24Model-based comparative analysis of two catastrophic hazardous chemical pipeline accidents

ABSTRACT Objectives. This study conducted a comparative analysis of two catastrophic pipeline accidents in China in order to identify some common mistakes and lessons learned to prevent similar accidents. Methods. The 24Model was used in this study, which provides a universal pathway for accident analysis from the individual level to the organizational level. Results. There were similarities between the two cases in the aspects of the occurrence, development, emergency and causation at different levels: both were caused by leaks of pipelines and evolved into multiple explosions during emergency response; both leaks were caused by the corrosion of pipelines in the confined space of a damp or salt-spray environment; both were classified as ‘responsibility accidents’, and unsafe acts, such as the failure to identify hidden hazards of pipelines that were the direct cause of accidents, reflected the shortcomings of individual safety habitual behaviour in terms of knowledge, awareness, habits and psychology; weaknesses in the organizational management mainly concerned hazard identification, pipeline maintenance, emergency disposal, etc.; and there is not a good safety climate within the organization. Conclusions. Organizations should develop a closed-loop management system and strengthen the construction of safety culture, and the government should supervise the implementation of procedures.

With the advent of the 4th industrial revolution due to the development of the communication industry, convergence between systems is actively taking place, with IoT, VR, Metaverse and AI at the center of it. Currently, simulators are used for education and simulation training in order to improve the safety culture or reduce human error in the main tasks of power facilities. However, detailed information on the phenomena occurring in power facilities during education and simulation training is not provided. Therefore, in this study, in order to improve the understanding of the relationship between driving operation and power facilities (human-machine system), we went one step further from the 3D educational video that has been used as an auxiliary material so far, and grafted virtual reality (VR) technology. This is a study on how to establish an integrated safety culture of electric power facility simulation training device (HMI), virtual reality (VR) and metaverse that can be directly experienced. And, as a research method to achieve the purpose of this study, a method combining OPC communication and Socket communication, which enables large-capacity signal processing, multi-connection, and two-way communication, was used. Because it can reduce human error, it responds to similar environments such as disasters, and furthermore, BCMS (Business Simulation Training in connection with the Continuity Management System) is also possible. In the future, it is expected that the concept to be conceived through this study will be able to proceed with additional research on the metaverse and safety culture construction related to the psychological therapy effect of ergonomics compared to the age of artificial intelligence.

Research on radiation sensitive biomarkers and status of radiation safety literacy under low dose irradiation: Radiation biomarkers and radiation safety culture

ObjectiveTo analyze the health status and radiation sensitivity index of radiation workers in provincial, county and township hospitals. To explore the methods of evaluation indicators, evaluation standards and scoring principles of radiation safety culture in medical institutions. MethodsIn 2021, the radiation safety status of all 172 medical institutions in a city was investigated by using retrospective investigation, current situation investigation and laboratory study. The occupational health monitoring data of all 1547 radiation workers were statistically analyzed, and the radiation safety status and health indicators of radiation workers in provincial, county and township level hospitals were compared. ResultsThe results of occupational health monitoring of radiation workers showed that there were statistically significant differences in ALT, lens opacity (P < 0.05) and lymphocyte micronucleus (P < 0.05) between the two groups, and there were statistically significant differences in the influence of radiation service on the detection rate of lens opacity (P < 0.05). The survey results of radiation safety culture construction in medical institutions show that there are differences in the level of radiation safety culture in hospitals of different grades. The radiation safety culture construction of provincial and municipal public hospitals is obviously better than that of county hospitals, especially township hospitals. ConclusionInterventional radiologists and nuclear medicine workers engaged in iodine therapy were exposed to higher radiation agents, and radiation workers in township health centers were not aware of radiation protection, and there was a certain occupational risk. At the same time, the evaluation index of radiation safety literacy in medical institutions was determined, Disease prevention and control personnel stationed in hospitals is an effective measure to strengthen the construction of radiation safety culture in hospitals.

