Atrial fibrillation induced by carbon monoxide poisoning: Case report

Background: Carbon monoxide (CO) poisoning is known to have direct toxic effects on the cardiovascular system, including the potential to cause arrhythmias by prolonging the QTc interval. The arrhythmogenic properties of CO are believed to be attributed to its impact on cardiac membranes. Case Presentation: This case report presents the clinical course and management of a 21-year-old male who presented to the Emergency Department (ED) with an altered level of consciousness following exposure to carbon monoxide while cooking on a gas cylinder in a closed room. The patient had no history of chronic medical illness and was a non-smoker. Physical examination revealed stable vital signs, clear breath sounds, and a lax, soft, and non-tender abdomen. Laboratory investigations showed elevated troponin levels indicative of cardiac muscle injury but normal potassium levels. ECG revealed atrial fibrillation with a fast ventricular rate and transient ST elevation. The patient received high flow oxygen. The carboxyhemoglobin level decreased significantly after high-flow oxygen administration, confirming the successful treatment of carbon monoxide poisoning. Echocardiography and coronary angiography demonstrated normal cardiac function and ruled out coronary artery disease, respectively. The patient received high-flow oxygen therapy, continuous cardiac monitoring, and supportive care during hospitalization. Following stabilization and restoration of sinus rhythm, the patient was discharged in stable condition. Conclusion: This case highlights the importance of prompt recognition and management of carbon monoxide poisoning, as well as the need for careful evaluation of cardiac function in such cases. The patient responded well to high-flow oxygen therapy, leading to a decrease in the carboxy hemoglobin level and resolution of symptoms. Timely recognition and appropriate management of carbon monoxide poisoning are crucial in preventing adverse cardiac events and ensuring a favorable outcome for the patient.

Cost-effectiveness of atrial fibrillation detection with insertable cardiac monitors in large-artery or small-vessel disease stroke in the United Kingdom

Abstract Background The randomised STROKE-AF trial demonstrated clinical effectiveness of continuous cardiac monitoring to detect atrial fibrillation (AF) with insertable cardiac monitors (ICMs) for patients with ischemic stroke attributed to large-artery or small-vessel disease (LAD/SVD). ICMs were ten-fold more effective at detecting AF compared to the standard of care (SoC). However, cost-effectiveness of this monitoring strategy is unknown. Purpose The aim of this economic evaluation was to assess the cost-effectiveness of long-term continuous cardiac monitoring via ICMs, for detection of AF (and, in event of AF diagnosis, subsequent anticoagulation), in patients with ischemic stroke attributed to large-or-small vessel disease compared to the SoC of conventional follow up in the United Kingdom. Methods We constructed a Markov model with a cycle length of three months and a lifetime horizon to conduct the cost-effectiveness analysis, from the National Health Service (NHS) perspective. Costs and health-related benefits were discounted at an annual rate of 3.5%. We expressed outcomes in terms of quality-adjusted life-years (QALYs). Patient characteristics in the analysis and detection probabilities were based on the STROKE-AF trial. Detection of AF resulted in a switch in treatment from antiplatelet to direct oral anticoagulant (DOAC). Risk of cerebrovascular events were dependent on a patient’s AF detection status and subsequent treatment. Scenario, deterministic, and probabilistic sensitivity analyses were conducted to test the robustness of the base-case incremental cost-effectiveness ratio (ICER) and account for uncertainties in the model. We stratified patients into risk groups based on CHA2DS2-VASc scores and risk of AF detection. Results The base case incremental cost-effectiveness ratio of the implantation of an ICM was £15,485 per QALY, substantially under the NICE willingness-to-pay threshold of £20,000-£30,000 per QALY. ICM implantations resulted in 0.139 QALYs gained per patient at an incremental cost of £2,147. There were 0.054 fewer stroke events per patient in the ICM arm versus SoC. Results were robust under sensitivity analysis. Conclusion ICMs were cost-effective compared to SoC for detection of AF in patients with LAD/SVD stroke in the United Kingdom driven largely by more stroke events avoided. Subgroup analysis showed that ICMs were more cost-effective in patients with higher CHA2DS2-VASc scores and those with a higher individual risk of AF.Markov Model SchematicSummary ICER Results and Events avoided

Long term clinical outcomes and cost-effectiveness of catheter vs thoracoscopic surgical ablation in long-standing persistent atrial fibrillation using continuous cardiac monitoring: CASA-AF randomised controlled trial.

