An overview of the different digital facebow methods for transferring the maxillary cast into the virtual articulator.

The purposes of this study were to classify the described digital facebow techniques for transferring the maxillary cast into the semi-adjustable virtual articulator based on the digital data acquisition technology used and to review the reported accuracy values of the different digital facebow methods described. Digital data acquisition technologies, including digital photographs, facial scanners, cone beam computed tomography (CBCT) imaging, and jaw tracking systems, can be used to transfer the maxillary cast into the virtual articulator. The reported techniques are reviewed, as well as the reported accuracy values of the different digital facebow methods. Digital photographs can be used to transfer the maxillary cast into the virtual articulator using the true horizontal reference plane, but limited studies have assessed the accuracy of this method. Facial scanning and CBCT techniques can be used to transfer the maxillary cast into the virtual articulator, in which the most frequently selected references planes are the Frankfort horizontal, axis orbital, and true horizontal planes. Studies analyzing the accuracy of the maxillary cast transfer by using facial scanning and CBCT techniques are restricted. Lastly, optical jaw trackers can be selected for transferring the maxillary cast into the virtual articulator by using the axis orbital or true horizontal planes, yet the accuracy of these systems is unknown. Digital data acquisition technologies, including digital photographs, facial scanning methods, CBCTs, and optical jaw tracking systems, can be used to transfer the maxillary cast into the virtual articulator. Studies are needed to assess the accuracy of these digital data acquisition technologies for transferring the maxillary cast into the virtual articulator.

A 3D Evaluation of Condyle Position of Skeletal Class I and III Patients: A Cone-Beam Computed Tomography Technique

Purpose: The present study aims to assess the differences in the condyle position for two skeletal classes using Cone-Beam Computed Tomography (CBCT) reconstructions for both sides and genders.
 Materials and Methods: In this cross-sectional descriptive study, the CBCT images of 96 patients (20-60 years) were assessed. The participants were divided according to their Angle malocclusion classifications (Angle Classes I and III). The variables of the Anterior-Posterior position of the Condyle (APC), condylar angle in the axial plane (ACA), the Lateral Position of the Condyle in the axial plane (LPC), the Vertical Position of the Condyle (VPC), condylar angle in coronal dimension (CCA), and the difference of APC and VPC on both sides were measured. The measurements were analyzed using a one‑way ANOVA and Tukey’s post hoc test.
 Results: The variables of APC, LPC, ACA, VDC, and the difference of the APC on both sides in the two skeletal classes were similar. The VPC and CCA were greater in Class III than in Class I. All variables representing the 3D position of the condyle were similar in men and women, as well as on the right and left in both skeletal classes, I and III.
 Conclusion: Based on the 3D evaluation results of the condylar position, the skeletal classes III and I differed in the VPC and CCA; however, for the rest variables, there were no statistical differences.

Three-Dimensional Modeling and Quantitative Assessment of Mandibular Volume in Ectodermal Dysplasia: A Case Series.

Background and Objectives: Ectodermal dysplasia (ED)-a genetic disorder-is characterized by severe tooth deficiency. We compared the mandibular volume and the sagittal and horizontal mandibular widths between patients with ED (ED group) and individuals without tooth deficiency (control group) using three-dimensional modeling. We hypothesized that the mandibular volume differs in ED cases owing to congenital tooth deficiency. Materials and Methods: We used previously obtained cone-beam computed tomography (CBCT) images of 13 patients with ED. The control group data comprised retrospective CBCT images of patients of similar age and sex with a skeletal relationship of class 1. Further, using the three-dimensional image analysis software, the tooth crowns were separated from the mandible, the mandible was reconstructed and the gonion-to-gonion distance in the mandible was marked, the distance to the menton point was measured, and the distance between the two condyles was measured and compared with the control group. Results: Overall, 46.2% and 53.8% of the participants were men and women, respectively. In the ED group, the mean age of the participants was 15.46 (range, 6-24) years, and the mean number of mandibular teeth was 4.62. Notably, the edentulous mandible volume of the ED group (27.020 mm3) was statistically significantly smaller than that of the control group (49.213 mm3) (p < 0.001). There was no difference between the two groups in terms of the marked points. For data analysis, the Shapiro-Wilk test, independent samples t-test, and Mann-Whitney U test were used. Conclusions: It has been considered that mandible volume does not develop in ED cases because of missing teeth. Modern practices, such as the CBCT technique and three-dimensional software, may be effective in identifying the true morphologic features, especially in patients with genetic syndromes affecting the maxillofacial structure.

