Substance Use Diagnoses Among Persons with Community-Onset Methicillin-Resistant Staphylococcus aureus Bloodstream Infections

Background: In recent years, the historic declines in the incidence of methicillin-resistant Staphylococcus aureus (MRSA) bloodstream infections (BSIs) in the United States have slowed. We examined trends in the incidence of community-onset (CO) MRSA BSIs among hospitalized persons with and without substance-use diagnoses. Methods: Using data from >200 US hospitals reporting to the Premier Healthcare Database (PHD) during 2012–2017, we conducted a retrospective study among hospitalized persons aged ≥18 years. MRSA BSIs with substance use were defined as hospitalizations having both a blood culture positive for MRSA and at least 1 International Classification of Disease, Ninth Revision, Clinical Modification (ICD-9-CM) or ICD-10-CM diagnostic code for substance use including opioids, cocaine, amphetamines, or other substances (excluding cannabis, alcohol, and nicotine). MRSA BSIs were considered community onset when a positive blood culture was collected within 3 days of admission. We assessed annual trends and described characteristics of CO MRSA BSI hospitalizations, stratified by substance use. Results: Of 20,049 MRSA BSIs from 2012 to 2017, 17,634 (88%) were CO. Overall, MRSA BSI incidence decreased 7%, from 178.5 to 166.2 per 100,000 hospitalizations during the study period; However, CO MRSA BSI rates remained stable (152.7 to 149.9 per 100,000 hospitalizations). Among CO MRSA BSIs, 1,838 (10%) were BSIs with substance-use diagnoses; the incidence of CO MRSA BSIs with substance use increased 236% (from 8.2 to 27.6 per 100,000 hospitalizations) and represented a greater proportion of the CO MRSA rate over the study period (Fig. 1). The incidence of CO MRSA BSIs without substance use decreased 15% (from 144.5 to 122.4 per 100,000 hospitalizations). Patients with CO MRSA BSIs with substance use were younger (median, 40 vs 65 years), more likely to be female (50% vs 40%), white (79% vs 69%), and to leave against medical advice (15% vs 1%). Among patients not leaving against medical advice, CO BSI patients with substance-use diagnoses had longer lengths of stay (median, 11 vs 9 days), lower in-hospital mortality (9% vs 14%), and higher hospitalization costs (median, $22,912 vs $17,468) compared to patients without substance-use diagnoses. Conclusions: Although the overall CO MRSA BSI rate remained unchanged from 2012 to 2017, infections with substance use diagnoses increased >3-fold, and infections without substance use diagnoses decreased. These data suggest that the emergence of MRSA associated with substance-use diagnoses threatens potential progress in reducing the incidence of CO MRSA infections. Additional strategies may be needed to prevent MRSA BSI in patients with substance-use diagnoses, and to maintain national progress in the reduction of MRSA infections overall.Funding: NoneDisclosures: None

A Spatial Analysis of Health Disparities Associated with Antibiotic Resistant Infections in Children Living in Atlanta (2002-2010).

Background:Antibiotic resistant bacteria like community-onset methicillin resistant Staphylococcus aureus (CO-MRSA) have continued to cause infections in children at alarming rates and are associated with health disparities. Geospatial analyses of individual and area level data can enhance disease surveillance and identify socio-demographic and geographic indicators to explain CO-MRSA disease transmission patterns and risks.Methods:A case control epidemiology approach was undertaken to compare children with CO-MRSA to a noninfectious condition (unintentional traumatic brain injury (uTBI)). In order to better understand the impact of place based risks in developing these types of infections, data from electronic health records (EHR) were obtained from CO-MRSA cases and compared to EHR data from controls (uTBI). US Census data was used to determine area level data. Multi-level statistical models were performed using risk factors determined a priori and geospatial analyses were conducted and mapped.Results:From 2002–2010, 4,613 with CO-MRSA and 34,758 with uTBI were seen from two pediatric hospitals in Atlanta, Georgia. Hispanic children had reduced odds of infection; females and public health insurance were more likely to have CO-MRSA. Spatial analyses indicate significant ‘hot spots’ for CO-MRSA and the overall spatial cluster locations, differed between CO-MRSA cases and uTBI controls.Conclusions:Differences exist in race, age, and type of health insurance between CO-MRSA cases compared to noninfectious control group. Geographic clustering of cases is distinct from controls, suggesting placed based factors impact risk for CO-MRSA infection.