Long-term clinical outcomes of catheter ablation (CA) compared to thoracoscopic surgical ablation (SA) to treat patients with long-standing persistent atrial fibrillation (LSPAF) are not known. To compare long-term (36-months) clinical efficacy, quality of life and cost-effectiveness of SA and CA in LSPAF. Participants were followed up for 3 years using implantable loop recorder (ILR) and questionnaires to assess change in quality of life. Intention-to-treat analyses were used to report the findings. Of 115 LSPAF patients treated, 104 (90.4%) completed 36-months follow-up (CA=57, SA=47). Following a single procedure without anti-arrhythmic drugs (AAD) 7 (12%) patients in the CA arm and 5 (11%) in the SA arm (HR 1.22, 95% CI 0.81 to 1.83, p = 0.41) were free from AF/AT ≥30 sec at 36 months. Thirty-three patients (58%) in the CA arm and 26 (55%) in the SA arm (HR 1.04, 95% CI 0.57 to 1.88, p = 0.91) had their AF/AT burden reduced by ≥75%. The overall impact on health-related quality of life was similar, with mean QALY estimates of 2.45 (95% CI 2.31 to 2.59) for CA and 2.32 (2.13 to 2.52) for SA. Estimated costs were higher for SA (mean £24,682, 95% CI £21,746 to £27,618) than for CA (mean £18,002, 95% CI £15,422 to £20,581). In symptomatic LSPAF, CA and SA were equally effective at achieving arrhythmia outcomes (freedom from AF/AT ≥30s and ≥75% burden reduction) following a single-procedure without AADs. However, SA is significantly more costly than catheter ablation.

A case report on digoxin toxicity

Digoxin is an inotropic drug that is commonly prescribed in patient with heart related diseases. The effective dose of digoxin is 0.8-2.0 ng/ml beyond which digoxin causes toxic effects like visual dysfunction, irregular heartbeat, and cardiovascular collapse. Digoxin toxicity is caused due to increased automaticity and inotropy due to intracellular calcium and decreased dromotropy due to poisoning of sodium potassium transporter and AV nodal blockade. Digoxin toxicity should be assessed by continuous hemodynamic and cardiac monitoring including 12-lead electrocardiogram. There should be timely and immediate evaluation of electrolyte levels like potassium, calcium, serum creatinine and digoxin levels and prompt intensive care unit admission, if necessary. In this case study we studied a 43 old female patient prescribed with digoxin 0.25 mg, who had a medical history of congestive cardiac failure presented with symptoms of digoxin toxicity. The patient was assessed with electrocardiography (ECG) and managed by keeping digoxin on hold and effective patient counselling.

Continuous Fetal Cardiac Monitoring during Fetoscopic Myelomeningocele Repair and Relationship to Spectral Doppler Changes.

No evidence-based protocols exist for fetal cardiac monitoring during fetoscopic myelomeningocele (fMMC) repair and intraprocedural spectral Doppler data are limited. We determined the feasibility of continuous fetal echocardiography during fMMC repair and correlated Doppler changes with qualitative fetal cardiac function during each phase of fMMC repair. Patients undergoing fMMC repair had continuous fetal echocardiography interpreted in real-time by pediatric cardiology. Fetal data included fetal heart rate (FHR), qualitative cardiac function, mitral and tricuspid valve inflow waveforms, and umbilical artery (UA), umbilical vein (UV), ductus arteriosus (DA), and ductus venosus (DV) Dopplers. UA abnormalities were noted in 14/25 patients, UV abnormalities were observed in 2 patients, and DV and DA abnormalities were each noted in 4 patients. Qualitative cardiac function was normal for all patients with the exception of one with isolated left ventricular dysfunction during myofascial flap creation, concurrent with an abnormal UA flow pattern. All abnormalities resolved by the first postoperative day. Continuous fetal echocardiography was feasible during all fMMC repairs. Spectral Doppler changes in the UA were common during fMMC procedures but qualitative cardiac dysfunction was rare. Abnormalities in the UV, DV, and DA Dopplers, FHR, and cardiac function were less common findings.

Hierarchical Piezoelectric Composites for Noninvasive Continuous Cardiovascular Monitoring.

