A suitable procedure of dose reduction factor measurements of X-ray shields during computed tomography examination - The importance of considering positional changes of an X-ray tube

During computed tomography (CT) examinations, radio-sensitive organs located outside the field of view (FOV) are usually exposed to radiation caused by both direct and scattered X-rays. Traditionally, the use of radiation protection products has been an option for reducing exposure doses. The catalog value of the dose reduction factor (DRF) for a commercial X-ray shield is generally determined by the manufacturer using a plain X-ray irradiation system to measure the difference in dose with and without the shield. In contrast, actual shielding ability measurements during clinical CT examinations are usually evaluated using a similar equation, but it is not guaranteed that the incident directions of X-rays will be the same between the measurement data with and without a shield. The purposes of this study are to introduce a novel method for evaluating DRF by accounting for the influence of the X-ray incident direction in helical scanning and to obtain knowledge about the correctness of previously reported DRF values. Experiments pertaining to chest CT examinations were performed using a human body phantom and small-type dosimeters which consist of an optically stimulated luminescence element using Al2O3:C. A total of 100 examinations were iteratively performed, both with and without a shield. The corresponding DRF was then calculated. The novelty of the proposed analysis procedure lies in a paired analysis method, where the DRF is calculated by reorganizing numerous acquired data sets with and without the shield to pair similar incident angles of X-rays. As a result, the DRF values were estimated to be 48.5 ± 4.3%, 48.7 ± 2.7%, and 48.9 ± 1.5% based on the data results within the 10–40 data sets, 40–70 data sets, and 70–100 data sets, respectively. When the DRF values were obtained by the conventional method without applying our sorting procedure, the results were 48.4 ± 8.1%, even when the 70–100 data sets were used. This result indicates that the proposed method can analyze DRF with higher accuracy. In the example of thyroid X-ray shield, while there was no significant difference in the most probable values of DRF between the conventional and proposed methods coincidentally, we believe it is important to evaluate DRF based on a scientifically correct approach because the mathematical formulas for calculating these two values differ. In conclusion, when calculating the DRF of shielding materials during CT examination, it is imperative that data analysis considers the significant impact of differences in incident X-ray directions on the measured exposure dose data.

Impact of Sarcopenia on Survival in Patients Treated with FOLFIRINOX in a First-Line Setting for Metastatic Pancreatic Carcinoma.

Sarcopenia, defined as decreased muscle mass and strength, can be evaluated by a computed tomography (CT) examination and might be associated with reduced survival in patients with carcinoma. The prognosis of patients with metastatic pancreatic carcinoma is poor. The FOLFIRINOX (a combination of 5-fluorouracil, irinotecan, and oxaliplatin) chemotherapy regimen is a validated first-line treatment option. We investigated the impact of sarcopenia on overall survival (OS) and progression-free survival (PFS) in patients with metastatic pancreatic carcinoma. Clinical data and CT examinations of patients treated with FOLFIRINOX were retrospectively reviewed. Sarcopenia was estimated using baseline CT examinations. Seventy-five patients were included. Forty-three (57.3%) were classified as sarcopenic. The median OS of non-sarcopenic and sarcopenic patients were 15.6 and 14.1 months, respectively (p = 0.36). The median PFS was 10.3 in non-sarcopenic patients and 9.3 in sarcopenic patients (p = 0.83). No differences in toxicity of FOLFIRINOX were observed. There was a trend towards a higher probability of short-term death (within 4 months of diagnosis) in sarcopenic patients. In this study, the detection of sarcopenia failed to predict a longer OS or PFS in selected patients deemed eligible by a physician for triplet chemotherapy and receiving the FOLFIRINOX regimen in a first-line setting, confirming the major importance of a comprehensive patient assessment by physicians in selecting the best treatment option.

