Effects of muscle quantity and bone mineral density on injury and outcomes in older adult motor vehicle crash occupants

Objectives: Quantify the independent and combined effects of abdominal muscle quantity and lumbar bone mineral density (BMD) on injury risk and in-hospital outcomes in severely injured motor vehicle crash (MVC) occupants ages 50 and older. Methods: Skeletal muscle area measurements of MVC occupants were obtained through semi-automated segmentation of an axial computed tomography (CT) slice at the L3 vertebra. An occupant height-normalized Skeletal Muscle Index (SMI) was calculated - a defining metric of sarcopenia and low muscle mass (sarcopenia thresholds: <38.5 cm2/m2 females; <52.4 cm2/m2 males). Lumbar BMD was obtained using a validated, phantomless CT calibration method (osteopenia threshold: <145 mg/cm3). SMI and BMD values were used to categorize occupants, and logistic regression was used to associate sarcopenia, osteopenia, and osteosarcopenia predictors to injury outcomes (e.g., Injury Severity Score (ISS), maximum Abbreviated Injury Scale (MAIS) score, fractures) and hospital outcomes (e.g., length of stay, ICU days). Results: Of the 336 occupants, 210 (63%) were female (mean ± SD: age 66.3 ± 10.6). SMI was 41.7 ± 8.0 cm2/m2 in females and 51.2 ± 10.8 cm2/m2 in males. Based on SMI, 40% of females and 55% of males were classified as sarcopenic. BMD was 163.2 ± 38.3 mg/cm3 in females and 164.1 ± 35.4 mg/cm3 in males, with 41% of females and 33% of males classified as osteopenic. Prevalence of both conditions (osteosarcopenia) was similar between females (21%) and males (22%). Incidence of low SMI and BMD increased with age. Sarcopenic individuals were less likely to sustain a MAIS 2+ thorax injury and had longer ICU stays. Osteopenic individuals were more likely to sustain upper extremity injuries and fractures, and were less likely to be discharged to a rehabilitation facility. Osteosarcopenic individuals were less likely to be ventilated or admitted to the ICU but tended to spend more time on the ventilator if placed on one. Conclusions: Osteosarcopenia was not associated with any injury outcomes, but sarcopenia was associated with thoracic injury and osteopenia was associated with upper extremity injury incidence. Sarcopenia was only associated with ICU length of stay, while osteopenia was only associated with discharge destination. Osteosarcopenia was associated with likelihood of being ventilated, being admitted to the ICU, and with increased length of ventilation.

Technical note: Evaluation of artificial 120-kilovolt computed tomography images for radiation therapy applications.

The purpose of this work is to evaluate the scaled computed tomography (CT) number accuracy of an artificial 120kV reconstruction technique based on phantom experiments in the context of radiation therapy planning. An abdomen-shaped electron density phantom was scanned on a clinical CT scanner capable of artificial 120kV reconstruction using different tube potentials from 70 to 150kV. A series of tissue-equivalent phantom inserts (lung, adipose, breast, solid water, liver, inner bone, 30%/50% CaCO3 , cortical bone) were placed inside the phantom. Images were reconstructed using a conventional quantitative reconstruction kernel as well as the artificial 120kV reconstruction kernel. Scaled CT numbers of inserts were measured from images acquired at different kVs and compared with those acquired at 120kV, which were deemed as the ground truth. The relative error was quantified as the percentage deviation of scaled CT numbers acquired at different tube potentials from their ground truth values acquired at 120kV. Scaled CT numbers measured from images reconstructed using the conventional reconstruction demonstrated a strong kV-dependence. The relative error in scaled CT numbers ranged from 0.6% (liver insert) to 31.1% (cortical bone insert). The artificial 120kV reconstruction reduced the kV dependence, especially for bone tissues. The relative error in scaled CT number was reduced to 0.4% (liver insert) and 2.6% (30% CaCO3 insert) using this technique. When tube potential selection was limited to the range of 90 to 150kV, the relative error was further restrained to<1.2% for all tissue types. Phantom results demonstrated that using the artificial 120kV technique, it was feasible to acquire raw projection data at the desired tube potential and then reconstruct images with scaled CT numbers comparable to those obtained directly at 120kV. In radiotherapy applications, this technique may allow optimization of tube potential without complicating clinical workflow by eliminating the necessity of maintaining multiple sets of CT calibration curves.

