Aims & Objectives: Bronchobiliary fistula is a rare developmental abnormality. Until 2019 only 44 cases had been reported worldwide. Clinicians should have a high index of suspicion to make an early diagnosis and start prompt treatment to reduce mortality. All cases should undergo surgery in order to avoid pulmonary complications. Methods: We report a case of a 45-day-old female patient admitted to the Pediatric Intensive Care Unit after a Brief Resolved Unexplained Event. Severe gastroesophageal reflux was suspected but failed to conventional treatment and developed respiratory distress requiring mechanical ventilation Results: Bile was obtained through the orotracheal tube and a 3D computed tomography and bronchoscopy were performed to demonstrate anatomic malformation between the biliary tract and the bronchial tree. A bronchobiliary fistula was observed between the main right bronchus and the biliary tract. Surgery was performed on day 95 after birth requiring mechanical ventilation for 75 days. Pulmonary chronic disease was diagnosed at time of discharge and inhaled steroid was started immediately. Conclusions: Congenital bronchobiliary fistula is a rare disease; survival rates and prognosis depends on a high index of suspicion to make an early diagnosis and rule out other biliary tract malformations. Prognosis rates are improved managing pulmonary complications due to biliary fluid in the pulmonary tree until a surgical treatment can be carried out.

Congenital bronchobiliary fistula (CBBF) is a rare anomaly in which a fistulous opening exists between the biliary tract and the tracheobronchial tree. CBBF may be accompanied by many congenital anomalies, with biliary system anomalies being the most common. CBBF was also reported to be associated with esophageal atresia in one case and right diaphragmatic hernia in another case. However, as far as we have known, CBBF case accompanied by multiple congenital anomalies has not been reported previously. In this article, we firstly present a case of a neonatal CBBF, incidentally diagnosed intraoperatively, accompanied by multiple congenital anomalies, including congenital diaphragmatic hernia. Secondly, we present our conclusions and updates about CBBF based on what we have learned from both our case and the literature.

Anesthesia management in neonatal congenital bronchobiliary fistula: case report and literature review

BackgroundThere is very little published literature and none that discussed care in a neonate regarding anesthetic risk and management of neonate with congenital bronchobiliary fistula during thoracoscopy and thoracotomy. This article analyzes related risk factors and literature review from perioperative ventilation, circulation and other aspects of management.Case presentationA neonate diagnosed as congenital bronchobiliary fistula combined with severe chemical pneumonia, consolidation of the lungs, and infection was facing the risk of anaesthesia under thoracoscopy exploration surgery, who experiened more than 20 days diagnostic period before operation. Many risk factors have led to conversion from minimally invasive surgery to thoracotomy, including persistent hypoxemia, hypercapnia, difficult surgical exposure and extremly difficulty of intraoperative ventilation management. Anesthesia maintenance after conversion to open access remained problematic. Fortunately the patient showed no sign of any adverse CNS effects after 4 months of follow-up.ConclusionsThe most prominent anesthesia challenges are hypoxemia, increased airway resistance, impaired ventilation, and the risk of metabolic acidosis. Close cooperation among the entire neonatal medical team is the key factors in successful management of this rare case.

Minimally invasive thorascopic surgery in treating congenital bronchobiliary fistula

Abstract We summarized our minimally invasive thorascopic surgical experience of a rare congenital bronchobiliary fistula case and made a brief review of the literature. To our knowledge this is the first reported case treated by a total minimally invasive thorascopic surgery. Typical symptoms and signs, surgical experience, and pathological results were summarized with pictures. Intra-operative fistulography is of great importance to surgical decision making. Minimally invasive thorascopic surgery is safe and feasible in treating congenital bronchobiliary fistula without coexisting biliary tract malformation.

Surgical Experience in a Baby With Congenital Broncho-Biliary Fistula

Congenital tracheobiliary and bronchobiliary fistulae are rare malformations in which patent communications exist between the respiratory system and biliary tract, respectively. We present a newborn who was admitted with respiratory distress and bilious tracheal discharge. Investigation revealed a bronchobiliary fistula originating from the left main bronchus, as well as biliary atresia. Excision of the bronchobiliary fistula was successful and the connection between biliary tract and gastrointestinal system was established by performance of a Roux-en-Y cholecysto-jejunostomy. Diagnostic tools, differential diagnosis, and surgical correction strategies are discussed.