Safety Culture system elements of Small and Medium-sized Construction Enterprises and its evaluation method

While the construction industry has contributed significantly to the construction of national infrastructure and the enhancement of residents’ living quality, many small and medium-sized construction industries have still been troubled by frequent accidents. Safety accidents have become an essential factor affecting their development of them. The construction of a safety culture is a critical way to solve this kind of problem and enhance the essential safety of these enterprises. Here, adopting the expert survey method, integrating the construction of safety culture with the existing system and work content, an element system of safety culture construction that contains eight primary elements and 32 secondary elements was developed, which could provide a practical and theoretical reference for the construction of safety culture in small and medium-sized construction enterprises. Through the expert survey method and hierarchical analysis method, the weights and sorting of each element were determined, which could guide the construction of enterprise safety culture efficiently. Next, a quantitative evaluation based on a fuzzy comprehensive evaluation of the overall effect of the construction of safety culture was proposed. It could support small and medium-sized construction enterprises to discover the short boards of construction, formulate strategies for continual improvement, and assess the safety culture of construction. Meanwhile, this safety culture system and evaluation method could be a reference for management departments or third-party organizations to manage and evaluate this type of enterprise.

Current challenges of university laboratory: Characteristics of human factors and safety management system deficiencies based on accident statistics

Introduction: In recent years, with the rapid development of university laboratory construction, frequent laboratory accidents have aroused widespread concern. There is an urgent need to improve laboratory safety management's effectiveness further, enhance laboratory accident prevention ability, and reduce the occurrence of accidents. Method: Based on the accident causation theory, this paper uses the accident analysis path of 24Model and the logical idea of WBA (Why-Because-Analysis) to statistically analyze the causative factors of 64 typical college laboratory fire and explosion accidents and find out the defects of current college laboratory management. Results: The study showed that unsafe human actions at the individual level were the most critical factors affecting laboratory safety management, with a high frequency of violations of experimental procedures (105 times) and managers' failure to perform their supervisory duties (98 times); low safety awareness and insufficient safety knowledge among laboratory personnel were key factors triggering unsafe actions. At the organizational level, the lack of training programs (92 times) and the lack of systematic procedures (106 times) are the weaknesses of the laboratory safety management system in general in all universities; the lack of safety culture construction is the root cause of laboratory management deficiencies. Conclusions: Based on the above statistical results, and taking into account the characteristics of university laboratories themselves, the root causes of poor safety are specifically analyzed and preventive measures are proposed in six areas to address the key causes of accidents. Practical Applications: The results of this study are essential for improving the ability to prevent accidents in flammable and explosive laboratories in universities.

Disaster Capitalism within Aviation Industry: Putting Corporate Profits Ahead of Safety

It is predicted that Indonesia will soon serve more than 140 million passengers a year. However, the safety records of the aviation industry have not shown a significant progress. Airlines operated in Indonesia has repeatedly experienced serious accidents killed hundreds of passengers and crew. The purpose of this study is to explore corporate communications related to safety culture. This study is concerned with the ideological role played by language in the discursive construction of safety culture. Using Thompson’s (1990) schema regarding the modes of ideology, this study analyzes annual reports of two airlines operated in Indonesia published in the last fourteen years to establish the linguistic strategies used in their communications related to safety culture. The results suggest that the companies used particular linguistic strategies in their communication of safety culture to advance the worldviews of economic efficiency and cost control based on capitalist logic. The prioritization of business growth may contribute to avoiding allocating resources on aspect that is matter most. Given high expectation of safety standards of the aviation industry, the findings have clear and significant implications. It is recommended that aviation industry must improve their safety cultures to prevent unnecessary accidents in the future.

Safety culture system construction in coal mine enterprises from the perspective of hazards: A case study on the Yimei Coal Group.