CAD-based x-ray CT calibration and error compensation

Artifacts due to imperfect determination of the scanner geometry, beam hardening (BH), and diffuse Compton scattering, limit the quantitative exploitation of radiographs or tomographies for non-destructive evaluation. Exploiting the computer-aided design (CAD) model of an industrial part, a methodology is proposed to refine the estimation of the computed tomography (CT)-scanner geometry up to a scale factor, to correct or account for artifacts, and to assess the metrology of the part. A projective model describing the formation of x-ray images in CT-scanners is first introduced. The optimal parameters of the projective model are identified using a novel CAD-based calibration method that relies on the registration of simulated projections onto experimental ones. A metrological analysis based on the comparison between acquired and simulated x-ray images is proposed. A turbine blade, for which an automatic inspection procedure from few views is under development, is used as an example to illustrate the proposed methodology. The parametrization accounts for the refinement of the projection geometry, the calibration of BH, and the estimation of scattering. It is shown that, using the proposed procedure, the differences between acquired and simulated radiographic images are significantly reduced, indicating that the optimal parameters are properly identified. These differences are then exploited to detect flaws in the part.

Remote dose imaging from Cherenkov light using spatially resolved CT calibration in breast radiotherapy.

Imaging Cherenkov light during radiotherapy allows the visualization and recording of frame-by-frame relative maps of the dose being delivered to the tissue at each control point used throughout treatment, providing one of the most complete real-time means of treatment quality assurance. In non-turbid media, the intensity of Cherenkov light is linear with surface dose deposited, however the emission from patient tissue is well-known to be reduced by absorbing tissue components such as hemoglobin, fat, water, and melanin, and diffused by the scattering components of tissue. Earlier studies have shown that bulk correction could be achieved by using the patient planning computed tomography (CT) scan for attenuation correction. In this study, CT maps were used for correction of spatial variations in emissivity. Testing was completed on Cherenkov images from radiotherapy treatments of post-lumpectomy breast cancer patients (n=13), combined with spatial renderings of the patient radiodensity (CT number) from their planning CT scan. The correction technique was shown to provide a pixel-by-pixel correction that suppressed many of the inter- and intra-patient differences in the Cherenkov light emitted per unit dose. This correction was established from a calibration curve that correlated Cherenkov light intensity to surface-rendered CT number ( and ). The corrected Cherenkov intensity per unit dose standard error was reduced by nearly half (from ∼30% to ∼17%). This approach provides evidence that the planning CT scan can mitigate some of the tissue-specific attenuation in Cherenkov images, allowing them to be translated into near surface dose images.

Accuracy of proton stopping power estimation of silicone breast implants with single and dual-energy CT calibration techniques.

A major contributing factor to proton range uncertainty is the conversion of computed tomography (CT) Hounsfield Units (HU) to proton relative stopping power (RSP). This uncertainty is elevated with implanted devices, such as silicone breast implants when computed with single energy CT (SECT). In recent years, manufacturers have introduced implants with variations in gel cohesivity. Deriving the RSP for these implants from dual‐energy CT (DECT) can result in a marked reduction of the error associated with SECT. In this study, we investigate the validity of DECT calibration of HU to RSP on silicone breast implants of varying cohesivity levels. A DECT capable scanner was calibrated using the stoichiometric method of Bourque et al for SECT and DECT using a tissue substitute phantom. Three silicone breast implants of increasing gel cohesivity were measured in a proton beam of clinical energy to determine ground‐truth RSP and water equivalent thickness (WET). These were compared to SECT‐derived RSP at three CT spectrum energies and DECT with two energy pairs (80/140 kVp and 100/140 kVp) as obtained from scans with and without an anthropomorphic phantom. The RSP derived from parameters estimates from CT vendor‐specific software (syngo.via) was compared. The WET estimates from SECT deviated from MLIC ground truth approximately +11%–19%, which would result in overpenetration if used clinically. Both the Bourque calibration and syngo.via WET estimates from DECT yielded error ≤0.5% from ground truth; no significant difference was found between models of varying gel cohesivity levels. WET estimates without the anthropomorphic phantom were significantly different than ground truth for the Bourque calibration. From these results, gel cohesivity had no effect on proton RSP. User‐generated DECT calibration can yield comparably accurate RSP estimates for silicone breast implants to vendor software methods. However, care must be taken to account for beam hardening effects.

Technical Note: CT calibration for proton treatment planning by cross-calibration with proton CT data.

