Effect of surface electrode recording area on compound muscle action potential scan processing for motor unit number estimation.

MScanFit is a model-based algorithm for motor unit number estimation (MUNE) from compound muscle action potential (CMAP) scan data. It is a clinically applicable tool because of its quick and automatic implementation. Electrodes with different recording areas were employed to record CMAP scan data in existing studies. However, the effect of electrode recording area on MScanFit MUNE and other CMAP scan parameters has not been studied. CMAP scan was performed on the abductor pollicis brevis muscle of both hands on 14 healthy subjects using three different electrodes with recording areas of 10 mm × 10 mm, 11 mm × 14 mm, and 22 mm × 26 mm, respectively. Motor unit number was estimated using MScanFit for each CMAP scan. Two motor unit number index parameters, i.e., D50 and step index (STEPIX), were also derived from the CMAP scan data. No significant difference in D50, STEPIX, and MScanFit MUNE was observed across three different electrode recording areas, although the amplitude of CMAP decreased significantly when a larger electrode was used. Intraclass correlation coefficients of 0.792 and 0.782 were obtained for MScanFit MUNE and STEPIX, respectively. Compared with CMAP amplitude, D50, STEPIX, and MScanFit MUNE are less sensitive to variation in electrode recording area. However, the repeatability of MScanFit MUNE could be compromised by the inconsistency in the electrode recording area.

Electrophysiological assessment of radial shock wave therapy for carpal tunnel syndrome.

This study presents an electrophysiological assessment of radial extracorporeal shock wave therapy on patients with carpal tunnel syndrome (CTS). Sixteen CTS subjects received radial extracorporeal shock wave therapy once a week for five consecutive weeks. Outcome performance was assessed using the Boston Carpal Tunnel Questionnaire (BCTQ) and electrodiagnostic measurements including a nerve conduction study of the median nerve and a compound muscle action potential (CMAP) scan of the abductor pollicis brevis muscle. The BCTQ and the sensory conduction test measurements were all statistically improved after the treatment. However, the motor conduction test measurements were not significantly different before and after the treatment. The CMAP scan examination revealed MScanFit motor unit number estimation (MUNE) was significantly higher after the treatment, while no significant change was found in StairFit MUNE and step index. These results confirmed the effectiveness of shock wave therapy for treating CTS symptoms and the associated sensory property changes. The reasons for the inconsistencies from different CMAP scan processing methods are worthwhile targets for further investigation.

Sensitivity Analysis of CMAP Scan Step Index to Different Stimulation Parameters and Examination of Muscles Affected by Spinal Cord Injury.

Step index (STEPIX) is a recently developed compound muscle action potential (CMAP) scan method for evaluating motor unit loss and remodeling changes. This study investigates the influence of different stimulation parameters during CMAP scan on STEPIX and its examination of muscles affected by spinal cord injury (SCI). CMAP scan of the first dorsal interosseous (FDI) muscle was performed using different stimulus pulse widths (0.1 ms, 0.2 ms) and different numbers of stimuli (500, 1000) in 12 neurologically intact subjects. STEPIX was derived from each CMAP scan of all subjects. A significantly higher STEPIX was obtained using 1000 stimuli than 500 stimuli, while no significant difference in STEPIX was observed using 0.1 and 0.2 ms stimulus pulse widths. STEPIX was further applied to process CMAP scans of the FDI muscle from 13 tetraplegia and 13 healthy control subjects using the same stimulation parameter setting (0.1 ms, 500 stimuli), along with other methods including MScanFit motor unit number estimation (MUNE) and D50. STEPIX was significantly lower for the SCI subjects compared with the healthy control subjects. STEPIX was significantly correlated with MscanFit MUNE and D50, but had a smaller relative width of the overlapping zone (WOZ%) between tetraplegic and healthy control groups compared with MScanFit MUNE and D50. The findings of the study highlight the importance of maintaining a consistent stimulation parameter setting in CMAP scan studies and confirm the usefulness of STEPIX as a convenient CMAP scan parameter for examination of motor unit number changes.

