A Clinically Viable Medial Olivocochlear Reflex Assay Using Transient-Evoked Otoacoustic Emissions.

The contralateral medial olivocochlear reflex (MOCR) strength may indicate various auditory conditions in humans, but a clinically viable assay and equipment are needed for quick, accurate, and reliable measurements. The first experiment compared an earlier version of the assay, which used a nonlinear-mode chirp stimulus, with a new assay using a linear-mode click stimulus, designed to give reliable MOCR measurements in most normal-hearing ears. The second experiment extended the improved assay on a purpose-built binaural hardware platform that used forward-pressure level (FPL) calibration for both the stimulus and the contralateral MOCR elicitor. Transient-evoked otoacoustic emission (TEOAE) tests were measured with and without a 60-dB SPL MOCR-evoking contralateral broadband noise. The normalized MOCR strength (MOCR%) was derived from the TEOAE responses for each trial pair using the complex pressure difference weighted by the TEOAE magnitude. Experiment 1 compared MOCR% within-subject and across-day using two TEOAE stimuli: nonlinear-mode chirps (50 dB SPL, bandpass 1-5 kHz, 14 ms window delayed by 2 ms) and linear-mode clicks (50 dB SPL, bandpass 0.5-2.5 kHz, 13 ms window delayed by 5 ms). TEOAE responses were analyzed in the 0.5 to 2.5 kHz band. Thirty adult participants with normal hearing (30 ears) completed the study. The TEOAE stimulus was calibrated in situ using spectral flattening, and the contralateral noise was calibrated in a coupler. Twelve TEOAE trial pairs were collected for each participant and condition. Experiment 2 used a purpose-built binaural system. The TEOAE stimuli were linear-mode clicks (50 dB SPL, bandpass 1-3 kHz, 13 ms window delayed by 5 ms), analyzed in the 1 to 3 kHz band over ~12 trial pairs. After a probe refit, an additional trial pair was collected for the two early-stopping signal-to-noise ratio criteria (15 and 20 dB). They were evaluated for single-trial reliability and test time. Nineteen adult participants with normal hearing (38 ears) completed the study. The TEOAE clicks and contralateral elicitor noise were calibrated in situ using FPL and delivered with automated timing. MOCR% for linear-mode clicks was distinguishable from measurement variability in 98% to 100% of participants' ears (both experiments), compared with only 73% for the nonlinear-mode chirp (experiment 1). MOCR detectability was assessed using the MOCR% across-subject/within-subject variance ratio. The ratio in experiment 1 for linear-mode clicks was higher (8.0) than for nonlinear-mode chirps (6.4). The ratio for linear-mode clicks (8.9) in experiment 2 was slightly higher than for the comparable linear-mode stimulus (8.0) in experiment 1. TEOAEs showed excellent reliability with high signal-to-noise ratios in both experiments, but reliability was higher for linear-mode clicks than nonlinear-mode chirps. MOCR reliability for the two stimuli was comparable. The FPL pressure response retest reliability derived from the SPL at the microphone was higher than the SPL retest reliability across 0.4 to 8 kHz. Stable results required 2 to 3 trial pairs for the linear-mode click (experiments 1 and 2) and three for the nonlinear-mode chirp (experiment 1), taking around 2 min on average. The linear-mode click assay produced measurable, reliable, and stable TEOAE and MOCR results on both hardware platforms in around 2 min per ear. The stimulus design and response window ensured that any stimulus artifact in linear mode was unlikely to confound the results. The refined assay is ready to produce high-quality data quickly for clinical and field studies to develop population norms, recognize diagnostic patterns, and determine risk profiles.

Middle ear muscle and medial olivocochlear activity inferred from individual human ears via cochlear potentials.

The peripheral auditory system is influenced by the medial olivocochlear (MOC) and middle ear muscle (MEM) reflexes. When elicited by contralateral acoustic stimulation (CAS), these reflexes reduce cochlear amplification (MOC reflex) and limit low-frequency transmission through the middle ear (MEM reflex). The independent roles of these reflexes on auditory physiology and perception are difficult to distinguish. The amplitude of the cochlear microphonic (CM) is expected to increase or decrease when the MOC and MEM reflexes are elicited by CAS, respectively, which could lead to a straightforward interpretation of what reflex is dominant for a given CAS level. CM and ear canal sound pressure level (SPL) were measured for a 500 Hz, 90 dB SPL probe in the presence of contralateral broadband noise (CBBN) for levels ranging from 45-75 dB SPL. In most subjects, CM amplitude increased for CBBN levels of 45 and 55 dB SPL, while no change in ear canal SPL was observed, consistent with eliciting the MOC reflex. Conversely, CM amplitude decreased, and ear canal SPL increased in the presence of 65 and 75 dB SPL CBBN, consistent with eliciting the MEM reflex. A CM-based test of the MOC reflex may facilitate detection of MEM effects and the assessment of adults with cochlear hearing loss.

