Optimizing the Measurement of Information on the Context of Alcohol Consumption Within the Drink Less App Among People Drinking at Increasing and Higher Risk Levels: Mixed-Methods Usability Study.

There is a growing public health evidence base focused on understanding the links between drinking contexts and alcohol consumption. However, the potential value of developing context-based interventions to help people drinking at increasing and higher risk levels to cut down remains underexplored. Digital interventions, such as apps, offer significant potential for delivering context-based interventions as they can collect contextual information and flexibly deliver personalized interventions while addressing barriers associated with face-to-face interventions, such as time constraints. This early phase study aimed to identify the best method for collecting information on the contexts of alcohol consumption among users of an alcohol reduction app by comparing 2 alternative drinking diaries in terms of user engagement, data quality, usability, and acceptability. Participants were recruited using the online platform Prolific and were randomly assigned to use 1 of the 2 adapted versions of the Drink Less app for 14 days. Tags (n=31) included tags for location, motivation, and company that participants added to drink records. Occasion type (n=31) included a list of occasion types that participants selected from when adding drink records. We assessed engagement and data quality with app data, usability with a validated questionnaire, and acceptability with semistructured interviews. Quantitative findings on engagement, data quality, and app usability were good overall, with participants using the app on most days (tags: mean 12.23, SD 2.46 days; occasion type: mean 12.39, SD 2.12 days). However, around 40% of drinking records in tags did not include company and motivation tags. Mean usability scores were similar across app versions (tags: mean 72.39, SD 8.10; occasion type: mean 74.23, SD 6.76). Qualitative analysis found that both versions were acceptable to users and were relevant to their drinking occasions, and participants reported increased awareness of their drinking contexts. Several participants reported that the diary helped them to reduce alcohol consumption in some contexts (eg, home or lone drinking) more than others (eg, social drinking) and suggested that they felt less negative affect recording social drinking contexts out of their home. Participants also suggested the inclusion of "work drinks" in both versions and "habit" as a motivation in the tags version. There was no clearly better method for collecting data on alcohol consumption as both methods had good user engagement, usability, acceptability, and data quality. Participants recorded sufficient data on their drinking contexts to suggest that an adapted version of Drink Less could be used as the basis for context-specific interventions. The occasion type version may be preferable owing to lower participant burden. A more general consideration is to ensure that context-specific interventions are designed to minimize the risk of unintended positive reinforcement of drinking occasions that are seen as sociable by users.

Alcohol consumption: context and association with mortality in Switzerland

PurposeNon-communicable diseases generate the largest number of avoidable deaths often caused by risk factors such as alcohol, smoking, and unhealthy diets. Our study investigates the association between amount and context of alcohol consumption and mortality from major non-communicable diseases in Switzerland.MethodsGeneralized linear regression models were fitted on data of the cross-sectional population-based National Nutrition Survey menuCH (2014–2015, n = 2057). Mortality rates based on the Swiss mortality data (2015–2018) were modeled by the alcohol consumption group considering the amount and context (i.e., during or outside mealtime) of alcohol consumption and potential confounders. The models were checked for spatial autocorrelation using Moran’s I statistic. Integrated nested Laplace approximation (INLA) models were fitted when evidence for missing spatial information was found.ResultsHigher mortality rates were detected among drinkers compared to non-drinkers for all-cancer (rate ratio (RR) ranging from 1.01 to 1.07) and upper aero-digestive tract cancer (RR ranging from 1.15 to 1.20) mortality. Global Moran’s I statistic revealed spatial autocorrelation at the Swiss district level for all-cancer mortality. An INLA model led to the identification of three districts with a significant decrease and four districts with a significant increase in all-cancer mortality.ConclusionSignificant associations of alcohol consumption with all-cancer and upper aero-digestive tract cancer mortality were detected. Our study results indicate the need for further studies to improve the next alcohol-prevention scheme and to lower the number of avoidable deaths in Switzerland.

Peer pressure and alcohol consumption in adults living in the UK: a systematic qualitative review

BackgroundPeer pressure to drink alcohol may influence excessive alcohol consumption, which can have adverse impacts on health and wellbeing. While peer pressure to drink alcohol is extensively studied among youth, less examination exists among adults. This systematic review examined qualitative research studies which explored the role and concept of peer pressure within the context of alcohol consumption in adults living in the UK.MethodsQualitative studies which explored peer pressure within the context of alcohol consumption or alcohol related behaviours and views in adults (age range approximately 18–52 years) living in the UK were included. Systematic searches conducted in Medline, PsycINFO and Web of Science identified 1462 references, of which 13 studies met inclusion criteria. Thematic analysis was conducted.ResultsFive overarching themes were identified. Four of these themes directly address aspects of peer pressure, including: experiences of peer pressure; consequences of peer pressure; strategies to deal with peer pressure; and conditions perceived to affect peer pressure. The fifth overarching theme explains the wider social context influencing peer pressure. Pressure to drink alcohol affects individuals across the life span and can be experienced as overt and aggressive, or subtle and friendly. Those consuming little or no alcohol are more likely to feel overt forms of peer pressure. Some developed strategies to cope with pressure from drinkers. Peer pressure can result in feelings of social isolation, or giving in by consuming alcohol against ones wishes.ConclusionPeer pressure to drink alcohol is a complex and multifaceted phenomenon experienced across adulthood requiring better understanding to support initiatives to decrease the impact of pressure-inducing environments and develop strategies to deal with perceived pressure conditions.Trial RegistrationThe protocol for this review is registered with PROSPERO (CRD42019122201). A safe level of alcohol consumption: the right answer demands the right question.