Pericervical Dentin Metrics in Mandibular First Molars Determined with Digital Periapical Radiography and Cone-beam Computed Tomography

IntroductionPericervical root dentin is decisive for the long-term mechanical integrity of root-filled teeth. Current treatment protocol does not include a customized step to determine the pretreatment residual pericervical root dentin. ObjectiveTo determine and compare the residual root dentin and canal width using digital periapical radiography (DPR) and cone-beam computed tomography (CBCT) at the apical limit of the pericervical area (PCA) in mandibular first molars. MethodsDPR and CBCT images of 60 patients with age between 22 and 76 years were used to determine (a) the mesiodistal widths of the root canal (pericervical dimensions [PCL]-C) and the root (PCL-R) of mandibular first molars at the apical limit of the PCA and (b) the intracanal distance from the apical limit of the PCA to the radiographic apex (intracanal distance [ICD]). The correlation between the PCL and ICD measurements obtained from DPR and CBCT were evaluated. ResultsValues between 0.10–0.80 mm and 0.00–1.10 mm were obtained for PCL-C using DPR and CBCT respectively (95% CI). The PCL values between 0.90–2.30 mm and 0.00–2.30 mm were obtained from DPR and CBCT respectively (95% CI). The ICD ranged between 4.6–12.3 mm in DPR and 4.40–12.0 mm in CBCT (95% CI). The comparative analysis showed differences from −0.9 to 0.5 mms for PCL and −2.00 to 1.5 mms for ICD between DPR and CBCT techniques respectively. ConclusionThe PCL and ICD determined from DPR and CBCT provided the pericervical dentin metrics that could be utilized clinically as a guideline for decision-making in endodontic treatment.

Evaluation of diagnostic accuracy of cone beam computed tomography and multi-detector computed tomography for detection of anatomical variations in rhinoplasty

BackgroundDifferent imaging techniques, such as multi-detector computed tomography (MDCT) scan and cone beam computed tomography(CBCT), are used to check the structure of the nose before rhinoplasty. This study aimed to evaluate the accuracy of two imaging techniques, MDCT scan, and CBCT, in diagnosing structural Variations in rhinoplasty for the first time.MethodsThis diagnostic accuracy study was conducted on 64 rhinoplasty candidates who complained of snoring and sleep apnea or had a positive result in the examination with Cottle’s maneuver or modified Cottle technique between February 2021 and October 2022 at 15- Khordad Hospital affiliated to Beheshti University of Medical Sciences. Before rhinoplasty, patients were randomly assigned to one of the CT and CBCT techniques with an equal ratio. Scans were assessed for the presence of Nasal septum deviation (NSD), Mucocele, Concha bullosa, and nasal septal spur by two independent radiologists. The findings of the two methods were evaluated with the results during rhinoplasty as the gold standard.ResultsNSD was the most common anatomical variation based on both imaging techniques. The accuracy of CBCT for diagnosing Nasal Septum Deviation and Mucocele was 80% and 75%, respectively. The sensitivity, specificity, and accuracy of CBCT in detecting Concha bullosa were 81.3% and 83.3%, respectively. The Kappa coefficient between CBCT and intraoperative findings for diagnosing NSD and Concha Bullosa was 0.76 and 0.73, respectively (p < 0.05).ConclusionCBCT can be considered a suitable method with high accuracy and quality to evaluate the anatomical variations before rhinoplasty.

Comparative Assessment of Three Different Alveolar Ridge Dimension Measurement Methods before Implant Placement: An In Vivo Study.

