Parent's knowledge, attitude, and practice of children car seats in Saudi Arabia: a cross-sectional study

Objective: This study aimed to examine parents' awareness of car seats in Saudi Arabia. Methods: This was a cross-sectional study. Data for this study were collected through a structured self-administered questionnaire distributed among parents with children aged less than seven years in various areas of Saudi Arabia. The survey was conducted from June to September 2023. The questionnaire was developed in english and translated into Arabic, and the parents took 3-5 minutes to complete the questionnaire. Results: A total of 767 parents took part in the study, and it was discovered that 60.4% used child car seats and a high willingness (93.8%) to use care seats in response to legal violations. The findings discovered significant discrepancies in parental beliefs and behaviors in Saudi Arabia. Conclusion: Saudi Arabia needs targeted educational programs and interventions to improve parental awareness of using child safety seats and encourage healthy attitudes and behavior.

“Current incidence of injuries in Iran; findings of STEPS survey 2021”

BackgroundThe updated epidemiology of injuries at the national and sub-national levels are required for policymakers to effectively handle the burden of injuries. This paper aimed to assess the incidence and risk factors of different injuries in Iran based on a recent national survey. MethodsWe used data from Iran Stepwise approach to surveillance (STEPS) Survey 2021, a population-based study in urban and rural areas of Iran's 31 provinces. A multistage clustered probability design and weighting adjustments were used to select eligible individuals and generate estimations. We estimated the incidence of injuries, assessed sociodemographic variables, and identified potential behavioral risk factors associated with injuries, and results were reported for sociodemographic and geographic stratifications. ResultData from 27,874 participants of the STEPS survey were assessed, of which 1538 (5.5 %, 95 % CI: [5.2–5.8]) reported having an injury in the past 12 months. Falls (44.4 %) were the most common cause of injury, followed by road traffic injury (21.7 %) and exposure to mechanical forces (16.5 %). Except for falls and burns, males had a higher proportion of all types of injuries. Logistic regression analysis showed that being male (OR: 1.7, [1.5, 2.0]) and being an occasional or heavy alcohol drinker (OR: 2.0, [1.3, 3.0] and OR: 2.7, [1.7, 4.1] respectively) were significant risk factors associated with road traffic injuries. Seatbelt use was 90.0 % among both drivers and front-seat passengers, while the use of safety car seats for children was as low as 9.4 %. Injury incidence varied significantly among provinces, with the highest incidence among males observed in Razavi Khorasan (11.2 %) and among females observed in Tehran (12.0 %). ConclusionThis study investigated the updated epidemiology of injuries in Iran and revealed socioeconomic and geographic disparities across country. This epidemiological information can be used to modify injury prevention programs.

Construction and analysis of prediction model for sales of child car seats

Based on the Carseats data set in the ISLR package, this paper constructs a regression model of the data set and predicts some variables. In addition, it also includes the establishment of regression trees and other models for variables. The final task requirement is to construct the classification model of the data, divide the classification model into the training set and the test set, and construct the linear discriminant analysis, logistic regression, random forest and so on with another variable. By making some models, the prediction accuracy on the test set is relatively high, and the evaluation effect of the model is also very good, which can provide a sufficient theoretical basis for the prediction of the sales of children's car seats.

Car Safety Seats and Restraints for Children in Al Ahsa, Saudi Arabia: Knowledge, Attitudes, and Practices

