Associations of plasma phospholipid cis-vaccenic acid with insulin resistance markers in non-diabetic men with hyperlipidemia

BackgroundThe role of fatty acids (FA) in the pathogenesis of insulin resistance and hyperlipidemia is a subject of intensive research. Several recent works have suggested cis-vaccenic acid (cVA) in plasma lipid compartments, especially in plasma phospholipids (PL) or erythrocyte membranes, could be associated with markers of insulin sensitivity and cardiovascular health. Nevertheless, not all the results of research work testify to these beneficial effects of cVA. Therefore, we decided to investigate the relations of proportion of cVA in plasma PL to markers of insulin resistance in hyperlipidemic men.SubjectsIn 231 men (median age 50) with newly diagnosed hyperlipidemia, we analyzed basic clinical parameters together with FA composition of plasma PL and stratified them according to the content of cVA into upper quartile (Q4) and lower quartile (Q1) groups. We examined also small control group of 50 healthy men.ResultsThe individuals in Q4 differed from Q1 by lower plasma insulin (p < 0.05), HOMA-IR values (p < 0.01), and apolipoprotein B concentrations (p < 0.001), but by the higher total level of nonesterified FA (p < 0.01). Both groups had similar age, anthropometrical, and other lipid parameters. In plasma PL, the Q4 group had lower content of the sum of n-6 polyunsaturated FA, due to decrease of γ-linolenic and dihomo-γ-linolenic acids, whereas the content of monounsaturated FA (mainly oleic and palmitoleic) was in Q4 higher.ConclusionsOur results support hypothesis that plasma PL cVA could be associated with insulin sensitivity in men with hyperlipidemia.

Lipidome Analysis in Brain and Peripheral Plasma Following Milk Fat Globule Membrane Supplementation in Rodents.

Milk fat globule membrane (MFGM) is an essential component of milk. Bovine MFGM (bMFGM) has been shown to support cognitive development and increase relative concentrations of serum phospholipids. This study investigates bioavailability of bMFGM components after oral administration in two preclinical models to explore whether dietary bMFGM induces parallel changes to plasma and brain lipidomes. Transgenic APOE*3.Leiden mice (n = 18 per group) and Sprague-Dawley rats (n = 12 per group) are fed bMFGM-enriched (MFGM+) or Control diet, followed by phospholipid profile-determination in plasma, hippocampus, and prefrontal cortex tissue by targeted mass spectrometry. Multivariate analysis of lipidomic profiles demonstrates a separation between MFGM+ and Control plasma across rodents. In plasma, sphingomyelins contributed the most to the separation of lipid patterns among both models, where three sphingomyelins (d18:1/14:0, d18:1/23:0, d18:1/23:1[9Z]) are consistently higher in the circulation of MFGM+ groups. A similar trend is observed in rat prefrontal cortex, although no significant separation of the brain lipidome is demonstrated. bMFGM-enriched diet alters plasma phospholipid composition in rodents, predominantly increasing sphingomyelin levels in the systemic circulation with similar, but non-significant, trends in central brain regions. These changes may contribute to the beneficial effects of bMFGM on neurodevelopment during early life.

The FADS1 Genotype Modifies Metabolic Responses to the Linoleic Acid and Alpha-linolenic Acid Containing Plant Oils-Genotype Based Randomized Trial FADSDIET2.