Predicting Infection Related Complications After Endovascular Repair of Infective Native Aortic Aneurysms

Endovascular aortic repair (EVAR) as surgical treatment for infective native aortic aneurysm (INAA) is associated with superior survival compared with open surgery, but with the risk of infection related complications (IRCs). This study aimed to assess the association between baseline clinical and computed tomography (CT) features and the risk of post-operative IRCs in patients treated with EVAR for INAA. It also sought to develop a model to predict long term IRCs in patients with abdominal INAA treated with EVAR. All initial clinical details and CT examinations of INAAs between 2005 and 2020 at a major referral hospital were reviewed retrospectively. The images were scrutinised according to aneurysm features, as well as peri-aortic and surrounding organ involvement. Data on post-operative IRCs were found in the patient records. Cox regression analysis was used to derive predictors for IRCs and develop a model to predict five year IRCs after EVAR in abdominal INAA. Of 3 780 patients with the diagnosis of aortic aneurysm or aortitis, 98 (3%) patients were treated with EVAR for abdominal INAAs and were thus included. The mean follow up time was 52 months (range 0 ‒ 163). The mean transaxial diameter was 6.5 ± 2.4 cm (range 2.1 ‒14.7). In the enrolled patients, 38 (39%) presented with rupture. The five year IRC rate in abdominal INAAs was 26%. Female sex, renal insufficiency, positive blood culture, aneurysm diameter, and psoas muscle involvement were predictive of five year IRC in abdominal INAA after EVAR. The model had a C-index of 0.76 (95% CI 0.66 - 0.87). Pre-operative clinical and CT features have the potential to predict IRC after endovascular aortic repair in INAA patients. These findings stress the importance of rigorous clinical, laboratory, and radiological follow up in these patients.

340P Prediction of relapse in colon cancer patients by machine learning models combining radiomics and deep features extracted from baseline computed tomography

Up to 40% of localized colon cancer (CC) patients relapse despite optimal initial treatment. Circulating tumor DNA has emerged as a new prognostic biomarker in this setting. However, liquid biopsy tests provide a limited accuracy in the early prediction of relapse. On this basis, radiomics and artificial intelligence (AI) are posed to provide relevant insights in the prediction of relapse and patient outcome. Here we propose a predictive model of CC recurrence using clinical data in combination with radiomics and deep features extracted from baseline computed tomography (CT) images. A single-center retrospective observational study was designed. Real-world clinical data and CT examinations were collected from 2015 to 2017. Manual segmentation of the tumor was performed slice by slice by a radiologist with 10+ years of experience in CC using ITK-SNAP. Feature extraction techniques were applied to the tumor voxels through an analysis pipeline in Python adapted from Quibim Precision® software (Quibim, Valencia, ES). Feature reduction techniques were applied to select the characteristics providing independent information. The types of features extracted were: Radiomics, Deep features and fractal dimension. Several classifiers were trained with the clinical and image features as the input, to predict relapse from CC. Relapse and non-relapse classes were balanced to the number of slices containing tumor. Train-validation data ratio was 70:30. Baseline CT exams from 60 localized CC patients were included. 48% and 20% of them had stage III and high-risk stage II, respectively. The remaining patients were diagnosed with low-risk CC. 36.7% of patients relapsed. Merging the clinical data with the radiomics, fractal and deep features provided a high and increased accuracy 95% (CI 95%: 85 – 100) when compared to clinical variables alone (55%, CI 95%: 32 – 78). Random Forest and cross validation model provided the best performance. The extraction of radiomics and deep features from CT exams in CC patients and their combination in an AI model provide promising insights towards the early prediction of relapse, leading to identify new and accurate prognostic imaging panels.

