Policy action points and approaches to promote fertility care in The Gambia: Findings from a mixed-methods study.

In the Global South, (in)fertility care is scarcely recognized as a priority, yet the government of The Gambia has recently included it as one of the key priorities in its reproductive health strategic plan. This inclusion appears to be the result of years of engagement between policy actors, academic researchers, and activists in the field of reproductive health and specifically of infertility. However, the operationalization of the strategic plan may be hampered by multiple factors. The research aims to identify and analyze challenges that may impede the effective implementation of the strategic plan, thereby providing policy action points and practical guidance into the operationalization of (in)fertility care in the context of The Gambia's health system. This is a mixed-methods study with data from a survey and semi-structured interviews collected between 2020 and 2021 in The Gambia that were separately published. In this paper, we present the triangulation of quantitative and qualitative data using a convergence coding matrix to identify relevant policy action points. Six fertility care policy action points, driven by data, arose from the triangulation and interpretation process, specifically: (i) establishing and maintaining political commitment and national priority for fertility care; (ii) creating awareness and increasing the involvement of men in SRH and fertility; (iii) ensuring data-driven health policymaking; (iv) offering and regulating affordable IVF alternatives; (v) improving knowledge of and means for fertility care provision; and (vi) enhancing the collaboration among stakeholders and building links with the private healthcare sector. This study found the implementation of the fertility care-related activities in the reproductive health strategic plan may face challenges that require careful mitigation through a holistic approach. Such an approach conceptualizes infertility not just as a biomedical issue but as a broader one that incorporates educational and socio-emotional aspects, including male and (not only) female involvement in sexual and reproductive health. Moreover, it is supported by a comprehensive health management information system that includes capturing data on the demand for, and access to, infertility services in The Gambia health system.

Association between long-term exposure to fine particulate matter and its chemical constituents and premature death in individuals living with HIV/AIDS

Long-term exposure to fine particulate matter (PM2.5) is associated with an increased total mortality. However, the association of PM2.5 with mortality in people living with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS, PLWHA) and the relationship between its constituents and adverse outcomes remain unknown. In this cohort study, 28,140 PLWHA were recruited from the HIV/AIDS Comprehensive Response Information Management System of the Hubei Provincial Centre for Disease Control and Prevention in China between 2001 and 2020. The annual PM2.5 chemical composition data, including sulfate (SO42−), nitrate (NO3−), ammonium (NH4+), black carbon (BC), and organic matter (OM), was extracted from the Tracking Air Pollution (TAP) dataset in China. A Cox proportional hazard model with time-varying exposure and time-to-event quantile-based generalized (g) computation was used to assess the associations between PM2.5 chemical constituents, and mortality in PLWHA. A multivariate Cox proportional hazard model estimated an excess hazard ratio (eHR) of 0.32% [95% confidence interval (CI): (0.01%, 0.64%)] for AIDS-related death (ARD), associated with 1 μg/m3 rise in PM2.5 exposure. An increase of 1 μg/m3 in NH4+ was associated with 5.13% [95% CI: (2.89%, 7.43%)] and 2.97% [95% CI: (1.52%, 4.44%)] increase in the risk of ARD and all-cause deaths (ACD), respectively. When estimated using survival-based quantile g-computation, the eHR for ARD with a joint change in a decile increase in all five components was 6.10% [95% CI: 3.77%, 8.48%)]. Long-term exposure to PM2.5 chemical composition, particularly NH4+ increased the risk of death in PLWHA. This study provides epidemiological evidence that SO42− and NH4+ increased the risk of ARD and that NH4+ increased the risk of ACD in PLWHA. Multi-constituent analyses further suggested that NH4+ may be a key component in increasing the risk of premature death in patients with HIV/AIDS. Individuals aged ≥65 with HIV/AIDS are more vulnerable to SO42−, and consequent ACD.