The article investigates the FADS1 rs174550 genotype interaction with dietary intakes of high linoleic acid (LA) and high alpha-linolenic acid (ALA) on the response of fatty acid composition of plasma phospholipids (PLs), and of markers of low-grade inflammation and glucose-insulin homeostasis. One-hundred thirty homozygotes men for FADS1 rs174550 SNP (TT and CC genotypes) were randomized to an 8-week intervention with either LA- or ALA-enriched diet (13 E% PUFA). The source of LA and ALA are 30-50mL of sunflower oil (SFO, 62-63% LA) and Camelina sativa oil (CSO, 30- are randomized to an 35% ALA), respectively. In the SFO arm, there is a significant genotype x diet interaction for the proportion of arachidonic acid in plasma phospholipids (p < 0.001), disposition index (DI30 ) (p = 0.039), and for serum high-sensitive c-reactive protein (hs-CRP, p = 0.029) after excluding the participants with hs-CRP concentration of >10mg L-1 and users of statins or anti-inflammatory therapy. In the CSO arm, there are significant genotype x diet interactions for n-3 polyunsaturated fatty acids, but not for the clinical characteristics. The FADS1 genotype modifies the response to high PUFA diets, especially to high-LA diet. These findings suggest that approaches considering FADS variation may be useful in personalized dietary counseling.

Oleic Acid - Health Benefits and Status in Plasma Phospholipids in the Serbian Population

Abstract Oleic acid has many beneficial effects on human health. One of the main dietary sources of oleic acid is olive oil. Non-Mediterranean European countries, including Serbia, have low habitual olive oil consumption, but other vegetable oils also contain different amounts of oleic acid. In infants and young children milk is the most important source of fatty acids, including oleic acid. Furthermore, fatty acid composition of plasma phospholipids reflects dietary intake and fatty acid metabolism. In patients with serious chronic diseases fatty acid status is altered independently on the intake. Here we reviewed status of oleic acid in healthy persons as well as in patients with different chronic diseases in Serbia.

Association between plasma phospholipid omega-3 polyunsaturated fatty acids and type 2 diabetes is sex dependent: The Hunter Community Study

Chronic inflammation drives the development of insulin resistance and type 2 diabetes. Long-chain omega-3 polyunsaturated fatty acids (LCn-3PUFA) eicosapentaenoic acid (EPA, c20:5n-3) and docosahexaenoic acid (DHA, c22:6n-3) may protect against type 2 diabetes development. The aim of this current study is to determine whether LCn-3PUFA status is associated with type 2 diabetes in the Hunter Community Study. Men and women aged 55-85 years were randomly selected from the electoral roll and invited to participate. Participants were included in the current study if they had plasma phospholipid fatty acid composition data available and diabetes status could be determined. LCn-3PUFA status was determined by fatty acid composition of plasma phospholipids (EPA+DHA, %,w/w). Diabetes was determined according to World Health Organisation criteria. Insulin was measured in n=251 participants and HOMA-IR calculated. In total, n=2092 (diabetes: n=249) participants were included. After adjusting for confounders of diabetes, LCn-3PUFA status was inversely associated with diabetes in overweight/obese females (OR [95%CI]: 0.90 [0.80, 1.00], p=0.045) but not males (p-interactionsex=0.041). Overweight/obese females with diabetes had significantly lower levels of DHA than those without diabetes (mean difference [95%CI]:-0.53 [-0.87,-0.20], p=0.002), with no difference in EPA. LCn-3PUFA was inversely associated with HOMA-IR (r=-0.175, p=0.005). This study provides further evidence of a sex-dependent association between LCn-3PUFA and type 2 diabetes. Causal pathways between LCn-3PUFA and type 2 diabetes merits delineation.

Blood Plasma Phospholipids and Cholesterol during Hibernation of the Long-Tailed Ground Squirrel