A pilot study of clinical cell therapy for patients with vascular dementia

Background:Vascular dementia (VD) is a series of clinical and neurophysiological manifestations caused by cerebrovascular disease. As the human lifespan increases, the number of people affected by age-related dementia is growing at an alarming pace, but no proved therapeutic methods can stop it from getting worse.Objective:To investigate the neurorestorative effects of injecting olfactory ensheathing cells (OECs), Schwann cells (SCs), and olfactory receptor neurons (ORNs) into olfactory sub-mucosa in VD patients.Methods:A pilot study of double-blind randomized controlled cell therapies was conducted in VD patients (n = 5). Cells were injected into the patients’ olfactory sub-mucosa. Two patients received OEC treatment, one received SC treatment, one ORN treatment, and one OEC combined with ORN. Mental state and cognitive function were observed before treatment and 1, 3, 6, and 12 months after treatment. magnetic resonance imaging (MRI) or computed tomography (CT) was performed before treatment and 12 months after treatment.Results:The directional function score on the Mini-Mental Status Examination (MMSE) in the patient who received SC treatment had increased slightly 1 and 3 months after treatment. The scores for orientation, attention, delayed verbal recall, and repetition increased in the ORN group patient 1 month after treatment. The orientation and repetition scores of the ORN group patient continued to increase 3 months after treatment. The scores for attention, delayed verbal recall, and phase 3 command decreased in the OEC and the OEC + ORN group patients after treatment assessment Scores on the Montreal Cognitive Assessment (MoCA) and Clinical Dementia Rating (CDR) scale also improved in the ORN group patient. Clinical and MRI or CT examinations did not find any side effects from the cell therapy or transplanting procedure.Conclusion:All of the cell transplantations were found to be safe. ORN was shown to be a promising therapy for VD patients. Phase II clinical trials of ORN, SC, and OEC therapy are required to verify their effects on VD symptoms, especially ORNs.

Development of CT Effective Dose Conversion Factors from Clinical CT Examinations in the Republic of Korea

The aim of this study was to determine the conversion factors for the effective dose (ED) per dose length product (DLP) for various computed tomography (CT) protocols based on the 2007 recommendations of the International Commission on Radiological Protection (ICRP). CT dose data from 369 CT scanners and 13,625 patients were collected through a nationwide survey. Data from 3793 patients with a difference in height within 5% of computational human phantoms were selected to calculate ED and DLP. The anatomical CT scan ranges for 11 scan protocols (adult-10, pediatric-1) were determined by experts, and scan lengths were obtained by matching scan ranges to computational phantoms. ED and DLP were calculated using the NCICT program. For each CT protocol, ED/DLP conversion factors were calculated from ED and DLP. Estimated ED conversion factors were 0.00172, 0.00751, 0.00858, 0.01843, 0.01103, 0.02532, 0.01794, 0.02811, 0.02815, 0.02175, 0.00626, 0.00458, 0.00308, and 0.00233 mSv∙mGy−1∙cm−1 for the adult brain, intra-cranial angiography, C-spine, L-spine, neck, chest, abdomen and pelvis, coronary angiography, calcium scoring, aortography, and CT examinations of pediatric brain of <2 years, 4–6 years, 9–11 years, and 13–15 years, respectively. We determined ED conversion factors for 11 CT protocols using CT data obtained from a nationwide survey in Korea and Monte Carlo-based dose calculations.

In situ identification of Action 4, SECA and QD-PEP bullets from special police ammunitions by computed tomography.