Long-term exposure to ambient particulate matter is associated with prognosis in people living with HIV/AIDS: Evidence from a longitudinal study

BackgroundEvidence on the association between particulate matter (PM) exposure and prognosis in people living with HIV/AIDS (PWHA) is scarce. We aim to investigate the associations of long-term exposure to PM with AIDS-related deaths and complications. MethodsWe collected follow-up information on 7444 PWHAs from 2000 to 2021 from the HIV/AIDS Comprehensive Response Information Management System of the Wuhan Center for Disease Control and Prevention. The AIDS-related deaths and complications were assessed by physicians every 3 to 6 months, and the monthly average PM concentrations for each PWHA were extracted from the China High Air Pollutants dataset. We employed time-varying Cox regression models to evaluate the associations of the average cumulative PM exposure concentrations with AIDS-related deaths and complications, as well as the mediating effects of AIDS-related complications in PM-induced AIDS-related deaths. ResultsFor each 1 μg/m3 increase in PM1, PM2.5, and PM10, the adjusted hazard ratios (HRs) for AIDS-related deaths were 1.021 (1.009, 1.033), 1.012 (1.005, 1.020), and 1.010 (1.005, 1.015), respectively; and the HRs for AIDS-related complications were 1.049 (1.034, 1.064), 1.029 (1.020, 1.038), and 1.031 (1.024, 1.037), respectively. AIDS-related complications mediated 18.38 % and 18.68 % of the association of exposure to PM1 and PM2.5 with AIDS-related deaths, respectively. The association of PM exposure with AIDS-related deaths was more significant in older PWHA. Meanwhile, the association between PM exposure and AIDS-related complications was stronger in PWHA with a BMI ≥ 24 kg/m2. ConclusionLong-term exposure to PM is positively associated with AIDS-related deaths and complications, and AIDS-related complications have mediating effects in PM-induced AIDS-related deaths. Our evidence emphasizes that enhanced protection against PM exposure for PWHAs is an additional mitigation strategy to reduce AIDS-related deaths and complications.

Ambient temperature variations and AIDS-related mortality: A time-stratified case-crossover study in 103 counties, China

BackgroundClimate change, characterized by the steady ascent of global temperatures and the escalating unpredictability of climate patterns, poses multifaceted challenges to public health worldwide. However, vulnerable groups, particularly the population affected by HIV/AIDS, have received little attention. ObjectivesWe aimed to examine the impacts of temperature variations on AIDS-related mortality. MethodsData on individuals with HIV/AIDS were collected from the HIV/AIDS Comprehensive Response Information Management System between 2013 and 2019. Temperature variation metrics were constructed by diurnal temperature range (DTR), temperature changes between neighboring days (TCN), and temperature variability (TV0-t). Time-stratified case-crossover design with conditional logistic regression models was used to investigate the associations between ambient temperature variations and AIDS-related mortality. ResultsEach 1 °C elevated in DTR was linked with a 5.28 % [95 % confidence intervals (CIs): 1.61, 9.08] increment in AIDS-related mortality at a lag of 0–6 days. Stronger associations between DTR and AIDS-related mortality were observed in the married than in single, with corresponding excess ORs (%) of 5.33 (95 % CIs: 0.29, 10.62) versus 4.79 (95 % CIs: −0.50, 10.36) for 1 °C increased in DTR at lag 0–6 days. Additionally, we noticed the impact of DTR was more pronounced in the warm season, leading to a 7.32 % (95 % CIs: 0.57, 14.51) elevation in the risks of AIDS-related mortality for 1 °C increase in DTR at lag 0–6 days, while the effect value decreased to 5.16 % (95 % CIs: 0.71, 9.81) in the cold season. ConclusionsOur findings indicated that DTR might be a significant risk factor for AIDS-related deaths among ambient temperature variation indicators, and underscored the importance of considering temperature variability in public health interventions aimed at mitigating this risk of AIDS-related mortality.