Seasonal changes in the levels of phospholipids, diglycerides, cholesterol, and total protein in the blood plasma were investigated during hibernation of the long-tailed ground squirrel Spermophilus undulatus. During the winter period, the levels of phosphatidylcholine, phosphatidylinositol, phosphatidylethanolamine, lysophosphatidylcholine, and sphingomyelin phospholipids (per 1 mg of plasma protein) were increased in both torpid and active ground squirrels by 70–80, 50, 600–700, 70, and 150–200%, respectively; the level of phosphatidylserine did not change in comparison to the summer period. The plasma phospholipid composition differed between hibernating and active summer animals: in winter, the phosphatidylcholine mol % decreased by 20%, phosphatidylinositol and phosphatidylethanolamine increased by 3–4 times, and the phosphatidylserine mol % decreased by 50%, while sphingomyelin and lysophosphatidylcholine did not change in comparison to summer animals. In hibernating ground squirrels, the plasma cholesterol levels increased by two times, the diglyceride content diminished by 60%, and the level of protein (in milligrams per 1 mL plasma) increased by 20%. The simultaneous increase in the levels of cholesterol and total phospholipids, as well as the pronounced specific changes in the levels of individual phospholipids in the blood plasma of hibernating ground squirrels, indicate the involvement of plasma lipoprotein lipids in the molecular mechanisms of adaptation to natural hypobiosis in mammals and a possible role of these mechanisms in systemic reactions to damaging factors.

The study of the membrane-protective potential of the combination of 2-ethyl-6-methyl-3-hydroxypyridine-succinate and citicoline

To assess the changes in the composition of plasma phospholipids in patients with chronic cerebrovascular disease treated with neuroprotectors 2-ethyl-6-methyl-3-hydroxypyridine succinate (neurox) and citicoline (neipilept), the natural metabolites involved in biochemical processes in the body, and their composition. The study included 40 patients, 18 men and 22 women, aged from 54 to 72 years, with chronic cerebrovascular disease at the decompensation stage complicated with the hypertensive crisis and/or arrhythmia. During extraction of phospholipids from blood cells, a significant decrease in the amount of total lipids was found to the end of treatment of patients who received neurox or neipilept or their combination. The study of quantitative composition of phospholipids showed no significant changes in patients treated with neurox, while the use of citicoline or combination of citicoline with 2-ethyl-6-methyl-3-hydroxypyridine succinate resulted in the increase of their total mass. There were no significant changes in the qualitative composition of phospholipid classes in blood plasma in patients treated with neurox. In patients treated with neipilept or with the combination of citicoline with 2-ethyl-6-methyl-3-hydroxypyridine succinate, plasma phosphatidylcholine was significantly increased. No significant changes in the content of phosphatidylinositol, phosphatidylserine and sphingomyelin were observed.

LIPID METABOLITE PLASMA LEVELS AS BIOMARKERS OF EARLY-ONSET ALZHEIMER DISEASE

In recent years, in addition to the post-mortem diagnosis established practice, the demand of peripheral biomarkers of Alzheimer's disease (AD) has become a major focus of research due to the low cost and facility of access. The lipids and related molecules comprise 60% of the nonaqueous portion of the brain and are major constituents of membrane of neuronal and glial cells. Lipids play an increasingly recognized role in the neuronal function and plasticity of the brain. The lipid composition of brain substantially influences perception, mood and behavior. Since lipid metabolism is altered differently in neuropsychiatric diseases. Alterations in the membrane lipid profile can allow discrimination between patients with AD and MCI, thus suggesting a specificity of the findings for these diseases. Plasma is a good matrix in the search for candidates for biomarkers because it is easy to obtain and interacts with all tissues, providing an integration between the central and peripheral nervous system. Determine changes in the composition of plasma phospholipids and other metabolites, such as acylcarnitines, may help to elucidate the pathophysiological mechanisms in brain and the search for candidate biomarkers. Our objective was to determine plasma levels of lipids of patients with AD, MCI and healthy controls and propose metabolites candidates of peripheral biomarker and cutoff values capable to differentiate each diagnosis. Plasma of 32 patients with AD, 20 MCI and 30 healthy controls were analyzed by mass spectrometry - Flow injection analysis, using AbsoluteIDQ p180® kit (Biocrates Life Sciences). Statistical analyzes were performed using a classification method - Classification And Regression Tree. A combination of three metabolites are able to differentiate the diagnostic groups: Dodecanedioylcarnitine (C12-DC), Dodecanoylcarnitine (C12) and Phosphatidylcholine aa C26:0 (PC aa C26:0). The sensitivities are 64% for healthy controls, 80% for MCI and 79.4% for AD, with accuracy of 74.7%. Our results suggest that alterations of some plasma phospholipids levels may be previous of AD clinical signals and these alterations can differentiating MCI patients from healthy controls. Since these alterations are already present in the early cognitive deficits, the levels of these phospholipids can be a potential biomarker for early onset AD.

