A changing landscape: diagnosis and management of COPD.

Chronic obstructive pulmonary disease (COPD) is a common and often particularly debilitating disease. Progressively worsening breathlessness can limit normal daily functioning, reduce quality of life (QoL) and increase the risk of premature death. Importantly, early diagnosis, improving symptoms and QoL, along with minimising exacerbations and hospital admissions, are primary goals of patient care. In recent years, the assessment of COPD has moved away from equating disease severity solely with the degree of obstructive lung impairment to include patient symptoms, exacerbation history and comorbidities, as well as smoking status. There are now more therapies that reduce symptoms and prevent exacerbations, thereby improving QoL. This review explores the diagnosis and management of COPD and positive clinical approaches to managing patients.

Measuring fatigue as a symptom in COPD: From descriptors and questionnaires to the importance of the problem.

Fatigue is a symptom commonly diagnosed in cancers and in many other chronic debilitating diseases and is one of the main therapeutic targets for various pharmacologic and non-pharmacologic interventions. However, in chronic obstructive pulmonary disease (COPD), this symptom, which can be considered as the main extrapulmonary clinical feature of the disease, can impact significantly on the health-related quality of life of the patients. The aims of this review are to discuss the issues related to fatigue assessment in COPD and to highlight the importance of this symptom in this setting based on the data retrieved from articles published between 1987 through August 2014 available on MEDLINE database. Fatigue can be measured by various scales or questionnaires that are designed for generic purposes or for COPD-related purposes but is still underdiagnosed and undertreated. This is due to the fact that its clinical and prognostic relevance are not appropriately acknowledged. The early identification of fatigue clinical descriptors from patients' reports could help with better management of this symptom.

The effect of comorbidities on COPD assessment: a pilot study.

IntroductionPatients with chronic obstructive pulmonary disease (COPD) frequently suffer from comorbidities. COPD severity may be evaluated by the Global initiative for chronic Obstructive Lung Disease (GOLD) combined risk assessment score (GOLD score). Spirometry, body plethysmography, diffusing capacity of the lung for carbon monoxide (DLCO), and high-resolution computed tomography (HR-CT) measure lung function and elucidate pulmonary pathology. This study assesses associations between GOLD score and measurements of lung function in COPD patients with and without (≤1) comorbidities. It evaluates whether the presence of comorbidities influences evaluation by GOLD score of COPD severity, and questions whether GOLD score describes morbidity rather than COPD severity.MethodsIn this prospective study, 106 patients with stable COPD were included. Patients treated for lung cancer were excluded. Demographics, oxygen saturation (SpO2), modified Medical Research Council Dyspnea Scale, COPD exacerbations, and comorbidities were recorded. Body plethysmography and DLCO were measured, and HR-CT performed and evaluated for emphysema and airways disease. COPD severity was stratified by the GOLD score. Correlation analyses: 1) GOLD score, 2) emphysema grade, and 3) airways disease and lung function parameters, described by: forced expiratory volume in the first second in percent of expected value (FEV1%), inspiratory capacity (IC%), total lung volume (TLC%), IC/TLC, and SpO2. Correlation analyses between subgroups and hierarchical cluster analysis were performed.ResultsSignificant associations were found between GOLD score and both emphysema grade (correlation coefficients [cc]: −0.2, P=0.03) and lung function parameters (cc: −0.5 to −0.7, P-values all <0.001) weakened in patients with >1 comorbidity (cc: −0.4 to −0.5, P-values all 0.001). Significant differences between subgroups were found in GOLD score and both FEV1% (cc: −0.2, P=0.02) and IC/TLC (cc: −0.2, P=0.02). Comorbidities were associated with GOLD score and composite measures in hierarchical cluster analysis.ConclusionThe presence of comorbidities influences the relationship between GOLD score and lung function measurements. GOLD score may be more representative of morbidity than of COPD severity.

Prevalence and global initiative for chronic obstructive lung disease group distribution of chronic obstructive pulmonary disease detected by preoperative pulmonary function test.