Survey and Mapping Geographic Information Archive Informationization Management Construction Analysis

With the rapid development of science and technology and the rapid accumulation of geographic information data, the management of surveying and mapping geographic information archives is facing increasing challenges. The traditional manual management mode makes it difficult to meet the needs of large-scale data processing and sharing, so information management has become a key strategy to improve the efficiency of data management and optimize the use of resources. In this context, this paper discusses the construction of informatization management around surveying and mapping geographic information archives. The key steps in the construction of information management include the nodes of technical infrastructure, data standardization and organization, digital conversion of archives, archive retrieval and query system, security and authority management, and continuous updating and maintenance of archives. At the same time, to promote the construction of information management, work can be started from the formulation of a comprehensive information management plan, the establishment of cross-departmental collaboration mechanisms, the strengthening of personnel training and technical support, as well as the establishment of a data quality management system, to meet the needs of multiple fields and to realize the greater value of data.

Comprehensive information management system for a medical research cohort biobank based on quality by design

BackgroundWith the development of big health and big data, cohort research has become a medical research hotspot. As an important repository of human genetic resources, biobanks must adapt to the requirements of large-scale and efficient operation. Thus, biobanks urgently need to design and build a legal, convenient, and efficient information management system.MethodsThis study applies the concept of “quality by design” to build a comprehensive biobank information management system based on the analysis of user requirements, legal and regulatory risks, and industry-standard requirements. The system integrates the management of scientific research projects, biological specimens, clinical information, quality control, and multi-dimensional information query and development. After 10 months of its operation, the comprehensive management system was evaluated through statistical analysis of the efficiency of the construction of the pregnancy–birth cohort and the quality of genetic resources.ResultsSince the system’s launch, the statistics on cohort construction efficiency show that the enrollment rate of eligible pregnant women has increased, and the rate of missing volunteers has dropped. The time needed to establish a 1000-person cohort (with complete biological samples and clinical information in early, middle, and late pregnancy) was reduced, and the effective tracking rate of the samples was 77.42%. The error rate of the deep cryogenic refrigerator decreased, with a clinical information integrity rate of 96.47%.ConclusionsThe comprehensive biobank information management system constructed with the “quality by design” concept is well suited to meet the requirements of medical research. This study provides a solution for designing a comprehensive information system for medical institutions’ biobanks.

Economic valuation of seagrass ecosystem in Maribojoc Bay, Bohol, Philippines

Abstract. Mascari?as HJC, Otadoy JB. 2023. Economic valuation of seagrass ecosystem in Maribojoc Bay, Bohol, Philippines. Biodiversitas 24: 4427-4435. The seagrass ecosystem is one of the most productive and precious ecosystems. In the Philippines, however, more data on the economic valuation of all seagrass ecosystem services must be collected. The study estimated the total economic value of the seagrass ecosystem, including use and non-use values. Resource user groups include municipal ?shers, gleaners, and tourism business operators. The sample size was determined based on the total number of resource users in Maribojoc Bay, Bohol, Philippines and using a confidence coefficient of 95% (P = 0.05). The total economic value of the seagrass ecosystem in Maribojoc Bay is PhP 52.8 million (US$ 1.1 million). Direct use values, or market benefits, are PhP 35.4 million (US$ 708 thousand), accounting for 67% of the total economic value. Indirect and non-use values, are PhP 17.3 million (US$ 346 thousand), accounting for 33% of the total economic value. On a per hectare basis, the economic value of the seagrass ecosystem in Maribojoc Bay is PhP 46,722 (US$ 934). The results of this valuation can be an input in decision-making to manage and conserve the seagrass ecosystem as well as maintain the economic security of Maribojoc Bay. The findings of this study are valuable for developing a comprehensive information management and decision-support system for the management and protection of the seagrass ecosystem. However, more research is required to evaluate the range of ecosystem services provided by seagrass ecosystems across the country, focusing on qualitative value attributions linked to the social-ecological dynamics of seagrass systems.