Incidence of community onset MRSA in Australia: least reported where it is Most prevalent

BackgroundThis is the first review of literature and synthesis of data on community onset methicillin resistant Staphylococcus aureus (CO-MRSA) infections in Australia. Incidence of CO-MRSA varies considerably in Australia, depending on geographic and demographic factors.MethodsData for the rates of MRSA infections were collected from articles identified using PubMed, Scopus, the grey literature and data from State and Federal Government Surveillance Systems. We synthesized data and developed a framework for how data was selected, collated, linked, organized and interpreted.ResultsThe results of our literature search demonstrates considerable gaps in the reporting of CO-MRSA in Australia. Consequently, total incidences were under reported; however the available data suggests the incidence varied between 44 (Tasmania) and 388 (southern Northern Territory) cases per 100,000 person years. Hospitalised cases of CO-MRSA varied between 3.8 (regional Victoria) and 329 (southern Northern Territory). Taking the median percentage of infections by site for all regions available, skin and soft tissue infections (SSTIs) consisted of 56% of hospitalized CO-MRSA, compared with bacteremias, which represented 14%. No region had a complete data set of CO-MRSA infections treated in out-patient settings and so incidences were underestimates. Nevertheless, estimates of the incidence of CO-MRSA treated outside hospitals varied between 11.3 (Melbourne) and 285 (Northern Territory) per 100,000 person-years. These infections were chiefly SSTIs, although urinary tract infections were also noted.Incidences of CO-MRSA blood-stream infections and outpatient skin and soft tissue infections have been increasing with time, except in Tasmania. CO-MRSA is observed to affect people living in remote areas and areas of socioeconomic disadvantage disproportionately.ConclusionsWe generated the first estimates of the incidence of CO-MRSA infections in Australia and identified stark regional differences in the nature and frequency of infections. Critically, we demonstrate that there has been a lack of consistency in reporting CO-MRSA and a general dearth of data. The only government in Australia that requires reporting of CO-MRSA is the Tasmanian, where the infection was least prevalent. Some regions of Australia have very high incidences of CO-MRSA. To improve surveillance and inform effective interventions, we recommend a standardized national reporting system in Australia that reports infections at a range of infection sites, has broad geographic coverage and consistent use of terminology. We have identified limitations in the available data that hinder understanding the prevalence of CO-MRSA.

Multidrug and Mupirocin Resistance in Environmental Methicillin-Resistant Staphylococcus aureus (MRSA) Isolates from Homes of People Diagnosed with Community-Onset MRSA Infection.

