346 Health literacy and the ability to understand postoperative instructions in patients undergoing Mohs micrographic surgery

Health literacy refers to an individual’s ability to navigate the healthcare system and make health-related decisions. Numerous studies link low health literacy with poor outcomes in chronic diseases; however, few studies investigate the impact of health literacy in a surgical setting. Appropriate wound care and pain medication use are imperative for safe recovery after Mohs surgery. Because of this, it is of interest to explore the relationship between health literacy and comprehension of medical instructions in Mohs patients.

A Physician's Perspective on the Incorporation of Pictograms as a Supplement to Medical Instructions in Chile: A Pilot Study

Introduction and objectivesCurrent models of health care are progressively migrating to more participatory models, where for treatment to achieve results that last over time, there must be effective communication between the patient, his or her caregivers, and health professionals. Although the use of pictograms in the context of medical instructions has been widely studied, in our country, there are no studies about their usefulness, or which set of symbols should be used by the systems. This work aims to present the first step towards the development of platforms that automatically suggest pictograms to supplement medical instructions for primary care settings in Chile. Materials and methodsIn this pilot study, we collected and analyzed the physicians' opinions on the selection of medical instructions that later will automatically be supplemented by the software that is under construction. We designed an expert validation survey using a set of 66 medical instructions with pictograms. This survey provided three rating options for each medical instruction: Not necessary (supplementing the instruction with a pictogram does not carry any value), Useful (supplementing the instruction with a pictogram may help patients to understand and remember the instruction), and Essential (supplementing the instruction with a pictogram is essential). Seventy-one physicians responded to the survey. Results22 out of 66 medical instructions were considered “essential” by >=51% of the experts, and 12 of the 66 were considered to be “useful but not essential” by >=51% of the experts. ConclusionResults of our survey validate the potential use of pictograms as a complement to better comprehension of medical instructions in our country.

Using pictograms to assist caregivers in liquid medication administration: a systematic review.

It has been reported that more than 80% of out-of-hospital medication errors among the young children involve liquid formulations. The usefulness of pictorial aids to improve communication of medication instructions has not been extensively investigated for child health. The objective of this study was to determine the effectiveness of pictorial aids used to assist caregivers in the administration of liquid medications. MEDLINE, CINAHL, PsycINFO, ScienceDirect, Scopus and the Cochrane Library were searched for articles published up to February 2015. Studies that used pictorial aids with liquid medications and measured at least one of the following outcomes were included: dosing accuracy, comprehension of medication instructions, recall of information and adherence of caregivers. Two authors independently selected studies, extracted data and assessed methodological quality of studies using the Cochrane Collaboration's tool. Five experimental studies (four hospital based and one community based) with a total of 962 participants were included. A wide range of liquid formulations were studied, including both prescription and over-the-counter medications. The existing findings suggest that pictographic interventions reduced dosing errors, enhanced comprehension and recall of medication instructions and improved adherence of caregivers. Incorporating pictorial aids into verbal medication counselling or text-based instructions was more beneficial than using the single approach alone. Mixed results were identified for the relationship between health literacy of caregivers and effectiveness of pictorial aids. The evidence remains limited due to the small number of studies found and variations in methodological quality. This review suggests that pictorial aids might be potential interventions, but more high-quality studies are needed to support the routine use of any pictogram-based materials with liquid medications in the clinical settings.

Information about medication in HIV-infected patients and its relation to adherence

In HIV-infected patients, comprehension of medication instructions is an essential condition for adherence to Highly Active Antiretroviral Therapy (HAART). In this study, we used a self-reported questionnaire to know which sources of medication information HIV-infected patients used and their impact on adherence. In secondary objectives, we determined profiles of non-adherent patients and specified the role of the pharmacist. A cross-sectional, observational study was conducted in one community pharmacy and one French university hospital pharmacy, in HAART-naïve or not patients, from April to June 2009. During the 3-month study period, 233 HIV-infected patients were included. The majority of patients sought information about their HAART treatments from the hospital physician (79.8%), the community physician (74.2%), and patient information leaflets (73.8%). The community and hospital pharmacists were consulted by respectively 16.3% and 3.4% of patients. According to multivariate regression analysis, adherence seemed to be associated with the sources of information "community physician", "hospital physician", "internet", and the potential support of patient associations. A total of 65.7% of patients were considered to be adherent. In our study, among sources used by HIV-infected outpatients, their physicians are the most helpful sources of information about HAART. Regarding practice implications, the key role of the pharmacist is underutilised, indicating the need for improved communication between the pharmacist and outpatients.

Cognitive and psychiatric predictors of medical treatment adherence among older adults in primary care clinics

Medical treatment non-adherence among older adults is common and represents a significant public health care concern. Treatment non-adherence has been associated with a number of factors in older adults; however few studies have delineated the role of cognition and psychiatric status. Data were collected from 212 ethnically diverse older primary care patients as part of a larger study. Cognitive status was evaluated with the Mattis Dementia Rating scale (DRS). Psychiatric status was evaluated using the Geriatric Depression Scale (GDS) and the Beck Anxiety Inventory (BAI). Treatment adherence was assessed by the total number of missed healthcare appointments and by physician and patient ratings. Physician ratings of patients' understanding of medical instructions were also obtained. A series of multiple regression analyses were conducted to determine cognitive and psychiatric predictors for each measure of treatment adherence. GDS and DRS memory scores were both independent predictors of the total number of missed medical appointments, F(7,55) = 2.34, p = 0.038. GDS score was also shown to be a significant predictor of physician ratings of patients' understanding of medical instructions, F(7,33) = 0.89, p = 0.031. Neither cognitive performance nor psychiatric status was associated with patient or physician ratings of treatment adherence. Measures of cognitive functioning and depression severity were supported as predictors of objective measures of treatment adherence but they were not associated with physician or patient ratings of adherence. Patient depression may influence physician ratings of patients' comprehension of medical instructions.