Contralateral noise effects on otoacoustic emissions and electrophysiologic responses in normal-hearing adults.

Contralateral noise inhibits the amplitudes of cochlear and neural responses. These measures may hold potential diagnostic utility. The medial olivocochlear (MOC) reflex underlies the inhibition of cochlear responses but the extent to which it contributes to inhibition of neural responses remains unclear. Mertes and Leek [J. Acoust. Soc. Am. 140, 2027-2038 (2016)] recently examined contralateral inhibition of cochlear responses [transient-evoked otoacoustic emissions (TEOAEs)] and neural responses [auditory steady-state responses (ASSRs)] in humans and found that the two measures were not correlated, but potential confounds of older age and hearing loss were present. The current study controlled for these confounds by examining a group of young, normal-hearing adults. Additionally, measurements of the auditory brainstem response (ABR) were obtained. Responses were elicited using clicks with and without contralateral broadband noise. Changes in TEOAE and ASSR magnitude as well as ABR wave V latency were examined. Results indicated that contralateral inhibition of ASSRs was significantly larger than that of TEOAEs and that the two measures were uncorrelated. Additionally, there was no significant change in wave V latency. Results suggest that further work is needed to understand the mechanism underlying contralateral inhibition of the ASSR.

Effects of Aging Versus Noise Exposure on Auditory System in Individuals With Normal Audiometric Thresholds.

Background: The study aims at investigating the effect of aging and noise exposure on the auditory system using auditory brainstem responses (ABRs), distortion product otoacoustic emissions (DPOAEs), and contralateral suppression of OAEs (CSOAEs). The objective was to compare DPOAEs, CSOAEs, and ABR in aged and noise-exposed individuals with the normal, to find an indicator for early diagnosis of auditory damage.Methods: Sixty adult male participants were divided into 3 groups. Group 1 included individuals not exposed to occupational noise and group 3 included individuals exposed to occupational noise who were <35 years of age. Group 2 consisted of individuals with an age range of 45-65 years without any occupational noise exposure. DPOAE fine structure was studied at 8 points per octave at different F2 frequencies. Transient evoked otoacoustic emissions (TEOAEs) were measured with and without contralateral broad band noise (BBN) at 30 dB SL (CSOAEs). ABR was recorded using click stimuli at different levels, from 90 dB nHL down to 50 dB nHL. The absolute amplitude and peak latencies for peaks I, III, and V; and the wave V/I amplitude ratio were analyzed.Results: In CSOAEs, group 1 showed greater contralateral suppression when compared to group 2 and group 3. The amplitude of ABR wave I and the wave V/I ratio showed a significant difference between the 3 groups, and there was a reduction in amplitude of wave I for groups 2 and 3.Conclusion: The findings indicate that the functioning of the auditory system is affected by occupational noise exposure and aging. CSOAEs, ABR wave I amplitude, and wave V/I amplitude ratio serve as reliable markers in the identification of hidden hearing loss.

Medial Olivocochlear Reflex Effect on Cochlear Response in Humans: Elicitor Side and Level.