Continuous monitoring of blood pressure (BP) and multiparametric analysis of cardiac functions are crucial for the early diagnosis and therapy of cardiovascular diseases. However, existing monitoring approaches often suffer from bulky and intrusive apparatus, cumbersome testing procedures, and challenging data processing, hampering their applications in continuous monitoring. Here, a heterogeneously hierarchical piezoelectric composite is introduced for wearable continuous BP and cardiac function monitoring, overcoming the rigidity of ceramic and the insensitivity of polymer. By optimizing the hierarchical structure and components of the composite, the developed piezoelectric sensor delivers impressive performances, ensuring continuous and accurate monitoring of BP at Grade A level. Furthermore, the hemodynamic parameters are extracted from the detected signals, such as local pulse wave velocity, cardiac output, and stroke volume, all of which are in alignment with clinical results. Finally, the all-day tracking of cardiac function parameters validates the reliability and stability of the developed sensor, highlighting its potential for personalized healthcare systems, particularly in early diagnosis and timely intervention of cardiovascular disease.

Region-Disentangled Diffusion Model for High-Fidelity PPG-to-ECG Translation

The high prevalence of cardiovascular diseases (CVDs) calls for accessible and cost-effective continuous cardiac monitoring tools. Despite Electrocardiography (ECG) being the gold standard, continuous monitoring remains a challenge, leading to the exploration of Photoplethysmography (PPG), a promising but more basic alternative available in consumer wearables. This notion has recently spurred interest in translating PPG to ECG signals. In this work, we introduce Region-Disentangled Diffusion Model (RDDM), a novel diffusion model designed to capture the complex temporal dynamics of ECG. Traditional Diffusion models like Denoising Diffusion Probabilistic Models (DDPM) face challenges in capturing such nuances due to the indiscriminate noise addition process across the entire signal. Our proposed RDDM overcomes such limitations by incorporating a novel forward process that selectively adds noise to specific regions of interest (ROI) such as QRS complex in ECG signals, and a reverse process that disentangles the denoising of ROI and non-ROI regions. Quantitative experiments demonstrate that RDDM can generate high-fidelity ECG from PPG in as few as 10 diffusion steps, making it highly effective and computationally efficient. Additionally, to rigorously validate the usefulness of the generated ECG signals, we introduce CardioBench, a comprehensive evaluation benchmark for a variety of cardiac-related tasks including heart rate and blood pressure estimation, stress classification, and the detection of atrial fibrillation and diabetes. Our thorough experiments show that RDDM achieves state-of-the-art performance on CardioBench. To the best of our knowledge, RDDM is the first diffusion model for cross-modal signal-to-signal translation in the bio-signal domain.

Cryoballoon pulmonary vein isolation as first-line treatment of typical atrial flutter: long-term outcomes of the CRAFT trial.

CRAFT was an international, multicentre, randomised controlled trial across 11 sites in the United UK and Switzerland. Given the evidence that pulmonary vein triggers may be responsible for atrial flutter (AFL) as well as atrial fibrillation (AF), we hypothesised that cryoballoon pulmonary vein isolation (PVI) would provide greater symptomatic arrhythmia reduction than cavotricuspid isthmus (CTI) ablation, whilst also reducing the subsequent burden of AF. Twelve-month outcomes were previously reported. In this study, we report the extended outcomes of the CRAFT study to 36months. Patients with typical AFL and no evidence of AF were randomised 1:1 to cryoballoon PVI or radiofrequency CTI. All patients received an implantable loop recorder (ILR) for continuous cardiac rhythm monitoring. The primary outcome was time-to-symptomatic arrhythmia recurrence > 30s. Secondary outcomes included time-to-first-AF episode ≥ 2min. The composite safety outcome included death, stroke and procedural complications. A total of 113 patients were randomised to cryoballoon PVI (n = 54) or radiofrequency CTI ablation (n = 59). Ninety-one patients reconsented for extended follow-up beyond 12months. There was no difference in the primary outcome between arms, with the primary outcome occurring in 12 PVI vs 11 CTI patients (HR 0.97; 95% CI 0.43-2.20; p = 0.994). AF ≥ 2min was significantly less frequent in the PVI arm, affecting 26 PVI vs 36 CTI patients (HR 0.48; 95% CI 0.29-0.79; p = 0.004). The composite safety outcome occurred in 5 PVI and 6 CTI patients (p = 0.755). Cryoballoon PVI shows similar efficacy to radiofrequency CTI ablation in reducing symptomatic arrhythmia recurrence in patients presenting with isolated typical AFL but significantly reduces the occurrence of subsequent AF.

