The Construction of a Socialized Service Platform and System for “Internet + Agriculture” in China

The mismatch between supply and demand and inefficient supply in China’s agricultural product market is quite severe, making agricultural supply-side structural reform an important topic and task for rural economic work in China. The successful implementation of agricultural supply-side reform requires a comprehensive agricultural social service platform and system to support it. However, the current agricultural social service platform system in China faces issues such as a lack of coordination among service entities and poor information communication, making it difficult to meet the demands of supply-side reform. To address this issue, under the new circumstances of supply-side reform, this paper proposes the idea of reorganizing the agricultural social service platform and system through industrial chain collaboration theory, and applying “internet +” for technological reengineering of the agricultural social service platform system. Based on this, a new agricultural social service platform system architecture is constructed, which includes service entities, service platforms, service content, and operational models. The research findings provide guidance for agricultural product producers, distributors, sellers, and related service entities along the agricultural industry chain on how to use “internet +” for collaborative decision-making. This approach is beneficial for addressing the supply-demand imbalance and low resource allocation efficiency in China’s agricultural product market, thereby advancing the structural reform of China’s agricultural supply side.

Effects of early childhood growth and development interventions on children less than two years of age

The objective of this study was to determine the effects of early childhood growth and development interventions on children under two years of age. We used PubMed, Science Direct, Scopus, Springer Link, and Wiley to select studies that explored the effects of interventions on the growth and development of children under two years published growth from January 2015 to December 2023. The study used the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. We found 1235 articles, while 26 articles were analyzed thematically. The thematic areas included: impact of intervention, health services implemented, and growth and development of children under two years of age. The results show that providing early and comprehensive education, health and social services has an impact on the growth and development of children under two years of age. Further studies regarding the long-term impact and effectiveness of interventions designed to improve the growth and development of children under two years of age are required.

ANALYSIS OF AGRICULTURAL DEVELOPMENT IN GUANGXI UNDER THE BACKGROUND OF RURAL REVITALIZATION

Chinese economy is undergoing a transformation towards high-quality development. As the foundation of the national economy, agriculture must also adapt to the trend of social and economic development, transform its development mode, and transform towards high-quality agricultural development. The implementation of the rural revitalization strategy must be supported by high-quality agriculture; In the context of rural revitalization, we should focus on the new forms and new connotations of high-quality agricultural development. By establishing an agricultural development quality evaluation index system with 13 indicators from 5 dimensions, including economic benefits, comprehensive production capacity, social services, coordinated development, and sustainable development, we can use the comprehensive index evaluation system to analyze the current situation of agricultural development quality in Guangxi and identify the shortcomings and deficiencies that constrain high-quality agricultural development. The research results of comprehensive indicator evaluation and comparative analysis based on factor analysis method indicate that the overall level of agricultural development quality in Guangxi needs to be improved, with problems such as slow development and improvement speed, common shortcomings that restrict development, relative dispersion of agricultural leading enterprises, and insufficient utilization of agglomeration effect advantages. Based on this, it is proposed to promote the development of agriculture in Guangxi towards branding and intensive scale, cultivate diversified agricultural management entities, improve the agricultural production guarantee system, and accelerate the development of agriculture towards green direction.

ANALYSIS OF THE IMPLEMENTATION OF THE PILOT PROJECT ON THE IMPLEMENTATION OF SOCIAL SERVICES FOR THE FORMATION OF VITALITY IN TERRITORIAL COMMUNITIES

State policy in the field of social protection is designed to promote the well-being of people by providing support to the population at the level of territorial communities. Especially people who find themselves in difficult life situations and the most unprotected categories of the population need such support: persons with disabilities, war veterans and their family members, large families, orphans, internally displaced persons, etc. The introduction of a new social service aimed at helping people and increasing their internal potential, maintaining mental health is fully justified.The purpose of the article is to analyze the implementation of a pilot project for the introduction of a social service for the formation of sustainability in certain territorial communities. A number of tasks were solved during the research. The content of the concept of «sustainability» was analyzed, and the personal qualities necessary for the formation of vitality were determined. We consider resilience as a person's ability to resist trials and stresses, maintain emotional and psychological stability and continue to develop even in difficult situations. It has been found that resilience as a skill can be formed, developed, and taught to others even in crisis conditions.The regulatory and legal documents that determine the specifics of the implementation of the social service for the formation of sustainability have been analyzed, and the conditions for the implementation of the project have been disclosed. Attention is drawn to the resolution of the Cabinet of Ministers of Ukraine «On the implementation of an experimental project on the introduction of a comprehensive social service for the formation of vitality», Order of the Ministry of Social Policy of Ukraine «On the approval of sample forms of documents and reports for the implementation of an experimental project on the introduction of a complex social service on the formation of vitality», orders and orders of individual territorial communities related to the implementation of the project.The activity of the Smilian, Uman and Mliiv territorial communities of Cherkasy region on the implementation of the project on the introduction of social services for the formation of sustainability was analyzed. The expediency of implementing a social service for the formation of vitality in the OTG has been proven.