Simulating progressive motor neuron degeneration and collateral reinnervation in motor neuron diseases using a dynamic muscle model based on human single motor unit recordings

Objective. To simulate progressive motor neuron loss and collateral reinnervation in motor neuron diseases (MNDs) by developing a dynamic muscle model based on human single motor unit (MU) surface-electromyography (EMG) recordings. Approach. Single MU potentials recorded with high-density surface-EMG from thenar muscles formed the basic building blocks of the model. From the baseline MU pool innervating a muscle, progressive MU loss was simulated by removal of MUs, one-by-one. These removed MUs underwent collateral reinnervation with scenarios varying from 0% to 100%. These scenarios were based on a geometric variable, reflecting the overlap in MU territories using the spatiotemporal profiles of single MUs and a variable reflecting the efficacy of the reinnervation process. For validation, we tailored the model to generate compound muscle action potential (CMAP) scans, which is a promising surface-EMG method for monitoring MND patients. Selected scenarios for reinnervation that matched observed MU enlargements were used to validate the model by comparing markers (including the maximum CMAP and a motor unit number estimate (MUNE)) derived from simulated and recorded CMAP scans in a cohort of 49 MND patients and 22 age-matched healthy controls. Main results. The maximum CMAP at baseline was 8.3 mV (5th–95th percentile: 4.6 mV–11.8 mV). Phase cancellation caused an amplitude drop of 38.9% (5th–95th percentile, 33.0%–45.7%). To match observations, the geometric variable had to be set at 40% and the efficacy variable at 60%–70%. The Δ maximum CMAP between recorded and simulated CMAP scans as a function of fitted MUNE was −0.4 mV (5th–95th percentile = −4.0 – +2.4 mV). Significance. The dynamic muscle model could be used as a platform to train personnel in applying surface-EMG methods prior to their use in clinical care and trials. Moreover, the model may pave the way to compare biomarkers more efficiently, without directly posing unnecessary burden on patients.

Diagnostic accuracy of nerve excitability and compound muscle action potential scan derived biomarkers in amyotrophic lateral sclerosis.

The lack of reliable early biomarkers still causes substantial diagnostic delays in amyotrophic lateral sclerosis (ALS). The aim was to assess the diagnostic accuracy of a novel electrophysiological protocol in patients with suspected motor neuron disease (MND). Consecutive patients with suspected MND were prospectively recruited at our tertiary referral centre for MND in Utrecht, The Netherlands. Procedures were performed in accordance with the Standards for Reporting of Diagnostic Accuracy. In addition to the standard diagnostic workup, an electrophysiological protocol of compound muscle action potential (CMAP) scans and nerve excitability tests was performed on patients' thenar muscles. The combined diagnostic yield of nerve excitability and CMAP scan based motor unit number estimation was compared to the Awaji and Gold Coast criteria and their added value was determined. In all, 153 ALS or progressive muscular atrophy patients, 63 disease controls and 43 healthy controls were included. Our electrophysiological protocol had high diagnostic accuracy (area under the curve [AUC] 0.85, 95% confidence interval [95% CI] 0.80-0.90), even in muscles with undetectable axon loss (AUC 0.78, 95% CI 0.70-0.85) and in bulbar-onset patients (AUC 0.85, 95% CI 0.73-0.95). Twenty-four of 33 (73%) ALS patients who could not be diagnosed during the same visit were correctly identified, as well as 8/13 (62%) ALS patients not meeting the Gold Coast criteria and 49/59 (83%) ALS patients not meeting the Awaji criteria during this first visit. Our practical and non-invasive electrophysiological protocol may improve early diagnosis in clinically challenging patients with suspected ALS. Routine incorporation may boost early diagnosis, enhance patient selection and generate baseline measures for clinical trials.

Compound muscle action potential (CMAP) scan examination of paretic and contralateral muscles reveals motor unit alterations after stroke

This study presents a novel compound muscle action potential (CMAP) examination of motor unit changes in paretic muscle post stroke. CMAP scan of the first dorsal interosseous (FDI) muscle was performed bilaterally in 16 chronic stroke subjects. Various parameters were derived from the CMAP scan to examine paretic muscle changes, including CMAP amplitude, D50, step index (STEPIX) and amplitude index (AMPIX). A significant decrease in CMAP amplitude and STEPIX was observed in paretic muscles compared with contralateral muscles (CMAP amplitude: paretic (9.0±0.5) mV, contralateral (11.3±0.9) mV, P=0.024; STEPIX: paretic 101.2±7.6, contralateral 121.9±6.5, P=0.020). No significant difference in D50 and AMPIX was observed between the paretic and contralateral sides (P>0.05). The findings revealed complex paretic muscle changes including motor unit degeneration, muscle fiber denervation, reinnervation and atrophy, providing useful insights to help understand neuromuscular mechanisms associated with weakness and other functional deterioration post stroke. The CMAP scan experimental protocols and the applied processing methods are noninvasive, convenient, and automated, offering practical benefits for clinical application.

