on either task at a single SNR (p > 0.05), contrary to hypothesis. However, contralateral suppression was significantly correlated with the slope of the psychometric function, computed as the difference between performance levels at the two SNRs divided by 3 (decibel difference between the 2 SNRs) for the coordinate response measure task (partial rs = 0.59; p = 0.04) and for the Institute of Electrical and Electronics Engineers task (partial rs = 0.60; p = 0.03). In a group of primarily older adults with normal hearing or mild hearing loss, olivocochlear efferent activity assessed using contralateral suppression of TEOAEs was not associated with speech-in-noise performance at a single SNR. However, auditory efferent activity appears to be associated with the slope of the psychometric function for both a word and sentence recognition task in noise. Results suggest that individuals with stronger MOC efferent activity tend to be more responsive to changes in SNR, where small increases in SNR result in better speech-in-noise performance relative to individuals with weaker MOC efferent activity. Additionally, the results suggest that the slope of the psychometric function may be a more useful metric than performance at a single SNR when examining the relationship between speech recognition in noise and MOC efferent activity.

Role of Active Listening and Listening Effort on Contralateral Suppression of Transient Evoked Otoacousic Emissions.

Background and ObjectivesThe present study aimed to investigate the effect of active listening and listening effort on the contralateral suppression of transient evoked otoacoustic emissions (CSTEOAEs).Subjects and MethodsTwenty eight young adults participated in the study. Transient evoked otoacoustic emissions (TEOAEs) were recorded using ‘linear’ clicks at 60 dB peSPL, in three contralateral noise conditions. In condition 1, TEOAEs were obtained in the presence of white noise in the contralateral ear. While, in condition 2, speech was embedded into white noise at +3, −3, and −9 dB signal-to-noise ratio (SNR) and delivered to the contralateral ear. The SNR was varied to investigate the effect of listening effort on the CSTEOAE. In condition 3, speech was played backwards and embedded into white noise at −3 dB SNR. The conditions 1 and 3 served as passive listening condition and the condition 2 served as active listening condition. In active listening condition, the participants categorized the words in to two groups (e.g., animal and vehicle).ResultsCSTEOAE was found to be largest in the presence of white noise, and the amount of CSTEOAE was not significantly different between active and passive listening conditions (condition 2 and 3). Listening effort had an effect on the CSTEOAE, the amount of suppression increased with listening effort, when SNR was decreased from +3 dB to −3 dB. However, when the SNR was further reduced to −9 dB, there was no further increase in the amount of CSTEOAE, instead there was a reduction in the amount of suppression.ConclusionsThe findings of the present study show that listening effort might affect CSTEOAE.

Contralateral Suppression of Transient Otoacoustic Emissions and Sentence Recognition in Noise in Young Adults

One purported role of the medial olivocochlear (MOC) efferent system is to reduce the effects of masking noise. MOC system functioning can be evaluated noninvasively in humans through contralateral suppression of otoacoustic emissions. It has been suggested that the strength of the MOC efferent activity should be positively associated with listening performance in noise. The objective of the study was to further explore this notion by examining contralateral suppression of transient evoked otoacoustic emissions (TEOAEs) and sentence recognition in two noises with normal hearing young adults. A repeated measures multivariate quasi-experimental design was employed. Thirty-two normal hearing young adult females participated. Reception thresholds for sentences (RTSs) were determined monaurally and binaurally in quiet and in competing continuous and interrupted noises. Both noises had an identical power spectrum and differed only in their temporal continuity. "Release from masking" was computed by subtracting RTS signal-to-noise ratios in interrupted from continuous noise. TEOAEs were evoked with 80 dB peSPL click stimuli. To examine contralateral suppression, TEOAEs were evaluated with 60 dB peSPL click stimuli with and without a contralateral 65 dB SPL white noise suppressor. A binaural advantage was observed for RTSs in quiet and noise (p < .0001) while there was no difference between ears (p >.05). In noise, performance was superior in the interrupted noise (i.e., RTSs were lower vs. continuous noise; p < .0001). There were no statistically significant differences in TEOAE levels between ears (p >.05). There was also no significant difference in the amount of suppression between ears (p = .41). There were no significant correlations or predictive linear relations between the amount of TEOAE suppression and any indices of sentence recognition in noise (i.e., RTS signal-to-noise ratios and release from masking; p > .05). The findings are not consistent with the notion that increased medial olivocochlear efferent feedback, as assessed via contralateral suppression of TEOAEs, is associated with improved speech perception in continuous and interrupted noise.