Background: Injuries caused by road traffic are a significant public health concern. In the 0-12-year age group, among the leading causes of death, unintentional injuries are considered the second and main cause of morbidity due to the safety of children in cars. According to statistics, approximately 4.7% of deaths in Saudi Arabia are caused by road traffic, while fatalities from road traffic do not exceed 1.7% in Australia, the United Kingdom, or the United States. Road traffic injuries killed more people in Saudi Arabia than in any other country with a high income. Aim: In this study, parents' knowledge, attitudes, and practices were evaluated regarding child safety regarding child car seats in Al Ahsa community. Methods: A cross?sectional descriptive study was done from November 10 to December 31, 2022. The data were collected using a 19-question survey. A convenience sampling of 379 samples was conducted. Results: In one-quarter of the participants, (24.8%) had the wrong answer when we asked them whether it is safer to hold a newborn baby in their arms than placing them in baby car safety seats, while (5.8%) did not know what is safe. Regarding the knowledge about the right way to put the car safety seat for children aged 2-4 years, near to one quarter (23%) answered wrong answer, while (11.1%) did not know how to fix it in the car. In relation to fixing the car safety seat for children aged below 2 years, only half the participants (54.6%) knew the right way. The mean score of knowledge was 3.37/5 with Std dev. ±1.52.in relation to practices and attitudes, (11.9%) said that the child is afraid to sit in the back seats, while (68.3%) said they do. (92.9%) of respondents believe that child safety seats are important. (14%) reported not using the seatbelt for children while (18.7%) reported using it sometimes. (67.3%) of those surveyed reported using seatbelts for all their children under the age of eight. There were (66.6%) of participants with acceptable knowledge and attitude scores, while (31.4%) had low scores. Parents' education level and their knowledge level were found to be significantly correlated. Parents' knowledge about care safety increased with the number of children, but there was no significant correlation between knowledge level and gender or number of children. Conclusion: According to the study, car safety seat noncompliance includes multiple variables that can be modified. The study could raise awareness by focusing on a major public health issue and providing outreach on child safety in cars. Residents of Al Ahsa generally had a positive attitude and good knowledge of car safety seats.

The use of car safety seats for children in China: A questionnaire survey based on the theory of planned behavior

Objective The purpose of this study was to analyze the psychological characteristics underlying Chinese parents’ behaviors in using child car seats and to understand their decision-making processes. Based on the theory of planned behavior (TPB), three extended variables of perceived accident severity, perceived benefits, and perceived barriers were introduced. From the perspective of social psychology, the psychological factors that influence parents’ use of child car seats and their interrelationships were explored. Methods A questionnaire was designed to collect data, including information on demographic characteristics, basic components of the TPB, and relevant extension variables. Using on online survey, 585 valid questionnaires were collected. Structural equation modeling was used to calibrate the data, and multiple group analysis was performed on the demographic variables. Results The extended TPB can effectively explain and predict parents’ behaviors when using children’s car seats. The results of the model show that parents’ positive attitudes toward child safety seats (CSSs), others’ recognition of their own use and perceptual control of the use of CSSs increase their willingness to use CSSs. Parents’ willingness to use has a positive impact on the use of CSSs. Additionally, for the three extended variables introduced, perceived benefit significantly promoted parental intention and behavior to use CSS for children; perceived barriers significantly reduced parental use of CSS; and perceived accident severity had no significant effect on parental use of CSS. Conclusions This study established the validity of the extended TPB model in predicting parents’ behaviors in using car seats for their children. In addition, the current findings may provide a theoretical basis for policy development to promote CSS use.

ROAD ACCIDENTS SAFETY AMONG TEENAGERS

Speed is a contributing factor in around one-third of all fatal road traffic crashes in high-income countries, and up to half in low- and middle-income countries. Long, straight roads which pass by schools, residences and businesses and which facilitate travel at high-speed place children at significant risk. A major risk to children as pedestrians, cyclists and passengers in vehicles are people who drink and drive. Consuming alcohol before driving increases not only the chance of a road traffic crash occurring, but also the likelihood that death or serious injury will result. For children, wearing a helmet is the single most effective strategy for reducing the risk of injury to the head while riding bicycles or motorcycles. For children who are occupants of a vehicle, a range of restraints is available to protect them. These include infant car seats, child car seats, booster seats and seat-belts, and their use depends on the age, weight and height of the child.