Estimating motor unit numbers from a CMAP scan: Repeatability study on three muscles at 15 centres

ObjectiveTo assess the repeatability and suitability for multicentre studies of MScanFit motor unit number estimation (MUNE), which involves modelling compound muscle action potential (CMAP) scans. MethodsFifteen groups in 9 countries recorded CMAP scans twice, 1–2 weeks apart in healthy subjects from abductor pollicis brevis (APB), abductor digiti minimi (ADM) and tibialis anterior (TA) muscles. The original MScanFit program (MScanFit-1) was compared with a revised version (MScanFit-2), designed to accommodate different muscles and recording conditions by setting the minimal motor unit size as a function of maximum CMAP. ResultsComplete sets of 6 recordings were obtained from 148 subjects. CMAP amplitudes differed significantly between centres for all muscles, and the same was true for MScanFit-1 MUNE. With MScanFit-2, MUNE differed less between centres but remained significantly different for APB. Coefficients of variation between repeats were 18.0% for ADM, 16.8% for APB, and 12.1% for TA. ConclusionsIt is recommended for multicentre studies to use MScanFit-2 for analysis. TA provided the least variable MUNE values between subjects and the most repeatable within subjects. SignificanceMScanFit was primarily devised to model the discontinuities in CMAP scans in patients and is less suitable for healthy subjects with smooth scans.

Longitudinal prospective cohort study to assess peripheral motor function with extensive electrophysiological techniques in patients with Spinal Muscular Atrophy (SMA): the SMA Motor Map protocol

BackgroundHereditary spinal muscular atrophy (SMA) is a motor neuron disorder with a wide range in severity in children and adults. Two therapies that alter splicing of the Survival Motor Neuron 2 (SMN2) gene, i.e. nusinersen and risdiplam, improve motor function in SMA, but treatment effects vary. Experimental studies indicate that motor unit dysfunction encompasses multiple features, including abnormal function of the motor neuron, axon, neuromuscular junction and muscle fibres. The relative contributions of dysfunction of different parts of the motor unit to the clinical phenotype are unknown. Predictive biomarkers for clinical efficacy are currently lacking. The goals of this project are to study the association of electrophysiological abnormalities of the peripheral motor system in relation to 1) SMA clinical phenotypes and 2) treatment response in patients treated with SMN2-splicing modifiers (nusinersen or risdiplam).MethodsWe designed an investigator-initiated, monocentre, longitudinal cohort study using electrophysiological techniques (‘the SMA Motor Map’) in Dutch children (≥ 12 years) and adults with SMA types 1–4. The protocol includes the compound muscle action potential scan, nerve excitability testing and repetitive nerve stimulation test, executed unilaterally at the median nerve. Part one cross-sectionally assesses the association of electrophysiological abnormalities in relation to SMA clinical phenotypes in treatment-naïve patients. Part two investigates the predictive value of electrophysiological changes at two-months treatment for a positive clinical motor response after one-year treatment with SMN2-splicing modifiers. We will include 100 patients in each part of the study.DiscussionThis study will provide important information on the pathophysiology of the peripheral motor system of treatment-naïve patients with SMA through electrophysiological techniques. More importantly, the longitudinal analysis in patients on SMN2-splicing modifying therapies (i.e. nusinersen and risdiplam) intents to develop non-invasive electrophysiological biomarkers for treatment response in order to improve (individualized) treatment decisions.Trial registrationNL72562.041.20 (registered at https://www.toetsingonline.nl. 26–03-2020).

A Novel Analysis of CMAP Scans From Perspective of Information Theory: CMAP Distribution Index (CDIX).

The compound muscle action potential (CMAP) scan is a useful technique for examination of neuromuscular disorders. The objective of this study is to develop a novel analysis of CMAP scans from the perspective of information theory. A novel index parameter called CMAP distribution index (CDIX) was developed to characterize CMAP scan based on calculation of the information entropy. The performance of CDIX was evaluated using CMAP scan data from healthy control and spinal cord injury (SCI) subjects, and compared with D50 and MScanFit motor unit number estimation (MUNE). CDIX was significantly lower for the SCI subjects compared with the healthy control subjects (p < 0.001). A significant correlation ( R2 = 0.58, p < 0.001) was found between CDIX and MScanFit MUNE. Among all tested parameters (maximum CMAP, D50, MScanFit MUNE and CDIX), CDIX achieved the smallest relative width of the overlapping zone (WOZ%) between SCI and healthy control subjects. CDIX can be inferred as a useful index reflecting motor unit loss and muscle fiber reinnervation changes.

