The temporal relationship between marriage and risk for suicidal ideation

BackgroundMarriage is consistently identified as a protective factor for suicidality, but it remains unclear whether this relationship varies by time elapsed since the transition to marriage. MethodsParticipants were 15,870 individuals (52 % female, mean age = 44.63 years, age range = 18–99 years) from the Collaborative Psychiatric Epidemiology Surveys. Cox proportional hazards models were used to test the relationship between marriage, as well as time elapsed since the transition to marriage (0–5 years, 6–10 years, or 11+ years), and suicidal ideation. Years of education and race and ethnicity were included as covariates, and analyses were stratified by sex. Separate hazard ratios were estimated for individuals aged <30 years and 30+ years to address violations of the proportionality assumption. ResultsBeing married was associated with lower risk for suicidal ideation across age and sex. Among individuals aged <30 years, marriage was reliably associated with lower risk for suicidal ideation, regardless of the time elapsed since marriage. For individuals aged 30+ years, being married for 0–5 years or 6–10 years was associated with increased risk for suicidal ideation, particularly in females. Being married for 11+ years was associated with decreased risk across sex. LimitationsAnalyses focused on participants' first marriage and did not examine mediators of the association between marriage and suicidal ideation. ConclusionsOverall, being married protects against suicidal ideation. However, among individuals aged 30 years or older, the first 10 years of marriage are associated with elevated risk for suicidal thoughts, and clinical outreach may be warranted.

Associations between Shyness and Victimization with Substance Use Outcomes in the National Comorbidity and Collaborative Psychiatric Epidemiological Surveys

Background : There is a lack of consensus among researchers on the association between shyness and substance use. This may be due to unexamined modifiers of this association, such as childhood victimization. Objective : The purpose of this study was to examine if experiencing different types of victimization (emotional, physical, sexual, and poly-victimization) modifies the association between shyness and substance use outcomes in adults. In this study, we performed moderation analyses to investigate whether victimization moderates the association between shyness and substance use/abuse. Data came from the National Comorbidity Survey Baseline (NCS-1; 1990-1992) and the Collaborative Psychiatric Epidemiological Surveys (CPES; 2001-2003). Substance use outcomes included were binge drinking, tobacco use, other drug use, and DSM-III-R (NCS-1)/DSM-IV (CPES) classifications of alcohol and drug abuse. Results : Results from NCS-1 supported a moderating role of childhood victimization on the relationship between shyness and tobacco use only, specifically for emotional (p = .031) and physical (p < .001) victimization, and poly-victimization (p < .001). Results from CPES showed a moderating role of lifetime sexual abuse for binge drinking (p = .017), other drug use (p = .028), and alcohol abuse (p = .004). For both datasets, the associations between shyness and substance use outcomes were stronger when there were no victimization histories. Conclusion : These findings give insight on the complexity of the interaction between shyness and victimization. Future research could focus on mechanisms, such as cognitive processes, that may contribute to interactions between shyness and victimization history on substance outcomes.

Clinical significance of psychotic-like experiences across U.S. ethnoracial groups.

Prevalence of psychotic-like experiences (PLEs) - reports of hallucinations and delusional thinking not meeting criteria for psychotic disorder - varies substantially across ethnoracial groups. What explains this range of PLE prevalence? Despite extensive research, the clinical significance of PLEs remains unclear. Are PLE prevalence and clinical severity differentially associated across ethnoracial groups? We examined the lifetime prevalence and clinical significance of PLEs across ethnoracial groups in the Collaborative Psychiatric Epidemiology Surveys (N = 11 139) using the Composite International Diagnostic Interview (CIDI) psychosis symptom screener. Outcomes included mental healthcare use (inpatient, outpatient), mental health morbidity (self-perceived poor/fair mental health, suicidal ideation or attempts), and impairment (role interference). Individuals with outcome onsets prior to PLE onset were excluded. We also examined associations of PLEs with CIDI diagnoses. Cox proportional-hazards regression and logistic regression modeling identified associations of interest. Contrary to previous reports, only Asian Americans differed significantly from other U.S. ethnoracial groups, reporting lower lifetime prevalence (6.7% v. 8.0-11.9%) and mean number (0.09 v. 0.11-0.18) of PLEs. In multivariate analyses, PLE clinical significance showed limited ethnoracial variation among Asian Americans, non-Caribbean Latinos, and Afro-Caribbeans. In other groups, mental health outcomes showed significant ethnoracial clustering by outcome (e.g. hospitalization and role interference with Caribbean-Latino origin), possibly due to underlying differences in psychiatric disorder chronicity or treatment barriers. While there is limited ethnoracial variation in U.S. PLE prevalence, PLE clinical significance varies across U.S. ethnoracial groups. Clinicians should consider this variation when assessing PLEs to avoid exaggerating their clinical significance, contributing to mental healthcare disparities.