BackgroundDespite being a major public health problem, chronic obstructive pulmonary disease (COPD) remains underdiagnosed, and only 2.4% COPD patients are aware of their disease in Korea. The objective of this study was to estimate the prevalence of COPD detected by spirometry performed as a preoperative screening test and to determine the Global Initiative for Chronic Obstructive Lung Disease (GOLD) group distribution and self-awareness of COPD.MethodsWe reviewed the medical records of adults (age, ≥40 years) who had undergone spirometry during preoperative screening between April and August 2013 at a tertiary hospital in Korea. COPD was defined as a postbronchodilator forced expiratory volume in 1 s/forced vital capacity ratio of <0.7. We analyzed self-administered COPD questionnaires for the assessment of the frequency of acute exacerbation over the previous year and dyspnea severity using the modified Medical Research Council dyspnea scale and COPD assessment test.ResultsAmong 3029 patients aged >40 years who had undergone spirometry as a preoperative screening test, 474 (15.6%; 404 men; median age, 70 years; range, 44–93 years) were diagnosed with COPD. Only 26 (5.5%) patients reported previous diagnosis of COPD (2.1%), emphysema (0.8%), or chronic bronchitis (2.5%). The GOLD group distribution was as follows: 63.3% in group A, 31.2% in group B, 1.7% in group C, and 3.8% in group D.ConclusionsThe prevalence of COPD diagnosed by preoperative spirometry was 15.6%, and only 5.5% patients were aware of their disease. Approximately one-third of the COPD patients belonged to GOLD groups B, C, and D, which require regular treatment.

COPD assessment tests scores are associated with exacerbated chronic obstructive pulmonary disease in Japanese patients.

Guidelines recommend chronic obstructive pulmonary disease (COPD) assessment tests (CATs) for evaluation of symptoms and management risks. To investigate whether CAT can predict moderate or severe exacerbations in Japanese COPD patients, a single-blinded prospective study was performed. A 123 Japanese COPD patients were classified into high-CAT (n=64) and low-CAT (n=59) groups. The frequencies and periods of moderate or severe exacerbation and hospitalization were compared between the two groups. Multivariate logistic regression analysis was performed to investigate whether CAT could predict exacerbations. A receiver operating characteristic (ROC) curve analysis was employed to find an appropriate CAT score for exacerbation. The high-CAT group was significantly older, had a lower body mass index, and had a lower airflow obstruction as compared to the low CAT group. The frequency of moderate or severe exacerbation (1.3±1.3 events per patient per year, p<0.0001) and hospitalizations (0.2±0.4, p=0.0202) in the high-CAT group was significantly higher than in the low-CAT group (0.4±0.7 and 0.0±0.1, respectively). Multivariate logistic regression analysis showed that both high CAT score and low airflow obstruction were independently predictive of frequent moderate or severe COPD exacerbation. ROC analysis showed that the best cut-off CAT score for moderate or severe COPD exacerbation was 8 points. Our present results indicate that COPD Japanese patients showing high CAT scores have a poor prognosis, and that the CAT score is able to predict exacerbation in Japanese COPD.

The Value of Assessment Tests in Patients With Acute Exacerbation of Chronic Obstructive Pulmonary Disease

The aim of our study was to investigate the chronic obstructive pulmonary disease (COPD) assessment test (CAT), serum copeptin, procalcitonin and C-reactive protein (CRP) levels as potential predictive factors for recurrence of acute exacerbation and all-cause mortality in 6 months of COPD inpatients. One hundred fifty-nine patients who met the inclusion criteria were enrolled and followed up for 6 months. The CAT scores, serum copeptin, procalcitonin and CRP levels were measured on admission and 14 days and 3 months later in all patients. The primary endpoint was recurrence of acute exacerbation in 6 months. The secondary endpoint was all-cause mortality after 6 months. The CAT scores, serum copeptin, procalcitonin and CRP levels were significantly elevated on admission and stabilized at 14 days (P < 0.01). In a univariate logistic regression analysis, CAT scores (odds ratio [OR] = 1.10), forced expiratory volume in 1 second % (OR = 1.01), serum copeptin (OR = 1.32) and CRP levels (OR = 1.01) were significantly related to recurrence of acute exacerbation in 6 months (P < 0.05). In a multivariate logistic regression model, increasing CAT scores (OR = 1.10) and serum copeptin levels (OR = 1.29) were still associated with an increased odds of exacerbation (P < 0.05). in a univariate logistic regression analysis, increasing CAT scores (OR = 119), forced expiratory volume in 1 second % (OR = 1.05), serum copeptin levels (OR = 1.44) and hospitalization in the previous years (OR = 1.24) were significant determinants of death over a follow-up period of 6 months (P < 0.05). But only serum copeptin (OR = 1.53) and CAT scores (OR = 1.37) were associated with mortality in multivariate logistic regression analysis. Hence, high CAT scores and serum copeptin levels link with recurrence of acute exacerbation and all-cause mortality during 6 months in patients with acute exacerbation of COPD.

