Comparison of single and double incision leg fasciotomy in disaster settings—Experience from 2023 Türkiye earthquakes

IntroductionAlthough there are studies comparing methods for leg fasciotomy in compartment syndrome after fractures, choice of single or double fasciotomies in disasters was not investigated. The aim of this study was to compare the efficacy of single and double incision leg fasciotomy in the setting of disaster. MethodsPatients that have undergone fasciotomy after 2023 Kahramanmaraş earthquakes were retrospectively analyzed. The cases were separated into two groups as single incision and double incision according to the method of the first fasciotomy. The number of debridements after each fasciotomy, muscle group excisions, completion time of treatment, presence of amputation, the method of closure (primary closure or graft/flap) and positive results of wound cultures were analyzed and compared between two groups. Results62 legs of 52 patients (22 females, 30 males, age 36.9 ± 11.2 years) with compartment syndrome that have undergone fasciotomy after 2023 Kahramanmaraş earthquakes were included in the study. Single-incision group included 27 legs and double incision group included 35 legs. Amputation was needed in 15 patients (%24.2), six in single incision group and nine in double incision group. (p = 0.75). Compartment excision (eight patients in single incision, nine patients in double incision groups, p = 0.81), number of debridements (median 4 in both groups, p = 0.55), wound closure time (median 17 days in single incision, 22 days in double incision groups, p = 0.52), graft or flap requirement (11 patients in single incision, 16 patients in double incision groups, p = 0.53), positive culture results (15 patients in single incision, 16 patients in double incision groups, p = 0.44) were not different statistically between two groups. ConclusionSingle and double incision fasciotomy methods are equally effective and safe in treatment of compartment syndrome of the leg in disaster situations. To our knowledge, this is the first study comparing outcomes of single and double incision fasciotomy in disaster settings.

Does pes anserinus tenotomy affect surgical site infection rate in operative tibial plateau fractures?

To assess if pes anserinus tenotomy (PAT) during definitive open reduction and internal fixation (ORIF) of tibial plateau fractures is associated with a decreased risk of surgical site infection (SSI) and other postoperative complications. A retrospective review of all adults who underwent ORIF for tibial plateau fractures from April 2005 to February 2022 at single level 1 trauma center was performed. Patients who had a medial approach to the plateau with minimum three-month follow-up were required for inclusion. All patients with fasciotomy for compartment syndrome or with traumatically avulsed or damaged pes anserinus prior to ORIF were excluded. Two groups were created: those who received a pes anserinus tenotomy with repair (PAT group) and those whose pes anserinus were spared and left intact (control group). Patient demographics, injury and operative characteristics, and surgical outcomes were compared. The primary outcomes were rates of deep and superficial SSI. The PAT group had significantly lower rates of deep SSI (9.2% vs. 19.7%, P = 0.009), superficial SSI (14.2% vs. 26.5%), P = 0.007), and any SSI (15.8% vs. 28.9%, P = 0.005). Multiple logistic regression showed that heart failure (aOR = 7.215, 95% CI 2.291-22.719, P < 0.001), and presence of open fracture (aOR = 4.046, 95% CI 2.074-7.895, P < 0.001) were independently associated with increased odds of deep SSI, while PAT was associated with a decreased odds of deep SSI (aOR = 0.481, 95% CI 0.231-0.992, P = 0.048). PAT had significantly lower rates of unplanned return to the operating room (20.8% vs. 33.7%, P = 0.010) and implant removal (10.0% vs. 18.0%, P = 0.042). While these data do not allow for discussion of functional recovery or strength, pes anserinus tenotomy was independently associated with significantly lower rates of infection, unplanned operation, and implant removal. Fasciotomy for Compartment Syndrome

Compartment syndrome is an orthopedic emergency due to its potential complications and mortality. Many studies have discussed the supporting examination and closure of phasiotomy wounds related to compartment syndrome, but there is still no agreed reference standard for additional diagnostic tool and fasciotomy management. Surgeon's experience and preference are important in the management of compartment syndrome. Sindrom kompartemen merupakan salah satu kegawatdaruratan ortopedi karena potensi komplikasi dan risiko kematiannya. Banyak penelitian yang membahas mengenai pemeriksaan penunjang dan penutupan luka fasiotomi terkait sindrom kompartemen, namun belum ada referensi yang disetujui sebagai baku standar. Pengalaman dan preferensi dokter bedah terkait merupakan unsur terpenting dalam tata laksana sindrom kompartemen.

