Translation of monosynaptic circuits underlying amygdala fMRI neurofeedback training.

fMRI neurofeedback using autobiographical memory recall to upregulate the amygdala is associated with resting-state functional connectivity (rsFC) changes between the amygdala and the salience and default mode networks (SN and DMN, respectively). We hypothesize the existence of anatomical circuits underlying these rsFC changes. Using a cross-species brain parcellation, we identified in non-human primates locations homologous to the regions of interest (ROIs) from studies showing pre-to-post-neurofeedback changes in rsFC with the left amygdala. We injected bidirectional tracers in the basolateral, lateral, and central amygdala nuclei of adult macaques and used bright- and dark-field microscopy to identify cells and axon terminals in each ROI (SN: anterior cingulate, ventrolateral, and insular cortices; DMN: temporal pole, middle frontal gyrus, angular gyrus, precuneus, posterior cingulate cortex, parahippocampal gyrus, hippocampus, and thalamus). We also performed additional injections in specific ROIs to validate the results following amygdala injections and delineate potential disynaptic pathways. Finally, we used high-resolution diffusion MRI data from four post-mortem macaque brains and one in vivo human brain to translate our findings to the neuroimaging domain. Different amygdala nuclei had significant monosynaptic connections with all the SN and DMN ipsilateral ROIs. Amygdala connections with the DMN contralateral ROIs are disynaptic through the hippocampus and parahippocampal gyrus. Diffusion MRI in both species benefitted from using the ground-truth tracer data to validate its findings, as we identified false-negative ipsilateral and false-positive contralateral connectivity results. This study provides the foundation for future causal investigations of amygdala neurofeedback modulation of the SN and DMN through these anatomic connections.

7T Magnetic Resonance Imaging Probabilistic Tractography-Based Evidence of Decussation of the Fibers Between the Lateral Geniculate Nucleus and the Primary Visual Area

Background and ObjectivesGeniculocalcarine fibers are thought to be exclusively ipsilateral. However, recent findings challenged this belief, revealing bilateral recruiting responses in occipito-temporo-parietal regions upon unilateral stimulation of the lateral geniculate nucleus (LGN) in humans. This raised the intriguing possibility of bilateral projections to primary visual areas (V1). This study sought to explore the hypothetical decussation of the geniculocalcarine tract. MethodsForty healthy individuals' 7T MR images from the Human Connectome Project were examined. Employing MRtrix3 software with the constrained spherical deconvolution algorithm, scans were processed. LGN served as the seed region, and contralateral regions of interest – ROIs (splenium of the corpus callosum – SCC, posterior commissure, LGN, V1, pulvinar, and superior colliculus) were defined to reconstruct the hypothetical decussated fibers. Tractography included contralateral V1 as the target region in all segmentations, excluding ipsilateral V1 to eliminate fibers leading to or originating from this area. Additionally, a segmentation of the tract originating from LGN and projecting to the ipsilateral V1 was performed. Mean fraction anisotropy and mean diffusivity metrics were extracted from the density maps. ResultsObservations revealed a substantial volume of decussated fibers between LGN and contralateral V1 via the SCC, albeit much smaller than ipsilateral fibers. The volume of ipsilateral fibers was similar in both sides. Left LGN-originating decussated fibers were more than double those originating from the right LGN. Tract segmentation to other ROIs yielded no fibers. ConclusionsThis study suggests a partial decussation of the fibers between LGN and V1, likely constituting the geniculocalcarine tract.

Characteristics of corticomuscular coupling during wheelchair Tai Chi in patients with spinal cord injury

