Abstract 602: Biological monitoring to clarify potential causes of colorectal cancer in Korean diet

Abstract We performed a pilot study (N=30) to look for the Korean specific risk factors, which can explain an increased incidence rate of colorectal cancer (CRC) among Korean population. Based on the promising data from the pilot study, we performed a case-control study (N=218, age: 64.4 ±13.4 years; N of men=116, 53.2%) with biological monitoring to analyze various biomarkers including clinical/hematological indices, oxidative or inflammatory biomarkers, e.g., c-reactive protein (CRP), malondialdehyde (MDA) or homocysteine, the metabolites of polycyclic aromatic hydrocarbons (PAHs) and heterocyclic amines (HCAs), e.g., urinary 1-OHP, MeIQx and PhIP, and their DNA adducts, e.g., dG-C8-MeIQx and -PhIP, with UPLC/MS/MS. We also obtained participants’ epidemiological, clinical, and behavioral information and developed food frequency questionnaires (FFQ) to collect their food information for one year before cancer diagnoses. Total subjects consumed 53.4±74.0g/day of red meat, and 1.1±3.7g/day of processed meat. After adjustment for sex and age, there were no associations between CRC risk and red or processed meat intake, or urinary MeIQx or PhIP levels. However, there were inverse associations between CRC and milk or calcium intake (ps<0.05). In addition, urinary MeIQx levels were positively associated with intake of lipid or coffee (p=0.06, p<0.01, respectively). LDL-C and CRP levels in the cases were higher than ones from controls (ps<0.05). Among biomarkers, MDA levels were positively correlated to urinary levels of 1-OHP, PhIP and MeIQx (ps<0.05). These relations support the fact that PAHs or HCAs indirectly induce oxidative stress. Finally, fruits and milk intake showed protective effects, however, gender (man), CRP, and lipid intake were risk factors to CRC, based on multivariable regression analyses. Therefore, total lipid intake, oxidative stress, or inflammation may increase risks of CRC in Korean population rather than red or processed meat intake. Abstract 1488: Association between microrna binding site polymorphism and susceptibility to colorectal cancer in Korean population

Abstract Background : MicroRNAs (miRNAs) play an important role in the colorectal cancer tumorigenesis by regulation of gene expression through binding to 3′ untranslational region (UTR) of target gene. Single nucleotide polymorphism (SNP) within 3′UTR of gene could be affected by miRNA binding. Docking protein 3 (DOK3) is adapter protein that acts as tumor suppressor through modulating tyrosine kinase signaling. Methods : One hundred thirty five SNPs were selected in Silico analysis for the current study from 3′UTR SNPs of SNP database, which was based on several miRNA database, calculating miRNA bind efficiency on polymorphism, and HapMap database. Here independent two set of study was designed, 380 healthy controls and 371 colorectal adenocarcinoma patients were enrolled in discovery set, and 524 healthy controls and 521 colorectal adenocarcinoma patients were enrolled in replication set. The SNP genotyping was performed using the Sequenom MassARRAY Result: In discovery set, 16 SNPs were significantly associated with colorectal cancer in any genetic model from 135 SNP genotyping. Among 16 SNPs, DOK3 rs2279398G>A was significantly associated in replication set recessive model (adjusted odds ratio (OR) = 0.65, 95% confidence interval (CI) = 0.44-0.97, P = 0.03). In combined analysis, DOK3 rs2279398G>A was associated with significantly decreased risk of colorectal cancer in codominant and recessive model (adjusted OR = 0.84, 95% CI = 0.73-0.96, P = 0.012, adjusted OR = 0.65, CI = 0.49-0.88, P = 0.004, respectively). Conclusion : DOK3 rs2279398G>A may affect the expression of DOK3 by altering miRNA binding efficiency on miRNA-binding sites of the 3′-UTR of DOK3 and impact on colorectal cancer tumorigenesis. This finding suggests that DOK3 rs2279398G>A may be a useful biomarker for determining the susceptibility to colorectal cancer. Overweight is Associated with a Favorable Survival in Patients with Colorectal Cancer: A Prospective Cohort Study in an Asian Population

The influence of body mass index (BMI) on the outcome of patients with colorectal cancer (CRC) is largely unknown, particularly in an Asian population. Therefore, we investigated the influence of BMI on survival of patients who underwent surgical treatment for CRC in Korean population. This prospective cohort study included CRC patients who underwent surgery between June 2003 and May 2005. Study participants were divided into two BMI groups: normal weight (<23 kg/m(2)) and overweight (≥23 kg/m(2)). A total of 509 CRC patients were included in this study. Of these, 318 patients (62.5%) were male, and the mean ± SD age was 59.1 ± 11.3 years. Two hundred eighteen patients (42.8%) were included in the normal-weight group and 291 (57.2%) were included in the overweight group. During a follow-up of 62.8 ± 27.2 months, 100 (19.6%) of the 509 eligible patients died, and 72 (15.0%) of 479 experienced recurrence. The overall survival of overweight group was more favorable compared with that of the normal-weight group (P = 0.001), while there was no significant difference in recurrence (P = 0.735). Compared with the normal-weight group, the hazard ratios of death and recurrence, respectively, were 0.615 (95% confidence interval 0.408-925; P = 0.020) and 0.992 (95% confidence interval 0.613-1.604; P = 0.973) for the overweight group. Overweight is not associated with an increased risk of cancer recurrence but rather is associated a favorable overall survival in Asian CRC patients undergoing surgery.

Multiple primary malignancies involving colorectal cancer—clinical characteristics and prognosis with reference to surveillance

The purpose of this study was to investigate clinically useful information for effective screening for synchronous and metachronous second primary cancers and to suggest potential surveillance tool. We retrospectively reviewed 1,063 patients who were treated with potentially curative surgery for colorectal cancer at Gachon University, Gil Hospital from 1997 to 2007. The incidence of synchronous or metachronous cancer in addition to colorectal cancer was 5.3% (57 patients). The most common second primary cancer was stomach (54.3%), followed by cancers in cervix (12.2%). The time interval between the first and second tumor in colorectal cancer associated with metachronous extracolonic malignancy ranged from 1.1 to 10.8 years. The incidence of early-stage tumor was higher in patients with synchronous cancer than in those with a metachronous cancer with statistical significance (p = 0.034). The 5-year survival rate of the colorectal cancer group without second primary cancer was 70.1%, whereas that for the second primary cancer group was 63.8% (p = 0.253). The 5-year survival rate of the colorectal cancer group with stomach cancer was 70.5%, whereas that for the second primary cancer other than stomach cancer group was 56.6% (p = 0.282). The frequent association between colorectal cancer and gastric cancer suggests an inclusion of gastrofiberscope when surveillancing patients with colorectal cancer in Korean population. Second primary cancers may develop even 10 years after the initial operation; thus, a need for lifelong surveillance even more than 5 years may be necessary.