A Single Nucleotide Polymorphism in the FADS1 Gene is Associated with Plasma Fatty Acid and Lipid Profiles and Might Explain Gender Difference in Body Fat Distribution

BackgroundGenotyping of the rs174547 polymorphism in the fatty acid desaturase 1 gene (FADS1) shows that it is associated with the FA composition of plasma phospholipids and lipid metabolic indices among several ethnic groups. However, this association requires further confirmation in the Chinese population, and little is known about the effect of polymorphisms in fatty acid-related genes on body fat distribution.MethodsAnthropometric measurements of 951 Chinese adults aged 18–79 were obtained and body fat distribution was estimated using dual-energy X-ray absorptiometry. The FA composition of plasma phospholipids was measured by gas chromatography. Multiple linear regression assessed whether the rs174547 genotype was associated with FA composition, body fat distribution, and metabolic traits in additive, dominant, and recessive models.ResultsThe rs174547 C minor allele was associated with a higher proportion of linoleic acid, lower arachidonic acid and docosahexaenoic acid, as well as lower delta-6-desaturase and delta-5-desaturase activity. Female C allele carriers had lower android fat percentages and lower levels of low-density lipoprotein-cholesterol, while male C allele carriers had lower gynoid fat percentages and higher triglyceride after adjusting for age, income, BMI, behavioral risk factors, and regional fat percentages.ConclusionAn association of FADS1 rs174547 with the FA composition of plasma phospholipids was identified among this Chinese adult population. The association with body fat distribution and lipid metabolic indices differed between men and women, which might explain sexual differences in body fat distribution and lipid metabolism.

Abstract P012: Associations Between Plasma Phospholipid and Cholesterol Ester Fatty Acids and Reproducibility During 10-year Follow-up of Elderly Individuals: A Population Based Study

Background: Fatty acid (FA) composition in different blood compartments is used as biomarkers of dietary fat quality, but it is unclear how FA compositions differ between compartments and which biomarker is most appropriate for epidemiological studies. Furthermore, as most cohorts use single FA measurements to reflect long-term intake, reproducibility data of plasma FAs are clearly warranted. Objective: To investigate inter-correlations of FA compositions in plasma phospholipids (PP) and cholesterol esters (CE), and to evaluate 10-y reproducibility of FA composition in plasma CE. Methods: Baseline FA composition of PP and CE was assessed in a population-based cohort of 70-year-old Swedes (n=940), and in a subsample (n=539), FA was additionally measured in CE during follow-up at ages 75 and 80 y. FA in PP and CE was measured by gas chromatography. Spearman’s rank correlation coefficients were calculated between proportions of individual FA measured in plasma PP and CE collected at age 70 y. Plasma CE compositions at ages 70, 75, and 80 y were utilized to assess 10-year reproducibility by calculating intraclass correlation coefficients (ICC) in one-way random-effect models. Results: Strong correlations between PP and CC were observed for most FA, with the only exceptions palmitic acid and stearic acid (Table). Fair to good reproducibility was observed for most FA, and was greatest for palmitoleic acid (ICC [95% CI] = 0.72 [0.69-0.76]) and all n-6 polyunsaturated FA (ICC=0.64-0.68). Poor reproducibility was only observed for α-linoleic acid, pentadecanoic acid, and stearic acid, with ICC (95% CI) 0.19 (0.14-0.25), 0.22 (0.17-0.28), and 0.23 (0.18-0.29), respectively. Conclusions: Overall, FA levels measured in plasma PP and CE correlates strongly, suggesting that FA composition in these compartments generally can be used interchangeably as biomarkers in epidemiological studies. Overall good reproducibility of FA levels in CE indicates that single a measurement can reliably estimate average levels over a 10-y period.