An Intersectional Analysis of Women's Social Role Engagement and Mental Health.

Guided by role theory and the intersectionality framework, this study assesses whether social role volume, role type, and role configuration influence the mental health of Non-Latina White, African American, Afro-Caribbean, Mexican, Cuban, Puerto Rican, Chinese, Filipina, and Vietnamese American women. Contemporary shifts in the primary roles (i.e., worker, spouse, parent) women occupy and in the ethnic composition of the U.S. necessitate a re-examination of how roles impact U.S. women's mental health. Moreover, family member and friend roles are relatively understudied. Drawing data from the nationally representative Collaborative Psychiatric Epidemiology Surveys (CPES) (N=7370), ordinary least squares (OLS) regression analysis is used to assess the relationship between role volume, role type, role configuration, and mental health for women across nine ethnic groups. We report ethnicity-stratified models. On one hand, role accumulation was psychologically beneficial for Non-Latina White, African American, Puerto Rican, and Chinese women. On the other hand, the psychological benefits of social roles diminished after accumulating three social roles for Cuban, Mexican, and Filipina women. The psychological influence of specific roles and role configurations for women was dependent on ethnicity. This study demonstrates the powerful impact of ethnicity on social role engagement as well as the influence of such roles on women's psychological health.

Psychiatric disorders among employment requiring firearms.

Individuals who carry guns as a requirement of employment frequently experience hazards that can be stress inducing, violent, traumatizing, or cause personal injury. This study used data from the Collaborative Psychiatric Epidemiological Surveys (CPES; n=20,013), to examine mental health diagnoses of individuals that ever worked at a job requiring a firearm. Consistent with existing literature, the findings indicated that those who worked in professions requiring a firearm showed similar risk of mental health diagnoses as law enforcement officers which includes symptoms of trauma, mood disorders, and alcohol use. Further, race/ethnic differences emerged in patterns of mental health diagnoses, suggesting sociocultural differences influence diagnoses. These findings indicate the necessity for further investigation of the understudied area of mental health of those within employment positions that require firearms.

Early Age of Cannabis Initiation and Its Association With Suicidal Behaviors.

Background:Suicide rates in the U.S. have continued to rise over the last 2 decades. The increased availability and broader legalization of cannabis is a public health concern, particularly among adolescents. The objective of this study was to examine the association between the age of cannabis initiation and lifetime suicidal ideations and attempts in a sample of adults aged 18 or older.Methods:Data are from the Collaborative Psychiatric Epidemiology Surveys (CPES), 2001 to 2003 (N = 15 238). The primary objective of the CPES was to collect data about the prevalence of mental disorders, impairments associated with these disorders, and their treatment patterns from representative samples of majority and minority adult populations in the U.S. Logistic regression analyses were conducted to test the association between cannabis initiation age (early ⩽14 years old; later >14 years old) and outcomes of lifetime suicide ideation and attempts. Cigarette use, cannabis use, gender, income, race, education, and age were controlled for the analyses.Results:Overall, 12.5% of participants reported suicide ideation, while 4.2% reported attempt. Early cannabis use was associated with a higher risk of suicide ideation (AOR = 3.32, 95% CI [2.75, 3.80]) than later cannabis use (AOR = 2.15, 95% CI [1.92, 2.39]). Early cannabis use was associated with a higher risk of suicide attempt (AOR = 4.38, 95% CI [3.48, 5.52]) than later cannabis use (AOR = 2.56, 95% CI [2.14, 3.06]). Wald chi-squared tests revealed significant differences between the early and late initiation for both ideation (χ2 = 26.99; P < .001) and attempts (χ2 = 26.02; P < .001).Conclusions:Significant associations were found between early initiation of cannabis and suicide behaviors. As suicide rates continue to rise, it is recommended that clinicians, treatment providers, and other professionals consider the use of cannabis at an early age as a risk for subsequent suicide behaviors.