BackgroundWheelchair Tai Chi (WCTC) has been proved to have benefits for the brain and motor system of spinal cord injury (SCI) patients. However, the characteristics of corticomuscular coupling during WCTC are scarcely known. We aimed to investigate changes following SCI on corticomuscular coupling, and further compare the coupling characteristics of WCTC with aerobic exercise in SCI patients.MethodsA total of 15 SCI patients and 25 healthy controls were recruited. The patients had to perform aerobic exercise and WCTC, while healthy controls needed to complete a set of WCTC. The participants accomplished the test following the tutorial video in a sitting position. The upper limb muscle activation was measured from upper trapezius, medial deltoid, biceps brachii and triceps brachii with surface electromyography. Cortical activity in the prefrontal cortex, premotor cortex, supplementary motor area and primary motor cortex was simultaneously collected by functional near-infrared spectroscopy. The functional connectivity, phase synchronization index and coherence values were then calculated and statistically analyzed.ResultsCompared to healthy controls, changes in functional connectivity and higher muscle activation were observed in the SCI group. There was no significant difference in phase synchronization between groups. Among patients, significantly higher coherence values between the left biceps brachii as well as the right triceps brachii and contralateral regions of interest were found during WCTC than during aerobic exercise.ConclusionThe patients may compensate for the lack of corticomuscular coupling by enhancing muscle activation. This study demonstrated the potential and advantages of WCTC in eliciting corticomuscular coupling, which may optimize rehabilitation following SCI.

Abstract WP106: Hemispheric Comparison Of Magnetic Susceptibility 24-72 Hours After Reperfusion

Introduction: Magnetic susceptibility is a radiological contrast mechanism that may provide a measure of oxygenation and contribute to an understanding the pathophysiology of stroke. Currently, there are few studies that have explored magnetic susceptibility in ischemic tissue as a marker of tissue injury. Our study assesses day one ischemic tissue post-reperfusion for changes in magnetic susceptibility and evaluates its relationship with radiological markers of ischemic injury - apparent diffusion coefficient (ADC). Methods: In a prospective observational study, thirty-five ischemic stroke patients who underwent endovascular thrombectomy were scanned with MRI (24 – 72 hours after reperfusion) to obtain multi-echo Gradient Echo and DWI images. An experienced neuroradiologist manually delineated ischemic tissue ROIs (region of interest) on DWI images. Contralateral ROIs were obtained using an automated method. The cerebrospinal fluid and regions of haemorrhagic transformation were excluded from the ROIs. A Morphology Enabled Dipole Inversion (MEDI) pipeline was employed to generate Quantitative Susceptibility Mapping (QSM) images, and these were used to quantify magnetic susceptibility. Mean magnetic susceptibility and mean ADC values were measured in these ROIs, and these values were then correlated. Results: Mean magnetic susceptibility of the ischemic ROI was significantly higher than that of the contralateral ROI (0.71±11.72 ppb, -5.59±7.30 ppb; p = 0.003). Mean ADC values of the ROI in the ischemic hemisphere were significantly lower than those in the contralateral hemisphere (0.65±0.09 х 10 -3 mm 2 /s, 0.81±0.05 х 10 -3 mm 2 /s; p = 1.31 х 10 -11 ). No significant correlation between susceptibility and ADC was found (ρ = -0.278, p = 0.106). Conclusion: Magnetic susceptibility in the ischemic tissue is measurable and is elevated when compared to the normal tissue. This may be attributed to the increased oxygen extraction fraction in the ischemic tissue. Further voxel wise analysis and larger longitudinal multi-parametric analysis correlating magnetic susceptibility with other imaging measures of tissue injury over time will help characterise the significance of lesional magnetic susceptibility changes in ischemic tissue.

Infrared Thermal Imaging and Artificial Neural Networks to Screen for Wrist Fractures in Pediatrics

Paediatric wrist fractures are commonly seen injuries at emergency departments. Around 50% of the X-rays taken to identify these injuries indicate no fracture. The aim of this study was to develop a model using infrared thermal imaging (IRTI) data and multilayer perceptron (MLP) neural networks as a screening tool to assist clinicians in deciding which patients require X-ray imaging to diagnose a fracture. Forty participants with wrist injury (19 with a fracture, 21 without, X-ray confirmed), mean age 10.50 years, were included. IRTI of both wrists was performed with the contralateral as reference. The injured wrist region of interest (ROI) was segmented and represented by the means of cells of 10 × 10 pixels. The fifty largest means were selected, the mean temperature of the contralateral ROI was subtracted, and they were expressed by their standard deviation, kurtosis, and interquartile range for MLP processing. Training and test files were created, consisting of randomly split 2/3 and 1/3 of the participants, respectively. To avoid bias of participant inclusion in the two files, the experiments were repeated 100 times, and the MLP outputs were averaged. The model’s sensitivity and specificity were 84.2% and 71.4%, respectively. Further work involves a larger sample size, adults, and other bone fractures.