Understanding the functional differences between crossed and uncrossed medial olivocochlear (MOC) neurons has been of interest to researchers for decades. Previous reports revealed conflicting results about which MOC pathway, crossed or uncrossed, is stronger in humans. Both crossed and uncrossed MOC neurons synapse at the base of the outer hair cells (OHCs) in each ear. OHCs generate the cochlear microphonic, which is a major contributor to the cochlear response (CR) PURPOSE: The current study investigated the effects of eliciting the crossed and uncrossed MOC reflex (MOCR) on CR in humans with three levels of noise. Normal-hearing, young adults (n = 16) participated in this study. The CR was recorded using 500 Hz tone-burst stimuli presented at 80 dB nHL. To examine the crossed and uncrossed MOCR, CR was recorded without and with continuous ipsilateral or contralateral broadband noise (BBN) at three levels (40, 50, and 60 dB SPL). Analysis of the CR was completed using the amplitude of the response extracted using fast Fourier transform. Statistical analysis was completed using repeated measures analysis of variance and post-hoc analysis. Compared with baseline, the presentation of BBN, specifically contralaterally, resulted in CR enhancement with no significant difference as a function of the three BBN levels. Greater enhancement of the CR amplitude was observed with contralateral than ipsilateral BBN elicitor. The current findings suggest that a contralateral elicitor of the uncrossed MOC pathway results in a larger CR amplitude enhancement compared with an ipsilateral elicitor of the crossed MOC pathway, regardless of the elicitor level. Eliciting the MOCR appears to modulate the OHCs function. Furthermore, assessing the MOCR with the 500 Hz CR with BBN elicitors at moderate levels should separate its effects (i.e., increase response amplitude) from those associated with the middle ear muscle reflex (i.e., reduce response amplitude).

The Role of the Medial Olivocochlear Reflex in Acceptable Noise Level in Adults.

The acceptable noise level (ANL) is a measurement used to quantify how much noise a person is willing to accept while listening to speech. ANL has been used to predict success with hearing aid use. However, physiological correlates of the ANL are poorly understood. One potential physiological correlate is the medial olivocochlear reflex (MOCR), which decreases the output of the cochlea and is thereby expected to increase noise tolerance. This study investigates the relationship between contralateral activation of the MOCR and tolerance of background noise. This study recruited 22 young adult participants with normal hearing. ANL was measured using the Arizona Travelogue recording under headphones presented at the most comfortable level (MCL) with and without multitalker babble noise. The MOCR strength was evaluated in all participants by measuring the cochlear microphonic (CM) with and without 40 dB sound pressure level contralateral broadband noise (CBBN). The CM observed in response to a 500-Hz tone was measured with and without CBBN, and changes in response to fast Fourier transform amplitude at 500 Hz were used as an indicator of the MOCR effect. The ANL was calculated by subtracting the maximum acceptable background noise level from the MCL. Participants were divided into two groups based on their ANL: low-ANL (ANL < 7 dB) and moderate-ANL (ANL ≥ 7 dB). An independent samples t-test was used to compare CM enhancement between low-ANL and moderate-ANL groups. Additionally, Pearson's correlation was used to investigate the relationship between the ANL and the MOCR effect on the CM. The results indicated that presentation of CBBN increased the CM amplitude, consistent with eliciting the MOCR. Participants in the low-ANL group had significantly larger CM enhancement than moderate-ANL participants. The results further revealed a significant correlation between the ANL and the MOCR effect on the CM. This study suggests that a stronger MOCR, as assessed using CM enhancement, is associated with greater noise tolerance. This research provides a possible objective measure to predict background tolerance in patients and adds to our understanding about the MOCR function in humans.

Does the Presence of Spontaneous Components Affect the Reliability of Contralateral Suppression of Evoked Otoacoustic Emissions?

The function of the medial olivocochlear system can be evaluated by measuring the suppression of otoacoustic emissions (OAEs) by contralateral stimulation. One of the obstacles preventing the clinical use of the OAE suppression is that it has considerable variability across subjects. One feature that tends to differentiate subjects is the presence or absence of spontaneous OAEs (SOAEs). The purpose of the present study was to investigate the reliability of contralateral suppression of transiently evoked OAEs (TEOAEs) measured using a commercial device in ears with and without SOAEs. OAEs were recorded in a group of 60 women with normal hearing. TEOAEs were recorded with a linear protocol (identical stimuli), a constant stimulus level of 65 dB peSPL, and contralateral broadband noise (60 dB SPL) as a suppressor. Each recording session consisted of three measurements: the first two were made consecutively without taking out the probe (the "no refit" condition); the third measurement was made after taking out and refitting the probe (a "refit" condition). Global (for the whole signal) and half-octave band values of TEOAE response levels, signal-to-noise ratios (SNRs), raw dB TEOAE suppression, and normalized TEOAE suppression, and latency were investigated. Each subject was tested for the presence of SOAEs using the synchronized SOAE (SSOAE) technique. Reliability was evaluated by calculating the intraclass correlation coefficient, standard error of measurement (SEM) and minimum detectable change. The TEOAE suppression was higher in ears with SSOAEs in terms of normalized percentages. However, when calculated in terms of decibels, the effect was not significant. The reliability of the TEOAE suppression as assessed by SEM was similar for ears with and without SSOAEs. The SEM for the whole dataset (with and without SSOAEs) was 0.08 dB for the no-refit condition and 0.13 dB for the refit condition (equivalent to 1.6% and 2.2%, respectively). SEMs were higher for half-octave bands than for global values. TEOAE SNRs were higher in ears with SSOAEs. The effect of SSOAEs on reliability of the TEOAE suppression remains complicated. On the one hand, we found that higher SNRs generally provide lower variability of calculated suppressions, and that the presence of SSOAEs favors high SNRs. On the other hand, reliability estimates were not much different between ears with and without SSOAEs. Therefore, in a clinical setting, the presence of SOAEs does not seem to have an effect on suppression measures, at least when testing involves measuring global or half-octave band response levels.

