Pleural Amebiasis without Hepar Involvement

Background: Pleuropulmonary amebiasis is the most common complication of amoebic liver abscess, occurring in 15% of patients with amoebic liver disease and in 1% of patients with amoebic dysentery. Most commonly occurs by direct extension of a right superior lobe hepatic abscess through the diaphragm to the right lower lobe of the lung with a mortality rate of 5-16%. Primary pleural amebiasis without liver involvement is rare.Case Presentation: Reported an 18-year-old male patient with pleural amebiasis from the results of parasitological examination of pleural fluid found Entamoeba histolytica. History, physical examination and support showed normal liver function. The patient has a history of poor sanitation with a dissertation of unhygienic habits. Patients without liver problems remain at risk for amoebiasis pleuraConclusion: Pleural amebiasis without liver involvement was a rare case. Further investigation was needed in patients with pleural amoebiasis to other organs, such as the liver and digestive tract. Diagnose pleural amebiasis should be the main focus in determining the etiology. Entamoeba histolytica may be involved in pleural inflammation and cause effusion.

Mediastinal extension of amoebic liver abscess: A case report on the rare thoracic complication of Entamoeba histolytica

Amoebic liver abscess is one frequently encountered intra-abdominal infection, caused by Entamoeba histolytica and has various abdominal and thoracic life-threatening complications. Herein, describe a case of a 30-year-old female, who suffered from multiple amoebic liver abscesses and presented with massive right-sided pleural effusion. She was managed with recommended antibiotics and pigtail catheter for liver abscess and an intercostal drainage tube for massive pleural effusion. However, postintercostal drainage, her chest X-ray demonstrated a right mediastinal shadow, which was confirmed as an intrathoracic extension of the hepatic liver abscess on computed tomography. An ultrasound-guided pigtail catheter was manipulated and placed at the site of thoracic communication. This led to clinical and radiological improvement, but unfortunately, she developed ventilator-acquired pneumonia and died due to her illness. The case represents an uncommon complication of amoebic liver abscess, which is rarely described and adds more knowledge on the thoracic complexities of this infectious disease.

Analysis of ruptured amoebic liver abscess

Introduction: Amoebic liver abscess (ALA) is the most commonly seen extra intestinal manifestation of Entamoeba infection.[1] Out of which, a rare complication of amoebic liver abscess is rupture of liver abscess. We report 43 cases of ruptured amoebic liver abscess coming with acute abdomen which were operated at Department of Surgery, Dhiraj general hospital, Vadodara from January 2020 to December 2021. Surgical treatment in these cases should be carried out in a timely fashion prior to the presentation of systemic repercussions or death. result: Out of all patients, 37 patients were male and 6 were female and 37 patients were chronic alcoholic. The mean age was 47 years. At presentation, most cases had clinical signs of generalized peritonitis. Ultrasonography for collection in peritoneal cavity showed moderate free fluid with internal echoes suggestive of pyoperitoneum. Out of which 09 patients had free air under the diaphragm. All 43 patients underwent resuscitation and then shifted for surgery. All underwent exploratory laparotomy for ruptured liver abscess. Intraoperatively, 05 patients had a single abscess in the right lobe, 06 had abscess in both lobes of liver and 33 had multiple abscesses in right lobe.

A RARE CASE OF COMPLICATED LIVER ABSCESS WITH MIDDLE HEPATIC VEIN THROMBOSIS: A CASE REPORT AND REVIEW OF LITERATURE

In developing countries Amoebic liver abscess is commonly encountered disease and it’s also the commonest extraintestinal manifestation of Entamoeba histolytica infection. Usual complication of Amoebic liver abscess arises due to collection of pus in various cavities, like in peritoneal cavity following perforation, in the pleural cavity which is known as empyema thoracis, and rarely it is complicated by life threatening conditions such as venous extension of the disease involving the hepatic veins and IVC, with only few cases reported. Here we describe a case of amoebic liver abscess extending across middle hepatic vein.

Secondary pleuropulmonary amoebiasis due to liver abscess rupture: A complication case report in low resource setting

BackgroundPleuropulmonary amoebiasis caused by complications of amoebic liver abscess (ALA) is rare.Case presentationA 23 years old male, presented with shortness of breath, cough with yellowish phlegm, right chest pain, fever, bulging stomach, yellow eyes, and swelling of both legs. Abdominal ultrasound and CT scan thorax and abdomen revealed right fluidopneumothorax and liver abscess. Serological testing leads to Entamoeba histolytica infection, which was treated with metronidazole but no significant improvement on empyema and abscess liver size. Surgery was performed after percutaneous aspiration drainage failed to evacuate the abscess. HE and PAS staining from surgical tissue showed Entamoeba hystolitica infection.DiscussionSerological testing and radiological examination will be more useful in the early detection of cases of Entamoeba hystolitica infection. Surgery may be considered when purulent drainage does not show improvement in the patient's condition.ConclusionALA complication that causes pulmonary empyema can be surgically treated if the pus cannot be drained.