Comparative prognostic utility of visual grade and heart-to-contralateral lung ratio in patients undergoing 99mTc-pyrophosphate radiotracer cardiac scintigraphy

Abstract Introduction Transthyretin (ATTR) cardiac amyloidosis (CA) is an underdiagnosed form of restrictive cardiomyopathy leading to a rapid progression into heart failure. 99mTc-labeled radiotracer cardiac scintigraphy is crucial for the evaluation of CA. Semiquantitative (Perugini visual grade) and quantitative (heart-to-contralateral lung ratio, H/CL) parameters are used to assessing CA via cardiac scintigraphy. Purpose We aimed to assess the comparative prognostic utility of the Perugini visual grade and heart to contralateral lung ratio. Methods The study population was identified based on an institutional registry of consecutive patients undergoing 99mTc-PYP radiotracer cardiac scintigraphy for suspected CA between January 2020-October 2021. The H/CL is calculated by the fraction of heart region of interest (ROI) mean uptake counts to the contralateral chest ROI. The visual scoring system compares uptake between bone (rib) and heart where 0 = absent cardiac uptake, 1 = uptake less than bone, 2 = uptake equal to bone, and 3 = is uptake greater than bone. Ejection fraction (EF) was obtained from echocardiographic studies done around the time of cardiac scintigraphy. MACE was defined as a composite of inclusive of all-cause death, myocardial infarction, admission for heart failure and late revascularization – percutaneous coronary intervention (PCI) or coronary artery bypass graft (CABG) >90 days after imaging. Results A total of 330 patients (mean age 70.9±12.2 years, 62.2% male) were included in the analysis. Risk factors were prevalent (59.7% hypertension, Heart Failure 68.3%, 26.5% dyslipidemia, 26.2% diabetes) (Table 1). In total, 32.6% of the cohort had studies suggestive of ATTR CA. Median time between echocardiography and cardiac scintigraphy was 7 days (IQR 5–42 days). A higher frequency of MACE was reported in patients with a 2+ visual grade (30% vs 42%, p=0.03). A H/CL >1.5 was associated with borderline significance (43% vs 35% p=0.07). Conclusion Our analysis showed that the Perugini visual grading of transthyretin cardiac amyloidosis conferred the best prognostic utility contrary to heart to contralateral lung ratio in 99mTc-PYP radiotracer cardiac scintigraphy. Assessment of Irreversible Tissue Injury in Extensive Ischemic Stroke-Potential of Quantitative Cerebral Perfusion.

Computed tomography perfusion (CTP) is used as a tool to select ischemic stroke patients for endovascular treatment (EVT) and is currently investigated in the setting of extensive stroke with low Alberta Stroke Program Early CT scores (ASPECTS). The purpose of this study was to perform a comprehensive quantitative analysis of cerebral blood flow within the ischemic lesion compared to threshold-derived core lesion volumes. We hypothesized that the degree of cerebral blood volume (CBV) reduction within the ischemic lesion is predictive of irreversible tissue injury and functional outcome in patients with low ASPECTS. Ischemic stroke patients with an ASPECTS ≤ 5 who received multimodal CT on admission and underwent thrombectomy were analyzed. The ischemic lesion on CTP was identified, and CTP-derived parameters were measured as absolute means within the lesion and relative to the physiological perfusion measured in a contralateral region of interest. The degree of irreversible tissue injury was assessed using quantitative net water uptake (NWU). Functional endpoint was good outcome defined as modified Rankin Scale (mRS) scores 0-3 at day 90. One hundred eleven patients were included. The median core lesion volume was 71ml (IQR: 25-107), and the median quantitative NWU was 9.5% (IQR: 6-13). Relative CBV (rCBV) reduction and ASPECTS at baseline were independently associated with NWU in multivariable linear regression analysis (ß: 12.4, 95%CI: 6.0-18.9, p < 0.0001) and (ß: - 0.78, 95% CI: - 1.53 to - 0.02; p = 0.045), respectively. Furthermore, rCBV was significantly associated with good outcome in patients with core volumes > 50ml (OR: 0.16, 95% CI: 0.05-0.49, p = 0.001). Our study shows that rCBV reduction serves as an early surrogate for increase of NWU as a marker of irreversible tissue injury and lesion progression. Thus, the analysis of rCBV reduction within ischemic lesions may add another dimension to acute stroke triage in addition to core volumes or ASPECTS as indicators of the infarct extent and viability.

