Psychological Impact of the COVID-19 Pandemic on Children and Adolescents With and Without Mental Disorders.

Background: The previous and current studies highlight the psychological distress caused by coronavirus disease 2019 (COVID-19)-associated restrictions among the general population, especially among children and adolescents; however, few studies have examined children and adolescents with a mental disorder. The current study aimed to explore whether youth with mental disorders show a higher pandemic-associated psychological burden than healthy children and adolescents and to determine which psychiatric diagnoses are particularly associated with a higher distress level.Methods: In this study, 144 children and adolescents between the ages of 6 and 18 years with a mental disorder and 48 children and adolescents within the same age range without a mental disorder, and their caregivers, completed questionnaires assessing the pandemic-associated trauma symptoms (the Child Report of Post-Traumatic Symptoms [CROPS] and the Parents Report of Post-Traumatic Symptoms [PROPS]). Additionally, we asked specific questions about the pandemic-associated stress factors, such as financial problems, prolonged screen times, or loneliness.Results: Children and adolescents with a mental illness showed a significantly higher psychological burden than their mentally healthy peers. Female gender was a risk factor for a higher self-reported psychological burden, and younger age was associated with a more extensive parent-reported psychological burden. The patients with a depressive disorder showed significantly higher levels of psychological distress associated with the COVID-19 pandemic than the patients with an attention deficit and/or a conduct disorder.Conclusions: Children and adolescents with a mental illness, particularly, female children and individuals with a depressive disorder, are at an increased risk of suffering from pandemic-associated psychological distress. Adequate mental health care options, such as telepsychiatry, are indispensable.

Screening of Acute Traumatic Stress Disorder and Posttraumatic Stress Disorder in Pediatric Trauma Patients: A Pilot Study.

Children who experience traumatic physical injuries are at risk of developing acute stress disorder and posttraumatic stress disorder (PTSD). Early identification and treatment of these high-risk children can lead to improved mental health outcomes in this population. This study assesses the feasibility of a study protocol that compares 3 screening tools for identifying patients at a high risk of later development of acute stress disorder or PTSD among pediatric trauma patients. This pilot study compared 3 questionnaires used as screening tools for predictors of later development of PTSD in a convenience sample of pediatric trauma patients aged 7-17 years. Patients were randomized to one of 3 screening tools. Families were contacted at 30, 60, and 90-120 days postinjury to complete the Child Report of Post-Traumatic Symptoms questionnaire. The sensitivity and negative predictive value of the screening tools were compared for the diagnosis of PTSD defined using the Child Report of Post-Traumatic Symptoms questionnaire. Of the 263 patients identified for possible enrollment, 52 patients met full inclusion criteria and agreed to participate. Only 29 (55.7%) patients completed at least one follow-up questionnaire. The prevalence of acute stress disorder and PTSD in our population was 41% (95% CI [24, 61]) and 31% (95% CI [15, 51]), respectively. In this pilot study, we sought to determine the utility of the 3 commonly used screening instruments for measuring traumatic stress symptoms in pediatric trauma patients to predict the diagnosis of acute stress disorder or PTSD. Limitations include the use of the Child Report of Post-Traumatic Symptoms screening tool as the gold standard for calculating test characteristics and lack of 24/7 enrollment capabilities. As such, a significant portion of patients were discharged prior to our teams' engagement for enrollment.

Posttraumatic stress disorder in children and adolescents with chronic pain.

Despite high comorbidity between posttraumatic stress disorder (PTSD) and chronic pain evident in adult populations, little research has been conducted in the pediatric population to assess this association. Therefore, this study aimed to estimate the prevalence of trauma and PTSD in pediatric patients with chronic pain. Additionally, investigation into whether the Child Report of Posttraumatic Symptoms (CROPS) is a valid PTSD screening tool for this patient population and identification of an appropriate clinical cutoff for screening PTSD was undertaken. Data of N = 297 children and adolescents aged 8 to 17 years were collected during an intensive interdisciplinary pain treatment. At admission, patients self-reported pain characteristics and posttraumatic stress symptoms (assessed with the CROPS). During the inpatient stay, therapists completed a checklist that enabled a PTSD diagnosis according to criteria of the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Overall, 18.2% of the patients reported a trauma that met criterion A of DSM-5. Of those patients, approximately 59.3% had a PTSD diagnosis, equivalent to 10.8% of the whole sample. Overall, the CROPS demonstrated good reliability and validity in terms of internal consistency, item characteristics, and factor structure. Patients with PTSD had higher CROPS scores than patients without trauma. To identify patients with PTSD, a CROPS cutoff point of 21 is most appropriate in this population. This study demonstrated high prevalence of PTSD in children and adolescents with severe chronic pain. Regarding PTSD screening in this population, results revealed that valid PTSD assessment and identification of patients with PTSD seems possible despite overlapping symptoms. Preliminary Cross-Sectional Validation of the CROPS 14-Item for Juvenile Offenders.

