From Pamphlets to Podcasts: Health Information at Great Ormond Street Hospital for Children NHS Trust

This paper outlines the development of information for children and families at Great Ormond Street Hospital for Children NHS Trust. Starting in 1995, guidelines for producing written information have been developed and a regular audit programme carried out, resulting in information of consistently high quality being produced. The ‘rules’ set out for the provision of written information have now been developed to cover multimedia information, so that technology can be harnessed to develop podcasts and other non‐written information materials. Technical processes are explained both for production and delivery. Future plans for multimedia information are also outlined.

Healthcare-associated rotavirus gastroenteritis in a large paediatric hospital in the UK

Rotavirus is a major cause of acute, community-acquired gastroenteritis in infants and young children but its importance in healthcare-associated paediatric gastroenteritis is much less well understood. A prospective study was undertaken at the Royal Liverpool Children's NHS Trust, Liverpool, UK. We enrolled 243 children with acute gastroenteritis (AGE) who were hospitalised between January and May 2006. Rotavirus was detected in faecal specimens by enzyme immunoassay. This virus was responsible for 17/91 cases (19%) of healthcare-associated AGE and 54/152 cases (36%) of community-acquired AGE. Sixteen children with healthcare-associated acute rotavirus gastroenteritis required source isolation and eight received intravenous rehydration. We conclude that rotavirus is an important cause of healthcare-associated AGE in a large paediatric hospital. Rotavirus vaccines represent a public health tool that could prevent the majority of symptomatic rotavirus infections within this healthcare setting.

Children's nursing education: what is the consensus?

Faith Gibson MSc (Cancer Nursing) PhD, RSCN, RGN, ONC, Cert Ed, RNT, Lecturer in Children’s Nursing Research, The Centre for Nursing and Allied Health Professions, Great Ormond Street Hospital for Children NHS Trust London In 2001, the UKCC invited the nursing professions to consider six possible alternative models for the future of pre-registration education. This paper stimulated considerable debate and has been further fuelled by the Nursing and Midwifery Council consultation on the new nursing register (NMC 2002). In an earlier article, we argued that any changes to the current approach to education must have sound evidential support (Whiting et al 2002). That article reported on the first stage of a Delphi study that involved 27 of the 35 delegates who attended a one-day invitational conference on the future of children’s nursing education. This paper outlines the second and final stage of this Delphi survey in which an attempt was made to explore the ‘real issues’ for children’s nurses and specifically to investigate why the prospect of a generalist pre-registration programme seemed to present such a significant challenge to the smaller branches of nursing.

Pressure sores in children - the acute hospital perspective

There is very little published literature on pressure sores in children and most of the existing literature is qualitative. Using literature from paediatric and adult studies, a schedule was designed to collect quantitative data on aspects that may predispose children to pressure injury. The schedule was piloted in an incidence and a prevalence study at the Royal Liverpool Children's NHS Trust. The sample size was 82 children for the incidence study and 183 children for the prevalence study. Six children in the incidence study and 12 children in the prevalence study sustained pressure injury. Data indicated that factors most strongly associated with pressure injury were nutritional status, mobility and consciousness level. Other factors that were implicated in increasing susceptibility to pressure injury were skin condition, body weight, haemodynamic status and hydration. Infants and young children most frequently sustained pressure injury on the occipital scalp area and heels. Although this was a small study, it produced some useful preliminary data, and was a valuable exercise to develop a tool for data collection on a larger scale.

Illness induction syndrome: Paper I—A series of 41 children from 37 families identified at the great ormond street hospital for children NHS trust

At the hospital 41 children from 37 families were identified as having had illness induced by a parent who in all but three cases was the mother. Their case records were reviewed. Four patterns of presentation occurred: failure-to-thrive through the active withholding of food; allegation of allergy and withholding of food; allegation and fabrication of medical symptoms; and active interference by poisoning or disrupting medical treatment. Four of the children died, two as a result of the illness induction. In 35% of the families a sibling had been previously subjected to some type of abuse. All the children had been presented with potentially serious symptoms, but post-identification only five were found to have serious medical problems requiring ongoing treatment. There were no specific characteristics of either the child or family associated with each type of presentation. Seventeen children had previously presented with failure-to-thrive, feeding problems or food allergies. All the mothers had suffered at least one of the following: privation, child abuse, psychiatric illness, or significant loss or bereavement, whereas only half the fathers had grown up in a deprived family situation and/or had earlier or current health difficulties. Forty percent of the parents had serious marital problems. A combined medical/psychosocial team identified the abuse and attempted to understand the family's belief system regarding the illness. The process of Illness Induction was conceptualized as being initiated by the parents perceiving the child to be ill and using this focus on illness as a way of solving major personal, marital, and/or family difficulties.