Analysis on epidemiological characters and HIV care continuum of HIV-infected students: a retrospective cohort study in Shandong province, China

BackgroudThe proportion of HIV-infected students in China showed an increasing trend. This study aimed to identify the epidemiological characteristics and the HIV care continuum for HIV-infected students in Shandong Province, China.MethodsCase report and follow-up data of HIV-infected students were obtained from the National HIV/AIDS comprehensive response information management system. Logistic regression analyses were used to analyze the associating factors of HIV-infected students accepting CD4 + T cells (CD4) test and antiviral therapy (ART) in 30 days, and ArcGIS software was used for the spatial anlysis.ResultsFrom 2017 to 2019, 403 HIV-infected students were reported in Shandong Province. The majority of them were male (99.5%) and transmitted through homosexual sexual activity(92.1%). Most of them lived in Jinan city and Qingdao city. 68.5% (276 cases) accepted CD4 test in 30 days, and 48.6% (196 cases) started ART in 30 days. The heterosexual transmitted cases (AOR = 0.458, 95%CI: 0.210–0.998), patients accepting HIV care in western area (AOR = 0.266,95%CI: 0.147–0.481) were less likely to test CD4 within 30 days; patients aged 23–25 (AOR = 2.316, 95%CI: 1.009–5.316) and patients who had tested CD4 within 30 days (AOR = 4.377; 95%CI: 2.572–7.447) prefered to receive ART within 30 days; patients accepted HIV care in central area (AOR = 0.407; 95%CI: 0.251–0.657) and western area (AOR = 0.508; 95%CI: 0.261–0.989) and patients diagnosed by voluntary blood donation (AOR = 0.352; 95%CI: 0.144–0.864) were less willing to receive ART in 30 days.ConclusionsThe HIV care continuum of HIV-infected students in Shandong Province still needed strenghthing. More health education and case management should be done for cases transmitted through heterosexual behavior, accepted HIV care in central and western area, and diagnosed by voluntary blood donation.

Knowledge Management System for Handcrafted Reog Ponorogo Products

Reog is one of the distinctive cultural practices of Ponorogo. Typically, this art is performed in an open space with many entertainers. Reog Ponorogo enthusiasts will primarily purchase trinkets and handicrafts. Numerous artisans in Reog Ponorogo are members of Micro, Small, and Medium-Sized Enterprises (MSMEs). However, because of the global Covid-19 pandemic, Reog Ponorogo artisans have encountered numerous challenges, beginning with the procurement of raw materials, and continuing through the production process and marketing. This even put them out of business with the Reog artisans. A knowledge Management System (KMS) is one of the technologies that can be utilized to surmount the problem under these conditions. Knowledge Management System (KMS) is a centralized database utilized to organize, store, and disseminate organizational knowledge with employees and customers. In this study, the KMS was developed to assist MSME actors in acquiring information and knowledge concerning MSME Products in Reog Ponorogo Handicrafts. Currently, information about Reog Ponorogo is widely available in print and digital media, but it is not governed by a comprehensive information management system. Therefore, building KMS for Reog Ponorogo Handicraft MSMEs is still necessary. In this research, a modified variant of the Knowledge Management System Life Cycle (KMSLC) was used to develop the KMS. This research was conducted in multiple phases, including infrastructure evaluation, team formation, knowledge capture, and KMS design. This study contributes to the creation of a knowledge management system for MSME Reog Ponorogo artisans based on the life cycle of knowledge management systems. This study's findings serve as the basis for the Reog Ponorogo craft knowledge management system. Further research can concentrate on developing the Reog Ponorogo UMKM KMS on Android and iOS mobile phones, enhancing the KMS user interface, and granting both experts and UMKM administrative access to the KMS.