The Child Report of Posttraumatic Symptoms (CROPS) is an effective 26-item trauma assessment tool. Research has indicated a 14-item version of the CROPS for juvenile offenders with improved predictive accuracy for detecting trauma exposure among male offenders and commensurate accuracy for female offenders. However, the 14-item scale has yet to be validated for juvenile offenders with an established trauma measure. Cross-sectional retrospective data of 74 adjudicated youth (59.5% male) from the original CROPS 14-item psychometric study sample were used to examine the factor structure, internal consistency, and convergent and divergent validity of the 14-item scale. Findings indicated strong internal consistency and significant correlations with all scales of an established and more extensive trauma measure for children and adolescents. Findings also revealed a CROPS 12-item model explaining 36.9% of variance. Results supported both convergent and divergent validity, suggesting both the CROPS 14-item and 12-item may be used as valid trauma symptom screeners for juvenile offenders.

Objectives : This study was conducted to examine the effects of trauma on depression manifesting as post-traumatic stress symptoms (PTSS) among adolescents. Methods : Participants included 113 adolescent students living in Korea. For evaluation of psychiatric symptoms, participants were asked to complete self-reports; including the Children’s Depression Inventory (CDI), the Child Report of Post-traumatic Symptoms (CROPS), and Lifetime Incidence of Traumatic Events-Child (LITE-C). Self-reports were completed twice including a baseline questionnaire (T1) and again six months later (T2). Results : The direct effect of trauma on T2 depression was not significant (B = 0.028, p = 0.903). The indirect effect of trauma on T2 depression was mediated by T2 PTSS [B = 0.414, CI = [0.019, 0.956]); (LITE-C(T1)→CROPS(T1): B = 0.794, p＜0.05, LITE-C(T1)→CROPS(T2): B = 0.649, p＜0.05, CROPS(T1)→CROPS(T2): B = 679, p＜0.001, CROPS(T2) →CDI(T2): B = 0.638, p＜0.001]. Conclusion : The results of this study show that an intervention to lower prolonged PTSS is required to prevent depression among adolescents.

EMDR Group Protocol With Children: A Field Study

The Eye Movement Desensitization and Reprocessing Group Protocol with Children (EMDR-GP/C) was first developed by Korkmazlar following the Marmara earthquake in Turkey in 1999 and can be adapted for different populations. This study focused on EMDR-GP with children who lost their fathers in the mine explosion that occurred in 2014 in Soma, Turkey. The EMDR-GP/C was used with 41 children (7–12 years old) in the early intervention, 3 weeks after the disaster, and used with 25 other children (6–13 years old) in the late intervention, 18 months after the disaster, when posttraumatic stress disorder symptoms had developed. The differences between the early and late implementations of EMDR-GP/C are presented in this article. In the early intervention, children processed the trauma by focusing on the “events” as they saw or heard them; however, 18 months after the disaster, children processed their “emotions” about the event in the desensitization phase. Results show a significant decrease in scores of subjective units of disturbance (SUDs) for both intervention periods. An analysis was also conducted, comparing decreases in SUD scores for younger and older children, with no differences found in their response to treatment. Pre and follow-up data were collected for the late intervention condition, using the Child Report of Posttraumatic Symptoms (CROPS), and showed a significant decrease at 18-month follow-up. Further studies are suggested to determine effectiveness of EMDR-GP/C with other populations.