The Combination of Amoebiasis with Helminthiasis and Maduromycosis Complicated by Secondary Amyloidosis: A Case Report

A rare case of secondary amyloidosis was found in a cadaver with amoebiasis and intrathoracic complications of amoebic liver abscess related with Helminthiasis and Maduromycosis, which proved the peculiarity of Amoebiasis to combine with polyparasitoses, namely Enterobiasis, strongyloidiasis and Wuchereriasis. The authors confirmed that the progression of secondary amyloidosis, most likely hepatopathic amyloidosis, can be accelerated by longstanding course of advanced, severe underlying illness with the development of amoebic complications, especially combined with Helminthiasis and Mycosis.

Amoebic Liver Abscess with Hepatic Artery Pseudoaneurysm: Successful Treatment by Interventional Radiology

Amoebic liver abscess is most common extra-intestinal presentation of amoebiasis. It is rarely complicated with vascular involvement including thrombosis of hepatic vein or IVC and pseudo-aneurysm of hepatic artery. We describe a case of hepatic artery pseudo-aneurysm as a complication of amoebic liver abscess treated with percutaneous embolization.

Amoebic liver abscess: presentation and complications

Amoebic liver abscess (ALA) is the most common inflammatory space occupying lesion of the liver. It has a highly variable presentation causing diagnostic difficulties. Untreated, complicated ALA has high morbidity and mortality. To study the various types of clinical presentation and complications of ALA in order to establish early diagnosis and prevent complications. A prospective study was conducted in the Department of Surgery of Calcutta National Medical College and Hospital from July 2005 to June 2008. All patients with a confirmed diagnosis of ALA during this period were included in the study. Seventy-two patients with 76 ALAs were studied over a 3-year period. The age ranged from 21 to 72 years (mean age being 43.64 years). There were 66 males (91.67% of the patients) and 6 females (8.33% of the patients). The (31-40 years) age group showed the highest incidence consisting of 40.28% of the cases. Pain was located most commonly in the right hypochondrium in 60 patients (83.33%). Fever was observed in 58 patients (80.56%). Solitary abscess cavity was present in 68 patients (94.44%). The right lobe alone accounted for 65 (85.53%) of the abscesses. The most common echo pattern encountered in ultrasonography was the echorich pattern in 54 (71.05%) abscesses. Forty-four patients (61.11%) gave history of alcohol consumption. Diagnosis was missed in 21 patients (29.17%), particularly in those with atypical presentations. The most common complication was pleuropulmonary in 24 patients (33.33%), followed by intraperitoneal rupture in 19 patients (26.39%). Other complications were jaundice (n = 11), ascites (n = 9), subhepatic effusion (n = 7), subphrenic abscess (n = 3) and intrapleural rupture (n = 2). Ultrasonography was useful in diagnosing ALA. Despite successful therapy 23 patients (31.94%) had residual abscess cavities on ultrasonography even after 6 months. In an endemic area a patient presenting with lower chest or upper abdominal pain along with tender hepatomegaly should raise the suspicion of ALA. Sonography remains an important tool in the diagnosis of ALA.

Amoebic liver abscess: sonographic patterns and complications in Enugu, Nigeria

This study evaluated the sonographic patterns and complications of amoebic liver abscess (ALA) in Enugu, Nigeria. Request forms, case notes, sonographic reports and images of all patients who had amoebic liver abscess diagnosed at Hansa Clinics Enugu and at the UNTH Enugu within a period of 8 months of 2001 were reviewed. Sonograms obtained were analyzed for size, shape, location, echocharacteristics and complications.

Hemoptysis demonstrated on Tc99m-sulfur colloid scanning: A rare complication of amoebic liver abscess

We report the finding of bleeding into right lung from a ruptured amoebic liver abscess. A 25-year-old male presented with pain in the right lower chest and cough associated with expectoration of chocolate-colored sputum. Radiographs of chest showed right-sided pleural effusion. Serial images of Tc99m-sulfur colloid study revealed progressive accumulation of radiotracer in the upper photopenic area extending into the right lung region suggestive of active bleeding from the abscess cavity.

Case report: Amoebic liver abscess complicated by a hepatoduodenal fistula

Amoebic liver abscess (ALA) is a common extra-intestinal presentation of amoebiasis caused by Entamoeba histolytica. The liver abscess may be complicated by rupture into adjacent structures. Common organs involved include thorax, peritoneum and pericardium. Rupture into the gastrointestinal tract is extremely rare. We report a patient who developed a hepatoduodenal fistula complicating an amoebic liver abscess. Suspicions were raised on finding air in the liver abscess on ultrasound scanning. Diagnosis was confirmed on a water-soluble (Gastrografin) swallow (Fig. 1 a,b). Complications of ALA are associated with a high morbidity and mortality and early diagnosis is important. To our knowledge only one previous case of a hepatoduodenal fistula complicating an ALA with radiological confirmation has been reported.

Intrathoracic complications of amoebic liver abscess.

Amoebiasis is a world-wide parasitic infestation but is more prevalent and virulent in warm countries. Of 33 patients with amoebic liver abscess (ALA) seen over a 5-year period, 24 had intrathoracic complications. Management consisted mainly of the administration of the antiamoebic agent metronidazole (Flagyl), repeated needle aspiration of ALA and surgical drainage when aspiration failed. Closed tube thoracostomy was the preferred management of amoebic empyema. Associated cerebral involvement in 4 cases improved with parenteral metronidazole. Pericardial amoebic abscess claimed 100% mortality in our series.
