ABSTRACT Community Re Entry Program (CREP) is a specialized rehabilitation program for individuals with mental disorders, recovering from mental illness. This program is specifically designed to develop and integrate the skills that enable individuals to relearn the skills necessary to fully participate in activities of daily living successfully and return to home or community.

Data Sharing Between Jail and Community Health Systems: Missing Links and Lessons for Re-Entry Success.

Data sharing and interoperability between jail systems and community health providers are critical for successful re-entry of incarcerated individuals into the mainstream community. Using a case study approach, we present an account of interoperability efforts between jail and community health systems in the County of Orange (California, USA), including the overall infrastructure comprising of the jail management system, jail health system, and the community health system. We also describe outcomes and lessons from the Jail to Community Re-entry Program implemented in the County of Orange, along with recommendations and common data elements required for effective care transitions from custody to community.

Peer recovery services for persons returning from prison: Pilot randomized clinical trial investigation of SUPPORT

Previously incarcerated persons with substance use disorder (SUD) need recovery supports, given the overrepresentation of this population in prison and community supervision. Peer support programs have the potential to fill gaps in postrelease support for persons with SUD. To assess the effectiveness of peer support approaches, this pilot study randomized access to peer recovery coaches within a well-established community reentry program. We examined several proximal outcomes to determine potential mechanisms of action, along with several exploratory outcomes. While attrition due to re-incarceration, death, and program disengagement was high, our findings suggest that those who received peer recovery coach support in the reentry program had recovery-based improvements, including improved self-reported mental and physical health and reductions in substance use behaviors. The treatment group also saw improvements in measures of treatment motivation and self-efficacy. Both groups saw similar positive trends in some outcomes, likely due to the relative success of the well-established reentry program regardless of the inclusion of peer support coaches. This study contributes lessons learned and potential mechanisms of action to limited research on the effectiveness of peer recovery supports for reentry populations with SUD.

Effect of Community Re-Entry Program (CRP) on Insight of Patients with Schizophrenia

Introduction: The use of rehabilitation treatments along with drug therapy can greatly help in the treatment of schizophrenic patients and their return to society. The purpose of this study was to evaluate the effectiveness of the Community Re-entry Program on improving insight in people with schizophrenia.Methods: The present study is a pre-test, post-test clinical trial with the statistical population including all patients with schizophrenia admitted to the Tolue rehabilitation center. For this purpose, 40 patients admitted to Tolue Rehabilitation Center who met the inclusion criteria were selected by convenience sampling method and were randomly assigned to experimental (n = 20) and control (n = 20) groups. The experimental group received a community Re-entry program for sixteen sessions. The groups were tested by Anderson's positive and negative symptoms questionnaire before and after the intervention, after completion and two months later. Data were analyzed using t-test for two independent groups, and covariance analysis by SPSS Inc., Chicago, IL; Version 18 Results: The results showed that patients in experimental group compared to control group after implementation of community Re-entry program on negative symptom scales (P = 0.000), disruption (P = 0.000), excitement (P = 0.000)). Anxiety, depression (P = 0.000), positive symptoms (P = 0.000) and total positive and negative symptoms (P = 0.000) at posttest and follow-up on negative symptom scales (P = 0.000). Significant differences were observed between rupture (P = 0.000), excitement (P = 0.01), anxiety and depression (P = 0.000), positive symptoms (P = 0.000) and total positive and negative symptoms (P = 0.001). Examining Trauma and Readiness to Change among Women in a Community Re-Entry Program

ABSTRACTBackground: Posttraumatic stress disorder (PTSD) and co-occurring substance use disorders (SUDs) are common among women who are incarcerated. Objective: The purpose of this study was to examine the relationship between trauma and readiness to change substance use behaviors. Methods: This study used data from 103 participants enrolled in a residential re-entry program for women with SUDs and trauma history. Women reporting clinically elevated Trauma Symptom Inventory (TSI) subscale scores were compared to those without elevated scores on the University of Rhode Island Change Assessment (URICA) readiness to change instrument. Primary analyses included t-tests and ANCOVA to control for age and ethnicity. Results: In general, women with clinically elevated trauma scores also reported greater readiness to change. The analyses revealed significant differences on the URICA Readiness to Change scores between women who had elevated Defensive Avoidance and Impaired Self-Reference according to the TSI. Results approached significance for women who had elevated TSI subscale scores for Sexual Concerns and Dissociation. Conclusions: These results point to a need to further understand links between trauma and readiness to change, particularly, the role of posttraumatic growth and psychological distress. This study has implications for social workers and clinicians delivering evidence-based treatment. Women who had high trauma symptoms were more willing to address change. Findings also suggest a need to tailor interventions to include motivational components that are also trauma-informed.