Post-traumatic stress disorder and behavioral problems of parents and children after the 2015 Nepal earthquakes

This study examined the prolonged effects of parents’ PTSD and internalizing and externalizing problems on their children’s PTSD and internalizing and externalizing problems. The subjects were 200 pairs of Nepalese parents and children (aged 6–17 years) who had resided in refugee camps for 24 months following the 2015 Nepal earthquakes. PTSD among the parents was measured using the Impact Event Scale-Revised (IES-R) while their behavioral problems were measured using the Adult Self-Report in Nepali (ASR). PTSD among the children was measured using the Parent Report of Post-traumatic Symptoms (PROPS) and the Child Report of Post-traumatic Symptoms (CROPS) while their behavioral problems were measured using the Nepali version of the Child Behavior Checklist 6-18 (CBCL 6-18). The results showed that approximately half of the parents and children exhibited PTSD symptoms and over half of the parents and children exhibited internalizing problems requiring clinical intervention. Correlation analysis found a relatively strong connection between the parents’ PTSD and behavioral problems and their children’s PTSD and behavioral problems. Finally, the study found that the children’s reported PTSD score increased as the parents’ education level increased, and the parents’ internalizing score decreased. Parental PTSD was a significant variable in explaining children’s internalizing problems. Given the more serious the parents’ PTSD, the higher the children’s internalizing problems score, the results highlight the need for proper intervention for the parent and child survivors of natural disasters such as earthquakes.

School Bullying and Post-traumatic Stress Disorder Symptoms: The Role of Parental Bonding.

Much research on school bullying and victimization have outlined several individual, family, and school parameters that function as risk factors for developing further psychosocial and psychopathological problems. Bullying and victimization are interrelated with symptoms of psychological trauma, as well as emotional/ behavioural reactions, which can destabilize psychosocial and scholastic pathways for children and adolescents. The current study explored the various dimensions of psychological trauma (depressive symptoms, somatization, dissociation, avoidance behaviours) associated with school bullying/victimization in relation to parental bonding among 433 students (8–16 years old) from representative large cities in Greece. The following scales were employed: (a) Olweus Bully/Victim Questionnaire, (b) Child Report of Post-traumatic Symptoms (CROPS), and (c) Parental Bonding Inventory instrument (PBI). Pathways analysis extracted a series of models which showed that maternal and paternal overprotection (anxious-controlling/aggressive) had positive association with post-traumatic stress symptoms. Specifically, the quality of parental bonding was related with children's bullying/victimization experiences and post-traumatic symptomology. Conversely, results indicated that maternal and paternal care can reduce the manifestation of post-traumatic stress symptoms. Implications for interventions are discussed.

Post-Traumatic Stress, Quality of Life and Alcohol Use Problems among Out-of-School Youth.

Objective Post-traumatic stress symptoms (PTSS) in out-of-school youths (OSYs) may be influenced by alcohol use, but there is a lack of evidence explaining how PTSS affect alcohol use problems in OSYs. The present study aimed to investigate the relationships among PTSS, quality of life, and alcohol use in OSYs. Methods In total, 125 OSYs (46.4% male) in South Korea completed the Korean Version of the Child Report of Post-Traumatic Symptoms (CROPS), the KIDSCREEN-27 Quality of Life Measure for Children and Adolescents, and the Alcohol Use Disorders Identification Test-Consumption. Results Mean CROPS score was 18.7 (SD=11.6); 37.6% were problem drinkers. Quality of life in the domain of parent relations and autonomy significantly mediated the relationship between PTSS and alcohol use problems. OSYs with high parental satisfaction and autonomy were less likely to have alcohol use problems even with PTSS. Conclusion Family assessments and therapeutic approaches are needed for OSYs with both PTSS and alcohol use problems.

The Child-Centered EMDR Approach: A Case Study Investigating a Young Girl’s Treatment for Sexual Abuse

This article seeks to address the underrepresented area of eye movement desensitization and reprocessing (EMDR) treatment for sexually abused children. It presents the child-centered EMDR approach–a new protocol derived from Klaff (2016). This approach is built on Shapiro’s Adaptive Information Processing model and assumes that a successful outcome can be achieved without directly processing memories of the abuse since children have the ability to open channels of association to uncover, process, and resolve emotional trauma relating to the abuse. Instead of focusing on traumatic memories, the treatment targets whatever current concerns the child presents, as it is thought that these concerns may provide a pathway to covert and possibly unconscious issues. This approach is tested with a case study, which explores its use with a 9-year-old girl who presented with emotional triggers originating from sexual abuse by her biological father. She received six sessions addressing memories of being taunted by other children and of her father using drugs, but her memories of the sexual abuse were not directly addressed. Following treatment, her scores on the Child Report of Post-Traumatic Symptoms (CROPS) and Parent Report of Post-Traumatic Symptoms (PROPS) decreased to nonclinical levels, with results maintained 2 months later at follow-up. The promising results suggest the value of future research to further test this approach.