Elevated concentrations of halogenated flame retardants in waste childcare articles from Ireland

Concentrations of legacy and alternative halogenated flame retardants (HFRs) including chlorinated organophosphate esters (Cl-OPEs), were measured in waste childcare articles (n = 275 for Cl-OPEs, n = 187 for other HFRs) from the Republic of Ireland between 2019 and 2020. Articles studied comprised foams and fabrics from: child car seats, cot mattresses, changing mats, pushchairs, prams, and related items. Fifteen articles (7.7%) exceeded the European Union limit value of 1000 mg/kg for polybrominated diphenyl ethers (PBDEs) (all due to BDE-209), an additional 15 exceeded the limit for hexabromocyclododecane (HBCDD), with 7 articles exceeding the limit for both PBDEs and HBCDD. An even greater proportion of articles contained concentrations exceeding 1000 mg/kg for: tris(1-chloro-2-propyl) phosphate (TCIPP) (n = 73, 27%) and tris(1,3-dichloro-2-propyl) phosphate (TDCIPP) (n = 58, 21%), with concentrations greater than 1000 mg/kg also observed for: tris(2-chloroethyl) phosphate (TCEP) (n = 14, 5.1% articles), 2-ethylhexyl tetrabromobenzoate (EH-TBB) (n = 7, 3.7%), decabromodiphenyl ethane (DBDPE), and bis(2-ethylhexyl)tetrabromophthalate (BEH-TEBP) (both n = 5, 2.7%). Overall, 120 samples contained at least one HFR at a concentration exceeding 1000 mg/kg. In addition to the waste management implications of our findings, our data raise concerns about child exposure to HFRs during the use phase of these everyday items.

Modelling factors that impact the use of child safety seats for nursery school travel

Children, as one of the most vulnerable road user groups, are exposed to a high risk of traffic crash-related injuries and fatalities. Children’s incomplete physical and cognitive development leads to inaccurate detection of traffic hazards and inadequate road crossing decisions. When young children travel as car occupants, their body structures are immature, and restraint over larger and sometimes different body areas is necessary. Therefore, children’s vulnerability as car occupants is subject to their age and physical development. This research investigates factors influencing child car seat usage, particularly child restraint systems or booster seats, for commuting to nursery schools. An experimental study was conducted in three suburbs of Athens, Greece, with distinctive socioeconomic status (SES) and built environment characteristics. For this purpose, a self-administered parental questionnaire was completed for 734 children attending municipality nursery schools, exploring the traffic safety practices for preschool children, child-parent interactions, the parent’s risk perception, behaviour, and knowledge of traffic safety. Responses reveal that the surveyed children—irrespective of the residence area—travel to the nursery school mainly as car occupants (89.6% overall, ranging between 84.8% for the area with the lowest SES and 96.1% for the area with the highest SES). In addition, the results indicate that three-quarters of the children are often placed in a car seat, while almost one-quarter are never restrained. Regression and structural equation models are estimated and used to examine factors affecting the usage of child safety seats. Finally, conclusions are drawn, along with suggestions to overcome the limitations and complete this research with ongoing and future data collection and analyses.

Database selection matters: A case study in child restraint use and injury patterns using North Carolina motor vehicle traffic crash report and trauma registry data

Objective While proper restraint use is protective against motor traffic vehicle crash (MVTC)-associated morbidity and mortality, it is inconsistently measured across health and MVTC data sources. This project addresses this gap by assessing differences in child restraint measures between two North Carolina (NC) datasets and comparing the utility of these sources to evaluate patterns of child restraint use and associated health outcomes. Methods We analyzed 2018 NC MVTC and NC Trauma Registry (NCTR) data for children ≤15 years old, both separately and as linked MVTC-NCTR records. We calculated mean and standard deviation for continuous variables and frequency and proportion for categorical variables. Among linked records, we compared reported restraint use and performed age-adjusted logistic regression to estimate associations between restraint use and severe injury. Results A lower proportion of pediatric MVTC victims were reported as unrestrained in the MVTC (14.7%) versus NCTR (25.8%) data. Among linked MVTC-NCTR records, only 41.3% featured perfect concordance of restraint information between datasets. Among linked records, child restraint was reported for 31.3% of children, while the NCTR data allowed more granular coding of child restraint (30.3% overall), including child booster seat (13.5%), child car seat (8.7%), infant car seat (4.8%), and unrestrained in child car seat (3.4%). Age-adjusted regression analyses of the linked data revealed that lap/shoulder seatbelt use was significantly associated with lower likelihood of severe injury compared to being unrestrained whether informed by MVTC (OR = 0.39; 95% CI: 0.16, 0.93) or NCTR (OR = 0.38; 95% CI: 0.15, 0.96) data. While the association between reported use of a child car seat and severe injury was not statistically significant in the MVTC data (OR = 0.50; 95% CI: 0.19, 1.32), child car seat use reported in the NCTR data revealed a significant association (OR = 0.16; 95% CI: 0.03, 0.93). Conclusions Linked crash and trauma center data allow for identifying important patterns of restraint use among pediatric passengers in MVTCs. Dataset-dependent differences in measuring restraint use have critical public health implications and illustrate the importance of careful dataset selection prior to analysis, as the use of different data sources may impact overall study conclusions.

