Potent and biostable inhibitors of the main protease of SARS-CoV-2

Potent and biostable inhibitors of the main protease (Mpro) of SARS-CoV-2 were designed and synthesized based on an active hit compound 5h (2). Our strategy was based not only on the introduction of fluorine atoms into the inhibitor molecule for an increase of binding affinity for the pocket of Mpro and cell membrane permeability but also on the replacement of the digestible amide bond by a surrogate structure to increase the biostability of the compounds. Compound 3 is highly potent and blocks SARS-CoV-2 infection invitro without a viral breakthrough. The derivatives, which contain a thioamide surrogate in the P2-P1 amide bond of these compounds (2 and 3), showed remarkably preferable pharmacokinetics in mice compared with the corresponding parent compounds. These data show that compounds 3 and its biostable derivative 4 are potential drugs for treating COVID-19 and that replacement of the digestible amide bond by its thioamide surrogate structure is an effective method.

Mass COVID-19 patient screening using UvsX and UvsY mediated DNA recombination and high throughput parallel sequencing

The Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2), also known as 2019 novel coronavirus (2019-nCoV), is a highly infectious RNA virus. A percentage of patients develop coronavirus disease 2019 (COVID-19) after infection, whose symptoms include fever, cough, shortness of breath and fatigue. Acute and life-threatening respiratory symptoms are experienced by 10–20% of symptomatic patients, particularly those with underlying medical conditions. One of the main challenges in the containment of COVID-19 is the identification and isolation of asymptomatic/pre-symptomatic individuals. A number of molecular assays are currently used to detect SARS-CoV-2. Many of them can accurately test hundreds or even thousands of patients every day. However, there are presently no testing platforms that enable more than 10,000 tests per day. Here, we describe the foundation for the REcombinase Mediated BaRcoding and AmplificatioN Diagnostic Tool (REMBRANDT), a high-throughput Next Generation Sequencing-based approach for the simultaneous screening of over 100,000 samples per day. The REMBRANDT protocol includes direct two-barcoded amplification of SARS-CoV-2 and control amplicons using an isothermal reaction, and the downstream library preparation for Illumina sequencing and bioinformatics analysis. This protocol represents a potentially powerful approach for community screening of COVID-19 that may be modified for application to any infectious or non-infectious genome.

Knowledge, attitudes, and practices of Indonesian medical and non-medical undergraduate students toward COVID-19

As the coronavirus disease 2019 (COVID-19) pandemic spread across Indonesia, good knowledge, proper attitude, and appropriate practices among undergraduate students must be achieved before starting a face-to-face lecture. These are also important since most students are active in social media and can spread true or false rumors regarding COVID-19. This study aimed to assess the level of knowledge, attitudes, and practices of medical and non-medical undergraduate students toward COVID-19. A 51-item online questionnaire was developed and sent to random undergraduate students from different faculties in Universitas Diponegoro, Indonesia. A total of 482 students completed the survey. The Chi-square test showed significant associations in the level of knowledge, attitudes, and practices towards the COVID-19 between medical and non-medical students, where medical students have better knowledge, attitudes, and practices than non-medical students. This study's findings may become the basis for an awareness campaign planning among students in particular and the public in general, which at this time most student activities are still carried out online and to prepare face-to-face lectures and also to deepen the material regarding COVID-19 among students, especially non-medical students, and furthermore, help to guide the efforts and plans of state health authorities for better containment of COVID-19.

Community engagement and compliance monitoring of COVID-19 safety protocols: innovative approach combining indigenous practice and GIS technology in Oyo State, Nigeria