This study explores the possibility of a new method for x-ray computed tomography (CT) calibration by means of cross-calibration with proton CT (pCT) data. The proposed method aims at a more accurate conversion of CT Hounsfield Units (HU) into proton stopping power ratio (SPR) relative to water to be used in proton-therapy treatment planning. X-ray CT scan was acquired on a synthetic anthropomorphic phantom, composed of different tissue equivalent materials (TEMs). A pCT apparatus was instead adopted to obtain a reference three-dimensional distribution of the phantom's SPR values. After rigid registration, the x-ray CT was artificially blurred to the same resolution of pCT. Then a scatter plot showing voxel-by-voxel SPR values as a function of HU was employed to link the two measurements and thus obtaining a cross-calibrated x-ray CT calibration curve. The cross-calibration was tested at treatment planning system and then compared with a conventional calibration based on exactly the same TEMs constituting the anthropomorphic phantom. Cross-calibration provided an accurate SPR mapping, better than by conventional TEMs calibration. The dose distribution of single beams optimized on the reference SPR map was recomputed on cross-calibrated CT, showing, with respect to conventional calibration, minor deviation at the dose fall-off (lower than 1%). The presented data demonstrated that, by means of reference pCT data, a heterogeneous phantom can be used for CT calibration, paving the way to the use of biological samples, with their accurate description of patients' tissues. This overcomes the limitations of conventional CT calibration requiring homogenous samples, only available by synthetic TEMs, which fail in accurately mimicking the properties of biological tissues. Once a heterogeneous biological sample is provided with its corresponding reference SPR maps, a cross-calibration procedure could be adopted by other PT centers, even when not equipped with a pCT system.

Patient-specific CT calibration based on ion radiography for different detector configurations in 1H, 4He and 12C ion pencil beam scanning

The empirical conversion of the treatment planning x-ray computed tomography (CT) image to ion stopping power relative to water causes dose calculation inaccuracies in ion beam therapy. A patient-specific calibration of the CT image is enabled by the combination of an ion radiography (iRad) with the forward-projection of the empirically converted CT image along the estimated ion trajectories. This work investigated the patient-specific CT calibration for list-mode and integration-mode detector configurations, with reference to a ground truth ion CT (iCT) image. Analytical simulations of idealized carbon ion and proton trajectories in a numerical anthropomorphic phantom and realistic Monte Carlo simulations of proton, helium and carbon ion pencil beam scanning in a clinical CT image of a head-and-neck patient were considered. Controlled inaccuracy and noise levels were applied to the calibration curve and to the iRad, respectively. The impact of the selection of slices and angles of the iRads, as well as the choice of the optimization algorithm, were investigated. Accurate and robust CT calibration was obtained in analytical simulations of straight carbon ion trajectories. Analytical simulations of non-straight proton trajectories due to scattering suggested limitations for integration-mode but not for list-mode. To make the most of integration-mode, a dedicated objective function was proposed, demonstrating the desired accuracy for sufficiently high proton statistics in analytical simulations. In clinical data the inconsistencies between the iRad and the forward-projection of the ground truth iCT image were in the same order of magnitude as the applied inaccuracies (up to 5%). The accuracy of the CT calibration were within 2%–5% for integration-mode and 1%–3% for list-mode. The feasibility of successful patient-specific CT calibration depends on detector technologies and is primarily limited by these above mentioned inconsistencies that slightly penalize protons over helium and carbon ions due to larger scattering and beam spot size.

Radiotherapy in oncological emergencies: fast-track treatment planning

Background and purposeTo report on our clinical experience with a newly implemented workflow for radiotherapy (RT) emergency treatments, which allows for a fast treatment application outside the regular working-hours, and its clinical applicability.MethodsTreatment planning of 18 emergency RT patients was carried out using diagnostic computed tomography (CT) without a dedicated RT simulation CT. The cone-beam CT (CBCT) deviations of the first RT treatment were analyzed regarding setup accuracy. Furthermore, feasibility of the “fast-track” workflow was evaluated with respect to dose deviations caused by different Hounsfield unit (HU) to relative electron density (rED) calibrations and RT treatment couch surface shapes via 3D gamma index analysis of exemplary treatment plans. The dosimetric uncertainty introduced by different CT calibrations was quantified.ResultsMean patient setup vs. CBCT isocenter deviations were (0.49 ± 0.44) cm (x), (2.68 ± 1.63) cm (y) and (1.80 ± 1.06) cm (z) for lateral, longitudinal and vertical directions, respectively. Three out of four dose comparisons between the emergency RT plan calculated on the diagnostic CT and the same plan calculated on the treatment planning CT showed clinically acceptable gamma passing rates, when correcting for surface artifacts. The maximum difference of rED was 0.054, while most parts of the CT calibration curves coincided well.ConclusionIn an emergency RT setting, the use of diagnostic CT data for treatment planning might be time-saving and was shown to be suitable for many cases, considering reproducibility of patient setup, accuracy of initial patient setup and accuracy of dose-calculation.