An epidemiological study of externally determined and self-identified mental and physical disability across the spectrum of bipolar disorders

Background: Studies exploring disability in bipolar disorder (BD) have primarily assessed clinical samples of full-threshold BD-I and BD-II alongside so-called objective criteria such as unemployment or receipt of government disability payments. This study extends research on disability by examining externally determined and self-identified disability in a community sample and by including subthreshold BD (BDS). Methods: Data were extracted from the USA Collaborative Psychiatric Epidemiology Surveys about individuals who met recognized criteria for BD-I, BD-II and BDS who had completed self-ratings of physical and mental disability, comorbidities, and health risk factors (e.g., obesity). Rates of disability were estimated, and logistic regression analyses were used to determine demographic and clinical variables associated with externally determined and self-identified of disability. Results: Of 408 individuals who met eligibility criteria (BD-I = 100; BD-II=104; BDS=204), 35% met criteria for externally determined disability, about 40% self-reported mental disability and about 23% self-reported physical disability. The odds were three-fold (Odds Ratio (OR): 3.05; 95% Confidence Intervals (CI): 1.69, 5.53) that someone with self-identified physical disability would meet criteria for externally determined disability, but associations with mental disability were non-significant (OR: 1.06; 95% CI: 0.63, 1.80). Regression analyses demonstrated that mental disability was associated with being a current or past smoker and physical disability was associated with BD-I. Limitations: the adequacy of the assessments of disability and definition of BDS can be questioned. Conclusions: Future clinical and community studies need to consider both externally determined and self-identified disability across the entire Bd spectrum.

Epidemiology of disturbing dreams in a diverse US sample

Study objectivesDisturbing dreams are a common complaint among trauma survivors, but little epidemiological research exists to document the frequency in the general population. In addition, information about their occurrence in diverse adults is lacking. MethodsUsing the Collaborative Psychiatric Epidemiology Surveys we had the unique opportunity to examine disturbing dreams in a diverse sample of adults in the US. Disturbing dreams were measured in three different variables including trauma dreams, dreams of the worst event, and dreams of separation. A fourth variable was created to assess for any disturbing dream reported across the three categories. ResultsDisturbing dream prevalence varied in the overall sample (N = 20,013) based on the assessment question. Dreams of trauma was 2.01% in the overall sample, dreams of the worst event was 7.21% and dreams of separation was 1.73%. Prevalence rates of all dream variables varied by racial and ethnic group membership, gender, and age. Odds ratios were also calculated for the three variables of interest by racial and ethnic group with significant differences emerging. Women had significantly higher rates of disturbing dreams in all variables assessed (p < 0.01). Older adults reported significantly lower rates of disturbing dreams compared to younger and middle-aged adults (p < 0.001). ConclusionsThis study establishes prevalence rates for disturbing dreams in a diverse sample of US adults. Understanding the prevalence can aid in reducing barriers to care for evidence-based treatments currently underutilized.

