Squamous Cell Carcinoma in the Lacrimal Sac of Young Patient Who Had Conjunctival Squamous Papilloma

ëª©ì  ì Šì€ 환자에서 흔하게 나타나지 않는 누낭 íŽ¸í‰ì„¸í¬ì•”ì¢ ì„ 경험하여 이를 ë³´ê³ í•˜ê³ ìž 한다. 증례요약 15ë „ ì „ 하비측 ê²°ë§‰íŽ¸í‰ì„¸í¬ìœ ë‘ì¢ ì„ ì ˆì œí•œ ì  있는 30세 여자 환자가 1주 ì „ë¶€í„° 발생한 우측 하부눈물길 부위의 통증과 ì¢ ê´´ë¥¼ 주소로 내원하였다. 누낭염 의심하에 10일간 í•­ìƒì œ 치료하였으며, ì˜ìƒí•™ì  검사상 비루관폐쇄 진단받은 후 1.7 × 1.7 cm 크기의 누관 ë‚­ì¢ , 누낭류 의심 소견으로 내시경하 ëˆ„ë‚­ë¹„ê°•ì—°ê²°ìˆ ê³¼ ì ˆì œìƒê²€ì„ 시행하였다. ì¡°ì§ë³‘ë¦¬í•™ì  검사 결과상 초기 ì¹¨ìŠµì  íŽ¸í‰ì„¸í¬ì•”ì¢ ìœ¼ë¡œ 진단되어 이후 ì¶”ê°€ì ì¸ ì ˆì œìƒê²€ì„ 시행하였다. ì•”ì¢ ìœ¼ë¡œ 진단된 ì¢ ê´´ëŠ” ì™„ì „ì ˆì œë¥¼ 시행하였으며, ì ˆì œëœ 주위로 ì• ì„± 세포가 없음을 확인하였다. ê²°ë¡ ëˆ„ë‚­ íŽ¸í‰ì„¸í¬ì•”ì¢ ì€ 드물게 나타나며 ì Šì€ 환자에게서는 더 흔하지 않은 것으로 ì•Œë ¤ì ¸ 있다. 본 증례를 통해 ì—°ë ¹ê³¼ 상관없이 누낭의 ì• ì„± ì¢ ì–‘ 발생 가능성을 ë°°ì œí•´ì„œëŠ” 안 ë ê²ƒìœ¼ë¡œ 생각되며, 누낭 íŽ¸í‰ì„¸í¬ì•”ì¢ ì€ ê³ ìœ„í—˜ ì‚¬ëžŒìœ ë‘ì¢ ë°”ì´ëŸ¬ìŠ¤ì™€ 연관성이 있기에 ê²°ë§‰íŽ¸í‰ì„¸í¬ìœ ë‘ì¢ ì˜ ë³‘ë ¥ì´ 있는 경우 ê³ ë ¤í•´ì•¼ 되는 질환으로 생각된다. Combined subconjunctival pegylated (Peg) interferon alpha 2a injection and amniotic membrane graft for management of extensive conjunctival papillomatous neoplasia- A case report

Purpose: To report the management of a case presented with recurrent extensive conjunctival papillomatous neoplasia. Case Report: A 2 year old girl presented with left recurrent extensive conjunctival squamous papilloma being treated surgically thrice along with adjunctive cryotherapy and topical Mitomycin C (MMC). We managed the case with surgical excision again with adjunctive subconjunctival or sublesional pegylated (peg) interferon alpha 2a injection along with the whole surface amniotic membrane graft. The patient had upper canalicular papillomatous lesion which was also managed by surgical excision and subcanalicular peg-interferon alpha 2a injection. There was no recurrence in subsequent 3 months follow up. Conclusion: Subconjunctival pegylated interferon alpha 2a injection and amniotic membrane graft may be a novel combined modality to manage extensive conjunctival squamous papillomatous neoplasia that may yield successful outcome. MACE RNA sequencing analysis of conjunctival squamous cell carcinoma and papilloma using formalin-fixed paraffin-embedded tumor tissue