Factors Associated with Complicated Grief in Students Who Survived the Sewol Ferry Disaster in South Korea

Objective The Sewol ferry disaster caused shock and grief in South Korea. The aim of this study was to identify the factors associated with symptoms of complicated grief (CG) among the surviving students 20 months after that disaster. Methods This study was conducted using a cross-sectional design and a sample of 57 students who survived the Sewol ferry disaster. Data were collected using the following instruments: Inventory of Complicated Grief (ICG), the Lifetime Incidence of Traumatic Events-Child, the Child Report of Post-Traumatic Symptoms (CROPS), KIDSCREEN-27, Family Adaptability and Cohesion Evaluation Scales-III, the Peri-traumatic Dissociation–Post-traumatic Negative Beliefs–Post-traumatic Social Support scale, and the Strengths and Difficulties Questionnaire. A generalized linear model using a log link and Poisson distribution was performed to identify factors associated with symptoms of CG. Results The mean score on the ICG was 15.57 (standard deviation: 12.72). Being born in 1999, a higher score on the CROPS and a lower score in autonomy and relationship with parents on the KIDSCREEN-27 were related to higher levels of CG. Conclusion Twenty months after the Sewol ferry disaster, 24.5% of surviving students were suffering from CG. This study uncovered a vulnerable population of bereaved children at high risk for CG.

A Retrospective and Prospective Follow-up Study of Psychological Distress in the Danwon High School Survivors of the Sewol Ferry Disaster.

Objective We monitored a group of students from Danwon High School who survived the Sewol ferry disaster for 27 months to examine the course of their psychological symptoms. Methods We performed a chart review at the Danwon High School Mental Health Center at the following time points (T): 9 months (T1), 12 months (T2), and 15 months (T3) after the disaster. Additionally, we performed a follow-up review at 27 months (T4). Subjects completed the ‘State’ section of the State-Trait Anxiety Inventory for Children, the Center for Epidemiological Studies-Depression assessment, the Child Report of Post-traumatic Symptoms, and the Inventory of Complicated Grief. Data from the 32 subjects who completed all four assessments were used in the statistical analyses. Results Scores of psychological variables tended to increase until T2 and then slowly decreased until T4. The severity of anxiety and complicated grief symptoms changed significantly over time, but the severity of depression and posttraumatic stress symptoms did not. Conclusion We found that the symptoms of anxiety and complicated grief reported by Sewol ferry survivors from Danwon High School were exacerbated at the first anniversary of the disaster, but these symptoms subsided after the students graduated from high school.

Validation Study of a Korean Version of the Abbreviated University of California, Los Angeles, PTSD Reaction Index (Abb-UCLA-PTSD RI) for Adolescents.

BackgroundWe aimed to verify a Korean version of the Abbreviated University of California, Los Angeles, Post-Traumatic Stress Disorder Reaction Index (Abb-UCLA-PTSD RI-Korean version) among Korean adolescents to assess post-traumatic stress in this population.MethodsWe recruited 1,254 adolescents who completed the Abb-UCLA-PTSD RI-Korean version scale, the Child Report of Post-Traumatic Symptoms (CROPS), the Lifetime Incidence of Traumatic Events (LITE), and the Children's Depression Inventory (CDI). Test-retest reliability was computed for a randomly selected 314 participants. Among all the students, 765 were included in the analysis because 143 students were classified in the normative group (LITE 1) and 622 were in the traumatized group (LITE 4).ResultsThe internal consistencies of the nine-item and eight-item versions of the Abb-UCLA PTSD RI-Korean version were found to be high (Cronbach's α = 0.843 and 0.842, respectively). The criterion-related validity was based on comparison of the Abb-UCLA PTSD RI total scores between the normative and trauma groups. For the eight-item version, the normative group (4.6 ± 4.6) and the traumatized group (8.8 ± 6.0) demonstrated better criterion-related validity than those in the nine-item version (4.5 ± 4.3 and 8.5 ± 5.0, respectively). The test-retest reliability of the eight-item version was better than the nine-item version (r = 0.85 vs. 0.77). A two-factor model with eight items (two items regarding sleep, and the other six items made up the second factor) showed the best fit.ConclusionOur results indicate that the eight-item Abb-UCLA PTSD RI-Korean version is a useful screening tool for post-traumatic stress in Korean adolescents.