Early Age of Alcohol Initiation and its Association with Suicidal Behaviors

Objective The relationship between alcohol use and suicidal behaviors is well-accepted, but less is known about the contribution of its early initiation. This study was designed to test the association of early alcohol initiation versus later initiation with suicidal ideation and attempt in an ethnically diverse sample. Methods The Collaborative Psychiatric Epidemiology Surveys (CPES), 2001-2003 (n = 20,013), database was used. A total of 13,867 participants were selected included 56.9% females and 43.1% males. Race and ethnicity were reported as 28.8% non-Hispanic White, 39.1% Black, 20.3% Latino, and 11.9% Asian. Logistic regression analyses tested the associations between early (< =14 years) and later (> =15) age alcohol initiation with suicide ideation and attempts. Alcohol initiation was indexed by self-report of the first time that any alcohol product was consumed. Potential confounders were controlled. Results Early alcohol initiation was associated with higher odds (AOR = 3.64, 95% CI [2.51, 5.28]) of suicide ideation as compared with adults who had initiated > = age 15 (AOR = 2.11, 95% CI [1.46, 3.04]). Early age initiation was also associated with higher odds (AOR = 3.81, 95% CI [2.02, 7.18]) of lifetime suicide attempt versus later age initiators (AOR = 2.03, 95% CI [1.08, 3.79]). Significant differences were found between early and later age of initiation. Conclusion: Early age of alcohol initiation has profoundly increased odds of suicide ideation or attempt. It is critical that effective prevention programs for children and their caregivers be implemented to prevent or delay alcohol initiation and lessen the risk for future suicidal behaviors.

Prescription Drug Misuse among African Americans and Afro Caribbeans: Results from the National Survey of American Life

Background The misuse of prescription medications is an understudied topic among the non-Hispanic Black population in the U.S. Even less is known about the prevalence and correlates of misuse among African Americans and Afro-Caribbeans, despite documented differences in their substance use behaviors. Objectives We aimed to examine the prevalence and correlates of lifetime prescription drug misuse among African Americans and Afro Caribbeans using data from the National Survey of American Life. Methods A national household probability sample of non-institutionalized African Americans and Afro Caribbeans from the Collaborative Psychiatric Epidemiology Surveys was used. Pooled and ethnicity-stratified logistic regressions were estimated. Results Findings showed no significant differences between African Americans (6.36%) and Afro-Caribbeans (5.71%) in lifetime prescription drug misuse. Several correlates were found to be associated with misuse among only African American or Afro-Caribbean respondents. Conclusions While research has shown prescription drug misuse is an issue among Blacks overall, our results show that the role of ethnicity must not be overlooked, especially when tailoring prevention and intervention programs. Limitations of this study are discussed.

Aggressive outbursts among adults with major depressive disorder: Results from the Collaborative Psychiatric Epidemiological Surveys

IntroductionLittle is known about the epidemiology of aggressive outbursts, including physical assault, property destruction, and verbal aggression, among adults with unipolar Major Depressive Disorder (MDD). MethodsWe examined the prevalence and correlates of aggressive outbursts among adults with primary MDD (N = 2539) from the Collaborative Psychiatric Epidemiological Surveys (N = 20,013) using generalized linear models. ResultsThe prevalence estimate of any aggressive outbursts was 58.7% among adults with MDD. MDD was associated with aggressive outbursts independent of other psychiatric diagnoses. The prevalence of aggressive outbursts differed significantly by age, race/ethnicity, sex, education, marital status, and employment status, and psychiatric diagnoses. Aggressive outbursts were associated with greater severity and earlier age of onset for MDD. Positive associations were also found between the frequency of aggressive outbursts and depressive symptoms including weight/appetite change, fatigue, recurrent thoughts of death. Moreover, aggressive outbursts significantly multiplied the odds of experiencing more severe functional impairment, suicidal ideation, suicide plan, and suicide attempt. ConclusionAggressive outbursts are much more common than previously recognized among adults with MDD. The significant association of aggressive outbursts with MDD characteristics, increased functional impairment, and suicidal thoughts and behaviors make them an important target for assessment and treatment for adults with MDD.