COPD assessment: I, II, III, IV and/or A, B, C, D

In 2001 the Global Initiative for Chronic Obstructive Lung Disease (GOLD) committee published its first consensus report [1]. At that time the authors suggested that the assessment of chronic obstructive pulmonary disease (COPD) should be primarily based on the extent of airflow limitation. In the following years, evidence accumulated that COPD is a complex and heterogeneous disease and that airflow limitation is not closely correlated to a variety of patient-related outcomes [2]. These findings, and the intent to create a more comprehensive system to better reflect the situation of an individual patient, were the basis for a novel concept recently published by GOLD. Now, the assessment is no longer based on the extent of airflow limitation alone; in addition, the patient's exacerbation history and symptoms are taken into account [3]. Based on the severity of symptoms and the exacerbation risk, four categories (A, B, C and D) were defined. Since then, this novel assessment scheme has been studied in several existing cohorts, ranging from more than 2000 clinically stable COPD patients in the ECLIPSE study (Evaluation of COPD Longitudinally to Identify Predictive Surrogate Endpoints) [4], over a more broadly composed cohort of 4000 smokers from the COPDGene study [5], to population samples from Copenhagen [6] …

New developments in the assessment of COPD: early diagnosis is key.

Chronic obstructive pulmonary disease (COPD) is the most common chronic lung disease in the world, and its associated health burdens and costs are mounting. Until recently, it was generally accepted that targeting the diagnosis of COPD early in its course was a relatively fruitless effort, since treatments other than already ubiquitous smoking-cessation efforts were unlikely to alter its course. However, there is strong evidence to suggest that the majority of patients with objective COPD are not aware of their condition, and this leads to a significant delay in diagnosis, more aggressive smoking-cessation intervention, and potential treatment. Novel methods of diagnostic testing, community health programs, and primary-care provider recommendations hold promise to expand the recognition of COPD in its incipient stages – where recent evidence suggests a rapid decline in lung function occurs and may be prevented if acted upon. This review explores the evidence to support the efforts to justify programs aimed at early diagnosis, alternative diagnostic strategies that may augment traditional spirometry, therapeutic modalities that could potentially be used in the future to alter early lung-function decline, and emphasizes the necessary cooperative role that physicians, patients, communities, and governments need to play to realize the significant health impact that stands to be gained.

Evaluation of the chronic obstructive pulmonary disease assessment test in Japanese outpatients

The Chronic Obstructive Pulmonary Disease (COPD) Assessment Test (CAT) is a quality-of-life (QOL) questionnaire that proved to correlate with St. George's Respiratory Questionnaire. Correlations between CAT scores and other COPD parameters have not been thoroughly evaluated in Japanese outpatients. Cross-sectional study of 85 outpatients with COPD at a Japanese community-based hospital. We observed 70 men and 15 women, whose average age was 72.0 ± 9.0 years. Mean forced expiratory volume in 1 s (FEV1) (% predicted) was 45.8 ± 14.7%. Mean CAT score was 10.1 ± 7.9 (range: 0-31). We calculated Spearman's rank correlation coefficient for CAT score and the following variables: r=0.81 for 'the Body Mass Index, Airflow Obstruction, Dyspnea and Exercise Capacity Index'; r=-0.05 for body mass index; r=-0.56 for FEV1 (% predicted); r=0.88 for Modified Medical Research Council Dyspnea Scale; r=-0.71 for 6-min walk distance; r=0.68 for 'the Age, Dyspnoea,and Airflow Obstruction Index'; and r=-0.40 for oxygen saturation in artery. Each COPD parameter, except for body mass index, had a significant (P<0.001) correlation with the CAT score. The CAT score, which is obtainable by a simple questionnaire originally designed for QOL assessment, had strong correlations with airflow obstruction, dyspnea, exercise tolerance, prognostic index and oxygenation in Japanese outpatients.

