Facial Disseminated Superficial Actinic Porokeratosis

Porokeratosis is a disorder of keratinization and is genetically trans mitted. It is characterized by one or more atrophic macules or patches and surrounded by a distinctive hyperkeratotic ridge-like border. Multiple clinical variants of porokeratosis exist. It is histologically characterized by the presence of a cornoid lamella. The cornoid lamella is due to an expanding number of unusual keratinocytes. Here, we present a case report of poro kera tosis over the nose in a 22-year-old female.

Treatment of disseminated superficial actinic porokeratosis with topical diclofenac gel: a case series

Disseminated superficial actinic porokeratosis (DSAP) is the most common of the of five clinical variants of porokeratosis. These are disorders of keratinization and the distinctive pathological feature is the cornoid lamella at the margin. DSAP usually manifests in the third or fourth decades of life with a female preponderance and with multiple lesions over sun-exposed areas. A diverse range of treatments is employed though evidence of efficacy remains largely anecdotal. We report a series of eight patients with DSAP treated with 3% diclofenac gel (Solaraze gel).

There are several reports of porokeratosis in the context of immmunosuppressive diseases. These mainly include organ transplant, HIV infection, lymphomas and some inflammatory and autoimmune diseases commonly treated with immunosuppresive drugs or chemotherapy. Disseminated superficial actinic porokeratosis is the clinical variant of porokeratosis that most frequently develops in immunosuppressive states. We report a case of porokeratosis in a woman with dermatomyositis.

Sudden onset of disseminated porokeratosis of Mibelli in a renal transplant patient

Porokeratosis is a disorder of epidermal keratinization of uncertain cause. Five clinical variants of porokeratosis have been described. These include porokeratosis of Mibelli, punctate porokeratosis, linear porokeratosis, porokeratosis palmaris plantaris et disseminata, and disseminated superficial porokeratosis. Disseminated superficial porokeratosis and single plaque porokeratosis of Mibelli have each been documented to occur in association with immunosuppression. To our knowledge, only 5 cases of disseminated porokeratosis of Mibelli in transplant recipients have been reported. We present a patient who developed explosive onset of disseminated porokeratosis of Mibelli shortly after renal transplantation. It is important to differentiate this unusual variety of porokeratosis from other cutaneous manifestations in transplant patients so that appropriate therapy can be instituted. Ten cases of porokeratosis (Mibelli) seen during the last year are reviewed. The disease may assume different clinical forms. Lesions may be faint or prominent, small or extensive, atrophic or hyperplastic, hyperpigmented or hypopigmented, asymptomatic or pruritic. They may simulate other dermatoses like lichen planus, verrucae, actinic keratoses, epithelial nevi and carcinoma. The demonstration of the cornoid lamella in histologic sections, however, is diagnostic. The results of histochemical studies showed that the cornoid lamella is composed of parakeratotic epidermal cells. These findings and the occurrence of buccal lesions in one of the patients tend to refute the concept that the eccrine sweat duct unit is the only primary site of involvement in this disease.