The implementation of Circles of Support and Accountability in the United States

Abstract According to estimates from the U.S. National Reentry Resource Center (NRRC), at least 95% of state prisoners are released back to their communities after a period of incarceration. The complex issue of returning individuals convicted of sex offenses to their communities often evokes particular concern for both criminal justice agencies and the general public. Amid increases in the scope and intensity of sex offenders’ supervision, there has been a growing interest among academics, criminal justice practitioners, and faith groups in using restorative justice approaches with this population. Circles of Support and Accountability (CoSA) is a restorative justice-based community reentry program for sex offenders deemed to be at the highest risk of reoffending and with little or no pro-social community support. This integrative review synthesizes both the previous literature on the effectiveness of CoSA in reducing reoffending with findings from a recent evaluability assessment of CoSA provision in the US. It describes the various forms of implementation, the methods by which CoSA has and can be evaluated, and the possible obstacles that impede rigorous evaluation. The implications for the future implementation and evaluation of CoSA are discussed along with the implications for reentry policy and practice in general.

Women Reentering the Community: Understanding Addiction and Trauma-Related Characteristics of Recidivism

Women represent 26% of those arrested for serious crimes; many have posttraumatic stress and co-occurring substance use disorders, which may influence recidivism. This study examined 57 women residing in a community re-entry program after exiting prison. Participants completed the Trauma Symptom Inventory and Substance Abuse Subtle Screening Inventory. A discriminant function analysis accounted for 34% of between-group variability and provided a profile of women who recidivated. Women who recidivated had greater alcohol dependence and lower rates of sexual dysfunction and sexual concerns. Practitioners should consider potential links between substance abuse, trauma, and sexual behavior in treatment settings.

Psychosocial Rehabilitation: The Community Re-Entry Program (CREP) in Psychiatric Inpatient Settings, Systematic Review

Aim: This review is an update on the research evidence supporting psychosocial rehabilitation – the Community Re-Entry Program (CREP) in psychiatric inpatient settings. Method: This review is guided by the following questions: Is the Community Re-Entry Program (CREP) effective in psychiatric inpatient settings? What are the main results and benefits of using this program in this patient population? Search strategy on Databases= MEDLINE and CINAHL (EBSCO), Cochrane Library, and SciELO, with no restriction on publication date and with the following keywords: Community Re-Entry Program OR CREP AND inpatients, in full text, in consecutive studies conducted between February, 2013, and October, 2014, in order to find all studies published that had used this program. Results: Results show that Patients with Experience of Mental Illness (PEMI) in the community re-entry program significantly improved their knowledge and performance of the skills taught in the sessions, compared to other patients. Community re-entry participants were also significantly more likely to attend their first aftercare appointment than other participants. Conclusions: The findings of the study clearly demonstrate the effectiveness of the CREP for clinical practice in psychiatric inpatient settings. From the patients’ perspective, quality of life is enhanced. By learning skills to achieve their own personal relevant goals in life, individuals with serious mental disorders are empowered to function more autonomously from mental health professionals. Our general conclusion is that the Community Re-Entry Program for psychiatric inpatients is effective in teaching them about their illness and symptoms as well as improving their ability to make and keep appointments. Further studies should address the dissemination of the CREP in European countries, including a Portuguese Nursing version, as a way to increase the likelihood that the individual will be successful with skills training within the treatment and care areas of psychiatric and mental health nurses.

Predicting Success in Residential Substance Abuse Interventions

Halfway houses (HWHs) are a common intervention for substance-abusing offenders. Facilities utilized to provide treatment for correctional populations are typically privately managed, often leading to variations in amounts and types of services and treatments. This study examines recidivism outcomes of substance-abusing offenders receiving halfway house interventions within New Jersey. Pre-intervention assessment characteristics are matched for participants with propensity score analysis. Using frailty models, this study found participants had a lower propensity to have their parole revoked than non-participants. Policy implications are made regarding further halfway house improvement to increase its role as a successful community reentry program.

Examining Masculine Norms and Peer Support within a Sample of Incarcerated African American Males.