The olivocochlear reflex strength in awake chinchillas is relevant for behavioural performance during visual selective attention with auditory distractors

The auditory efferent system comprises descending projections from the cerebral cortex to subcortical nuclei, reaching the cochlear receptor through olivocochlear fibres. One of the functions attributed to this corticofugal system is to suppress irrelevant sounds during selective attention to visual stimuli. Medial olivocochlear neurons can also be activated by sounds through a brainstem reflex circuit. Whether the individual variability of this reflex is related to the cognitive capacity to suppress auditory stimuli is still controversial. Here we propose that the individual strength per animal of the olivocochlear reflex is correlated with the ability to suppress auditory distractors during visual attention in awake chinchillas. The olivocochlear reflex was elicited with a contralateral broad-band noise at ~ 60 dB and ipsilateral distortion product otoacoustic emissions were obtained at different frequencies (1–8 kHz). Fourteen chinchillas were evaluated in a behavioural protocol of visual attention with broad-band noise and chinchilla vocalizations as auditory distractors. Results show that the behavioural performance was affected by both distractors and that the magnitudes of the olivocochlear reflex evaluated at multiple frequencies were relevant for behavioural performance during visual discrimination with auditory distractors. These results stress the ecological relevance of the olivocochlear system for suppressing natural distractors.

Human medial olivocochlear reflex: Contralateral activation effect on low and high frequency cochlear response

The role of the medial olivocochlear (MOC) reflex has been investigated by assessing changes of cochlear responses (CR) in humans. The CR consists of pre-neural and neural potentials originating from the inner ear, and at high signal levels is dominated by cochlear microphonic (CM). The CM originates from the outer hair cells, where the MOC fibers synapse, and there is little research about using it to investigate the MOC reflex in humans. The current study aimed to investigate the effect of contralateral activation of the MOC reflex on the CR in humans. The CR was recorded in female adults (n = 16) to 500 and 2000 Hz tone burst stimuli presented at 80 dB nHL with and without contralateral broadband noise (CBBN) at 40 dB SPL. Two different methods were utilized to quantify and analyze the CR data: peak amplitude and power spectrum. Results revealed enhancement of the CR amplitude with activation of the MOC reflex. Furthermore, on average, enhancement in the CR amplitude was observed to 500 Hz, but not 2000 Hz stimulus. The CR power spectrum findings revealed similar findings to the peak amplitude. These findings indicate the MOC effect is measurable when using a low frequency stimulus, but not high frequency. Moreover, the CR could be used as a potential tool to study the MOC reflex in humans.

The Influence of Efferent Inhibition on Speech Perception in Noise: A Revisit Through Its Level-Dependent Function.

Purpose The study aimed to assess the relationship between the level-dependent function of efferent inhibition and speech perception in noise across different intensities of suppressor and across different signal-to-noise ratios (SNRs) of speech. Method Twenty-six young normal-hearing adults participated in the study. Contralateral suppression of transient evoked otoacoustic emissions (TEOAEs) was measured for 3 levels of suppressor (40, 50, and 60 dB SPL). Speech identification score (SIS) was measured at 5 ipsilateral SNR conditions (quiet, 0, -5, -10, and -15 dB), with and without contralateral broadband noise at 3 levels (40, 50, and 60 dB SPL). Furthermore, SNR-50 was measured with and without the same 3 levels of contralateral broadband noise. Results The results showed that the suppression magnitude of TEOAE increased with an increase in suppressor level. However, neither SIS nor SNR-50 was influenced by the contralateral noise. In addition, SIS and SNR-50 did not show significant correlation with contralateral suppression of TEOAEs. This was true at all the SNRs and contralateral noise levels used in the study. Conclusions The findings suggest that the intensity of noise directly influences medial olivocochlear bundle-mediated efferent inhibition. Assessment of the efferent auditory system in children with suspected auditory processing disorder: the Middle ear muscle reflex and contralateral inhibition of OAEs