The purpose of this study was to compare the three various techniques for measuring the alveolar ridge's dimensions prior to implant insertion. For this study, a total of 36 participants were chosen. To prepare a surgical stent, a study model was created from an alginate impression. A first point (reference point) was marked on the crest of the ridge in relation to the adjacent teeth. Then, one point (point 1) and another point (point 2) were marked at distances of 3 and 6 mm, respectively, from the reference point. Based on the procedure for measuring the size of the alveolar ridge, the study was divided into the following groups. Group I: Cone-beam computed tomography (CBCT) measurement method; Group II: Ridge mapping measurement method; Group III: Direct caliper measurements method. Descriptive statistics were used to estimate the mean and standard deviation (SD). The Student's unpaired t-test was utilized for the statistical analysis. The 5% level of significance was used. There was no significant difference found between CBCT with ridge mapping and direct caliper measurements. However, on comparison of ridge mapping and direct caliper measurements technique, at point 1, the ridge mapping was 3.88 ± 0.12 and the direct caliper measurement was 3.62 ± 0.08. At point 2, the ridge mapping was 6.58 ± 0.06 and the direct caliper measurement was 6.32 ± 0.04. There was a statistically significant difference found between these two measurement methods. Within the limitation, the current study came to the conclusion that when CBCT and ridge mapping measurements were individually compared with the gold standard-the surgical open method, CBCT-demonstrated to be a highly specific and sensitive method for detecting the residual alveolar ridge width in the treatment planning of dental implants. Evaluation of alveolar bone is necessary during treatment planning for dental implant placement. Using simply panoramic and/or periapical radiographs to evaluate the bone may not be sufficient because it only provides two-dimensional information regarding the implant locations. Therefore, for better implant placement, three-dimensional information of the implant site, such as CBCT and ridge mapping technique, should be assessed.

Evaluation of Different Techniques of Working Length Determination in Comparison with CBCT.

To estimate the working length of teeth using conventional tactile technique, radiovisiography technique, Apex locator, and confirmation of working length by cone beam computed tomography technique. Forty patients with the age group between 20 and 50 years with irreversible pulpitis were included in this study. After taking a preoperative radiograph, the procedure was started. The cavity wall buildup was done using composite in the cases needed and then rubber dam application was done. The working length of mesiobuccal and mesiolingual canal was taken using cone beam computed tomography and this was considered as the standard value. Then using tactile method, length of the canal was measured using radiovisiographic software followed by apexlocator. The Apex locator has showed best result as compared to tactile and radiovisiography. Apex locator values were closest to cone beam computed tomography working length measurement values and electronic apexlocator can be used efficiently as an alternative method for working length determination. The quality of various techniques according to ranks of working length determination was found to be Cone beam computed tomography (CBCT) > Apexloc > Radiovisiography (RVG) > Tactile. Thus, Apex locator can be used as an alternative to cone beam computed tomography for working length determination.

Genotoxicity and cytotoxicity effects of x-rays on the oral mucosa epithelium at different fields of view: A cone beam computed tomography technique.

Cone beam computed tomography (CBCT) is considered a common examination for dentistry problems. Cellular biology can be affected by exposure to ionizing radiations procedures. In this study, we aimed to assess the genotoxicity and cytotoxicity effects of CBCT dental examinations at two different fields of view (FOVs) in exfoliated buccal epithelial cells. Sixty healthy adults participated in the current study. They were divided into two identical groups; CBCT with FOV of 6*6 cm2 and 8*11 cm2. Exfoliated oral mucosa cells were prepared immediately before and after 10-12 days of CBCT exposure. The cytological smears were stained with the Papanicolaou technique. The amounts of micronuclei and other cytotoxicity cellular changes (Pyknosis, Karyolysis, and Karyorrhexis) were evaluated. The variables of the parameters before and after CBCT examination in the two investigated FOVs were performed using Wilcoxon test and paired-samples t-test in SPSS software. The micronuclei and other cytotoxic changes parameters before and after CBCT exposure for both FOVs (6*6 and 8*11 cm2) increased significantly (p<0.001). Furthermore, a significant difference (p<0.05) was observed between the investigated parameters at the two FOVs. Notably, the FOV of 8*11 cm2 had more side effects than that of 6*6 cm2. There were no statistically significant among males and females for both FOVs. CBCT examinations of dental disorders would increase the risks of inducing genetic damage. The cytotoxicity and chromosomal damage were considered in males and females in both investigated FOVs (6*6 and 8*11 cm2). In this regard, the use of CBCT must be following the ALARA principle.