Glutamate-Weighted Magnetic Resonance Imaging (GluCEST) Detects Effects of Transcranial Magnetic Stimulation to the Motor Cortex

Transcranial magnetic stimulation (TMS) is used in several FDA-approved treatments and, increasingly, to treat neurological disorders in off-label uses. However, the mechanism by which TMS causes physiological change is unclear, as are the origins of response variability in the general population. Ideally, objective in vivo biomarkers could shed light on these unknowns and eventually inform personalized interventions. Continuous theta-burst stimulation (cTBS) is a form of TMS observed to reduce motor evoked potentials (MEPs) for 60 min or longer post-stimulation, although the consistency of this effect and its mechanism continue to be under debate. Here, we use glutamate-weighted chemical exchange saturation transfer (gluCEST) magnetic resonance imaging (MRI) at ultra-high magnetic field (7T) to measure changes in glutamate concentration at the site of cTBS. We find that the gluCEST signal in the ipsilateral hemisphere of the brain generally decreases in response to cTBS, whereas consistent changes were not detected in the contralateral region of interest (ROI) or in subjects receiving sham stimulation.

Distinguishing tumor recurrence from radionecrosis in brain metastases after stereotactic radiosurgery using hybrid FDG-PET and MRI coregistered images

Objectives Dual phase 18 FDG-PET has been proven helpful to assess FDG-avid cerebral tumors as tracer will build up in metastases or tumor recurrences while its retention remains stable within normal tissue or inflammatory process. This is useful when MRI can’t discriminate brain tumor recurrence (TR) from radionecrosis (RN) after stereotaxic radiosurgery (SRS) for brain metastases (BM). Many studies sought to improve diagnostic performance by associating FDG-PET and MRI with interesting results but many bias, mostly within image post-processing. Coregistered MRI and dual phase FDG-PET images could alleviate these biases and be used to extract prognostic biomarkers. Methods We retrospectively evaluated patients treated with SRS for BM which developed a contrast-enhanced MRI lesion with non-conclusive diagnosis for TR or RN. All patients underwent MRI and FDG-PET at least 3 months after their last SRS session. Dual FDG-PET comprised a one hour “based image” (H1) followed by a 4 hours “delayed image” (H4). MRI included contrast enhanced T1 (T1), FLAIR, ASL (BAT,CBF), T1 Perfusion (ktrans, SER) and T2* Permeability (k2, rBF, rBV). PET and MRI data were all coregistered on the T1 images. Semi-automated Regions of Interest (ROI) of the tumor were drawn on the T1 MRI, H1 and H4 FDG-PET images; a reference contralateral white-matter ROI (WM) was drawn for standardization. Imaging metrics were then evaluated for their association with TR or RN based on histological, radiological and clinical criteria after at least 6 months follow-up. Results Eleven patients matched the inclusion criteria; 7 with TR and 4 with RN; no additional treatment was introduced. On visual analysis, FDG-PET and MRI respectively obtained 100% and 83% Sensitivity and 100% and 80% Specificity. The two MRI-false negative diagnoses were subsequently identified on follow-up (1 patient, RN) and histology (1 patient, TR). When standardized on WM, PET SUVmax values were significantly different across groups at H1 (P Conclusion Coregistered PET-MRI images accurately discriminates between TR and RN. With FDG being the most commonly used PET radiotracer, this protocol remains easily transposable and should be encouraged to obtain non-invasive prognostic and clinically relevant biomarkers.