Chronic Pain in Relation to Depressive Disorders and Alcohol Abuse

Chronic pain disorders have been associated separately with neuropsychiatric conditions such as depression and alcohol abuse. However, in individuals who suffer from non-cancer chronic pain disorders, it is not clear if the burden of depressive disorders is similar for those with and without a history of alcohol abuse. Using data from the Collaborative Psychiatric Epidemiology Surveys (CPES), we found depressive disorders to have a high burden in men and women with a history of alcohol abuse, independently of the presence or absence of chronic pain. We also found that, although the incidence of persistent depressive disorder was comparable in men and women with a history of alcohol abuse, and significantly higher than in control men and women, the incidence of a major depressive episode was higher in women with a history of alcohol abuse independently of the presence or absence of chronic pain. The age of onset of depressive disorders, independently of pain status, was younger for individuals with a history of alcohol abuse. The findings of this study have important implications for the clinical management of individuals who suffer from chronic pain comorbidly with depression and/or alcohol abuse.

Increased burden of cardiovascular risk among youth suicide attempters.

Suicide and cardiovascular disease rank among the leading causes of disability and premature mortality worldwide. Young adult suicide attempters are at increased risk of mortality from cardiovascular disease even compared to those with major depressive disorder suggesting an increased burden of cardiovascular risk factors. We compared the cardiovascular risk burden between youth attempters and other high-risk individuals. Participants were from the Collaborative Psychiatric Epidemiology Surveys (CPES), a U.S. population-based study, aged 18-30 years [suicide attempt (SA): n = 303; suicidal ideation (SI): n = 451; controls: n = 3671]; and psychiatric inpatients admitted for a SA (n = 38) or SI (n = 40) and healthy controls (n = 37) aged 15-30 years. We computed a cardiovascular risk score and high- and low-risk latent classes based on risk factors of high blood pressure, obesity, and smoking. Suicide attempters showed an increased cardiovascular risk score (CPES: B = 0.43, 95% confidence interval (CI) 0.31-0.54, p < 0.001; inpatient sample: B = 1.61, 95% CI 0.53-2.68, p = 0.004) compared to controls. They were also more likely to be classified in the high cardiovascular risk group (CPES: odds ratio (OR) 3.36, 95% CI 1.67-6.78, p = 0.001; inpatient sample: OR 9.89, 95% CI 1.38-85.39, p = 0.03) compared to those with SI (CPES: OR 1.15, 95% CI 0.55-2.39, p = 0.71; inpatient sample: OR 1.91, 95% CI 0.25-15.00, p = 0.53). Youth attempters show an increased burden for cardiovascular risk compared to other high-risk individuals in inpatient and population-based samples. Clinicians should pay particular attention to cardiovascular risk factors among suicide attempters in order to reduce their risk for cardiovascular events.

Hotline Use in the United States: Results from the Collaborative Psychiatric Epidemiology Surveys.

Crisis hotlines are a fixture in providing mental health services to individuals experiencing mental and behavioral problems in the United States (U.S.). Despite this, and the growing need for easily-accessible, anonymous, and free services amidst the suicide and opioid crises, there is no study reporting U.S. national prevalence and correlates of hotline use. Data on n = 18,909 participants from the Collaborative Psychiatric Epidemiology Surveys (CPES), a group of three nationally-representative, population-based studies, were used to estimate the prevalence of lifetime and past 12-month hotline use. A series of logistic regression models examined sociodemographic, clinical history and service use correlates of hotline use. Lifetime and past 12-month hotline use was estimated at 2.5% and 0.5%, respectively. Being female, having a mental or behavioral disorder, experiencing suicidality, or interacting with other formal and informal sectors of the mental health service system were significant correlates of use. This study provides the first national estimates of crisis hotline usage in the U.S. Hotlines are more likely to be used by certain sociodemographic subgroups, but these differences may be due to differing psychiatric history and service use patterns. Efforts should be made to ensure that crisis hotlines are being utilized by other marginalized populations at high risk of suicide or overdose amidst the current public health crises in the U.S., such as racial/ethnic minorities or youth. To evaluate the role that crisis hotlines play in the mental health service system, national surveys should aim to monitor trends and correlates over time.