The construction of a safety culture in coal mine enterprises is an essential component of coal mine safety management. Current studies do not address the mechanism for forming and constructing systems for coal mine enterprise safety culture, and many studies are based on imperfect theoretical frameworks and unsystematic empirical research, their policy recommendations are not systematic or operable, and they offer no feasible safety culture construction system. This study is devoted to analysing the theoretical basis of safety culture construction in coal mine enterprises from the perspective of hazards and proposing the content, stages and targets of constructing a safety culture and develops pragmatic approaches for coal mining enterprises to improve safety culture. A theoretical basis for safety culture construction in coal mine enterprises is proposed from the perspective of hazards by considering accident-causing mechanisms based on hazards. Furthermore, this study applied the case analysis and application to conduct empirical research on the proposed theoretical basis for safety culture construction from the perspective of hazards. Four aspects are proposed to capture the content and objectives of safety culture construction from the perspective of hazards: safety concept, behaviour safety, material state safety, and safety institutions. Furthermore, this paper provides a case study of safety culture construction by the Yimei coal group from the perspective of hazards, identifies the hazards based on the above four aspects, and then identifies preventative measures and controls for the identified hazards. Constructing a safety culture in coal mine enterprises from the perspective of hazards is operable and practical, and thus this study provides essential theoretical and practical value for improving coal mine safety.

An Evaluation Approach of Community Emergency Management Ability Based on Cone-ANP.

In order to improve the emergency management ability of urban communities in response to emergencies and reduce the losses caused by emergencies, based on the method of Cone-Analytic Network Process (Cone-ANP), a whole-process community emergency management ability evaluation method was proposed. Using around 25 evaluation indexes from six dimensions, namely infrastructure resilience, community organization resilience, risk management, emergency material support, emergency force building, and emergency literacy, this method established the dominant relationship of each index by the analysis of the cone network structure. It determined the community safety culture construction as the cone-top element, and obtained the limit weight vector of all the evaluation indexes by expert evaluation. The membership degree of each index was calculated to quantify the evaluation results of community emergency management ability. The results could provide a guidance and reference basis for community emergency management.

Construction of Emergency Culture and Safety Evaluation Index System Based on Behavioral Safety "2-4" Model

With the frequent occurrence of production safety accidents in enterprises in China, it has become a broad consensus of the whole society to attach importance to the emergency management of production safety in enterprises. Taking the coal mine as an example, the frequent safety accidents in coal mine enterprises have seriously affected human health, safety and environment, highlighting the fragility of safety management and safety culture construction in coal mine enterprises. To fundamentally reduce or eliminate the occurrence of safety accidents in coal mining enterprises, the most important and effective safety management strategy is to build a long-term safety culture. The "2-4" model of behavior safety is introduced into the construction of coal mine emergency culture, hoping to further reduce coal mine safety accidents and build an evaluation system of enterprise culture construction with the characteristics of coal mine industry. In the development stage of coal mine, a reasonable mining plan should be made, and comprehensive preparations should be made. In the tunneling stage, the construction safety and progress should be strictly managed and supervised. It will be beneficial to the standardization and scientific development of coal mine commercial enterprise culture, is the inherent requirement of building advanced enterprise culture, and promotes the safe and healthy development of coal mines.

Comprehensive Evaluation of University Safety Culture Construction Level Based on “2–4” Model

Based on the "2-4" model of accident causation, a comprehensive index system of the safety culture construction level in colleges and universities is set up. This system consists of 4 primary indicators and 28 secondary indicators. Taking a university as an example, applying the analytic hierarchy process and fuzzy comprehensive evaluation method, this study establishes a comprehensive evaluation model to evaluate the construction level of the university's safety culture. The results show that the construction level of the university's safety culture is consistent with the actual situation. This study provides useful insights and feasible paths for promoting the safety and stability of colleges and universities and building a higher level of safety on campus.