Comparison of continuous 24-hour and 14-day ECG monitoring for the detection of cardiac arrhythmias in patients with ischemic stroke or syncope.

Previous studies show that using 12-lead electrocardiogram (ECG) or 24-h ECG monitor for the detection of cardiac arrhythmia events in patients with stroke or syncope is ineffective. The 14-day continuous ECG patch has higher detection rates of arrhythmias compared with conventional 24-h ECG monitoring in patients with ischemic stroke or syncope. This cross-sectional study of patients with newly diagnosed ischemic stroke or syncope received a 24-h ECG monitoring and 14-day continuous cardiac monitoring patch and the arrhythmia events were measured. This study enrolled 83 patients with ischemic stroke or syncope. The detection rate of composite cardiac arrhythmias was significantly higher for the 14-day ECG patch than 24-h Holter monitor (69.9% vs. 21.7%, p = .006). In patients with ischemic stroke, the detection rates of cardiac arrhythmias were 63.4% for supraventricular tachycardia (SVT), 7% for ventricular tachycardia (VT), 5.6% for atrial fibrillation (AF), 4.2% for atrioventricular block (AVB), and 1.4% for pause by 14-day ECG patch, respectively. The significant difference in arrhythmic detection rates were found for SVT (45.8%), AF (6%), pause (1.2%), AVB (2.4%), and VT (9.6%) by 14-day ECG patch but not by 24-h Holter monitor in patients with ischemic stroke or syncope. A 14-day ECG patch can be used on patients with ischemic stroke or syncope for the early detection of AF or other cardiac arrhythmia events. The patch can be helpful for physicians in planning medical or mechanical interventions of patients with ischemic stroke and occult AF.

Monitoring of patients with cardiovascular disease

Cardiovascular disease accounts for about one-third of deaths worldwide. Not surprisingly, health administrators and insurances are interested in monitoring healthcare performance and outcomes through standardized indicators obtained from reliable data. Hospital-based registries of cardiovascular disease patients are currently considered the reference standard in documenting how patients are treated relative to best practice recommendations outside the ideal world of clinical trials. Monitoring is done by special monitors designed to observe patients’ physiological state like heart rate and rhythm, blood pressure, respiration and oxygen saturation and have become a standard device for diagnosis, observation, assessment and treatment in coronary care. Changes in vital signs frequently precede deterioration in a patient’s condition and monitors provide valuable information and increase patient safety. Cardiac monitoring is a vital component in the management of the patients with cardiovascular disease. Knowledge of the general condition, location, and extent of a myocardial infarction and effective use of continuous cardiac monitoring will aid the nurse participate in the process of diagnosis and treatment.

Arrhythmia and Time of Day in Maintenance Hemodialysis: Secondary Analysis of the Monitoring in Dialysis Study

Rationale & ObjectiveThe incidence of arrhythmia varies by time of day. How this impacts individuals on maintenance dialysis is uncertain. Our objective was to quantify the relationship of arrhythmia with the time of day and the timing of dialysis. Study DesignSecondary analysis of Monitoring in Dialysis study- a multicenter prospective cohort study. We additionally analyzed rates according to presence of heart failure, time of dialysis shift, and dialysis vs. non dialysis day. ResultsRates of clinically significant arrhythmia peaked between 0:00 AM-<6:00 AM and were more than 1.5-fold as frequent during this interval compared with the rest of the day. In contrast, variations in atrial fibrillation peaked between 6:00 AM-<12: PM, but variations across the day were qualitatively small. Clinically significant arrhythmia occurred at numerically higher rate in individuals with end stage kidney disease and heart failure (5.9 events/month, 95% CI: 1.3-26.8) than those without heart failure (4.0 events/month, 95% CI: 0.9-17.9). Although differences in overall rate were not significant, their periodicity was significantly different (P<0.001) with a peak between 0:00 AM-6:00 AM with kidney failure alone and between 6:00 AM-<12:00 PM in those with heart failure. Although the overall clinically significant arrhythmia rate was similar with morning compared with evening dialysis shifts (P=0.43), their periodicity differed with a peak between 0:00 AM-<6:00 AM in those with AM dialysis and a later peak between 6:00 AM-<12:00 PM in those with PM shifts LimitationsPost hoc analysis, unable to account for unmeasured confounders ConclusionClinically significant arrhythmias showed strong diurnal patterns with a maximal peak between 0:00-<6:00 AM and noon. Although overall arrhythmia rates were similar the peak rate occurred overnight in individuals without heart failure and during the morning in individuals with heart failure. Further exploration of the influence of circadian rhythm on arrhythmia in the setting of hemodialysis is needed.