Dosimetric verification of clinical radiotherapy treatment planning system

Background/Aim. In the past two decades, we have witnessed the emergence of new radiation therapy techniques, radiotherapy treatment planning system (TPS) with calculating algorithms for the dosage calculation in a patient, units for multislice computed tomography (CT) and image-guided treatment delivery. The aim of the study was investigating the significant difference in dosimetric calculation of radiotherapy TPS in relation to the values obtained by measuring on the linear accelerator (LINAC), and the accuracy of dosimetric calculation between calculating algorithms Analytical Anisotropic Algorithm (AAA) and Acuros XB in various tissues and photon beam energies. Methods. For End-to-End test we used the hetero-geneous phantom CIRS Thorax002LFC, which anatomically represents human torso with a set of inserts known as relative electron densities (RED) for obtaining a CT calibration curve, comparable to the ?reference? CIRS 062M phantom. For the AAA and Acuros XB algorithms and for 6 MV and 16 MV photon beams in the TPS Varian Eclipse 13.6, four 3D conformal (3DCRT), and one intensity modulated (IMRT) and volumetric modulated arc (VMAT) radiotherapy plans were made. Measurements of the absolute dose in Thorax phantom, by PTW-Semiflex ionization chamber, were carried out on three Varian-DHX LINACs. Results. The difference between ?reference? and measured CT conversion curves in the bone area was 3%. For 476 phantom measurements, the difference between measured and TPS calculated dose of 3?6%, was found in 30 (6.3%) cases. According to regression analysis, the standardized Beta coefficient for relative errors, 6 MV vs. 16 MV, was 0.337 (33.7%, p &lt; 0.001). Mean relative errors for AAA and Acuros XB, using Mann-Whitney test, for bones were 1.56% and 2.64%, respectively (p = 0.004). Conclusion. End-to-End test on Thorax002LFC phantom proved the accuracy of TPS dose calculation in relation to the one delivered to a patient by LINAC. There was a significant difference for photon energies relative errors (higher values are obtained for 16 MV vs. 6 MV). A statistically significant minor relative error in AAA vs. Acuros XB was found for the bone.

Dynamic lung phantom commissioning for 4D dose assessment in proton therapy

Anthropomorphic phantoms mimicking organ and tumor motion of patients are essential for end-to-end testing of motion mitigation techniques in ion beam therapy. In this work a commissioning procedure developed with the in-house designed respiratory phantom ARDOS (Advanced Radiation DOSimetry system) is presented. The phantom was tested and benchmarked for 4D dose verification in proton therapy, which included: characterization of the tissue equivalent materials from computed tomography (CT) imaging, assessment of dose calculation accuracy in critical structures of the phantom, and testing various detectors for proton dosimetry in the ARDOS phantom.To prove the validity of the CT calibration curve, measured relative stopping powers (RSP) of the ARDOS materials were compared with values from CTs: original and overwritten with known material parameters. Override of rib- and soft-tissue phantom components improved RSP accuracy while inhomogeneous lung tissue, represented by the balsa wood, was better modelled by the CT Hounsfield units.Monte Carlo (MC) dose calculations were benchmarked against measurements with a reference Farmer chamber embedded in ARDOS material samples showing less than 3% relative dose difference. Differences between MC calculated dose distributions and those calculated by analytical algorithms for the ARDOS geometry were higher than 20% of the prescribed dose, depending on the position in the phantom.Pinpoint ionization chambers and thermoluminescence dosimeters showed differences of up to 5.5% compared to MC dose calculations for all lung setups in the static phantom. They were also able to detect dose distortions due to motion. EBT3 film dosimetry was shown to be suitable for 2D relative dose characterization, which could provide extended information on dose distributions in the penumbra area.The presented methodology and results can be used for drafting general recommendations for dynamic phantom commissioning, which is an essential step towards end-to-end evaluation of motion mitigation techniques in ion beam therapy.