Special deformation bullets were developed for police forces to achieve a defined penetration depth (avoiding over-penetration) and a controlled energy transfer (avoiding collateral damage). This article focuses on Action 4, SECA and QD-PEP bullets. These 9 mm bullets were specially designed for controlled deformation. The characteristic slight mushrooming with a front cross-section of approximately 11.5 mm after entering a ballistic simulant was verified in ballistic tests. To achieve such slight mushrooming, the projectile's core is hollowed. The purpose of this study was to investigate the feasibility of visualising the hollowed cores of Action 4, SECA and QD-PEP bullets using a standard clinical computed tomography (CT) scanner for non-invasive identification of these special bullets from police ammunitions. First, undeformed specimens were scanned to reveal the shape of the hollowed core of each type of special bullet. Second, Action 4, SECA and QD-PEP bullets were fired towards animal cadaver models to visualise their hollow core after deformation inside biological tissue. Third, two reviewers were tasked with identifying special bullets from police ammunition (Action 4 bullets: n = 3) among 10 CT examinations of humans with lodged projectiles who were selected by the supervisor of the study. The CT scans of the undeformed specimens revealed the special design of the bullets' metal core. All special bullets from police ammunitions that were fired towards an animal cadaver model demonstrated the characteristic slight mushrooming. In accordance with the CT scans of the undeformed bullets, visualisation of the individual internal cavities of the special bullets allowed the Action 4, SECA and QD-PEP bullets to be clearly distinguished. With regard to the real forensic cases, both reviewers clearly identified each of the three Action 4 bullets among all other lodged projectiles. This study demonstrates the feasibility of identifying Action 4, SECA and QD-PEP bullets from special police ammunitions by CT. The individual shapes of the cavity inside the bullets were clearly visible on CT. In situ identification of these bullets can aid in the assessment of injuries, and since these bullets are fabricated from non-ferromagnetic metals, their clear identification allows for magnetic resonance imaging (MRI) without the risk of bullet movement inside the body due to the magnetic pull of the MRI unit. Furthermore, this approach could be of great interest to forensic investigators if patients who received gunshot wounds underwent non-operative treatments and the projectile remains in the body. Since the use of CT is also increasing for medico-legal post-mortem examinations, the identification of lodged projectiles is of interest for a virtual autopsy or 'Virtopsy'.

Assessment of uncertainties associated with Monte Carlo-based personalized dosimetry in clinical CT examinations

The clinical value of x-ray computed tomography (CT) has skyrocketed in the last decade while at the same time being the main source of medical exposure to the population. Concerns regarding the potential health hazards associated with the use of ionizing radiation were raised and an appropriate estimation of absorbed dose to patients is highly desired. In this work, we aim to validate our developed Monte Carlo CT simulator using in-phantom dose measurements and further assess the impact of personalized scan-related parameters on dosimetric calculations. We developed a Monte Carlo-based CT simulator for personalized organ level dose calculations, in which the CT source model, patient-specific computational model and personalized scanning protocol were integrated. The CT simulator was benchmarked using an ionization chamber and standard CT Dose Index phantom while the dosimetry methodology was validated through experimental measurements using thermoluminescent dosimeters (TLDs) embedded within an anthropomorphic phantom. Patient-specific scan protocols extracted from CT raw data and DICOM image metadata, respectively, were fed as input into the CT simulator to calculate individualized dose profiles. Thereby, the dosimetric uncertainties associated with using different protocol-related parameters were investigated. The absolute absorbed dose difference between measurements and simulations using the ionization chamber was less than 3%. In the case of the anthropomorphic phantom, the absolute absorbed dose difference between simulations and TLD measurements ranged from −8.3% to 22%, with a mean absolute difference of 14% while the uncertainties of protocol-related input parameters introduced an extra absolute error of 15% to the simulated results compared with TLD measurements. The developed methodology can be employed for accurate estimation of organ level dose from clinical CT examinations. The validated methodology can be further developed to produce an accurate MC simulation model with a reduced computational burden.

Full Length Spine CT and MRI

Spine is a complex and long structure in the human body. Visualization of the spine is essential to treat and manage spine disease and commonly requires further imaging modalities such as computed tomography (CT) and magnetic resonance imaging (MRI). In most clinical fields, spine CT and MRI examinations are focused on the region of interest. However, spine is composed of cervical, thoracic, lumbar, sacrum and coccyx and sometimes demands examination of entire structure as well as regional spine depending on disease, patient’s state and physician’s decision. This review considers the available literature to describe when and how full length spine evaluation by CT and MRI is applied according to each spinal disease such as spinal trauma, deformity, infection, axial spondyloarthropathy and metastatic tumor.