Patients with community-onset (CO) methicillin-resistant Staphylococcus aureus (MRSA) infections contribute to MRSA contamination of the home environment and may be reexposed to MRSA strains from this reservoir. This study evaluates One Health risk factors, which focus on the relationship between humans, animals, and the environment, for the increased prevalence of multiple antimicrobial-resistant MRSA isolates in the home environment. During a trial of patients with CO-MRSA infection, MRSA was isolated from the household environment at the baseline and 3 months later, following randomization of patients and household members to mupirocin-based decolonization therapy or an education control group. Up to two environmental MRSA isolates collected at each visit were tested. MRSA isolates were identified in 68% (65/95) of homes at the baseline (n = 104 isolates) and 51% (33/65) of homes 3 months later (n = 56 isolates). The rates of multidrug resistance (MDR) were 61% among isolates collected at the baseline and 55% among isolates collected at the visit 3 months later. At the baseline, 100% (14/14) of MRSA isolates from rural homes were MDR. While antimicrobial use by humans or pets was associated with an increased risk for the isolation of MDR MRSA from the environment, clindamycin use was not associated with an increased risk for the isolation of MDR MRSA. Incident low-level mupirocin-resistant MRSA strains were isolated at 3 months from 2 (5%) of 39 homes that were randomized to mupirocin treatment but none of the control homes. Among patients recently treated for a CO-MRSA infection, MRSA and MDR MRSA were common contaminants in the home environment. This study contributes to evidence that occupant use of antimicrobial drugs, except for clindamycin, is associated with MDR MRSA in the home environmental reservoir. NCT00966446.)IMPORTANCE MRSA is a common bacterial agent implicated in skin and soft tissue infections (SSTIs) in both community and health care settings. Patients with CO-MRSA infections contribute to environmental MRSA contamination in these settings and may be reexposed to MRSA strains from these reservoirs. People interact with natural and built environments; therefore, understanding the relationships between humans and animals as well as the characteristics of environmental reservoirs is important to advance strategies to combat antimicrobial resistance. Household interactions may influence the frequency and duration of exposure, which in turn may impact the duration of MRSA colonization or the probability for recurrent colonization and infection. Therefore, MRSA contamination of the home environment may contribute to human and animal recolonization and decolonization treatment failure. The aim of this study was to evaluate One Health risk factors that may be amenable to intervention and may influence the recovery of MDR and mupirocin resistance in CO-MRSA isolates.

The burden of community onset MRSA in Australia

Introduction Little is known about the health and economic burden of community onset methicillin-resistant Staphylococcus aureus (CO-MRSA) in Australia. However, this information is imperative for developing healthcare policy and planning, for prioritising and directing resources and for implementing evidence-based practice. Methods We have collated data from academic literature and government reports on the numbers of CO-MRSA infections, the proportions of infections occurring at various anatomical sites and indicators of treatment costs and morbidity, such as length of admissions and mortality rates. Direct costs of treating patients, such as the cost of hospitalisation, were calculated as well as indirect costs to society, such as loss of productivity because of morbidity or death. Results We estimated that the national incidence of CO-MRSA infections admitted to hospital was at least 24.8 /100,000 person-years. Of these, the majority were skin and soft tissue infections (21.2 /100,000 person-years), while bloodstream (BSI) and lower respiratory tract infections (LRTI) and incidences of 3.9 and 2.8 /100,000 person-years, respectively. Considerable regional differences in the nature and frequency of infection were identified. Mortality was found to be approximately 1 in 5 patients for BSIs and slightly lower for pneumonia. Conclusions CO-MRSA presents a significant health issue and the costs associated merit attention to support medical decision making. Standardised definitions and regular surveillance of the incidence of CO-MRSA infections are necessary. Surveillance should also capture data on the consequences and treatments required for a better understanding of the cost of CO-MRSA to Australia.

Reduction of Methicillin-ResistantStaphylococcus aureusInfection among Veterans in Atlanta