Snakebite envenomation in children: An ongoing burden in Morocco

IntroductionSnakebites are a leading cause of mortality and permanent disabilities especially among children in tropical countries and rural areas such as Morocco. Thus, a nationwide management protocol including specific antivenom therapy along with prevention strategies was implemented to reduce the overall snakebites morbimortality. Patients and methodsOur retrospective study aimed to describe the clinical aspects of snakebite envenomation before and after the implementation of this protocol in children admitted to the pediatric intensive care unit (PICU) in Marrakesh-Morocco for a period of 11 years. ResultsA total of 75 cases were included and were mostly male (70%) with a mean age of 10 years old. Most envenomations were mild or severe (75%) and often occurred during outdoor activities in limb extremities. Altered hemostasis frequently occurred in 67% of cases but was rarely associated with severe exteriorized hemorrhage. Moderate anemia and PNN- predominant leukocytosis were often observed at admission (52.2% and 58%) but quickly tended to normalize before 48 h. Local symptoms were the main dread as they quickly evolve to a compartment syndrome and necrosis in the absence of antivenom therapy. Fasciotomy was performed in 33% of cases while 5 children required limb amputation. Antivenom administration (n = 39) was statistically significant for rapid improvement in hemostasis disorders, reduced blood transfusions and fasciotomy for compartment syndrome as well as a shortened length of stay in PICU. The onset of acute kidney injury was observed in 18 cases but restored in most patients within 48 h (77%). Five children died of which only two had received delayed antivenom immunotherapy due to its unavailability and deferred hospital admission. ConclusionThe advent of specific serotherapy has made it possible to optimize the management of patients and to prevent and treat local and systemic complications thus improving the overall prognosis; nevertheless, primary prevention remains the key to reducing snakebites morbimortality.

721 Defining Bleeding Characteristics in Frostbite Patients Managed with tPA

IntroductionFrostbite is caused by exposure to cold temperatures and can be a severe injury leading to hospital admissions, surgeries, or amputations. Disease progression involves endothelial injury, thrombosis, and tissue necrosis; therefore, management of patients involves a process of rewarming and restoration of blood flow to the affected area. Tissue plasminogen activator (tPA) is a thrombolytic agent that has demonstrated efficacy at restoring tissue perfusion in patients with frostbite. The goal of frostbite management with tPA is to salvage tissue without causing clinically significant bleeding, a documented adverse effect of tPA administration. The purpose of this study was to characterize specific bleeding complications associated with tPA administration. The secondary objective was to compare the rate of bleeding complications in frostbite patients treated with intravenous (IV) tPA to frostbite patients that did not receive tPA.MethodsThis single center retrospective study included all adult patients with severe frostbite who presented between October 2013 and March 2020. tPA was given to patients per institutional protocol. To assess for bleeding events, patient charts were reviewed and any instance of bleeding was categorized based on severity. Bleeding was categorized as: 1) none, 2) mild: not clinically significant (bandage or moved IV site), 3) moderate: change of management (tPA stopped, enoxaparin held, or specialty consult), and 4) severe: included a change and intervention (transfusion, fasciotomy for compartment syndrome). Any change in management or any additional therapies used to control bleeding were documented, as well as the timing of bleeding in relation to tPA administration.ResultsOver a 7-year period 209 patients were analyzed and 202 patients were included. For patients with bleeding events requiring intervention, the mean time to bleed was 105.5 hours (range 4 to 576 hours). Of these, 4 (3 transfusions and 1 fasciotomy for compartment syndrome) were temporally associated with tPA administration (within 24 hours). Two of the 4 patients had minor to moderate traumatic injury prior to admission, the 3rd patient had incomplete work-up at referring center that initiated tPA prior to transport, and the 4th patient was in restraints. Of the patients who did not receive tPA, 3.39% had a severe bleeding event requiring intervention compared to 6.99% of patients treated with tPA (P=0.516).ConclusionsThough there was a higher incidence of bleeding in tPA-treated patients, for the majority of patients studied, tPA was safe for the treatment of severe frostbite. Bleeding events occur in frostbite patients treated with or without tPA and warrant close follow-up for these infrequent complications.