Validation of algorithmic CT image quality metrics with preferences of radiologists

Automated assessment of perceptual image quality on clinical Computed Tomography (CT) data by computer algorithms has the potential to greatly facilitate data-driven monitoring and optimization of CT image acquisition protocols. The application of these techniques in clinical operation requires the knowledge of how the output of the computer algorithms corresponds to clinical expectations. This study addressed the need to validate algorithmic image quality measurements on clinical CT images with preferences of radiologists and determine the clinically acceptable range of algorithmic measurements for abdominal CT examinations. Algorithmic measurements of image quality metrics (organ HU, noise magnitude, and clarity) were performed on a clinical CT image dataset with supplemental measures of noise power spectrum from phantom images using techniques developed previously. The algorithmic measurements were compared to clinical expectations of image quality in an observer study with seven radiologists. Sets of CT liver images were selected from the dataset where images in the same set varied in terms of one metric at a time. These sets of images were shown via a web interface to one observer at a time. First, the observer rank ordered the CT images in a set according to his/her preference for the varying metric. The observer then selected his/her preferred acceptable range of the metric within the ranked images. The agreement between algorithmic and observer rankings of image quality were investigated and the clinically acceptable image quality in terms of algorithmic measurements were determined. The overall rank-order agreements between algorithmic and observer assessments were 0.90, 0.98, and 1.00 for noise magnitude, liver parenchyma HU, and clarity, respectively. The results indicate a strong agreement between the algorithmic and observer assessments of image quality. Clinically acceptable thresholds (median) of algorithmic metric values were (17.8, 32.6) HU for noise magnitude, (92.1, 131.9) for liver parenchyma HU, and (0.47, 0.52) for clarity. The observer study results indicated that these algorithms can robustly assess the perceptual quality of clinical CT images in an automated fashion. Clinically acceptable ranges of algorithmic measurements were determined. The correspondence of these image quality assessment algorithms to clinical expectations paves the way toward establishing diagnostic reference levels in terms of clinically acceptable perceptual image quality and data-driven optimization of CT image acquisition protocols.

22. Impact of iterative reconstructions on image quality in clinical CT images demonstrated by a novel noise power spectrum measurement tool

Introduction To quantify noise texture and magnitude characteristics of clinical Computed Tomography (CT) images obtained with Iterative Reconstructions (IR) techniques. Methods An algorithm was developed to extract noise texture and magnitude metrics from 10 chest and abdomen clinical CT-scans (Discovery CT750 HD; GE Healthcare, Wisconsin) performed for oncology follow-up by measuring a global Noise Power Spectrum (NPS). After subtraction of adjacent slices, a segmentation algorithm was applied to exclude remaining anatomical structures. Noise magnitude (i.e. area under the 1D-NPS curve) and peak frequency (fpeak) were measured and compared for chest and abdomen on image subsets of all CT scans reconstructed using Filtered Back Projection (FBP), Adaptive Statistical Iterative Reconstruction 30% (ASIR30), 50% (ASIR50) and Model-Based Iterative Reconstruction (MBIR). Results Compared to FBP, ASIR30 and ASIR50, MBIR reduced noise magnitude by 31%, 17% and 13% in chest and by 47%, 34% and 29% in abdomen respectively ( p −1 respectively), ASIR50 (0.13 and 0.14 mm −1 respectively) and ASIR30 (0.16 and 0.18 mm −1 respectively) compared to FBP (0.23 and 0.27 mm −1 respectively; p Conclusions Assessing NPS of clinical CT examinations can demonstrate the reduction of noise magnitude and the changes of noise texture associated to IR. This method could be used to tailor CT protocols according to radiologists’ preferences regarding noise texture and magnitude.