Proposed Farm Information Management System Design and Database Management for Recommendation Fertilizer and Financial Record Feature

This study proposes a comprehensive Farm Information Management System (FIMS) design and database management strategy for addressing the specific requirements of Indonesian agriculture, with a particular emphasis on fertilizer recommendation and financial record management. The study employs the Software Development Life Cycle (SDLC) methodology as a framework for directing the system’s design, development, and implementation. The goal of the FIMS is to provide producers with a user-friendly interface to access accurate fertilizer recommendations based on crop-specific requirements and local soil conditions. Additionally, the system includes a robust financial record function to aid producers in effectively managing their income, expenses, and investments. The proposed SDLC-based approach assures a structured and methodical development process, including requirements analysis, system design, implementation, testing, and maintenance. Using the SDLC methodology, this study seeks to develop a scalable, secure, and interoperable FIMS that can be effectively integrated into Indonesia’s existing agricultural infrastructure. The effectiveness and usability of the proposed system will be determined through extensive testing and evaluation to ensure its suitability and functionality in actual agricultural contexts. Ultimately, the proposed Farm Information Management System, developed using the SDLC method, aims to empower Indonesian farmers with optimized fertilizer recommendations and streamlined financial record management, thereby contributing to sustainable agriculture practices, and enhanced economic incomes for the nations.

Remote sensing identification of urban water pollution source types using hyperspectral data

Owing to accelerated urbanisation, increased pollutants have degraded urban water quality. Timely identification and control of pollution sources enable relevant departments to effectively perform water treatment and restoration. To achieve this goal, a remote sensing identification method for urban water pollution sources applicable to unmanned aerial vehicle (UAV) hyperspectral images was established. First, seven fluorescent components were obtained through three-dimensional excitation-emission matrix fluorescence spectroscopy of dissolved organic matter (DOM) combined with parallel factor analysis. Based on the hierarchical cluster analysis of the seven fluorescence components and three spectral indices, four pollution source (PS) types were determined, namely, domestic sewage, terrestrial input, agricultural and algal, and industrial wastewater sources. Second, several water colour and optical parameters, including the absorption coefficient of chromophoric DOM at 254 nm, humification index, chlorophyll-a concentration, and hue angle, were utilised to develop an identification method with a recognition accuracy exceeding 70% for the four PSs that is suitable for UAV hyperspectral data. This study demonstrated the potential of identifying PSs by combining the fluorescence characteristics of DOM with the optical properties of water, thus expanding the application of remote sensing technologies and providing more comprehensive and reliable information for urban water quality management.

Spatiotemporal responses of net primary productivity of alpine ecosystems to flash drought: The Qilian Mountains

Understanding the impact of high-altitude flash droughts on vegetation net primary productivity (NPP) is crucially important for ecosystem management. Here, we investigated the response of NPP to flash drought with a case of the Qilian Mountains (QLMs). Our results indicated that the intensity of flash drought in the QLMs increased significantly during 1982–2018, and NPP was highly sensitive to flash drought, with 64.3 % (124,245 km2) of the region showing highly negative responses. The partial correlation analysis between NPP and meteorological factors in terms of different drought intensities showed that intensified flash drought enhanced the coupling between NPP and temperature/VPD. In the short-duration flash drought events with positive temperature anomalies, temperature increase will promote the accumulation of NPP; however, the carbon uptake of plants was significantly compromised by VPD in the flash drought events with negative temperature anomalies. Our study may provide valuable information for drought monitoring and comprehensive ecosystem management.

Analysis on survival time and influencing factors among reported HIV/AIDS in Yunnan Province, 1989-2021