A Novel Analysis of Compound Muscle Action Potential Scan: Staircase Function Fitting and StairFit Motor Unit Number Estimation.

Compound muscle action potential (CMAP) scan provides a detailed stimulus-response curve for examination of neuromuscular disease. The objective of the study is to develop a novel CMAP scan analysis to extract motor unit number estimation (MUNE) and other physiological or diagnostic information. A staircase function was used as the basic mathematical model of the CMAP scan. An optimal staircase function fitting model was estimated for each given number of motor units, and the fitting model with the minimum number of motor units that meets a predefined error requirement was accepted. This yields MUNE as well as the spike amplitude and activation threshold of each motor unit that contributes to the CMAP scan. The significance of the staircase function fit was confirmed using simulated CMAP scans with different motor unit number (20, 50, 100 and 150) and baseline noise (1 µV, 5 µV and 10 µV) inputs, in terms of MUNE performance, repeatability, and the test-retest reliability. For experimental data, the average MUNE of the first dorsal interosseous muscle derived from the staircase function fitting was 57.5 ± 26.9 for the tested spinal cord injury subjects, which was significantly lower than 101.2 ± 16.9, derived from the control group (p < 0.001). The staircase function fitting provides an appropriate approach to CMAP scan processing, yielding MUNE and other useful parameters for examination of motor unit loss and muscle fiber reinnervation.

A single center report of MScanFit motor unit number estimation in five muscles of healthy subjects.

The objective of this study was to estimate the number of motor units in 5 muscles from healthy individuals using the MScanFit program based on compound muscle action potential (CMAP) scan recordings. The examined muscles included first dorsal interosseous (FDI), abductor pollicis brevis (APB), abductor digiti minimi (ADM), second lumbrical (SL), and abductor hallucis (AH). CMAP scans were recorded from a total of 24 healthy participants. Motor unit number estimation (MUNE) values were derived from the MScanFit program. The average MUNE was 136.1 ± 31.1 (mean ± standard deviation) for the FDI, 134.9 ± 37.4 for the APB, 127.3 ± 32.3 for the ADM, 39.6 ± 8.3 for the SL, and 143.9 ± 28.9 for the AH muscles. Findings of the study provide useful information of the MScanFit MUNE for the examined muscles of healthy subjects from a single center.

Feasibility and tolerability of multimodal peripheral electrophysiological techniques in a cohort of patients with spinal muscular atrophy

ObjectiveElectrophysiological techniques are emerging as an aid in identifying prognostic or therapeutic biomarkers in patients with spinal muscular atrophy (SMA), but electrophysiological assessments may be burdensome for patients. We, therefore, assessed feasibility and tolerability of multimodal peripheral non-invasive electrophysiological techniques in a cohort of patients with SMA. MethodsWe conducted a single center, longitudinal cohort study investigating the feasibility and tolerability of applying multimodal electrophysiological techniques to the median nerve unilaterally. Techniques consisted of the compound muscle action potential scan, motor nerve excitability tests, repetitive nerve stimulation and sensory nerve action potential. We assessed tolerability using the numeric rating scale (NRS), ranging from 0 (no pain) to 10 (worst possible pain), and defined the protocol to be tolerable if the NRS score ≤ 3. The protocol was considered feasible if it could be performed according to test and quality standards. ResultsWe included 71 patients with SMA types 1–4 (median 39 years; range 13–67) and 63 patients at follow-up. The protocol was feasible in 98% of patients and was well-tolerated in up to 90% of patients. Median NRS score was 2 (range 0–6 at baseline and range 0–4 at follow-up (p < 0.01)). None of the patients declined follow-up assessment. ConclusionsMultimodal, peripheral, non-invasive, electrophysiological techniques applied to the median nerve are feasible and well-tolerated in adolescents and adults with SMA types 1–4. SignificanceOur study supports the use of non-invasive multimodal electrophysiological assessments in adolescents and adults with SMA types 1–4.

MScanFit motor unit number estimation of abductor pollicis brevis: Findings from different experimental parameters.