Comparative analysis of setup margin calculation in cone beam CT, by van Herk formula, using two different image registration methods

ABSTRACT Introduction: This study aimed to quantify the difference in setup margin in cone beam computed tomography (CBCT) setup imaging, utilising the van Herk formula for two different image registration methods. Two alternative techniques of registration, bony landmark (BL) matching and soft tissue matching (ST) for head and neck cancer patients, were investigated. Methods: This study included 30 head and neck cancer patients who received a simultaneous integrated boost of 54–60–66 Gy in 30 fractions, using volumetric modulated arc treatment. A total of 867 CBCT images were acquired during patient setup and further analysed for setup margin calculation. A region of interest was described using a clip box between the reference and CBCT image to calculate the patient’s positional inaccuracy in three translational directions, X, Y and Z, where X was mediolateral, Y was the cranial-caudal, and Z was the anterior-posterior direction in the patient-based coordinate system, respectively. The shifts were captured by altering the BL and ST matching, and the setup margin was calculated using the van Herk formula (=2·5Σ + 0·7σ where Σ was the systematic and σ was the random error). Results: The difference between bony and ST matching in most cases was observed to be 1·4 mm in all translational directions at a 95% confidence interval and &lt;1° in all rotational directions. The rotational error was found to be below the action level (±3°); hence, no corrections related to rotational error were made. The translational setup margin for bone and ST-based registration was X (BL) = 4·6 mm, X (ST) = 4·4 mm, Y (BL) = 6·3 mm, Y (ST) = 4·7 mm, Z (BL) = 3·0 mm, Z (ST) = 3·6mm. Conclusion: Two distinct registration approaches for head-neck patient setup did not yield any significant difference in the setup margin calculation. A suitable approach for CBCT and reference CT registration technique was required for the setup margin calculation. Confusion in selecting the correct image registration procedure can result in incorrect treatment execution. The compatibility of the two registration approaches was established in this study. Image fusion was neutralised before the second match (ST) to avoid hysteresis. For setup verification using CBCT for the head and neck region, both bone and ST registration were compatible for setup verification.

Comprehensive assessment of dentists knowledge and awareness about Cone Beam Computed Tomography technique used in dental implant therapies: An original research study

Aim: The aim of this study was to assess the existing knowledge and awareness about Cone Beam Computed Tomography among general dental practitioner in Bhopal District, India. Materials &amp; Methods: Total 250 private general dental practitioners of Bhopal were approached for the study. However, after considering various factors this number was reduced to 160 general dental practitioners. Questions of CBCT knowledge and awareness were prepared in a questionnaire style. It was an exclusive close ended questionnaire containing 8 questions. Questionnaire was emailed to the email addresses of the participating dentists. Informed consent was obtained from the respondents those were willingly ready for involvement. The study was conducted over a period of 1 month in which dentists are asked to fill and send back the questionnaire. P value less than 0.05 was considered significant (p&lt; 0.05). Statistical Analysis &amp; Results: Statistical analysis was completed by using statistical software Statistical Package for the Social Sciences version. Out of 160 practitioners, males were 109 and females were 51. Total 41 practitioners were belonging to age group &gt;55 years. Total 120 practitioners were practicing for less than 5 years. Nevertheless, 24 practitioners were practicing in the range of 5 to 10 years.

Assessment of lower third molar space: A comparative radiographic study.

The aim of this study was to assess lower third molar space using four different radiographic reconstructed Cone Beam Computed Tomography (CBCT) images: orthopantomogram, lateral cephalogram, multiplanar CBCT and a newly introduced three-dimensional (3D) simulation technique. The CBCT scans of 32 individuals (20.97 ± 2.152 years) with a total of 50 lower third molars were collected and analyzed. The ratio between the necessary space and available space for lower third molars was calculated on each radiographic reconstructed image. Repeated-measure analysis of variance followed by multiple comparison tests were used. The mean ratio was significantly smaller with cephalograms (0.611 ± 0.263), followed respectively by orthopantomograms (0.756 ± 0.221), multiplanar CBCT (0.789 ± 0.191) and 3D simulation technique (0.807 ± 0.193) (p < 0.001). The lowest mean difference was recorded between multiplanar CBCT and 3D simulation technique (0.017). Intraclass correlation coefficient was strong (>0.90) for all techniques except cephalograms. The assessment of the lower third molar space differs according to the adopted imaging technique. Three-dimensional simulation technique is a useful novel technology that allows an exploration of the crown in many different dimensions and orientations, giving more predictable results than the conventional methods.