Relationship between chronic obstructive pulmonary disease (COPD) assessment test score and prognostic factors of COPD patients

Objective To observe the correlation between chronic obstructive pulmonary disease(COPD)assessment test (CAT)score and prognostic factors,so as to investigate the value of CAT score in predicting the prognosis of COPD. Methods A total of 81patients with newly diagnosed COPD in our hospital during Jul.2011to Sep.2012,without using inhaled corticosteroid(ICS)/long-actingβ2agonist(LABA)or long-acting antimuscarinic agent(LAMA),were divided into group A (low risk,less symptoms),B (low risk,more symptoms),C (high risk,less symptoms)and D (high risk,more symptoms)groups according to Global Initiative for Chronic Obstructive Lung Disease(GOLD,2011edition),and the patients were given ICS/LABA or ICS/LABA+LAMA treatment for 3months.The CAT score,age,smoking quantity,pulmonary function indices,body mass index (BMI),6-min walking distance (6MWD),modified medical British research council (mMRC)dyspnea scale,and the times of acute exacerbation of COPD (AECOPD)in previous one year were collected before and after treatment.The clinical characteristics analysis and correlation analysis were performed.Results The average age of the 81COPD patients was (66.27±8.52)years,with 88.89% being males and 85.19% having smoking history.The proportions of group A,B,C and D were 8.64%,30.86%,4.94%and 55.56% before treatment,respectively.The values of the forced expiratory volume in one second (FEV1),predicted amount as a percentage of FEV1(FEV1%Pred),forced vital capacity(FVC),predicted amount as a percentage of FVC (FVC%Pred),peak expiratory flow (PEF),predicted amount as a percentage of PEF(PEF%Pred),and 6MWD in CAT score≥10groups were significantly less than those in CAT score10 group(P0.05).The above parameters were not significantly different between patients with CAT score being 10-20,20- 30and≥30groups.mMRC scale and times of AECOPD in CAT score≥20groups were significantly higher than those in CAT score10group(P0.05).No significant difference in FEV1/FVC was found in different CAT score groups.The CAT score was significantly correlated with mMRC scale(pre-treatment r2=0.417,P0.001;post-treatment r2=0.19,P0.001), 6MWD (pre-treatment r2=0.320,P0.001;post-treatment r2=0.19,P0.001),pre-treatment FEV1(r2=0.177,P= 0.001 5),FEV1%Pred(r2=0.125,P=0.002),PEF(r2=0.164,P=0.002 4),PEF%Pred(r2=0.129,P=0.007 6),FVC (r2=0.098,P=0.021),FVC%Pred(r2=0.094,P=0.024),FEV1/FVC(r2=0.101,P=0.005 7),and AECOPD number (r2=0.059,P=0.028);and not correlated with the quantity of smoking (r2=0.041,P=0.083),BMI(r2=0.00,P= 0.89),and post-treatment FEV1(r2=0.01,P=0.22)or FEV1%Pred(r2=0.003,P=0.09).Conclusion COPD is prone to occur in the male smokers,with the highest proportion found in group D.CAT score has a good correlation with pre-and post -treatment mMRC scale and exercise capacity,suggesting it has a potential for predicting prognosis of COPD.

Chronic obstructive pulmonary disease (COPD) is a major cause of morbidity and mortality worldwide. However, despite being one of the most common causes of hospital admission, COPD has been a neglected respiratory condition. Nurses working in all care settings can make an important contribution to the care of patients with COPD. This article discusses assessment, diagnosis and management of COPD. Reference is made to recent policy developments, emphasising the importance of transforming care, and improving quality of life and health outcomes for patients.

Evaluation of Quality of Life with the Chronic Obstructive Pulmonary Disease Assessment Test in Chronic Obstructive Pulmonary Disease and the Effect of Dyspnea on Disease-Specific Quality of Life in These Patients