Background: One of the major challenges that has driven the spread of Coronavirus Disease 2019 (COVID-19) worldwide is the burden of enforcing the preventive measures required to contain the pandemic. Enforcement of COVID-19 precautionary behaviour should not be homogenous; every country needs to be creative to ensure that humane considerations guide all decisions during the extraordinary experience that COVID-19 pandemic portends. The model of self-policing is acceptable and maintained principally because the citizens of any communities operate, recognize, and accept them as preferred alternatives to the official models of policing for enforcement. Hence the approach presented in this paper, which deployed existing indigenous alternative systems in ensuring compliance with COVID-19 precautionary behaviour. This article therefore documents the unique approach deployed for the containment of COVID-19 in Oyo State, Nigeria. Objective: This intervention was designed to explore established indigenous alternative systems and models of control, justice, law, security, and enforcement in Nigeria. Additionally, geographic information system (GIS) technology and investigative journalism was used to monitor and evaluate the effectiveness of the intervention. Method: The method employed was community conversation; a method of increasing inclusive, community-based engagement harnessing the expertise and motivation of key stakeholders. The community conversations were convened after the pattern of a traditional Town-hall meeting. Community conversations were organized as a qualitative framework focusing on deploying the indigenous practice of self-policing associated with Nigerias trade unions and aims to inform COVID-19 preventive behaviour at the community level. Geographical information system technology was used to develop COVID-19 Containment Compliance Citizens Reporter App. The App was developed using ESRI ArcGIS online platform to crowd source public feedback on compliance or contravention of COVID-19 protocols. Social media platforms were also deployed for monitoring and evaluation of the intervention post townhall meeting. Results: The establishment of a State-wide Containment response network provided the required inroad for advocacy and deployment of state-wide community conversation framework in the different communities comprising diverse ethnic groups, religious leaders, market leaders, National Union of Road Transport Workers (NURTW), and so on. Testimonials from the various communities showed that the people have embraced the self-policing strategy and the network system was effective with good outcomes in terms of response to decontamination, containment, and advocacy. The COVID-19 Containment Compliance Citizens Reporter App, investigative reporting by mass media were highly effective tools for monitoring and evaluation of the outcome of the intervention as well as possible evidence for melting out incentive and disincentive measures as necessary. This approach is a template, which could be adapted and replicated in other parts of Nigeria and other African societies with similar structures, demographics, and indigenous practices.

Comparison of the Rapid Antigen Testing Method With RT-qPCR for the Diagnosis of COVID-19.

Background: Coronavirus disease 2019 (COVID-19) has till now affected about 110 million people globally. It has not spared any country and has led to 24 lakh deaths. As a result, the testing had to be increased manifold leading to depletion in the number of the quantitative reverse transcription polymerase chain reaction (RT-qPCR) kits. Point-of-care rapid antigen-based tests were developed in order to meet the increasing demands. The objective of this study was to compare the performance of a rapid chromatographic test (index test) with a gold standard test (RT-qPCR).Methods: A retrospective analysis was done at a tertiary care teaching hospital in Eastern Uttar Pradesh, India. Paired samples were taken from all patients reporting to the clinic for antigen-based rapid diagnostic testing (RDT) and RT-qPCR. The sensitivity and specificity were calculated to evaluate the performance of the RDT.Results: The overall sensitivity and specificity of the RDT were observed to be 53.6% (39.7-67.0) and 97.35% (94.6-98.9), respectively. In symptomatic individuals, the sensitivity was higher 61.0% (44.5-75.8). The test positivity rates of RDT were found to be higher at a ​​cycle threshold value ≤20.Conclusion: RDT can be used as a screening test to rule in the infection especially in symptomatic patients who are more prone to spread the disease. It is an important weapon in the armamentarium of public healthcare for the containment of COVID-19.

Modeling population-wide testing of SARS-CoV-2 for containing COVID-19 pandemic in Okinawa, Japan.

To break the chains of SARS‐CoV‐2 transmission and contain the coronavirus disease 2019 (COVID‐19) pandemic, population‐wide testing has been practiced in various countries. However, scant research has addressed this topic in Japan. In this modeling exercise, we extracted the number of daily reported cases of COVID‐19 in Okinawa from October 1 to November 30, 2020, and explored possible scenarios for decreasing COVID‐19 incidence by combining population‐wide screening and/or social distancing policy. We reveal that permanent lockdown can be theoretically replaced by mass testing but sufficient target population at an adequate frequency must be mobilized. In addition, solely imposing a circuit breaker will not bring a favorable outcome in the long run, and mass testing presents implications for minimizing a period of lockdown. Our results highlight the importance of incentivizing citizens to join the frequent testing and ensure their appropriate isolation. This study also suggests that early containment of COVID‐19 will be feasible in prefectures where the mobility is low and/or can be easily controlled for its geographic characteristics. Rigorous investment in public health will be manifestly vital to contain COVID‐19.