Five Months Follow-up Study of School-based Crisis Intervention for Korean High School Students Who Experienced a Peer Suicide.

BackgroundThe purpose of this study was to examine post-traumatic stress, depression, anxiety, and complicated grief in Korean high school students who experienced a peer suicide.MethodsA total of 956 students were included in the statistical analysis. One week after a peer suicide, a school-based crisis intervention program was conducted. The cohort was followed-up at one week and five months after a peer suicide. The program consists of screening tests, educational sessions, and further interview with psychiatric specialists for the selected group. Screening tests were conducted for all students to measure the Child Report of Post-traumatic Symptoms (CROPS), the post-traumatic stress symptoms (The University of California at Los Angeles post-traumatic stress disorder [PTSD] reaction index; UCLA-PTSD-RI), the anxiety symptoms (The Korean-Beck Anxiety Inventory; K-BAI), the depressive symptoms (The Korean-Beck Depression Inventory-II; K-BDI-II), and the complicated grief reaction (The Inventory of Complicated Grief; ICG). For statistical analysis, the SPSS Statistics 21.0 program was used.ResultsAt baseline and five months follow-ups, 8.6% and 2.9% of the students showed post-traumatic stress symptoms. At five months follow-up, there was a statistically significant decline in the post-traumatic stress symptoms, anxiety, depression, and complicated grief among the ‘trauma group.’ A higher proportion of the female students showed post-traumatic stress symptoms after the incident of peer suicide than the male students.ConclusionSchool-based crisis intervention helps improvement of trauma-related symptoms. It might be an effective way to prevent suicide spreading among students by alleviating trauma-related symptoms.

The Effect of Parent Group Sandplay Therapy on the Nepal Earthquake Survivors: Post-Traumatic Stress Symptoms, Parenting Stress and Psychological Well-Being of Parents as well as Post-Traumatic Stress Symptoms in Children*

People who have experienced property damage and loss from the Nepal earthquake in 2015 are living in a temporary tent area. These earthquake survivors experience symptoms of post-traumatic stress and exhibit altered thoughts and behaviors. This study was conducted on parents who reside in the tent city at Chuchepati and who have children aged from 7 to 17. For the group sandplay therapy program, the Impact of Events Scale-Revised, Child Report of Post-traumatic Symptoms, and the Parent Report of Post-traumatic Symptoms were administered. There were 24 individuals who agreed to participate in this study and whose child received 19 or more points from child post-traumatic symptom reports, along with 16 or more points from parent post-traumatic symptom reports, and 33 points from the IES-R. The results of this study indicate that parent group sandplay therapy is effective in reducing the symptoms of post-traumatic stress and parenting stress, and also in enhancing the psychological well-being of parents.

Predictive accuracy and factor structure of the Child Report of Posttraumatic Symptoms (CROPS) among adjudicated youth.

Trauma exposure and trauma symptom manifest have been associated with issues unique to the adjudicated youth population (Perkins, Calhoun, Glaser, & Kunemund, 2016), which necessitates accurate screening tools to facilitate appropriate allocation of resources (Briggs et al., 2013; Kerig, Moeddel, & Becker, 2011). The Child Report of Posttraumatic Symptoms (CROPS; Greenwald & Rubin, 1999) is a short and effective trauma assessment tool; however, predictive accuracy of the CROPS in classifying previous trauma exposure(s) as well as the factor structure of the CROPS has not been examined with this population. Retrospective data of 215 adjudicated youth (50.2% boys) were used to investigate the predictive accuracy of the CROPS in detecting previous trauma exposures. Further, researchers examined the factor structure of the CROPS to identify principle components that most strongly contributed to accurate classification. Logistic regression analyses indicated moderate predictive accuracy (64.2%) in identifying reported trauma histories for the total sample and among both adjudicated males (61.1%) and females (66.4%). Principle components analyses revealed a stable 3-factor solution (accounting for 47% of total variance) and yielded a 14-item CROPS factor accounting for 32.3% of total variance. The 14-item factor demonstrated improved predictive accuracy over the full 26-item CROPS for the total sample (65.1%) and among adjudicated males (66.7%). Overall findings provide moderate support for the utility of the CROPS in accurately identifying previous trauma exposure(s) among adjudicated youth and provide preliminary support for a 14-item version of the CROPS for screening for trauma symptoms in adjudicated youth. Web-Based Supportive Psychotherapy to Prevent Posttraumatic Stress Disorder: A Cross Sectional Study on the Iranian National Under-14 Girls’ Football Team After Nepal Earthquake in 2015