Recent advances in the field of biomedical research allow for elucidation of the transcriptional signature of rare tumors such as conjunctival squamous cell carcinoma (SCC). In this study we compare its expression profile to conjunctival papilloma (Pap) and healthy conjunctival tissue (Ctrl) and develop a classification tool to differentiate these entities. Seven conjunctival SCC, seven Pap and ten Ctrl were formalin-fixed and paraffin-embedded (FFPE) and analyzed using Massive Analysis of cDNA Ends (MACE) RNA sequencing. Differentially expressed genes (DEG) and gene ontology (GO) clusters were explored and the abundance of involved cell types was quantified by xCell. Finally, a classification model was developed to distinguish SCC from Pap and Ctrl. Among the most prominent DEG in SCC a plethora of keratins were upregulated when compared to Pap and Ctrl. xCell analysis revealed an enrichment of immune cells, including activated dendritic cells and T-helper type 1 cells (Th1), in SCC when compared to Ctrl. The generated classification model could reliably discriminate between the three entities according to the expression pattern of 30 factors. This study provides a transcriptome-wide gene expression profile of rare conjunctival SCC. The analysis identifies distinct keratins, as well as dendritic and Th1 cells as important mediators in SCC. Finally, the provided gene expression classifier may become an aid to the conventional histological classification of conjunctival tumors in uncertain cases.

Morphological spectrum of excised conjunctival lesions in a tertiary hospital in North-Western Nigeria

Context: Conjunctiva is a transparent membrane lining the inner surface of the eyelids and the globe up to the corneoscleral limbus. It is a common site for the growth of lesions in the eye. Depending on their origin, conjunctival lesions could be non-neoplastic and neoplastic. The neoplastic lesions are either benign or malignant. These lesions can be distinguished based on the patient's history, clinical presentation and histopathological features. Aims: In this study, the pattern of excised conjunctival lesions was determined. Methods and Material: This was a 6-year (2010–2015) retrospective study of all excised conjunctival lesions diagnosed at the Pathology Laboratory of Aminu Kano Teaching Hospital. Statistical analysis used: Data were analysed with Statistical Package for Social Sciences (version 20.0; SPSS, Chicago, IL, USA). Results: A total of 198 conjunctival lesions were diagnosed during the period. The male patients were 136 (68.70%), whereas the females were 62 (31.30%) with male-to-female ratio of 2.2:1, with a mean age ± standard deviation of 37.69 ± 16.39 years. The neoplastic nature of the lesions was as follows: 54 (27.3%) were malignant, 61 (30.8%) were pre-malignant and 83 (41.9%) were benign. The most common lesions observed were conjunctival squamous cell carcinoma, dysplasia and squamous papilloma with frequencies of 50 (25.30%), 46 (23.20%) and 24 (12.10%), respectively. Conclusions: Benign lesions are the most common conjunctival lesions, whereas squamous papilloma and squamous cell carcinoma are the most common benign and malignant lesions, respectively.

Ocular Human Papillomavirus Infections.

- Human papillomavirus (HPV) has a well-known role in the pathogenesis of squamous cell carcinoma and precursor lesions of the cervix, anogenital region, and head and neck, but its role in the development of squamous neoplasms of the eye, particularly the conjunctiva, remains unclear. - To review recent evidence implicating HPV in the pathophysiology of ocular lesions. - Published articles obtained from a PubMed search of the English literature were the primary sources for this review. - The low-risk HPV types 6 and 11 appear to play a role in the development of at least a subset of conjunctival squamous papillomas. The role of HPV in the pathogenesis of pterygium and ocular surface squamous neoplasia is less well defined. There is evidence to suggest that HPV may be a cofactor in the development of these lesions, acting in concert with ultraviolet radiation and/or human immunodeficiency virus infection in a subgroup of cases.

Squamous Papillomas of the Conjunctiva in Dogs: A Condition Not Associated With Papillomavirus Infection.