Digital health interventions for COVID-19 in China: a retrospective analysis

BackgroundThe use of digital health technologies was an integral part to China's early response to coronavirus disease 2019 (COVID-19). Existing literatures have analyzed and discussed implemented digital health innovations from the perspective of technologies, whereas how policy mechanisms contributed to the formulation of the digital health landscape for COVID-19 was overlooked. This study aimsed to examine the contexts and key mechanisms in China’s rapid mobilization of digital health interventions in response to COVID-19, and to document and share lessons learned. MethodsPolicy documents were identified and retrieved from government portals and recognized media outlets. Data on digital health interventions were collected through three consecutive surveys administered between 23 January 2020 and 31 March 2020 by China Academy of Information and Communication Technology (CAICT) affiliated to the Ministry of Industry and Information Technology (MIIT). Participants were member companies of the Internet Health alliance established by MIIT and the National Health Commission (NHC) in June 2016. Self-report digital interventions focusing on social and economic recovery were excluded. Two hundred and sixty-six unique digital health interventions meeting our criteria were extracted from 175 narratives on digital health interventions submitted by 116 participating companies. Thematic analysis was conducted to describe the scope and priority of policies advocating for the use of digital health technologies and the implementation pattern of digital health interventions. Data limitations precluded an evaluation of the impact of digital health interventions over a longer time frame. ResultsBetween January and March 2020, national policy directives promoting the use of digital technologies for the containment of COVID-19 collectively advocated for use cases in emergency planning and preparedness, public health response, and clinical services. Interventions to strengthen clinical services were mentioned more than the other two themes (n = 15, 62.5%(15/24)). Using digital technologies for public health response was mentioned much less than clinical services (n = 5, 20.8%(5/24)). Emergency planning and preparedness was least mentioned (n = 4, 16.7%(4/24)). Interventions in support of clinical services disproportionately favored healthcare facilities in less resource-constraint settings. Digital health interventions shared the same pattern of distribution. More digital health technologies were implemented in clinical services (n = 103, 38.7%(103/266)) than that in public health response (n = 91, 34.2%(91/266)). Emergency planning and preparedness had the least self-reported digital health interventions (n = 72, 27.1%(72/266)). We further identified case studies under each theme in which the wide use of digital health technologies highlighted contextual factors and key enabling mechanisms. ConclusionsThe contextual factors and key enabling mechanisms through the use of policy instruments to promote digital health interventions for COVID-19 in China include pathway of policy directives influencing the private sector using a decentralized system, the booming digital health landscape before COVID-19, agility of the public sector in introducing regulatory flexibilities and incentives to mobilize the private sector.

Evaluation of errors during different phases of coronavirus disease 2019 TrueNat testing in a tertiary care hospital

Background: Although diagnostic errors have always significantly attributed to clinical and economical losses, now in the ongoing coronavirus disease 2019 (COVID-19) pandemic, the impact of these errors has unquestionably increased multifold. In the present scenario, reporting of false-negative or false-positive test results may adversely affect the public health measures and government restrictive policies, established for the containment of COVID-19 in addition to enhancing the individual suffering and economic burden. The only way to reduce errors in various phases of COVID-19 testing is to evaluate them thoroughly and subsequently follow them up with effective corrective actions. Materials and Methods: The present study was conducted at a tertiary care center, approved by Indian Council of Medical Research for COVID-19 TrueNat testing. Information regarding errors and subsequent corrective actions in preanalytical, analytical, and post-analytical phases of TrueNat testing for COVID-19 were collected. Results: A total of 112 testing errors were observed in the TrueNat testing of 1108 samples during the study duration. Around 58.93% errors occurred in the preanalytical phase of TrueNat testing, followed by 39.29% and 1.79% errors in the analytical phase and post-analytical phases, respectively. Poor quality samples constituted the predominant cause of error in the preanalytical phase. Conclusion: Delivery of accurate results is of paramount importance in this COVID-19 pandemic. A thorough analysis of the various phases of COVID-19 testing will help identify and understand the errors and implement effective corrective actions.

Global health diplomacy at the intersection of trade and health in the COVID-19 era.