Contralateral Suppression of Transient-Evoked Otoacoustic Emissions in Children With Sickle Cell Disease

In previous studies, otoacoustic emissions (OAEs) have been found to be larger in normal-hearing children with sickle cell disease (SCD). It was hypothesized that some dysfunction or reduction in the medial olivocochlear efferent suppression of outer hair cell activity was responsible for this phenomenon. To test this hypothesis, contralateral suppression of transient-evoked otoacoustic emissions (TEOAEs) was examined in children with and without SCD. Thirteen African American school-aged normal-hearing children with homozygous SCD and 13 age- and gender-matched control children participated. TEOAEs were obtained bilaterally with 80 dB peSPL nonlinear click stimuli. To examine contralateral suppression, TEOAEs were obtained with 60 dB peSPL linear click stimuli with and without a contralateral 65 dB SPL white noise suppressor. Overall and half-octave band TEOAE levels were found to be larger in children with SCD relative to the normal control children (p < 0.05), consistent with previous reports of increased OAE levels. There was no significant difference (p > 0.05) in the absolute or proportional amount of TEOAE suppression as a function of group and ear. There were also no significant correlations or linear predictive relationships between TEOAE suppression and TEOAE level for either ear or group (p > 0.05). These findings do not support the notion that increased OAE levels in children with SCD are a consequence of abnormal medial olivocochlear system function as assessed with contralateral suppression of TEOAEs.

Contralateral Suppression of Transient Evoked Otoacoustic Emissions in Children with Auditory Processing Disorder

This study concerns contralateral white noise suppression of transient evoked otoacoustic emissions (TEOAEs) in children with auditory processing disorder (APD). Fifty-one children between 7 and 11 years were assigned to 1 of 3 experimental groups: those without auditory complaints (n = 15), those with APD who scored high on a standardized test (n = 20) and those with APD who scored lower on the same test (n = 16). For all groups TEOAE suppression was determined in both linear and nonlinear acquisition mode. The results provide evidence that abnormal TEOAE suppression was significantly more common in the APD groups than in the control group. Contralateral suppression of TEOAE is an additional tool for assessing the efferent pathway in children with APD.

The effect of noise bandwidth on the contralateral suppression of transient evoked otoacoustic emissions

The purpose of this study was to determine whether the bandwidth or loudness of a contralateral stimulus is the most important factor in evoking suppression of transient evoked otoacoustic emissions (TEOAEs). TEOAEs were measured in both ears of 10 women in quiet and in the presence of one of three contralateral noise bands; narrow band (NB), wide band (WB) and equalized (EQ), all centered at 2000 Hz. The NB (100 Hz bandwidth) and WB (2200 Hz bandwidth) noises were presented at 60 dB SPL. The SPL of the EQ (100 Hz bandwidth) noise was adjusted such that it was equal in loudness to the WB noise as determined using a psychoacoustic procedure. Only the WB noise was associated with a significant reduction of TEOAE levels. It is believed that this effect occurred because the WB noise has greater effective energy representation across frequency on the basilar membrane as it may receive more gain from the action of the cochlear amplifier. Results of the present study indicate that noise bandwidth is the most important factor in the contralateral suppression of TEOAEs.

Contralateral suppression of TEOAE in diabetic children. Effects of 1.0 kHz and 2.0 kHz pure tone stimulation--preliminary study.