PurposeThe chronic obstructive pulmonary disease (COPD) assessment test (CAT) was recently introduced for use in assessing disease-specific quality of life and follow-up of patients with COPD. The purpose of this study was to evaluate the effect of the dyspnea on disease-specific quality of life detected by CAT score in patients with COPD.Materials and MethodsIn this study, 90 stable patients with COPD as defined by the Global Initiative for Chronic Obstructive Lung Disease (GOLD) criteria were included. The level of dyspnea was assessed with the Medical Research Council (MRC) dyspnea scale, and disease-specific quality of life was assessed with CAT score.ResultsThe mean±SD age was 68.5±10.9 (range 41-97) years. A significant relationship was established between CAT score, MRC dyspnea scale score and GOLD stage in patients with COPD. There was also a positive correlation between dyspnea scale scores and GOLD stage in the patients (p<0.001), as well as positive correlation between CAT score and dyspnea scale score (p<0.001). CAT score showed a significant correlation with hospitalization and exacerbations (p<0.05).ConclusionDyspnea is an important symptom that may impact quality of life in patients with COPD. CAT was shown to be a simple, fast and intelligible measurement of disease-specific quality of life, and was correlated with levels of dyspnea in patients with COPD.

Chronic Obstructive Pulmonary Disease Assessment Test Can Predict Depression: A Prospective Multi-Center Study

This study was conducted to investigate the association between the chronic obstructive pulmonary disease (COPD) assessment test (CAT) and depression in COPD patients. The Korean versions of the CAT and patient health questionnaire-9 (PHQ-9) were used to assess COPD symptoms and depressive disorder, respectively. In total, 803 patients with COPD were enrolled from 32 hospitals and the prevalence of depression was 23.8%. The CAT score correlated well with the PHQ-9 score (r=0.631; P<0.001) and was significantly associated with the presence of depression (β±standard error, 0.452±0.020; P<0.001). There was a tendency toward increasing severity of depression in patients with higher CAT scores. By assessment groups based on the 2011 Global Initiative for Chronic Obstructive Lung Disease guidelines, the prevalence of depression was affected more by current symptoms than by airway limitation. The area under the receiver operating characteristic curve for the CAT was 0.849 for predicting depression, and CAT scores ≥21 had the highest accuracy rate (80.6%). Among the eight CAT items, energy score showed the best correlation and highest power of discrimination. CAT scores are significantly associated with the presence of depression and have good performance for predicting depression in COPD patients.

Chronic obstructive pulmonary disease assessment test score correlated with dyspnea score in a large sample of Chinese patients

Background The chronic obstructive pulmonary disease assessment test (CAT) is an easy to use health-related quality of life questionnaire, the modified Medical Research Council (mMRC) dyspnea scale is a classic dyspnea scale which is widely used, while the correlation between them is still not clear. This study investigated the use of the Chinese translation of CAT in chronic obstructive pulmonary disease patients and its correlation with the mMRC dyspnea scale. Methods The multicenter cross-sectional study was conducted in 329 hospitals throughout China from March 1 to April 30, 2010. Chronic obstructive pulmonary disease patients completed both the assessment test and the dyspnea scale during a single study visit. Results Six thousand, four hundred and thirty-seven patients were evaluated; 74.9% were male and the mean age was (64.9±10.0) years. Median test scores in dyspnea grades 0 to 4 were 14, 16, 22, 26 and 32, respectively; these differences were statistically significant. The CAT score was moderately correlated with mMRC dyspnea grade (r=0.579, P &lt;0.001). There was no significant difference in mean CAT score between males and females, and patients of high and low socioeconomic status. Primary analysis suggested that CAT scores were higher in older patients (&gt;65 years) than in younger patients (≤65 years) and increased with duration of formal education, but these findings were repudiated by further analysis of subgroups according to mMRC dyspnea grade. Conclusions There was no obvious confounding factor influencing use of the CAT in Chinese patients. CAT scores were moderately correlated with the mMRC dyspnea scale.

A Validation Study for the Korean Version of Chronic Obstructive Pulmonary Disease Assessment Test (CAT).