Case Report: Sustained mitochondrial damage in cardiomyocytes in patients with severe propofol infusion syndrome

Introduction: Propofol infusion syndrome (PRIS) is rare but a potentially lethal adverse event. The pathophysiologic mechanism is still unknown. Patient concerns: A 22-year-old man was admitted for the treatment of Guillain-Barré syndrome. On day six, he required mechanical ventilation due to progressive muscle weakness; propofol (3.5 mg/kg/hour) was administered for five days for sedation. On day 13, he had hypotension with abnormal electrocardiogram findings, acute kidney injury, hyperkalemia and severe rhabdomyolysis. Diagnosis and interventions: The patient was transferred to our intensive care unit (ICU) on suspicion of PRIS. Administration of noradrenaline and renal replacement therapy and fasciotomy for compartment syndrome of lower legs due to PRIS-rhabdomyolysis were performed. Outcomes: The patient gradually recovered and was discharged from the ICU on day 30. On day 37, he had repeated sinus bradycardia with pericardial effusion in echocardiography. Cardiac 18F-FDG PET on day 67 demonstrated heterogeneous 18F-FDG uptake in the left ventricle. Electron microscopic investigation of endomyocardial biopsy on day 75 revealed mitochondrial myelinization of the cristae, which indicated mitochondrial damage of cardiomyocytes. He was discharged without cardiac abnormality on day 192. Conclusions: Mitochondrial damage in both morphological and functional aspects was observed in the present case. Sustained mitochondrial damage may be a therapeutic target beyond the initial therapy of discontinuing propofol administration.

IP153. Patients Undergoing Fasciotomy in the Setting of Extracorporeal Membrane Oxygenation Have a High Rate of Amputation With Poor Overall Survival and Functional Status

The rate of extracorporeal membrane oxygenation (ECMO) placement is increasing. The large cannulas needed for ECMO can result in significant peripheral vascular complications. Lower extremity fasciotomy is performed relatively frequently in the setting ECMO and compartment syndrome. The outcomes of lower extremity fasciotomy in the setting of ECMO are not well-described in the literature. The purpose of our study was to evaluate survival, limb salvage rate and functional status at 6 months of patients undergoing lower extremity fasciotomy in the setting of ECMO. A single institution, a retrospective review of a prospectively maintained database identifying all patients placed on ECMO at our institution from 2015 through 2018 was performed. All patients who underwent fasciotomy were evaluated. The primary end point was amputation-free survival at time of discharge. Five hundred two patients were placed on ECMO from 2015 to 2018 (Fig). Twenty-three patients (4.5%) required one or more fasciotomies for compartment syndrome. Twenty-two of 23 patients were placed on VA ECMO for cardiogenic shock, with one patient on veno-venous (VV) ECMO for acute respiratory distress syndrome. The mean age of patients receiving fasciotomy was 54.6 ± 14.9 years. Sixteen patients had reperfusion catheters in place at the time of diagnosis of compartment hypertension. No patients had reperfusion catheters placed after the time of diagnosis. Of the 23 who received fasciotomies, 16 patients died during their index hospitalization. Seven patients survived to discharge. Five of the surviving seven (71%) required major amputation. The two patients whose limbs were salvaged continued to have limited mobility at the 6-month follow-up. The only patient with no functional deficits, received an above the knee amputation while hospitalized (Fig). This is the first study to specifically evaluate the morbidity and mortality of ECMO patients who require fasciotomy. These data demonstrate that the majority of individuals requiring fasciotomy die during their hospitalization and of those who survive, most require amputation. Overall amputation-free survival is less than 9% in this patient population, with those surviving having poor functional status at the 6-month follow-up. The high mortality rates and poor long-term functional status question efficacy of fasciotomy in ECMO patients.