The adherence to masculine norms has been suggested to be influenced by social settings and context. Prisons have been described as a context where survival is dependent on adhering to strict masculine norms that may undermine reintegration back into the larger society. This study attempted to examine the relationship between masculine norms, peer support, and an individual's length of incarceration on a sample of 139 African American men taking part in a pre-release community re-entry program. Results indicate that peer support was associated with length of incarceration and the interaction between the endorsement of masculine norms and peer support significantly predicted the length of incarceration for African American men in this sample. Implications for incarcerated African American men and future research directions are discussed.

Faith-Based Organization Welcomes Women Back Home Into the Community

This article reviews the current research related to the many issues affecting women's journey from incarceration back into community living. The complex transition process that comes when ex-offenders attempt to reenter society post-release is examined, and the story of one faith-based community reentry program for women located in southern California is described. The women in this program currently achieve overall recidivism rates of 3% as compared with national averages of 50%. The program uses peers to help newly released women achieve long-term success. The specific steps taken by this organization to obtain such significant measurable positive outcomes are described.

Early Versus Later Admission to Postacute Rehabilitation: Impact on Functional Outcome After Traumatic Brain Injury

Objective To examine the impact of participation in a postacute community reentry program on functional outcome after traumatic brain injury (TBI). Design Cohort, nonrandomized, intervention study. Pretest-posttest, follow-up design. Setting Nonprofit outpatient community reentry program affiliated with an inpatient rehabilitation hospital. Participants Three groups of persons with moderate to severe TBI differing in length of time between injury and admission. The first group entered postacute rehabilitation within 6 months of injury (n=115); the second group, between 6 and 12 months (n=23); and the third group, greater than 12 months (n=29). Interventions Persons with TBI participated in a postacute community reentry program (average, 4.3mo) that emphasized (1) teaching compensatory strategies to address residual cognitive deficits; (2) arranging environmental supports to maximize functioning; (3) counseling and education to address personal and family adjustment and to improve accurate self-awareness; and (4) transition from simulated activities in the clinic to productive activities in the community. Main Outcome Measures Disability Rating Scale, Supervision Rating Scale, and the Community Integration Questionnaire. Results All groups showed improvements between admission and discharge on measures of overall disability, independence, home competency, and productivity, and these gains were maintained at follow-up. For the group beginning postacute rehabilitation the earliest (<6mo postinjury) independence continued to improve after discharge. Community integration total score and home competency also continued to improve even after discharge. Conclusions The results point toward the effectiveness of postacute rehabilitation in improving functional outcome after TBI even for persons who have reached stable neurologic recovery at 12 or more months postinjury.

Evaluation of a community re-entry program

Summary The Community Re-Entry Program is a brief videotape-based skills training module designed to: (1) make the patient an active collaborator with the mental health system in fashioning an individualized aftercare plan; and (2) equip the patient with the skills and knowledge needed to implement the plan and engage in longer-term, community treatments. Results from two studies on the impact of the Community Re-Entry Program point to its efficacy in linking patients with aftercare shortly after discharge, but more research is needed to determine the durability of aftercare engagement.

Training hospitalized patients with schizophrenia in community reintegration skills.

The study examined the effectiveness of the Community Re-Entry Program, a brief, time-limited skills training module designed to help acutely ill inpatients become engaged in community-based treatment programs. Of 84 consecutive admissions to a chronic psychotic disorders unit, 44 completed assessments and attended the Community Re-Entry Program. The program consists of 16 daily small-group therapy sessions that engage the patient in efforts to define discharge readiness, identify symptoms and medication effects, and assist with discharge planning. Skill levels and positive and negative symptoms were assessed on admission and on completion of training, and a subsample of patients received two-week postdischarge follow-up assessments. From admission to discharge, positive symptoms diminished substantially, negative symptoms diminished to a lesser but statistically significant degree, and skill levels increased significantly. Posttraining skill level was predicted by pretraining skill level and level of participation in the skills training module. Patients' symptom levels did not predict participation in the program or skill acquisition. Skill level at discharge was also more predictive of two-week postdischarge community adjustment than were symptom levels. Although further controlled studies are required to fully establish the efficacy of the Community Re-Entry Program, these data suggest that brief, focused skills training may play an important role in augmenting optimal pharmacotherapy for hospitalized patients with chronic psychotic disorders.