Acceptance Test and Clinical Commissioning of CT Simulator

This study examined the clinical use of two newly installed computed tomography (CT) simulators in the Department of Radiation Oncology. The accreditation procedure was performed by the Korean Institute for Accreditation of Medical Imaging. An Xi R/F dosimeter was used to measure the CT dose index for each plug of the CT dose index phantom. Image qualities such as the Hounsfield unit (HU) value of water, noise level, homogeneity, existence of artifacts, spatial resolution, contrast, and slice thickness were evaluated by scanning a CT performance phantom. All test items were evaluated as to whether they were within the required tolerance level. CT calibration curves—the relationship between CT number and relative electron density—were obtained for dose calculations in the treatment planning system. The positional accuracy of the lasers was also evaluated. The volume CT dose indices for the head phantom were 22.26 mGy and 23.70 mGy, and those for body phantom were 12.30 mGy and 12.99 mGy for the first and second CT simulators, respectively. HU accuracy, noise, and homogeneity for the first CT simulator were −0.2 HU, 4.9 HU, and 0.69 HU, respectively, while those for second CT simulator were 1.9 HU, 4.9 HU, and 0.70 HU, respectively. Five air-filled holes with a diameter of 1.00 mm were used for assessment of spatial resolution and a low contrast object with a diameter of 6.4 mm was clearly discernible by both CT scanners. Both CT simulators exhibited comparable performance and are acceptable for clinical use.

Patient Dose Audit in Computed Tomography at Cancer Institute of Guyana

Objective: This study is the first investigation on computed tomography (CT) unit in Guyana at the Cancer Institute of Guyana (CIG), aimed at performing an audit on the radiation dose estimated by the GE LightSpeed QXi CT unit for common computed tomography examinations. Method: A RaySafe X2 CT calibration detector were used to obtained measurements for common CT examinations (head, neck, chest, abdomen, pelvis, upper extremity and lower extremity) done in free air as the control (36 data) and also with patients (35 data). Patient’s measurement was limited to the head, chest, abdomen and pelvis CT examinations. Exposure/electro-technical parameters and dose metrics (CTDIvol, DLP) were recorded and the k-coefficient conversion factor established by National Radiology Protection Board (NRPB) was used to calculate the effective dose associated with each examination. Results: The results indicated that the CT unit overestimated the dose for patient measurements and underestimated the dose for measurements taken In-Air with the exception of the head protocol which showed overestimation with the patient measurements. Both over-and underestimation were documented for the neck protocol. Comparison of estimated dose between published data shows that there are variations in techniques and radiation dose across institution for similar examinations and that the protocols reported by CIG documented an overall higher effective dose. Conclusion: The variation in estimated effective dose reported by CIG, is as a consequence of differences in CT scanner design and examination protocols.

Revisiting the single-energy CT calibration for proton therapy treatment planning: a critical look at the stoichiometric method

Despite extensive research in dual-energy computed tomography (CT), single-energy CT (SECT) is still the standard imaging modality in proton therapy treatment planning and, in this context, the stoichiometric calibration method is considered to be the most accurate to establish a relationship between CT numbers and proton stopping power. This work revisits the SECT calibration for proton therapy treatment planning, with special emphasis on the stoichiometric method. Three different sets of tissue-substitutes of known elemental composition (Gammex, CIRS and Catphan) were scanned with the same scanning protocol. A stoichiometric fit was performed for each set of tissue-substitutes. Based on that, the CT number, relative electron density and relative proton stopping power were calculated for ICRU 46 biological tissues and the different sets of tissue-substitutes. Despite common belief, it was found that the stoichiometric fit depends on the elemental composition of the tissue-substitutes used in the calibration, leading to differences in relative stopping power up to 3.5% for cortical bone. In addition, according to Rutherford et al (1976 Neuroradiology 11 15–21) parametrization of the atomic cross-section, CT numbers of Gammex tissue-substitutes and ICRU 46 biological tissues were found to be similar within the whole energy range relevant to computed tomography. Consequently, it was found that, for Gammex tissue-substitutes, the CT calibration curve resulting from the stoichiometric method agrees with that obtained by simple interpolation of experimental data. In conclusion, the stoichiometric method for SECT calibration seems to depend on the tissue-substitutes used for calibration—which could be regarded as an additional source of uncertainty in proton range for bone tissues. Furthermore, Gammex tissue-substitutes appear to be a good representative of biological tissues within the energy range relevant to computed tomography—making the stoichiometric method unnecessary.