Incidence and patterns of atrial fibrillation after catheter ablation of typical atrial flutter-the FLUTFIB study.

In patients with atrial flutter (AFL), ablation of the cavotricuspid isthmus (CTI) is a highly effective procedure to prevent AFL recurrence, but atrial fibrillation (AF) may occur during follow-up. The presented FLUTFIB study was designed to identify the exact incidence, duration, timely occurrence, and associated symptoms of AF after CTI ablation using continuous cardiac monitoring via implantable loop recorders. One hundred patients with AFL without prior AF diagnosis were included after CTI ablation (mean age 69.7 ± 9.7 years, 18% female) and received an implantable loop recorder for AF detection. After a median follow-up of 24 months 77 patients (77%) were diagnosed with AF episodes. Median time to first AF occurrence was 180 (43-298) days. Episodes lasted longer than 1 h in most patients (45/77, 58%). Forty patients (52%) had AF-associated symptoms.Patients with and without AF development showed similar baseline characteristics and neither HATCH- nor CHA2DS2-VASc scores were predictive of future AF episodes. Oral anticoagulation (OAC) was stopped during FU in 32 patients (32%) and was re-initiated after AF detection in 15 patients (15%). No strokes or transient ischaemic attack episodes were observed during follow-up. This study represents the largest investigation using implantable loop recorders (ILRs) to detect AF after AFL ablation and shows a high incidence of AF episodes, most of them being asymptomatic and lasting longer than 1 h. In anticipation of trials determining the duration of AF episodes that should trigger OAC initiation, these results will help to guide anticoagulation management after CTI ablation.

Continuous cardiac monitoring in epilepsy: an implantable loop manual activation algorithm for improving ECG signal acquisition accuracy

BackgroundThe muscle artifacts, caused by prominent muscle contractions, mimicking cardiac arrhythmias, might compromise the ECG signal quality and the implantable loop recorder memory capacity in patients with epilepsy. We developed an epileptic seizures clinical pattern-based implantable loop recorder manual activation algorithm, presenting its real-world efficacy here.MethodsOne hundred ninety-three patients (18–60 years) with drug-resistant focal epilepsy were consecutively enrolled and underwent a subcutaneous loop recorder implantation. Patients with focal onset-aware seizures and patients with focal impaired awareness seizures /bilateral tonic-clonic seizures without aura were recommended to use the activator once - just after the episode. Patients with focal impaired awareness seizures/bilateral tonic-clonic seizures with aura, the caregivers of patients experiencing status epilepticus, were advised to use the activator twice - during the aura and after the episode/ regaining consciousness.ResultsSix thousand four hundred ninety-four ECG traces (4826 - auto-triggered events, 1668 - person-activated events) were recorded and analyzed. The rate of true positive events in the person-activated group was statistically higher than in the autoactivation group (72.5% vs.19.4%, p < 0.0001). Person-activated false-positive events were observed in 30.5% of patients with focal impaired awareness seizures and 27.7% in patients with bilateral tonic-clonic seizures. The highest rate of false-positive events (61.5%) was detected in patients undergoing epileptic status, and the lowest rate (3.8%) - was in patients with focal onset aware seizures. The rate of false-positive events was significantly higher in patients with impaired awareness seizures without aura both in focal impaired awareness (45.5% vs. 19.3%, p < 0.0001) and bilateral tonic-clonic seizure groups (38.8% vs. 5.9%, p < 0.0001).ConclusionsArrhythmias with varying clinical outcomes are expected in epilepsy patients and have been monitored continuously. The specified loop recorder external activation algorithm can improve the clinically relevant cardiac arrhythmia detection accuracy in epilepsy patients and the value of future studies.