Congenital Bronchobiliary Fistula in a 65-year-old Woman

Congenital bronchobiliary fistula (CBBF) is quite a rare malformation and the diagnosis is usually made within a few hours or years from birth because of lower respiratory diseases beginning from early infancy. Surgical repair is necessary. Of the 29 cases reported, 4 occurred in adults aged 22-32 years. We detected CBBF incidentally in a 65-year-old woman. During bronchoscopy and thoracic computed tomographic study of the pulmonary nodules, we found an accessory bronchus descending from the carina and composed of a dark green secretion that contained 10% bilirubin. Drip infusion cholangiography revealed air in the left bile duct. Cholescintigraphy showed dilatation of the left bile duct and radiotracer pooling at the top edge of the left hepatic lobe. These findings indicated a narrow fistula between the airway and biliary duct. We attributed the patient's long survival without major complications to the narrowness of the communication. To our best knowledge, this is the fifth and oldest reported adult diagnosed with CBBF.

The liver was herniated through the diaphragmatic defect into the right chest where a fistula tract was identified from the surface of the liver to the right mainstem bronchus. Congenital bronchobiliary fistula: Diagnosis and postoperative surveillance with HIDA scan

Congenital bronchobiliary fistula (CBBF) is an extremely rare anomaly with myriad presentations that often include common bile duct abnormalities. Traditionally, bronchoscopy and bronchography have been used to establish the diagnosis. A full-term neonate with pneumonia and bilious secretions was diagnosed as having CBBF by means of a HIDA scan. Successful repair consisted of a right thoracotomy, high ligation of the fistula via an extrapleural approach, and cholecystography to confirm biliary drainage into the duodenum. The HIDA scan is a safe and efficient means to diagnose CBBF. When used in combination with surgical contrast studies, it provides clear delineation of all variations of this abnormality, enabling one-stage correction.

Congenital bronchobiliary fistula: Management of the involved hepatic segment

We present three children with congenital bronchobiliary fistula (CBBF). The first infant underwent resection of the fistula at age 3 weeks and was reported as a success. She returned 7 years later with bilious expectoration for which a second thoracotomy was performed. One year thereafter a bile collection in the right chest required open drainage. Subsequently, the left hepatic lobe was resected. The second child was initially diagnosed as suffering from gastroesophageal reflux. He underwent a fundoplication and a gastrostomy at age 9 months. Persistence of bilious sputum led to the diagnosis of CBBF at age 23 months. The fistula was excised through a right thoracotomy. The distal end was injected demonstrating an arborization involving the entire left hepatic lobe. Since there was no communication with the remaining biliary tree or the intestine, a left hepatectomy was performed. The third child was reported in a communication focusing on the diagnosis of CBBF by hepatobiliary scintigraphy. Her history is almost identical to the second patient. However, instead of a hepatectomy, she underwent a Roux-en-Y drainage of the fistula. The three children had uneventful recoveries and are well. We believe that an abnormal biliary tract should be studied by contrast injection. If no decompressing communication with the normal biliary ducts or the duodenum can be demonstrated, the involved hepatic segment should be resected or internally drained. Children who underwent simple fistula excision must have close long-term follow-up.

Congenital bronchobiliary fistula is a rare malformation. We have presented a case that was diagnosed and treated successfully in an adult. The diagnosis was made by bronchoscopy and bronchography. After excision of the fistula the patient recovered completely. For each patient in whom this anomaly is suspected, bronchoscopy and bronchography are useful for diagnosis.

The 12th case of congenital bronchobiliary fistula is reported. This is the eighth and youngest successfully treated case of this malformation. Bronchoscopy without unnecessary delay is the key to early diagnosis in this anomaly.

Summary A seventh case of congenital bronchobiliary fistula is presented in a 4-day-old infant. This is also the first bronchobiliary fistula associated with esophageal atresia and tracheoesophageal fistula.

A sixth case of congenital bronchobiliary fistula is reported in a 3-week-old infant. This is the fourth surgical success and the youngest patient reported.

Congenital bronchobiliary fistula is a rare but distinct congenital anomaly. In the5 recorded cases the lesions were apparently identical: a large fistula extending from the right main stem bronchus to the left hepatic duct system. Clinically the patients presented with aspiration pneumonia and atelectasis of variable degree. The diagnosis can be made only by bronchoscopic and bronchographic examination. Definitive treatment consists of excision of the thoracic portion of the fistula; it was successful in 3 patients.