Alcohol has been produced by humans for nearly ten millennia, but gold-standard evidence by which to judge the health effects of limited alcohol consumption remains elusive, introducing serious difficulty in considering the safety of alcohol consumption. To do so, physicians and policymakers must consider the population, dose and context of alcohol consumption and the end-point, preferably a holistic composite, of interest. The limitations of new research trends, such as mega-cohorts, genetic instrumental variable analysis and modelling studies, must also be viewed against the much larger backdrop of existing evidence. Some existing guidelines, such as the 2015-2020 Dietary Guidelines for Americans, succeed remarkably in this task. Nonetheless, large-scale randomized trials are urgently needed if future generations are to enjoy any greater insight into the health effects of population-wide alcohol consumption than the current one has.

Alcohol consumption in India- An epidemiological review.

One of the most important products of global addiction demand is an alcoholic beverage. In developing countries like India, alcohol consumption tends to be a major problem because of the various socio-cultural practices across the nation, different alcohol policies and practices across the various states, lack of awareness of alcohol-related problems among the community, false mass media propaganda about alcohol use, various alcohol drinking patterns among the alcohol consumers and the emergence of social drinking as a habit because of the widespread urbanisation across the country. Stringent alcohol policies are needed across the various states to reduce alcohol consumption, and alcohol consumers have to be educated about the various harmful effects of alcohol consumption and the effects it can have on their mind, body and soul. This review article focuses on the burden of alcohol consumption in context with its various harmful effects on the mind and body with a note on the alcohol policies in the country.

Communication and interpretation of emotional distress within the friendships of young Irish men prior to suicide: a qualitative study.

The potential for young men in crisis to be supported by their lay networks is an important issue for suicide prevention, due to the under-utilisation of healthcare services by this population. Central to the provision of lay support is the capability of social networks to recognise and respond effectively to young men's psychological distress and suicide risk. The aim of this qualitative study was to explore young men's narratives of peer suicide, in order to identify how they interpreted and responded to behavioural changes and indications of distress from their friend before suicide. In-depth qualitative interviews were conducted during 2009/10 with 15 Irish males (aged 19-30 years) who had experienced the death by suicide of a male friend in the preceding 5 years. The data were analysed using a thematic approach. Through the analysis of the participants' stories and experiences, we identified several features of young male friendships and social interactions that could be addressed to strengthen the support available to young men in crisis. These included the reluctance of young men to discuss emotional or personal issues within male friendships; the tendency to reveal worries and emotion only within the context of alcohol consumption; the tendency of friends to respond in a dismissive or disapproving way to communication of suicidal thoughts; the difficulty of knowing how to interpret a friend's inconsistent or ambiguous behaviour prior to suicide; and beliefs about the sort of person who takes their own life. Community-based suicide prevention initiatives must enhance the potential of young male social networks to support young men in crisis, through specific provisions for developing openness in communication and responsiveness, and improved education about suicide risk.

The context of alcohol consumption among adolescents and their families

Alcohol consumption in adolescence has been an important topic in the study of public health policies. This study aimed to analyze and interpret the influence of the family in teenage alcohol consumption. A qualitative research was undertaken, using interviews, observation, genogram and eco-map, in order to investigate 22 participants at a Family Health Unit in the city of Recife, Pernambuco, Brazil. The family structure, composition, interaction, communication, and religion of the participants were considered. The results were analyzed using inductive thematic analysis and indicated three themes, namely, the internality of the family: fragility and conflicts; among us: limits, responsibilities and hope; and the decision to drink: fun permeated by risks. The family and sociocultural context of adolescents can positively and negatively influence the use and abuse of alcohol and, therefore, can be a target of intervention in nursing.