Objective: To determine whether children aged 7 to 12 years with listening difficulties show objective evidence for efferent auditory function based on measurements of medial olivo-cochlear and middle ear muscle reflexes.Design: Click-evoked otoacoustic emissions recorded with and without contralateral broadband noise and ipsilateral and contralateral tonal (1000, 2000 Hz) middle ear muscle reflex thresholds were examined.Study sample: 29 children diagnosed with suspected auditory processing disorder (APD) and a control group of 34 typically developing children participated in this study.Results: Children with suspected APD had poorer performance on auditory processing tests than the control group. Middle ear muscle reflex thresholds were significantly higher at 2000 Hz in the suspected APD group for contralateral stimulation. MOC inhibition effects did not differ between APD and control groups.Conclusions: This research supports earlier studies showing altered acoustic reflexes in children with APD. No group differences were found for the MOC reflex measures, consistent with some earlier studies in children with APD.

Synchronized Spontaneous Otoacoustic Emissions Provide a Signal-to-Noise Ratio Advantage in Medial-Olivocochlear Reflex Assays

Detection of medial olivocochlear-induced (MOC) changes to transient-evoked otoacoustic emissions (TEOAE) requires high signal-to-noise ratios (SNR). TEOAEs associated with synchronized spontaneous (SS) OAEs exhibit higher SNRs than TEOAEs in the absence of SSOAEs, potentially making the former well suited for MOC assays. Although SSOAEs may complicate interpretation of MOC-induced changes to TEOAE latency, recent work suggests SSOAEs are not a problem in non-latency-dependent MOC assays. The current work examined the potential benefit of SSOAEs in TEOAE-based assays of the MOC efferents. It was hypothesized that the higher SNR afforded by SSOAEs would permit detection of smaller changes to the TEOAE upon activation of the MOC reflex. TEOAEs were measured in 24 female subjects in the presence and absence of contralateral broadband noise. Frequency bands with and without SSOAEs were identified for each subject. The prevalence of TEOAEs and statistically significant MOC effects were highest in frequency bands that also contained SSOAEs. The median TEOAE SNR in frequency bands with SSOAEs was approximately 8dB higher than the SNR in frequency bands lacking SSOAEs. After normalizing by TEOAE amplitude, MOC-induced changes to the TEOAE were similar between frequency bands with and without SSOAEs. Smaller MOC effects were detectable across a subset of the frequency bands with SSOAEs, presumably due to a higher TEOAE SNR. These findings demonstrate that SSOAEs are advantageous in assays of the MOC reflex.

Weak Middle-Ear-Muscle Reflex in Humans with Noise-Induced Tinnitus and Normal Hearing May Reflect Cochlear Synaptopathy.

Chronic tinnitus is a prevalent hearing disorder, and yet no successful treatments or objective diagnostic tests are currently available. The aim of this study was to investigate the relationship between the presence of tinnitus and the strength of the middle-ear-muscle reflex (MEMR) in humans with normal and near-normal hearing. Clicks were used as test stimuli to obtain a wideband measure of the effect of reflex activation on ear-canal sound pressure. The reflex was elicited using a contralateral broadband noise. The results show that the reflex strength is significantly reduced in individuals with noise-induced continuous tinnitus and normal or near-normal audiometric thresholds compared with no-tinnitus controls. Due to a shallower growth of the reflex strength in the tinnitus group, the difference between the two groups increased with increasing elicitor level. No significant difference in the effect of tinnitus on the strength of the middle-ear muscle reflex was found between males and females. The weaker reflex could not be accounted for by differences in audiometric hearing thresholds between the tinnitus and control groups. Similarity between our findings in humans and the findings of a reduced middle-ear muscle reflex in noise-exposed animals suggests that noise-induced tinnitus in individuals with clinically normal hearing may be a consequence of cochlear synaptopathy, a loss of synaptic connections between inner hair cells (IHCs) in the cochlea and auditory-nerve (AN) fibers that has been termed hidden hearing loss.