Objective: To analyze the survival time of reported HIV/AIDS and influencing factors of Yunnan Province from 1989 to 2021. Methods: The data were extracted from the Chinese HIV/AIDS comprehensive response information management system. The retrospective cohort study was conducted. The life table method was applied to calculate the survival probability. Kaplan-Meier was used to draw survival curves in different situations. Furthermore, the Cox proportion hazard regression model was constructed to identify the factors related to survival time. Results: Of the 174 510 HIV/AIDS, the all-cause mortality density was 4.23 per 100 person-years, the median survival time was 20.00 (95%CI:19.52-20.48) years, and the cumulative survival rates in 1, 10, 20, and 30 years were 90.75%, 67.50%, 47.93% and 30.85%. Multivariate Cox proportional risk regression model results showed that the risk of death among 0-14 and 15-49 years old groups were 0.44 (95%CI: 0.34-0.56) times and 0.51 (95%CI:0.50-0.52) times of ≥50 years old groups. The risk for death among the first CD4+T lymphocytes counts (CD4) counts levels of 200-349 cells/μl, 350-500 cells/μl and ≥501 cells/μl groups were 0.52 (95%CI: 0.50-0.53) times, 0.41 (95%CI: 0.40-0.42) times and 0.35 (95%CI: 0.34-0.36) times of 0-199 cells/μl groups. The risk of death among the cases that have not received antiretroviral therapy (ART) was 11.56 (95%CI: 11.26-11.87) times. The risk for death among the cases losing to ART, stopping to ART, both losing and stopping ART was 1.66 (95%CI:1.61-1.72) times, 2.49 (95%CI:2.39-2.60) times, and 1.65 (95%CI:1.53-1.78) times of the cases on ART. Conclusions: The influencing factors for the survival time of HIV/AIDS cases were age at diagnosis in Yunnan province from 1989 to 2021. The first CD4 counts levels, antiretroviral therapy, and ART compliance. Early diagnosis, early antiretroviral therapy, and increasing ART compliance could extend the survival time of HIV/AIDS cases.

A Semi-Explicit Practical Coding Method for Prefabricated Building Component Parts in China

In the context of building industrialization, the dissemination and transmission of information related to prefabricated building component parts are of paramount importance throughout the building’s life cycle and the entire industry chain, necessitating high standards of information integration. At present, the development of information coding for prefabricated building component parts in China is hindered by various challenges, such as the lack of uniform norms and standards across different regions, as well as significant differences in the composition and character length of coded information categories. Through reviewing academic papers and society, group, and local standards related to the coding of prefabricated building component parts, this article identifies the importance of component part coding information categories. Considering the readability of codes, a practical coding system is adopted for the coding design, proposing a semi-implicit practical coding method for prefabricated building component parts. This method has strong flexibility and wide applicability. Additionally, a common coding system for component parts is devised to address the issue of missing information caused by the limited characters of component parts codes. This system enables comprehensive life cycle information management of component parts of prefabricated buildings and promotes the application of prefabricated buildings in China.

Deep learning enables super-resolution hydrodynamic flooding process modeling under spatiotemporally varying rainstorms

Real-time information on flooding extent, severity, and duration is necessary for effective metropolitan flood emergency management. Existing pluvial flood analysis methods are unable to simulate real-time regional flooding processes under spatiotemporally varying rainstorms. This paper presents a deep learning-enabled super-resolution hydrodynamic flood analysis method to simulate the real-time pluvial flooding process over a large area under spatiotemporally varying rainstorms. Compared with existing flood downscaling techniques, which are limited to flow depth, the proposed method produces high-resolution flow depth and velocity predictions, providing more comprehensive information for flood emergency management. The proposed method adopts a coarse-grid hydrodynamic model to generate a low-resolution flood map time series, which is subsequently converted to high-resolution flood maps by a deep learning model. The deep learning model can be trained using a limited number of assumed rainfall scenarios, which greatly reduces data preparation effort. The proposed method is applied to a complex terrain of 352 km2 in Hong Kong that covers both mountainous and urban areas. Results show that the proposed method simulates the spatiotemporal variations of flood depth and velocity with root mean square errors as low as 0.082 m and 0.088 m/s, respectively, and correlation coefficients of 0.962 and 0.921, respectively. The computation time for a 48-h rainfall event in the study area is less than 30 s, which is 2690 times faster than the direct fine-grid hydrodynamic analysis. The deep learning-enabled super-resolution hydrodynamic flood analysis method provides a promising computational tool for emergency flood risk management.