Describe local changes in the incidence of community-onset and hospital-onset methicillin-resistant Staphylococcus aureus (MRSA) infection and evaluate the impact of MRSA active surveillance on hospital-onset infection. Observational study using prospectively collected data. Atlanta Veterans Affairs Medical Center (AVAMC). All patients seen at the AVAMC over an 8-year period with clinically and microbiologically proven MRSA infection. All clinical cultures positive for MRSA were prospectively identified, and corresponding clinical data were reviewed. MRSA infections were classified into standard clinical and epidemiologic categories. The Veterans Health Administration implemented the MRSA directive in October 2007, which required active surveillance cultures in acute care settings. The incidence of community-onset MRSA infection peaked in 2007 at 5.45 MRSA infections per 1,000 veterans and decreased to 3.14 infections per 1,000 veterans in 2011 ([Formula: see text] for trend). Clinical and epidemiologic categories of MRSA infections did not change throughout the study period. The prevalence of nasal MRSA colonization among veterans admitted to AVAMC decreased from 15.8% in 2007 to 11.2% in 2011 ([Formula: see text] for trend). The rate of intensive care unit (ICU)-related hospital-onset MRSA infection decreased from October 2005 through March 2007, before the MRSA directive. Rates of ICU-related hospital-onset MRSA infection remained stable after the implementation of active surveillance cultures. No change was observed in rates of non-ICU-related hospital-onset MRSA infection. Our study of the AVAMC population over an 8-year period shows a consistent trend of reduction in the incidence of MRSA infection in both the community and healthcare settings. The etiology of this reduction is most likely multifactorial.

Predictors of Clinical Virulence in Community-Onset Methicillin-Resistant Staphylococcus aureus Infections: The Importance of USA300 and Pneumonia

Though USA300 community-onset methicillin-resistant Staphylococcus aureus (CO-MRSA) has emerged as a major public health concern in the United States, its relative virulence is unknown. We sought to evaluate if the USA300 strain of CO-MRSA causes more severe infections than other MRSA (ie, USA100, -500, -800, and others) strains. An epidemiologic study was conducted from 2000 to 2007 to measure rates of severe infection. A matched case-control study was conducted from 2004 to 2006 to assess the relationship of strain type, syndrome, and severity of infection. Severe illness was defined as CO-MRSA infections with medical intensive care unit (MICU) admission or death within 1 week of admission. Controls were those with CO-MRSA infection without MICU admission. We found an incidence of 75 cases per 100000 people of CO-MRSA infection in 2000, which increased to a rate of 396 per 100000 in 2007 (relative risk [RR], 5.3; 95% confidence interval [CI], 4.47-6.27). The incidence of severe infections increased from 5 cases per 100000 in 2000 to 17 per 100000 in 2007 (RR, 3.4; 95% CI; 1.67-6.43). USA300 strains were negatively associated with severe clinical courses or death as compared with other MRSA strain types. The highest risk of severe infection was found in those with pulmonary embolic infiltrates and bacteremia in the setting of USA300 infection (odds ratio, 31.41; 95% CI, 6.40-154.23). Our findings suggest that USA300 infections are negatively associated with severe clinical courses, suggesting less virulence than other MRSA strains, except in the setting of pneumonia with septic pulmonary emboli.

Evaluation of International Classification of Diseases, Ninth Revision, Clinical Modification Codes for Reporting Methicillin-Resistant Staphylococcus aureus Infections at a Hospital in Illinois

States, including Illinois, have passed legislation mandating the use of International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) codes for reporting healthcare-associated infections, such as methicillin-resistant Staphylococcus aureus (MRSA). To evaluate the sensitivity of ICD-9-CM code combinations for detection of MRSA infection and to understand implications for reporting. We reviewed discharge and microbiology databases from July through August of 2005, 2006, and 2007 for ICD-9-CM codes or microbiology results suggesting MRSA infection at a tertiary care hospital near Chicago, Illinois. Medical records were reviewed to confirm MRSA infection. Time from admission to first positive MRSA culture result was evaluated to identify hospital-onset MRSA (HO-MRSA) infections. The sensitivity of MRSA code combinations for detecting confirmed MRSA infections was calculated using all codes present in the discharge record (up to 15); the effect of reviewing only 9 diagnosis codes, the number reported to the Centers for Medicare and Medicaid Services, was also evaluated. The sensitivity of the combination of diagnosis codes for detection of HO-MRSA infections was compared with that for community-onset MRSA (CO-MRSA) infections. We identified 571 potential MRSA infections with the use of screening criteria; 403 (71%) were confirmed MRSA infections, of which 61 (15%) were classified as HO-MRSA. The sensitivity of MRSA code combinations was 59% for all confirmed MRSA infections when 15 diagnoses were reviewed compared with 31% if only 9 diagnoses were reviewed (P < .001). The sensitivity of code combinations was 33% for HO-MRSA infections compared with 62% for CO-MRSA infections (P < .001). Limiting analysis to 9 diagnosis codes resulted in low sensitivity. Furthermore, code combinations were better at revealing CO-MRSA infections than HO-MRSA infections. These limitations could compromise the validity of ICD-9-CM codes for interfacility comparisons and for reporting of healthcare-associated MRSA infections.