The Drug Effectiveness and Side Effects of Remdesivir in Neonatal COVID-19: A Case Series Study

: Due to the novelty of COVID-19, no definite treatment has yet been found for this infectious disease, especially in neonates and early infants. Although most of the neonates affected by this virus have a mild or silent course of the disease, in the severe form, there is no approved antiviral drug. This study was done to evaluate the safety and efficacy of Remdesivir as the known effective antiviral drug in both adults and children’s populations. In this case series study, we present our experience of the administration of Remdesivir in the treatment of 15 infants with severe neonatal COVID-19, with severe respiratory distress, admitted to our neonatal intensive care unit. We prescribed the drug with a 2 to 3-hour infusion of a loading dose of 5 mg/kg on day 1, followed by 2.5 mg/kg/day 4 days later, concomitant with continuous cardiac monitoring and blood pressure monitoring every 15 minutes. Fortunately, no side effects were reported either during or after the administration of the drug. Long-term patient follows up, for 10 months after birth, did not reveal any particular complications. It seems that the use of Remdesivir to treat severe forms of neonatal and early infancy COVID-19 is safe and effective.

A 36-nW Electrocardiogram Anomaly Detector Based on a 1.5-bit Non-Feedback Delta Quantizer for Always-on Cardiac Monitoring.

An always-on electrocardiogram (ECG) anomaly detector (EAD) with ultra-low power (ULP) consumption is proposed for continuous cardiac monitoring applications. The detector is featured with a 1.5-bit non-feedback delta quantizer (DQ) based feature extractor, followed by a multiplier-less convolutional neural network (CNN) engine, which eliminates the traditional high-resolution analog-to-digital converter (ADC) in conventional signal processing systems. The DQ uses a computing-in-capacitor (CIC) subtractor to quantize the sample-to-sample difference of ECG signal into 1.5-bit ternary codes, which is insensitive to low-frequency baseline wandering. The subsequent event-driven classifier is composed of a low-complexity coarse detector and a systolic-array-based CNN engine for ECG anomaly detection. The DQ and the digital CNN are fabricated in 65-nm and 180-nm CMOS technology, respectively, and the two chips are integrated on board through wire bonding. The measured detection accuracy is 90.6% ∼ 91.3% when tested on the MIT-BIH arrhythmia database, identifying three different ECG anomalies. Operating at 1 V and 1.4 V power supplies for the DQ and the digital CNN, respectively, the measured long-term average power consumption of the core circuits is 36 nW, which makes the detector among those state-of-the-art always-on cardiac anomaly detection devices with the lowest power consumption.

Determinants of Urine Output Using Advanced Hemodynamic Monitoring in Critically Ill Patients Undergoing Continuous Renal Replacement Therapy

Introduction: Low cardiac output and hypovolemia are candidate macrocirculatory mechanisms explanatory of de novo anuria in intensive care unit (ICU) patients undergoing continuous renal replacement therapy (CRRT). We aimed to determine the hemodynamic parameters and CRRT settings associated with the longitudinal course of UO during CRRT. Methods: This is an ancillary analysis of the PRELOAD CRRT observational, single-center study (NCT03139123). Enrolled adult patients had severe acute kidney injury treated with CRRT for less than 24 h and were monitored with a calibrated continuous cardiac output monitoring device. Hemodynamics (including stroke volume index [SVI] and preload-dependence, identified by continuous cardiac index variation during postural maneuvers), net ultrafiltration (UF_NET), and UO were reported 4-hourly, over 7 days. Two study groups were defined at inclusion: non-anuric participants if the cumulative 24 h UO at inclusion was ≥0.05 mL kg⁻¹ h⁻¹, and anuric otherwise. Quantitative data were reported by its median [interquartile range]. Results: Forty-two patients (age 68 [58–76] years) were enrolled. At inclusion, 32 patients (76%) were not anuric. During follow-up, UO decreased significantly in non-anuric patients, with 25/32 (78%) progressing to anuria within 19 [10–50] hours. Mean arterial pressure (MAP) and UF_NET did not significantly differ between study groups during follow-up, while SVI and preload-dependence were significantly associated with the interaction of study group and time since inclusion. Higher UF_NET flow rates were significantly associated with higher systemic vascular resistances and lower cardiac output during follow-up. Multivariate analyses showed that (1) lower UO was significantly associated with lower SVI, lower MAP, and preload-independence; and (2) higher UF_NET was significantly associated with lower UO. Conclusions: In ICU patients treated with CRRT, those without anuria showed a rapid loss of diuresis after CRRT initiation. Hemodynamic indicators of renal perfusion and effective volemia were the principal determinants of UO during follow-up, in relation with the hemodynamic impact of UF_NET setting.