Traumatic brain injury: Comparison between autopsy and ante-mortem CT

PurposeThe aim of this study was to compare pathological findings after traumatic brain injury between autopsy and ante-mortem computed tomography (CT). A second aim was to identify changes in these findings between the primary posttraumatic CT and the last follow-up CT before death. MethodsThrough the collaboration between clinical radiology and forensic medicine, 45 patients with traumatic brain injury were investigated. These patients had undergone ante-mortem CT as well as autopsy. During autopsy, the brain was cut in fronto-parallel slices directly after removal without additional fixation or subsequent histology. Typical findings of traumatic brain injury were compared between autopsy and radiology. Additionally, these findings were compared between the primary CT and the last follow-up CT before death. ResultsThe comparison between autopsy and radiology revealed a high specificity (≥80%) in most of the findings. Sensitivity and positive predictive value were high (≥80%) in almost half of the findings. Sixteen patients had undergone craniotomy with subsequent follow-up CT. Thirteen conservatively treated patients had undergone a follow-up CT. Comparison between the primary CT and the last ante-mortem CT revealed marked changes in the presence and absence of findings, especially in patients with severe traumatic brain injury requiring decompression craniotomy. ConclusionThe main pathological findings of traumatic brain injury were comparable between clinical ante-mortem CT examinations and autopsy. Comparison between the primary CT after trauma and the last ante-mortem CT revealed marked changes in the findings, especially in patients with severe traumatic brain injury. Hence, clinically routine ante-mortem CT should be included in the process of autopsy interpretation.

Advanced Knee Structure Analysis (AKSA): a comparison of bone mineral density and trabecular texture measurements using computed tomography and high-resolution peripheral quantitative computed tomography of human knee cadavers

BackgroundA change of loading conditions in the knee causes changes in the subchondral bone and may be a cause of osteoarthritis (OA). However, quantification of trabecular architecture in vivo is difficult due to the limiting spatial resolution of the imaging equipment; one approach is the use of texture parameters. In previous studies, we have used digital models to simulate changes of subchondral bone architecture under OA progression. One major result was that, using computed tomography (CT) images, subchondral bone mineral density (BMD) in combination with anisotropy and global homogeneity could characterize this progression.The primary goal of this study was a comparison of BMD, entropy, anisotropy, variogram slope, and local and global inhomogeneity measurements between high-resolution peripheral quantitative CT (HR-pQCT) and CT using human cadaveric knees. The secondary goal was the verification of the spatial resolution dependence of texture parameters observed in the earlier simulations, two important prerequisites for the interpretation of in vivo measurements in OA patients.MethodThe applicability of texture analysis to characterize bone architecture in clinical CT examinations was investigated and compared to results obtained from HR-pQCT. Fifty-seven human knee cadavers (OA status unknown) were examined with both imaging modalities. Three-dimensional (3D) segmentation and registration processes, together with automatic positioning of 3D analysis volumes of interest (VOIs), ensured the measurement of BMD and texture parameters at the same anatomical locations in CT and HR-pQCT datasets.ResultsAccording to the calculation of dice ratios (>0.978), the accuracy of VOI locations between methods was excellent. Entropy, anisotropy, and global inhomogeneity showed significant and high linear correlation between both methods (0.68 < R2 < 1.00). The resolution dependence of these parameters simulated earlier was confirmed by the in vitro measurements.ConclusionThe high correlation of HR-pQCT- and CT-based measurements of entropy, global inhomogeneity, and anisotropy suggests interchangeability between devices regarding the quantification of texture. The agreement of the experimentally determined resolution dependence of global inhomogeneity and anisotropy with earlier simulations is an important milestone towards their use to quantify subchondral bone structure. However, an in vivo study is still required to establish their clinical relevance.

Organ-based tube current modulation: are women's breasts positioned in the reduced-dose zone?