Obesity, food insecurity, and depression among females

BackgroundNutritional psychiatry is an emerging field of research and it is currently exploring the impact of nutrition and obesity on brain function and mental illness. Prior studies links between obesity, nutrition and depression among women. However, less is known how food insecurity may moderate that relationship.MethodsData were employed from the Collaborative Psychiatric Epidemiology Surveys (CPES), 2001–2003. Two logistic regression models were Logistic regression was used to determine the association between obesity, gender, food insecurity, and past year Major Depressive Disorder (MDD). We then stratified by gender, and tested the association between obesity and past year MDD, and if food insecurity moderated the association.ResultsObesity was associated with an increased risk for past year Major Depressive Disorder (MDD) among females (AOR = 1.35; 95% CI 1.17–1.55) and was not associated among males (AOR = 1.07; 95% CI, 0.86–1.32). Women who reported that reported both obesity and food insecurity reported higher odds of past year MDD episode (AOR = 3.16; 95% CI, 2.36–4.21, than women who did not report food insecurity (AOR = 1.08; 95% CI, 1.02–1.38).ConclusionWith rising rates of mental health problems, females should be closely monitored to understand how poor diets, food insecurity, and obesity play a role in mental health outcomes. It is recommended that clinicians and treatment providers consider the patient’s diet and access to nutritious foods when conducting their assessment.

What Is the General Factor of Psychopathology? Consistency of the p Factor Across Samples.

Bifactor analyses have become a popular way to model a general psychopathology factor (or p factor) that accounts for correlations among higher order internalizing, externalizing, and thought-disorder dimensions. However, to advance dimensional nosology and construct validation of the p factor, this body of research needs to be synthesized. This study focuses on the p factor as a substantive construct by testing its consistency across samples and models. Bifactor analyses were run for three large epidemiological and clinical data sets: National Comorbidity Survey, Collaborative Psychiatric Epidemiology Surveys, and Methods to Improve Diagnostic Assessment and Services. Despite generally good model fit and strong factor congruence, interpretation of the p factor varied across samples and methods. This limited consistency suggests a lack of clear, generalizable operationalization of the p factor for applications such as measure development. Results are interpreted in terms of implications for the p factor as a substantive construct.

Reducing racial/ethnic disparities in mental health service use among emerging adults: community-level supply factors

ABSTRACT Objectives: Emerging adulthood—spanning 18–29 years of age—is associated with the highest risk for onset of certain behavioral health disorders (e.g. major depression, bipolar disorder, psychosis, substance use disorders) and high prevalence of many behavioral health disorders. Yet, rates of mental health service use remain low in this age range. Racial/ethnic minorities are particularly impacted by individual, cultural/linguistic, and community-level barriers to mental health care. This study examined community-level factors associated with mental health service use and investigated whether these associations varied by race/ethnicity. Design: This study analyzed individual- and county-level data for emerging adults in the United States (N=3,294) from the nationally representative Collaborative Psychiatric Epidemiological Surveys (CPES). Using the Andersen Model of Health Care Utilization, analyses examined predisposing, enabling, and need factors utilized in prior studies with adult samples as well as novel community characteristics hypothesized to impact service use among emerging adults of diverse racial/ethnic backgrounds. Past-year use of both specialty and any mental health services were assessed, controlling for individual- and community-level variables, and adjusting for presence of past-year mental health disorder, overall health status, and functional impairment. Differences between racial/ethnic minority groups and Non-Latino Whites were tested through a multilevel model incorporating random intercepts logistic regression, with analysis focusing on the interaction between race/ethnicity and community-level supply variables. Results: For past-year use of specialty mental health services, density of hospitals with child wellness programs was linked to service use among Black emerging adults, whereas density of hospitals with linguistic/translation services was linked to service use among Latino emerging adults. Conclusions: This study expands on previous research in behavioral health disparities to examine ways to improve behavioral health services for an emerging adult population with unmet service needs and identifies specific community-level factors that can improve mental health for racial/ethnic minority emerging adults.

Does the Everyday Discrimination Scale generate meaningful cross-group estimates? A psychometric evaluation.