BackgroundHealth status measure is not only important for clinical research studies but also for clinical practices of chronic obstructive pulmonary disease (COPD) patients. The objective of this study is to evaluate the validity of the Korean Version of COPD Assessment Test (CAT) in primary care clinics as well as in referral hospitals.MethodsSmokers or ex-smokers, aged 40 years or older, with a smoking history of >10 pack-years; and a COPD diagnosis in the past 6 months or more, were recruited from 4 primary care clinics and 2 referral hospitals. Demographic, medical, and spirometry data was collected from patients who completed the CAT and St. George Respiratory Questionnaire (SGRQ), and had their dyspnea been assessed. The primary endpoint was the correlation between of the Korean version of CAT with SGRQ in patients with COPD.ResultsA total 100 patients were enrolled. The mean age and smoking amounts were 69.2±8.4 years and 40.6±22.3 pack-years, respectively. Sixty-seven percent of the patients reported at least one exacerbation in the past year. The mean CAT score was 16.9±8.0. The internal consistency assessed by Cronbach's alpha was 0.85. The CAT score was positively correlated with the SGRQ score (r=0.76, p<0.0001) and each component of SGRQ: symptoms, activity and impacts; r=0.68, r=0.61, and r=0.72, respectively (all p<0.0001). These positive correlations were preserved in the different groups (r=0.86, p<0.0001 in primary care clinic group; r=0.69, p<0.0001 in hospital group). The CAT score was also positively correlated to the Medical Research Council dyspnoea scale (r=0.46, p<0.0001).ConclusionThe Korean version of CAT had good internal consistency and showed good correlations with SGRQ. It can be used for assessing the impacts of COPD on the patient's health including primary care setting.

A study to assess COPD Symptom-based Management and to Optimise treatment Strategy in Japan (COSMOS-J) based on GOLD 2011

Background and objectiveThe Global initiative for chronic Obstructive Lung Disease (GOLD) Committee has proposed a chronic obstructive pulmonary disease (COPD) assessment framework focused on symptoms and on exacerbation risk. This study will evaluate a symptom and exacerbation risk-based treatment strategy based on GOLD in a real-world setting in Japan. Optimal management of COPD will be determined by assessing symptoms using the COPD Assessment Test (CAT) and by assessing the frequency of exacerbations.MethodsThis study (ClinicalTrials.gov identifier: NCT01762800) is a 24-week, multicenter, randomized, double-blind, double-dummy, parallel-group study. It aims to recruit 400 patients with moderate-to-severe COPD. Patients will be randomized to receive treatment with either salmeterol/fluticasone propionate (SFC) 50/250 μg twice daily or with tiotropium bromide 18 μg once daily. Optimal management of patients will be assessed at four-weekly intervals and, if patients remain symptomatic, as measured using the CAT, or experience an exacerbation, they have the option to step up to treatment with both drugs, ie, SFC twice daily and tiotropium once daily (TRIPLE therapy). The primary endpoint of the study will be the proportion of patients who are able to remain on the randomized therapy.ResultsNo data are available. This paper summarizes the methodology of the study in advance of the study starting.ConclusionThe results of this study will help physicians to understand whether TRIPLE therapy is more effective than either treatment strategy alone in controlling symptoms and exacerbations in patients with moderate-to-severe COPD. It will also help physicians to understand the GOLD recommendation work in Japan.

Reliability and validity of Turkish version of COPD assessment test

This study is aimed to evaluate the reliability and validity of the Turkish version of chronic obstructive pulmonary disease (COPD) Assessment Test (CAT) in seven centers. 321 patients between 4-75 years of age, diagnosed and staged by Global Initiative for Obstructive Lung Disease (GOLD) 2011 criteria were included. The Breathlessness, Cough, Sputum scale (BCSS), mMRC (Modified Medical Research Council ) dyspnea index, St. George Respiratory Questionnaire (SGRQ), CAT and Short Form-36 (SF-36) were used concurrently. In the statistical analyses, internal consistency, item-total score correlation, explorative factor analysis, correlation with other scales were calculated. The mean age was 62.4 ± 8.9 years and 89.7% of the patients were male (n= 288). Mean FEV1% was 51.9 ± 19.2 and most of the patients were in Stage 3. CAT total score was 17.8 ± 9.5. In the internal consistency, Cronbach alpha coefficient was found as 0.9116 and item-total score correlation coefficients were between 0.62-0.79 and all were statistically significant (p< 0.0001). The correlation of the test-retest score calculated after two weeks with the initial score was 0.96 (p< 0.0001). In the structural validity, factor analysis with principle component analysis and varimax rotation was performed. One factor solution was achieved with eigenvalue of 4.956 and it represented 61.9% of the total variance. All the items were contained in the factor and the factor loads were between 0.71-0.85. The correlation coefficients of CAT with other indexes were moderate to good. The discrimination of CAT among disease stages has been shown to be significant (p< 0.0001) and a significant correlation was found with pulmonary function tests (p< 0.0001). It is demonstrated The Turkish version of COPD Assessment Tool is reliable and valid.