Program of All-Inclusive Care for the Elderly: an untapped setting for research to advance pain care in older persons.

The Program of All-Inclusive Care for the Elderly (PACE) is a community-based care model in the United States that provides comprehensive health and social services to frail, nursing home-eligible adults aged 55 years and older. PACE organizations aim to support adequate pain control in their participants, yet few evidence-based pain interventions have been adopted or integrated into this setting. This article provides a roadmap for researchers who are interested in collaborating with PACE organizations to embed and evaluate evidence-based pain tools and interventions. We situate our discussion within the Consolidated Framework for Implementation Research (CFIR), a meta-theoretical framework that considers multi-level influences to implementation and evaluation of evidence-based programs. Within each CFIR domain, we identify key factors informed by our own work that merit consideration by research teams and PACE collaborators. Inner setting components pertain to the organizational culture of each PACE organization, the type and quality of electronic health record data, and availability of staff to assist with data abstraction. Outer setting components include external policies and regulations by the National PACE Association and audits conducted by the Centers for Medicare and Medicaid Services, which have implications for research participant recruitment and enrollment. Individual-level characteristics of PACE organization leaders include their receptivity toward new innovations and perceived ability to implement them. Forming and sustaining research-PACE partnerships to deliver evidence-based pain interventions pain will require attention to multi-level factors that may influence future uptake and provides a way to improve the health and well-being of patients served by these programs.

A longitudinal study on the effects of social support on self-stigma, psychiatric symptoms, and personal and social functioning in community patients with severe mental illnesses in China.

Few studies have examined whether social support contributes to better consequences among chronic patients with severe mental illnesses (SMI) in their community recovery stage and whether self-stigma would be a mechanism through which social support impacts psychiatric symptoms and personal and social functioning. This study aimed to examine prospective associations of social support with long-term self-stigma, psychiatric symptoms, and personal and social functioning, and to investigate whether self-stigma would mediate the associations of social support with psychiatric symptoms and personal and social functioning among patients with SMI. A total of 312 persons with SMI (schizophrenia and bipolar disorder) in their community recovery stage participated in the study. Social support, self-stigma, psychiatric symptoms, and personal and social functioning were evaluated at baseline. The follow-up assessment was conducted at 6 months with the baseline measures except for social support. Hierarchical linear regression and mediation analysis were performed. The results showed that baseline social support predicted decreases in stigma (β = -.115, p = .029) and psychiatric symptoms (β = -.193, p < .001), and increases in personal and social functioning (β = .134, p = .008) over 6 months, after adjusting for relevant covariates. Stigma at 6 months partially mediated the association between baseline social support and 6-month psychiatric symptoms (indirect effect: β = -.043, CI [-0.074, -0.018]). Stigma and psychiatric symptoms at 6 months together mediated the association between baseline social support and 6-month personal and social functioning (indirect effect: β = .084, 95% CI [0.029, 0.143]). It is necessary to provide comprehensive social support services and stigma reduction interventions at the community level to improve the prognosis of SMI.

Social welfare institutions that provide comprehensive social welfare services based on the community respond more sensitively to environmental changes than other types of social welfare organizations. As a result, there is a growing demand for not only improving the quality of services provided by social welfare institutions but also creating a safe organizational culture. In this regard, the purpose of this study is to propose a safety culture scale to examine the level of safety culture in social welfare institutions and provide evidence for its validity. To achieve the purpose of the study, a mail survey was conducted targeting social workers at social welfare institutions(community welfare centers, senior welfare centers, and welfare centers for people with disability) located in Busan Metropolitan City. The results of confirmatory factor analysis performed on the 1,260 returned questionnaires were as follows. First, the safety culture scale, which consists of 52 questions on 7 factors: safety activities, teamwork, education/training, policy/system, fair culture, leadership, and perception/attitude, was found to have secured construct validity. Second, the composite reliability of the safety culture scale was above .7, and the average variance extraction(AVE) value was above .5, showing that convergent validity was secured. The safety culture scale of social welfare institutions presented in this study can be used to monitor the level of safety culture of individual institutions, identify areas requiring improvement, and prepare alternatives. In addition, it is expected to provide meaningful information for follow-up researchers regarding the safety culture of social welfare institutions.