Global health diplomacy has gained significant importance and undoubtedly remained high on the agendas of many nations, regional and global platforms amid the coronavirus disease 2019 (COVID-19) pandemic. Many countries have realized the importance of the health sector and the value of a healthy workforce. However, there is little control on issues related to trade that impact on human health due to the dominance of profit-oriented business lobbies. A balance, however, needs to be struck between economic profits and a healthy global population. This paper aimed to highlight the importance of building capacity in global health diplomacy, especially during the COVID-19 pandemic so that health personnel may effectively negotiate on the multisectoral stage to secure the resources they need. The recent proposal to waive off certain provisions of the Trade-Related Aspects of Intellectual Property Rights (TRIPS) agreement for the prevention, containment and treatment of COVID-19 by India and South Africa at the World Trade Organization (WTO) presents an important opportunity for all governments to unite and stand up for public health, global solidarity, and equitable access at the international level so that both developed and developing nations may enjoy improved health outcomes related to the COVID-19 pandemic.

Laboratory Diagnosis of SARS Corona Virus-2 Infection: Single Centre Experience of First 12,000 Samples

Introduction: First spotted in Wuhan, China, World Health Organisation declared the deadly outbreak caused by novel Severe Acute Respiratory Syndrome Coronavirus-2 (SARS CoV-2) as global pandemic on March 11, 2020. With over 72 million cases globally till December 2020, countries need to gear up to detect, isolate and treat Corona Virus Disease-2019 (COVID-19) cases. Laboratories play a crucial role in diagnosis thereby instituting early contact tracing and quarantine measures. The laboratory based COVID-19 diagnostic testing data may help in formulating strategies to contain the spread of infection. Aim: To describe key patient variables of the respiratory samples processed for SARS-CoV-2 by Real Time Polymerase Chain Reaction (RT-PCR) at a diagnostic laboratory. Materials and Methods: In this descriptive, single centre study carried out at ICMR approved COVID-19 diagnostic laboratory, nasopharyngeal swabs received in Viral Transport Medium (VTM) were tested for SARS-CoV-2 infection by RT-PCR. Key patient variables, like age, gender, symptoms and sample positivity rate were tabulated. The demographic and clinical data of samples tested were summarised by medians and Interquartile Range (IQR) for continuous variables and by proportions for categorical variables. Results: A total of 12,187 samples were received between 21st March to 8th July 2020. The data from 11,196 individuals were complete and were included in the analysis. Overall, 2,053 samples were tested positive for SARS-CoV-2 indicating positivity rate of 18.33%. Sample positivity was highest (63.91%) among high-risk contacts of a laboratory confirmed case. The maximum number of samples tested belonged to age group of 21-40 years and male predominance was observed. Conclusion: Although social distancing, mask usage, hand hygiene and respiratory etiquettes are important measures for containment of COVID-19, strengthening and capacity building of laboratory network is crucial for mitigating the pandemic.

Management of COVID-19 infection in patients undergoing hemodialysis.

Coronavirus disease 2019 (COVID-19), a pandemic affecting the world's population, is particularly threatening to dialysis patients. The purpose of this study was to propose recommendations for prevention and containment of COVID-19 in hemodialysis center situated in a regional hospital in Mauritania. Both COVID-19-infected (n = 11) and -uninfected (n = 16) dialysis patients were hospitalized up to three weeks to improve patient management. All COVID-19-infected patients were cured. Patient care and hygiene in a safe environment are the key factors for a favorable outcome, even in resource-limited settings.

Pregnancy During COVID-19 Outbreak: The Impact of Lockdown in a Middle-Income Country on Antenatal Healthcare and Wellbeing.

IntroductionSince the emergence of coronavirus disease 2019 (COVID-19) as a major worldwide health threat, countries have implemented preventative measures to limit the spread of the disease and reduce the strain on their health services. The Hashemite Kingdom of Jordan is one of the countries where relatively strict lockdown measures were enforced. This study aims to evaluate the impact of the lockdown circumstances in Jordan on antenatal care services and health circumstances of pregnant women during this period.Participants and MethodsThis study involved a cross-sectional survey of Jordanian women who are currently pregnant. An electronic survey was distributed among pregnant women in Jordan. A total of 944 completed responses were returned and analyzed. The sample size was statistically determined to be representative of the population.ResultsThe results revealed a significant increase in the percentage of pregnant women not receiving antenatal care from 4% to 59.53% (p<0.001) during the lockdown period, despite the fact that some of these women were suffering from significant underlying medical conditions or serious pregnancy complications that require close antenatal surveillance. The survey also demonstrated that the lockdown circumstances impacted the physical, social, and mental wellbeing of the participants.ConclusionAlthough the lockdown measures in Jordan have succeeded so far in relative containment of COVID-19, this research suggests that significant disruption to antenatal services has occurred and that the lockdown has affected the wellbeing of pregnant women in a number of aspects, which has the potential to cause antenatal health issues that are not directly related to the spread of COVID-19, but rather are a side effect of the way in which the outbreak is managed.