Evaluation of Computed Tomography Scanners for Feasibility of Using Averaged Hounsfield Unit-to-Stopping Power Ratio Calibration Curve.

Purpose:The purpose of this study was to quantify the variability of stoichiometric calibration curves for different computed tomography (CT) scanners and determine whether an averaged Hounsfield unit (HU)–to–stopping power ratio (SPR) calibration curve can be used across multiple CT scanners.Materials and Methods:Five CT scanners were used to scan an electron density phantom to establish HU values of known material plugs. A stoichiometric calibration curve was calculated for CT scanners and for the average curve. Animal tissue surrogates were used to compare the water-equivalent thickness (WET) of the animal tissue surrogates calculated by the treatment planning system (TPS) and the WET values measured with a multilayered ionization chamber. The calibration curves were optimized to reduce the percentage of difference between measured and TPS-calculated WET values. A second set of tissue surrogates was then used to evaluate the overall range of uncertainty for the optimized CT-specific and average calibration curves.Results:Overall, the average variation in HU for all 6 calibration curves before optimization was 8.3 HU. For both the averaged and CT-specific calibrations, the root mean square error (RMSE) of the percentage of difference between TPS-calculated and measured WET values before optimization was 4%. The RMSE of the percentage of difference for the TPS-calculated and multilayered ionization chamber measured WET values after the optimization for both averaged and CT-specific calibration curves was reduced to less than 1.5%. The overall RMSE of the TPS and the measured WET percentage of difference after optimization was 2.1% for both averaged and CT-specific calibration curves.Conclusion:Averaged CT calibration curves can be used to map the HU-to-SPR in TPSs, if the variations in HU values across all scanners is relatively small. Performing tissue surrogate optimization of the HU-to-SPR calibration curve has been shown to reduce the overall uncertainty of the calibration for averaged and CT-specific calibration curves and is recommended, especially if an averaged HU-to-SPR calibration curve is used.

13. Dual Energy Computed Tomography for Perfusion Imaging

The metastatic spreading ability of cancer heavily depends on microcirculation, requiring the formation of new blood vessels in the vascular network, or angiogenesis. Indirect measurement of angiogenesis can be performed by computed tomography (CT) perfusion imaging, where blood flow is observed using dynamic contrast-enhanced techniques. However, contrast-enhancement in tissues can be poor due to inadequate CT calibration, leading to an increase in radiation dose to the patient. It is hypothesized that the use of dual-energy CT will provide a more robust basis for calibration of contrast-enhanced imaging without increasing dose. The purpose of this study is to improve the sensitivity and accuracy of perfusion imaging by using atomic number extraction of Iodine contrast agent concentration from dual-energy datasets. A stoichiometric calibration for CT was done with the effective atomic number and relative electron density extracted in MATLAB, resulting in a wider dynamic range of effective atomic number per mgI/mL. Dual-energy CT as a tool for calibrating perfusion imaging shows promise of increasing the range and signal to noise ratio of contrast-enhanced CT imaging. Future work will extend the study from a static to dynamic quantification to explore dual-energy CT for perfusion imaging, with the goals of developing a robust stoichiometric calibration for quality assurance purposes, and further insights on tumour angiogenesis and cancer metastasis.

Experimental Approach for the Uncertainty Assessment of 3D Complex Geometry Dimensional Measurements Using Computed Tomography at the mm and Sub-mm Scales.

The dimensional verification of miniaturized components with 3D complex geometries is particularly challenging. Computed Tomography (CT) can represent a suitable alternative solution to micro metrology tools based on optical and tactile techniques. However, the establishment of CT systems’ traceability when measuring 3D complex geometries is still an open issue. In this work, an alternative method for the measurement uncertainty assessment of 3D complex geometries by using CT is presented. The method is based on the micro-CT system Maximum Permissible Error (MPE) estimation, determined experimentally by using several calibrated reference artefacts. The main advantage of the presented method is that a previous calibration of the component by a more accurate Coordinate Measuring System (CMS) is not needed. In fact, such CMS would still hold all the typical limitations of optical and tactile techniques, particularly when measuring miniaturized components with complex 3D geometries and their inability to measure inner parts. To validate the presented method, the most accepted standard currently available for CT sensors, the Verein Deutscher Ingenieure/Verband Deutscher Elektrotechniker (VDI/VDE) guideline 2630-2.1 is applied. Considering the high number of influence factors in CT and their impact on the measuring result, two different techniques for surface extraction are also considered to obtain a realistic determination of the influence of data processing on uncertainty. The uncertainty assessment of a workpiece used for micro mechanical material testing is firstly used to confirm the method, due to its feasible calibration by an optical CMS. Secondly, the measurement of a miniaturized dental file with 3D complex geometry is carried out. The estimated uncertainties are eventually compared with the component’s calibration and the micro manufacturing tolerances to demonstrate the suitability of the presented CT calibration procedure. The 2U/T ratios resulting from the validation workpiece are, respectively, 0.27 (VDI) and 0.35 (MPE), by assuring tolerances in the range of ± 20–30 µm. For the dental file, the EN < 1 value analysis is favorable in the majority of the cases (70.4%) and 2U/T is equal to 0.31 for sub-mm measurands (L < 1 mm and tolerance intervals of ± 40–80 µm).