ATM technique - a novel radiographic technique to assess the position of buccal shelf implants.

Success of the buccal shelf implants depends on its proper placement. Radiographic techniques like cone beam computed tomography (CBCT), posteroanterior cephalogram (PA cephalogram) have been used to evaluate the position of buccal shelf implant placement. However, these techniques have disadvantages like increased radiation exposure, metal artifact, less availability in dental offices and are more expensive.The aim of Arvind's Trans Mandibular (ATM) technique is to overcome this drawback by using an intraoral periapical radiograph (IOPA) film or radiovisiography (RVG) sensor to assess the post-operative placement of buccal shelf implants. Radiographs were taken for four patients after the placement of buccal shelf implants. The IOPA sensor was placed beneath the lower border of the mandible using this modified extraoral technique to assess the position of the buccal shelf implants in relation to the adjacent molars. The findings from the IOPA image were verified with CBCT images. The radiographic images taken using ATM technique were compared with the gold-standard CBCT technique. The ATM technique was able to accurately assess the postoperative buccolingual position of the implant with respect to the roots of molars in all the four cases. The proximity of the implant to the roots of the molar can be clearly visualized in all the cases. ATM is a novel radiographic technique using IOPA radiographic film or RVG sensor to assess the post-operative buccolingual placement of buccal shelf implants in relation to lower molar roots.

A novel cone-beam CT scanning technique for measuring periodontal soft tissues in the esthetic area.

This study investigated and tested a novel cone-beam computed tomography (CBCT) scanning technique capable of obtaining clear contours of soft tissues in the esthetic area. Twenty-three periodontally healthy participants underwent this novel CBCT scanning technique. Soft tissue morphological parameters were measured on the CBCT images obtained. Intraoral clinical data were also collected at the same locations, and the accuracy of the CBCT method was tested. The median (interquartile range [IQR]) of the supracrestal gingival tissue thickness as 0.91 (0.73-1.13) mm, and the thickness of the central incisors was significantly greater than that of the canines (P < 0.05). The median (IQR) of keratinized tissue thickness was 0.73 (0.55-0.91) mm, which also showed a significantly greater thickness in the central incisors than in the canines (P < 0.05). Bland-Altman analysis suggested that CBCT could be accurate for measuring soft tissues in the esthetic area. The novel CBCT technique described yields clear contours of soft tissues in the esthetic area without the need for auxiliary tools. Moreover, measurements of soft tissue morphological parameters on CBCT appear to be accurate.

Evaluation the Relationship between Mandibular Molar Root Apices and Mandibular Canal among Residents of the Moscow Population using Cone-Beam Computed Tomography Technique.

Background: The relationship between the inferior alveolar nerve and the root apices of the mandibular molars and premolars is of clinical importance. The aim of this study was to determine the relationship between the mandibular canal (MC) and the mandibular molar root apices using cone-beam computed tomography (CBCT) scanning among residents of the Moscow population.Materials and Methods: Three hundred CBCT scans for patients aged 20–70 years were analyzed. Patients were divided into three age groups: young group (20–44 years), middle-age group (45–59 years), and elderly group (60–70). The distance from the MC and the mandibular molar root apices was measured in each group in the coronal view of CBCT scans. Statistical analysis was set on P < 0.05.Results: The mean distance from the mesial root apices of the first, second, and third molars to the MC was 4.92, 2.85, and 2.24 mm, respectively. The distal root showed to be the closest root to the MC in multirooted teeth. The young age group showed smaller distances to MC than other age groups (P < 0.05). Females showed smaller distances to MC in mandibular molars than males (P < 0.05).Conclusions: The distance between the root apices and the MC has to be taken into consideration when performing surgical or endodontic procedures.