Microwave thermal ablation using CT-scanner for predicting the variation of ablated region over time: advantages and limitations

This study aims at investigating in real-time the structural and dynamical changes occurring in an ex vivo tissue during a microwave thermal ablation (MTA) procedure. The experimental set-up was based on ex vivo liver tissue inserted in a dedicated box, in which 3 fibre-optic (FO) temperature probes were introduced to measure the temperature increase over time. Computed tomography (CT) imaging technique was exploited to experimentally study in real-time the Hounsfield Units (HU) modification occurring during MTA. The collected image data were processed with a dedicated MATLAB tool, developed to analyse the FO positions and HU modifications from the CT images acquired over time before and during the ablation procedures. The radial position of a FO temperature probe (rFO) and the value of HU in the region of interest (ROI) containing the probe (HUo), along with the corresponding value of HU in the contralateral ROI with respect to the MTA antenna applicator (HUc), were determined and registered over time during and after the MTA procedure. Six experiments were conducted to confirm results. The correlation between temperature and the above listed predictors was investigated using univariate and multivariate analysis. At the multivariate analysis, the time, rFO and HUc resulted significant predictive factors of the logarithm of measured temperature. The correlation between predicted and measured temperatures was 0.934 (p < 0.001). The developed tool allows identifying and registering the image-based parameters useful for predicting the temperature variation over time in each investigated voxel by taking into consideration the HU variation.

Diagnostic accuracy of automatic normalization of CBV in glioma grading using T1- weighted DCE-MRI

PurposeAim of this retrospective study was to compare diagnostic accuracy of proposed automatic normalization method to quantify the relative cerebral blood volume (rCBV) with existing contra-lateral region of interest (ROI) based CBV normalization method for glioma grading using T1-weighted dynamic contrast enhanced MRI (DCE-MRI). Material and methodsSixty patients with histologically confirmed gliomas were included in this study retrospectively. CBV maps were generated using T1-weighted DCE-MRI and are normalized by contralateral ROI based method (rCBV_contra), unaffected white matter (rCBV_WM) and unaffected gray matter (rCBV_GM), the latter two of these were generated automatically. An expert radiologist with >10years of experience in DCE-MRI and a non-expert with one year experience were used independently to measure rCBVs. Cutoff values for glioma grading were decided from ROC analysis. Agreement of histology with rCBV_WM, rCBV_GM and rCBV_contra respectively was studied using Kappa statistics and intra-class correlation coefficient (ICC). ResultThe diagnostic accuracy of glioma grading using the measured rCBV_contra by expert radiologist was found to be high (sensitivity=1.00, specificity=0.96, p<0.001) compared to the non-expert user (sensitivity=0.65, specificity=0.78, p<0.001). On the other hand, both the expert and non-expert user showed similar diagnostic accuracy for automatic rCBV_WM (sensitivity=0.89, specificity=0.87, p=0.001) and rCBV_GM (sensitivity=0.81, specificity=0.78, p=0.001) measures. Further, it was also observed that, contralateral based method by expert user showed highest agreement with histological grading of tumor (kappa=0.96, agreement 98.33%, p<0.001), however; automatic normalization method showed same percentage of agreement for both expert and non-expert user. rCBV_WM showed an agreement of 88.33% (kappa=0.76,p<0.001) with histopathological grading. ConclusionIt was inferred from this study that, in the absence of expert user, automated normalization of CBV using the proposed method could provide better diagnostic accuracy compared to the manual contralateral based approach.

A longitudinal study of the mechanical properties of injured brain tissue in a mouse model

Mechanical properties of brain tissue are crucial to understand the mechanism of traumatic brain injury (TBI). Over the past several decades, most of the studies focused on healthy brain tissues, while few of them are about the injured tissues. Therefore, limited knowledge is known about the mechanical properties of the injured brain tissues. In this study, we used an in vivo mouse model with a weight drop device to study injured brain tissues. Around the injury site, mechanical properties of the injured, neighboring, and the corresponding contralateral regions of interest (ROIs) were measured over five temporal points by indentation. Longitudinal and regional comparisons of the mechanical properties revealed that the ROI of the injured tissue had a higher elastic modulus than the contralateral counterpart one-hour post-injury. However, the elastic modulus decreased one-day post-injury and recovered to be close to the contralateral ROI in 7 days. The elastic modulus curves of the injured and the contralateral counterpart ROIs crossed at time points of 12h and 1 day post-injury, where two significant increases of glial fibrillary acidic protein (GFAP) positive cells were observed. Biological staining results indicated that both the astrocytic responses and the morphological structure could affect the mechanical properties of the injured tissue. The observed longitudinal changes of the mechanical properties at the tissue level and the morphological and biological changes at the cellular level provide insights into understanding the mechanism of TBI. Results are also meaningful for applying emerging in vivo diagnostic tools such as magnetic resonance elastography (MRE) in TBI detection.