Methicillin-Resistant Staphylococcus aureus Decolonization “Yes, We Can,” But Will It Help?

DOI: 10.1086/599020 Approximately 100,000 invasive methicillin-resistant Staphylococcus aureus (MRSA) infections occurred in 2005 in the United States, and the number of associated deaths was estimated at 19,000, which is more than the corresponding annual number of associated deaths for human immunodeficiency virus (HIV) and AIDS. With the increasing number of community-onset MRSA infections, prevention of staphylococcal infections is now more important than ever. For more than 50 years, it has been known that carriage of S. aureus plays an important role in the pathogenesis of staphylococcal infections and represents a potential target for preventive interventions. One of the first reports (1952) that clearly demonstrated the relationship between S. aureus carriage and subsequent infection involved miners who experienced beat disorders of the knees and elbows. A carefully performed microbiological survey showed that there were 24 “heavy” carriers of S. aureus among 45 beat case patients, compared with 5 heavy carriers among 45 matched control subjects without disease (odds ratio, 9.1 [95% confidence interval {CI}, 3.1–26.5]). Phagetyping showed that, in the majority of cases, the carriage strain matched the strain that caused disease. Hospital-based studies in the 1950s and 1960s confirmed this relationship, especially for surgical patients. More recently, these studies have been repeated in various other patient populations—for example, dialysis patients, patients with HIV, organ transplant recipients, and critically ill patients with intravascular catheters—with similar results. It seems obvious that eradication of S. aureus carriage can reduce the risk for infection. This strategy has been studied in several groups of patients, and a recent Cochrane review aggregated the evidence with regard to the effect of eradication of carriage on the S. aureus infection rate. Eight randomized controlled trials studied the effect of mupirocin nasal ointment in various groups of patients. The pooled estimate showed a significant reduction of the S. aureus infection rate (relative risk [RR], 0.55 [95% CI, 0.43–0.70]; ). An P ! .001 analysis of subgroups showed a pronounced effect on surgical patients and on patients who were receiving dialysis, and this fact confirms results of a previously published systematic review. Recently, a large trial was completed in which patients were screened on hospital admission for S. aureus nasal carriage by means of a 2-hour polymerase chain reaction–based assay, and carriers were subsequently randomly assigned to mupirocin nasal ointment and chlorhexidine skin washings or to placebo. The treated carriers had a significantly lower S. aureus infection rate (RR, 0.42 [95% CI, 0.23–0.75]). In addition, the length of hospital stay was significantly shortened for the group of treated carriers (mean reduction of the length of stay, 1.8 days; ). Despite the evidence that P p .04 treatment of proven carriers lowers the S. aureus infection risk, there are several issues remaining. Patients who underwent elective surgery were probably the most important group to benefit from treatment of proven carriers. Because most studies of surgical patients have included both S. aureus carriers and S. aureus noncarriers, it is difficult to assess the overall effect of treatment of proven carriers. Moreover, some studies have found that the infection rate caused by microorganisms other than S. aureus was significantly higher among patients treated with mupirocin, compared with that for control subjects (RR, 1.38 [95% CI, 1.11–1.72]). This finding was based mainly on one large study of dialysis patients who were treated repeatedly. Compared with suppression or eradication of carriage of methicillin-susceptible S. aureus, suppression or eradication of MRSA carriage remains a more difficult task. Rather than being related to pathogen-associated factors that are not fundamentally different between most methicillin-susceptible S. aureus and MRSA clones, the frequent failure of eradication treatment is related to common characteristics of MRSA carriers, such as skin lesions, catheters, and comorbidities, that make MRSA