Application of Wireless Sensor Network Computer Technology in Financial Management System

In the current enterprise competition, the core of the management information system is the financial management information system. Building a more comprehensive and efficient financial management information system can effectively establish a more efficient innovative enterprise and promote the transformation and development of the enterprise. This paper first designs a set of financial management system templates, including general ledger, accounts receivable, and accounts payable. Second, according to the characteristics of wireless sensor network technology, this paper proposes a financial management system based on the wireless sensor network and designs a load balancing topology control algorithm (LHTCA algorithm), which balances the energy consumption of network nodes and helps to improve the network life cycle. Finally, the management system is tested and validated. The test results show that the financial management system can realize the electronicization of financial data and paperless vouchers, improve the efficiency of financial work, and promote the sharing and integration of resources.

A deep learning method for intelligent decision-making in enterprise management based on the Internet of Things

With the development and progress of various sensors, the Internet of Things technology has continued to penetrate into people’s lives. Successful application cases of the Internet of Things technology have been seen in various industries. Some scholars have begun to try to apply the Internet of Things technology to the management of various industries. Deep learning technology belongs to the category of artificial intelligence technology. Its main goal is to combine a large number of training samples to train the trained object into an object that can learn sample patterns independently. At present, deep learning has a large number of applications in various intelligent operations and key intelligent recognition fields. Due to the characteristics of its learnable training set, deep learning techniques are often used in the direction of decision analysis. The research adopts a combination of questionnaire survey and modeling analysis to explore the construction of in-depth learning methods for enterprise management intelligent decision-making in the context of the Internet of Things. The research was conducted by telephone interview and online questionnaire, and was supported by nearly 100 technical backbone personnel from 25 large enterprises. The research finds that applying the Internet of Things and deep learning technology to enterprise intelligent management decisions can improve the decision-making accuracy of enterprises, and also provide enterprises with more significant decision-making analysis advantages in the industry competition. This paper first describes the concepts of the Internet of Things, deep learning and intelligent decision-making of enterprise management. Then, in order to sort out the problems existing in the current intelligent decision-making of enterprise management, the author conducted interviews and questionnaires. Through the analysis of the questionnaire data, he found the problems existing in the current intelligent decision-making of enterprise management. Finally, he designed a new strategy of intelligent decision-making of enterprise management based on the Internet of Things and deep learning technology, through the simulation and subsequent investigation and analysis of the newly designed strategy. It is found that the enterprise management intelligent decision-making method designed by combining the Internet of Things and deep learning technology is superior to the traditional enterprise decision-making method in all aspects. The design of the system has been reformed mainly from the aspects of human resource management, digital file management and real-time monitoring of production and operation, so that the popularity of the enterprise’s comprehensive information management has reached 84.2%, the efficiency of intelligent decision-making increased by 28.7% on average. The achievements of the research not only prove the role of IoT technology in promoting enterprises’ in-depth learning, but also put forward new ideas for enterprises’ scientific decision-making.

Excess mortality and associated factors among people living with HIV initiating highly active antiretroviral therapy in Luzhou, China 2006–2020