Gemfibrozil disrupts the metabolism of circulating lipids in bobwhite quails

The circulating lipids of birds play essential roles for egg production and as an energy source for flight and thermogenesis. How lipid-lowering pharmaceuticals geared to prevent heart disease in humans and that are routinely released in the environment affect their metabolism is unknown. This study assesses the impact of the popular drug gemfibrozil (GEM) on the plasma phospholipids (PL), neutral lipids (NL), and nonesterified fatty acids (NEFA) of bobwhite quails (Colinus virginianus). Results show that bird lipoproteins are rapidly altered by GEM, even at environmentally-relevant doses. After 4days of exposure, pharmacological amounts cause an 83% increase in circulating PL levels, a major decrease in average lipoprotein size measured as a 56% drop in the NL/PL ratio, and important changes in the fatty acid composition of PL and NEFA (increases in fatty acid unsaturation). The levels of PL carrying all individual fatty acids except arachidonate are strongly stimulated. The large decrease in bird lipoprotein size may reflect the effects seen in humans: lowering of LDL that can cause atherosclerosis and stimulation of HDL that promote cholesterol disposal. Lower (environmental) doses of GEM cause a reduction of %palmitate in all the plasma lipid fractions of quails, but particularly in the core triacylglycerol of lipoproteins (NL). No changes in mRNA levels of bird peroxisome proliferator-activated receptor (PPAR) could be demonstrated. The disrupting effects of GEM on circulating lipids reported here suggest that the pervasive presence of this drug in the environment could jeopardize reproduction and migratory behaviours in wild birds.

Plasma F2-isoprostane class VI isomers at 12–18 weeks of pregnancy are associated with later occurrence of preeclampsia

Preeclampsia (PE) has long been associated with early oxidative stress, although the symptoms occur later in pregnancy. We have hypothesized that the oxidative stress in PE, as characterized by the presence of F2-isoprostane (F2-isoP) isomers in late pregnancy, should already be present in plasma at the first regular visit of the obstetrical follow-up. There are 64 possible isomers of F2-isoPs derived from the oxidation of arachidonic acid (AA), but only one of these isomers has been investigated so far in PE, the classical 8-iso-PGF2α. Here, we have investigated two regioisomers of class III (8-iso-15(R)-PGF2α and 8-iso-PGF2α) and a mix of two isomers of class VI ((±)5-iPF2α-VI) in plasma samples collected prospectively at 12–18 weeks from normotensive controls (n = 60) and pregnant mothers who developed PE later in pregnancy (n = 33). The plasma samples were subjected to alkaline hydrolysis followed by liquid–liquid extraction to extract total F2-isoPs for later quantification by HPLC-MS/MS. The F2-isoPs were normalized to either plasma volume or polyunsaturated fatty acid (PUFA) levels measured by GC-FID in plasma phospholipids. Early in pregnancy, only the class VI F2-isoP isomers were found at concentrations significantly higher in women developing PE later in pregnancy (+13%; p = 0.0074). Normalization of F2-isoPs to their substrate, AA, or the omega-3 to omega-6 ratio improved the predictability of PE as determined by receiver operating characteristic (from area under the curve of 0.67 to 0.68 and 0.70 respectively). Interestingly, omega-3 fatty acids were 25% higher in the control group than in the PE group (P = 0.0225). Omega-6 PUFAs correlated with F2-Isop isomers only in cases of PE (r > 0.377; P >0.03, Spearman correlation). In sum, this study indicates that specific isomers of class VI are significant predictors of PE. This work also suggests that F2-isoP isomers are not all generated and eliminated to the same extent and are influenced by the PUFA composition of plasma phospholipids.