The impact of car seat giveaways on compliance among newborns in Saudi Arabia

ObjectivesRoad traffic injuries are a leading cause of morbidity and mortality among children in Saudi Arabia. Previous literature suggests that poor compliance to traffic safety measures is exacerbating the burden of childhood injuries. Although car seats have been found to reduce injuries among children, their compliance remains poor nationally. The aim of this study was to evaluate whether providing free car seats for newborns is associated with the compliance of car seat use 3 months after hospital discharge. MethodsThis study was conducted in three regions of the country following a national public health campaign aimed to improve traffic safety. In November 2020, five hundred car seats were given to mothers at the time of discharge. Three months later, parents were contacted to evaluate the prevalence of compliance. In addition to estimating the prevalence of car seat use at 3 months, the analysis assessed predictors of use using a multiple logistic regression model. ResultsOf the 486 initial participants, 375 (77%) responded. According to the parents, 76% of children still use the car seat 3 months after hospital discharge. The age of the parents and the number of family members were significant predictors of car seat use. Smaller families (≤4 members) were 2.2 (95% confidence interval: 1.36–3.82) times more likely to comply with the car seat law 3 months after discharge than larger families. Most of the respondents (70.5%) strongly agree that child restraint systems are an essential device while driving with children. ConclusionsWe found that providing free car seats for newborn children is associated with uptake of improved traffic safety, but compliance remains lower than in developed countries. Further studies are needed to facilitate improving child seat safety among large families. As the country strives to invest in reducing traffic injuries, similar initiatives may facilitate adopting safety measures among parents, which may reduce preventable injuries and improve population health.

Resolving transportation disparity in cancer care.

96 Background: Improving healthcare value requires new models of care including higher utilization of outpatient services. A significant social determinant of health in this mode of care is transportation, and the most vulnerable segments of society are disproportionately affected. Transportation disparity is especially troubling in radiation oncology, due to the protracted daily treatment schedule. Disruptions in treatment adversely impact clinical outcomes and survivals. This study is a pilot trial of a novel virtual logistics hub, capable of managing rideshare, taxis, wheelchair accessible vehicles, vehicles with car seats for children and stretcher vehicles in radiation oncology to measure utilization, patient satisfaction and financial impact. Methods: Over 4 months, using predictive analytics, patients with transportation barriers in a community cancer center were enrolled in a rideshare program. Those who received rides for radiation treatment were included in this study. Each patient's personal historic no-show rate in the health system was recorded. The estimated cost per no-show for a radiation treatment was set at $250. The opportunity cost was calculated as the product of the no-show rate, the no-show cost per treatment and the number of treatments. In addition, the cost of the transportation was recorded and subtracted from the opportunity cost to calculate the return on investment ROI. Qualitative data were also collected through patient surveys after each ride. Results: 29 subjects were included in this IRB-approved pilot study for a total of 419 rides. The most common diagnoses were Breast (27.6%), Lung (17.2%) and Prostate (17.2%). Most patients were racial minorities (70.0%) and had non-metastatic disease (75.9%). The median number of radiation treatments was 10 (3-40). The historic average no-show rate was 7% (0%-36%). The average expected no show cost per patient was $219.14 ($0.00-$1,837.50). The opportunity cost representing the benefit to the health system was $6,355.00. Per patient, the average transportation cost was $325.81 ($13.26-$1,034.39). The total cost of rides was $11,864.77. The ROI was -$5,509.77. When asked 'How would you rate your transportation experience today?' 82% responded Above Average. To the question 'Would you have been able to attend your appointment today if this program did not exist?' 92% answered No. Conclusions: This study shows that the cost of rideshare transportation can partially offset the cost of no-shows. This suggests that a proactive virtual transportation hub can help address transportation barriers, drive patient satisfaction and reduce the waste of no-shows. Radiation therapy represents an ambulatory medicine crucible for patients with limited transportation and social support. Scaling up rideshare innovations from radiation oncology has the potential to drive broader ambulatory strategy.