Studies linking discrimination to poor mental and physical health constitute one of the most robust branches of health inequities research. For more than two decades, and in more than a dozen countries, scholars working in this field have used the Everyday Discrimination Scale (EDS) to assess perceptions of discrimination. Two recent studies (Harnois et al., 2020; Harnois et al., 2019) cast doubt on the instrument's psychometric equivalence across diverse social groups, however. Our study builds on these previous analyses using a larger and more ethnically and geographically diverse sample of adults in the US, the Collaborative Psychiatric Epidemiology Surveys. Multi-group Confirmatory Factor Analyses were carried out to compare the configural, metric, and scalar structures of the EDS. Analyzing perceptions of racial/ethnic discrimination, we find a lack of equivalence across race/ethnicity, consistent with previous research. Reports of general mistreatment are found to be equivalent across gender-based groups, but not across race/ethnicity, age- or education-based groups. Our study provides further evidence that the EDS should be used with caution, particularly when assessing general perceptions of discrimination, and particularly when making cross-group comparisons. Measurement invariance is required to effectively assess the relationship between discrimination and health; further refinement of the scale may be needed to achieve this goal.

Lifetime patterns of comorbidity in eating disorders: An approach using sequence analysis.

Eating disorders (EDs) have high rates of psychiatric comorbidity. This study aimed to characterize longitudinal patterns of comorbidities in adults with EDs. Sequence analysis and hierarchical clustering were applied to ages of onset and recency for select eating, substance, mood, and anxiety disorders from the 479 participants in the Collaborative Psychiatric Epidemiology Surveys with lifetime DSM-IV bulimia nervosa, binge eating disorder, or anorexia nervosa. External validators were compared across clusters using chi-square tests. Five clusters were identified among individuals with any lifetime ED based on longitudinal sequence of psychiatric disorder onset and remission, characterized as: (1) multi-comorbid with early onset of comorbid disorder (46%); (2) moderate preeminent anxiety with moderate comorbidity and low ED persistence (20%); (3) late ED onset with low comorbidity (15%); (4) early onset, persistent ED with low comorbidity (14%); and (5) chronic, early onset depression (5%). Clusters were well differentiated by significant differences in age, body mass index, race, and psychiatric indicators. This study demonstrates a new method to assess clustering of comorbidity among individuals with lifetime EDs. Having a psychiatric diagnosis prior to an ED was associated with greater psychopathology and illness duration. Information on timing of diagnoses may allow for more refined comorbidity classification.

Can Prescription Drug Use Disorder Predict Suicidality in US Adults With Chronic Pain? A Pilot Study Based on Collaborative Psychiatric Epidemiological Surveys.

Individuals with chronic pain are at higher risk for suicide than other populations, and their potential for prescription drug use disorder (PDUD) enormously exists. This study aimed to estimate the prevalence and correlates of suicidality and to determine its relation to PDUD, among US adults with a history of chronic pain. Data were obtained from the Collaborative Psychiatric Epidemiology Surveys (2001-2003). This analysis was limited to respondents who had a history of chronic pain (N = 5301). Logistic regression was used to determine the association between suicidality and prescription drug use disorder among those populations. Among study respondents, there was a strong positive association between PDUD and suicidal ideation and attempts (P value < 0.001, = 0.006, respectively). Adjusted logistic regression analysis showed those with PDUD had 1.5 times greater likelihood of suicidal ideation (95% CI 1.2-1.8). Among respondents with a history of chronic pain and PDUD, Non-Hispanic Whites, female gender, lifetime mood and/or anxiety, and substance use disorders were associated with increased suicide risk (P value = 0.020, = 0.011, < 0.001, < 0.001, respectively). There is a robust relationship between PDUD and suicidality in US adults with a history of chronic pain. Ethnicity, gender, and comorbid mood, anxiety or substance use disorders can predict suicidality among individuals with both chronic pain and PDUD. Longitudinal studies need to investigate the causal relationship between PDUD and suicidality among those populations.