The Mechanism of Safety Culture Construction in Chinese Universities Based on POI Data and Gravity Model

Safety culture includes concept culture, system culture, behavior culture, and material culture. With the development of social economy, the importance of safety culture has been recognized by the whole society. Campus security is not only related to the common interests of the campus teachers and students but also related to the fundamental interests of each individual in the campus. In order to build a “harmonious society,” building a “harmonious campus” is an important work. Strengthening the construction of safety culture in colleges and universities is not only the objective need of building a harmonious society, promoting the reform and development of colleges and universities, and running a university with the satisfaction of the people but also the urgent requirement of cultivating high-quality professionals with safety responsibility consciousness and safety behavior ability. Starting from the history of safety culture construction, this paper reviews and summarizes the course of safety culture construction in colleges and universities, analyzes the present situation of safety culture construction in colleges and universities, and takes the safety construction theory as the guide. This paper introduces an automatic extraction technology of Internet map POI data. The organization form of Html webpage content, the automatic analysis, and the extraction of POI are realized by programming. The experimental results show that the address standardization method proposed in this paper has high accuracy of address standardization, and the weighted multiattribute similarity POI matching method can also effectively update the increment of POI data.

How resilient safety culture correct unsafe behavior of new generation of construction workers: the mediating effects of job crafting and perceived work meaningfulness

PurposeThe purpose of this paper is to explore the correction mechanism of resilient safety culture on new generation of construction workers (NGCWs)' unsafe behavior and test the multiple mediation effects of job crafting and perceived work meaningfulness based on the context of Chinese construction industry in order to find a new way to effectively correct the NGCWs' unsafe behavior.Design/methodology/approachA theoretical model of correction mechanism was established based on literature research and theoretical deduction. An empirical study was employed based on confirmatory factor analysis and regression analysis with a sample of 404 NGCWs in China.FindingsThe results indicated that resilient safety culture can effectively correct NGCWs' unsafe behavior through job crafting and perceived work meaningfulness. Job crafting and perceived work meaningfulness can play independent and serial mediating roles between resilient safety culture and unsafe behavior.Research limitations/implicationsResearch results only represent a short-term law about the correction mechanism of NGCWs' unsafe behavior based on a questionnaire study from China's construction industry. It is necessary to continue to implement a longitudinal study to test it in a relatively long period in future research. The findings also need to be verified based on the young construction workers in other countries.Practical implicationsThis study provides a theoretical basis and feasible management reference for construction enterprises in China to correct NGCWs' unsafe behavior from the perspective of resilient safety culture. Furthermore, the construction of resilient safety culture in construction enterprises can help NGCWs better carry out job crafting and perceive the meaning of work.Originality/valueThis paper clarifies the correction mechanism of resilient safety culture on unsafe behavior of NGCWs, and further tests the independent mediating roles and a serial mediating role of job crafting and perceived work meaningfulness between resilient safety culture and unsafe behavior, which fills the research gap about the influence mechanism of resilient safety culture on young construction workers' unsafe behavior and enriches the theoretical system of unsafe behavior correction of construction workers.

The status of infection prevention and control structures in Eastern China based on the IPCAF tool of the World Health Organization

BackgroundThe burden of healthcare-associated infections (HAIs) and the spread of antimicrobial resistance can be potentially preventable through comprehensive infection prevention and control (IPC) programs. However, information on the current state of IPC implementation is rare in China.MethodsA cross-sectional study was conducted in Zhejiang province, China, from April to June 2021. The Zhejiang nosocomial infection control and quality improvement center (NICQI) cooperated with 11 municipal NICQI centers to introduce the purpose of this study and invite all licensed secondary and tertiary hospitals in Zhejiang province through WeChat group. The questionnaire had three sections, including information about participating hospitals, demographic information about IPs, and the Chinese version of the Infection Prevention and Control Assessment Framework that covered eight core components (CC).ResultsOf the 382 hospitals invited, 222 (58.1% response rate) accepted and completed the online questionnaire. The overall median score of the participating hospitals was 682 (630–723), which corresponded to an advanced level of IPC. There was a significant difference in scores between hospitals types (P < 0.001). Profound differences were revealed regarding the scores of the individual components, with CC2 (IPC guidelines) and CC6 (Monitoring/audit of IPC practices and feedback) having the highest (100) and lowest (65) median scores, respectively. Only 23 (10.4%) hospitals reported assessing facility safety culture.ConclusionsIPC structures are at a relatively high level in acute care hospitals in Eastern China. The identified potential areas for improvement were similar to those identified in developed countries, particularly regarding multimodal strategies for implementation and safety culture construction. Meanwhile, the Chinese government should pay more attention to IPC resources and practices among secondary care hospitals.