To retrospectively determine the potential of organ-based tube current modulation ( OBTCM organ-based tube current modulation ) to reduce the radiation dose delivered to breast tissue by computed tomography (CT) by determining breast angular position in relation to the zones of decreased versus increased radiation. The authors obtained institutional review board approval for this study and patients' written informed consent. In two academic centers (center A: Beth Israel Deaconess Medical Center, Boston, Mass; and center B: Hôpital André Vésale, Montignies-le-Tilleul, Belgium), data were collected from clinical thoracic CT examinations performed in 498 women (mean age, 60 years; age range, 18-95 years) in the supine position and 34 women (mean age, 53 years; age range, 18-84 years) in the prone position. One radiologist in each center determined breast tissue location and measured its inner and outer boundaries with respect to the isocenter of the CT examination. The percentages of women with breast tissue within and those with breast tissue outside the zone of decreased radiation delivered by OBTCM organ-based tube current modulation were determined. The location of breast tissue was correlated with patient age and with sagittal and coronal diameters of the thorax by using the Student t test, Fisher exact test, and Pearson correlation. None of the women lying in the supine position had the entirety of the breast tissue located within the reduced-dose zone. Breast tissue was located in the increased-dose zone in 99% of women lying supine and in 82% of women lying prone. The breast angular position of almost all women was higher than the angular limit of the reduced versus the increased dose in OBTCM organ-based tube current modulation . No woman, regardless of supine or prone position, had all breast tissue within the reduced-dose zone.

Measurement of Single Scan Dose Profiles in CT Using a Commercial Photodiode

Computed tomography (CT) is a significant source of medical X-ray for the general population. As an alternative method to the 100 mm long ionization chamber, which is normally used to measure the CT dose index (CTDI), some studies use smaller semiconductor photodetectors to measure the single scan dose profile (SSDP). In this case, the CT dose profiles are obtained by scanning the device through the beam using multiple rotations of the X-ray source. Some advantages of using electronic devices as radiation detectors include their high sensitivity, small physical size, no need for high voltage, and the capability of real time measurements. In this study the SSDP was obtained by using a photodiode in two ways: a detector built with only one device, and an innovative detector based on a time-multiplexing signal from an array of 31 photodiodes. Moreover, since most clinical CT examinations involve multiple scans as the patient is translated through the gantry, the novel detector was also used to obtain the multiple scan average dose (MSAD). Unprecedented results provide details of the single slice dose profile and MSAD curves, showing that such an innovative detector would be an advance in CT dosimetry and quality assurance.

Inner-focusing reconstruction method for grating-based phase-contrast CT

Grating-based phase-contrast imaging has been a hot topic for several years due to its excellent imaging capability for low-density materials and easy implementation with a laboratory x-ray source. Compared with traditional x-ray computed tomography (CT) systems, the standard data collection procedure, "phase-stepping" (PS), in the grating-based phase-contrast CT (GPC-CT) is time consuming. The imaging time of a GPC-CT scan is usually up to hours. It is unacceptable in clinical CT examinations, and will cause serious motion artifacts in the reconstructed images. Additionally, the radiation dose delivered to the object with the PS-based GPC-CT is several times larger than that by a conventional CT scan. To address these problems, in this paper, we followed the interlaced PS method and proposed a novel image reconstruction method, namely the inner-focusing (IF) reconstruction method. With the interlaced PS method, the sample rotation and the grating stepping in GPC-CT occur at the same time. Thus, the interlaced GPC-CT scan can have a comparable temporal resolution with existing CT systems. Without any additional requirements, the proposed IF reconstruction method can prevent the artifacts existing in the conventional interlaced PS method. Both numerical simulations and real experiments were carried out to verify the proposed IF reconstruction method. And the results demonstrated it was effective in archiving a fast and low-dose GPC-CT.

Comparison of pure and hybrid iterative reconstruction techniques with conventional filtered back projection: Image quality assessment in the cervicothoracic region