Impact of Microcredit on Income and Social Status of Rural Women Microentrepreneurs in Southeast Nigeria

This study investigates the impact of microcredit on poverty reduction and social status enhancement among rural women microentrepreneurs in patriarchal Southeast Nigeria. The objective is to assess whether microcredit improves economic conditions and elevates social standing within a traditionally patriarchal society. Employing a quantitative approach, the study surveyed 340 rural women who have accessed microcredit across four states in Southeast Nigeria. Data were collected using a structured questionnaire, and statistical analyses were conducted to evaluate the effects of microcredit on income generation and social status. The findings reveal that while microcredit significantly boosts income generation, its impact on social status is less pronounced, constrained by persistent patriarchal norms and limited supportive measures. To address this limited impact, it is recommended that microcredit programs be complemented with comprehensive social support services. Such initiatives should focus on improving community relationships and addressing gender-specific barriers within the socio-cultural context. By integrating these support mechanisms, microcredit programs can more effectively enhance both the economic and social standing of women, fostering a more supportive environment for their empowerment.

The Limited Scope of Social Work Intervention at Zunheboto Sadar, Nagaland, India: An In-depth Study on the Vulnerabilities Faced by Adolescents

This research investigates the limitations and challenges defining the scope of social work intervention in Zunheboto Sadar, an idyllic township in Nagaland, India, with a population of approximately 31,100. The study focuses on the constrained access to essential services, particularly affecting the adolescent community. Issues such as illicit drug use, mobile phone addiction, anxiety, depression, and low self-esteem are explored. The research unveils the harsh reality of vulnerable adolescents navigating these challenges with minimal assistance, as the sole NGO in the region primarily targets individuals aged 18 and above. The inquiry also highlights the community’s widespread lack of awareness regarding the consequences of these issues. The absence of comprehensive social work services emphasizes the urgent need for a broader range of interventions in Zunheboto Sadar, crucial for addressing the predicaments faced by its adolescent population.

Integrated Health and Social Care In A Woman's Shelter and Multi-Service Agency

Introduction: Evidence exists demonstrating that integrated health and social care (IHSC) can support enhanced care for highly vulnerable and marginalized population groups. One such group are homeless women. Street Haven (SH) is a multi-service women’s agency that serves homeless women who suffers from the intersection of critical health, social and financial disadvantages living in one of the poorest urban neighbourhoods in Toronto, Canada. SH provides shelter, supported housing, addictions treatment and training support services. Over the past ten years, client acuity has increased the health and social care needs of vulnerable women. The pandemic has exacerbated this. Clients have experienced chronic homelessness which is connected to trauma. They also suffer from mental illness conditions, and chronic diseases including respiratory illnesses, and gastro-intestinal illness, and diabetes. Street Haven has engaged in integrated health and social care (IHSC) to ensure that more comprehensive care can respond to the increasing client care complexity. &#x0D; Approach: IHSC coordination with community partners have been undertaken in order to address unmet health and social care. Within the SH shelter services, partnerships with primary health care partners, hospital services and community-based crisis and mental health teams have been established. As a result, a primary care physician and psychiatrist can be accessed once weekly on site by clients. Homeless women are then attached to a physician and psychiatrist while accessing SH services. In addition, when clients experience crisis, community-based outreach mental health and crisis teams are dispatched by other organizations to SH locations. The SH addiction treatment residence services also integrate with psychiatry services with a community clinical services agency and with the community crisis team to introduce crisis and psychiatric supports for addiction service clients. Lastly, SH works with community policing to ensure that women who exhibit behaviours stemming from crisis, may have benefit of community policing services that provide SH and client with protection while offering crisis supports at the same time. They help to divert clients from hospital and correctional services.&#x0D; Results: IHSC enables SH to better meet the needs of its highly complex client base within its premises. Several benefits of the IHSC include the identification of unmet health and social care needs of our clients including psychiatric and mental health needs and management of chronic diseases and medication prescription needs; enhanced client community engagement through the referral and coordination of services with community health and social care partner agencies; minimization of crisis events on SH premise sites; and minimizing utilization of acute car and correctional services. The competencies that are absent in the organization are available due to IHSC.&#x0D; Lessons Learned: IHSC can act as a lever for women’s shelter and addiction treatment services to provide more comprehensive health and social care services. IHSC is crucial considering the austerity in government program funding in Toronto and Ontario. IHSC is another means to better serve marginalized, homeless and impoverished women to ensure that they have more equitable access to health and social care services especially where there is a gender-bias against women’s services in Toronto. &#x0D;