Successful interruption of seasonal influenza transmission under the COVID-19 rapid response in Zhejiang Province, China

ObjectivesThe objective of this study was to evaluate whether the non-pharmaceutical interventions (NPIs) introduced to curb the spread of coronavirus disease 2019 (COVID-19) also interrupted the transmission of influenza. Study designThis is a descriptive epidemiological study. MethodsData on changes in the number of reported influenza cases, number of influenza-like illness (ILI) visits, ILI percentage and influenza virus positivity were compared between the first 18 weeks of 2020 and the same period of 2019. ResultsThe changes in the weekly average number of influenza cases were statistically significant between 2020 and 2019 (−4319 vs −525 per week; P < 0.05). The slopes of regression lines for the number of ILI visits were also statistically significant between 2020 and 2019 (−911 vs −98 per week; P < 0.05). ConclusionsThis study found that the prevalence of influenza was substantially decreased when NPIs were implemented for the containment of COVID-19.

The concerns generated by coronavirus disease 2019 (COVID-19) pandemic are having profound impact on solid organ transplantation (SOT). Non-pharmaceutical interventions (NPI) are currently the only measures available to contain COVID-19 in the general population and in more vulnerable recipients of any organ transplant.In this cross-sectional case control study from a patient survey undertaken in 2 transplant centers (TxC) in the Kingdom of Saudi Arabia and Italy, we aimed to appraise awareness of the NPI implemented by respective these governments. We have also evaluated the impact of COVID-19 on our kidney transplant (KT) recipients and a control group of kidney living donors (KLD).In our series, there were zero cases of COVID-19 among 111 KT recipients and 70 KLD of the control group. Demography, transplant type, immunosuppression regimes, and, importantly, the different COVID-19 prevalence in the 2 regions of the TxC did not appear to influence incidence of COVID-19 in our KT recipients.The absence of COVID-19 cases in our series was unexpected. Our findings suggest that awareness of NPI is associated with a successful containment of COVID-19 in vulnerable, immunosuppressed KT recipients.

Virological assessment of hospitalized patients with COVID-2019.

Coronavirus disease2019 (COVID-19) is an acute infection of the respiratory tract that emerged in late 20191,2. Initial outbreaks in China involved 13.8% of cases with severe courses, and 6.1% of cases with critical courses3. This severe presentation may result from the virus using a virus receptor that is expressed predominantly in the lung2,4; the same receptor tropism is thought to have determined the pathogenicity-but also aided in the control-of severe acute respiratory syndrome (SARS) in 20035. However, there are reports of cases of COVID-19 in which the patient shows mild upper respiratory tract symptoms, which suggests the potential for pre- or oligosymptomatic transmission6-8. There is an urgent need for information on virus replication, immunity and infectivity in specific sites of the body. Here we report a detailed virological analysis of nine cases of COVID-19 that provides proof of active virus replication in tissues of the upper respiratory tract. Pharyngeal virus shedding was very high during the first week of symptoms, with a peak at 7.11×108RNA copies per throat swab on day4. Infectious virus was readily isolated from samples derived from the throat or lung, but not from stool samples-in spite of high concentrations of virus RNA. Blood and urine samples never yielded virus. Active replication in the throat was confirmed by the presence of viral replicative RNA intermediates in the throat samples. We consistently detected sequence-distinct virus populations in throat and lung samples from one patient, proving independent replication. The shedding of viral RNA from sputum outlasted the end of symptoms. Seroconversion occurred after 7days in 50% of patients (and by day14 in all patients), but was not followed by a rapid decline in viral load. COVID-19 can present as a mild illness of the upper respiratory tract. The confirmation of active virus replication in the upper respiratory tract has implications for the containment of COVID-19.