The medial efferent system and its regulating outer hair cell function have not been previously studied in diabetic children. In this study, the group comprised 32 diabetic children, aged 6.0-16.0 years, with diabetes lasting 2.0-9.0 years, with normal tonal and impedance audiometry. A control group consisted of 30 healthy children with similar age and sex distribution. Contralateral stimulation (CS) was performed using 1.0 and 2.0 kHz pure tones on the level of 30 and 50 dB SL. Effects of CS on transient evoked otoacoustic emissions (TEOAE) elicited by click of a level equal to 70 and 60 dB SPL were investigated. Analysis included assessment of TEOAE amplitude and 0.8 kHz frequency bandwidth (0.8-FBW) amplitudes (signal/noise) centred at 1.0; 2.0; 3.0; 4.0; 5.0 kHz. TEOAE-RA recorded for stimulus 80, 70 and 60 dB SPL without CS were decreasing: average values respectively 7.3, 4.7 and 3.9 dB SPL. In the group of diabetic children TEOAE amplitudes, recorded for different click levels without CS, were similar to these recorded in healthy children. It suggested that normal function of the cochlea was preserved, mostly outer hair cells. However, the obtained effects of CS, in comparison with healthy children, were weaker and not so regular. Statistical analysis revealed that the reduction of TEOAE amplitudes for adequate 0.8-FBW in the control group was significantly higher, for both 1.0 kHz and 2.0 kHz CPTs of 30 dB SL and 50dB SL, in comparison with diabetic children. It is concluded that the suppressive effect on OAE in diabetic children is rather weak and seems to be associated with pathological changes in medial olivo-cochlear myelinated fibres.

Peripheral factors in loudness adaptation with OAE effects

Tone decay (TD), the ipsilateral comparison paradigm (ICP), and the monaural simple adaptation (SA) procedure assess loudness adaptation differently; this may all be peripheral but it has not been previously compared in one study. Transient evoked oto-acoustic emissions (TEOAE) have their source in the outer hair cells of the cochlear periphery. A significant correlation (r=−0.60) between contralateral suppression of transient evoked OAEs and tone decay adaptation (r=−0.60) has been reported [Collet et al., Audiology 31, 1–7 (1992)]. A significant correlation (r=−0.36) between ICP adaptation and contralateral suppression of TEOAEs has been found [Ernest M. Weiler, Hongwei Dou et al., J. Acoust. Soc. Am. 103, 3052 (1998)]. Comparison of TD, ICP, and SA with suppression of TEOAEs used repeated measures testing of 75 students (20–35 yrs) of mixed nationalities. Significant correlations were found, but not at all values, between TD, ICP, SA, and suppression of TEOAEs at 80, 60, and 70 dB. A principle components factor analysis found factor I based on suppression, and factor II based on monaural adaptation. It was concluded that the ICP, TD, and SA are primarily peripheral in origin. TD and ICP each include variation in stimulus intensity which may play a role in triggering perception of loudness adaptation.

Contralateral suppression of TEOAE. Attempts to find a latency.

Latency of the contralateral suppression of transient-evoked otoacoustic emissions (TEOAE) has been investigated using the ILO88 system for ipsilateral stimulation and recording, and a laboratory computer interface producing contralateral noise bursts. Reduction of rms amplitude in the last 10.24 ms of the response was found to produce the most consistent indication of suppression. Onset latencies varied from less than 40 ms to 140 ms, offset latency from 20 ms to more than 80 ms. Acoustic reflex latencies have not been reported with such short latencies and this makes it unlikely that this reflex can be responsible for the contralateral suppression of TEOAE. This study used contralateral stimuli at 70 dB HL. Lower level stimuli may result in increased amplitude and it is also possible that the effect of contralateral stimulation may be frequency specific. The long offset latency necessitates long intervals between stimuli with an inherent risk of variation in stimulus and recording conditions. This makes an elaborate study of the latency difficult to perform in humans.