Fasciotomy in compartment syndrome from snakebite.

BackgroundLocal symptoms and signs of snake envenomation mimic the clinical features of compartment syndrome. It is important to measure the intracompartmental pressure to diagnose compartment syndrome. In this study, we present our experiences of confirming compartment syndrome and performing fasciotomy in snakebite patients based on high intracompartmental pressure findings.MethodsThe medical records of patients who visited the trauma center of Wonju Severance Christian Hospital from January 2010 to December 2015 for the management of venomous snakebite were retrospectively reviewed. Starting in 2014, fasciotomy was performed in patients with an intracompartmental pressure of more than 40 mmHg in addition to the clinical symptoms of compartment syndrome.ResultsA total of 158 patients with snakebite came to the hospital within 48 hours for treatment. Most patients (110 patients) were bitten at the upper extremities (69.6%). Since 2014, 33 out of 59 patients were suspected to have compartment syndrome, and their intracompartmental pressures were measured. Seventeen of those patients had a high intracompartmental pressure (average, 49.6 mmHg; range, 37–88 mmHg), and fasciotomy was performed. ConclusionsIn this study, as many as 10.8% of all cases were in need of fasciotomy when compartment syndrome was diagnosed by measuring the intracompartmental pressure. Previously, it was reported that fasciotomy was not required in many cases of compartment syndrome originating from snakebite. However, some patients may develop very severe compartment syndrome, requiring fasciotomy.

Negative-pressure wound therapy (NPWT) can improve fasciotomy wound closure, but its effects on skeletal muscle are largely unknown. The purpose of this study was to evaluate NPWT effects on skeletal muscle after fasciotomy for compartment syndrome in an animal model and to assess regional variability in muscle fiber regeneration. Compartment syndrome was induced in the hindlimb of twenty-two adult female pigs with use of a continuous intracompartmental serum-infusion model. Fasciotomy was performed after six hours, and animals were randomized to receive either wet-to-dry gauze dressings (control group) or NPWT dressings (-125 mm Hg, continuous suction) for seven days. Delayed primary wound closure was attempted at seven days, and the peroneus tertius was harvested for analysis seven days or twenty-one days after fasciotomy. Muscles were weighed, and hematoxylin and eosin-stained samples from four regions of the muscle (superficial central, deep central, lateral, and proximal) were mapped for different cellular morphologies. Muscle weight was greater in the affected limb at all time points with no difference between treatment groups. At seven days, only the deep central samples in the NPWT group had a significantly greater cross-sectional area containing normal fibers as compared with that found in the controls. By twenty-one days, the deep central, lateral, and proximal regions of the NPWT-treated muscles had a smaller cross-sectional area containing normal fiber morphology and a greater cross-sectional area containing only mononucleated cells as compared with the controls. NPWT did not decrease muscle weight. At twenty-one days, the extent of muscle fiber regeneration after fasciotomy for compartment syndrome was reduced in muscles treated with NPWT for seven days compared with the values in the control group treated with wet-to-dry gauze dressings. NPWT may be harmful to skeletal muscle after compartment syndrome requiring fasciotomy and local wound care.

Pseudo-Popeye syndrome

A 75-year-old woman with medullary plasmacytoma reported progressive swelling of the forearms, left more than right (figure 1), with median nerve contribution pain. Examination revealed distal sensory loss and left hand weakness with atrophy of the median and ulnar nerve–innervated muscles. Arterial pulses were palpable. Electrodiagnostic studies showed ulnar nerve conduction block and left thenar muscle denervation. MRI showed subcutaneous and muscle edema in the forearm flexor muscles, mimicking Popeye syndrome.1 Fasciotomy for compartment syndrome and muscle biopsy were performed. Muscle showed lymphoplasmacytic infiltrates (figure 2), a rare extramedullary manifestation of plasmacytoma.2,3 Sensory loss and muscle strength improved after fasciotomy. Therapy for plasmacytoma was bortezomib/dexamethasone plus radiotherapy of both forearms.