BackgroundContralateral suppression of click-evoked otoacoustic emissions (CEOAEs) is a potentially useful clinical tool. Recent studies have provided descriptions of the reliability of this measure. In terms of their clinical relevance, the studies were limited as they utilised custom-built measurement systems or were conducted by a single tester over a short time. Further, previous studies generally reported only group data. The present study addresses these limitations by reporting individual and group data collected by two testers, using standard clinical equipment over longer time-frames.Material and MethodsContralateral suppression of CEOAEs was recorded from 12 ears using the ILO 292 system. Clicks and contralateral broadband noise (BBN) were presented at 60 dB p.e. SPL and 65 dB SPL respectively. Global and best half-octave band suppression values (in dB) were measured on four separate occasions by two testers spanning an average period of 35.5 days. Reliability was assessed via the intraclass correlation coefficient (ICC) and the standard error of measurement (SEm). Multilevel regression analysis was used to explore potential causes of variation in suppression.ResultsGlobal suppression reliability was shown to be worse than previous reports, with only fair to good reliability observed. ICC and SEm values were 0.57 and 0.47 dB respectively. Corresponding values for best half-octave band suppression were 0.49 and 0.64 dB. Further analysis revealed no significant effect on contralateral suppression for a range of variables tested. Substantial variation (up to 2 dB) in contralateral suppression between test sessions was seen for individual subjects.ConclusionsFindings suggest that contralateral suppression of CEOAEs, measured by separate testers using standard clinical equipment, is not reliable over long time periods.

Contralateral Inhibition of Click- and Chirp-Evoked Human Compound Action Potentials.

Cochlear outer hair cells (OHC) receive direct efferent feedback from the caudal auditory brainstem via the medial olivocochlear (MOC) bundle. This circuit provides the neural substrate for the MOC reflex, which inhibits cochlear amplifier gain and is believed to play a role in listening in noise and protection from acoustic overexposure. The human MOC reflex has been studied extensively using otoacoustic emissions (OAE) paradigms; however, these measurements are insensitive to subsequent “downstream” efferent effects on the neural ensembles that mediate hearing. In this experiment, click- and chirp-evoked auditory nerve compound action potential (CAP) amplitudes were measured electrocochleographically from the human eardrum without and with MOC reflex activation elicited by contralateral broadband noise. We hypothesized that the chirp would be a more optimal stimulus for measuring neural MOC effects because it synchronizes excitation along the entire length of the basilar membrane and thus evokes a more robust CAP than a click at low to moderate stimulus levels. Chirps produced larger CAPs than clicks at all stimulus intensities (50–80 dB ppeSPL). MOC reflex inhibition of CAPs was larger for chirps than clicks at low stimulus levels when quantified both in terms of amplitude reduction and effective attenuation. Effective attenuation was larger for chirp- and click-evoked CAPs than for click-evoked OAEs measured from the same subjects. Our results suggest that the chirp is an optimal stimulus for evoking CAPs at low stimulus intensities and for assessing MOC reflex effects on the auditory nerve. Further, our work supports previous findings that MOC reflex effects at the level of the auditory nerve are underestimated by measures of OAE inhibition.

Concurrent measures of contralateral suppression of transient-evoked otoacoustic emissions and of auditory steady-state responses.

Contralateral suppression of otoacoustic emissions (OAEs) is frequently used to assess the medial olivocochlear (MOC) efferent system, and may have clinical utility. However, OAEs are weak or absent in hearing-impaired ears, so little is known about MOC function in the presence of hearing loss. A potential alternative measure is contralateral suppression of the auditory steady-state response (ASSR) because ASSRs are measurable in many hearing-impaired ears. This study compared contralateral suppression of both transient-evoked otoacoustic emissions (TEOAEs) and ASSRs in a group of ten primarily older adults with either normal hearing or mild sensorineural hearing loss. Responses were elicited using 75-dB peak sound pressure level clicks. The MOC was activated using contralateral broadband noise at 60 dB sound pressure level. Measurements were made concurrently to ensure a consistent attentional state between the two measures. The magnitude of contralateral suppression of ASSRs was significantly larger than contralateral suppression of TEOAEs. Both measures usually exhibited high test-retest reliability within a session. However, there was no significant correlation between the magnitude of contralateral suppression of TEOAEs and of ASSRs. Further work is needed to understand the role of the MOC in contralateral suppression of ASSRs.