Vietnam's Responses to Dementia - An Assessment of Service Delivery.

This study was conducted to assess Vietnam's dementia service delivery. Using WHO methodology, website searches of key organisations focused on three aspects of Vietnam's healthcare system: (1) Health and social workforce; (2) Services, supports and treatment programs; and (3) Promotion of awareness and understanding. Data were analysed using content analysis. While key members of the healthcare workforce receive some education in dementia competencies during their training, the skill-mix of staff in the current workforce appears inadequate to address the complex needs of people with dementia. Although Vietnam's general healthcare system comprises a good variety of service types, there is a lack of dementia-specific services. Available diagnosis and treatment services are concentrated in the hospital system and are mainly located in metropolitan areas, impacting their accessibility. While both community-based and institutional long-term care is available, institutional care is not universally accessible and home-based care is mainly provided by family carers who don't have access to dementia care training. There is no active dementia prevention or public awareness campaign. To improve the ability of Vietnam's service delivery to meet the needs of people with dementia and their carers, the skill-mix of the healthcare workforce should be strengthened by ensuring that dementia core competencies are embedded within undergraduate and graduate education programs and making post-qualification dementia care training available. The capacity of existing community-level health and social services should be expanded to ensure that integrated, specialised and comprehensive health and social services are accessible to all people with dementia. Expanding access to institutional long-term care and making dementia education available to family and other informal carers could increase choice and improve quality of care. Finally, Vietnam could look to other countries in the region with regards to the development of a dementia prevention and public awareness campaign.

Mental Health and Public Health: Improving Mental Health and Well-being of Communities in the Modern World: A Pragmatic Approach Using the Global Mental Health Assessment Tool/PC

The World Health Organization report of 2022 on mental health highlights the slow progress in reducing the treatment gap for people with mental illness worldwide. It emphasizes that everyone should participate in improving the mental health and well-being of the communities. The governments should put their act together and all agencies should work in coordination to address the mental health needs of their population. They should also focus on mental health promotion, early intervention, and prevention of suicide. The Mental Health Action Plan 2013–2030 outlines the importance of leadership and governance for mental health in providing comprehensive, integrated, and responsive mental health and social care services in community-based settings. Stigma, human resource shortages, fragmented service delivery models, and poor commitment of authorities for mental health are some of the factors for the ongoing service gap in mental health. Providing mental health knowledge and assessment skills to frontline workers, supporting frontline workers by mental health specialists using their leadership skills, and fully utilizing mobile and Internet technology in providing education and training as well as providing services could bring down the treatment gap of mental disorders worldwide. The Global Mental Health Assessment Tool for Primary Care and General Health Setting (GMHAT/PC)-based training package has been developed and successfully implemented in training frontline health workers in different parts of the word. The authors propose that this may be a pragmatic way to integrate mental health in public health by skilling frontline workers in assessing mental health along with physical health and by providing holistic care.

Механізм управління комунальним майном як фактор активізації соціальної послуги життєстійкості в умовах децентралізації та війни