Abstract WP40: Quick CT Perfusion Evaluation of Leptomeningeal Collateral Circulation: Single Cortical CBV-ROI

Good collateral circulation (CC) is a strong outcome predictor in acute stroke patients. CT angiography (CTA) is wide-world available but does not provide accurate information about parenchymal status. CT perfusion (CTP) is frequently used to determine ischemic core and tissue at risk. Our aim was to identify an easy and quick method to evaluate CC status by CTP. Methods: Consecutive ischemic stroke patients &lt;8h from symptoms onset evaluated for reperfusion therapies were studied. Non-contrast CT, CTP and multiphase CTA were performed. Patients with confirmed M1-MCA or TICA occlusion on CTA were included. CC evaluation was determined by multiphase CTA (mCTA) according to the Calgary CC Scale and classified as poor (grades 0-2) or good (grades 3-5). In CTP maps, one single ipsi- and contralateral regions of interest (ROI) were defined in the MCA cortical territory (M4, M5, M6). We studied the association of absolute and relative to contralateral ROI-CTP values with CC degree determined by mCTA. Results: 33 patients were included, median NIHSS 17.5 (2-22). Twenty-five patients (75.8%) presented a M1 and 8 (24.2%) a TICA occlusion. On mCTA, 27 (81.8%) patients presented with a favourable CC status and 6 (18.2%) with poor CC. Mean ROI values in the ischemic MCA territory were: CBV 3.5±1.5 ml/100mg, CBF 46.9±29.3 ml/100mg/min, MTT 8.1±3.1 s, Tmax 23.2±4.4 s. In the contralateral non-ischemic MCA, the mean ROI values were: CBV 3.48±1.4, CBF 66.5±32.7, MTT 5.6±2.3, Tmax 20.4±4.8. Absolute and relative CBV-ROI data (relCBV= ischemic CBV value / contralateral CBV value) were the only values significantly associated with CC status on mCTA (good CC mean CBV: 3.8 ml/100g VS poor CC mean CBV: 1.9, p=0.006; good CC mean relCBV 1.1 vs poor CC mean relCBV 0.6, p=0.019). A ROC curve defined 2.5 ml/100mg as the better cut-off point of ROI-CBV that identified patients with good CC status (sensitivity 96%, specificity 84%, VPP 0.96, VPN 0.83). Patients with a ROI-CBV &gt;2.5 presented lower median NIHSS after 24 hours (4 vs 18, p= 0.012) and smaller mean infarct volume on control CT (27.9 vs 88.3, p=0.021). Conclusion: A single cortical ROI-CBV allows an easy and quick accurate evaluation of collateral circulation in CTP. ROI-CBV&gt;2.5 ml/100mg is related to good clinical and radiological outcomes.

Arterial spin labeling: Pathologically proven superiority over conventional MRI for detection of high-grade glioma progression after treatment.

Standard of care for high-grade gliomas (HGGs) includes surgical debulking and adjuvant chemotherapy and radiation. Patients under treatment require frequent clinical and imaging monitoring for therapy modulation. Differentiating tumor progression from treatment-related changes can be challenging on conventional MRI, resulting in delayed recognition of tumor progression. Arterial spin labeling (ASL) may be more sensitive for tumor progression. ASL and associated conventional MR images obtained in patients previously treated for HGGs and before biopsy or re-resection were reviewed by three neuroradiologists who were blinded to the histopathologic results. Regions of interest (ROIs) of greatest perfusion were chosen by consensus, and mirror-image contralateral ROIs were also placed. Sensitivity of ASL for tumor progression was compared with those of contrast-enhanced T1-weighted (T1W-CE) and fluid-attenuated inversion recovery (FLAIR) images using McNemar's test. We tested for an association between cerebral blood flow (CBF) and apparent diffusion correlation (ADC) using a Hotelling-Lawley trace. Finally, we used a Pearson's analysis to test for a correlation between CBF and percentage of tumor within each resection. Twenty-two patients were studied. ASL demonstrated hyperperfusion in all cases with mean CBF ratio 3.37 (±1.71). T1W-CE and FLAIR images were positive in 15 (p = 0.0233) and 16 (p = 0.0412) cases, respectively. There was no association between ADC and CBF (p = 0.687). There was a correlation between CBF and percentage of tumor (p = 0.048). ASL may be more sensitive than conventional MR sequences for the detection of tumor progression in patients treated for HGGs.