MScanFit motor unit number estimation (MUNE) based on the recording of the compound muscle action potential (CMAP) scan has wide applications. This study evaluated the effect of different CMAP scan settings on MScanFit MUNE. CMAP scan of the abductor pollicis brevis (APB) muscle was performed in 10 healthy subjects at a United States (US) research center using different stimulus pulse widths (0.1, 0.2 ms) and total number of stimuli or steps (500, 1,000), and in 12 healthy subjects at a China research center using a 0.1 ms pulse width and 500 steps. MScanFit MUNE was derived using the default model parameters. A significantly higher MUNE was obtained using the shorter than longer pulse width; 84.70 ± 21.56 (500 steps) and 77.90 ± 27.62 (1,000 steps) at a pulse width of 0.1 ms vs. 67.60 ± 18.72 (500 steps) and 62.20 ± 15.82 (1,000 steps) at a pulse width of 0.2 ms (p < 0.05). However, MUNE was unrelated to the number of steps (500 vs. 1,000, p > 0.1). MUNE was significantly higher in persons studied in the China center (136.42 ± 32.46) than the US center (84.70 ± 21.56) despite each center using the same pulse widths and steps (p < 0.001). After excluding the ethnicity, age and experimenter factors, this significant difference is speculated to be partly related to different electrode size used in the two centers. The findings suggest that CMAP scan experimental parameters should remain consistent, so the MScanFit MUNE will not be compromised by non-physiological factors.

Feasibility and reliability of MScanFit motor unit number estimation in peroneus longus muscle.

Motor unit number estimation (MUNE) methods may be valuable to detect motor involvement earlier than compound muscle action potential (CMAP) amplitude. The most recent MUNE method, MScanFit, has been shown to have advantages compared with previously described methods. However, MScanFit has only been applied in a few lower extremity muscles. In this study we examined the feasibility and reliability of MScanFit in peroneus longus muscle. Twenty healthy controls (16 males and 4 females; mean age, 36.05 ± 2.58 years) were examined twice within a 1- to 2-week interval. Fibular nerve was stimulated at the knee and CMAP scans were recorded from peroneus longus muscle. From this, MScanFit MUNE and size parameters were calculated, as was the CMAP amplitude. The reliability was examined using coefficient of variation (CV) and intraclass correlation coefficient (ICC). MUNE was correlated with CMAP amplitude using linear regression analysis. The CV between sessions was higher for CMAP amplitude (11.63 ± 1.88%) than MScanFit MUNE (3.13 ± 0.78%). Among the size parameters, mean unit amplitude (μV) showed the lowest CV (11.46 ± 1.77%). Using ICC, CMAP amplitude exhibited good reliability (0.787), whereas that of MScanFit MUNE was excellent (0.902). Reliability was good for all size parameters. There was no significant correlation between MScanFit MUNE and CMAP amplitude (R=0.25, P > .05). MScanFit MUNE is feasible in the peroneus longus muscle, with high test-retest reliability in healthy subjects. Studies in patients are needed to examine the sensitivity of this muscle in disease.

Analysis of the compound muscle action potential scan: Step index (STEPIX) and amplitude index (AMPIX)

ObjectiveThe compound muscle action potential (CMAP) scan is useful to study motor unit (MU) loss. It is of interest to develop simple measurements of the scan. MethodsCMAP scan recordings were performed in the abductor pollicis brevis muscle of 20 control subjects and 26 patients with amyotrophic lateral sclerosis (ALS). They were analyzed using two new measurements called Step index (STEPIX) reflecting the number of steps, and Amplitude index (AMPIX) for amplitude of these steps. ResultsIn control subjects, STEPIX ranged from 71 to 172 while AMPIX was 78–158 µV. In ALS patients STEPIX was reduced and AMPIX was increased. The degree of change in STEPIX and AMPIX varied among patients reflecting the success or failure of reinnervation. Follow up studies in 9 muscles demonstrated reduced STEPIX and increased AMPIX despite minimal change in the CMAP. ConclusionsSTEPIX and AMPIX are deterministic measurements of the CMAP scan made using a spreadsheet program. STEPIX and AMPIX can be inferred as indices for the number of motor units and their size, and demonstrate the expected pattern in ALS patients. SignificanceThe new algorithm for CMAP scan analysis may be useful to study disease progression in patients with ALS.

Motor Unit Number Estimation of the Second Lumbrical Muscle in Human Hand.

The number of motor units of the lumbrical muscles in human hand has not been explored. The objective of this study was to fill this gap by estimating the number of motor units in the second lumbrical muscle. Compound muscle action potential scan of the second lumbrical muscle was performed in 12 healthy subjects, with 10 of them being tested on two separate occasions. Motor unit number estimation (MUNE) was derived from the MScanFit program. The average MUNE of the second lumbrical muscle was 41.6 ± 2.1 (mean ± standard error) from 12 subjects in the first test, and 42.0 ± 2.2 from 10 of the 12 subjects in the retest, demonstrating excellent measurement reliability. Findings of the study provide novel information about the motor unit number of the second lumbrical muscle in human hand. The relatively low motor unit number in the muscle can facilitate motor unit investigations, especially at high level muscle activation.