The essence of a comprehensive social service for the formation of resilience is disclosed. The purpose and expected results from the implementation of the pilot project for the provision of social services for resilience are outlined. The tasks of the authorized bodies of the project participants are presented. The conditions for the participation of the territorial community in the introduction of a comprehensive social service for the formation of resilience are analyzed. The path of the community to the formation of resilience is considered. The difficulties of territorial communities on the way to the efficient use of benefits from the point of view of the interests of their residents are analyzed, in particular: arrangement of premises for the provision of a comprehensive service and ensuring its maintenance from the community budget; a large number of objects of communal property that is not used or that is in an abandoned state; a significant share of municipal infrastructure that is damaged as a result of the full-scale russian invasion, which has become a real challenge for each territorial community, etc. It is noted that the development of social services in territorial communities, in particular resilience services, will be facilitated by the efficient use of communal property resources, including: inventory of available communal property; determination of the number of communal property objects (including by starostats), the premises of which can be used for the work of social service providers, in particular those that meet the established requirements for the provision of viability services, taking into account the geography of their location (remoteness, quantity of population, conveyance, etc.), the needs of the population of specific settlements (starostats) in social services, depending on age, social status, social and demographic tendencies, etc.; formation of an open electronic database of communal property objects, on the basis of which potential providers of social services can work to attract/interest potential partners, including non-governmental organizations. A mechanism of communal property management in the context of activating the social service of resilience in the context of decentralization and war is proposed.

THE END OF PACE: OPPORTUNITY FOR ADVOCACY AND EDUCATION

Abstract The Program for All-Inclusive Care for the Elderly (PACE) is a Medicare/Medicaid Managed Care benefit for frail older adults age 55 years or older who, although certified by the state as long-term care (LTC) eligible, choose to live independently in the community. PACE features comprehensive medical and social services coordinated by an interdisciplinary team whose goal is to promote independence. Cheyenne Regional Medical Center has served as Wyoming’s only PACE site since 2007. However, an effort to reduce Medicaid expenditures resulted in elimination of funding and closure of the program by the state. The purpose of this study was to examine the impacts of PACE closure on transition to LTC, concomitant Medicaid expenditures, as well as impacts on participant well-being. Results showed, as expected, that nursing home placement exceeded that projected, thereby increasing Medicaid expenditures in Wyoming. Additional impacts on participant well-being and advocacy benefits of GWEP partnership are discussed.

Development and Implementation of State and Federal Child Welfare Laws Related to Drug Use in Pregnancy.

Amid increasing drug use among pregnant individuals, legislators have pursued policies intended to reduce substance use during pregnancy. Many states have utilized child welfare mechanisms despite evidence that these policies might disincentivize treatment-seeking. Recent federal changes were intended to facilitate care for substance-exposed infants and their families, but implementation of these changes at the state level has been slowed and complicated by existing state policies. We seek to provide a timeline of state child welfare laws related to prenatal drug use and describe stakeholder perceptions of implementation. We catalogued child welfare laws related to prenatal drug use, including laws that defined child abuse and neglect and established child welfare reporting standards, for all 50 states and the District of Columbia (DC), from 1974 to 2019. In the 19 states that changed relevant laws between 2009 and 2019, qualitative interviews were conducted with stakeholders to capture state-level perspectives on policy implementation. Twenty-four states and DC have passed laws classifying prenatal drug use as child abuse or neglect. Thirty-seven states and DC mandate reporting of suspected prenatal drug use to the state. Qualitative findings suggested variation in implementation within and across states between 2009 and 2019 and revealed that implementation of changes to federal law during that decade, intended to encourage states to provide comprehensive social services and linkages to evidence-based care to drug-exposed infants and their families, has been complicated by existing policies and a lack of guidance for practitioners. Many states have enacted laws that may disincentivize treatment-seeking among pregnant people who use drugs and lead to family separation. To craft effective state laws and support their implementation, state policymakers and practitioners could benefit from a treatment-oriented approach to prenatal substance use and additional state and federal guidance.

Human Trafficking Survivors’ Self-Reported Satisfaction and Performance in Daily Life Activities

ABSTRACT The purpose of the study is to identify occupational performance problems self-identified by survivors of human trafficking through the use of the Canadian Occupational Performance Measure to further understand the occupational needs of this population. This retrospective cross-sectional study categorized 428 occupational performance problems into areas of occupation. The results indicated that social and emotional health promotion and maintenance (n = 57), leisure exploration/participation (n = 52), sleep preparation and participation (n = 49), financial management (n = 49), and symptom and condition management (n = 42) were the most common areas of occupation that victims identified as occupational performance problems. The National Action Plan to Combat Human Trafficking calls for comprehensive social and health care services to support recovery and for identification and meeting of research gaps. This study answers these calls by identifying areas of daily life that are difficult for individuals who exit trafficking. Additionally, this information identifies the need for occupational therapy as part of the comprehensive service team to support stability and recovery.