Identifying the optimal time period for detection of atrial fibrillation after ischaemic stroke and TIA: An updated systematic review and meta-analysis of randomized control trials.

Atrial fibrillation (AF) is a major risk factor for ischaemic stroke (IS) and transient ischaemic attack (TIA). The timely detection of first-diagnosed or "new" AF (nAF) would prompt a switch from antiplatelets to anticoagulation to reduce the risk of stroke recurrence; however, the optimal timing and duration of rhythm monitoring to detect nAF remains unclear. We searched MEDLINE, PubMed, Cochrane database, and Google Scholar to undertake a systematic review and meta-analysis of randomized controlled trials (RCT) between 2012 and 2023 investigating nAF detection after IS and TIA. Outcome measures were overall detection of nAF (control; (usual care) compared to intervention; (continuous cardiac monitoring >72 h)) and the time period in which nAF detection is highest (0-14 days, 15-90 days, 91-180 days, or 181-365 days). A random-effects model with generic inverse variance weights was used to pool the most adjusted effect measure from each trial. A total of eight RCTs investigated rhythm monitoring after IS, totaling 5820 patients. The meta-analysis of the studies suggested that continuous cardiac monitoring was associated with a pooled odds ratio of 3.81 (95% CI 2.14 to 6.77), compared to usual care (control), for nAF detection. In the time period analysis, the odds ratio for nAF detection at 0-14 days, 15-90 days, 91-180 days, 181-365 days were 1.79 (1.24-2.58); 2.01 (0.63-6.37); 0.98 (0.16-5.90); and 2.92 (1.30-6.56), respectively. There is an almost fourfold increase in nAF detection with continuous cardiac monitoring, compared to usual care. The results also demonstrate two statistically significant time periods in nAF detection; at 0-14 days and 6-12 months following monitoring commencement. These data support the utilization of different monitoring methods to cover both time periods and a minimum of 1 year of monitoring to maximize nAF detection in patients after IS and TIA.

OGC P50 Evaluating the outcomes of routine level one ward care post major oesphago-gastric surgery

Abstract Background Oesophago-gastric cancer resections are complex operations. Many centres routinely manage patients in higher acuity settings such as surgical HDU, ITU or fast track beds. The UK has 2.4 critical care beds per 1000, compared to an average of 5 per 1000 across OECD-EU nations. The COVID-19 pandemic has also had a significant impact on availability of these beds. Patients in our unit are routinely admitted to a standard level one surgical bed with continuous non-invasive cardiac monitoring and nursing care ratio of 1 to 5. This has been in practice for over 10 years with good outcomes. Methods Consecutive major oesophago-gastric resections between April 2020 and March 2023 in a single tertiary unit were included. Patients were split into two groups: Patients who were admitted directly to the surgical ward or had a planned admission to ITU based on pre-operative assessment and routine Integrated Cardiopulmonary Exercise Testing (CPEX) outcomes were considered to have completed a standard pathway. The primary outcome was unplanned ITU admission rate. Secondary outcomes included: total length of stay (TLOS), 30- and 90-day mortality. Results 238 patients (187 M: 51 F) were identified with a median age of 69 (33 – 84 years). The resections performed were Ivor Lewis oesophago-gastrectomy (ILGO) (n=196, 81%), total/extended total (n=30), subtotal (n=10) or distal gastrectomy (n=3). 34 patients were admitted to ITU, of which 23 were unplanned (9.2%) and all had undergone an ILGO. The median TLOS and mortality data are outlined in Table 1, with &amp;lt;1% 30 and 90-day mortality for patients on the standard pathway. The median ITU length of stay was 5 days (1 – 48 days) with reasons for admission listed in Table 2. Conclusions Major oesophago-gastric cancer resections can be managed safely in the early post-operative period, outside of higher acuity care settings with good outcomes.