Multicenter analysis of CIREN occupant lumbar bone mineral density and correlation with age and fracture incidence

ABSTRACTObjective: This study aimed to quantify lumbar volumetric bone mineral density (vBMD) for 873 seriously injured Crash Injury Research and Engineering Network (CIREN) motor vehicle crash occupants (372 male, 501 female) from 8 centers using phantomless computed tomography scans and to associate vBMD with age, fracture incidence, and osteopenia/osteoporosis diagnoses. The novelty of this work is that it associates vBMD with region of injury by applying an established method for vBMD measurement using phantomless computed tomography (CT).Methods: A validated phantomless CT calibration method that uses patient-specific fat and muscle measurements to calibrate vBMD measured from the L1–L5 trabeculae was applied on 873 occupants from various CIREN centers. CT-measured lumbar vBMD < 145 mg/cc is indicative of osteopenia using a published threshold. CIREN occupant lumbar vBMD in milligrams per cubic centimeter was regressed against age, osteopenia/osteoporosis comorbidities, height, weight, body mass index (BMI), and the incidence of fracture in vertebral (cervical, thoracic, lumbar) and rib/sternum regions.Results: Among the 873 occupants analyzed, 11% (92 occupants) were diagnosed as osteopenic in CIREN. Of these 92 occupants, 42% (39 occupants) had normal vBMD measures (≥145 mg/cc), suggesting possible misclassification in CIREN. Of the 134 occupants classified as osteopenic in vBMD analysis, 60% were not classified as osteopenic in CIREN, suggesting undiagnosed osteopenia, and 40% were correctly classified in CIREN. Age was negatively correlated with vBMD (P <.0001) and occupants with <145 mg/cc vBMD sustained a median number of 2 rib/sternum fractures compared to a median value of 0 rib/sternum fractures for the ≥145 mg/cc vBMD group (P <.0001). Vertebral fracture analysis revealed that the thoracolumbar region was the most common region of injury in the spine. Though the incidence of fracture was not significantly different in the thoracic (10% versus 6%, P =.122) and lumbar (16% versus 13%, P =.227) regions between the 2 bone quality groups, the proportion of thoracolumbar fractures was significantly higher in occupants with <145 mg/cc vBMD versus occupants with ≥145 mg/cc vBMD (24% versus 17%, P =.043).Conclusions: Low lumbar vertebral bone quality is associated with an increased number of rib/sternum fractures and a greater incidence of thoracolumbar vertebral body fractures within the CIREN population analyzed.

A simulation study on proton computed tomography (CT) stopping power accuracy using dual energy CT scans as benchmark

ABSTRACTBackground. Accurate stopping power estimation is crucial for treatment planning in proton therapy, and the uncertainties in stopping power are currently the largest contributor to the employed dose margins. Dual energy x-ray computed tomography (CT) (clinically available) and proton CT (in development) have both been proposed as methods for obtaining patient stopping power maps. The purpose of this work was to assess the accuracy of proton CT using dual energy CT scans of phantoms to establish reference accuracy levels.Material and methods. A CT calibration phantom and an abdomen cross section phantom containing inserts were scanned with dual energy and single energy CT with a state-of-the-art dual energy CT scanner. Proton CT scans were simulated using Monte Carlo methods. The simulations followed the setup used in current prototype proton CT scanners and included realistic modeling of detectors and the corresponding noise characteristics. Stopping power maps were calculated for all three scans, and compared with the ground truth stopping power from the phantoms.Results. Proton CT gave slightly better stopping power estimates than the dual energy CT method, with root mean square errors of 0.2% and 0.5% (for each phantom) compared to 0.5% and 0.9%. Single energy CT root mean square errors were 2.7% and 1.6%. Maximal errors for proton, dual energy and single energy CT were 0.51%, 1.7% and 7.4%, respectively.Conclusion. Better stopping power estimates could significantly reduce the range errors in proton therapy, but requires a large improvement in current methods which may be achievable with proton CT.