Radiologic-pathologic analysis of quantitative 3D tumour enhancement on contrast-enhanced MR imaging: a study of ROI placement.

To investigate the influence of region-of-interest (ROI) placement on 3D tumour enhancement [Quantitative European Association for the Study of the Liver (qEASL)] in hepatocellular carcinoma (HCC) patients treated with transcatheter arterial chemoembolization (TACE). Phase 1: 40 HCC patients had nine ROIs placed by one reader using systematic techniques (3 ipsilateral to the lesion, 3 contralateral to the lesion, and 3 dispersed throughout the liver) and qEASL variance was measured. Intra-class correlations were computed. Phase 2: 15 HCC patients with histosegmentation were selected. Six ROIs were systematically placed by AC (3 ROIs ipsilateral and 3 ROIs contralateral to the lesion). Three ROIs were placed by 2 radiologists. qEASL values were compared to histopathology by Pearson's correlation, linear regression, and median difference. Phase 1: The dispersed method (abandoned in phase 2) had low consistency and high variance. Phase 2: qEASL correlated strongly with pathology in systematic methods [Pearson's correlation coefficient = 0.886 (ipsilateral) and 0.727 (contralateral)] and in clinical methods (0.625 and 0.879). However, ipsilateral placement matched best with pathology (median difference: 5.4 %; correlation: 0.89; regression CI: [0.904, 0.1409]). qEASL is a robust method with comparable values among tested placements. Ipsilateral placement showed high consistency and better pathological correlation. Ipsilateral and contralateral ROI placement produces high consistency and low variance. Both ROI placement methods produce qEASL values that correlate well with histopathology. Ipsilateral ROI placement produces best correlation to pathology along with high consistency.

Cerebral glucose metabolism changes in rat brain upon forepaw electrical stimulation at different frequencies

PET imaging techniques and statistical parametric mapping analysis have been developed to identify neuronal functional activation from brain imaging. The purpose of this study was to examine the efficacy of the glucose metabolism in rat brain during forepaw electrical stimulation with different frequencies (3, 10, or 20 Hz) compared with a nonstimulated group (control). Fluorine-18 fluorodeoxyglucose was injected after confirmation of the range of normal physiology. For quantitative analysis, we used cerebral metabolism rate of glucose consumption (CMRglc) responses in the primary somatosensory cortex (S1FL) and motor cortex (M1). On comparing CMRglc responses in the contralateral S1FL and M1 with those of the ipsilateral areas in intragroup analysis, a significant increase (P<0.05) was observed in two electrical stimulation groups (10 and 20 Hz) but not at the 3 Hz level. In intergroup analysis, the CMRglc responses in the contralateral region of interest were compared with those of the control group to validate which electrical frequency conditions were appropriate to induce neuronal functional activation. Among the stimulation groups, significant increases in CMRglc response were only observed at 10 Hz (P<0.05). Therefore, 10 Hz is the most suitable frequency to confirm changes in CMRglc in the S1FL and M1 of the rat brain, and also fluorine-18 fluorodeoxyglucose PET could be useful to investigate recovery and plasticity in neurological diseases associated with primary sensory-motor cortex function.