Effect of Contralateral Noise on Acoustic Reflex Latency Measures.

The present study was conducted to determine the effect of contralateral broadband noise on acoustic reflex latency (ARL). Acoustic reflex latency changes for 10 and 90% on- and off-time acoustic reflexes with contralateral broadband noise were measured in 30 adults with normal hearing. The results of the study demonstrate that there was a latency prolongation for reflex on-time (10 and 90%) and latency reduction for reflex off-time (10 and 90%). This effect was seen for 500, 1000, and 2000 Hz reflex-eliciting signals. The results also showed that there was no effect of gender on latency changes in acoustic reflexes. Latency changes may explain efferent auditory system mechanisms used for the protection of the cochlea and improvement in speech perception. Thus, contralateral changes of ARL can serve as an additional tool to assess the efferent system functioning.

Medial olivocochlear efferent reflex inhibition of human cochlear nerve responses

Inhibition of cochlear amplifier gain by the medial olivocochlear (MOC) efferent system has several putative roles: aiding listening in noise, protection against damage from acoustic overexposure, and slowing age-induced hearing loss. The human MOC reflex has been studied almost exclusively by measuring changes in otoacoustic emissions. However, to help understand how the MOC system influences what we hear, it is important to have measurements of the MOC effect on the total output of the organ of Corti, i.e., on cochlear nerve responses that couple sounds to the brain. In this work we measured the inhibition produced by the MOC reflex on the amplitude of cochlear nerve compound action potentials (CAPs) in response to moderate level (52–60 dB peSPL) clicks from five, young, normal hearing, awake, alert, human adults. MOC activity was elicited by 65 dB SPL, contralateral broadband noise (CAS). Using tympanic membrane electrodes, approximately 10 h of data collection were needed from each subject to yield reliable measurements of the MOC reflex inhibition on CAP amplitudes from one click level. The CAS produced a 16% reduction of CAP amplitude, equivalent to a 1.98 dB effective attenuation (averaged over five subjects). Based on previous reports of efferent effects as functions of level and frequency, it is possible that much larger effective attenuations would be observed at lower sound levels or with clicks of higher frequency content. For a preliminary comparison, we also measured MOC reflex inhibition of DPOAEs evoked from the same ears with f2's near 4 kHz. The resulting effective attenuations on DPOAEs were, on average, less than half the effective attenuations on CAPs.

Concurrent Acoustic Activation of the Medial Olivocochlear System Modifies the After-Effects of Intense Low-Frequency Sound on the Human Inner Ear.

>Human hearing is rather insensitive for very low frequencies (i.e. below 100 Hz). Despite this insensitivity, low-frequency sound can cause oscillating changes of cochlear gain in inner ear regions processing even much higher frequencies. These alterations outlast the duration of the low-frequency stimulation by several minutes, for which the term 'bounce phenomenon' has been coined. Previously, we have shown that the bounce can be traced by monitoring frequency and level changes of spontaneous otoacoustic emissions (SOAEs) over time. It has been suggested elsewhere that large receptor potentials elicited by low-frequency stimulation produce a net Ca(2+) influx and associated gain decrease in outer hair cells. The bounce presumably reflects an underdamped, homeostatic readjustment of increased Ca(2+) concentrations and related gain changes after low-frequency sound offset. Here, we test this hypothesis by activating the medial olivocochlear efferent system during presentation of the bounce-evoking low-frequency (LF) sound. The efferent system is known to modulate outer hair cell Ca(2+) concentrations and receptor potentials, and therefore, it should modulate the characteristics of the bounce phenomenon. We show that simultaneous presentation of contralateral broadband noise (100 Hz-8 kHz, 65 and 70 dB SPL, 90 s, activating the efferent system) and ipsilateral low-frequency sound (30 Hz, 120 dB SPL, 90 s, inducing the bounce) affects the characteristics of bouncing SOAEs recorded after low-frequency sound offset. Specifically, the decay time constant of the SOAE level changes is shorter, and the transient SOAE suppression is less pronounced. Moreover, the number of new, transient SOAEs as they are seen during the bounce, are reduced. Taken together, activation of the medial olivocochlear system during induction of the bounce phenomenon with low-frequency sound results in changed characteristics of the bounce phenomenon. Thus, our data provide experimental support for the hypothesis that outer hair cell calcium homeostasis is the source of the bounce phenomenon.