Effect of Sex Hormones on Plasma Phospholipid Fatty Acid Composition in Intact Rats and Rats With Bilaterally Occluded Carotid Arteries

The effects of 8-days treatment with 17alpha-estradiol (33.3 microg/kg) and progesterone (1.7 mg/kg) on plasma lipids and fatty acid composition of plasma phospholipids were examined in intact (INT) and bilaterally common carotid arteries occluded (BCO) male Wistar rats. Significant decrease of triglyceride level was found in BCO rats after the estradiol treatment. Both hormones elevated proportion of 18:1n-7 fatty acid in INT, but they failed to have such an effect in BCO. Estradiol increased 22:5n-3 and total n-3 polyunsaturated fatty acids (PUFA) in intact, and decreased 18:2n-6 in BCO rats. Significantly lower level of total n-3 was found in progesterone-treated than in estradiol-treated BCO rats. Given that n-3 PUFA have many beneficial effects on cell and tissue function, while n-6 PUFA have mostly the opposite effects, estradiol, rather than progesterone, was seen to improve plasma lipids and phospholipids FA profiles in INT and BCO animals. Estradiol significantly elevated the estimated activity of delta9-desaturases and progesterone of delta5-desaturase in BCO group, with no effects in INT rats.

Cardiometabolic risk factors are influenced by Stearoyl‐CoA Desaturase (SCD) −1 gene polymorphisms and n‐3 polyunsaturated fatty acid supplementation

To determine if single nucleotide polymorphisms (SNPs) in stearoyl-CoA desaturase (SCD)-1 gene that encodes a key enzyme for fatty acid metabolism are associated with the response of cardiometabolic risk factors to n-3 PUFA supplementation. Two hundred and ten subjects completed a 2-week run-in period followed by 6-week supplementation with 5 g of fish oil (1.9-2.2 g eicosapentaenoic acid and 1.1 g docosahexaenoic acid). Risk factors were measured pre and post n-3 supplementation. Fatty acid composition of plasma phospholipids was analyzed by GC and the desaturase indices SCD16 (16:1n-7/16:0) and SCD18 (18:1n-9/18:0) were calculated. Genotyping of eight SNPs of the SCD1 gene was performed. N-3 PUFA supplementation decreased plasma triglycerides, as well as SCD16 and SCD18 indices, but increased fasting plasma glucose concentrations. SNPs in SCD1-modified cardiometabolic risk factors pre and post n-3 PUFA supplementation: triglyceride (rs508384, p = 0.0086), IL6 (rs3071, p = 0.0485), C-reactive protein (rs3829160, p = 0.0489), and SCD18 indices (rs2234970, p = 0.0337). A significant interaction effect between the SNP and n-3 PUFA supplementation was also observed for fasting plasma glucose levels (rs508384, p = 0.0262). These results suggest that cardiometabolic risk factors are modulated by genetic variations in the SCD1 gene alone or in combination with n-3 PUFA supplementation.

Inverse association between 18-carbon trans fatty acids and intelligence quotients in smoking schizophrenia patients

This study aimed to investigate polyunsaturated (PUFA) and trans isomeric fatty acid status in schizophrenia patients. Fatty acid composition of plasma phospholipids (PL) and triacylglycerols (TG) was analyzed by gas chromatography in 29 schizophrenia patients and 15 healthy controls. We found no difference in PL n-3 fatty acid status between the two groups, while the values of 22:5n-6 were significantly higher in patients with schizophrenia than in controls. In TG, values of docosatrienoic acid (20:3n-3) and docosapentaenoic acid (20:5n-3) were significantly higher in schizophrenia patients than in controls. We found no difference in the trans fatty acid status between patients and controls. In smoking schizophrenia patients significant negative correlations were detected between Wechsler adult full-scale intelligence quotients and values of total trans fatty acids in PL lipids, whereas no such correlation was seen either in non-smoking schizophrenia patients, or in healthy controls. While data obtained in the present study fail to furnish evidence for n-3 PUFA supplementation to the diet of patients with schizophrenia, they indicate that in smoking schizophrenia patients high dietary exposure to trans fatty acids is associated with lower intelligence quotients.