Papillomas of the conjunctival surface in people can be of viral or nonviral origin and are found in high association with human papillomavirus. Canine conjunctival papillomas are seldom described, and published accounts have mostly been associated with canine oral papillomavirus infection. Here, we describe conjunctival squamous papillomas that do not express papillomavirus proteins and compare them with papillomavirus-associated conjunctival papillomas. Conjunctival squamous papillomas presented a distinct histopathologic profile and lacked the cytopathic effects seen in viral papillomas. They appeared as exophytic, papilliferous, pedunculated lesions with delicate fronds and angular terminal margins. Squamous papillomas presented with a delicate fibrovascular core and were associated both clinically and grossly with a feeder vessel. Pigmentation was variable within the epithelium and stroma of these lesions, and inflammatory infiltrates were characteristically minimal. Conjunctival squamous papillomas resembled squamous papillomas of the skin; however, they lacked significant hyperkeratosis. Compared with conjunctival viral papillomas, these masses occurred in older dogs and were smaller and solitary. Furthermore, polymerase chain reaction and immunohistochemistry failed to demonstrate papillomavirus genetic material and antigens in conjunctival squamous papillomas. Both viral and nonviral conjunctival papillomas were considered benign.

Ocular rhinosporidiosis mimicking conjunctival squamous papilloma in Kenya – a case report

BackgroundOcular rhinosporidiosis is a chronic granulomatous infection caused by a newly classified organism that is neither a fungus nor bacterium. It often presents as a benign conjunctival tumour but may mimic other ocular conditions. It is most often described in India. In Africa cases have been reported from South Africa, Kenya, Tanzania, Malawi, Uganda, Congo and Ivory Coast.Case presentationA 54 year old man was seen in Kenya with a lesion that resembled a conjunctival papilloma. We report resemblance to conjunctival papilloma and the result of vital staining with 0.05% Toluidine Blue.ConclusionOcular rhinosporidiosis occurs in East Africa. It may resemble conjunctival squamous papilloma. Vital staining with 0.05% Toluidine blue dye did not distinguish the two lesions well.

Clinicopathologic and Immunohistochemical Studies of Conjunctival Large Cell Acanthoma, Epidermoid Dysplasia, and Squamous Papilloma

To evaluate clinicopathologically and immunohistochemically a spectrum of conjunctival squamous proliferations. Retrospective clinicopathologic study. One large cell acanthoma, 7 epidermoid dysplasias, and 4 squamous papillomas were evaluated with microscopy and biomarkers Ki-67, p53, epithelial membrane antigen (EMA), Ber-EP4, AE1, AE3, and 8 individual cytokeratins. Normal associated conjunctiva served as a baseline for interpretation. The large cell acanthoma recurred 4 times but retained its benign histopathologic features. The cells were 2-3 times larger than the keratinocytes of the normal conjunctiva and did not display atypia. Immunohistochemistry revealed a low Ki-67 proliferation index (PI) in the large cell acanthoma compared with high indices in dysplasias and papillomas. p53 was negative in the nuclei of normal epithelium while positive in all neoplasms, most intensely in the dysplasias. Immunostaining showed similar staining patterns for cytokeratins in large cell acanthoma and normal conjunctiva, except for full-thickness CK14 positivity and CK7 negativity in the lesion. Dysplasias generally lost normal CK7 expression and frequently abnormally expressed CK17. The papillomas displayed a normal cytokeratin pattern but exhibited a higher than normal PI and weak p53 positivity. Conjunctival large cell acanthoma is a morphologically distinctive clonal entity with clinical and immunohistochemical phenotypic characteristics denoting a dysplasia of minimal severity. Because of recurrences without invasion, it requires treatment. Dysplasias exhibited more deviant biomarker abnormalities including frequent aberrant full-thickness CK17 positivity and CK7 negativity. The absence of major cytokeratin derangements in the squamous papillomas may beof ancillary diagnostic value for lesions displaying borderline cytologic features.

A pathologic review of ophthalmic tumors in Kano, Northern Nigeria

Background and Objective: Tumors of eye and ocular adnexa are reported to be common in Africa, but no formal study has been conducted here in Kano, the largest Northern Nigerian city. This study is a documentation of the pattern of ophthalmic tumors in our locality. Materials and Methods: This was a 12-year (1999-2010 inclusive) retrospective study of all orbito-ocular neoplasms diagnosed at the pathology department of a teaching hospital. Results : 438 ophthalmic tumors were diagnosed during the study period, of which 79.7% were malignant and 20.3% benign. Males were slightly more preponderant with M/F ratio of 1.3:1. Retinoblastoma and conjunctival squamous carcinoma were the commonest tumors respectively comprising 44.5% and 18.9%. These two ocular malignancies also largely accounted for the bimodal age distribution peaks of ophthalmic cancer in the 1 st and 4 th decades. Conjunctival squamous papilloma and hemangioma were the most prevalent benign ophthalmic tumors. Conclusion : Our findings were broadly similar to most other Nigerian and African studies but significantly at variance with those of the western world.