Ride-hailing with kids: Who’s got their back?

Introduction: As transportation network companies (TNC) are on the rise, assessing the safety of children traveling in these vehicles is imperative. For this reason, this study developed and adopted a scoring system to assess states’ safety standards for children traveling in TNC vehicles. Methods: The scoring was based on two parameters pertaining to child car seat laws for TNCs: clarity and stringency. For each parameter, three criteria that could impact child safety in TNC vehicles were formulated. If a state met a certain criterion it got 1 point and 0 otherwise. The authors gathered all the necessary information by reviewing state statutes in Nexus Uni, a legal research database. These reviews took place between December 2019 and October 2020, and this study evaluated state laws in effect on October 28, 2020. Results: During this assessment, the authors observed a lack of clarity in state child car seat laws, which could compromise safety of children traveling in TNC vehicles. For clarity of laws, Georgia and Indiana received the highest scores (3 out of 3 points), while 16 states scored only 1 point, which was the lowest score in this category. In terms of stringency of laws, Pennsylvania received the highest score (3 out of 3 points), while Indiana scored the least (0 points). Conclusions: Besides one state (Oregon), all other states defined TNCs in their state laws. All states except for Indiana and Washington required child car seats in TNC vehicles. The responsibility for child car seat use was clearly defined in 35 states. The fine for child car seat violation was $50 or more in 28 states. Practical Application: This study will help TNCs, policymakers, and stakeholders identify states that need to improve their standards for child safety in TNC vehicles, and comprehensively address the issue.

Family readiness for evidence-based injury prevention and car seat safety in Jordan

Primary prevention involves developing positive health behaviours designed to avert adverse health outcomes, whereas evidence-based primary prevention adopts practices that reduce illness and injury. This study’s objective was to determine whether families in Jordan would adopt car safety measures after participating in an evidence-based injury prevention programme. Parents of children attending a regional government hospital in Jordan took part in an evidence-based educational class on child car restraints and car safety, six months later follow-up phone calls were made to see if the parents implemented the knowledge obtained in the class. Fifty families participated in the injury prevention class. The results showed that the evidence-based class on child restraint systems led to the adoption of the three interventions by the parents. At the time of the class 45% of parents allowed their infants to sit in the front seat, in the lap of an adult, this was reduced to 12%, being a 33% reduction. In addition, for children aged 6–12 using a seat belt in the back seat, the parents reported that their children using a seat belt increased from 13% to 70%. Finally, parents who participated were more likely to use a child car seat.

A Process Evaluation Protocol for Examining the Impact of Instructions for Correct Use of Child Car Seats Designed through a Consumer-Driven Process and Evaluated in a Field-Based Randomised Controlled Trial.

The incorrect use of child car seats is common, with significant negative effects on crash protection for child passengers. There is currently little evidence for effective, practical countermeasures for incorrect use. The provision of clear and comprehensible materials on correct use supplied with restraints at the point of sale could be highly cost-effective and achieve similar benefits to restraint-fitting services or hands-on training; however, routinely supplied instructions in their current form are frequently difficult to understand. We are conducting a randomised controlled trial of the consumer-driven redesign of instructional materials, consisting of an instruction sheet, swing tags and online training videos. This paper presents the protocol that will be used in an innovate process evaluation that will use the primary outcome of overall serious misuse assessed at six months, together with a survey and semi-structured interviews to determine fidelity, dose and outcomes for all intervention participants. The study will assess intervention delivery and external factors that may impact the effectiveness of the intervention, including experience, health literacy, confidence and attitudes. When it has been conducted, this process evaluation will provide enhanced understanding of the mechanisms through which the intervention works or not, aspects of the implementation process key to success of the intervention and insight into how external factors influence the success of the intervention.

Social and environmental risk factors for road traffic injuries among children under five in rural China.