Estimating the Entrance Surface Dose in the Eyes, Thyroid, and Parotid Gland Regions in Adult and Pediatric Groups: A Cone-Beam Computed Tomography Technique

Purpose: This study aimed to determine the Entrance Surface Dose (ESD) of sensitive organs in Cone-Beam Computed Tomography (CBCT) imaging of the maxillofacial region in the two age groups of adult and pediatric.&#x0D; Materials and Methods: In this work, the measurements were performed using Thermo Luminescent Dosimeters (TLD-GR200). The imaging was performed using a PROMAX 3D CBCT scanner for 30 adults and 20 pediatric patients. The ESD value for each patient in the region of eyes, thyroid, and parotid glands was measured by 15 TLDs during CBCT of maxillofacial.&#x0D; Results: The highest and lowest mean values of ESDs were related to the parotid and thyroid gland regions in adults, 4.77 ± 0.61 mGy and 0.37 ± 0.16 mGy, respectively. In addition, these values were obtained 2.97 ± 0.36 mGy and 0.35 ± 0.12 mGy in pediatric groups as the highest and lowest values in that order. The results showed that the ESD values of the parotid gland regions in maxilla and mandible examinations had a significant difference (P &lt;0.05). In addition, there was a significant difference between the ESD values of the parotid gland regions among the adults and pediatric groups (P &lt;0.05).&#x0D; Conclusion: According to the results, the ESD values in both age groups were higher in the parotid gland region during maxillofacial CBCT examinations. Therefore, it is recommended to set radiation parameters like mAs as low as possible for reducing the patient dose, especially pediatric patients due to the more sensitive organs.

Accuracy of Working Length Determination In Primary Molar Teeth Using Three Different Methods: An Invitro Study

Background: An accurate working length plays a pivotal role for the success of treatment in endodontics. It involves a wide array of procedures. Aim: Use of digital radiovisiography (RVG), apex locator (EAL) and cone beam computed tomography technique (CBCT) to evaluate working length in primary teeth. Materials and Methods: This study included thirty extracted primary teeth having at least 2/3rd of root. Alginate was combined with 0.9% normal saline solution and teeth were inserted in it after access opening. Two qualified examiners recorded working length for all canals twice with RVG, EAL and CBCT. In control group, a Kerr- file from coronal reference to the apical foramen was used to visually determine the actual (AL) under stereomicroscope. Statistical analysis: Intraclass correlation coefficient (ICC) and Bland and Altman analysis were used to calculate the agreement on measurements of all methods using actual values of different methods. Results: Electronic working length measured with AL (0.0007) was statistically significant compared to RVG (1.9570) and CBCT (0.8440). Amongst all methods for determination of working length the ICC showed the highest reliability for EAL and least for RVG. Conclusion: In comparison to RVG and CBCT, the EALperformed the best for determining working length in primary teeth.

Validation and application of a novel in vivo cervical spine kinematics analysis technique

To validate the accuracy of Cone beam computed tomography (CBCT) cervical spine modeling with three dimensional (3D)-3D registration for in vivo measurements of cervical spine kinematics. CBCT model accuracy was validated by superimposition with computed tomography (CT) models in 10 healthy young adults, and then cervical vertebrae were registered in six end positions of functional movements, versus a neutral position, in 5 healthy young adults. Registration errors and six degrees of freedom (6-DOF) kinematics were calculated and reported. Relative to CT models, mean deviations of the CBCT models were < 0.6 mm. Mean registration errors between end positions and the reference neutral position were < 0.7 mm. During flexion–extension (F–E), the translation in the three directions was small, mostly < 1 mm, with coupled LB and AR both < 1°. During lateral bending (LB), the bending was distributed roughly evenly, with coupled axial rotation (AR) opposite to the LB at C1–C2, and minimal coupled F–E. During AR, most of the rotation occurred in the C1–C2 segment (29.93 ± 7.19° in left twist and 31.38 ± 8.49° in right twist) and coupled LB was observed in the direction opposite to that of the AR. Model matching demonstrated submillimeter accuracy in cervical spine kinematics data. The presently evaluated low-radiation-dose CBCT technique can be used to measure 3D spine kinematics in vivo across functional F–E, AR, and LB positions, which has been especially challenging for the upper cervical spine.