Bedside fasciotomy under local anesthesia for acute compartment syndrome: a feasible and reliable procedure in selected cases

BackgroundFasciotomy for compartment syndrome is an emergent procedure that is usually done in the operating theater under general anesthesia. Delay in performing the procedure can lead to worse outcome. Various reasons can cause delay in performing the surgery. Bedside fasciotomy under local anesthesia can be done in these cases to avoid delay in compartment release.Materials and methodsThis was a retrospective study of 34 cases of acute compartment syndrome for which fasciotomy was done at the bedside under local anesthesia. The minimum follow-up period was 6 months.ResultsAll patients had immediate and marked improvement in pain. Thirty-three patients regained their normal muscle strength. Thirty-two patients regained normal range of motion of adjacent joints. One patient developed flexion contracture of the great toe. There was no deep infection, chronic osteomyelitis, or amputation. Superficial wound infection was noted in three patients; one patient had persistent foot drop.ConclusionBedside fasciotomy under local anesthesia is a feasible, safe, and effective choice for treating compartment syndrome in patients with delayed presentation or those with anticipated delay to undergo surgery in the operating theater under general or regional anesthesia. The results of this study are encouraging, as all wounds healed satisfactory and there were no cases of deep infections. The formal release of compartments in the operating room under general anesthesia continues to be the standard of care. This is the first description in the literature for bedside fasciotomy under local anesthesia with a relatively large number of patients.

Compartment Syndrome in Patients with Massive Venous Thrombosis After Inferior Vena Cava Filter Placement

Massive venous thrombosis, which can occur acutely after inferior vena cava filter placement, has 2 forms: phlegmasia cerulea dolens and phlegmasia alba dolens. In phlegmasia cerulea dolens, complete occlusion of venous outflow occurs. In the milder phlegmasia alba dolens version, collateral venous flow out of the limb remains despite the venous thrombosis. This article presents, to our knowledge, the first 2 cases of massive venous thrombosis (1 phlegmasia cerulea dolens, 1 phlegmasia alba dolens) below inferior vena cava filters occurring after the acute period. Phlegmasia cerulea dolens and phlegmasia alba dolens can present as compartment syndrome. Prompt fasciotomies were performed, but the underlying massive venous thrombosis was not addressed surgically. Phlegmasia cerulea dolens and phlegmasia alba dolens have high morbidity and mortality. The patient with phlegmasia alba dolens required leg and thigh fasciotomies and eventually required an above-knee amputation. The patient with phlegmasia cerulea dolens developed compartment syndrome in the left leg, right leg, and right thigh. Although he underwent decompression of all of these compartments, he died from multiple organ failure. A multidisciplinary approach with the vascular service and the intensivists is required in the treatment of patients with massive venous thrombosis. Treatment goals include preventing additional propagation of the thrombus via anticoagulation, with strong consideration for catheter-directed thrombolysis or thrombectomy and fasciotomies for compartment syndrome. The orthopedic surgeon should keep phlegmasia cerulea dolens and phlegmasia alba dolens in the differential for compartment syndrome, especially in patients who have had a history of acute or chronic inferior vena cava filter placement.

Fluid Lavage of Open Wounds (FLOW): design and rationale for a large, multicenter collaborative 2 × 3 factorial trial of irrigating pressures and solutions in patients with open fractures