Squamous papillomas of the conjunctiva: A retrospective clinicopathological study

There is very limited literature on squamous papillomas of the conjunctiva from Nigeria and sub-Saharan Africa. In an attempt to contribute to the literature on the subject, we studied the clinicopathological characteristics of patients histologically diagnosed with squamous papilloma of the conjunctiva in Ibadan, Nigeria. Clinical and pathological records of patients with histological diagnosis of squamous papilloma of the conjunctiva made in the Department of Pathology, University College Hospital, Ibadan, between January 1985 and December 2004, were reviewed. There were totally 26 cases. Patients' ages ranged from 2 to 58 years with a mean age of 32 years. Male to female ratio was 1.4:1. Size of tumors ranged from 2 to 10 mm. Duration of presenting complaints was from 2 months to 10 years. The lesions in 10 cases were located in the medial canthus, at or close to the limbus in another 10 cases, and in the tarsal conjunctiva in a single case. Five cases had no documentation of location. Sixteen cases (61.5%) had multiple papillomas. Four cases had a history of chemical injury (alkaline based - hair relaxer in a single case, acid based - wet cell car battery fluid in two cases, and unknown chemical in a single case) preceding the lesion by at least 6 weeks. Fourteen cases had koilocytosis on histology suggestive of Human Papilloma Virus (HPV) etiology. HIV screening test was negative in the three patients who had the screening done. Preoperative clinical diagnosis was squamous papilloma in 16 cases, pterygium in 6 cases and squamous cell carcinoma in 2 cases. In the environment where we practice, conjunctival squamous papilloma occurs most commonly in the fourth decade of life. Only very few cases are submitted for histological diagnosis. HPV infection and chemical injury are the main etiology. We strongly advocate that all excisions of the conjunctiva, irrespective of the clinical impression, should be submitted routinely for histological assessment.

Detection of Canine Oral Papillomavirus DNA in Conjunctival Epithelial Hyperplastic Lesions of Three Dogs

Papillomavirus infections are responsible for plaques and papillomas in various locations on the skin and in mucous membranes. The aim of this report was to describe morphologic features of a viral pigmented conjunctival plaque and 2 conjunctival squamous papillomas in 3 dogs, and to investigate these lesions for the presence of papillomavirus DNA by polymerase chain reaction (PCR), DNA sequence analysis, and in situ hydridization (ISH). Histopathology revealed in all neoplasms various degrees of epithelial hyperplasia, acanthosis, and hyperkeratosis with koilocytosis. In all lesions E6, E7, and L1 gene fragments of canine oral papillomavirus (COPV) DNA were detected by PCR and sequencing analysis. ISH revealed COPV DNA in a highly specific pattern within nuclei of the hyperplastic epithelium. The presence of canine papillomavirus in ocular conjunctival plaques and papillomas suggests these benign lesions may have the potential for malignant transformation. This is the first time that the lambdapapillomavirus COPV has been detected in ocular epithelial hyperplastic lesions.

Topical interferon α-2b as an adjunctive therapy in recurrent conjunctival papilloma

Conjunctival squamous papilloma is a benign tumor of presumed viral origin. Small sessile papillomas may regress spontaneously, but an extensive papilloma as seen in adults requires treatment [1]. Papillomas of the conjunctiva are however known for their high recurrence rate of 6–27% [2]. Several types of therapy have been described, including liquid nitrogen cryotherapy, cautery, diathermy, irradiation, and carbon dioxide laser treatment. Topical and systemic interferon have also been tried and found effective [3–6]. We report a case of recurrent papilloma treated successfully with surgical excision, amniotic membrane transplant, and topical interferon a-2b.