Abdominal adiposity is associated with fatty acid desaturase activity in boys: Implications for C-reactive protein and insulin resistance

Fatty acid composition, which is altered in patients with abdominal obesity, is influenced not only by dietary intake but also by the desaturating enzymes stearoyl-CoA desaturase (SCD), delta-6 desaturase (D6D) and delta-5 desaturase (D5D). We investigated desaturase activities and their associations with metabolic risk factors, C-reactive protein levels (CRP) and insulin resistance in Japanese children. There were 237 school children in this study; 115 were boys. The fatty acid composition of plasma phospholipids was analyzed, and the following desaturase activities were estimated: SCD (16:1n-7/16:0 and 18:1n-9/18:0), D6D (20:3n-6/18:2n-6) and D5D (20:4n-6/20:3n-6). D6D and D5D activities, but not SCD activity, were significantly associated with triglyceride levels, high-density lipoprotein cholesterol levels and insulin resistance in both sexes, and with CRP levels in boys. In addition, increased abdominal adiposity was significantly associated with increased D6D activity, and decreased D5D activity and insulin resistance in both sexes, and with increased CRP levels in boys. The n-6 polyunsaturated fatty acid desaturation pathway may be associated with metabolic risk factors, insulin resistance and increased inflammation in children with abdominal obesity, especially in boys.

Plasma phospholipid fatty acid composition and estimated desaturase activity in heart failure patients with metabolic syndrome

Metabolic syndrome is one of the major factors to increase the incidence of heart failure. In our study, we compared plasma fatty acid compositions among heart failure patients with and without Metabolic syndrome. Fatty acid (FA) composition of plasma phospholipids was analyzed and the activities of desaturase were estimated as the ratio of substrate and product fatty acids in 85 stable heart failure patients. Fatty acid and estimated desaturase activities were further examined for their associations with Metabolic syndrome components. Heart failure patients with Metabolic syndrome showed significant changes in fatty acid composition in comparison to those without Metabolic syndrome, which had a decreased proportion of lauric acid (C12:0) and an increased proportion of dihomo-γ-linolenic acid (C20:3n-6). Also, estimated desaturase activities (D5D and D6D) were closely related to Metabolic syndrome condition among heart failure patients. The content of dihomo-γ-linolenic acid showed positive correlations with BMI, waist circumference, and plasma triglyceride levels. D6D were positively associated with plasma triglyceride levels, whereas D5D showed a negative correlation with plasma triglyceride levels and waist circumferences. The content of dihomo-γ-linolenic acid as well as estimated D6D and D5D were altered in heart failure patients with Metabolic syndrome.

N-3 fatty acid-enriched parenteral nutrition regimens in elective surgical and ICU patients: a meta-analysis