Background:Road traffic injuries (RTIs) have become a considerable issue for children. In China, RTIs are among the top 3 contributors to injury-related mortality and disability-adjusted life years. The present study aimed to evaluate social and environmental factors that may contribute to RTIs among children under 5 in rural areas of China.Methods:The study was based on 1 year of data (October 1, 2015 to September 30, 2016) from the National Maternal and Child Health Surveillance System (NMCHSS) from all districts in 334 National Maternal and Child Health Surveillance Districts in 30 Chinese provinces, autonomous regions, and municipalities. Data were analyzed to identify environmental, social, and primary caregiver factors related to RTIs among children under 5.Results:Based on data for the 279 children registered in the NMCHSS during the study period, incidence of RTIs increased with increasing age and was higher for boys than girls. Risk of RTIs depended on distances from the child's home to roads and playgrounds. Enrollment in kindergarten and characteristics of primary caregivers affected risky road behaviors by children. Most primary caregivers (67.4%) reported never using child car seats, and 70.6% reported never using a child helmet. Among primary caregivers without a driver's license, 24.8% reported having driven motor vehicles or motorcycles.Conclusions:The living environment and behaviors of primary caregivers can affect risk of RTIs in children younger than 5 years in rural China. Road safety awareness should be strengthened at the community and kindergarten levels.

Perfluoroalkyl Substances in Drinking Water, Indoor Air and Dust from Ireland: Implications for Human Exposure.

Perfluoroalkyl substances (PFASs) were measured in air and dust from cars, homes, offices, and school classrooms in Ireland, along with drinking water from homes and offices. Perfluorooctanoic acid (PFOA) dominated air and drinking water, while perfluorobutane sulfonate (PFBS) dominated dust. This is the first report of PFOA, perfluorooctane sulfonate (PFOS), perfluorohexane sulfonate (PFHxS), PFBS, and perfluorononanoic acid (PFNA) in air inside cars and school classrooms. PFOS concentrations in classroom air exceeded significantly (p ≤ 0.05) those in homes. Atmospheric concentrations of PFOA, PFNA, and methyl perfluorooctane sulfonamido ethanol (MeFOSE) (p ≤ 0.05) were significantly higher in cars containing child car seats than in cars without. PFOS, PFOA, PFBS, and PFHxS were all detected frequently in drinking water, but concentrations of PFASs were low, and although ΣPFASs were 64 ng/L in one bottled water sample, this fell below a Swedish Action Level of 90 ng ΣPFASs/L. The Irish population's exposure to PFOS and PFOA via non-dietary sources is well below estimates of dietary exposure elsewhere in Europe. Moreover, even under a high-end exposure scenario, it falls below the European Food Safety Authority's (EFSA) provisional tolerable weekly intakes for PFOS and PFOA.

An investigation of awareness, perceptions, and usage of child car seats in Pakistan

IntroductionThe objectives of this study were to investigate the level of awareness and usage of child car seats in Pakistan, identify problems that influence non-usage among the knowledgeable parents, as well as to explore the need and interventions for conducting effective awareness campaigns or educational programs for parents in the country. MethodsTotal 375 vehicles with children occupants and 622 children were observed at several schools, day-care centers, pediatric departments in hospitals, and children recreational places within 3 cities of Pakistan. Child safety restraints usage was almost negligible in the study sample. Out of the total 375 vehicles, 318 parents/drivers participated in the interview survey. ResultsAlmost half of the participants were found to be unaware of child car safety restraints. Those who knew, most commonly expressed unavailability, time taken in child car seat installation, and absence of law as the reasons behind not using the child car seats while another common reason was their misperception about the children age group for using child car seats. Driver's education level was found to be the most significant (p < 0.0001) demographic characteristic associated with the driver's awareness on child car seats as well as support towards their usage, initiation of awareness campaigns, and law implementation. ConclusionsThe findings of the study indicate a great need and potential for child car seat educational programs as well as market for such devices especially among educated and affluent parents. The findings of the study also identify parents’ behaviors and attitudes, existing problems, and highlight several challenges and recommendations for planners, policy makers, transportation safety specialists, pediatricians, law enforcement agencies, as well as manufacturers for promoting children road safety.

Safety seat and seat belt use among child motor vehicle occupants, Cluj-Napoca, Romania