OC07: Monitoring disease progression with electrophysiological markers derived from compound muscle action potential scans in amyotrophic lateral sclerosis

To determine which electrophysiological markers derived from the compound muscle action potential (CMAP) scan are most sensitive to monitor disease progression in patients with amyotrophic lateral sclerosis (ALS), and whether they hold value for clinical trials. Methods: A multicenter collaboration was initiated between MND centers in Denmark, Turkey, Australia and the Netherlands to retrospectively assess longitudinal electrophysiological and ALS functional rating scale (ALSFRS-R) patterns. For each patient we determined the change over time in maximum CMAP (CMAPmax), D50 (number of largest discontinuities within CMAP scans), returners (number of increased CMAPs with decreasing stimulus currents), a motor unit number estimate (MUNE) and MU size properties (e.g. mean, largest unit). Linear mixed models were used to estimate variance components and population averages. Results were translated to required sample sizes for trials. Results: In total, 225 thenar CMAP scans from 65 patients were obtained. The ALSFRS-R decreased on average by 0.9 points/month (95% CI -1.2 to -0.7). MUNE, D50 and CMAPmax showed the largest decrease over time with -0.09, -0.09, and -0.05 standard deviations per month, respectively. In terms of sample size, the ALSFRS-R required 388 patients for a 6-month trial with visits every two months, whereas MUNE required 314 patients (-19.1%) to detect a 30% reduction in progression rate. Conclusion: The electrophysiological markers show considerable variability in their ability to monitor disease progression. MUNE showed to be the most suitable derivate. Further developments and standardization of the CMAP scan could refine its utility for ALS clinical trials.

HP51 : Motor unit integrity in multifocal motor neuropathy: A systematic evaluation with CMAP scans

To systematically study the yield of CMAP scan recordings in a large sample of MMN patients. Introduction: We identified motor unit (MU) integrity in a large cohort of patients with multifocal motor neuropathy (MMN). Methods: We performed compound muscle action potential (CMAP) scans in 40 MMN patients and 24 age-matched healthy controls. We derived the maximum CMAP amplitude (CMAPmax), a MU number estimate (MUNE) and the largest MU amplitude using MScanFit, stimulus current required to elicit 5% (S5), 50% (S50) and 95% (S95) of CMAPmax and relative ranges ([S95-S5]x100/S50). These MU characteristics were compared with clinical, laboratory and nerve conduction study (NCS) results. Results: CMAPmax and MUNE were reduced in MMN patients (both p<0.001). Largest MU amplitude as a percentage of CMAPmax was increased in MMN patients (p<0.001). Neither treatment duration nor presence of anti-GM1 antibodies were associated with MUNE. Relative range was larger in patients with confirmed presence of anti-GM1 antibodies than those without anti-GM1 antibodies (p=0.016) and controls (p<0.001). Largest MU amplitudes were larger in patients without anti-GM1 antibodies than patients with presence of anti-GM1 antibodies (p=0.037) and controls (p=0.044). Conclusion: We found that MU loss is common in MMN and accompanied by enlarged MUs. The presence of anti-GM1 antibodies is associated with an increase in relative range of MU thresholds and a reduction of largest MU amplitude, but not to MU loss. Our findings indicate that CMAP scan complements routine NCS, and may have potential for practical monitoring of treatment efficacy and disease progression.

Automatic detection of F waves and F-MUNE in two types of motor neuron diseases.

Motor unit number estimation by F waves (F-MUNE) is an uncommonly used MUNE technique. Our aim in this study was to assess the sensitivity of F-MUNE values elicited with newly developed software in motor neuron diseases. F waves were recorded by 300 submaximal stimuli from abductor digiti minimi and abductor pollicis brevis muscles of 35 patients with amyotrophic lateral sclerosis, 18 with previous poliomyelitis, and 20 controls. The software determined the surface motor unit action potentials (sMUAPs) and calculated the F-MUNE values. Compound muscle action potential scans were also recorded to obtain MScanFit. The sMUAP amplitudes were higher and F-MUNE values were lower in both muscles of the patients when compared with controls. F-MUNE values could distinguish patients from controls. Significant correlations were found between F-MUNE and MScanFit in the patient groups. The new F-MUNE software offered promising results in revealing motor unit loss caused by motor neuron diseases.