ObjectivesTo evaluate the impact on image quality of three different image reconstruction techniques in the cervicothoracic region: model-based iterative reconstruction (MBIR), adaptive statistical iterative reconstruction (ASIR), and filtered back projection (FBP). MethodsForty-four patients underwent unenhanced standard-of-care clinical computed tomography (CT) examinations which included the cervicothoracic region with a 64-row multidetector CT scanner. Images were reconstructed with FBP, 50% ASIR-FBP blending (ASIR50), and MBIR. Two radiologists assessed the cervicothoracic region in a blinded manner for streak artifacts, pixilated blotchy appearances, critical reproduction of visually sharp anatomical structures (thyroid gland, common carotid artery, and esophagus), and overall diagnostic acceptability. Objective image noise was measured in the internal jugular vein. Data were analyzed using the sign test and pair-wise Student's t-test. ResultsMBIR images had significant lower quantitative image noise (8.88±1.32) compared to ASIR images (18.63±4.19, P<0.01) and FBP images (26.52±5.8, P<0.01). Significant improvements in streak artifacts of the cervicothoracic region were observed with the use of MBIR (P<0.001 each for MBIR vs. the other two image data sets for both readers), while no significant difference was observed between ASIR and FBP (P>0.9 for ASIR vs. FBP for both readers). MBIR images were all diagnostically acceptable. Unique features of MBIR images included pixilated blotchy appearances, which did not adversely affect diagnostic acceptability. ConclusionsMBIR significantly improves image noise and streak artifacts of the cervicothoracic region over ASIR and FBP. MBIR is expected to enhance the value of CT examinations for areas where image noise and streak artifacts are problematic.

Hounsfield Units for Assessing Bone Mineral Density and Strength: A Tool for Osteoporosis Management

Measurements obtained from clinical computed tomography examinations may yield information leading to the diagnosis of decreased bone mineral density, without added expense to the patient. The purpose of the present study was to determine if Hounsfield units, a standardized computed tomography attenuation coefficient, correlate with bone mineral density and compressive strength. Twenty-five patients (including eighteen female and seven male patients with a mean age of 71.3 years) undergoing both lumbar spine dual x-ray absorptiometry scans and computed tomography imaging were evaluated to determine if Hounsfield units correlated with bone mineral density and T-scores. Normative data were generated from lumbar spine computed tomography examinations for eighty consecutive trauma patients and were stratified by age and sex. Separately, polyurethane foam blocks of varying densities were imaged with computed tomography and were subjected to mechanical testing to determine compressive strength. Compressive strength values and Hounsfield units were analyzed for correlation. Significant correlations were found between Hounsfield units and bone mineral density, age, and T-scores and between Hounsfield units and compressive strength (p < 0.001). Hounsfield units obtained from clinical computed tomography scans that are made for other purposes correlate with dual x-ray absorptiometry scores as well as compressive strengths based on osseous models and potentially provide an alternative method for determining regional bone mineral density at no additional cost to the patient. The information could conceivably be applied toward fracture risk assessment, diagnosis of osteoporosis, and early initiation of needed treatment.

Power Injection of Contrast Media via Peripherally Inserted Central Catheters for CT

To evaluate patient safety, catheter rupture rates, and computed tomography (CT) image quality when using peripherally inserted central catheters (PICCs) in vivo for the power injection of CT contrast media at standard injection rates. A prospective study to evaluate the safety and effectiveness of power injection of contrast media via indwelling PICCs was performed. Single-lumen and double-lumen polyurethane PICCs (5 F) were injected in vivo with contrast media for clinical CT examinations at injection rates ranging from 1 mL/sec to 4 mL/sec. Data collected included PICC rupture rate, patient complications, injection rate, peak injection pressure, PICC length, PICC age, and quality of contrast enhancement on the CT images. One hundred ten power injections of PICCs for CT examinations were performed. There were 12 injections of single-lumen PICCs and 98 injections of double-lumen PICCs. The most common injection rate was 2 mL/sec, accounting for 89 of the 110 injections (81%). Two PICCs ruptured during power injection, both as a result of operator error. One of the PICCs that ruptured was clamped at the time of injection and the other one was kinked at its venous entry site. One additional PICC showed evidence of dysfunction; it ballooned without actually rupturing. No significant patient complications occurred. Contrast enhancement of the CT images was subjectively rated as average or above average in 95% of cases. Contrast media can be power-injected via PICCs for routine CT examinations at a rate of 2 mL/sec, yielding satisfactory image quality without exposing patients to significant additional risk. Power injection rates greater than 2 mL/sec, as are typically used in CT angiography applications, were not fully evaluated by this study.