Guidelines (2008) for the prophylaxis and treatment of methicillin-resistant Staphylococcus aureus (MRSA) infections in the United Kingdom

These evidence-based guidelines are an updated version of those published in 2006. They have been produced after a literature review of the treatment and prophylaxis of methicillin-resistant Staphylococcus aureus (MRSA). The guidelines aim to complement those recently published for the antibiotic treatment of common and emerging community-onset MRSA infections in the UK. The guidelines have reviewed and updated, where appropriate, previous recommendations, taking into account any changes in the UK epidemiology of MRSA, ongoing national surveillance data and the value of new antistaphylococcal agents licensed for use in UK practice. Emerging therapies that have not been licensed for UK use are not reviewed, but their future potential role has been mentioned where deemed appropriate. Recommendations are given for the treatment of common infections caused by MRSA, elimination of MRSA from carriage sites and prophylaxis of surgical site infection.

Methicillin-resistant Staphylococcus aureus: risk factors associated with community-onset infections in Denmark

The proportion of methicillin-resistant Staphylococcus aureus (MRSA) in Denmark has been below 1% for more than 30 years. However, a marked increase in community-onset MRSA (CO-MRSA) started in 2002. To identify possible risk factors for CO-MRSA infections, a nationwide case-control study was conducted in 2004. Cases (34) were patients with CO-MRSA infections; controls (87) were patients with community-onset methicillin-sensitive S. aureus infections (CO-MSSA). Demographic and clinical data and exposures to possible risk factors during the last 24 months were collected with a structured telephone-administered questionnaire. Skin and soft tissue were the predominant sites of infection, both for cases (68%) and for controls (60%). A large proportion of cases (26%) and controls (38%) had an underlying skin disease. The majority of cases (76%) and controls (61%) had received antibiotics within the last 6 months, and 51% and 31%, respectively, had been hospitalized within the previous year. In a multivariate analysis, non-Danish origin, defined as being from or having parents from outside Denmark, was the only independent risk factor for CO-MRSA infection (OR 30.5, 95% CI 3.6-257.3). Prior hospitalization for >7 days within the previous 6 months tended to be associated with CO-MRSA infection (OR 5.7, 95% CI 0.9-36.4). The predominant MRSA clones found in this study were CC80 (26%), CC8 (24%) and CC5 (18%). Resistance to three or more antimicrobial drug classes was seen in 47% of CO-MRSA isolates. Panton-Valentine leukocidin was found in 47% of CO-MRSA isolates. Apart from a non-Danish origin, CO-MRSA shared the same risk factors as CO-MSSA, which makes control a challenge.

Personal Hygiene and Methicillin-resistantStaphylococcus aureusInfection

Methicillin-resistant Staphylococcus aureus (MRSA) infections outside the healthcare setting are an increasing concern. We conducted a case-control study to investigate an MRSA outbreak during 2002-2003 in a Missouri prison and focused on hygiene factors. Information on sociodemographic characteristics, medical history, and hygiene practices of study participants was collected by interview and medical record review. Logistic regression was used to evaluate MRSA infection in relation to hygiene factors individually and as a composite hygiene score; potential confounding factors were controlled. Selected MRSA isolates were analyzed by pulsed-field gel electrophoresis (PFGE). MRSA infection was significantly associated with a low composite hygiene score. Transmission among prison inmates appeared to be responsible for this outbreak. PFGE analysis showed that isolates were indistinguishable and associated with community-onset MRSA infections in other US prisons. Improving hygiene practices and environmental conditions may help prevent and interrupt future MRSA outbreaks in prison settings.