A stoichiometric calibration method for dual energy computed tomography

The accuracy of radiotherapy dose calculation relies crucially on patient composition data. The computed tomography (CT) calibration methods based on the stoichiometric calibration of Schneider et al (1996 Phys. Med. Biol. 41 111–24) are the most reliable to determine electron density (ED) with commercial single energy CT scanners. Along with the recent developments in dual energy CT (DECT) commercial scanners, several methods were published to determine ED and the effective atomic number (EAN) for polyenergetic beams without the need for CT calibration curves. This paper intends to show that with a rigorous definition of the EAN, the stoichiometric calibration method can be successfully adapted to DECT with significant accuracy improvements with respect to the literature without the need for spectrum measurements or empirical beam hardening corrections. Using a theoretical framework of ICRP human tissue compositions and the XCOM photon cross sections database, the revised stoichiometric calibration method yields Hounsfield unit (HU) predictions within less than ±1.3 HU of the theoretical HU calculated from XCOM data averaged over the spectra used (e.g., 80 kVp, 100 kVp, 140 kVp and 140/Sn kVp). A fit of mean excitation energy (I-value) data as a function of EAN is provided in order to determine the ion stopping power of human tissues from ED–EAN measurements. Analysis of the calibration phantom measurements with the Siemens SOMATOM Definition Flash dual source CT scanner shows that the present formalism yields mean absolute errors of (0.3 ± 0.4)% and (1.6 ± 2.0)% on ED and EAN, respectively. For ion therapy, the mean absolute errors for calibrated I-values and proton stopping powers (216 MeV) are (4.1 ± 2.7)% and (0.5 ± 0.4)%, respectively. In all clinical situations studied, the uncertainties in ion ranges in water for therapeutic energies are found to be less than 1.3 mm, 0.7 mm and 0.5 mm for protons, helium and carbon ions respectively, using a generic reconstruction algorithm (filtered back projection). With a more advanced method (sinogram affirmed iterative technique), the values become 1.0 mm, 0.5 mm and 0.4 mm for protons, helium and carbon ions, respectively. These results allow one to conclude that the present adaptation of the stoichiometric calibration yields a highly accurate method for characterizing tissue with DECT for ion beam therapy and potentially for photon beam therapy.

Measurement of Phantomless Thoracic Bone Mineral Density on Coronary Artery Calcium CT Scans Acquired with Various CT Scanner Models

To determine the accuracy and precision of thoracic phantomless bone mineral density (BMD) measurements obtained on coronary artery calcium (CAC) computed tomography (CT) scans by using a variety of commercially available CT scanners. The institutional review board approved this Health Insurance Portability and Accountability Act-compliant study. A total of 4126 asymptomatic subjects (2022 [49%] men, 2104 [51%] women; mean age, 63.7 years ± 11.8 [standard deviation]) underwent CAC CT with the use of a quantitative CT calibration phantom for evaluation of subclinical atherosclerosis. Two hundred eighty subjects also underwent CT of the chest, abdomen, and pelvis (C7 through L5). Mean BMD of three consecutive thoracic vertebrae (in the T7-T10 range) was measured in all 4126 subjects. Individual calibration factors for each phantom insert and a general calibration factor for the spine were determined for each CT scanner model. The study population was then divided into three subgroups: All calibration factors were generated from group 1 (n = 1536) and were applied and tested in group 2 (n = 1587), and effects of various image acquisition parameters were assessed in group 3 (n = 1003). Accuracy (bias) and precision of thoracic phantomless BMD measurements across 14 CT scanner models from five manufacturers were determined. Phantomless BMD values correlated highly with standard phantom-based quantitative CT BMD values (r = 0.987, P < .001). Bias was 3.9% ± 1.4 for phantomless BMD measurements, and the mean coefficient of variation for the general calibration factor was 4.9% ± 2.4. Phantomless BMD can be measured accurately on CAC CT scans acquired with a variety of CT scanners without additional radiation exposure.