A Qualitative Investigation of the Relationship Between Consumption, Physical Activity, Eating Disorders, and Weight Consciousness

BackgroundPrevious research has identified a positive relationship between alcohol consumption and disordered eating, alcohol consumption and physical activity, and physical activity and disordered eating. However, there is a paucity of published research examining the interrelatedness of all 3 behaviors together.PurposeThis study examines the relationship between alcohol consumption, disordered eating, and physical activity by exploring college students' weight management behaviors.MethodsParticipants included undergraduate students (n = 36) recruited from a Midwestern university. Five distinct focus groups were conducted. Data analytic techniques included constant comparison analysis, classical content analysis, word count, and theoretical coding.ResultsWeight consciousness, characterized by guilt/worry regarding calories from alcohol consumption, represented the most salient theme. Increasing physical activity and/or decreasing caloric intake prior to/following alcohol consumption were the primary mechanisms for suppressing weight concerns.DiscussionThe presence of physical activity and caloric restriction within the context of alcohol consumption is original and expands our understanding of the alcohol–physical activity association.Translation to Health Education PracticeThis study provides valuable information to guide campus health officials and health education specialists in the design and implementation of future intervention and policy efforts geared toward decreasing alcohol consumption while accounting for issues related to body image/weight management.

Home drinking – Is this the target of the new alcohol strategy?

In the foreword to the third alcohol strategy for England and Wales, Prime Minister David Cameron is clear that this is a response to the problems caused by binge drinking. This is within the context of alcohol consumption in England and Wales having fallen steadily for a number of years and a shift to more drinking at home, which until recently has largely been seen as non-problematic. Despite being portrayed as a response to binge drinking, the main headline that accompanied the strategy was the proposal to introduce minimum unit pricing. The main effect of this will be to narrow the price of alcohol purchased in supermarkets compared to licensed premises. This article suggests that policy makers may now be more receptive to the arguments that suggest they should now be considering alcohol-related general population harms. Despite the fact that the strategy has been presented as a response to binge drinking, there is also a focus on the cost of general population harms.

A phenotype-driven ENU mutagenesis screen for the identification of dominant mutations involved in alcohol consumption

The aim of this study was the application of a phenotype-driven N-ethyl-N-nitrosourea (ENU) mutagenesis screen in mice for the identification of dominant mutations involved in the regulation and modulation of alcohol-drinking behavior. The chemical mutagen ENU was utilized in the generation of 131 male ENU-mutant C57BL/6J mice (G0). These ENU-treated mice were paired with wild-type C57BL/6J mice to generate G1 and subsequent generations. In total, 3327 mice were generated. Starting with G1, mice were screened for voluntary oral self-administration of 10% (v/v) alcohol vs. water in a two-bottle paradigm. From these mice, after a total period of 5 weeks of drinking, 43 mutants fulfilled the criteria of an "alcohol phenotype," that is, high or low ethanol intake. They were then selected for breeding and tested in a "confirmation cross" (G2-G4) for inheritance. Although we did not establish stable high or low drinking lines, several results were obtained in the context of alcohol consumption. First, female mice drank more alcohol than their male counterparts. Second, the former demonstrated greater infertility. Third, all animals displayed relatively stable alcohol intake, although significantly different in two different laboratories. Finally, seasonal and monthly variability was observed, with the highest alcohol consumption occurring in spring and the lowest in autumn. In conclusion, it seems difficult to identify dominant mutations involved in the modulation or regulation of voluntary alcohol consumption via a phenotype-driven ENU mutagenesis screen. In accordance with the findings from knockout studies, we suggest that mainly recessive mutations contribute to an alcohol-drinking or alcohol-avoiding phenotype.

The social context of drinking among high school drinking drivers.

Over 2,000 high school students were surveyed with an anonymous questionnaire to determine their frequency of drunk driving, social context of alcohol consumption, beliefs about drunk driving, and quantity and frequency of alcohol consumed. About 10% of the sample was identified as drunk drivers and they were compared with non-drunk driving drinkers according to a number of measures of where and why they drank. The results revealed highly significant differences in the social context of alcohol consumption for beer, wine, and liquor consumption. Across all three beverage categories, the most important discriminating social context factors were drinking at a dormitory and drinking to get along better on dates. Beer and liquor consumption were more important than wine consumption for discriminating drunk drivers from the non-problem drinking high school population. Quantity and frequency measures of alcohol consumption along with belief measures about drinking and driving did not substantially increase the discrimination between the two groups, but significantly increased the predictive power in a multiple regression analysis including the social context of alcohol consumption items.

AN ANALYSIS OF CONTEXT AND ALCOHOL CONSUMPTION IN A GROUP OF CRIMINAL EVENTS

The aim of this analysis was to demonstrate the importance of examining the context of alcohol consumption with regard to a criminal event. The paper explores information which was part of a larger study of alcohol use by violent and non-violent offenders. Data were collected by interview from the offenders and correlated with other reports. Earlier findings of the study which have been reported suggested that level of consumption was associated with the type of offence. In this paper a more complex drinking/drinking context and crime relationship is shown to exist. The circumstances at the time of the crime were related more to the level of alcohol consumption than to the violent or non-violent nature of the crime. Type of beverage, companionship and time of day were found to be particularly good indicators of the amount of alcohol consumed.