Sustaining the delivery of malaria elimination services amidst an ongoing coronavirus disease 2019 outbreak in the African Region

The Corona Virus Disease-2019 (COVID-19) pandemic has become a global public health priority and it has compelled multiple nations to divert their resources for the containment of the disease at the earliest. With regard to malaria, the African region is the most affected one, accounting for 93% of the reported cases worldwide. However, since the emergence of the COVID-19 outbreak, there is a sense of fear about the disease among the health sector personnel. It won't be wrong to say that COVID-19 outbreak has exposed weaknesses in our preparedness activities, but then we have to identify a balance between the outbreak response activities and the activities which are ongoing in the region for decades for the control of malaria. It is of utmost importance that all the malaria-endemic nations in the African region should continue to sustain malaria-related prevention, diagnostic and treatment services. In conclusion, the activities pertaining to the containment of COVID-19 and malaria are equally important in the African region. The services pertaining to prevention and control of malaria should be continued as it is and the public health authorities should not allow COVID-19 to become a more important priority and thus reallocate the available resources.

The COVID-19 pandemic: broad partnerships for the rapid scale up of innovative virtual approaches for capacity building and credible information dissemination in Africa.

The Corona Virus Disease 2019 (COVID-19) pandemic has rapidly spread in Africa, with a total of 474,592 confirmed cases by 11th July 2020. Consequently, all policy makers and health workers urgently need to be trained and to access the most credible information to contain and mitigate its impact. While the need for rapid training and information dissemination has increased, most of Africa is implementing public health social and physical distancing measures. Responding to this context requires broad partnerships and innovative virtual approaches to disseminate new insights, share best practices, and create networked communities of practice for all teach, and all learn. The World Health Organization (WHO)-Africa region, in collaboration with the Extension for Community Health Outcome (ECHO) Institute at the University of New Mexico Health Sciences Center (UNM HSC), the West Africa college of nurses and the East Central and Southern Africa college of physicians, private professional associations, academia and other partners has embarked on a virtual training programme to support the containment of COVID-19. Between 1st April 2020 and 10th July 2020, about 7,500 diverse health professionals from 172 locations in 58 countries were trained in 15 sessions. Participants were from diverse institutions including: central ministries of health, WHO country offices, provincial and district hospitals and private medical practitioners. A range of critical COVID-19 preparedness and response interventions have been reviewed and discussed. There is a high demand for credible information from credible sources about COVID-19. To mitigate the “epidemic of misinformation” partnerships for virtual trainings and information dissemination leveraging existing learning platforms and networks across Africa will augment preparedness and response to COVID-19.

An overview of mobile applications (apps) to support the coronavirus disease 2019 response in India

Background & objectives:The potential benefits of mobile health (mHealth) initiatives to manage the coronavirus disease 2019 (COVID-19) pandemic have been explored. The Government of India, State governments, and healthcare organizations have developed various mobile apps for the containment of COVID-19. This study was aimed to systematically review COVID-19 related mobile apps and highlight gaps to inform the development of future mHealth initiatives.Methods:Google Play and the Apple app stores were searched using the terms 'COVID-19', 'coronavirus', 'pandemic', and 'epidemic' in the first week of April 2020. A list of COVID-19-specific functions was compiled based on the review of the selected apps, the literature on epidemic surveillance, and national and international media reports. The World Health Organization guideline on Digital Health Interventions was used to classify the app functions under the categories of the general public, health workers, health system managers, and data services.Results:The search yielded 346 potential COVID-19 apps, of which 50 met the inclusion criteria. Dissemination of untargeted COVID-19-related information on preventative strategies and monitoring the movements of quarantined individuals was the function of 27 (54%) and 19 (32%) apps, respectively. Eight (16%) apps had a contact tracing and hotspot identification function.Interpretation & conclusions:Our study highlights the current emphasis on the development of self-testing, quarantine monitoring, and contact tracing apps. India's response to COVID-19 can be strengthened by developing comprehensive mHealth solutions for frontline healthcare workers, rapid response teams and public health authorities. Among this unprecedented global health emergency, the Governments must ensure the necessary but least intrusive measures for disease surveillance.