Delivering Comprehensive Social Services during a Pandemic: Experience of a New York City Student-Run Free Clinic

Background: We describe the implementation of a remote operational model to provide targeted, multi-faceted social services during the coronavirus disease 2019 (COVID-19) pandemic at the East Harlem Health Outreach Partnership (EHHOP), a student-run, physician-supervised free clinic (SRFC) that serves uninsurable residents of East Harlem in New York City (NYC). The model attempts to mitigate the economic consequences of the pandemic while also safely meeting the needs of patients who were quarantined or otherwise medically vulnerable. &#x0D; Methods: We outline a step-by-step approach required to transition social services to a remote model, across six key workflows: (1) student volunteer recruitment, (2) fundraising, (3) grocery and financial grant allocation, (4) medication delivery, (5) mask delivery and patient education, and (6) broader community engagement.&#x0D; Results: Within 20 days of the first known case of COVID-19 in NYC, we established a protocol for remote care and expanded social services. From March to July 2020, EHHOP volunteers made 221 medication and 172 mask kit no-contact deliveries. To address food and housing insecurity, 140 patients were provided financial grants and an additional 109 received food deliveries. This comprehensive response was supported through emergency fundraising efforts that generated $66,690.&#x0D; Conclusions: By focusing on support for basic needs including food, medication, personal protective equipment, and patient education, EHHOP was able to bolster the safety-net for marginalized patients otherwise excluded from national economic recovery efforts and ensure continuous care for patients with chronic medical illness. EHHOP’s operational model for safe, remote delivery of social services provides other clinics with a framework to guide current and future emergency responses.

Program of All-Inclusive Care for the Elderly (PACE) versus Other Programs: A Scoping Review of Health Outcomes.

The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive health and social services to community-dwelling older United States (US) adults. However, little is known about how PACE outcomes compare to similar caregiving programs. This scoping review searched nine databases to identify studies that compared economic, clinical, or humanistic outcomes of PACE to other caregiving programs in the US. Two reviewers independently screened and extracted data from relevant articles and resolved discrepancies through consensus. From the 724 articles identified, six studies were included. Example study outcomes included: limitations and needs, survival and mortality, healthcare utilization, and economic outcomes. In conclusion, there are few published comparisons of PACE outcomes versus other caregiving programs for older US adults, and identified studies indicate mixed results. Further studies are needed to compare PACE outcomes to other programs so that policymakers are well informed to manage and optimize health outcomes for the growing US older adult population.

Cost and Utilization Impacts of a Medicaid Managed Care Organization's Behavioral Health Wraparound Program.

Wraparound programs that provide comprehensive evidence-based outpatient treatment, transportation, social services, and housing supports have shown promise for improving clinical behavioral health-related outcomes to reduce the need for institutionalized care; however, the majority of evidence is based on wraparound programs for children. This study examined the impact of a wraparound program for adult Medicaid managed care organization members with serious mental health or substance use disorders on health care costs and utilization. This retrospective observational study used 2013-2018 claims data collected from a large Medicaid managed care organization operating in multiple states. We used an intention-to-treat difference-in-difference study design to examine the association of the wraparound with costs and utilization. Adult Medicaid members with an emergency department (ED) or inpatient visit for a behavioral health condition (index visit) were eligible for the study. Outcomes included all-cause and behavioral health-related costs and utilization during follow-up after the index visit's admission date. Outcomes were calculated overall, as well as separately by inpatient, ED, and outpatient/wraparound settings. We found that during the first post-admission month, the wraparound program was associated with 27.6 percentage points (PP) and 27.2 PP reductions in the number of behavioral health-related inpatient nights and costs, respectively. However, during subsequent months (median follow-up ranging from 7 to 10months) there were no associations with per-member-per-month total all-cause or behavioral health-related costs. Nonetheless, the wraparound program was associated with 12.3 PP reduction in all-cause cost during the entire study period among a subset of members who were high cost at the baseline. Reduced hospital utilization and costs during the first month of wraparound services were fully counteracted by outpatient, housing, and other wraparound services costs during the following months. This indicates the importance of proper payment arrangements with value-based contracting or performance targets with wraparound services providers to align the objective of reducing inpatient use. Future wraparound programs may consider a more focused recruitment from high-cost members with complex care needs. However, our estimates were conservative given that it's from a single Medicaid managed care organization's perspective and some benefit from investing in addressing social needs may be realized in longer term (beyond our study period). States' Medicaid programs may consider the longer-term cost and broader, societal benefit of wraparound investment.