BackgroundOpen fractures frequently result in serious complications for patients, including infections, wound healing problems, and failure of fracture healing, many of which necessitate subsequent operations. One of the most important steps in the initial management of open fractures is a thorough wound irrigation and debridement to remove any contaminants. There is, however, currently no consensus regarding the optimal approach to irrigating open fracture wounds during the initial operative procedure. The selection of both the type of irrigating fluid and the pressure of fluid delivery remain controversial. The primary objective of this study is to investigate the effects of irrigation solutions (soap vs. normal saline) and pressure (low vs. high; gravity flow vs. high; low vs. gravity flow) on re-operation within one year among patients with open fractures.Methods/DesignThe FLOW study is a multi-center, randomized controlled trial using a 2 × 3 factorial design. Surgeons at clinical sites in North America, Europe, Australia, and Asia will recruit 2 280 patients who will be centrally randomized into one of the 6 treatment arms (soap + low pressure; soap + gravity flow pressure; soap + high pressure; saline + low pressure; saline + gravity flow pressure; saline + high pressure). The primary outcome of the study is re-operation to promote wound or bone healing, or to treat an infection. This composite endpoint of re-operation includes a narrow spectrum of patient-important procedures: irrigation and debridement for infected wound, revision and closure for wound dehiscence, wound coverage procedures for infected or necrotic wound, bone grafts or implant exchange procedures for established nonunion in patients with postoperative fracture gaps less than 1 cm, intramedullary nail dynamizations in the operating room, and fasciotomies for compartment syndrome. Patients, outcome adjudicators, and data analysts will be blinded. We will compare rates of re-operation at 12 months across soap vs. saline, low pressure vs. high pressure, gravity flow pressure vs. high pressure, and low pressure vs. gravity flow pressure. We will measure function and quality of life with the Short Form-12 (SF-12) and the EuroQol-5 Dimensions (EQ-5D) at baseline, 2 weeks, 6 weeks, 3 months, 6 months, 9 months, and 12 months after initial surgical management, and measure patients' illness beliefs with the Somatic Pre-Occupation and Coping (SPOC) questionnaire at 1 and 6 weeks. We will also compare non-operatively managed infections, wound healing, and fracture healing problems at 12 months after initial surgery.DiscussionThis study represents a major international effort to identify a simple and easily applicable strategy for emergency wound management. The importance of the question and the potential to identify a low cost treatment strategy argues strongly for global participation, especially in low and middle income countries such as India and China where disability from traumatic injuries is substantial.Trial RegistrationThis trial is registered at ClinicalTrials.gov (NCT00788398).

Retrograde Intramedullary Nailing of Femoral Diaphyseal Fractures Caused by Low-velocity Gunshots

The purpose of this study was to evaluate the results of retrograde intramedullary nailing of femoral diaphyseal fractures caused by low-velocity gunshots. Seventy-three patients (68 men and 5 women) with 74 femoral diaphyseal fractures caused by gunshots were treated with reamed, retrograde, statically locked intramedullary nailing. The main outcome measures were fracture union, the need for additional operations, shortening, angular deformity, postoperative knee range of motion, and complication rate. Twenty-nine patients sustained additional injuries. During initial treatment, 18 patients underwent further orthopedic and nonorthopedic elective procedures. Three patients had fasciotomies for compartment syndrome. At follow-up, 3 patients had shortening >10 mm (range, 12-18 mm) and 1 had angulation >10 degrees (13 degrees recurvatum). Four patients underwent surgeries after discharge. Complications included 1 case of heterotopic bone formation, 1 hypertrophic nonunion that went on to union after dynamization, 2 postoperative positive blood cultures, and 1 wound dehiscence. There were no cases of septic arthritis. Our data indicate that there is a low incidence of shortening, angular deformity, complication, and infection rates following treatment of femoral gunshot diaphyseal fractures with a retrograde nail. This study demonstrates that this is an acceptable alternative for the treatment of these injuries.

Study to prospectively evaluate reamed intramedually nails in patients with tibial fractures (S.P.R.I.N.T.): study rationale and design.