A multi-group assessment of safety culture among engineering students in the United Arab Emirates

PurposeThe purpose of this paper is to identify the factor structure of safety culture construct among engineering students at university context and to examine the measurement invariance of this instrument across different socio-demographic groups in a sample of engineering students in the United Arab Emirates (UAE).Design/methodology/approachAn exploratory online questionnaire was completed by 770 undergraduate and postgraduate engineering students across the UAE. Data were analyzed using a diversified multi-group and a robust and sophisticated cross-validation testing strategy. Confirmatory factor analysis (CFA) was used to test factor structures identified in previous studies. Multi-group invariance testing was conducted to determine the extent to which factor structure is comparable across groups (i.e. gender, educational and experiential background).FindingsThree-factor model was preferred for its parsimony. The results showed that the level of safety awareness and attitude is relatively satisfactory, whereas safety behaviour is inadequate. No significant difference was showed in multi-group invariance between demographic groups.Research limitations/implicationsThis research is a cross-sectional study and limited to the views of engineering students (informal group). The study would benefit from both informal and formal groups in assessing safety culture at university for a robust empirical evidence. The research highlights relevant implications for policy and program development, by pointing to the need to promote safety culture and mitigate safety-related accidents among engineering students.Originality/valueThis paper offers insight into benefit of understanding the level of safety culture among engineering students and extend knowledge of informal group involvement in safety-related accidents at university level.

The Significance of Ideological Education to the Construction of Enterprise Safety Culture

Multi‐agent evolutionary game of process safety culture in Chinese chemical industry based on system dynamics

AbstractIn the process of safety culture construction in Chinese chemical industry, various interests and relationships of stakeholders involved are complicated, which hinders the process safety culture in chemical industry from being implemented practically. Thus, evolutionary game theory was used to draw the relationships between the stakeholders, including China chemical safety supervision department, chemical plants, and chemical plant workers. A system dynamics model was built to explore the dynamic relationships of process safety culture in Chinese chemical industry, and study the dynamic stability of stakeholders. The simulation results show that the strategy selections of China chemical safety supervision department, chemical plants, and chemical plant workers could reach a stable condition, which indicates that the evolutionary stable strategy exist in the current dynamic interaction among the three agents. Furthermore, a sensitivity analysis between state variables and several auxiliary variables were accomplished to reveal the internal relationships. The proposed new method of applying evolutionary game to process safety culture offers a solution to optimize safety culture construction.

Study on WD Petrochemical Company Safety Management Improvement

The study takes the safety management of a domestic petrochemical company as the research object, collects and summarizes relevant information, and analyses the current situation in four aspects, including its organizational structure, safety management system, safety culture construction and behavioral control, so as to identify the problems that exist, such as the adverse impact of management changes, the lack of depth in the construction of safety management system, and the lack of strength in the construction of safety culture. The study also compares and draws on the safety management concepts and systems of well-known domestic and international chemical companies such as DuPont, Toyota and Sinopec, for example, establishing the concept that safety investment is efficiency and extending production safety to the lives of employees, and proposes measures to absorb and apply these achievements in the context of the actual situation of the company. The study proposes a "1-2-4 model" based on the existing "2-4 model" of accident causation: a safety information platform, two levels of organization and individual, the root cause of culture, the fundamental cause of system, the indirect cause of employee awareness and knowledge, and the direct cause of employee behavior. In view of the problems of enterprise safety management, based on the model, this paper proposes measures to improve WD's safety management, including planning, creating a safety information platform, establishing a safety management organization and employing professional staff, introducing solid system construction and benchmarking activities, cultivating a progressive corporate culture, creating a good working atmosphere, truly putting people first, putting strong training and education in place, raising staff safety awareness, teaching employees about safety, adopting effective motivational methods, etc. This will promote the continuous updating and improvement of WD's safety management and ensure a healthy and sustainable development of the company as well as a harmonious community business relationship.