Calgary Stroke Program, Calgary, AB Background : For ischemic stroke, CT Perfusion (CTP) may change thrombolytic/endovascular treatment decisions when compared to NCCT/CTA alone. Two distinct thresholds may be useful in this regard: separation of 1) non-salvageable tissue (infarct core) from salvageable electrically silent tissue (penumbra), and 2) penumbra from benign oligemia (hypoperfused, but will survive if clot persists). Methods : CTP (120s,8cm) was performed on 180 patients within 12hrs of ischemic stroke. Two patient cohorts were analyzed: (1) recanalization (TICI 2b,3) 6s. For group (2) CBF, CBV, and Tmax values were obtained from within the final infarct region, and total ipsilateral hemisphere, excluding infarction. CBF, CBV and Tmax were used in univariate regression models. Results : For group (1)[n=11], mean time from CTP to recanalization was 60±19min. CBF parameter (thresholds for gray and white matter 7.2 and 5.2 ml•min-1•(100g)-1) had the highest sensitivity (90.9%) and specificity (81.8%) for infarction. For group (2)[n=15], the Tmax parameter (thresholds for gray and white matter 11.3s and 11.8s) had the highest sensitivity (86.6%) and specificity (80.0%) for penumbra. INTRODUCTION: The translocator protein (TSPO) is a 18 kDa mitochondrial molecule associated with neuroinflammation and over-expressed in several brain diseases. 11C-(R)PK11195 is a specific ligand of TSPO for PET studies. We investigated the in vivo TSPO expression in high- and low-grade gliomas using 11C-(R)PK11195 PET, and applied it in guiding tumour biopsies. METHOD: 24 patients underwent volumetric MRI and dynamic 11C-(R)PK11195 PET scans. Reference tissue input function was obtained from the grey matter of cerebellum to determine the 11C-(R)PK11195 uptake in the tumours and the mirrored regions of interest (ROIs) in the contralateral hemispheres. Co-registered MR/PET images were used to guide tumour biopsies prior to surgical debulking, with high and/or low 11C-(R)PK11195 uptake foci defined as biopsy targets. Biopsy specimens were assessed for TSPO expression by immunohistochemical staining. RESULTS: In low-grade gliomas (14 cases), 11C-(R)PK11195 uptake within the tumours was lower than the contralateral ROIs (p = 0.001); sporadic high uptake foci were found in 8 tumours. In high-grade gliomas, 7 out of the 10 tumours showed higher uptake than the contralateral ROIs (p = 0.016), 4 of which displayed intensive 11C-(R)PK11195 signal despite only minor contrast enhancement on the MRI. There was low 11C-(R)PK11195 uptake and no contrast enhancement in 3 anaplastic asctrocytomas. Immunostains on tissue sections confirmed variable TSPO expression in neoplastic cells and activated microglia/macrophages. CONCLUSION: TSPO expression is decreased in low-grade gliomas and increased in most high-grade gliomas. TSPO expression within gliomas is inhomogeneous as detected by 11C-(R)PK11195 PET and confirmed by immunohistochemistry.

Investigation of fMRI neurofeedback of differential primary motor cortex activity using kinesthetic motor imagery

Functional MRI neurofeedback (fMRI NF) is an emerging technique that trains subjects to regulate their brain activity while they manipulate sensory stimulus representations of fMRI signals in "real-time". Here we report an fMRI NF study of brain activity associated with kinesthetic motor imagery (kMI), analyzed using partial least squares (PLS), a multivariate analysis technique. Thirteen healthy young adult subjects performed kMI involving each hand separately, with NF training targeting regions of interest (ROIs) in the left and right primary motor cortex (M1). Throughout, subjects attempted to maximize a laterality index (LI) of brain activity-the difference in activity between the contralateral ROI (relative to the hand involved in kMI) and the ipsilateral M1 ROI-while receiving real-time updates on a visual display. Six of 13 subjects were successful in increasing the LI value, whereas the other 7 were not successful and performed similarly to 5 control subjects who received sham NF training. Ability to suppress activity in the ipsilateral M1 ROI was the primary driver of successful NF performance. Multiple PLS analyses depicted activated networks of brain regions involved with imagery, self-awareness, and feedback processing, and additionally showed that activation of the task positive network was correlated with task performance. These results indicate that fMRI NF of kMI is capable of modulating brain activity in primary motor regions in a subset of the population. In the future, such methods may be useful in the development of NF training methods for enhancing motor rehabilitation following stroke.