Background: Earthquakes are devastating natural disasters. When an earthquake occurs during a significant event, such as Asian football confederation (AFC) under-14 girls� regional championship in Nepal (2015), all catastrophic casualties should be taken into account, including mental side effects on young sensitive groups. The present paper aimed to report risk-reduction activities for posttraumatic stress disorder (PTSD) in 18 young members of the Iranian football team. Methods: Distant location of the team members persuaded the team physician to use Internet-based social applications for supportive psychotherapy during 1 year after the shock. The child report of posttraumatic symptoms (CROPS), along with the parent report of posttraumatic symptoms (PROPS), was used to collect data from players and their parents. No randomization (except for replications) was applied and no control group was recruited in this study. Results: The results showed that some of the girls had acute stress symptoms and were prone to acute stress disorder if left unattended. The average CROPS score was 20.06 (SD, 2.48) in the second week, which decreased to 1.33 (SD, 1.29) after 1 year. Similar results in the sixth and twelfth months showed that the girls could reach a stable mental status using online supportive psychotherapy. Conclusions: Despite lack of access to patients, use of supportive psychotherapy is still possible. Online meetings can help psychologists follow the stages of treatment accurately. The results of CROPS showed that it is possible to provide psychotherapy support through online discussions with patients. © 2017, Asian Journal of Sports Medicine.

Factors associated with post-traumatic stress symptoms among adolescents exposed to the Sewol ferry disaster in Korea

This study evaluated the factors associated with post-traumatic stress symptoms in Korean adolescents who lived in a disaster-affected community. A total of 1101 students attending secondary and high schools in Jindo, the location of the Sewol ferry disaster, were enrolled in a cross-sectional survey. The Child Report of Post-traumatic Symptoms (CROPS), the Center for Epidemiological Studies Depression Scale (CES-D), and the State Anxiety Inventory for Children (SAIC) were administered. Female gender, older children, poor academic achievement, and directly witnessing the rescue scene were associated with post-traumatic stress symptoms. The CES-D and SAIC scores of subjects with witness of the rescue were significantly higher than those of respondents without such experiences. The regression analysis revealed that directly witnessing the rescue scene was significantly associated with post-traumatic stress symptoms after adjusting for other variables. The results of this study suggest that witnessing the rescue scene following a disaster might be a risk factor for post-traumatic stress symptoms in adolescents in disaster-affected communities.

Objectives: The Child Report of Post-traumatic Symptoms (CROPS) and the Parent Report of Post-traumatic Symptoms (PROPS) are screening measures for post-traumatic symptoms in children. The present study aimed to investigate the reliability and validity of the Korean versions of the CROPS and the PROPS. Methods: The Korean versions of the CROPS and the PROPS were administered to a sample of 304 children aged 6 to 15 years old. The internal reliability, test-retest reliability, factorial validity, predictive validity and concurrent validity were evaluated. Results: The reliability of the CROPS and the PROPS was shown to have excellent internal consistency and test-retest correlation. The single factor structure of the PROPS was good and that of the CROPS was borderline acceptable according to confirmatory factor analysis. Other validity measures such as the predictive validity and concurrent validity were also shown to be satisfactory. Conclusion: This study demonstrated that the Korean versions of the CROPS and the PROPS were reliable measures with satisfactory psychometric qualities. Because it takes less than 5 minutes to fill out the CROPS and the PROPS, respectively they can be quick and easy screening scales for assessing post-traumatic symptoms in Korean children.