IntroductionPrevious studies and a meta-analysis in surgical patients indicate that supplementing parenteral nutrition regimens with n-3 polyunsaturated fatty acids (PUFAs), in particular eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), is associated with improved laboratory and clinical outcomes in the setting of hyper-inflammatory conditions. Refined or synthetic fish oils are commonly used as a source of EPA and DHA. The objective of the present meta-analysis was to evaluate n-3 PUFA-enriched parenteral nutrition regimens in elective surgical and intensive care unit (ICU) patients.MethodsMedline was searched for randomized controlled trials comparing n-3 PUFA-enriched lipid emulsions with standard non-enriched lipid emulsions (i.e. soybean oil, MCT/LCT or olive/soybean oil emulsions) in surgical and ICU patients receiving parenteral nutrition. Extracted data were pooled by means of both random and fixed effects models, and subgroup analyses were carried forward to compare findings in ICU versus non-ICU patients.ResultsA total of 23 studies (n = 1502 patients: n = 762 admitted to the ICU) were included. No statistically significant difference in mortality rate was found between patients receiving n-3 PUFA-enriched lipid emulsions and those receiving standard lipid emulsions (RR= 0.89; 0.59, 1.33), possibly reflecting a relatively low underlying mortality risk. However, n-3 PUFA-enriched emulsions are associated with a statistically and clinically significant reduction in the infection rate (RR =0.61; 0.45, 0.84) and the lengths of stay, both in the ICU (-1.92; -3.27, -0.58) and in hospital overall (-3.29; -5.13, -1.45). Other beneficial effects included reduced markers of inflammation, improved lung gas exchange, liver function, antioxidant status and fatty acid composition of plasma phospholipids, and a trend towards less impairment of kidney function.ConclusionsThese results confirm and extend previous findings, indicating that n-3 PUFAs-enriched parenteral nutrition regimens are safe and effective in reducing the infection rate and hospital/ICU stay in surgical and ICU patients.

Association of Changes in Body Fatness and Fatty Acid Composition of Plasma Phospholipids During Early Puberty in Japanese Children

Plasma fatty acid composition can change with age, reflecting diet and levels of desaturating enzymes such as stearoyl-CoA desaturase (SCD), delta-6 desaturase (D6D) and delta-5 desaturase (D5D), which contribute to the development of insulin resistance. This study analyzed longitudinal changes in fatty acid composition in Japanese children during early puberty and the association between changes in desaturase indices and changes in body fatness and insulin resistance. The study included 77 children (38 boys and 39 girls) aged 9.6±0.5 years. Relative weight (RW) and waist-to-height ratio (WHtR) were determined. The fatty acid composition of plasma phospholipids was analyzed by gas chromatography, and the desaturase indices were calculated: SCD (16:1n-7/16:0: SCD16 and 18:1n-9/18:0: SCD18), D6D (20:3n-6/18:2n-6) and D5D (20:4n-6/20:3n-6) in 2006 and 2009. Obese children showed higher dihomo-gamma linolenic acid (DGLA; 20:3n-6), a higher D6D index and lower D5D index than non-obese children. Longitudinal changes in fatty acid com-position were generally similar in both sexes. Increased D6D index and DGLA and decreased D5D index were significantly associated with increased WHtR in boys and girls. In addition, increased D6D index was associated with an increased homeostasis model of assessment ratio (HOMA-R) only in girls. The change in abdominal adiposity is a determinant of longitudinal changes in D6D and D5D indices and DGLA during early puberty.

Sex-specific differences in essential fatty acid metabolism

Sex hormones may influence the enzymatic synthesis of long-chain polyunsaturated fatty acids (LC-PUFAs), which may lead to sex-specific differences in LC-PUFA status. Isotope studies with U-(13)C α-linolenic acid (ALA) have shown a considerably higher conversion rate of ALA to n-3 (omega-3) LC-PUFAs in women than in men. A review of the literature generally suggested that there was a higher contribution of arachidonic acid (AA) and docosahexaenoic acid (DHA) in blood lipids in women than in men; however, sex-specific differences were not seen in every study. The fatty acid composition of plasma phospholipids was recently reported separately for a large group of women and men (n > 3000) living in 15 regions of Europe. The contributions of saturated and monounsaturated fatty acids were higher, whereas those of AA and DHA were lower in men than in women; however, sex explained only ≈ 2% of the variability of plasma phospholipid DHA values. Results reported from a limited number of randomized controlled trials of perinatal LC-PUFA supplementation have, on occasion, shown sex-specific differences in some outcomes; however, the heterogeneity both in the interventions and outcomes measured made it difficult to draw conclusions on the direction or the extent of the effects. Data summarized in the current review highlight the importance of planning a subgroup analysis by sex in perinatal LC-PUFA supplementation trials.