BackgroundTo estimate crude mortality, excess mortality, and standardized mortality rates (SMR) among people living with HIV (PLHIV) initiating highly active antiretroviral therapy (HAART) in Luzhou, China 2006–2020, and assess associated factors.MethodsPLHIV initiating HAART in the HIV/AIDS Comprehensive Response Information Management System (CRIMS) in Luzhou, China 2006–2020 were included in the retrospective cohort study. The crude mortality, excess mortality, and SMR were estimated. Multivariable Poisson regression model was used for analyzing risk factors associated with excess mortality rates.ResultsThe median age among 11,468 PLHIV initiating HAART was 54.5 years (IQR:43.1–65.2). The excess mortality rate decreased from 1.8 deaths/100 person-years (95% confidence interval [CI]:1.4–2.4) in 2006–2011 to 0.8 deaths/100 person-years (95%CI:0.7–0.9) in 2016–2020. SMR decreased from 5.4 deaths/100 person-years (95%CI:4.3–6.8) to 1.7 deaths/100 person-years (95%CI:1.5–1.8). Males had greater excess mortality with the eHR of 1.6 (95%CI:1.2–2.1) than females. PLHIV with CD4 counts ≥ 500 cells/μL had the eHR of 0.3 (95%CI:0.2–0.5) in comparison to those with CD4 counts < 200 cells/μL. PLHIV with WHO clinical stages III/IV had greater excess mortality with the eHR of 1.4 (95%CI:1.1–1.8). PLHIV with time from diagnosis to HAART initiation ≤ 3 months had the eHR of 0.7 (95%CI:0.5–0.9) compared to those with time ≥ 12 months. PLHIV with initial HAART regimens unchanged and viral suppression had the eHR of 1.9 (95%CI:1.4–2.6) and 0.1 (95%CI:0.0–0.1), respectively.ConclusionsThe excess mortality and SMR among PLHIV initiating HAART in Luzhou, China decreased substantially from 2006 to 2020, but the mortality rate among PLHIV was still higher than general population. PLHIV who were male, with baseline CD4 counts less than 200 cells/μL, WHO clinical stages III/IV, time from diagnosis to HAART initiation ≥ 12 months, initial HAART regimens unchanged, and virological failure had a greater risk of excess deaths. Early and efficient HAART would be significant in reducing excess mortality among PLHIV.

Peer-Led Community-Based Support Services and HIV Treatment Outcomes Among People Living With HIV in Wuxi, China: Propensity Score-Matched Analysis of Surveillance Data From 2006 to 2021.

Community-based organizations deliver peer-led support services to people living with HIV. Systematic reviews have found that peer-led community-based support services can improve HIV treatment outcomes; however, few studies have been implemented to evaluate its impact on mortality using long-term follow-up data. We aimed to evaluate the associations between the receipt of peer-led community-based support services and HIV treatment outcomes and survival among people living with HIV in Wuxi, China. We performed a propensity score-matched retrospective cohort study using data collected from the Chinese National HIV/AIDS Comprehensive Information Management System for people living with HIV in Wuxi, China, between 2006 and 2021. People living with HIV who received adjunctive peer-led community-based support for at least 6 months from a local community-based organization (exposure group) were matched to people living with HIV who only received routine clinic-based HIV care (control group). We compared the differences in HIV treatment outcomes and survival between these 2 groups using Kaplan-Meier curves. We used competing risk and Cox proportional hazards models to assess correlates of AIDS-related mortality (ARM) and all-cause mortality. We reported adjusted subdistribution hazard ratio and adjusted hazard ratio with 95% CIs. A total of 860 people living with HIV were included (430 in the exposure group and 430 in the control group). The exposure group was more likely to adhere to antiretroviral therapy (ART; 396/430, 92.1% vs 360/430, 83.7%; P<.001), remain retained in care 12 months after ART initiation (402/430, 93.5% vs 327/430, 76.1%; P<.001), and achieve viral suppression 9 to 24 months after ART initiation (357/381, 93.7% vs 217/243, 89.3%; P=.048) than the control group. The exposure group had significantly lower ARM (1.8 vs 7.0 per 1000 person-years; P=.01) and all-cause mortality (2.3 vs 9.3 per 1000 person-years; P=.002) and significantly higher cumulative survival rates (P=.003). The exposure group had a 72% reduction in ARM (adjusted subdistribution hazard ratio 0.28, 95% CI 0.09-0.95) and a 70% reduction in all-cause mortality (adjusted hazard ratio 0.30, 95% CI 0.11-0.82). The nonrandomized retrospective nature of our analysis prevents us from determining whether peer-led community-based support caused the observed differences in HIV treatment outcomes and survival between the exposure and control groups. The receipt of peer-led community-based support services correlated with significantly improved HIV treatment outcomes and survival among people living with HIV in a middle-income country in Asia. The 15-year follow-up period in this study allowed us to identify associations with survival not previously reported in the literature. Future interventional trials are needed to confirm these findings.