BackgroundSurgeons agree on the benefits of operative treatment of tibial fractures – the most common of long bone fractures – with an intramedullary rod or nail. Rates of re-operation remain high – between 23% and 60% in prior trials – and the two alternative nailing approaches, reamed or non-reamed, each have a compelling biological rationale and strong proponents, resulting in ongoing controversy regarding which is better.Methods/DesignThe objective of this trial was to assess the impact of reamed versus non-reamed intramedullary nailing on rates of re-operation in patients with open and closed fractures of the tibial shaft. The study to prospectively evaluate reamed intramedullary nails in tibial fractures (S.P.R.I.N.T) was a multi-center, randomized trial including 29 clinical sites in Canada, the United States and the Netherlands which enrolled 1200 skeletally mature patients with open (Gustilo Types I-IIIB) or closed (Tscherne Types 0–3) fractures of the tibial shaft amenable to surgical treatment with an intramedullary nail. Patients received a statically locked intramedullary nail with either reamed or non-reamed insertion. The first strategy involved fixation of the fracture with an intramedullary nail following reaming to enlarge the intramedullary canal (Reamed Group). The second treatment strategy involved fixation of the fracture with an intramedullary nail without prior reaming of the intramedullary canal (Non-Reamed Group). Patients, outcome assessors, and data analysts were blinded to treatment allocation. Peri-operative care was standardized, and re-operations before 6 months were proscribed. Patients were followed at discharge, 2 weeks post-discharge, and at 6 weeks, 3, 6, 9, and 12 months post surgery. A committee, blinded to allocation, adjudicated all outcomes.DiscussionThe primary outcome was re-operation to promote healing, treat infection, or preserve the limb (fasciotomy for compartment syndrome after nailing). The primary outcome was a composite comprising the following re-operations: bone grafts, implant exchanges, and dynamizations, in patients with fracture gaps less than 1 cm post intramedullary nail insertion. Infections and fasciotomies were considered events irrespective of the fracture gap. We planned a priori to conduct a subgroup analysis of outcomes in patients with open and closed fractures. S.P.R.I.N.T is the largest collaborative trial evaluating alternative orthopaedic surgical interventions in patients with tibial shaft fractures. The methodological rigor will set new benchmarks for future trials in the field and its results will have important impact on patient care. The S.P.R.I.N.T trial was registered [ID NCT00038129] and received research ethics approval (REB#99-077).

Case Studies of an Infant, a Toddler, and an Adolescent Treated With a Negative Pressure Wound Treatment System

Three pediatric case studies (infant, toddler, and adolescent) using negative pressure wound treatment system (NPWS) in a children's hospital are described. Each child had complex surgical wounds and different goals for the therapy. The infant had an ulcerating labial hemangioma surgically removed, and the NPWS was used to expedite granulation tissue in preparation for a split-thickness skin graft. The critically ill toddler required abdominal fasciotomies for compartment syndrome, and the NPWS was applied to reduce interstitial abdominal edema and manage the excessive wound drainage. The adolescent initially had surgery to correct a pectus excavatum; when the sternal wound became infected and required debridement, the NPWS was used to expedite wound healing. The results of the NPWS were positive. Although this therapy has been used more commonly in adults, these case studies demonstrate its efficacy in several different types of wounds and age groups of children. The wound, ostomy, continence nurse should include the NPWS in his or her treatment differential as a valuable option for pediatric patients.

Vacuum-assisted Therapy in Vascular Surgery

Since the first use of vacuum-assisted therapy (V.A.C.) in wound care, the indications of this therapy have rapidly expanded. Vascular surgery presents many types of problematic wounds. In the current cost conscious atmosphere, there is a great demand for simple and effective therapies. The V.A.C. system has a lot of potential in the management of vascular wounds. In this article we present indications for vacuum-assisted therapy in vascular surgery: chronic leg ulcers, mesh skin graft, wound care after fasciotomy for compartment syndrome, problematic inguinal wound, false aneurysms, diabetic foot gangrene and amputations with marginal circulations.

The Loop Shoelace Technique; an Alternative Method for Closure of Fasciotomy Wounds

Fasciotomy for compartment syndrome in the limb is a surgical emergency to preserve future limb function. Compartment syndrome of the extremity may occur after severe trauma secondary to fractures, vascular ischemia, crush, or electrical injury. Treatment consists of expedient fasciotomy to avoid permanent injury to muscles and nerves. Following fasciotomy in the leg, the edematous muscle and the retracted skin edges make delayed primary closure difficult. Management of the wounds after fasciotomy has consisted traditionally of primary closure, healing by secondary intention, or split-thickness skin grafting to cover defects. If the wounds are closed under too much tension, skin edge or even flap necrosis may result. The shoelace technique is a method for gradual closure of open wounds, and it appears to be effective, simpler and more economical than the other techniques described so far for the successful delayed primary closure of fasciotomy wounds.
